Introduction. Midwifery is one of the world’s oldest professions. Midwives are found in every corner of the world but the scope of their professional expertise may differ in various countries. Although there are numerous documents listing the midwife’s essential competencies, these professionals have to cope with various problems trying to upgrade their skills. A great example of that struggle could be found in Italy. This is one of the countries where midwives keep on fighting for autonomy. Theoretically, an Italian midwife is an independent health care employee, holding a university diploma and a license. In practice, most midwifes work only in hospitals. Italy is a country with the highest rate of caesarean sections in the European Union. In 2013, the rate was 36.3%. The obstetric care is dominated by gynecologists, whilst only some 3% of pregnancies is handled by midwives. This is due to the poor organization of medical care. After all, the role of a midwife is important in making the childbirth more humanized. There are some actual similarities between the professional situation of a nurse in Italy and Poland. They face similar problems when it comes to the implementation of their activities. Aim. The aim of the study was to discuss the position of midwives in Italy, with special attention paid to the legal standards, the education system and the professional independence.

Abstract A midwife is an independent medical profession regulated by law. It is treated as a mission, a profession of public trust. The independence of midwife expresses in the professional activities which are performed in accordance with professional competences. Moreover, it is a universal feature of the midwife profession. International and national regulations are important in process of shaping the legal status of the midwife profession. Due to its nature, this profession requires from the midwives continuous, lifelong learning and professional improvement. National and European legislation on midwife profession indicates professional functions which are characteristic for this profession. Knowledge of professional legislation and its conscious application in professional practice is a basic obligation in the midwife’s practice. Lack of knowledge in this regard has serious legal and moral consequences, as well as might pose a risk to the patient safety. This study contains an analysis of the legislation that determine the professional functions of midwives and influence the rules of the professional performance for midwifery profession.

Research has identified midwifery as key to improving maternal and child health globally. Consequently, increasing the numbers, access, and quality of midwives is paramount as attention, funding, education, and support increases. Yet what a midwife and midwifery are is often misunderstood. The terms midwife and midwifery are often used interchangeably. Other cadres such as nurses, doulas, Skilled Birth Attendant, traditional birth attendant, and nurses with obstetric/perinatal experience are often referred to as midwives or providers of midwifery care. As health systems work to integrate midwives and midwifery, global clarity and understanding must exist on what midwives are and what they are not, and what midwifery is and is not. As the first step to establishing clarity; we undertook a critical comparison of existing different countries, ‘not for profit and professional organizations’ definitions and interpretation of a midwife and midwifery philosophy. The International Confederation of Midwives' definition of a midwife and midwifery philosophy, and their Global Standards for Midwifery Education acted as the baseline. A global consensus and commitment to educational systems and culture that teaches the midwifery model and the philosophy behind that care can positively impact and improve outcomes for women and babies.

Building experience in midwife-led care at freestanding midwifery units is needed to enhance assessment, technical, and care competencies specific to midwives. This study aimed to develop a self-assessment scale for midwifery practice competency based on the characteristics of midwife-led care practices in freestanding midwifery units. This study was conducted at 65 childbirth facilities in Japan between September 2017 and March 2018. The items on the scale were developed based on a literature review, discussion at a professional meeting, and a preliminary survey conducted at two timepoints. The validity and reproducibility of the scale were evaluated based on item analysis, compositional concept validity, internal consistency, stability, and criterion-related validity using data from 401 midwives. The final version of the scale consisted of 40 items. Cronbach’s α for the overall scale was 0.982. The results for compositional concept validity, internal validity, and criterion-related validity demonstrated that this scale is capable of evaluating a midwife’s practice competencies in intrapartum care. Repeated self-assessment using this scale could improve the competencies of midwives from an early stage, maximize the roles of physicians and midwives, and create an environment that provides high-quality assistance to women.

This article describes midwife practices and recommends a possible training program for midwives in South Texas. A 100-item questionnaire was administered to twenty-five practicing midwives in South Texas. All the interviews were tape-recorded. The midwives were all women and they ranged in age from thirty-eight to eighty years old. A qualitative analysis of these interviews indicates that a midwife training program is necessary to standardize and upgrade the practice of midwifery in this area. In addition, all midwives interviewed expressed an interest in participating in a training program. Finally, recommendations are made for designing a training program that would be efficient and culturally relevant to this midwife population.

