Introduction. Midwifery is one of the world’s oldest professions. Midwives are found in every corner of the world but the scope of their professional expertise may differ in various countries. Although there are numerous documents listing the midwife’s essential competencies, these professionals have to cope with various problems trying to upgrade their skills. A great example of that struggle could be found in Italy. This is one of the countries where midwives keep on fighting for autonomy. Theoretically, an Italian midwife is an independent health care employee, holding a university diploma and a license. In practice, most midwifes work only in hospitals. Italy is a country with the highest rate of caesarean sections in the European Union. In 2013, the rate was 36.3%. The obstetric care is dominated by gynecologists, whilst only some 3% of pregnancies is handled by midwives. This is due to the poor organization of medical care. After all, the role of a midwife is important in making the childbirth more humanized. There are some actual similarities between the professional situation of a nurse in Italy and Poland. They face similar problems when it comes to the implementation of their activities. Aim. The aim of the study was to discuss the position of midwives in Italy, with special attention paid to the legal standards, the education system and the professional independence.

A midwife is an agent of hygiene policy related to the child birth that is the basis of human society. Therefore, studying midwives is to clarify the path of modernization and civilization, and also to investigate the reality of women's employment problems and life in modern era. However, the amount of research on midwives in colonial Korea is still small, and there are still many questions for clarification about the labor of midwives in colonial Korea. The aim of this paper is to analyze the perception of midwives and the reality of midwives's Labor Environment in Gyeongseong. mainly through media materials from the 1920s. Moreover, this paper aims to establish the position of midwives in the female occupation of colonial Joseon. As a result, the following became clear. In the 1920s, the midwife of Gyeongseong was recognized as a considerably high-income profession among the profession of women. In addition, because it was easy for individuals to start a business, it was recognized as a good job for women to become independent. Furthermore, by investigating the midwife's own voices on the media, it was confirmed that they recognized that they had have a good earnings for a woman, but also they complained their income was unstable. And they were dissatisfied with and criticize Korean society because of lack of understanding about necessity of midwifery. From their criticisms, it was confirmed that they were internalizing a hygiene policy strategy based on the Governor-General's theory of civilization and helping to establish a discourse on medicalization of childbirth. Furthermore, the common statement of midwives of Gyeongseong that the Korean society lacked an understanding about them, which suggests that the hygienic idea of using midwives was not widespread in urban areas in the Korean society.

One of the innovations that can be applied in antenatal class is pregnancy exercises use of birth ball. Pregnancy exercises use of birth ball is physical exercise therapy for pregnant women use of birth ball as a tool to improve comfort and fitness during pregnancy. Socialization of pregnancy exercises use of birth ball for midwife purpose to increase the knowledge of midwives about the benefits of birth ball and improve the skills of midwifes in providing maternity care to pregnant women so that midwives can teach pregnant women pregnancy exercises use of birth ball . The obvective of this activity is midwives who are directly involved in maternal and child health services and pregnant womenz. The conclusion is socialization of pregnancy exercises use of birth in antenatal class for midwifes is well. Midwifes understand about benefit use of birth ball and can teach pregnant women pregnancy exercises use of birth ball.

Early initiation of breastfeeding (IMD) and so for the baby. Babies breastfed for 1 hour or more on the mother's chest immediately after birth. Implementation of the IMD on maternal need midwives’s role to IMD programs can work as expected. This study aims to determine the relationship of the midwife's role in the implementation of Early Initiation of Breastfeeding (IMD) in the Maternity with Implementatiton Early Initiation of Breastfeeding Program to Maternal in Regional Hospital Pekalongan City. Study used descriptive correlative with cross sectional approach. Samples were midwives in Regional Hospital Pekalongan City as many as 53 peoples. Sampling technique used total sampling. Collecting data used a questionnaire. Results of univariate analysis showed that midwife’s role in the implementation Early Initiation of Breastfeeding (IMD) that 31 (58.3%) was enough and Early Initiation of Breastfeeding (IMD) Program on maternal that 37 (69.8%) was not implemented. Spearman rank correlation test results showed that there was relationship of the midwife’s role to implementation Early Initiation of Breastfeeding (IMD) in the maternity with implementation Early Initiation of Breastfeeding Program to maternal in regional hospital Pekalongan City with ρ value 0.006 <0.05. Correlation spearman rank was 0,374 which means the relationship exactly. For midwives who have sufficient role should increase skill early breastfeeding initiation in the implementation of the IMD and midwife training should undertake delivery care in accordance with the operational standards specified as APN.

The phenomenon of maternal and infant mortality in Banten Province is still high. Based on data released by the United States Agency for International Development (USAID) Jalin, in Banten Province every week there are an average of 5 mothers and 27 newborns who die the highest deaths are in Pandeglang Regency with 59 cases. and when the public's need for health services increases, especially midwife services, it is not balanced by the expertise and skills of midwives to form a good service work mechanism. So often we also find midwife services not in accordance with their authority. The problems that the author will discuss are related tothe criminal law enforcement of the actions of midwives who are against the law and the authority of independent practice services in Pandeglang Regency and theimplementation of legal policies regarding criminal acts in cases of malpractice in Pandeglang Regency. The method used in this study is a normative juridical descriptive method using a problem approach with the Law by prioritizing primary data and secondary data. The results of this study that midwives who carry out obstetric services are not in accordance with applicable regulations, will be sanctioned in accordance with the Minister of Health of the Republic of Indonesia Number. 1464/Menkes/PER/X/2010 on the licensing and administration of midwife practices. Sanctions given to midwives can be in the form of revocation of the midwife's practice license, revocation of Surat Izin Praktek Bidan temporary, or it can also be a fine. In addition, midwives can also be sentenced to prison if they violate the laws and regulations. If a midwife violates the code of ethics, the settlement of this matter is carried out by the midwife professional forum, namely the Midwife Professional Council or the Indonesian Midwife association.Keywords: Law Enforcement, Midwives, and Unlawful Acts

ABSTRACTThe diptheria outbreak in Surabaya indicated that immunization program failure. Immunization is primary preventif effort to decrease morbidity of disease. An immunization service is very important to protect vaccine quality through cold chain so that vaccine potency be optimal. The purpose of this study was to analysis of factors which are assosiated with midwive’s practice of DPT vaccine distribution and storage to outreach. This study is applying observational approach using cross sectional method. Populations are all village midwives in public health center East Surabaya. The numbers of sample were 38 midwives taken using simple random sampling. The dependent variable was midwive’s practice of DPT vaccine distribution and storage to outreach and the independent variables of this study were work duration, a history of training of cold chain, sosialization, knowledge, attitude. Primary data were obtained through observation and interview. The results showed that 68.4 % midwive’s practice on DPT vaccine distribution and storage at outreach is good. Independent variable which are significant assosiated with midwive’s practice on DPT vaccine distribution and storage at outreach is sosialization about vaccine distribution and storage (p = 0.026) and value of phi and Cramer’s V = 0.431. Enhancement of socialization again be needed to village midwive as efforts for increase knowledge and attitude.Keywords: midwive, cold chain, vaccine distribution, DPT

