Pełny tekst źródłaProviding care for elderly people with dementia represents a considerable challenge for our societies, to which current models of dementia partially struggle to respond. For many years, person-centered care has advocated for a prioritized refocusing of caregiving for persons with dementia on their psycho-social needs and their personhood, regardless of cognitive deficits and disabilities.Implementing its principles into concrete clinical practices has been associated with beneficial effects for persons with dementia and their caregivers. However, person-centered care remains underdeveloped as comprehensive caregiving models, partially due to a lack of suitable tools. The Montessori method adapted for the elderly has been gaining popularity worldwide, particularly in France, as a comprehensive person-centered model of care. The objective of this thesis is to provide theoretical contributions, as well as valid and applicable tools to facilitate the implementation of such approach.In our first study, we conducted a literature review of the Montessori method to outline its development as a person-centered approach and to clearly define its core principles. In our second study, we conducted a literature review focusing on its impacts on caring for persons with dementia. This review highlighted positive effects on engagement, emotions, psychological and behavioral manifestations, independence, and caregiver experiences. It also highlighted the need for additional studies based on more rigorous methodologies and evaluating its effects within the French cultural context.In our third study, we assessed the psychometric properties of an assessment tool of preserved capabilities in individuals with severe dementia: the Montessori Assessment System (MAS). Our results demonstrated entirely satisfactory psychometric qualities of MAS. It holds significant potential to contribute to the construction of person-centered care plans. In our fourth study, we evaluated the impact of behavioral manifestations on the administration and outcomes of MAS. Our findings revealed a high number of behavioral manifestations within the studied population. The presence of hallucinations and euphoria was associated with a slightly extended administration time. Conversely, the presence of hallucinations and aberrant motor behaviors was linked to significantly lower MAS total scores. However, behavioral manifestations did not hinder the complete administration of MAS.Lastly, in our fifth study, we translated a tool assessing the implementation of person-centered care in institutions: the Person-Centered Assessment Tool (P-CAT) and conducted a psychometric validation study about it. Our results indicated satisfactory psychometric qualities of the French version of P-CAT, along with a factorial structure organized into three factors: Organizational and Environmental Support, Care Planning, and Adaptation to the Resident. The French version of P-CAT can be employed within research projects or for monitoring the implementation of person-centered care approaches in institutions for elderly people.Collectively, the findings of this thesis suggest that implementation of a person-centered approach, such as the Montessori method, is feasible in caring for persons with dementia. Such approach is yielding potentially significant benefits. Furthermore, our results offer tools aimed at contributing to the implementation of person-centered care within the French context

Pełny tekst źródłaThe main idea behind the PARAChute project is to reduce the number of falls in the elderly by preventing them. This thesis work which is a part of this project aims to develop a new non intrusive human gait analysis system which would be a component of a fall risk evaluation system. Many methods have been used to evaluate dynamic equilibrium of a person while walking. By studying these methods, we were able to identify some gait parameters which are pertinent for fall risk detection. All these methods previously used, are based on clinical tests, or on using wearable sensors. Thus, we opted to develop a new approach based on a completer marker less 3D human motion capture system. The new system we created uses video feeds, a virtual 3d body model and a novel particle filtering algorithm we developed. This algorithm reorganizes the model’s configurations search space in a efficient, optimal and deterministic way. In order to reduce the complexity of this algorithm, we also developed a factorized version of it, using a dynamic Bayesian network. In terms of quality of 3D reconstruction, the results of our method are satisfactory

However, some do not have the choice: when chronic disorders begin, a fall happens or that the mind does not follow any more, it is a turning point in life (Lesourd, 2009) and rapidly comes the exclusion from society for an inclusion in geriatric institution. The main question of this study is to try to understand how elderly people with various dependencies can live: “this inhumane condition” (Malherbe, 2015) “in a total institution" (Goffman, 1968). With this yearlong study of 6 residents, whilst I managed an institution, I present the results of a clinical approach with them, based on narrative accounts and creation of identity booklets. It is mainly, Charles Altesse who will talk of his life, his daily experiences, about himself, of the struggle of living. Together we understand in a context of learning, how there is a lifelong work of construction or identity reconstruction which enables us to advance in a different way. One of the characteristics of this research is also to demonstrate the importance of the method, the “clinical-dialogical” (Lani-Bayle, 2010), in an emphatic listening and an exchange which allows finding resources via an attachment figure and to construct experimental knowledge helping to modify in a way to bear the unbearable end of life or to build knowledge by experience

Home-deployed technological device could provide early and accurate diagnosis, alerts, and interventions and improve patient's management efficiency. This could also preserve economical and human resources. Deployment of such technological devices for health supposes a rigorous and global evaluation. This consists of evaluating technology consequences on individuals but also on health system and society. There are no recommendations to define evaluation rules, no specific health norms, particularly in real life use. This lack of early multidimensional thinking during technology development often leads to irrelevant or unworkable tools. The main objective of this thesis is to build multidimensional working and evaluation methodology for health technologies and more precisely for technologies aiming to prevent loss of autonomy

