This chapter provides a detailed examination of pharmaceutical pricing strategies in the United States. It points out that pharmaceutical expenditure as a share of total healthcare spending has historically been quite low in comparison to that of hospitalization and physician services. It identifies several common measures of pharmaceutical prices, and highlights the difference in conclusions reached based on different measures. It offers a critical review of several models used to explain pharmaceutical price behavior, which are grouped into three major categories: market structure models, R&D cost-based models, and product quality or value based models. The chapter concludes that prices of brand-name drugs in the United States are largely driven by product quality attributes, not cost of R&D. Lastly, the chapter examines the impact of generic entry on price.

In 1958, record producer Mitch Miller vehemently criticized the state of Top 40 radio. He argued that DJs were pandering excessively to the tastes of teenagers and playing low-quality popular music. This criticism was aimed largely at rock and roll records produced by the low-budget, independent recording firms with whom he now found himself in competition. This chapter traces the development of the major-labels’ novel pop music production practices in the 1950s, specifically the use of overdubbing, unconventional arrangements, added reverb, and Foley effects. These techniques are compared with indie-label recording, which captured more populist genres with less expensive postproduction capabilities. Finally, the chapter traces the aesthetics of DIY (do-it-yourself) records made by amateur musicians-turned-radio-stars, and concludes with a discussion of the lasting effects of these 1950s pop music crosscurrents.

Human activities associated with environmental degradation are threatening urban and rural water supplies throughout the world. Although water quality in the United States surpasses that in most of the world, increasing challenges from an aging infrastructure, poorly controlled disposal of pharmaceuticals, household chemical products, and biologically active industrial chemicals are causing widespread degradation. Toxicology studies have highlighted the role of chronic, low-level chemical exposures of children in the development of disease later in life and demonstrated the chemicals’ epigenetic effects. Despite billions of dollars spent annually to purify diminishing water resources, most purified water is used for waste disposal a for agricultural and industrial processes that do not necessarily require good-quality water. Chapter 5 addresses some of the current threats to water supplies and proposes approaches to increase awareness and provide solutions for the protection of human and environmental health.

This initial chapter presents a telling anecdote from the #8N protests in Argentina in 2012. It then introduces the overarching argument of the book: the rise of protest in certain Latin American regimes can be explained by the interaction of two specific phenomena in recent history: (1) the significant socioeconomic gains made in the region during the past decade, which in turn have produced the most politically “engaged” citizenries—via social media, community organizations, and professional or educational networks—in Latin American history; and (2) the persistence of weak political institutions and waning political legitimacy in the eyes of citizens. Thus, rather than emphasizing how worsening economic conditions and mounting grievances fuel protest, this theoretical approach attributes rising contention to the improvement of economic conditions amid low-quality political institutions.

This chapter discusses how the growth of precarious work and the polarization of the US labor market have produced major problems for the employment experiences of young workers. A prominent indicator of young workers’ difficulties in the labor market has been the sharp increase in their unemployment rates since the Great Recession. Another, equally if not more severe, problem faced by young workers today is the relatively low quality of the jobs that they were able to get. Other problems include the exclusion of young workers from the labor market and from education and training opportunities; the inability to find jobs that utilize their education, training, and skills; and the inability to obtain jobs that provide them with an opportunity to get a foothold in a career that would lead to progressively better jobs and thus be able to construct career narratives.

Cardiovascular disease (CVD) is the leading cause of disease burden globally. According to the 2017 Global Burden of Disease estimates, there were 14 million new cases of CVD, 80 million people living with this condition, and nearly 1 million deaths attributed to CVD in the Americas. Hypertension is the major risk factor for CVD, causing half of the cases, and is highly prevalent, affecting one in four adults, including 40% of those over age 25 years.To appropriately detect hypertension, accurate measurement of blood pressure is critical, and inaccurate measurement of BP has important consequences for policies to address hypertension, as well as for patient safety and quality of care. The Fourth World Health Organization (WHO) Global Forum on Medical Devices identified several critical issues related to medical devices. Among those issues were recommendations for increased regulation of medical devices in low- and middle-income countries and development of technical specifications to optimize procurement of priority medical devices. World Health Assembly Resolution 67.20 (Regulatory system strengthening for medical products) stresses the importance of regulation of medical devices for better public health outcomes and to increase access to safe, effective, and quality medical products. This publication seeks to contribute to meeting these recommendations by providing a practical tool for governments to improve their national regulatory frameworks to improve accuracy of blood pressure measuring devices (BPMDs), in turn contributing to the exclusive use of accuracy validated automated BPMDs in primary health care (PHC) facilities by 2025. This publication can also guide the development of procurement mechanisms that will ensure exclusive availability of BPMDs in PHC facilities. Specifically, this publication will provide a brief background on the importance of using validated BPMDs and highlight key elements of regulations related to pre-market approvals to promote accurate BPMDs.

