ABSTRACT Negative issues the arise in public become one determinant of the continuity of an institution. Institutions will gain more value if they can manage the issues into opportunities, but it can also be a threat if they can not prevent the onset of crisis symptoms. Post-operation catching hands conducted KPK to the head of the region, Ade Swara, and wife, Mrs. Nurlatifah in 2014 experienced a crisis of confidence from the community of Kabupaten Karawang. This study will use the management crisis Rhenald Kasali’s, agenda building strategy, dan media relations activities, and describe how the onset of this crisis emerged, the factors causing, and the anatomy of crisis. Prevention of the crisis should be sought so that the institution is able to restore the trust of the community. The appoinment of Mrs. Cellica as Acting Officer of the Regent has been one of the efforts to restore the crisis, because the institution has regained the leadership capable of providing policies for the institutions existance. The results showed that the crisis that occured in Karawang regency is a type of smoldering crisis, where the crisis is not common in the institution. In the process of disseminating information, PR do some media relations activities so that people get the informations related to the case. In addition, the researches also describe the pattern of institutions conducted on the media

The utilization of online media, known as Cyber Public Relations (CPR), is the online public relations activities for information dissemination using online media by government institutions as an agenda to build the image of the institution in the public eyes. The purpose of this study was to determine and analyze the model of Cyber ​​Public Relations and the obstacles in utilizing the online media of the DKI Jakarta Provincial Government in building a positive image of the institution. The paradigm used in this research is the constructivist paradigm. This research is using a qualitative research approach with a case study research method. Data collection techniques include: (1) in-depth interviews (2) observations, and (3) study of relevant literature. The process of analyzing qualitative data is carried out in stages: data reduction, then presenting the data and finally verifying (drawing conclusions). The results showed that through two official websites, namely the official website www.jakarta.go.id as a data information center and www.beritajakarta.com as the official portal for reporting. In addition, it has a number of social media, twitter, facebook, Instagram and YouTube as a means of information to the public. It is also supported by JAKI as an application that is the gateway for the community to access all online services for the DKI Jakarta Provincial Government. Cyber ​​PR is used in synergizing communication between DKI Jakarta regional government and the community so that it can produce a better image of government in the community.

<p>Mass media is one of the media publications to the public in the formation of the Image. Important positive images are formed to foster public trust. News in the newspaper media is still interesting to be appointed in an effort to build a positive image of educational institutions. In addition to targeting the masses of young people, higher education institutions also target parents who are still familiar with conventional media such as newspapers. The case study in this study was Dian Nuswantoro University (Udinus) and news in the Suara Merdeka newspaper. Suara Merdeka is interesting to be appointed because the local central Java newspaper has amounts of printing and is still an alternative reading in the Central Java region where this segmentation is in line with Udinus segmentation which is domiciled in Semarang, Central Java.<br />This study aims to analyze the theme of news that often arises about the institution, the frequency of occurrence of coverage in certain periodizations. Furthermore, this can be observed how the reporting of a media that has the power to shape the image of the institution. The method used is quantitative content analysis, which describes how the contents of the media using thematic and physical recording units are analyzed using the concept of public relations about media and the formation of the image of the institution.<br /> <br />The result can be seen that there are 9 sections in Suara Merdeka which present Udinus related news, namely Semarang Metro rubric, Edukasia, Screen, Page 1, Business Economy, Around Young Tugu, Salatiga News, Youth Voice Expressions. Screen Rubric occupies the highest position in the news, which is 70.3%. For the<br />theme that is often reported, there are four types of reporting, namely news related to the world of academics (products), related to environmental responsibility (CSR), reporting related the environment and reporting related<br />to the activities of communicating the activities of the institution.</p><p> </p>

This paper analyses a psychiatric institution resident?s identity (self)- construction processes. Our quest was grounded on constructivist theoretical paradigm that sees identity as a manner a person defines oneself in accordance with offered social repertoire. In other words, the total institution milieu offers limited range of identities to its residents. On the other hand, the latter are not just passive subjects that identity is imposed upon. They are able to use a number of adjustment mechanisms to the knowledge about themselves - ranging from symbolic escape to open resistance against the imposed image about them; to obedience, and we were interested to understand the (supposed) complex dynamic of identity (re)-building in, so-called, special or forensic patients. In order to understand this process and attempting to discover, besides the manifest also the latent layer of the story about self, the local, or personal, identity understanding was examined.

Indonesian local parliaments have an image as a corrupt, closed, outdated, and unreachable institution in the public’s eyes. However, the parliaments have never tried to more actively developing policies which encourage active community participation. The recent adoption of social media by local parliaments seems to be a sign of the parliament’s willingness to encourage greater community contributions. However, little can be understood about the implications of these activities. Moreover, whether social media contributes significantly to community participation in local parliaments. The purpose of this study is to explore the use of social media by local parliaments. This study employed qualitative multiple case studies by conducting a content analysis on Facebook and Twitter in four local parliaments. This study found three faces of local parliaments on social media which are performers, reporters, and marketers. In general, there has been a willingness of local parliaments to become more transparent and participatory institutions. Nevertheless, the massive adoption of social media to promote more participatory democratic practices was conducted half-heartedly by the local parliaments. Practically, if local parliaments want to use social media to enchance their image, they must publish content that promotes openness such as online assembly and educative information.

In this paper, the authors propose a method for medical image retrieval in distributed systems to facilitate telemedicine. The proposed framework can be used by a network of healthcare centers, where some can be remotely located, assisting in diagnosis without the necessary transfer of patients. Security and confidentiality issues of medical data are expected, which are handled at the local site following the procedures and protocols of each institution. To make the search more effective, the authors introduce a distributed index based on features that are extracted from each image. Considering network bandwidth limitations and other restrictions that are associated with handling medical data, the images are processed locally and a pointer is distributed in the network. For the distribution of this pointer, the authors propose a function that maps the pointer of each image to a node with similar contents.

Crisis can occur in all organizations and institutions both private and government. It can be caused by many factors, both internal and external, which one of which is the environmental changes that requires an institution to adapt. National Population and Family Planning Board (BKKBN) is a governmental institution that has successed in the socialization of Family Planning Program in the 1960s-1990s by reducing the rate of population growth. After post-reform era happened in Indonesia, Total Fertility Rate (TFR), children average number from childbearing age couples stagnant in 2,6. Environmental changes marked by Regional Autonomy Regulations (OTDA) arrangement in January 2004 where authority of the Central Government including FP Program is devolved to Local Governments. OTDA associated with political and economic policies impacts on institutional, budget, and personnel availability.This article is using qualitative method, explaining BKKBN as a government institution in organization communication crisis after OTDA enactment. The FP Program that proven improved society welfare is no longer a priority. Communication was only one command from Central Government to Local Government, in post-reform era it hampered, as most District Heads are more concerned with short-term development because of their short tenure. The budget support even personnel for the socialization of FP Program is reduced and some have none at all. There are four important stages in the crisis by Steven Fink, prodromal stage, the acute stage, chronic stage, and resolution stage. There must be different communication strategies at each stage. BKKBN need to regain public trust to overcome the crisis. According to Hardjana, reputation is crisis antithesis and the crystallization of the image. Determining the right communication strategies is needed by BKKBN as a strategic institution in improving society welfare to regain their confidence to achieve institution’s goal, decrease TFR which impact the declining rate of population growth in Indonesia.Keywords: Crisis, Communication Management, Reputation, Government

OBJECTIVE The aim of this study was to identify and discuss operative nuances utilizing image guidance in the surgical management of aggressive sacral tumors. METHODS The authors report on their single-institution, multi-surgeon, retrospective case series involving patients with pathology-proven aggressive sacral tumors treated between 2009 and 2016. They also reviewed the literature to identify articles related to aggressive sacral tumors, their diagnosis, and their surgical treatment and discuss the results together with their own experience. Information, including background, imaging, treatment, and surgical pearls, is organized by tumor type. RESULTS Review of the institutional records identified 6 patients with sacral tumors who underwent surgery between 2009 and 2016. All 6 patients were treated with image-guided surgery using cone-beam CT technology (O-arm). The surgical technique used is described in detail, and 2 illustrative cases are presented. From the literature, the authors compiled information about chordomas, chondrosarcomas, giant cell tumors, and osteosarcomas and organized it by tumor type, providing a detailed discussion of background, imaging, and treatment as well as surgical pearls for each tumor type. CONCLUSIONS Aggressive sacral tumors can be an extremely difficult challenge for both the patient and the treating physician. The selected surgical intervention varies depending on the type of tumor, size, and location. Surgery can have profound risks including neural compression, lumbopelvic instability, and suboptimal oncological resection. Focusing on the operative nuances for each type can help prevent many of these complications. Anecdotal evidence is provided that utilization of image-guided surgery to aid in tumor resection at our institution has helped reduce blood loss and the local recurrence rate while preserving function in both malignant and aggressive benign tumors affecting the sacrum.

