Streszczenie:
1. When a patient goes to a hospital, he is expected to give consent to the treatment his/her doctor may provide. The initial consent, however, does not include all the consents to the following treatment a patient is going to receive. For further treatments, especially for concrete treatments, the doctor must seek for additional consents. 2. A doctor must offer sufficient informations to his patient when getting a consent. While doing this a pre-written patient informed consent form can be convenient as well as effective. The doctor can explain systematically without leaving out any important point, and the patient can understand much better what the doctor is explaining by reading the paper repeatedly. Furthermore this same form can be useful later on when any legitimate question arise. 3. I reviewed many patient informed consent forms previously made by many academies of medical science in specific medical fields. I find the papers lacking in sufficient informations especially on alternative treatment measures. Even if there are, they are described negatively, which make the forms suspicious of giving biased informations. Above all the terms used are too difficult for the patients to understand. I, therefore, propose a patient informed consent form, which I believe might be ideal.

Streszczenie:
Objectives: The objective of this study was to assess the knowledge andpractice of informed consent among dental practitioners in Hyderabad city, Pakistan. StudyDesign: Descriptive cross sectional survey. Setting: Liaquat University of Medical and HealthSciences, Isra University and Dental practitioners working at private Dental clinics in Hyderabadcity. Period: This study was conducted over period of six months from November 2015 toApril 2016. Material and Method: This study used self-administered structured questionnaire, consist of ten questions to assess the knowledge and practice of dental practitioners towardsinformed consent. Data was collected by visiting offices of the study participants at their workplace or through email and courier with prepaid envelope where needed. The participants weresent two reminders at the interval of fifteen days to return the filled survey form. The responseswere represented in form of percentages. The t-test was used to determine the associationbetween the knowledge and qualification, clinical experience, and age. A significance level ofp<0.05 was considered as statistically significant. Results: Our response rate was 86% (103).Majority of the dentist merely knows the meaning of informed consent and overall had poorknowledge (mean score 7.84 + 2.44). The analysis showed that years of clinical practice andqualification was statistically significant with mean knowledge score (p < 0.01). The verbal formof consent was most preferred method of obtaining informed consent that accounted for 82%of study participants. 89% of dentist obtained informed consent from parents when treatingtheir children. Conclusions: From this study, it can be concluded that vast majority of dentistare unaware of informed consent process and their basic requirement. Although majority ofdentist believe that informed consent play an important role in routine dental practice yet manyof them did not use written form of informed consent. Our results would help the educatorsand policy makers to develop educational instruction on bioethics with special emphasis oninformed consent.

Streszczenie:
Introduction: Informed consent is the back bone of patients’ autonomy. Theadvancement in medical technology has further increased its importance. In the developingcountries including Pakistan, general physicians play a vital role in providing health careto the patients but unfortunately majority of them are unaware about the ethical aspects oftheir medical practice. Methodology: Objectives: 1. To determine the level of awarenessabout informed consent among general practitioners. 2. To assess the association betweenvarious socio-demographic factors to the awareness about informed consent. Study Design:Community based cross sectional study. Settings: General medical practitioners of districtHyderabad were the study population. Period Of Study: Two months. Material & Methods:One hundred & forty subjects were selected for the study through purposive non-probabilitysampling. A pretested self-administered questionnaire was the tool for the data collection. Thedata was analyzed by using SPSS version 16.The variables of interest were gender of generalpractitioners, their age, level of qualifications, residence & occasions when informed consentwas taken. The association between various socio-demographic variables was determined byapplying Chi-square test at ≤0.05 level of significance. Results: One hundred & forty generalmedical practitioners of varying ages from 32-60 years participated in the study. The meanage of the general physician was 39 ± 1.8 years. The awareness about informed consent wasfound among 128(91.4%) subjects but unfortunately only 45.7%of them actually practiced it.The results regarding awareness as well as practice of informed consent among males andfemales were however not significant (p=0.520). The young general practitioners i.e age 31-40years were less practicing informed consent as compared to older general practitioners i.e.age 51-60 years and onwards (p= 0.04).The physicians practicing in urban areas were morecognizant about informed consent (p=0.05).Informed consent from patients was obtainedbefore giving local anesthesia (80%), blood transfusion (24.3%)&before examination of femalepatients (46.4%). Conclusions: Informed consent taking is not a routine process adopted bygeneral medical practitioners so there is a strong need for general practitioners to change theirattitude and acknowledge the patient’s autonomy by taking informed consent, which is thebasis of modern medical ethics.
