Balance is critical to many aspects of life. Work-life balance ensures that we have time to enjoy the fruits of our labor. Hormones control bodily processes like – metabolism, homeostasis, growth and development, sexual function, reproduction, sleep-wake cycle, and mood. Most men see a gradual and progressive decline of testosterone, the primary male sex hormone, starting around age 30, at a rate of 1–2% per year, though in some men, the decline is faster than the body can adequately compensate. In these cases, the fallout can be felt throughout the body with symptoms that are both disruptive and inconvenient. It is produced in the testicles and the adrenal glands but regulated by the hypothalamic-pituitary-gonadal (HPG) axis, a feedback system that responds to circulating hormone levels. Hormonal imbalance in men closely parallels the aging process. The most effective way of balancing male hormones is using testosterone replacement therapy (TRT). Generally, a value < 300 ng/dL is considered low, leading to a condition called hypogonadism. While one-third of Indian men above 40 years of age suffer from hypogonadism, the proportion of them as appropriate candidates for testosterone therapy (TTh) is not clear, not only in India but also in most countries. TTh helps reverse the effects of hypogonadism, but it’s unclear if it would benefit otherwise healthy older men. Primary care physicians encounter clinical conditions of hormonal imbalances in males, such as precocious puberty, delayed puberty, and hypogonadism, in general practice, needing specialists’ consultation and guidance for their management. Quoting half a dozen case reports of hormonal imbalance and their management, this article provides the evidence to prescribe or not to prescribe hormones for men. Materials and Methods: Sample cases of precocious puberty, delayed puberty, male hypogonadism (MH) (general, diabetic, and intracranial hypertension), and a case of complication of TTh cases in adults and how they were diagnosed and managed. Literature review for current evidence for TTh in men globally and in Indian practices.

In many high-income countries, the rate of suicide is highest among middle-aged men. Despite this, few studies have explored the factors that underpin psychological distress and support-seeking among this cohort. This qualitative study used an intersectional approach to account for the plurality of middle-age masculinities and to offer deeper insights into middle-age men’s mental health experiences. Focus groups and interviews were conducted with nine demographic groups of middle-aged men considered ‘at risk’ of suicide in Ireland (n=34). Data collection and data analysis were informed by the principles of grounded theory. A master code list and conceptual maps were developed from which four themes emerged. Theme 1 Reconciling Increasing Expectations with Diminishing Capacities to Achieve at Middle-Age captures various tensions that emanated from expectations on men to have acquired mastery of various gendered norms by middle-age that coincided with a reality of different manifestations of middle-aged vulnerability as well as large scale societal change in Ireland. Theme 2 Isolation at Middle-Age broadly describes a sense of loneliness at middle-age as a result of narrowing social circles, a lack of social outlets, relationship breakdown and living alone. For those men who experienced discrimination, prejudice or racism, their isolation tended to be more deep-rooted and multifaceted and was a particular source of psychological distress. The notion of a ‘double burden’ was central to Theme 3 Barriers to Support Seeking, and encapsulated men’s sense of shame in having to ask for help and ‘failing’ to manage their own problems. Theme 4 Catalysts for Change & Strategies for Improved Self-Care highlights what were seen as the foundations of good mental health for middle-aged men, as well as the value of educational programmes and social contact. The use of an intersectional approach in this study fostered a deeper understanding of the blended identities and wide range of mental health experiences of middle-age men which have informed training and resources currently being implemented under the auspices of ‘Connecting for Life’, Ireland’s strategy to reduce suicide.

A total of 5950 male Malmö residents (from five cohorts, 1926–30) were health screened on two occasions, with an interval of 4–8 years. Disability pensions were granted to 215, of whom 123 were screened both before and after being pensioned. Those 123 constitute the study group reported on here. Among answers to the questionnaire, back and joint pain were the most common complaints mentioned. However, the most striking distinction between the study group and controls was the values of γ-GT (gamma glutamyltransferase), indicating overcon-sumption of alcohol in the group of prospective pensioners. Complaints concerning the back were also an out-standing feature of those belonging to the highest deciles of γ-GT after—but not before—the disability pension had been granted.

Elevated serum triglyceride and low serum HDL cholesterol levels have been reported to be risk factors for occlusive vascular disease. In 20 middle-aged men with psoriasis the serum lipoprotein pattern was compared with that in a group of healthy middle-aged men. In thirteen of the 20 patients the psoriasis was mild to moderate and in seven patients it was severe and widespread. Both groups of patients had elevated levels of both VLDL and LDL triglycerides. They also showed a tendency towards higher VLDL and lower HDL cholesterol levels than the controls. The abnormalities were more pronounced in patients with severe psoriasis. The lipoprotein pattern found in this study may possibly indicate that psoriasis is associated with an increased risk of occlusive vascular disease.