Background and purposeTo assess the performance of dual energy unenhanced CT in the detection of acute ischemia after mechanical thrombectomy.MethodsRetrospective study, approved by the local institutional review board, including all patients that underwent intra-arterial thrombectomy in our institution over a period of 2 years. The presence of acute ischemia and hemorrhage was evaluated by three readers. Sensitivity and specificity of the non-contrast CT weighted sum image (NCCT) and the virtual non-contrast reconstructed image (VNC) were estimated and compared using generalized estimating equations to account for the non-independence of regions in each patient.Results58 patients (27 women and 31 men; mean age 70.4 years) were included in the study, yielding 580 regions of interest. Sensitivity and specificity in detecting acute ischemia were higher for all readers when using VNC, with a significant increase in sensitivity for two readers (p<0.001 and 0.01) and a significant increase in specificity in one reader (p<0.001). Specificity in detecting hemorrhage was excellent for all readers.ConclusionsDual energy unenhanced CT VNC images were superior in the identification of acute ischemia in comparison with NCCT.

Many museums spend time and money fruitlessly competing with Wikipedia, creating online information resources and image galleries that will be mostly ignored, as Wikipedia is usually the highest ranked search result for any query. Wikimedia Commons can host searchable, downloadable images and Wikipedia can be easily edited by volunteers and specialists; both cost nothing to use and have a global audience. Yet most museums have no Wikipedia strategy, and often their institutional copyright policies – needlessly, for most natural history collections – prevent them from engaging and openly sharing collection information. I’ll illustrate this with the case study of the Critter of the Week project, a collaboration between Radio NZ and the Department of Conservation that relies on the open image libraries of Auckland Museum and Landcare Research. There are simple institutional policies and procedures any museum can take that will support the work of the 70,000 volunteer Wikipedia editors. An institution can also directly host edit-a-thons and Wikipedia events, organise backstage tours for local Wikipedians, and host a Wikipedian in Residence. Subject specialists in a museum can even edit and update Wikipedia themselves, reaching a larger audience than almost any other science communication medium. In some ways, this is the opposite of how GLAM (Galleries, Libraries, Archives and Museums) institutions are used to working: collaborating with non-experts, releasing imperfect and unfinished content, abandoning branding opportunities, and no longer being the single voice of authority. But if we’re serious about being relevant to our public, we need to meet them where they are – which is on Wikipedia.

Local agricultural institutions as an important structure to support the food security system have been marginalized and replaced by formal social institutions that seem foreign to certain communities. This study aims to reveal and explain the existence and role of local agricultural institutions in strengthening the food security of rural communities on the island of Lombok. The research was designed with a qualitative research model. Data collection using several techniques, namely: in-depth interviews, direct observation and literature study. Data analysis used qualitative analysis with interactive models. Analyzes were carried out during data collection and after the end of data collection. The results showed the existence of agricultural institutions in rural areas of Lombok island based on: land control, production processes, provision / utilization of labor, irrigation, harvest and production sharing, storage and distribution of results, the roots of cultural values ​​of the Sasak ethnic community which are the spirit of institutional development of food security are: a sense of togetherness, help to help, peace and peace of life, obedience and decency (the will to build self-image), and harmony in social and spiritual life. The roles and functions of local institutions for the Sasak ethnic community are: Regulating the system of division of labor, distribution of products and income, strengthening social cohesiveness, managing resources, strengthening social participation, building social harmony, and strengthening the food security system.

<p>The current paper features the following pressing issue: how to form a positive image of the region in schoolchildren at the lessons of regional geography. The authors emphasize the direct connection between the issue in question and the sustainable development of the region in the future. The authors point out that the crucial role belongs to the interaction between schools and universities in preparing students for practical activities in the conditions of sustainable development of regions. The paper offers methodical techniques that increase the effi of formation of a positive image of the region during a school course of local geography. The techniques are demonstrated with specific examples that may be used in teaching local geography. The paper enumerates the educational institution the Kemerovoregion that act as sites of formation of a positive image of the region among students in the aspect of sustainable development. It is noted that such methods aid the development of students’ independence and establish connections between disciplines.<strong></strong></p>

It is believed that the historical background of the Chinese community in Malaysia has significant impact on their emotional dimension towards the Malay cultural heritage, which is the heritage of the major ethnic group in Malaysia. This study aimed to examine the emotional expression of the Chinese ethnic towards Malay cultural heritage images in comparison to the Malay ethnic. Three different type of Malay cultural heritage images (Malay traditional food - ketupat, Malay traditional dresses – baju melayu/baju kurung and Malay traditional game – wau bulan) were presented to the Chinese (N=80) and the Malay (N=116) participants, recruited from the undergraduate population of a local institution. Participants self-rated their emotional feedback using a non-verbal pictorial measure (Self-Assessment Manikin) which measured the range of emotional response (valence domain), ranging from one (low valence) to nine (high valence). The Chinese indicated similar level of emotion expression as the Malays in response towards the images of Malay cultural heritage. Both Chinese and Malay showed higher self-rated emotional expression for traditional food image (i.e. ketupat) than non-food images (i.e. Malay dresses and wau bulan). Implication of this study points to the effect of acculturation on the emotional development of the ethnic minority and the emotional property of the food image in constructing the emotion.

Purpose This study aims to analyse the impacts of an institutional change process on a specific higher education institution in Europe and the trade-offs between the faculty perceptions of success and the organization image during this process, in light of the identity institutional theory. Design/methodology/approach The impacts of this institutional change are analysed and discussed based on in-depth interviews conducted with faculty members of the accounting department in which they reflected upon academic success vis-a-vis the career assessment system adopted, followed up by those faculty members’ answering an electronic questionnaire about organizational identity and image perception (Gioia et al., 2000). Findings Considering the individual perspectives, faculty are concerned about their vocations and aspirations, with feelings of apprehension and insecurity, perceiving the institutional goals as too high and potentially unattainable. By shifting the priority towards research, costs in terms of losing the institutional excellence in teaching might arise, which has been traditionally keen to the institute’s organizational identity and consistent with faculty’s perceptions of academic success. Research limitations/implications As in any research endeavour, some limitations might emerge. First, the authors addressed the context of a specific business school, in a European country. It is certainly true that culture plays a role in terms of both organizational and national levels. The authors acknowledge this as a limitation. Nevertheless, this research takes a “local” stance, the logic of academic evaluation and its impacts on institutional and individual identity formation processes is a worldwide phenomenon. Second, in defining the authors’ selection criteria, the authors excluded the possibility of other voices to be heard, both in the department itself and in the business school. Regarding the department, the authors argue that those are the ones who could influence future decisions, considering that they are the only ones eligible for the governing bodies under the institute’s regulations. Regarding the business school, adding other department(s) means adding other discipline(s) to the authors’ analysis with specific and different dynamics of researching, publishing and teaching, which also impacts the expectations regarding career and academic success. Practical implications First, before beginning an institutional change process, it is necessary to assess the vocations and aspirations of its members. The solution requires to reanalyse academic career premises and to reconsider the weights given to each academic activity, or furthermore, to offer more than one career path, so as to make it flexible for each faculty to follow their vocations and aspirations or to adapt to life demands. Second, in terms of organizational identity and image, the challenge is to minimize the gap between the construed external image and the internal identity, striving to achieve a balance between teaching, research, outreach and service. Originality/value Because of the nature of the academic work, the authors propose that the application of the theory should be preceded by a careful consideration of what is academic success. The misalignments studied and reported here reveal a multilevel phenomenon, wherein individual academic identities are often in conflict with the institutional image. The authors’ study entails a contribution to the application of the identity institutional theory to academic institutions.