Background The newborn and infant physical examination (NIPE) is a screening programme now undertaken by specially trained midwives. It is increasingly a feature within pre-registration midwifery educational programmes. Objectives To explore the experiences of student midwives completing the theory and practice aspects of the NIPE within a pre-registration midwifery programme. Design A qualitative design was utilised to analyse data collected by semi-structured interviews. Method This study was conducted using interpretative phenomenological analysis (IPA). Findings Three superordinate themes were generated: learning by doing, mentorship and transition to qualification. The findings demonstrate the benefits of student midwife exposure to practical aspects of NIPE. Conclusions Findings indicate that standardisation is required with NIPE education and clinical practice. Preceptorship programmes must effectively prepare newly qualified midwives to undertake the NIPE role. Higher educational institutes must provide greater support to the midwives providing mentorship of student midwife NIPE practitioners. Further research should examine the maintenance of the NIPE role following qualification as a midwife.

Aim To analyse how midwives communicate Down syndrome screening information and explore whether women's understanding of this information is influenced by midwives' communicative style. Methods Midwives (n=16) and women (n=100) were recruited from a regional NHS unit in the UK. A mixed-methods design encompassed two components; audio-recorded antenatal consultations to assess midwives' communication and quantitative surveys to assess women's understanding. Findings Midwife communication was not significantly related to women's understanding of Down syndrome screening information. However, qualitative thematic analysis revealed midwife communication was often insufficient in fully describing Down syndrome and screening. Communication was not very interactive, midwives dominated conversations and did not sufficiently check women's knowledge/understanding. Conclusions Policymakers need to consider these findings. Deficits in midwife communication in relation to established screening practice needs to be addressed through additional training ahead of full implementation of non-invasive prenatal testing into midwifery practice.

The authors study the institutionalization of midwife specialization among women in Russia in the period from the 18th through the early 20th centuries. The main sources are legislative acts, clerical documents, as well as reports on the activities of medical institutions and maternity departments. The authors use the approaches of gender history, and the concept of professionalization as developed by E. Freidson. Midwifery was the first area of womens work that was officially recognized by the state. There were three main stages on the way to professionalizing the midwifery profession among women. The first stage (covering the 18th century) is associated with attempts to study and systematize the activities of midwives. The practical experience of midwifes was actively sought by doctors whose theoretical knowledge was limited. The second stage of professionalization (corresponding to the first half of the 19th century) was associated with the normative regulation of midwife work and the formation of a professional hierarchy in midwifery. The third stage (comprising the second half of the 19th century and the early 20th century) saw a restriction of the midwives spheres of activity, as well as the active inclusion of male doctors in practical obstetrics and their rise to a dominant position. With the development of obstetric specialization, operative obstetrics, and the opening of maternity wards, midwives were relegated to a subordinate position in relation to doctors. In contrast to the United States and Western European countries, Russia did not have professional associations of midwives. Intra-professional communication was weak, and there was no corporate solidarity. In Soviet medicine, finally, the midwives subordinate place in relation to doctors was only cemented.

Abstract Aim. The aim of the study was to determine the state of knowledge of women using health services on the midwife’s tasks in the primary health care. Material and methods. The research was conducted online in the period from February to March 2018 among women receiving health services in primary health care. The study was attended by 200 respondents aged 18-60 – they were informed about the voluntariness, anonymity of participation in research and the use of results only for scientific purposes. The diagnostic survey method was used for the research using the author’s questionnaire. Results. Most of the respondents knew their family physician (87.5%) and only half of the respondents knew their midwife (50%). The midwife was mainly associated with childbirth and neonatal care. In the minds of women, the midwife can conduct education (72%), but most of them only knew about the possibility of counseling in the field of pregnancy, postpartum period, early motherhood. Conclusions. 1. Women receiving healthcare services have incomplete knowledge of who their physician, midwife and nurse are in primary health care. 2. In the consciousness of women, there is the idea that midwives (including midwives of primary health care) deal only with pregnant women and after childbirth, but they do not know that midwives are involved in education regarding the prevention of gynecological diseases. 3. In reference to the low level of public knowledge about primary health care midwife’s competence, it is necessary to conduct activities aimed at promoting the midwife profession and raising public awareness in this area.