Psychoeducation is an effort to prevent maternal anxiety which should be integrated into maternal health services. Puskesmas (Community Health Center) Sukorejo 1 Kendal in collaboration with Motherhope Indonesia held an online training program and psychoeducation for pregnant women for midwives. However, there are no reports related to the implementation of online training for the midwife to increase their knowledge and skills. The purpose of this study was to determine the implementation of psychoeducation online training for midwives during the COVID-19 pandemic. The method applied qualitative research with a case study approach. The informants in this study were 1 head of the Puskesmas and 5 midwives. Data collection used online interviews (synchronous and asynchronous) and the WhatsApp platform. The interview guide was used as a guide for interviews that were previously conducted by pilot interviews. The data was then analyzed using thematic analysis with the Miles and Huberman framework. It can be concluded that the benefits of training could be felt by midwives, namely improving mental health literacy, enhancing skill, and changing the midwife’s paradigm. So, needed to improve the online training method of midwives about psychoeducation to rise the psychoeducation midwife’s role for a pregnant woman.

Indicator of posyandu work performance in West Aceh was still far below the target determined and there was a decrease during 2011-2012. The results of the preliminary study showed that the motivation of village midwives were still low in posyandu cadre coaching. The aim of this research was to identify the factors associated village midwives motivation in posyandu cadre coaching. The study was conducted quantitavely and qualitatively. The quantitative research used survey method with a cross sectional design. The subjects were 146 village midwives selected by clustered random sampling at public healthcare center level. Data colection was conducted by interviews using structured questionnaire. The bivariate analysis used Chi Square tests, and the multivariate analysis used logistic regression. The qualitative data was collected by in-depth interviews to the village midwives and analyzed by content analysis. The results showed that 64.4% of the respondents had low performance, 54.8% had less responsibility, 62.3% had insufficient potential development, 57.5% felt inadequate compensation, and 52.7% had less supervision. There were associations between responsibility (C= 0.237, p=0.006), potential development (C= 0.268, p=0.001), compensation (C=0.518, p=0.0001), working condition (C=0.237, p=0.006), supervision (C=0.261, p=0.002,) and the village midwife performance in posyandu cadres’s coaching. Compensation had the most significant effect on the village midwive motivation in posyandu cadres coaching (Exp-B=17.383, p=0.0001). The obstacles experienced by the village midwives in posyandu cadres coaching were insufficient fund, unavailable special training for the posyandu cadre coaching and unavailable health facilities. Compensation has the most significant effect on village midwive motivation in posyandu cadres coaching. It is suggested to public healthcare centers and Health office to implement refreshing cadre coaching and improving village midwives performance in by giving appreciation to village midwives who conducted posyandu cadre.

The Program to prevent vitamin A deficiency in Indonesia includes the administration of vitamin A for post partum mothers by giving a high dose ofvitamin A (@ 200,000 international standard). However, midwives sometimes do not evaluate and follow up the program implantation to make sure thatall postpartum women receive the capsule. This study was aimed at evaluating the midwives’ attitude and health promotors’ support and the behavior ofmidwives in giving the vitamin A capsule to post partum mothers in integrated health center (puskesmas) in Klaten regency. This was a cross-sectionalstudy among 37 midwives in puskesmas of Klaten. The data was obtained using a structured questionnaire. Univariate analysis used frequency distributionand bivariate using chi square test. Most of the respondents (75.6%) had a good supports (68%), there was a correlation between the attitude and behaviorof midwife in giving the vit A capsule (p=0.002). There was a correlation between the support of health promoters with the behavior of midwives in givingvitamin A capsule to post partum mothers(p=0.004). There was a correlation between the attitude and the behavior of midwife and support of healthpromotors and the behavior of midwives in giving the vitamin A to mother in Klaten regency.

Building experience in midwife-led care at freestanding midwifery units is needed to enhance assessment, technical, and care competencies specific to midwives. This study aimed to develop a self-assessment scale for midwifery practice competency based on the characteristics of midwife-led care practices in freestanding midwifery units. This study was conducted at 65 childbirth facilities in Japan between September 2017 and March 2018. The items on the scale were developed based on a literature review, discussion at a professional meeting, and a preliminary survey conducted at two timepoints. The validity and reproducibility of the scale were evaluated based on item analysis, compositional concept validity, internal consistency, stability, and criterion-related validity using data from 401 midwives. The final version of the scale consisted of 40 items. Cronbach’s α for the overall scale was 0.982. The results for compositional concept validity, internal validity, and criterion-related validity demonstrated that this scale is capable of evaluating a midwife’s practice competencies in intrapartum care. Repeated self-assessment using this scale could improve the competencies of midwives from an early stage, maximize the roles of physicians and midwives, and create an environment that provides high-quality assistance to women.

Abstract Aim. The aim of the study was to determine the state of knowledge of women using health services on the midwife’s tasks in the primary health care. Material and methods. The research was conducted online in the period from February to March 2018 among women receiving health services in primary health care. The study was attended by 200 respondents aged 18-60 – they were informed about the voluntariness, anonymity of participation in research and the use of results only for scientific purposes. The diagnostic survey method was used for the research using the author’s questionnaire. Results. Most of the respondents knew their family physician (87.5%) and only half of the respondents knew their midwife (50%). The midwife was mainly associated with childbirth and neonatal care. In the minds of women, the midwife can conduct education (72%), but most of them only knew about the possibility of counseling in the field of pregnancy, postpartum period, early motherhood. Conclusions. 1. Women receiving healthcare services have incomplete knowledge of who their physician, midwife and nurse are in primary health care. 2. In the consciousness of women, there is the idea that midwives (including midwives of primary health care) deal only with pregnant women and after childbirth, but they do not know that midwives are involved in education regarding the prevention of gynecological diseases. 3. In reference to the low level of public knowledge about primary health care midwife’s competence, it is necessary to conduct activities aimed at promoting the midwife profession and raising public awareness in this area.