Ivan Petrović (1973) has been working in the fields of photography and art for twenty years as a researcher, creator and collector. Since 1997, he has been creating and publishing photographic projects that reflect the spirit of space and time in which they are created, while in his works he uses both documentary approaches as well as research principles. In 2011, together with photographer Mihail Vasiljević, he founded a para-institution, the Centre for Photography (CEF). Despite lacking its own premises, infrastructure or funds for performing its activities, the institution deals with the search, preservation, collection and analysis of local photographic materials from recent history. In the past ten years, Petrović also moved his artistic practice beyond mere artistic expression, since he addresses the phenomena of photography from an analytical-theoretical point of view. His interest lies in the nature of the photographic image and its role in society and historiography. In this spirit, long-term projects such as Documents (1997–2008), Images (2002–), Portfolio Belgrade (2015–) and the latest film production were created. The interview with Ivan Petrović took place on 1 September 2017 in Belgrade. The main themes were the role of photography in the dominant history, the boundary between one’s own practice and archival work, photography as an art and the likes. Abstract Objective: The aim of this study was to propose first established diagnostic reference levels (DRLs) in computed tomography (CT) for adults, based on volume-averaged CTDI and dose length product (DLP) metrics in Yazd Province. Materials and Methods: Six multislice CT scanners located at diverse areas of Yazd Province and seven common procedures were selected for the present study. For each procedure, at least twenty patients 18 years and older were sampled at each institution. For each patient, dose report data and scan parameters as well as patient’s information were abstracted from picture archiving and communication system. Results: Proposed DRLs in terms of computed tomography dose index (mGy) and DLP (mGy.cm) were as follows: brain (42, 527), sinus (25, 220), neck (14, 264), abdomen-pelvis (11, 295), routine chest (8, 247), CT pulmonary angiogram (32, 261) and chest HRCT (11, 455), respectively, slightly lower compared to other investigations. Conclusion: The proposed DRLs in this study should be considered as the local DRLs for the seven most common adult CT examinations in Yazd province so as to optimize the patient dose while maintaining acceptable image quality for the clinical task.

e637 Background: Cryotherapy is used in increasing frequency for the primary treatment of localized prostate cancer. While it is known that local recurrence can occur in > 20% of patients treated with primary cryotherapy there is a paucity of data on salvage treatments after failure. The use of external beam radiation therapy is an attractive option after cryotherapy failure but there is little data on efficacy and toxicity. We evaluate our data on a group of patients treated with image-guided dose-escalated radiotherapy (IGRT) at our institution. Methods: An IRB-approved database of patients treated with IGRT for prostate cancer from 2005 to 2013 was used to identify patients who underwent primary cryotherapy followed by salvage IGRT. Patients’ EMR were reviewed for age, cryotherapy-to-salvage interval, pre/post-salvage PSA and Gleason score (GS). All patients were treated with dose-escalated IGRT using standard treatment margins of 3 mm posterior and 7 mm in all other directions and daily CBCT or kv imaging to implanted fiducial markers. Narrative notes were used to identify GU/GI toxicity and scored according to CTCAE v4.03. The Phoenix definition of nadir + 2 was used to determine disease control following RT. Results: 5 patients were identified within the study period as having received salvage RT after primary cryotherapy. Mean age at the time of salvage RT was 74 (range 65-80). Median cryotherapy-salvage interval was 49 months (range 25-97) and prior to RT patients had GS of 7 (n = 3) or 8 (n = 2) and mean PSA of 8.1 ng/mL (range 4.2-14.38). Median total dose and follow-up were 76.7 Gy (range 75.6 – 81.0) and 52 months (range 6-63). One patient’s PSA did not decline post-RT and developed metastases while four patients responded post-RT with one experiencing biochemical recurrence after 30 months. No patients had acute GU/GI toxicities grade 2 or higher; one experienced late grade 2 GU toxicity (dysuria) which resolved with medication. There were no late grade 3 toxicities. Conclusions: High-dose radiotherapy as salvage treatment after cryotherapy failure is tolerated without significant GU or GI toxicities. Additionally, salvage IGRT is a viable option for long-term biochemical control in cases of local recurrence and warrants additional study.

Following the initiation of the policy of ‘Reform and Open Door’, the possibilities for public officials to trade power for private gain in China increased. To tackle the problem of corruption, different levels of Discipline Inspection Commissions (DICs) in the Chinese Communist Party (CCP) initiated investigation related to various corruption cases. However, due to the nature of the administrative set-up in China, the local DICs could not effectively carry out their functions. As a result, the Central Commission for Discipline Inspection (CCDI) was compelled to take on a bigger role, including controlling personnel appointments in local DICs and dispatching inspection teams to local jurisdictions. This strategy also enabled the central government and the top leadership of the CCP to build up a positive image with respect to fighting corruption. Nevertheless, the discretionary power of the top party leaders that has been enhanced through these centralisation measures leads to doubts over the real motives behind the CCP’s anti-corruption efforts. In March 2018, the National Supervision Commission (NSC) was established as the highest governmental anti-corruption agency, but more time is needed to judge the effectiveness of this new institution.

Objectives To evaluate the accuracy of fully automated liver volume quantification vs. manual quantification using unenhanced as well as enhanced CT-image data as well as two different radiation dose levels and also two image reconstruction kernels. Material and methods The local ethics board gave its approval for retrospective data analysis. Automated liver volume quantification in 300 consecutive livers in 164 male and 103 female oncologic patients (64±12y) performed at our institution (between January 2020 and May 2020) using two different dual-energy helicals: portal-venous phase enhanced, ref. tube current 300mAs (CARE Dose4D) for tube A (100 kV) and ref. 232mAs tube current for tube B (Sn140kV), slice collimation 0.6mm, reconstruction kernel I30f/1, recon. thickness of 0.6mm and 5mm, 80–100 mL iodine contrast agent 350 mg/mL, (flow 2mL/s) and unenhanced ref. tube current 100mAs (CARE Dose4D) for tube A (100 kV) and ref. 77mAs tube current for tube B (Sn140kV), slice collimation 0.6mm (kernel Q40f) were analyzed. The post-processing tool (syngo.CT Liver Analysis) is already FDA-approved. Two resident radiologists with no and 1-year CT-experience performed both the automated measurements independently from each other. Results were compared with those of manual liver volume quantification using the same software which was supervised by a senior radiologist with 30-year CT-experience (ground truth). Results In total, a correlation of 98% was obtained for liver volumetry based on enhanced and unenhanced data sets compared to the manual liver quantification. Radiologist #1 and #2 achieved an inter-reader agreement of 99.8% for manual liver segmentation (p<0.0001). Automated liver volumetry resulted in an overestimation (>5% deviation) of 3.7% for unenhanced CT-image data and 4.0% for contrast-enhanced CT-images. Underestimation (<5%) of liver volume was 2.0% for unenhanced CT-image data and 1.3% for enhanced images after automated liver volumetry. Number and distribution of erroneous volume measurements using either thin or thick slice reconstructions was exactly the same, both for the enhanced as well for the unenhanced image data sets (p> 0.05). Conclusion Results of fully automated liver volume quantification are accurate and comparable with those of manual liver volume quantification and the technique seems to be confident even if unenhanced lower-dose CT image data is used.

Abstract Aim The objective of this report is to describe the implementation and pilot-test of an integrated wireless local area network (WLAN) system that incorporated the Planmeca Promax® CCD based digital panoramic/ cephalometric x-ray system, Dolphin® software, and multiple remote user units to increase the efficiency of data management by the residents in the Department of Orthodontics. Background The Department of Orthodontics of the New Jersey Dental School (NJDS) acquired the Dolphin® cephalometric analysis software and the Planmeca Promax® digital panoramic/cephalometric x-ray units on separate occasions. Dolphin has been in use for many years at this institution, the current version being 10, and the Promax was acquired in the Fall of 2002. The digital panoramic and cephalometric radiographs were acquired and stored separately in the Planmeca's Dimaxis database. Report During the incorporation of the WLAN, there was an opportunity to research and install the best available security system for the WLAN so it could be a network model for the other departments within the dental school and perhaps other dental schools around the nation. Summary Digital radiographs, once obtained, can be stored locally or transmitted securely to remote locations via a local area network. This article describes the selection criteria and methodology that would optimize the transmission and retrieval of such images instantaneously on demand at chair side locations. Abstract In the present article, we demonstrate the utility of Open Badge certifications in documenting ESP, EAP and EMI courses. Open Badges are online records of achievement which document field-specific, soft and technical skills. They consist of a visual image and a set of embedded metadata, they are endorsed by the institution which issues them and are recognized at an international level. The information packaged within the badge image file is provided in an open source format and can be shared on social media platforms, as part of an online e-portfolio, as a link on an electronic CV, and on the platform which hosts the Badge. As of 2019, Turin University Language Centre offers Badges in “English for the Social Services” (B1 and B2), “English for Administrative Purposes” (A2, B1 and B2), “Academic Writing and Presentation Skills” (B2 and C1) and “English Medium Instruction” (B2 and C1). We will argue that Open Badge certifications are particularly suited to ESP, since they provide a record of specific hard and soft skills and of innovative teaching and learning practices which go undocumented in conventional certifications and on academic transcripts. We will emphasize the “glocal” quality of these certifications, inasmuch as they are internationally recognized awards tailored by local providers in response to local demands. We will suggest that this perspective is valuable for ESP training. Although ESP learners need to be able to share their credentials on international platforms, using a common language, it is important that we do not lose sight of the specific characteristics of professional activities at a local level. We will further argue that the University Language Centre is, in many ways, ideally suited to issue these kinds of certification.