Anaemia is an almost ubiquitous complication of chronic kidney disease, which has a number of implications for the patient. It is associated with adverse outcomes, an increased rate of red cell transfusions, poor quality of life, and reduced physical capacity. Severe anaemia also impacts on cardiac function, as well as on platelet function, the latter contributing to the bleeding diathesis of uraemia. Renal anaemia occurs mainly in the later stages of chronic kidney disease (stages 3B, 4, and 5), and up to 95% of patients on dialysis suffer from this condition. It is caused largely by inappropriately low erythropoietin levels, but other factors such as a shortened red cell survival also play a part. The anaemia is usually normochromic and normocytic, unless concomitant iron deficiency is present. The latter is also common in renal failure, partly due to low dietary iron intake and absorption, and partly due to increased iron losses. Prior to the 1990s, treatment options were limited, and many patients (particularly those on haemodialysis) required regular blood transfusions, resulting in iron overload and human leucocyte antigen sensitization. Correction of anaemia requires two main treatment strategies: increased stimulation of erythropoiesis, and maintenance of an adequate iron supply to the bone marrow. Ever since the introduction of recombinant human erythropoietin, it has been possible to boost erythropoietic activity, and both oral and intravenous iron products are available to provide supplemental iron. In dialysis patients, oral iron is usually poorly absorbed due to upregulation of hepcidin activity, and intravenous iron is often required. The physiological processes relevant to red cell production are described, as well as the prevalence, characteristics, pathogenesis, and physiological consequences of renal anaemia.

Atmospheric emissions urgently need to reduce for natural gas to fulfill its potential role in the energy transition to achieve the Paris Agreement on climate change. This paper establishes the magnitude and trends of flaring and venting in oil and gas operations, as well as their emissions and impact on air quality, health, and climate. While global flaring and venting comprise 7.5 per cent of natural gas produced, their combined impact on health and climate (in terms of Social Cost of Atmospheric Release) accounts for 54 per cent. Many low- and middle-income countries are economically dependent on oil and gas production. Most premature deaths from air pollution in 2016 were in developing countries. Most natural gas losses and emissions are avoidable. If all natural gas flared and vented globally is captured and brought to market, it could supply annually more than the total South and Central America gas consumption, plus all of Africa’s power needs. If 75 per cent of these volumes are captured, it provides an additional natural gas sales value of US$36 billion per annum (assuming an average gas price of US$4/MMBtu).

The world's largest demonstrator of a revolutionary energy system in desalination for drinking water production is in operation. MIDES uses Microbial Desalination Cells (MDC) in a pre-treatment step for reverse osmosis (RO), for simultaneous saline stream desalination and wastewater treatment. MDCs are based on bio-electro-chemical technology, in which biological wastewater treatment can be coupled to the desalination of a saline stream using ion exchange membranes without external energy input. MDCs simultaneously treat wastewater and perform desalination using the energy contained in the wastewater. In fact, an MDC can produce around 1.8 kWh of bioelectricity from the energy contained in 1 m3 of wastewater. Compared to traditional RO, more than 3 kWh/m3 of electrical energy is saved. With this novel technology, two low-quality water streams (saline stream, wastewater) are transformed into two high-quality streams (desalinated water, treated wastewater) suitable for further uses. An exhaustive scaling-up process was carried out in which all MIDES partners worked together on nanostructured electrodes, antifouling membranes, electrochemical reactor design and optimization, life cycle assessment, microbial electrochemistry and physiology expertise, and process engineering and control. The roadmap of the lab-MDC upscaling goes through the assembly of a pre-pilot MDC, towards the development of the demonstrator of the MDC technology (patented). Nominal desalination rate between 4-11 Lm-2h-1 is reached with a current efficiency of 40 %. After the scalability success, two MDC pilot plants were designed and constructed consisting of one stack of 15 MDC pilot units with a 0.4 m2 electrode area per unit. This book presents the information generated throughout the EU funded MIDES project and includes the latest developments related to desalination of sea water and brackish water by applying microbial desalination cells. This book goes well beyond simply growing commercial tree plantations for wood production. It explores new forests that can supply environmental services such as salinity mitigation and carbon sequestration together with commercial wood production in an environment beyond the boundaries of traditional forestry. New Forests targets agricultural landscapes affected by salinity and which generally have rainfall less than 650 mm per year. The book addresses vital issues such as where tree planting might best be pursued, what species and technologies should be used for establishment and later management, how productivity can be improved, what mix of environmental services and commercial goods is optimum, and whether the likely net benefits justify the change in land use and requisite investment. The book discusses the representations of civilization and barbarism considering the narratives of Conan Cycles by Robert Ervin Howard. The adventures of the character Conan the Barbarian were produced between 1932 and 1936. There are twenty-one literary and fictional texts that are part of a specific genre called “Sword and Witchcraft”. Such literary genre approaches fabulous worlds cha racterized by the presence of the superna tural, where fantastic characters venture into action and fantasy plots. Conan’s adventures were published in the so-cal led pulp magazines (or pulp fictions), low-quality graphic magazines — usually processed from paper pulp — that were very popular in the US between the 1920s and 1950s. Despite Howard placed his great famous character in the “Sword and Witchcraft” genre, he drew philosophical aspects in his plots, insofar as the central theme of these narratives is linked to the opposition between civilization and barbarism. Conan usually represents a violent, bloodthirsty, and crude human conduct, but honest and honorable in the face of the corrupt and greedy actions of civilized men, so an expression of barbarism would be somewhat necessary in his creator eyes, especially in the face of a Civilizational crisis. In addition, Conan and other characters have traces of the so-called western frontier men: the men who would represent the Ame rican trailblazers, so much worshiped by the creator of the character, largely because their rusticities were considered to be the basis for the formation of the country. Howard, a Texan native, was very concerned about the historical context of the economic and social crisis of the twentieth century, and more specifically, the Great Depression of the 1930s. Conan, therefore, expresses some aspects of a more rustic and truthful conduct, closer to the idealized manners of the men who made the West and the US, meaning that the narratives of the Conan Cycles are part of so-called fron tier literature. This is not just a study of civilization and barbarism, but it is also about the conception of the US border in Robert Howard’s own historical context