Background: Nurse midwives are often expected to make ethical decisions in the context of their professional practice, which can lead to ethical confrontations, some of which may lead to ethical dilemmas. The nurses and midwives in the delivery room must keep ethical considerations in mind while caring for a woman in labor so that correct judgments can be made. As a result, the current study emphasized raising ethical efficacy by creating scenarios that simulate the reality of working in the field of reproductive health in order to prepare nurse-midwives who comply with ethical rules during professional practice. Purpose: The main aim of this study is examining how an instructional nursing professional ethics-focused program may affect practice among nurses- midwives in the delivery room. Methods: The quiz experimental design (one-group pretest–posttest design) was used. The purposive non-probability sample consisted of 50 nurse-midwives who worked during the data collection phase in the delivery room. Compliance of midwife primary health center in implementation of the antenatal care integrated quality, disease detection and complications in pregnant womenPurpose This research aimed to determine the relationship of midwife’s compliance in the implementation of the antenatal care (ANC) integrated quality with disease detection and complications in pregnant women.MethodsThis research was an observational study with a retrospective cohort design. The sample was midwives who serve pregnant women in a primary health center, involving 125 midwives. The place of research was in 21 health centers throughout Kulon Progo. Research instruments used a questionnaire, with total sampling. Data analysis used: univariable analysis to determine the distribution of the data, bivariable with chi-square tests, multivariable with linear regression tests with 95% CI and significance of p=0.05, and a qualitative analysis.ResultsMidwives' compliance to the standard operating procedure has a 35% greater chance of detecting complications and illness in pregnant women than the midwife who did not follow standard operating procedure. Not all midwives in primary health care comply with ANC integrated quality.ConclusionThis study contributes to an understanding that compliance of midwives to ANC integrated quality is important as an effort to detect complications in pregnant women. The health office should establish a team for socialization, supervision, and assistance in the implementation of ANC integrated quality.

This study aims to find out and analyze the responsibility of a midwife for unlawful acts that occur and are carried out in the midst of society. The profession of a midwife must provide health services including maternal health, child health, women's reproductive health and family planning based on responsible, accountable and competent services. However, in practice there are always individuals who commit violations and negligence that are against the law in society. The issues raised from this study are how to regulate unlawful acts by midwives and the extent of midwives' responsibility for PMH carried out in the community. The research method used is normative juridical research which is descriptive analytical using literature and case techniques supported by secondary data. Data analysis was carried out using qualitative research methods. The results of this study indicate that the regulation of unlawful acts by midwives can be found in Articles 1365, 1366, 1367 of the Civil Code, up to Article 58 of Law Number 36 of 2009 concerning Health. The midwife's responsibility for unlawful acts committed in the midst of the community is given as long as it is proven to have committed a violation and caused the patient to suffer losses due to negligence from the medical action given by the midwife, so that the midwife will be fully responsible such as imposing administrative sanctions starting from written warning sanctions to revoking the license to practice . Furthermore, the imposition of civil sanctions, namely giving compensation to patients, to imposing criminal sanctions due to negligence which resulted in the patient being seriously injured causing death. This study concludes that the imposition of sanctions and reprimands on midwives is given on the basis of the magnitude of the errors and omissions they have made so that the consequences will be midwifery administrative responsibility, civil liability based on unlawful acts, and criminal liability if proven to have committed gross negligence which resulted in the patient being seriously injured up to death. From this study it can be seen that a midwife must and should prioritize a midwife's professional attitude such as the public's expectations of a midwife's profile, for this reason the community expects and wants a midwife to continue to prioritize a friendly, skilled and responsive attitude in their field according to their expertise and competence.

Abstract A midwife is an independent medical profession regulated by law. It is treated as a mission, a profession of public trust. The independence of midwife expresses in the professional activities which are performed in accordance with professional competences. Moreover, it is a universal feature of the midwife profession. International and national regulations are important in process of shaping the legal status of the midwife profession. Due to its nature, this profession requires from the midwives continuous, lifelong learning and professional improvement. National and European legislation on midwife profession indicates professional functions which are characteristic for this profession. Knowledge of professional legislation and its conscious application in professional practice is a basic obligation in the midwife’s practice. Lack of knowledge in this regard has serious legal and moral consequences, as well as might pose a risk to the patient safety. This study contains an analysis of the legislation that determine the professional functions of midwives and influence the rules of the professional performance for midwifery profession.

<p>Pinning and cutting the umbilical cord is a standard procedure that is always done when the baby is born. The benefits of delaying cord clamping for infants include the opportunity for red blood, stem cells, and immune cells to be transmitted to the baby's body outside the uterus. As for mothers, delaying cord clamping can reduce the complications of bleeding. This study aims to determine the Midwife's Compliance Level in the Implementation of SOP Cutting the Cord in Newborns in Bandar Lampung City. This type of research is correlation analytic with a cross-sectional approach. The research subjects are midwives who provide birth assistance services in the city of Bandar Lampung with a total of 131 people, the sample in this study was the total population. Data collection uses primary data. Univariate data analysis uses percentages and bivariate analysis uses chi-square. The results showed that midwives' level of compliance in implementing SOP of umbilical cord-cutting in newborns in the category of non-compliance was 83 respondents (63.4%), midwife knowledge was categorized as good as 102 respondents (77.9%) and the attitude of midwives in the category of no support as many as 76 respondents (58.02%). There is a significant relationship between midwives 'attitudes and midwives' compliance in the adoption of umbilical cord cutting SOPs in newborns and there is no relationship between midwives 'knowledge and midwives' level of compliance in implementing umbilical cord cutting SOPs. Professional organizations (IBI Bandar Lampung City) and midwives coordinator of the local health center need to program supervision to midwives who provide delivery assistance to always implement the SOP cutting the umbilical cord on newborns. </p>