One way to enhance quality of life for patients with metastatic sarcoma is to maximize time off chemotherapy—a chemotherapy-free interval. While image-guided ablation of sarcoma metastases may reduce the need for chemotherapy, it remains unknown how long ablation could extend the chemotherapy-free interval. The purpose of our study was to determine the chemotherapy-free interval in comparison to overall survival and progression-free survival in sarcoma patients who undergo ablation procedures. An IRB-approved, single institution, HIPAA compliant database was queried for sarcoma patients who underwent image-guided ablation procedures between 2007 and 2018. Patient demographics, histologic subtype, and other clinical characteristics were recorded. Kaplan-Meier analysis was performed to compute median overall survival, median progression-free survival (local and distant), and the median chemotherapy-free interval (systemic and cytotoxic) after ablation. Univariate and multivariate analyses were performed using the log-rank test and Cox proportional-hazards model, respectively. A total of 100 sarcoma patients were included in the analysis. The most common histologic subtype was leiomyosarcoma (38%). Median overall survival after ablation of sarcoma metastases was 52.4 months (95% CI: 46.9–64.0 months). The median systemic chemotherapy-free interval following ablation of sarcoma metastases was 14.7 months (95% CI: 8.6–34.3 months). The median cytotoxic chemotherapy-free interval following ablation of sarcoma metastases was 81.3 months (95% CI: 34.3-median not reached). In conclusion, ablation of sarcoma metastases can provide an extended systemic chemotherapy-free interval of greater than 1 year. Ablation of sarcoma metastases may improve patient quality of life by extending the chemotherapy-free interval.

OBJECTIVE An analysis of factors contributing to durable radiographic control of spinal metastases was undertaken, drawing from a large single-institution database in an attempt to elucidate indications and dose requirements for successful treatment. METHODS All patients treated at a single institution with stereotactic radiosurgery (SRS) of the spine as first-line therapy were assessed for local progression of the treated site, defined as radiographic enlargement of the treated tumor and/or biopsy-proven evidence of active tumor cells. All patients were followed with CT, PET, or MR imaging every 3–6 months until death. Treatment decisions were made by a multidisciplinary team of radiation oncologists, neurosurgeons, and neuroradiologists. Target volumes were defined according to the international consensus guidelines and were reviewed in a multidisciplinary conference. Image-guided techniques and intensity modulation were used for every case. The tumor's histological type, gross tumor volume (GTV), dose that covers 95% of the GTV (GTV D95), percentage of GTV covered by 95% of the prescribed dose (GTV V95), planning target volume (PTV), dose that covers 95% of the PTV (PTV D95), and percentage of PTV covered by 95% of the prescribed dose (PTV V95) were analyzed for significance in relation to local control, based on time to local progression. RESULTS A total of 811 lesions were treated in 657 patients between 2003 and 2015 at a single institution. The mean follow-up and overall survival for the entire cohort was 26.9 months (range 2–141 months). A total of 28 lesions progressed and the mean time to failure was 26 months (range 9.7–57 months). The median prescribed dose was 2400 cGy (range 1600–2600 cGy). Both GTV D95 and PTV D95 were highly significantly associated with local failure in univariate analysis, but GTV and PTV and histological type did not reach statistical significance. The median GTV D95 for the cohort equal to or above the GTV D95 1830 cGy cut point (high dose) was 2356 cGy, and it was 1709 cGy for the cohort of patients who received less than 1830 cGy (low dose). In terms of PTV D95, the median dose for those equal to or above the cut point of 1740 cGy (high dose) was 2233 cGy, versus 1644 cGy for those lesions below the PTV D95 cut point of 1740 cGy (low dose). CONCLUSIONS High-dose single-session SRS provides durable long-term control, regardless of the histological findings or tumor size. In this analysis, the only significant factors predictive of local control were related to the actual dose of radiation given. Although the target volumes were well treated with the intended dose, those lesions irradiated to higher doses (median GTV D95 2356 cGy, minimum 1830 cGy) had a significantly higher probability of durable local control than those treated with lower doses (median PTV D95 2232 cGy, minimum of 1740 cGy) (p < 0.001). Patients in the high-dose cohort had a 2% cumulative rate of local failure. Histological findings were not associated with local failure, suggesting that radioresistant histological types benefit in particular from radiosurgery. For patients with a favorable prognosis, a higher dose of SRS is important for long-term outcomes.

In the current old system the roll numbers are called out by the teachers and their presence or absence is marked accordingly which is time consuming and has a lot of ambiguity that caused inaccuracy and inefficiency of attendance marking. The productive time of the class can be utilized very efficiently by implementing automated attendance system. The main purpose of this project is to build a face recognition-based attendance monitoring system for any educational institution or organization where attendance marking is the demanding task. It enhances and upgrades the current attendance system into more efficient and effective as compared to before. This attendance system which uses HaarCascade a machine learning Object Detection Algorithm used to identify faces in an image or a real time video, Local Binary Pattern Histogram (LBPH) a face recognizer algorithm used to extract features and compare by using python programming and OpenCV libraries saves time and efficiently identifies and eliminates the chances of proxy attendance. This model integrates a camera that captures an input image and training database is created by training the system with the faces of the authorized students.

Abstract: This study examines the issue of the sociology of religion at the religious practices of local communities. Kampung Laut selected as a test site for two (2) major consideration. First, the practice of religion in the fom of ritual in Kampung Laut is different from the mainstream that is considered wrong and become targets for streamlining activities. Second, the typical characteristics of Kampung Laut people in the vortex of conflict govemment agencies on claims of ownership of natural resources. Correctional Institution (LP) as part of Kemenkumham, Cilacap regency govemment and PT. Perhutani conflict over land ownership arising from sedimentation along Segara Anak. Conflicts that exist tend to be maintained to a certain political interests. Even more, these institutions construct public issue by positioning Kampung Laut as threatening communities via label illegal loggers and destroyers of ecosystems. Bad image fomed through a process of marginalization is then formed the distinctive character of the community as a community of Kampung Laut resistant and latent. The method used in this research is descriptive qualitative sociological approach. Through sociological analysis, practice different religious rituals that seem to have rationalization based on the theory of action Weber in the category of value-oriented rationality. Different actions with mainstream thought to have more benefits in the context of the struggle for existence of identity in the practice of marginalization carried out by institutions conflictual. In the years during and immediately after the first world war, a number of Scottish burghs considered extending their range of activities to take in the provision of entertainment by means of the cinema, the most significant mass entertainment medium of the early twentieth century. In doing so, they sought to make use of a particular Scottish institution, the common good fund. This article explores the thinking, political, economic and cultural, behind this marked expansion in public enterprise. It examines in detail the working of businesses where full responsibility for cinema operations was assumed, paying particular attention to matters of pricing, programming and the disposition of profits, noting also the factors which made this experiment in municipal trading short-lived. Although largely abandoned by the early 1920s, municipal cinema provided a model for more sustained local initiatives and exemplifies Scotland's deep and varied engagement with the moving image across the twentieth century.