The atmospheric circulation in the mid-latitudes of both hemispheres is usually dominated by westerly winds and by planetary-scale and shorter-scale synoptic waves, moving mostly from west to east. A remarkable and frequent exception to this “usual” behavior is atmospheric blocking. Blocking occurs when the usual zonal flow is hindered by the establishment of a large-amplitude, quasi-stationary, high-pressure meridional circulation structure which “blocks” the flow of the westerlies and the progression of the atmospheric waves and disturbances embedded in them. Such blocking structures can have lifetimes varying from a few days to several weeks in the most extreme cases. Their presence can strongly affect the weather of large portions of the mid-latitudes, leading to the establishment of anomalous meteorological conditions. These can take the form of strong precipitation episodes or persistent anticyclonic regimes, leading in turn to floods, extreme cold spells, heat waves, or short-lived droughts. Even air quality can be strongly influenced by the establishment of atmospheric blocking, with episodes of high concentrations of low-level ozone in summer and of particulate matter and other air pollutants in winter, particularly in highly populated urban areas.Atmospheric blocking has the tendency to occur more often in winter and in certain longitudinal quadrants, notably the Euro-Atlantic and the Pacific sectors of the Northern Hemisphere. In the Southern Hemisphere, blocking episodes are generally less frequent, and the longitudinal localization is less pronounced than in the Northern Hemisphere.Blocking has aroused the interest of atmospheric scientists since the middle of the last century, with the pioneering observational works of Berggren, Bolin, Rossby, and Rex, and has become the subject of innumerable observational and theoretical studies. The purpose of such studies was originally to find a commonly accepted structural and phenomenological definition of atmospheric blocking. The investigations went on to study blocking climatology in terms of the geographical distribution of its frequency of occurrence and the associated seasonal and inter-annual variability. Well into the second half of the 20th century, a large number of theoretical dynamic works on blocking formation and maintenance started appearing in the literature. Such theoretical studies explored a wide range of possible dynamic mechanisms, including large-amplitude planetary-scale wave dynamics, including Rossby wave breaking, multiple equilibria circulation regimes, large-scale forcing of anticyclones by synoptic-scale eddies, finite-amplitude non-linear instability theory, and influence of sea surface temperature anomalies, to name but a few. However, to date no unique theoretical model of atmospheric blocking has been formulated that can account for all of its observational characteristics.When numerical, global short- and medium-range weather predictions started being produced operationally, and with the establishment, in the late 1970s and early 1980s, of the European Centre for Medium-Range Weather Forecasts, it quickly became of relevance to assess the capability of numerical models to predict blocking with the correct space-time characteristics (e.g., location, time of onset, life span, and decay). Early studies showed that models had difficulties in correctly representing blocking as well as in connection with their large systematic (mean) errors.Despite enormous improvements in the ability of numerical models to represent atmospheric dynamics, blocking remains a challenge for global weather prediction and climate simulation models. Such modeling deficiencies have negative consequences not only for our ability to represent the observed climate but also for the possibility of producing high-quality seasonal-to-decadal predictions. For such predictions, representing the correct space-time statistics of blocking occurrence is, especially for certain geographical areas, extremely important.