One indirect causes of maternal mortality in Indonesia is "Three Delays", delays in: (1) deciding to seek appropriate medical help for an obstetric emergency; (2) reaching an appropriate obstetric facility; and (3) receiving adequate care when a facility is reached, which has relation with the lack of women's ability in empowering herself to actively participate and to make suitable decisions about midwifery care they need. The research aims to explore the partnership between woman and midwife in midwifery care. A descriptive qualitative study was conducted using a convenience sample of six low-risk women after they had given birth and six midwives from six geographically distinct midwife-led care clinic in Bantul Regency. Data were collected through semi‑structured in‑depth interviews. The interview was conducted two to three times and recorded by audio record. Interviews were transcribed and analyzed by the thematic analysis according to Cresswell approach. Seven major themes were identified: (a) midwives ability to partner with women, (b) equality and reciprocity, (c) negotiation, (d) empowerment, (e) trust and time, (f) sharing power and responsibility, and (g) professional friendship. Most of the women are not capable to empower themselves such as less participating in decision making, less considerate in choosing a birthing position, and worsen by the limitations of midwives availability during the delivery process. Midwives need to reflect more the philosophy meaning inside the midwifery care and strengthen the midwifery professionals curriculum in order to persuade women as a partner who capable to empower themselves and to participate actively.

This paper aims to draft an overview of what being a midwife in Byzantium meant, according to the literary sources. Midwife in Byzantium appears to be a rather complex and elusive figure to study. This is due to both the scarce references in the sources and the vague definition of midwifery itself in the ancient world. The main fields of activity of Byzantine midwives were three: medical, religious and care. As a first attempt of studying Byzantine midwife all-round, this paper ends by suggesting further research perspectives in order to get a clearer and more complete understanding of such figure.

Following research ethics approval, 16 nurse/midwife prescribers participated in one-to-one audio recorded semi-structured interviews. Prescriptive authority both enhances the caring role of the nurse/midwife and supports safer care. The ways in which this happened have been illuminated though barriers have also been identified. In order for the full potential of nurses/midwives with prescriptive authority to be realised, measures such as increasing the number of prescribers and the provision on non-interruption zones need to be considered.

This article presents the notion of the postmodern midwife, defining her as one who takes a relativistic stance toward bio-medicine and other knowledge systems, alternative and indigenous, moving fluidly between them to serve the women she attends. She is locally and globally aware, culturally competent, and politically engaged, working with the resources at hand to preserve midwifery in the interests of women. Her informed relativism is most accessible to professional midwives but is also beginning to characterize some savvy traditional midwives in various countries. Thus the concept of the postmodern midwife can serve as a bridge across the ethnic, racial, and status gaps that divide the professional from the traditional midwife, and as an analytical focal point for understanding how the members of each group negotiate their identities and their roles in a changing world.

It is established that access to ongoing informational, emotional and social support from trained health professionals including midwives assists mothers in meeting their breastfeeding goals. Social media is increasingly being used to offer this support. Research has demonstrated that support via platforms such as Facebook can improve maternal knowledge and self-efficacy and ultimately breastfeeding duration. One specific form of support that is under researched is the use of Breastfeeding Support Facebook (BSF) groups that are aimed at supporting women in specific local areas, often with links to face to face support. Initial research highlights that mothers’ value these groups but the role that midwives play in offering support to local mothers through these groups has not been examined. The aim of this study was therefore to examine mothers’ perceptions of midwifery support for breastfeeding delivered through these groups, specifically when midwives played an active role in being a group ‘moderator’ or leader. An online survey was completed by 2028 mothers who were part of local BSF groups comparing the experiences of those participating in groups moderated by midwives versus other moderators such as peer supporters. Moderation was an important factor in mothers’ experiences, with trained support associated with greater engagement and more frequent visits, impacting on perceptions of group ethos, reliability and inclusivity. Midwife moderation was uncommon (5% of groups) but valued: midwife moderators offered a high level of support to mothers in their groups, with 87.5% having received midwife support often or sometimes and 97.8% rating this useful or very useful. Access to a midwife moderated group was also associated with viewing local face to face midwifery support for breastfeeding more positively. This is a significant finding, highlighting that online support complements face-to-face support in local settings (67% of groups were linked to a physical group), and improves continuity of care (14% of mothers who had midwife moderators received care from them). As such midwife moderated or supported groups have the potential to add value to local face to face services and improve breastfeeding experiences in communities. The findings have important implications to support the development of integrated online interventions to improve public health.