Objective. To evaluate the association between preprocedural hypertension and the risk of prolonged bleeding following image-guided core needle breast biopsy in nonpregnant/nonlactating women. Methods. A single institution-based, retrospective cohort study of 400 women who underwent image-guided core needle breast biopsy was conducted. Males and pregnant and lactating women were excluded. Preprocedural systolic or diastolic blood pressure greater than 140 or 90 mm of Hg, respectively, was defined as hypertension. Prolonged bleeding was defined >15 minutes of local, manual pressure required to achieve hemostasis following the biopsy. Severe bleeding complications defined as clinical significant hematoma formation, prolonged bleeding requiring an ER visit, hospitalization, or surgical intervention were also recorded. Results. The difference in the mean time for which manual pressure was held after biopsy for patients with and without preprocedural hypertension was not statistically significant (13 ± 7 vs. 13 ± 8 minutes, respectively, P = 0.856). There was no difference in the number of patients requiring manual postoperative pressure >15 minutes between those with preprocedural hypertension and the normotensive patients (13% vs. 12%, respectively, P = 0.765). Bivariate analysis demonstrated statistically significant association between prolonged bleeding and current antithrombotic or antiplatelet medication use ( P = 0.010), the use of stereotactic guidance ( P = 0.019), and a tethered vacuum-assisted device ( P = 0.045). The use of a tethered vacuum-assisted biopsy device was the only variable associated with prolonged bleeding in the multivariate model ( P = 0.044). Conclusion. Preprocedural hypertension is not a risk factor for prolonged bleeding following image-guided core needle breast biopsies in nonpregnant/nonlactating women.

The article is devoted to the topical problems of theory and practice of appraisal of documentary heritage objects value in scientific libraries, archives, museums taking into consideration functional and stock specifics of these institutions. Its goals and tasks, principles and axiological criteria are described. It is shown that the main goal of such appraisal is determining the place of documentary heritage objects as the objects of the appraisal in the system of cultural values of modern society, establishing the belonging of these objects to cultural heritage objects of world, national, regional, local significance. Appraisal of documentary heritage objects is analyzed as one of important areas of scientific and administrative activities which to a large extent ensures the functioning, improvement of information potential and accordingly the image of the library, archives, museum. The Ukrainian experience of the recent decades is highlighted in many aspects, namely: regulation, organization and appraising documentary heritage objects at the state, interdepartmental and departmental levels, at the level of the profile institution fund-holder. The features of the appraisal, the objects of which are archival heritage objects and book heritage objects, are revealed. It is emphasized that the appraisal of value of the existing funds and collections of documentary heritage objects is the basis for the development and implementation of modern individual concepts and integrative projects for libraries, archives and museums, which provides for the coexistence and interrelation of material expositions and various forms of virtual information.

This study aimed to establish the local diagnostic reference levels (LDRLs) of computed tomography pulmonary angiography (CTPA) examinations based on body size with regard to noise magnitude as a quality indicator. The records of 127 patients (55 males and 72 females) who had undergone CTPAs using a 128-slice CT scanner were retrieved. The dose information, scanning acquisition parameters, and patient demographics were recorded in standardized forms. The body size of patients was categorized into three groups based on their anteroposterior body length: P1 (14–19 cm), P2 (19–24 cm), and P3 (24–31 cm), and the radiation dose exposure was statistically compared. The image noise was determined quantitatively by measuring the standard deviation of the region of interest (ROI) at five different arteries—the ascending and descending aorta, pulmonary trunk, and the left and right main pulmonary arteries. We observed that the LDRL values were significantly different between body sizes (p < 0.05), and the median values of the CT dose index volume (CTDIvol) for P1, P2, and P3 were 6.13, 8.3, and 21.40 mGy, respectively. It was noted that the noise reference values were 23.78, 24.26, and 23.97 HU for P1, P2, and P3, respectively, which were not significantly different from each other (p > 0.05). The CTDIvol of 9 mGy and dose length product (DLP) of 329 mGy∙cm in this study were lower than those reported by other studies conducted elsewhere. This study successfully established the LDRLs of a local healthcare institution with the inclusion of the noise magnitude, which is comparable with other established references.

Organizing the major events which have non-negligible impact on economical, innovative, technological, investment processes has been limited due to the new coronavirus infection (COVID-19). Decision-making process of putting limitation on activities organization has been derived from external environment adverse events aligned with current sanitary-epidemiological situation. There has been analyzed the potential loss due to cancelling or rescheduling congress-exhibition events in 2020. Demand and supply reduction necessitate searching alternative development directions and core business diversification. In the capacity one of potential and perspective directions There was stated a social activity vector of Roscongress foundation as a non-financial development institution and organization of Saint-Petersburg International Economic Forum, and Expoforum International, Ltd. as operator of holding forum venue. Saint-Petersburg International Economic Forum has endowed with not only economic impact but also influence on forming-up communications intended to favor realization of the social programs during offline traditional holding event. There have been analyzed the quantitative indicators dynamics of St.-Petersburg International Economic Forum and qualitative indicators in the context of thematic zones of social orientation, which gives the ground to state the wide institutional influence on social economic processes in tourism destination and in other fields. Cooperation programs diversification with local communities has been allowing positive image creation as one of elements of destination strategic promotion on the world congress and tourist map.

Festivals not only fulfill their traditional role as an institution that allows to present, spread or preserve the culture of a society, but also stand out for their ability to generate wealth in the cities where they are celebrated, for their contribution to the diversification of the tourism product or the improvement of the image of the place (Devesa, Báez, Figueroa and Herrero, 2012). Specifically, film festivals have a much greater importance playing a fundamental, but not recognized, role for the development of the industry and the history of cinema (Vallejo, 2014, p.14). Under this perspective, the main objective of this work is to observe the organization of the SCC Express Short Film Festival, in order to propose a sequenced model for the organization of events as a tool for communication and promotion of the cultural industry. The application of the methodological design designed for the application of the proposed case study allows to establish a 9-phase model for the organization of festivals as a technique of promotion and cultural development at the local level.

The 2019 Simultaneous General Election was the first election joined by the Indonesia Solidarity Party (Partai Solidaritas Indonesia or popular as PSI) to compete. In the domain of the Regional House of Representatives, the party won the most seats in the Province of the Special Capital Region of Jakarta compared to other provinces that were won by the PSI. This study aims to determine the efforts and strategies used by the Indonesia Solidarity Party, especially in the Province of Jakarta, as well as the image that was generated during the 2019 Simultaneous General Election to the public so that they could win the most seats of the Regional House of Representatives from all other provinces. This study focuses on the realm of political communication and uses the 4Ps political marketing theory (product, promotion, price, place) which was adapted from the marketing theory by Firmanzah. This study also uses a qualitative approach with case study method. The results of the research found that Jakarta Indonesia Solidarity Party focused more on candidates’ campaigns rather than promoting the party as an institution. The use of digital media as a means of promotion during the campaign period was also carried out by the Indonesia Solidarity Party, but conventional methods such as the usage of banners, blusukan (face to face visit) activities, and the distribution of merchandise to the public were also carried out in the context of political marketing by Jakarta Indonesia Solidarity Party. This article deals with the continuous flow of resources, values and goods that takes place within a Cape Verdean institution called tabanca. It examines the effects of some practices of the so-called Cape Verdean diaspora on local forms of sociality in Santiago's tabancas, in order to show that these flows have a remarkable conservative tendency and contribute to the reproduction of traditional forms of social organization. The Cape Verde I present in this article is at variance with the standard image of the country in current anthropological literature, which approaches social life in the archipelago using analytical tools developed in interdisciplinary fields such as globalization theory and post-colonial, transnational or diasporic studies. Through the ethnographic analysis of the flows within the tabanca, I put the Cape Verde case in the general context of West African political culture to argue that some of its attributes, which appear in literature on transnationalism, diaspora and globalization as the outcome of contemporary transformation, can best be explained in terms of a conservative structural continuity with the political culture that evolved in the northern part of West Africa, known as Senegambia.

✓ The majority of cutaneous squamous cell carcinoma (SCC) are diagnosed early and cured using local treatment, although a minority of cases metastasize to regional structures. In this report the authors describe an unknown feature of skin SCC, namely, distant brain metastasis. This 54-year-old man, who had undergone surgery for moderately differentiated SCC of the dorsum (T2N0M0 stage), was admitted to our institution 11 months later with headache, vomiting, and ataxia. A magnetic resonance image documented a cerebellar lesion, which was totally removed. Results of histological studies revealed SCC. The patient received whole-brain radiotherapy (30 Gy over 2 weeks using a linear accelerator). A metastatic work-up showed enlarged inguinal and para-aortic lymph nodes that were histologically examined using excisional biopsy. Inguinal lymph nodes were tumor-positive and were dissected. The patient was subjected to two cycles of chemotherapy with cisplatin (75 mg/m2). After 3 months, a significant reduction in the size of the para-aortic lymph nodes was documented on control computerized tomography studies. Although the described case is unique, knowledge of the potential for this uncommon behavior in cutaneous SCC may be useful, especially because of its increasing incidence.