Abstract Introduction. To work safely, knowledge of law is crucial for midwives who should be familiar with the midwife’s rights and duties as well as their professional responsibility. Aim. Assessment of knowledge of midwives about their professional responsibility. Material and methods. A total of 103 MA Midwifery students of the Medical University of Warsaw, including 55 working and 48 not working as midwives. A diagnostic poll, original anonymous questionnaire, 25 close-ended questions, 8 openended questions. Statistical analysis: STATISTICA 10.0, Mann-Whitney U test, p<0.05. Results. Seventy-one percent of the study participants had knowledge of the binding provisions of law and 83% considered this kind of knowledge as necessary in their professional activity. Twenty-four percent of the total did not know any legal regulations. Thirty percent was not familiar with the Nurses and Midwives Act. Only 52% of the study participants were knowledgeable about the legal protection of midwives and indicated the protection established for public officers. Forty-six percent of the total said that the Act on Professional Self-Government of Nurses and Midwives of July 1st, 2011 regulates the issue of self-governance. As many as 30 study participants knew that membership in the self-governing body of midwives is obligatory. Conclusions. Knowledge of nurses about professional responsibility under amended provisions of law is insufficient and does not improve with experience as a midwife. Due to the fact that new acts on professional responsibility of midwives were implemented in Poland beginning in January 1st, 2012, it is advisable to extend qualifications and knowledge of midwives in order to improve their knowledge of professional issues. The authors study the institutionalization of midwife specialization among women in Russia in the period from the 18th through the early 20th centuries. The main sources are legislative acts, clerical documents, as well as reports on the activities of medical institutions and maternity departments. The authors use the approaches of gender history, and the concept of professionalization as developed by E. Freidson. Midwifery was the first area of womens work that was officially recognized by the state. There were three main stages on the way to professionalizing the midwifery profession among women. The first stage (covering the 18th century) is associated with attempts to study and systematize the activities of midwives. The practical experience of midwifes was actively sought by doctors whose theoretical knowledge was limited. The second stage of professionalization (corresponding to the first half of the 19th century) was associated with the normative regulation of midwife work and the formation of a professional hierarchy in midwifery. The third stage (comprising the second half of the 19th century and the early 20th century) saw a restriction of the midwives spheres of activity, as well as the active inclusion of male doctors in practical obstetrics and their rise to a dominant position. With the development of obstetric specialization, operative obstetrics, and the opening of maternity wards, midwives were relegated to a subordinate position in relation to doctors. In contrast to the United States and Western European countries, Russia did not have professional associations of midwives. Intra-professional communication was weak, and there was no corporate solidarity. In Soviet medicine, finally, the midwives subordinate place in relation to doctors was only cemented.

Background: Cervical cancer (cervix) is the second most common type of cancer in women, with an estimated of 530,000 new cases and more than 270,000 deaths each year. In 2012, cervical cancer cases in Central Java was 2,259 cases. Cervical cancer is easily recognizable and can be prevented with screening. A village midwife as the closest healthcare provider to the community is expected to function better in encouraging the prevention of cervical cancer.if they receive appropriate knowledge and attitude toward screening.Objective: to understand the correlation of knowledge, attitude and behavior of village midwife against cervical cancer and its prevention in Sragen district.Method: This analytic study is using quantitative and qualitative approach. Quantitative studies was done with cross-sectional study. The population of this research were 208 village midwifes from 20 sub-districts in Sragen and the subjects were 118 midwives whom randomly sampled. Data were analyzed using univariate and bivariate analysis.The qualitative study was done using depth interview.Result and Discussion: There is no correlation between age, level of education as well as work duration with their knowledge about cervical cancer prevention (P = 0.787; 0.344; 0.822); there is no correlation between village midwives’ knowledge with their behavior towards cervical cancer prevention (P = 0.664); there is no correlation between the village midwives’ attitudes and behavior towards prevention of cervical cancer (P = 0.460). Qualitative results support the quantitative results.Conclusion: There is no correlation between village midwives’ knowledge, attitudes and behavior towards cervical cancer prevention.Keywords: Cervical cancer, knowledge, attitude, behavior

Background: The maternal referral system is still a health problem, especially in Indonesia’s remote areas. That system is related to the maternal referral system regarding referral decision-making. At the same time, in deciding to refer anyone other than the husband, it must involve parents, shaman/confidant, then the hamlet head. If the patient still does not want to be referred, it must involve the village head, even the sub-district head. Besides, the referral facility's distance also takes a very long time, around 2 hours to travel; therefore, many mothers/babies die while traveling to the referral place. Health professionals, especially midwives, are responsible for explaining to patients and families because average midwives do not give detailed explanations, starting from the anamnesis, examination results, and actions done before and in the referral process. Method: The research design utilized the Participatory Action Research (PAR) approach, consisting of four stages: problem identification, planning, action, and evaluation. Qualitative data is obtained through in-depth interviews and FGD (Focus Group Discussion) to determine existing problems and programs that are not yet running. Contradictary, quantitative data were obtained from the intervention and control groups. Qualitative data analysis used content analysis, while quantitative data used Wilcoxon sign-rank and rank-sum. Result: This study aimed to determine the quality of midwives towards maternal referrals, to design a module for midwife empowerment programs, to implement a model design, and to evaluate the results of implementing a midwife empowerment model for maternal referrals so that midwives have guidelines and improve midwife skills so that pregnant women are given explanations by midwives, in order to facilitate the referral process. Conclusion: The PAR approach is one of the potential ways to determine the midwife’s quality, improving the maternal referral system, especially for the remote area in Indonesia.

Midwife performance management is a systematic and planned process for assessing the performance of midwives in providing midwifery services. Improving clinical quality is the main goal of midwife performance management. This study aims to analyze the implementation of midwife performance management in improving clinical quality at the TPMB (Midwife Independent Practice) Purwakarta Regency, Indonesia. This research uses a qualitative method with a case study approach. The research informants consisted of TPMB management midwives, implementing midwives, and pregnant/maternity women who had utilized TPMB services. Data collection was carried out through in-depth interviews, observation and document review. The research results show that the implementation of midwife performance management in TPMB Purwakarta Regency has been able to improve the clinical quality of midwifery services. This can be seen from the increase in knowledge, skills and attitudes of midwives in providing midwifery services. Midwives have been able to apply midwifery service standards consistently so that they can provide safe, effective, efficient, and quality midwifery services.

Banyak variabel yang dapat saling mempengaruhi untuk membentuk kemampuannya dalam menjalankan tugas. Penelitian ini menggunakan pendekatan sistem bertujuan untuk menetapkan korelasi dari beberapa variabel yang terdapat dalam ke-4 komponen pendidikan bidan yakni input, process, output, dan outcome. Dengan cara survei, sebanyak 261 responden dari 4 provinsi yaitu Sumatera Utara, Kalimantan Barat, Sulawesi Selatan, dan Nusa Tenggara Timur diminta untuk menilai kinerja bidan. Selanjutnya, suatusurvei mendalam diadakan terhadap 107 bidan untuk meneliti korelasi dari sejumlah variabel yang mempengaruhi proses pendidikannya. Hasil penelitian menunjukkan adanya hubungan bermakna antara kinerja bidan dengan semua aspek dalam sistem pendidikannya. Analisis statistik juga menunjukkan korelasi yang nyata antara kinerja bidan di tempat kerjanya dengan beberapa variabel antara lain sistem rekrutmen, latar belakang sekolah, dan motivasinya. Dapat disimpulkan bahwa sistem pendidikan bidan dewasa ini telah cukup efektif dalam menghasilkan bidan-bidan yang kompetendan termotivasi. Disarankan pula agar pendidikan bidan di masa mendatang dapat mempersiapkan bidan-bidan kita untuk meningkatkan kompetensinya dalam menghadapi liberalisasi jasa dalam pasar tenaga kerja dunia yang semakin bebas.Kata kunci: Pendidikan kebidanan, kinerja, kompetensi, liberalisasiAbstractPerformance in the work place for midwives are determined by several factors in the beginning of their professional education and training. A number of variables interact significantly, resulting in their competence to perform their tasks. This research, based on system approach, aims to determinethe correlation and validity of several predictors in 4 components of their midwive education i.e input, process, output and outcome. Using survey methodology, 261 respondents from 4 provinces, North Sumatra, WestKalimantan, South Sulawesi, and East Nusa Tenggara participated in this survey to rate the midwive’s performance. Further on, an indepth survey have been imposed on 107 midwives to study the correlation of some of their background variables. Research result shows significant correlationbetween midwives performance with all aspects of their education systems. Statistical analysis also shows that midwives performance in the workplace correlates with their recruitment system, training background, and motivation. In conclusion, performances of midwives are perceived to be satisfactory,showing correlation with their motivation and competence. It also suggest that the present training system have been considerably effective to produce competent and motivated midwives. Suggestion for future midwives in training is to prepare themselves for global competence in preparationfor service liberalization in the world free market.Key words: Midwive education, performance, competence, liberalization