The Nursing and Midwifery Council (NMC) Code provides the foundational ‘values and principles’ a midwife should follow throughout their practice. This article discusses the application of the four pillars of the Code – prioritise people, practice effectively, preserve safety, and promote leadership and trust – to the role of the midwife in antenatal care. In providing holistic care facilitated through communication, a midwife can demonstrate advocacy, accountability, competency and leadership to provide quality, safe care to women. However, following the Code is not always straightforward; organisational demands are often in opposition with NMC values. This article discusses the midwife's duty to reconcile these juxtapositions, fulfilling the needs of their employer whilst upholding the requirements of the professional body.

AIM:This article describes how newly qualified midwives experienced their rotation into birth suite and a continuity of midwifery care model. The findings are part of a larger study that aimed to describe graduate midwives’ expectations and experiences of their transition to practice.BACKGROUND:Knowledge and understanding of how midwives make the transition from student to registered midwife remain limited. However, the literature suggests that this time is a critical period for a new graduate. Although transition support programs for midwives exist in New South Wales, Australia, there appears to be an ad hoc approach to their design, implementation, and effectiveness.METHOD:A descriptive qualitative approach to elicit the experiences of 38 newly qualified Australian midwives. Telephone interviews and focus groups were used to collect the data. Content analysis was used to analyze the data set.FINDINGS:The birthing environment was identified as the clinical area, which elicited the greatest level of apprehension for the midwives, whereas those with the opportunity to rotate into a midwifery continuity of care model rated the experience positively.CONCLUSION:The findings of the study suggest that the newly graduated midwives felt a sense of social and professional belonging to the midwifery continuity of care models in which they worked.KEYWORDS:newly graduated midwife; transition support programs; birth suite; models of care; continuity of care; hierarchy

Clinical academic career pathways for nurses, midwives and allied health professionals are a strategic priority for NHS England, and there has been significant investment in the National Institute of Health Research integrated clinical academic programme for non-medical healthcare professionals. Nurses, midwives and allied health professionals face numerous barriers to successfully building a clinical academic career. For those outside the integrated clinical academic programme, clinical academic career approaches are varied and often driven by individual practitioners rather than robust organisational processes or strategy. The vision of ‘future midwife’ is that midwives maximise opportunities in research and scholarship. However, there is little clarity about how these academic aspirations may be supported. This reflective lived experience discussion paper explores key issues around clinical academic midwifery careers, including how space for clinical academic midwives can be assured and the steps midwives can take to start to develop this rewarding and important career.

The paradigm of midwifery services at this time has experienced a shift where the high public interest in quality midwifery services demands midwifery service innovation and creativity from midwives. Many forms of entrepreneurship from the Midwife's Independent Practice are an option for midwives to increase the types of complementary midwifery services holistically (Body, mind and Spirit) to reduce medical interventions during pregnancy and postpartum, one of which is postpartum acuyoga, a combination of acupressure and yoga. The purpose of this community service is to provide information on knowledge and skills about postpartum Acuyoga to alumni of the Curup Midwifery Study Program who already have a Midwife Practice License. The method used in this community service is a workshop which is carried out for 3 days including the activities on the first day, namely the delivery of material about the independent business opportunities of midwives and Postpartum Acuyoga followed by the second day, namely the practice of Postpartum Acuyoga. This activity went smoothly and was attended by 30 midwives, where all participants were able to participate in the activity for 3 days without any obstacles. Participants understand the importance of innovating in developing midwifery services so that they can increase their income and experience in entrepreneurship.