Background and Objectives. Hemipelvectomy is a major surgical procedure that associates with significant morbidity, functional impairment, and psychological and body image problem. Reconstruction of the defect is a challenged since a large amount of composite tissues are needed. We would like to share our eight-year experience with massive pelvic resection and reconstruction.Methods. A retrospective analysis of all cases of hemipelvectomy was conducted in our institution over eight-year period with particular attention given to the reconstruction choices and associated complications.Results. Thirteen patients were included with median age of 39 years (range 13–78) of which all had advanced tumour with stage IIb (54%) and Stage III (46%). External hemipelvectomy was performed in all cases, and resultant defects were reconstructed with variety type of flaps. These include fillet thigh flaps, regional pedicle flaps of different designs, and free flap.Conclusions. Massive pelvic tumour is rarely encountered in our population but can be seen across all age groups and usually due to late presentation. The defects should be reconstructed using local or regional flaps, incorporating the muscle component to enhance flap perfusion. The tissue should be harvested from the amputated limb, as it can limit the donor site morbidity.

Purpose: Lumbar epidural is an established procedure, which is commonly used to control pain in cases of disc herniation secondary to lumbar disc disease. We conducted the present study to evaluate the clinical effectiveness of lumbar epidural injection using image intensifier comparing to injection using loss of resistance technique with no fluoroscopy. We also evaluated the outcome of these patients with patients treated conservatively with opioid or other analgesia as a control group. Setting: Epsom General Hospital, an elective part of Epsom & St Helier’s University Hospital spine services. Study Design: Prospective randomized controlled trial. Period: April 2013 and October 2013. Material and Methods: Single center, after seeking appropriate ethical approval in our institution. Group allocation for loss of resistance and II was done by list schedulers independent to the knowledge of trial, operating list become available to research team on the day of procedure. Fifty-five patients were included in each group. A single clinician performed all procedures and the same combination of local anaesthetic and steroid were injected in all patients. Preoperative and postoperative visual analogue score (VAS) and Oswestry Disability Index (ODI) were obtained for each patient. Conclusions: There is a better outcome demonstrated in both injection groups when compared with control. The post-operative scores did not statistically differ significantly between fluoroscopic and loss of resistance groups. The final results of this trial question the efficacy and cost effectiveness of epidural injections using image intensifier and may be used as a reference to change the current practice within several hospitals as cost saving measure. Abstract Objective: To assess the feasibility of Facebook to promote a radiology education project and to expand it from our university community of medical students to a wider audience. Materials and Methods: A group of 12 medical students created a Facebook page in June 2015, to contribute to radiology education in our university. From August 2015, clinical cases, including a brief explanation of clinical findings, along with different imaging modalities, were posted weekly and subscribers were encouraged to choose the most appropriate diagnosis. All cases were followed by the appropriate answer and an explanation to highlight imaging findings and diagnosis. Aiming to reach a larger audience, we also shared cases to a public Latin-American Facebook group, comprising a collective total of 28,182 physicians and medical students. Using the Facebook Insights tracking tool, we prospectively analyzed subscriber interaction with our page for 14 months. Results: During the period analyzed, 35 cases were posted. The most common imaging modalities were X-ray (n = 15) and computed tomography (n = 13). Before we began posting the weekly cases, our page had 286 likes. By October 2016, that number had grown to 4244, corresponding to an increase of 1484% and eight times the size of the medical student community at our institution (n = 530). Medical students made up most (76%) of the subscribers, followed by radiology residents (6%). An excellent or moderate contribution to personal image interpretation skills was reported by 65.3% and 33.1% of the users, respectively. Conclusion: Creating a Facebook page and posting weekly clinical cases proved to be an effective method of promoting radiology education.

38 Background: To evaluate semiautomated quantitative analysis of the BPE in postmenopausal women who were diagnosed breast cancer and to correlate it with mammographic breast density and aggressiveness of the primary cancer. Methods: Informed patient consent was waived after institutional review board approved this study. Four hundred eighty two breast cancer patients performed breast MRI in our institution during 16 months, and 84 postmenopausal women of them were included in this study (median age, 67.2 years; range, 60-76 years). Ordinary values of BPE and breast density were described by an experienced breast radiologist using the BIRADS lexicon. The BPE was calculated with semiautomated quantitative method for quantitative analysis: BPE coefficient (BEC = SI1-SI0 / S0 × 100), where SI is signal intensity, SI1 is the SI enhancement measured in the largest dimension of the sagittal plane at the first postcontrast image, and S0 is the SI before contrast injection. On surgical pathology, pathologic subtype, nuclear and histologic grade, estrogen receptor (ER), progesterone receptor (PR), HER2≠neu, venous and lymphatic invasion, lymph node metastasis was evaluated. And then, the mean BEC of the patients who diagnosed local or systemic recurrence on follow-up was calculated and compared with that of the patients without recurrence. The statistical analysis was performed using Kruskal-Wallis test and Mann-Whitney U test. Results: The mean BEC of the postmenopausal women was 3.96 ± 6.71 in right breast and 3.41 ± 8.09 in left breast. There was no significant difference in the BEC between mammographic breast density groups (P > 0.05). There was no significant correlation between BPE and aggressiveness of the primary breast cancer. However, the mean BEC was higher in the recurrent patient group (n = 6, mean=16.23 ± 19.88) compared with the patients without recurrence (n = 78, mean=2.44 ± 5.62). Conclusions: No association was found between BPE and aggressiveness of the primary breast cancer, BPE and breast density. However, the mean BEC was higher in the patients who diagnosed local or systemic recurrence.

<p>In public space there is the information, that is always designed with a specific purpose. For example, signposts are placed to provide direction guidance and to highlight some of the most important objects. Public signs function as the visiting cards of some institution or enterprise, creating indirectly a definite image of these institutions or some ethnic or social groups, while graffiti is written to create and maintain a public image and to express emotions or attitudes towards some person, a group of people, events or processes. To achieve the expected objective the authors of signs often use the eye-catching texts that differ from linguistic and para-linguistic means, such as artistic expressive means, unusual combinations of words and cultural signs, bright colors, variations of letter fonts, shapes and sizes, different types of images and symbols.</p><p>There have been selected three types of signs available in the linguistic landscape database consisting of 7347 public signs obtained in nine Baltic cities: firstly the largest group of signs by its percentage – the signs (ergonyms), which function as visiting cards of the institution or enterprise and which attract potential customers; secondly, these are graffiti texts, that are the least regulated language signs, reflecting free linguistic and semiotic experiments; and thirdly, these are advertisements, which more than any other public texts contain visual information and referral to a reader with rhetorical phrases and metaphors. In some cases, in order to determine the dissemination of any verbal text approach in the public space, posters are also viewed.</p><p>The goal is to identify and characterize the most peculiar development means of excerpted public signs (linguistic and para-linguistic), as well as compositional characteristics of open space advertisements of institutions and enterprises. There has been used the semiotic landscape theory, in which the language is viewed in correlation with the visual discourse, culture and a way of thinking, accepting multimodality of signs (Kress, Van Leewen 1996; Scollon &amp; Scollon 2003; Jaworski, Thurlow 2010). There have been applied as well a theory of linguistic creativity and a theory of optical metaphors, which allow to determine and analyze different language and semiotic means, rarely used in public reports, as well as to interpret the transfer of meanings (Zawada 2005; Бутакова 2013; Kessler 2013). Interviews and e-mails to employees of companies and institutions justify a creation process and promotion of definite ergonyms.</p><p>The original creation of elements of public reports are viewed in the article according to five language levels proposed by the onomastic J. Butakova: orthographic and graphic, word-formation, lexical, morphological and syntactical level. There was also discussed about the semantic level, highlighting less frequently found thematic groups of ergonyms and metaphorical messages, as well as about the semiotic level, describing compositional characteristics of open space advertisements of definite institutions and enterprises.</p><p>Overall, public signs of the Baltic States are characterized by initial capitalization throughout the text, nominal phrases, lack of punctuation marks, a variety of signs and symbols, which directly reflect the scope of activities, functionality and specificity of institution or company, product or service. Only in signs of names it is typical to use nomenclature names and other onyms (antroponyms, toponyms), in advertisements there are large images, rhetorical questions or exclamation sentences, but graffiti is a visual magnification of casual users’ handwriting, stylized signatures or inaccurately written personal names, slangs with a negative connotation.</p><p>Orthographic and graphic level depicts the broadest diversity of linguistic and semiotic means, especially in the signs of names and graffiti. The most commonly found differences are the usage of lower and upper case of initial letters, variations of their artistic design, highlighting a particular letter of the sign, changing the size, font, color of a word or a part of a word, as well as ignoring the rules of language. Some graffitis demonstrate the examples of graphic hybridization, combining different pattern systems in a single report.</p><p>Word-formation is revealed in the article only in the signs of names, determining the fact, that the formation of diminutives, compound names, abbreviations and foreign formants is relatively rare, wherewith it is considered as a kind of linguistic approach in creation of signs.</p><p>Since the public signs selected for the research, are characterized by nominal phrases, mostly substantives and groups of words, then in morphological and syntactical point of view there are interesting examples, which are only the samples of the usage of adjectives, verbs or linking words and which have a full grammatical center (especially in ergonyms and graffiti). The fact that there are almost no punctuation marks in public signs, the usage of quotes and commas in ergonyms, as well as doubling or tripling of exclamation marks or the usage of full stops, ellipses and question marks in graffiti and advertising is considered to be a kind of linguistic technique to attract the attention.</p><p>Creative linguistic techniques in a lexical semantic level are considerably rarely used nomenclature words in name signs, symbolic ergonyms in the full names of public institutions, actualization of cultural and historical information in the local environment, time category, mythological and literary characters, human character traits, behavior and properties in ergonyms, romantic dedications and poems in graffiti, and linguistic metaphors in ergonyms, advertisements and graffiti.</p><p>Semiotic public signs attract attention if they contain visual images and metaphors are the only information providers and if there is an untraditional layout, unusual shape or interesting combinations of visual elements. Some institutions and companies demonstrate a uniform compositional message in their open space advertisements.</p><p>In general, it can be maintained that both linguistic and semiotic means play an essential role in providing and perceiving general message of public signs. There are not so many techniques for creative text presentations in the Baltic States’ public space, but they reflect authors’ perception of their scope (institution or company, products or services, events), near or distant space and values.</p>