Background/Aims In Indonesia, 26% of postpartum mothers experience perinatal mental health issues. Midwives have a vital role in supporting postpartum mothers with their mental health. The aim of this study was to determine midwives’ perceptions and experiences of providing postpartum mental healthcare. Methods This phenomenological qualitative study took place between November 2020 and January 2021. Data were collected through in-depth online interviews with 20 midwives working in healthcare settings in Surabaya, Indonesia, who had provided postpartum care for at least 3 years. Results The data were split into midwives’ perceptions and midwives’ experiences. The four themes for midwives’ perceptions were ‘a midwife's role as a companion and supporter’, ‘mental health needs to be addressed during pregnancy’, ‘a midwife's important role providing mental healthcare for postpartum mothers’, and ‘providing holistic care, not just physical’. The six sub-themes for midwives’ experiences were ‘identifying issues by examining attitude’, ‘an intensive approach to initial treatment’, ‘collaborative care or referrals for patients who need further treatment’, ‘openness and a co-operative attitude determine success’, ‘physical and social aspects of successful healthcare’ and ‘midwives need standard guidelines’. Conclusions Midwives have an essential role in postpartum mental healthcare, and adopt an intensive approach. There is a need for national guidelines to assist midwives in providing mental healthcare.

Abnormalities are the fourth cause of infant and neonatal mortality in Indonesia. In this situation, midwives need to give clear information about anomaly prenatal tests. This studi aims to compare of pregnant women‟ preferences and midwives‟ performance about prenatal congenital anomaly screening counseling in Indonesia. The study was carried out at two health centres and two hospitals in Bali from July to September 2016, with 40 midwives and 200 pregnant women. The QUOTE prenatal questionnaire and a cross-sectional descriptive correlative design was used in this study. The result shows that education was the only one of pregnant women demographic data that significant to counseling anomaly screening. The performance of midwives was concluded to perform well on counseling, however as pregnant women perspective, the midwives performance was not as higher as midwives perspective on counseling. The midwives‟ performance and pregnant women‟ preferences were influenced by individual`s midwife factors on pregnant women-midwives relationship and decision making. Different result was found that health education was more influenced by different person of midwives. There was a different between pregnant women‟ preferences and midwives‟ performance about prenatal congenital anomaly screening counseling. The women-midwives relationship and decision making, the variance of midwife‟s individual different (within variance) was much higher than different midwives (between variance). In terms of health education subscale, the variance of different midwives performance of congenital anomaly screening counseling (between variance) was much higher than individual midwives (within variance).

Service quality is determined by human resources, which can carry out services according to standards. Midwives' services will be carried out optimally if each midwife understands her motivation and work commitment as a midwife. Her motivation and work commitment are a promise from a midwife or determination to carry out her activities as a midwife following the goals, positions, and scopes that have been determined in her duties. This study aims to determine the motivation and commitment to providing services to the performance of midwives at Batam City Hospital. This research is quantitative analytic, the place of research is at Batam City Hospital in January - July 2021, and the research population is all midwives who work at the Batam City District Hospital. Sampling technique with total sampling technique, the number of samples is 34 respondents. The results showed that 20 respondents had good motivation and performance, and 15 had good commitment and performance. In conclusion, there is a relationship between motivation and commitment to the performance of the midwife. Suggestions for midwives is that midwives maintain their motivation and commitment to providing services.

On the empirical fact that Gampong Ara succeeded in reducing the stunting rate in badutanya, in 2017, there were 13 cases and decreased to 4 cases in 2019 and in 2020 there were no more. The purpose of the study was to determine the role of village midwives in reducing stunting rates in children under two years old (baduta) in Gampong Ara, Kembang Tanjong District, Pidie Regency. This research uses a qualitative type of research with a case study design. The sample informants were 6 people, namely the Midwife of Ara Village, the Village Head or Keuchik Gampong Ara, the Cadre of the Gampong Ara Posyandu, 2 beneficiary communities (1 person with a history of stunting children and 1 more person with the risk of stunting) plus the Head of the Kembang Tanjong Health Center. The research time was held in April 2022. The results showed that village midwives in Gampong Ara have carried out their role in reducing stunting rates, but there are several aspects that are still not met. The conclusion is that the village midwife has carried out her role as an executor, manager, educator and researcher in the stunting reduction program in Gampong Ara, but not all of them are like helping childbirth, because the village midwife does not live in place and holds 3 villages as her midwife. Advice to policymakers, can work on development for the residence of village midwives in Gampong Ara, so that village midwives can carry out their total roles such as helping childbirth and allocating other village midwives so that Gampong Ara village midwives no longer hold 3 villages as their midwives, so that they can focus more on carrying out their role as village midwives in Gampong Ara. The midwife of Gampong Ara village is expected to continue to increase her role as a midwife.

At the begining of pademic, Indonesiam goverment suggested that midwifery services be carried by using appropriate personal protective equipment according to the guidelines to avoid cross-infection between health worker to mother and child. The purposes of this strudy is to determine the midwife's compliance in following the use of PPE when administering mother and child health services during COVID 19 pandemic. This survey was conducted at the begining of pandemi COVID-19, 403 registered midwives participated by filled an online survey with Google form application for data collection. Variable of this research are midwife compliance of using PPE in midwifery services. The data were analyzed using the statistical test used was Pearson's chi-square. Results showed that 89.8% of midwives were not following the health protocol, as 49.8% offered inadequate PPE in the workplace and the price of PPE was very high during the pandemic. Conclusions: Midwives were unable to comply with the use of personal health and safety equipment (PPE) in services. Age and duration of work had factor had a significant impact on whether or not midwives were able to use PPE appropriately.