Pełny tekst źródłaRecent research consistently indicates that age is a major risk factor for cancer (Tubiana, 2002). Such is the case of prostate cancer, which is typically detected between the ages of 60 and 90 (average age of detection: 73 years). Coping with the physical effects of cancer is not the only challenge for elderly patients; cancer may also affect patient’s emotional and social lives as well. The current study presents results of an experience sampling study on the psychological impact of prostate cancer on the daily lives of elderly patients and their wives. We present data on 96 patients with prostate cancer and their spouse who were recruited from two Regional Center for the Fight Against Cancer in Bordeaux, France. Participants completed 3 daily assessments for 4 days (at the beginning of treatment, in the middle of treatment and at the end of treatment). The methodology has a number of advantages, including that it allows us to establish the relationship between variables (coping, quality of life, anxiety and depression), between subjects, and within the couple. The methodology also allows us to observe and compare the adjustment of patients and their partner following different ages (Swendsen, 1997, Czikszentmihalyi, 1984). Our results schowing three key points: First, we distinguiging the differences of feeling and coping strategies by spouses’age. The second point concern the concept of interindividual. This study discriminate behavior and emotions in everyday life of couples. The third important point concerns time management. There is an increased in feeling and psychosocial adjustement on the treatment. This study will help us to refine our knowledge and understanding of emotional adjustment and quality of life of patients with prostate cancer and their spouses. We will thus be able to realize the importance of taking into account the couple and not just patients in psychological interventions. Pełny tekst źródłaIndividual longitudinal follow-up, which aims at following the evolution of an individual state in time, is at the heart of numerous public health issues, particularly in the field of medical prevention. The increasing availability of non-invasive sensors that record various biosignals (e.g., blood glucose, heart rate, eye movements), has encouraged the quantification of human physiology, sensorimotricity, or behavior with the purpose of deriving markers for individual follow-up. This objective raises however several challenges related to signal modelling. Indeed, this particular type of data is complex to interpret, and, a fortiori, to compare across time. This thesis studies the issue of extracting interpretable representations from biosignals through the problematic of balance control follow-up in medical consultation, which has crucial implications for the prevention of falls and frailty in older adults. We focus in particular on the use of force platforms, which are commonly used to record posturography measures, and can be easily deployed in the clinical setting thanks to the development of low cost platforms such as the Wii Balance Board. For this particular application, we investigate the pros and cons of using feature extraction methods or alternatively searching for a generative model of the trajectories. Our contributions include first the review and study of a wide range of state-of-the-art variables that are used to assess fall risk in older adults, derived from the center of pressure (CoP) trajectory. This signal is commonly analyzed in the clinical literature to infer information about balance control. Secondly, we develop a new generative model, ``Total Recall'', based on a previous stochastic model of the CoP, which has shown to reproduce several characteristics of the trajectories but does not integrate the dynamic between the CoP and the center of mass (CoM) -- a dynamic which is considered to be central in postural control. We also review and compare the main methods of estimation of the CoM in quiet standing and conclude that it is possible to obtain an accurate estimation using the Wii Balance Board. The results show the potential relevance of the Total Recall model for the longitudinal follow-up of postural control in a clinical setting. Overall, we highlight the benefit of using generative models, while pointing out the complementarity of features-based and generative-based approachs. Furthermore, this thesis is interested in introducing representations learned on labeled data and tailored for a particular objective of follow-up. We propose new classification algorithms that take advantage of a priori knowledge to improve performances while maintaining complete interpretability. Our approach relies on bagging-based algorithms that are intrinsically interpretable, and a model-space regularization based on medical heuristics. The method is applied to the quantification of fall risk and frailty. This dissertation argues for the importance of researching interpretable methods, designed for specific applications, and incorporating a-priori based on expert knowledge. This approach shows positive results for the integration of the selected biosignals and statistical learning methods in the longitudinal follow-up of postural control. The results encourage the continuation of this work, the further development of the methods, especially in the context of other types of follow-up such as continuous monitoring, and the extension to the study of new biosignals

Pełny tekst źródłaFragility fractures represent a public health problem for elderly. The assessment of the bone strength and of the risk of fracture by the gold standard method (Dual X-ray Absorptiometry - DXA) is limited. Micro-finite element models (µFEM) have shown to better predict the bone strength, but it is not possible to confirm that they do better than the density measured by DXA to estimate the risk of fracture. Thus, the aim of this thesis was to evaluate whether including realistic loading conditions could improve the level of prediction of the FEM. First, we reproduced the loading conditions of a forward fall on 32 radii in an ex-vivo experiment. This experiment leaded to two groups: one fractured and one non - fractured. Then, we evaluated the prediction capability of a segment FEM (9 mm of the distal radius) created using the High Resolution peripheral Quantitative Computed Tomography. This segment FEM was tested under the axial loading (standard analysis), and under five additional non-axial configurations. It was found that the prediction capability of the segment FEM was not improved by the implementation of non-axial loadings. Finally, a heterogeneous FEM of the whole distal radius was created using data from a Cone Beam Computed Tomography. This model considers the fall loading configurations in orientation and speed of the ex-vivo experiment. The FEM of the whole distal radius has a better accuracy to predict the experimental failure load than the segment FEM. This study proposes original data for model validation dedicated to further improvements of fracture risk prediction