Abstract OBJECTIVE To evaluate the efficacy of CyberKnife (Accuray, Inc., Sunnyvale, CA) stereotactic radiosurgery (SRS) for patients with brain metastases of malignant melanoma and renal cell carcinoma. METHODS We conducted a retrospective review of all patients treated by image-guided radiosurgery at our institution between March 1999 and December 2005. Sixty-two patients with 145 brain metastases of renal cell carcinoma or melanoma were identified. RESULTS The median follow-up period was 10.5 months. Forty-four patients had malignant melanoma, and 18 patients had renal cell carcinoma. The median age was 57 years, and patients were classified as recursive partitioning analysis Class 1 (6 patients), 2 (52 patients) or 3 (4 patients). Thirty-three patients had been treated systemically with either chemotherapy or immunotherapy, and 33 patients were taking corticosteroids at the time of treatment. The mean tumor volume was 1.47 mL (range, 0.02–35.7 mL), and the mean prescribed dose was 20 Gy (range, 14–24 Gy). The median survival after SRS was 8.3 months. Actuarial survival at 6 and 12 months was 57 and 37%, respectively. On multivariate analysis, Karnofsky Performance Scale score (P &lt; 0.01) and previous immunotherapy/clinical trial (P = 0.01) significantly affected overall survival. One-year intracranial progression-free survival was 38%, and local control was 87%. Intracranial control was impacted by whole-brain radiotherapy (P = 0.01), previous chemotherapy (P = 0.01), and control of the primary at the time of SRS (P = 0.02). Surgical resection had no effect on intracranial or local control. Radiographic evidence of radiation necrosis developed in 4 patients (6%). CONCLUSION CyberKnife radiosurgery provided excellent local control with acceptable toxicity in patients with melanoma or renal cell brain metastases. Initial SRS alone appeared to be a reasonable option, as survival was dictated by systemic disease.

AbstractThe importance of the honours system as an institution in British politics and public life has frequently been underestimated. At the end of the First World War, the British government prioritized voluntary service to the state as an area which the honours system should reward more than others through the newly created Order of the British Empire. However, after the war the Order changed to focus more on civil servants, soldiers, and the broad category of ‘local service’. The latter could include volunteers, but more often did not. Various attempts to democratize honours through reforms from the 1960s focused on rewarding a wider range of service. The most successful of these was John Major's honours reform programme in 1993, which returned volunteer service to the forefront of the public image of honours. While these reforms were not as egalitarian as they seemed, they were successful because they integrated an ideology of crown honours with the other functions of the modern monarchy and opened up the honours system to a wider potential set of recipients. At the same time, they maintained a hierarchical structure that meant that elites who had traditionally enjoyed the exclusivity of high honours continued to do so.

On the evening of 8 October 1973, a group of physicians led a watchnight ceremony on the campus of the Korle Bu Teaching Hospital in Accra, Ghana. At midnight, Dr. Portuphy-Lamptey, the Chief Medical Administrator, pulled a lanyard to raise an official flag inaugurating the Hospital's Golden Jubilee Anniversary. The next day, the Ghanaian Commissioner for Health, Lieutenant Colonel A.H. Selormey, unveiled an anniversary plaque that thanked and praised the hospital staff who had worked at Korle Bu over the past fifty years. In a speech to assembled dignitaries, Selormey appealed to Ghanaians to use the Golden Jubilee Celebration as a means of arousing a “full consciousness” of Ghana's “great heritage.” In the months that followed, the 50th Anniversary Celebration Committee organized a series of events to commemorate the role of the institution in the history of the Gold Coast and Ghana, including a formal dinner during which the assembled guests joined together to sing Korle Bu Oyiwala doŋŋ, a popular tribute to the hospital sung in the local language of Ga (see Figure 1). Several months later, at the closing ceremony, the Committee unveiled a statue of Gordon Guggisberg, the British governor credited with building the hospital, an iconic image that is still standing in front of the hospital today.

The article examines the linguistic features of the heroization of medical workers in the mass media discourse during the pandemic of the novel coronavirus infection. During the fight against the spread of the coronavirus, doctors acquired the status of heroes. The heroic image of a doctor who is at the forefront of the fight against a new threat is fully expressed in various discursive and non-discursive constructs: mass media discourse, creolized texts, photo and video materials, comments from users of the Internet services, etc. The headlines of publications, the names of projects, competitions, and actions indicate the heroization of doctors. The process of heroization is also implemented in creolized texts of social advertising. The main linguistic means of heroization is metaphorization: the doctor is represented as a soldier in war, a superhero, a guardian angel. So, in the studied texts, the fight against the coronavirus is metaphorically represented as a war, medical personnel – as soldiers fighting with weapons in their hands against a terrible enemy – the virus, the walls of a medical institution – as a battlefield. As a result of the study, it is revealed that there is a tendency to local heroization and concretization of the heroic on the examples of real people

4622 Background: To evaluate the benefits of adjuvant intra-operative radiation therapy after curative resection in advanced pancreatic cancer (APC) patients, a multi-center phase III trial was conducted by 11 participating institutions in Japan. Methods: Eligibility included pts with potentially resectable APC (duct cell origin) by image diagnosis. Patients were randomized in a 1:1 ratio to adjuvant IORT or surgery alone less than a week before surgery. Stratification factors were tumor size (TS1/TS2,3,4), location (head/body and tail), and institution. Patients who were assigned to adjuvant IORT arm received IORT after curative resection before reconstruction. IORT consisted of 25Gy with electron beam energies of more than 6MeV and was delivered by the round shaped acrylic cylinder of 6–8cm diameter. The radiation field included the tumor bed and in most cases included the celiac axis, superior mesenteric artery, and the portal vein. The primary endpoint was overall survival. The secondary endpoint was local control rate at 2 years after surgery. Assuming 65 eligible pts in each arm, the study had 0,8 power to detect 20% difference in 2-year survival rate. Results: Between 05/2002 to 12/2006 198 pts were randomized and 153 pts underwent curative resection with assigned treatment. Among the 153 pts with curative resection, seven pts revealed ineligible by the histological examination. Finally full analysis sets were 144 pts. Seventy three pts were in the IORT arm and 71 pts in the surgery alone arm. There was only one hospital death in the IORT arm. Cause of the death was intraabdominal arterial bleeding due to pancreatic fistula. Conclusions: We now are collecting the final follow up data. The final analyses will be presented at the meeting. No significant financial relationships to disclose.