Purpose – The purpose of this paper is to describe what it is like to be a midwife in the professionally isolated and marginalised arena of home birth in Ireland and to explore whether the organisation of home birth services and professional discourse might be undermining the autonomy of home birth midwives. Design/methodology/approach – This paper is drawn from auto-ethnographic field work, with 18 of the 21 self-employed community midwives (SECMs) offering home birth support to women in Ireland from 2006 to 2009. The data presented are derived from field notes of participant observations and from interviews digitally recorded in the field. Findings – Home birth midwives must navigate isolated professional practice and negotiate when and how to interface with mainstream hospital services. The midwives talk of the dilemma of competing discourses about birth. Decisions to transfer to hospital in labour is fraught with concerns about the woman's and the midwife's autonomy. Hospital transfers crystallise midwives’ sense of professional vulnerability. Practical implications – Maternity services organisation in Ireland commits virtually no resources to community midwifery. Home birth is almost entirely dependent upon a small number of SECMs. Although there is a “national home birth service”, it is not universally and equitably available, even to those deemed eligible. Furthermore, restrictions to the professional indemnification of home birth midwives, effectively criminalises midwives who would attend certain women. Home birth, already a marginal practice, is at real risk of becoming regulated out of existence. Originality/value – This paper brings new insight into the experiences of midwives practicing at the contested boundaries of contemporary maternity services. It reveals the inappropriateness of a narrowly professional paradigm for midwifery. Disciplinary control of individuals by professions may countermand claimed “service” ideologies.

Competent and confident midwives can improve the quality of maternal and child health services. Mastery of competence and confidence possessed by prospective midwives in clinical practice is an important learning goal of the professional quality of graduates from health colleges, especially midwifery. The purpose of this activity is as an effort to increase the confidence of student midwives and midwives in carrying out their competencies. The method used is to hold a webinar and evaluate knowledge about the competence of midwives. This activity was held in 2 days with 740 registrants and the results showed that the average knowledge score of 317 participants increased by 0.74 from 4.82 to 5.56 with Pvalue=0.000. Midwifery Education Institutions and midwives can make the inosco instrument a measuring tool to determine the extent of the level of self-confidence possessed by prospective midwives and midwives to carry out their competence.

This article describes midwife practices and recommends a possible training program for midwives in South Texas. A 100-item questionnaire was administered to twenty-five practicing midwives in South Texas. All the interviews were tape-recorded. The midwives were all women and they ranged in age from thirty-eight to eighty years old. A qualitative analysis of these interviews indicates that a midwife training program is necessary to standardize and upgrade the practice of midwifery in this area. In addition, all midwives interviewed expressed an interest in participating in a training program. Finally, recommendations are made for designing a training program that would be efficient and culturally relevant to this midwife population.

Methods A qualitative descriptive study using semi-structured interviews was conducted with seven midwives, who commenced their post-registration clinical practice between December 2018 and September 2019. Results Midwives described transition to practice as a time when they felt challenged because of increased responsibility and accountability. It was a period of growth because of ‘learning by doing’, where a supportive working environment was paramount. Conclusions Newly qualified midwives have considerable need for support yet it appears they do not always receive this support.

Methods Midwives (n=16) and women (n=100) were recruited from a regional NHS unit in the UK. A mixed-methods design encompassed two components; audio-recorded antenatal consultations to assess midwives' communication and quantitative surveys to assess women's understanding. Findings Midwife communication was not significantly related to women's understanding of Down syndrome screening information. However, qualitative thematic analysis revealed midwife communication was often insufficient in fully describing Down syndrome and screening. Communication was not very interactive, midwives dominated conversations and did not sufficiently check women's knowledge/understanding. Conclusions Policymakers need to consider these findings. Deficits in midwife communication in relation to established screening practice needs to be addressed through additional training ahead of full implementation of non-invasive prenatal testing into midwifery practice.

The performance of midwives needs to be measured to provide guaranteed health services to mothers and children. Therefore, the aim of this study is to measure the performance of midwives using several variables, including internal branding, meaningfulness of work, work engagement, and innovative behavior, both directly and indirectly. The analysis was conducted in 29 districts and nine cities in East Java, Indonesia, with a sample of 330 midwives. Primary data were obtained by distributing questionnaires and analyzed using PLS-SEM. The research results show that all the variables tested have a positive and significant influence on midwifes’ performance. Innovative behavior has the strongest direct influence (73%), and meaningfulness of work has the weakest direct influence on midwifes’ performance (6%). Meanwhile, indirect relationships through work engagement provide a positive and significant influence, but are not as strong as the direct influence. These results offer practical insight for midwives to maintain and improve their individual performance, considering that more and more new midwives are ready to compete in providing health services for mothers and children.

Based on data obtained from city Health Department depok In 2014 there were 17 cases of maternal deaths that are scattered throughout the District in Depok. PHC which has the highest number of midwives health workers are expected to contribute aid delivery by health personnel is high, but in District Cimanggis deliveries with health professionals (90.1%) is still below Depok (91.0%). The purpose of this study was to determine the effect midwife competencies and motivation to work on the performance of midwife of normal delivery care (APN) in the sub-district Puskesmas Cimanggis Year 2015. Desain study used cross sectional using a qualitative approach. The population in this study throughout the midwife who works at sub-district Puskesmas Cimanggis. In this study, using a total sampling of 30 midwives. The analytical method used is Structural Equation Model (SEM) using SmartPLS 2.0 and SPSS 18. The results showed variable performance of midwife in the application of normal delivery care (APN) in the sub-district Puskesmas Cimanggis directly influenced by the competence of midwives (51.4%) and motivation to work (34.0%), while other factors not studied variables affect the performance of midwives at 14.6% is recommended to further improve the performance of midwives in applying the normal delivery care (APN) by providing training competence normal delivery care (APN) and provide reward for midwives who have good performance so as to motivate the other midwives to improve the performance.