Horseradish roots, due to the content of many valuable nutrients and substances with healing and pro-health properties, are used more and more in medicine, food industry and cosmetics. In Poland, the cultivation of horseradish is considered minor crops. In addition, its limited size causes horseradish producers to encounter a number of unresolved agrotechnical problems. Infectious diseases developing on the leaves and roots during the long growing season reduce the size and quality of root crops. The small range of protection products intended for use in the cultivation of horseradish generates further serious environmental problems (immunization of pathogens, low effectiveness, deterioration of the quality of raw materials intended for industry, destruction of beneficial organisms and biodiversity). In order to meet the problems encountered by horseradish producers and taking into account the lack of data on: yielding, occurrence of infectious diseases and the possibility of combating them with methods alternative to chemical ones in the years 2012–2015, rigorous experiments have been carried out. The paper compares the impact of chemical protection and its reduced variants with biological protection on: total yield of horseradish roots and its structure. The intensification of infectious diseases on horseradish leaves and roots was analyzed extensively. Correlations were examined between individual disease entities and total yield and separated root fractions. A very important and innovative part of the work was to learn about the microbial communities involved in the epidemiology of Verticillium wilt of horseradish roots. The effect was examined of treatment of horseradish cuttings with a biological preparation (Pythium oligandrum), a chemical preparation (thiophanate-methyl), and the Kelpak SL biostimulator (auxins and cytokinins from the Ecklonia maxima algae) on the quantitative and qualitative changes occurring in the communities of these microorganisms. The affiliation of species to groups of frequencies was arranged hierarchically, and the biodiversity of these communities was expressed by the following indicators: Simpson index, Shannon–Wiener index, Shannon evenness index and species richness index. Correlations were assessed between the number of communities, indicators of their biodiversity and intensification of Verticillium wilt of horseradish roots. It was shown that the total yield of horseradish roots was on average 126 dt · ha–1. Within its structure, the main root was 56%, whereas the fraction of lateral roots (cuttings) with a length of more than 20 cm accounted for 26%, and those shorter than 20 cm for 12%, with unprofitable yield (waste) of 6%. In the years with higher humidity, the total root yield was higher than in the dry seasons by around 51 dt · ha–1 on average. On the other hand, the applied protection treatments significantly increased the total yield of horseradish roots from 4,6 to 45,3 dt · ha–1 and the share of fractions of more than 30 cm therein. Higher yielding effects were obtained in variants with a reduced amount of foliar application of fungicides at the expense of introducing biopreparations and biostimulators (R1, R2, R3) and in chemical protection (Ch) than in biological protection (B1, B2) and with the limitation of treatments only to the treatment of cuttings. The largest increments can be expected after treating the seedlings with Topsin M 500 SC and spraying the leaves: 1 × Amistar Opti 480 SC, 1 × Polyversum WP, 1 × Timorex Gold 24 EC and three times with biostimulators (2 × Kelpak SL + 1 × Tytanit). In the perspective of the increasing water deficit, among the biological protection methods, the (B2) variant with the treatment of seedlings with auxins and cytokinins contained in the E. maxima algae extract is more recommended than (B1) involving the use of P. oligandrum spores. White rust was the biggest threat on horseradish plantations, whereas the following occurred to a lesser extent: Phoma leaf spot, Cylindrosporium disease, Alternaria black spot and Verticillium wilt. In turn, on the surface of the roots it was dry root rot and inside – Verticillium wilt of horseradish roots. The best health of the leaves and roots was ensured by full chemical protection (cuttings treatment + 6 foliar applications). A similar effect of protection against Albugo candida and Pyrenopeziza brassicae was achieved in the case of reduced chemical protection to one foliar treatment with synthetic fungicide, two treatments with biological preparations (Polyversum WP and Timorex Gold 24 EC) and three treatments with biostimulators (2 × Kelpak SL, 1 × Tytanit). On the other hand, the level of limitation of root diseases comparable with chemical protection was ensured by its reduced variants R3 and R2, and in the case of dry root rot, also both variants of biological protection. In the dry years, over 60% of the roots showed symptoms of Verticillium wilt, and its main culprits are Verticillium dahliae (37.4%), Globisporangium irregulare (7.2%), Ilyonectria destructans (7.0%), Fusarium acuminatum (6.7%), Rhizoctonia solani (6.0%), Epicoccum nigrum (5.4%), Alternaria brassicae (5.17%). The Kelpak SL biostimulator and the Polyversum WP biological preparation contributed to the increased biodiversity of microbial communities associated with Verticillium wilt of horseradish roots. In turn, along with its increase, the intensification of the disease symptoms decreased. There was a significant correlation between the richness of species in the communities of microbial isolates and the intensification of Verticillium wilt of horseradish roots. Each additional species of microorganism contributed to the reduction of disease intensification by 1,19%.