Category: Bunion Introduction/Purpose: Over 130 techniques have been described for the treatment of hallux valgus, ranging from bunionectomies, osteotomies, and arthrodesis. Modern attempts at bunion correction with less invasive joint sparing approaches, such as suture-button techniques, have introduced additional complications like second metatarsal fracture. Suture fixation techniques have evolved in an attempt to avoid these complications. We present our results, with up to 26 months of radiographic follow up including initial correction and complications, with a novel device which utilizes a 3D printed titanium plate on the lateral cortex of the second metatarsal without bony violation, and reduces the 1-2 intermetatarsal angle using suture tape passed through drill holes in the first metatarsal with PEEK interference screw fixation. Methods: This study includes 33 feet in 31 patients. A single board certified and fellowship trained orthopedic foot and ankle surgeon performed all operations over the course of 18 months at a single institution. He utilized a dual incision approach and followed previously described surgical technique for the application of the device. We retrospectively reviewed radiographs including pre-operative AP, lateral, and oblique images, along with another set at the two and six week post operative periods, and another at the last available follow up visit. Using standard technique we measured the 1-2 intermetatarsal angle (IMA), as well as the hallux valgus angle (HVA), which were used as the primary outcomes. Chart review was performed to determine patient satisfaction with the outcome. All secondary procedures and complications were recorded based on the chart review and radiographs. Results: Average radiographic follow up was 15 months. The average initial HVA was 31.4º and IMA was 16.0º. Two-week average HVA was 11.3º and IMA was 7.1º. At six weeks, weight-bearing radiographs showed an average HVA 14.5º and IMA 8.2º. Final radiographic measurements, accounting for either the latest available image or the final image prior to revision, showed average HVA of 24.3º and IMA of 13.7º. Six patients had revision for recurrence or fracture. One underwent removal of symptomatic hardware. Seventeen patients had at least mild radiographic recurrence, without revision. Two patients had an intraoperative fracture of the first metatarsal, and one had an intraoperative fracture of the second metatarsal shaft. There were no infections or wound healing issues. Conclusion: Although this device allows a minimally invasive hallux valgus correction, and provides a powerful initial correction with little bony manipulation, results presented in this study suggest a high rate of radiographic recurrence, and relatively high rates of intra-operative complication, post-operative complication, and revision surgery. We found that more severe deformity had a higher rate of recurrence, suggesting it may be more beneficial in mild deformity, but even in many of these patients we found recurrence and other complications. As a result, this technique has since been abandoned at our local institution with the last procedure being performed in 2016.

PURPOSE The incidence of breast cancer is rising in Nigeria, and one major barrier to care is the lack of affordable and appropriate breast cancer diagnosis by ultrasound (US)–guided biopsy. The prohibitive cost of US devices limits their availability in low- and middle-income countries. The emergence of mobile health (mHealth) imaging devices may offer an acceptable low-cost alternative. The purpose of this research was to perform a comprehensive needs assessment to understand knowledge, use, training needs, and attitudes as regards image-guided biopsy in Nigeria to inform the development of an mHealth-based US-guided biopsy training program. METHODS A multistakeholder needs assessment was conducted at the Sixth Annual African Research Group for Oncology Symposium. Voluntary anonymous surveys were administered to all attendees. A subset of attendees (ie, surgeons, radiologists, pathologists, and nurses) participated in six focus groups. Survey items and interview guides were developed collaboratively with local and international input. RESULTS Surveys focusing on use, training needs, and attitudes regarding US-guided biopsies were completed with a 55% response rate (n = 54 of 98) among participants from 22 hospitals across Nigeria. Respondents expressed dissatisfaction with the way breast biopsies were currently performed at their hospitals and high interest in having their institution participate in a US-guided biopsy training program. Focus group participants (n = 37) identified challenges to performing US-guided procedures, including equipment functionality and cost, staff training, and access to consumables. Groups brainstormed the design of an mHealth US-guided biopsy training program, preferring a train-the-trainer format combining in-person teaching with independent modules. CONCLUSION A multidisciplinary needs assessment of local stakeholders identified a need for and acceptability of an mHealth-based US-guided biopsy training program in Nigeria.

Purpose Country of origin has profound effects on consumer behavior; yet few studies have examined an antecedent of these effects: why some countries enjoy a positive image while others suffer a negative one. Developing an institutional theory of country image, the authors argue that weak legal institutions at the country level increase firm opportunistic behavior that expropriates consumers and decrease the product quality of local brands, thus decreasing the country’s image regarding its products and brands. Design/methodology/approach This study measures country image for products and brands using the number of valuable brands (i.e. brands included in the top 500 brands from 2008 to 2016) in a particular home country. Data concerning the rule of law in each country come from the World Bank, and data on the efficiency of countries’ judicial systems comes from Djankov et al. (2007). We also collect patent data from the US Patent and Trade Office, national culture from Hofstede Insights and GDP and GDP per capita from the World Bank as control variables. Panel Poisson regression, Tobit regression and truncated regression are used in the analyses. Findings Supporting the institutional theory of country image, both the rule of law and efficiency of the judicial systems show positive and significant effects on country image, even when economy size (GDP), degree of economic development (GDP per capita), level of technology and skill (patents) and culture are controlled. Practical implications To improve their country’s image and the brand value of local firms, policymakers should strive to strengthen legal institutions aimed at punishing firm opportunistic behavior in their countries. Originality/value Previous research on the country-of-origin effect has not yet appreciated the role of legal institutions in developing the construct of country image.

The purpose of this study was determine whether there was differences election crisis communicationresponse by category SCCT Theory in local media reports Solopos and Joglosemar period from februari until march 2014. The result showed that there was not difference in the election crisis communication response by SCCT Theory. This is evidenced by the data that the categorization of crisis response in the media Solopos was same categorization of crisis response found in Joglosemar media. Categorization crisis response is largest at Solopos that was Attack the accuser with 22 responses (28,9%) of 76 crisis response that was found. Whereas Joglosemar with 12 responses ( 31,5%) of 38 crisis response that was found.Response attack the accuser showed that the organization does not wanted to be blamed on the crisis, it is based on perspektive of the media.

The paper analyses the relation between political elites and political leaders. It is demonstrated that social shifts in the ranks of the elite are reflected in the profiles of heads of state, and the coming to power of a new president results in the renewal of elites. For years of existence of the Fifth republic several generations of political elite were replaced. At the time of Che. De Gaulle highest public servants were the main political actors. The logic of appointments in the system of executive power changed, a new type of political career was created. During the rule of F. Mitterrand, decentralization expanded the ranks of political elite, strengthened the positions of local elites. The president’s fellow party members came to power, political parties turned into an effective mechanism of recruitment of elite cadres. With E. Macron’s election there was a renewal of the deputy corps; reforms which will lead to further changes in the ranks of elites are planned. The paper investigates political biographies and career paths of presidents of the Fifth republic: the path of a notable, the path of a party functionary and the path of a member of administrative bureaucracy. The path of a notable assumes that the politician starts their career from election in local authorities and gradually works their way up through the ranks of power. The path of a party functionary demonstrates that the politician is rooted in party structures. The path of a bureaucrat assumes ascent to the political Olympus through promotion in the executive power branch and also by entering the immediate environment of the president, prime minister, key ministers. Local rootedness, good knowledge of public administration are characteristic of French presidents. Until recently it was thought that to achieve presidency in France it was necessary to be supported by a strong party. The new French president is an exception to this rule, and the movement he created is not a party in the traditional sense but a network entity. The study further brings to light qualities which the leader has to possess. The role of communication in the life of the French president is explained and it is shown how communication shapes the president’s image and influences public trust in the institution of presidency.

In 2015, the Greater Patchogue Historical Society in Long Island, New York, received a gift of nearly 2,000 glass plate negatives dating from the early 20th century. While the donor alluded to rare images of this small town and its people, the collection presented a series of preservation concerns. Many of the objects were soiled, and almost all were still in their original acidic paper sleeves. Determined to both protect and utilize the collection, this small institution, with the assistance of a graduate student intern, formalized a preservation plan that also created multiple points of access to the visual and historical information that these objects contained. After minimally cleaning, rehousing, and photographing a selected portion of the negatives, the digital files were later processed with editing software so that the organization's members and local citizens could see the historic images. In addition to digitizing the negatives, the original sleeves were imaged to preserve valuable information, such as people's names, the location of views, or a negative's date. This ongoing project is an example of how smaller institutions can make a meaningful influence in their local communities by preserving photographic objects and implementing methods of digitization.