To produce an independent and competent midwife, three main pillars are needed, namely education, regulation, and association. This study aims to analyze education, regulation, and association as the foundation of a solid midwifery profession. The method used in this study is to conduct an analysis and literature review of several supporting references. Several references cited and reviewed then made an analysis related to the topic of this study. Midwifery education is implemented to realize the learning atmosphere and learning process so that students actively develop their potential as midwives who have religious spiritual strength, self-control, personality, intelligence, noble character, and the skills needed for themselves, society, nation, and state in developing ability as Care Provider, Communicator, Community Leader, Decision Maker and Manager. Regulation is to promote regulatory mechanisms that protect the public by ensuring that competent midwives provide safe services for every mother and baby. The aim of this regulation is to support midwives to work independently within the scope of their practice. Background: In 2016 Zika virus infection became an international public health emergency concern.Objective: This study aimed to evaluate the knowledge and practice of midwives regarding the Zika virus.Methods: This research was an analytic study with a cross-sectional approach. A total of samples was 191 selected using a proportional random sampling technique. WHO questionnaire was used to collect data on knowledge and practice related to the Zika virus and a set of explanatory variables.Results: The midwife's knowledge of the Zika virus revealed that only the workplace and sources of information were associated with knowledge of the Zika virus infection. Public health center had a greater chance of having good knowledge than hospitals with OR: 5.130 (CI: 2.591-10.157) and first knew Zika information more than one year ago had a greater chance of having good knowledge than the first midwife knew about Zika on the day the study was conducted with OR: 3.035 (CI: 1.485-6.203).Conclusion: Knowledge and practice regarding Zika virus infection among midwives in Kendari city Indonesia was relatively good. Furthermore, midwives who work in the public health center had a five times chance compared to midwives who work in hospitals to have better knowledge related to the Zika virus.

The paradigm of midwifery services at this time has experienced a shift where the high public interest in quality midwifery services demands midwifery service innovation and creativity from midwives. Many forms of entrepreneurship from the Midwife's Independent Practice are an option for midwives to increase the types of complementary midwifery services holistically (Body, mind and Spirit) to reduce medical interventions during pregnancy and postpartum, one of which is postpartum acuyoga, a combination of acupressure and yoga. The purpose of this community service is to provide information on knowledge and skills about postpartum Acuyoga to alumni of the Curup Midwifery Study Program who already have a Midwife Practice License. The method used in this community service is a workshop which is carried out for 3 days including the activities on the first day, namely the delivery of material about the independent business opportunities of midwives and Postpartum Acuyoga followed by the second day, namely the practice of Postpartum Acuyoga. This activity went smoothly and was attended by 30 midwives, where all participants were able to participate in the activity for 3 days without any obstacles. Participants understand the importance of innovating in developing midwifery services so that they can increase their income and experience in entrepreneurship.

Consequently, increasing the numbers, access, and quality of midwives is paramount as attention, funding, education, and support increases. Yet what a midwife and midwifery are is often misunderstood. The terms midwife and midwifery are often used interchangeably. Other cadres such as nurses, doulas, Skilled Birth Attendant, traditional birth attendant, and nurses with obstetric/perinatal experience are often referred to as midwives or providers of midwifery care. As health systems work to integrate midwives and midwifery, global clarity and understanding must exist on what midwives are and what they are not, and what midwifery is and is not. As the first step to establishing clarity; we undertook a critical comparison of existing different countries, ‘not for profit and professional organizations’ definitions and interpretation of a midwife and midwifery philosophy. The International Confederation of Midwives' definition of a midwife and midwifery philosophy, and their Global Standards for Midwifery Education acted as the baseline. A global consensus and commitment to educational systems and culture that teaches the midwifery model and the philosophy behind that care can positively impact and improve outcomes for women and babies.

Advancement towards the professionalism of midwifery is closely linked to midwives’ professional autonomy. Although the perspectives of Belgian midwives on their professional autonomy have been studied, the views of other maternity care stakeholders are a blind spot. The aim of this study, therefore, was to explore maternity care stakeholders’ views on Belgian midwives’ professional autonomy. A qualitative exploratory study was performed using focus group interviews. A heterogenous group of 27 maternity care stakeholders participated. The variation between midwives, with different levels of autonomy, was reported. The analysis of the data resulted in five themes: (1) The autonomous midwife is adequately educated and committed to continuous professional further education, (2) The autonomous midwife is competent, (3) The autonomous midwife is experienced, (4) The autonomous midwife assures safe and qualitative care, and (5) The autonomous midwife collaborates with all stakeholders in maternity care. A maternity collaborative framework, where all maternity care professionals respect each other’s competences and autonomy, is crucial for providing safe and quality care. To achieve this, it is recommended to implement interprofessional education to establish strong foundations for interprofessional collaboration. Additionally, a regulatory body with supervisory powers can help ensure safe and quality care, while also supporting midwives’ professional autonomy and professionalisation.

The purpose of this study was to analyze the relationship between the characteristics of midwives and internal marketing efforts (empowerment, reward system, and capacity building) with midwife loyalty. This research is an analytical study with a population of midwives involved in PONED services at Puskesmas. The results showed that employment status has a positive correlation with loyalty (sig = 0.019). Empowerment efforts and reward systems have no relationship with midwife loyalty, while efforts to increase competence have a positive relationship with commitment (sig = 0.037). In conclusion, the certainty of employment status and the better the efforts to increase competence carried out by the Puskesmas will determine midwives' loyalty in PONED services. Midwife competence is the ability and characteristics that include knowledge, skills and professional attitudes in carrying out their duties and work effectively that must be owned by a midwife, but in reality the midwife has not fully carried out her duties according to professional competency standards and gaps from ideal conditions and has not been carried out effectively. Midwives are health workers who are qualified to contribute to reducing maternal and child mortality and play a role in reducing stunting rates, this is because midwives are directly involved with women who are program objectives. Knowledge of midwives that has an impact on behavior where with good knowledge of midwives about stunting prevention, the better the knowledge of midwives, the better the quality of Ante Natal Care (ANC) services in preventing stunting. Training is a process of increasing systematically and in accordance with the needs of employees by increasing skills, knowledge and understanding as well as self-motivation. The development of a Participatory Innovative Collaborative (Pillar) training management model is an intervention model that has been analyzed, developed, tested, revised and implemented to obtain feedback that has an effect on the training objectives.

A convenience sample of midwives (N= 520), members of the American College of Nurse-Midwives (ACNM), were surveyed using a 51-item Likert-type scale questionnaire. Midwives were assigned to 1 of 3 primary birth settings (hospital, birthing center, home birth) for data analysis to identify if certain characteristics (policies/protocols, use of technology, providers’ knowledge and beliefs, and midwives’ relationship with collaborating obstetricians) in the birth setting influence a midwife’s decision to offer nonpharmacological pain relief methods in labor and birth.RESULTS:Findings indicate that knowledge and beliefs of health care providers most influenced midwives’ decisions. Although factors of the birth environment influence midwives’ decisions to offer nonpharmacological pain management in labor, they do not prevent midwives from using most of the nonpharmacological pain relief methods during labor and birth.CONCLUSIONS:It would appear that the midwives’ philosophy of care more than the characteristics of the birth setting influence midwives’ decisions to offer nonpharmacological pain relief methods. Educating health care providers about nonpharmacological pain relief methods would further maximize midwives’ ability to use these strategies.