This qualitative study provides an illustration that a midwife must follow the recommendations by the International Confederation of Midwives that the care given to the clients must be holistic. This should also occur in the consultation given during the antenatal visit about the ideal range of weight gain for each individual client along with the recommended amount of calorie intake. Therefore, when the two basic kinds of knowledge are less mastered by midwives, the care will be less holistic. Therefore, collaboration with nutritionists can be done when the midwife has detected what has happened to her client. With the correct mastery of competencies, the practice of midwifery care will become holistic and comprehensive.

The problem of midwife scheduling is one of the most frequent problems in hospitals. Midwife should be available 24 hours a day for a full week to meet the needs of the patient. Therefore, good or bad midwife scheduling result will have an impact on the quality of care on the patient and the health of the midwife on duty. The midwife scheduling process requires a lot of time, effort and good cooperation between some parties to solve this problem that is often faced by the Regional Public Hospital Undata Palu Central Sulawesi Province. This research aimed to apply Memetics algorithm to make scheduling system of midwifery staff at Regional Public Hospital Undata Palu Central Sulawesi Province that can facilitate the process of midwifery scheduling as well as to produce optimal schedule. The scheduling system created will follow the rules and policies applicable in the hospital and will also pay attention to the midwife's preferences on how to schedule them according to their habits and needs. Memetics algorithm is an optimization algorithm that combines Evolution Algorithm and Local Search method. Evolution Algorithm in Memetics Algorithm generally refers to Genetic Algorithm so that the characteristics of Memetics Algotihm are identical with Genetic Algorithm characteristics with the addition of Local Search methods. Local Search in Memetic Algorithm aims to improve the quality of an individual so it is expected to accelerate the time to get a solution.

Background: Nigeria records high maternal deaths despite available antenatal care services in the country. This review aims to synthesise literature on barriers affecting access to midwife-led antenatal care (ANC) among pregnant women in Nigeria. Methods: This is a systematic literature review and involved searching of studies done in Nigeria and published between 2007 and 2018. MEDLINE 1946 to present, CINAHL, JBI, and Pubmed Central were electronically searched to identify studies on barriers affecting access to midwife-led ANC among pregnant women in Nigeria. Reference lists were hand-searched, Grey literature; Google scholar, and NuSearch e-dissertation were also searched. Citation chaining was also used. Twenty articles met the inclusion and exclusion criteria while only seven out of the twenty were included in the review after quality appraisal. Results: The data synthesis of the literature review showed disproportionate absence of the use of midwife-led ANC in this population. Several barriers were identified in the various works reviewed. The identified core barriers are economic relating to issues like high costs of services, poverty, unemployment, financial barriers; personal barrier relates to lack of autonomy, greater convenience of using other services and personal preference, low maternal educational levels, lack of information on healthcare services/ignorance about required health services, maternal age, marital status, parity/number of children; environmental relates to distance to health facility, location of residence/rural dwellers, geographical location, lack of transportation, and farness of ANC service provider; and cultural barriers relates to husband’s permission, culture of patriarchy, traditional beliefs, cultural sensitivity, cultural perception of the role of TBAs, ethnicity. Conclusions: This review highlights the need to thoroughly explore and address context-based barriers affecting access to midwife-led ANC in order to make safe motherhood a reality in Nigeria. The principal solution to these identified barriers is antenatal education by midwives which will help to empower women thereby increasing their access to midwife-led ANC. \textit{Recommendations:} Government/Policy makers should allocate sufficient fund to maternal healthcare and health promotion. They should improve ANC use by making it available, accessible and affordable to all women in Nigeria irrespective of their area of residence. Also incorporate in the educational curriculum of the approved schools of midwifery in Nigeria the suggested midwifery strategies such as named midwife, team midwife, caseload midwifery.Midwives should avail themselves of update courses in midwifery; play the necessary roles of a midwife especially being women-centered; develop interest in conducting midwifery research and using the best available evidence in their practice.