Objectives The purposes of this study is to explore The purpose of this study is to explore what middle-aged men are experiencing while using YouTube. Methods A case study using in-depth interviews was conducted on six middle-aged men who had watched YouTube three to five times a week or had experience running YouTube Channel. In order to secure validity and reliability, triangular verification and member check were performed through research interview records. Results For middle-aged men, the meaning of the experience of using YouTube was ‘personalization’. Among the study participants are YouTube channel operators and non-channel operators. Specifically, it was derived from the psychological personalization of “space to enjoy rest when you want” and “self-protection without exposing yourself,” economic personalization of “free learning center” and “preparation for the future,” and “a tool for communication for social exchange” and “a window for promoting existence.” YouTube channel operators usually act as media producers, but the participants in this study were mainly using YouTube as a consumer of information as they did not consistently operate their channels. Conclusions Through the voices of middle-aged men, they expanded their understanding of YouTube use. Since it suggests that media literacy education, which helps determine whether media information is true, is also required for middle-aged men, who are analog generations, media literacy education for them needs to be continuously conducted in terms of lifelong education. This study is believed to be meaningful in that it is used as basic data for designing and operating media literacy education for middle-aged men.

Numerical and functional impairment of circulating endothelial progenitor cells (EPCs) is thought to contribute to vascular aging and the associated increase in cardiovascular risk. We tested the following hypotheses: 1) EPC clonogenic and migratory capacity decrease progressively with age in healthy, sedentary adult men; and 2) regular aerobic exercise will improve EPC clonogenic and migratory capacity in previously sedentary middle-aged and older men. Peripheral blood samples were collected from 46 healthy sedentary men: 10 young (26 ± 1 yr), 15 middle-aged (47 ± 1 yr), and 21 older (63 ± 1 yr). Mononuclear cells were isolated and preplated for 2 days, and nonadherent cells were further cultured for 7 days to determine EPC colony-forming units. Migratory activity of EPCs was determined using a modified Boyden chamber. Ten sedentary middle-aged and older men (59 ± 3 yr) were studied before and after a 3-mo aerobic exercise intervention. The number of EPC colony-forming units was ∼75% lower ( P < 0.01) in middle-aged (12 ± 3) and older (8 ± 2) compared with young (40 ± 7) men. There was no difference in colony count between middle-aged and older men. EPC migration (fluorescent units) was significantly reduced in older (453 ± 72) compared with young (813 ± 114) and middle-aged (760 ± 114) men. The exercise intervention increased ( P < 0.05) both EPC colony-forming units (10 ± 3 to 22 ± 5) and migratory activity (683 ± 96 to 1,022 ± 123) in previously sedentary middle-aged and older men. These results provide further evidence that aging adversely affects EPC function. Regular aerobic-endurance exercise, however, is an effective lifestyle intervention strategy for improving EPC clonogenic and migratory capacity in middle-aged and older healthy men.

The endothelium is integral to the maintenance of vascular health in humans, and advancing age and low testosterone levels are associated with endothelial dysfunction in men. We compared the impacts of testosterone and exercise training, alone and in combination, on endothelium-dependent flow-mediated dilation % and endothelium-independent glyceryl trinitrate % responses. In this 2×2 factorial 12-week randomized controlled trial, 80 men aged 50 to 70 years with waist girth ≥95 cm and low-normal serum testosterone levels (6–14 nmol/L) were randomized to transdermal AndroForte5 (testosterone 5.0% w/v, 100 mg/2 mL; testosterone), or matching placebo; and to supervised centre-based exercise or no additional exercise. Testosterone increased serum testosterone levels (testosterone×time, P =0.003) to the extent that 62% of subjects in testosterone groups increased levels to >14 nmol/L, whereas placebo treatment had no impact on testosterone levels. Exercise training increased flow-mediated dilation % (exercise×time, P =0.033; testosterone+exercise: +0.5, placebo+exercise: +1.0 versus testosterone+no additional exercise: −0.7, placebo+no additional exercise: +0.2%), whereas testosterone did not impact flow-mediated dilation, nor was it additive to exercise (all P >0.05). There were no significant exercise or drug main effects on glyceryl trinitrate responses (all P >0.05). Exercise training improved endothelium-dependent vasodilator function, whereas administration of testosterone at therapeutic doses did not impact flow-mediated dilation % or add to the exercise benefit. Vascular smooth muscle sensitivity to nitric oxide was not modified by exercise, testosterone, or their combination. In middle-to-older-aged men with central adiposity and low/normal testosterone levels, we observed no evidence that testosterone added to the beneficial impact of exercise on vascular function and health. Sweat lactate is at least partly derived from eccrine-gland metabolism. This study examined whether potential age-associated changes in sweat rate and skin blood flow influence sweat lactate. Six middle-aged (51.5 ± 3.8 years) and 6 younger (25.8 ± 1.5 years) men similar in VO2max, height, weight, percent body fat, and surface area completed constant-load (CON) cycling and interval-cycling (INT) trials. During each trial, sweat and blood were analyzed for lactate concentration at 15, 25, 35, 45, and 60 min. Sweat rates and estimated total lactate secretion were not significantly different (p > .05) between trials or groups. Blood-lactate concentrations were not significantly different between groups during CON but were significantly higher in younger men at 35 min and 45 min during INT. Sweat-lactate concentrations were not significantly different (p > .05) between groups during CON or INT. These results suggest that differences in eccrine-gland metabolism between young and middle-aged men are minimal.