Current National Guidelines recommends that 100% of postpartum women receive 2 high doses of vitamin A capsules (200.000 IU) within 30 days after giving birth, so the mother can transfer vitamin A to baby through breast milk. Midwife as health worker have big role to achieve this program. This study was aimed to determine correlation of midwife’s knowledge and attitude, and District Health Service’s support in the administration of vitamin A supplementation for postpartum women. This was a qualitative study with cross sectional study design among 32 midwives in working area of Puskesmas Seberang Padang and Puskesmas Pemancungan Kota Padang. The data was obtained using a structured questionnaire. Data were analyzed univariate by using frequency distribution and bivariate using chi square test.Majority of midwife (81,3%) gave vitamin A supplementation to postpartum women, there was a correlation between midwife knowledge with vitamin A administration in postpartum women (p=0,030), there was no correlation between midwife attitude and vitamin A administration in postpartum women (p= 0,063, OR= 7,667, 95% CI= 1,035-56,770) and there was no correlation between district health service’s support with vitamin A administration in the postpartum women (p = 0,029, OR= 11,250, 95% CI= 1,125-112,535).There is a tendency correlation between midwife’s knowledge with vitamin A supplementation to postpartum women, no correlation between midwife’s attitude with vitamin A supplementation to postpartum women and there is a correlation between district health service’s support vitamin A supplementation to postpartum women . The COVID-19 pandemic has had a tremendous impact on the world of health, including delivery services at the independent midwife practice. Midwives must change its service system and follow maternity service procedures that the central government has set. The purpose of the study was to explore in-depth the preparation of midwives in providing delivery services at independent midwife practice during the COVID-19 pandemic. This study uses descriptive qualitative research through a phenomenological approach involving seven informants. Data were collected utilizing one-on-one in-depth online interviews using a semi-structured interview guide. Data analysis was done manually using thematic analysis. Data analysis shows that midwives are prepared to provide delivery services at independent midwife practice during the COVID-19 pandemic by modifying infrastructure, providing Personal Protective Equipment (PPE), providing information about COVID-19, disseminating delivery service guidelines, and providing support, vaccinations, rapid screening tests for midwives.

Midwives as the spearhead of antenatal care must be able to prevent maternal mortality rates (AKI) by increasing their performance in providing antenatal services in accordance with standards. Midwife's performance is influenced by several factors, namely individual, organizational and psychological factors. This study aims to analyze the factors related to the performance of midwives in filling the PoedjiRohyati score card on early detection of high-risk pregnancies in the Lima Puluh Kota District Health Center. This study uses a combination of quantitative and qualitative methods. Quantitative research samples were 74 respondents, data collection was carried out from January to February 2018 with interview techniques using questionnaires and observation sheets. While the qualitative research informants were the Head of Community Health and Nutrition, the head of the puskesmas and the midwife coordinator of the Piladang, Taram, Koto BaruSimalanggang and TanjungPati health centers. The results of the study prove that the factors associated with the performance of midwives in filling in the KSPR include the level of knowledge (p = 0.031), attitude (p = 0.004), motivation (p = 0.020) and supervision (p = 0.025). The factors that are most related to the performance of midwives in filling in KSPR are attitudes (p = 0.006). Based on the research, it can be concluded that the midwife will have a good performance in filling in the KSPR if it is based on a high level of knowledge, a positive attitude, high motivation, periodic supervision and supported by complete facilities.

AIM:This article describes how newly qualified midwives experienced their rotation into birth suite and a continuity of midwifery care model. The findings are part of a larger study that aimed to describe graduate midwives’ expectations and experiences of their transition to practice.BACKGROUND:Knowledge and understanding of how midwives make the transition from student to registered midwife remain limited. However, the literature suggests that this time is a critical period for a new graduate. Although transition support programs for midwives exist in New South Wales, Australia, there appears to be an ad hoc approach to their design, implementation, and effectiveness.METHOD:A descriptive qualitative approach to elicit the experiences of 38 newly qualified Australian midwives. Telephone interviews and focus groups were used to collect the data. Content analysis was used to analyze the data set.FINDINGS:The birthing environment was identified as the clinical area, which elicited the greatest level of apprehension for the midwives, whereas those with the opportunity to rotate into a midwifery continuity of care model rated the experience positively.CONCLUSION:The findings of the study suggest that the newly graduated midwives felt a sense of social and professional belonging to the midwifery continuity of care models in which they worked.KEYWORDS:newly graduated midwife; transition support programs; birth suite; models of care; continuity of care; hierarchy

Various factors can affect the number of referrals and the quality of referrals. This increase in referral rate can be an indication that the implementation of the referral system has not gone well. These indicators can be divided into indicators from the midwife's side, namely education and length of work, and indicators from the family in the form of support provided and speed of decision making. The purpose of this study was to analyze the effect of midwife education, midwives' tenure, and family support on referral success. This study is a type of intervention research with a cross sectional design. The location used for this study is at the Cikande Health Center in November-December 2023. The samples used in this study, using total sampling with samples were 30 obstetric referral cases. The data was collected by questionnaire. The independent variables were midwives' education, midwives' length of service, and family support. The dependent variable is referral success. The test used is the Chi Square test. The results of p value 0.007 < 0.05 which indicate that education has a significant relationship with referral success, p value 0.023 < 0.05 which indicates that midwives' tenure has a significant relationship with referral success, and p value 0.001 < 0.05 which indicates that the support provided by family has a very significant relationship with referral success.

The midwife's commitment in recording the PWS KIA is a form of responsibility in reporting the PWS KIA and contributing to the achievement of the PWS KIA target. The lack of data and information on PWS KIA can lead to achievement of low and not low targets and cause PWS KIA problems to be resolved. Observation results of the PWS KIA Kapuas report found problems, namely; the report is incomplete, only partially filled in, even the number of formats sent is different for each puskesmas.This research is a qualitative research. Methods of data collection using questionnaires, observation of documents and in-depth interviews. The object of this study was the coordinating midwife and the Head of the Yankesdas Section as the person in charge of PWS KIA in Kapuas Regency.The results of the study are of 2 categories: coordinator midwives with "high commitment" of 17 health centers and "low commitment" of 9 health centers. Midwife coordinator with high commitment supported, among others; long work experience, has the attitude and responsibility for the trust that was stabbed, SK was appointed as a coordinating midwife, had a SOP supporting document PWS KIA report, received support from kapus and coworkers as well as the monev from the Kapuas District Health Office where the condition was not obtained from coordinating midwives with "low commitment". It is recommended to increase support for all coordinating midwives in the recording and reporting of PWS KIA in the form of supervision of the Kapuas District Health Office with quality activities, both the time and the material delivered.Keywords: Commitment, Midwife Koordinaor, PWS KIA