January 8th is Midwife Day. On this day, it is customary to honor the so-called «midwives». In the history of obstetrics in the first half of the 19th century, the activity of midwives was firmly entrenched namely for women who dominated in obstetric activities due to established views on birth as the female area and the existing kind of «taboo» on «unpopularity» of this specialization among male doctors during the study period. At the same time, the very definition of «midwife» was applied both to those who received a special education, and to «uneducated rural midwives». The midwife in rural areas occupied the main place in maternity rituals and in obstetric practice. In the activities of the midwife, rational and irrational methods of traditional medicine were used. A sociological portrait of a midwife is presented.

A midwife is an agent of hygiene policy related to the child birth that is the basis of human society. Therefore, studying midwives is to clarify the path of modernization and civilization, and also to investigate the reality of women's employment problems and life in modern era. However, the amount of research on midwives in colonial Korea is still small, and there are still many questions for clarification about the labor of midwives in colonial Korea. The aim of this paper is to analyze the perception of midwives and the reality of midwives's Labor Environment in Gyeongseong. mainly through media materials from the 1920s. Moreover, this paper aims to establish the position of midwives in the female occupation of colonial Joseon. As a result, the following became clear. In the 1920s, the midwife of Gyeongseong was recognized as a considerably high-income profession among the profession of women. In addition, because it was easy for individuals to start a business, it was recognized as a good job for women to become independent. Furthermore, by investigating the midwife's own voices on the media, it was confirmed that they recognized that they had have a good earnings for a woman, but also they complained their income was unstable. And they were dissatisfied with and criticize Korean society because of lack of understanding about necessity of midwifery. From their criticisms, it was confirmed that they were internalizing a hygiene policy strategy based on the Governor-General's theory of civilization and helping to establish a discourse on medicalization of childbirth. Furthermore, the common statement of midwives of Gyeongseong that the Korean society lacked an understanding about them, which suggests that the hygienic idea of using midwives was not widespread in urban areas in the Korean society.

The LancetSeries on Midwifery and The State of the World’s Midwifery 2014 called on countries to expand the midwifery workforce as a key strategy to improve the health of women and newborns and lower maternal and newborn morbidity and mortality. Well-prepared midwife teachers and preceptors are required to prepare midwives to provide competent, high quality care for women and childbearing families. This article describes the design of competency-based education (CBE) capacity development workshops building on the International Confederation of Midwives’ (ICM) essential competencies and education standards, led by eight English-, Spanish-, and French-speaking CBE Master Educators (MEs). Common content and processes used in three English, one Spanish, and one French workshop are briefly described, noting the influence of participants’ backgrounds, location, and teacher preferences in modifications to common content. As of December 2016, 30 CBE Master Teachers and 22 CBE Teachers have attended a CBE capacity development workshop and, in turn, have provided CBE continuing education (CE) sessions for more than 300 midwife teachers and preceptors in over 20 countries in the Caribbean, Latin America, and Anglophone and Francophone Africa. Lessons learned are shared along with suggestions for next steps in assessment and evaluation of the use of CBE in midwifery education programs.

BACKGROUND:When practicing as a lead maternity carer, the first author (IC) found that following a traumatic practice experience, there appeared to be very little emotional support for the midwife unless provided by colleagues or family. Midwives were expected to continue as if nothing had happened and they had not been affected in any way by the event.AIM:To explore the effects of a traumatic practice experience on the midwifery practitioner.RESEARCH METHODOLOGY AND METHOD:A qualitative study using a narrative research method was implemented. Data were collected using an adapted biographical narrative interview method. An eclectic approach was used to analyze the data for content and form based on identity and ontology.FINDINGS:The study demonstrated that partnership and autonomous midwifery practice are key drivers that make New Zealand midwives more likely to be blamed for unfortunate outcomes, and their competence in practice challenged. The study identified that a breach of relational trust exacerbates or prolongs the initial physiological and/or psychological symptoms experienced by the participating midwives following a traumatic practice event. The perpetrators of this betrayal of trust were organizational and clinical managers, medical and midwifery colleagues, women, and their families.CONCLUSION:The participants’ stories have drawn attention to the effects of counterproductive behaviors that occur in dysfunctional health organizations and the need for professional emotional support.
