A growing human population requires sustainable solutions to regulate and dispose of municipal wastewater. Water treatment facilities in northern Idaho are permitted to apply reclaimed wastewater to forest land during the growing season at specified monthly hydraulic loading rates. We assessed the spatial and temporal variability of drainage below the rooting zone between non-irrigated (control) and irrigated (effluent) stands during the growing and dormant seasons in 2021. No drainage was observed during the two months of annual seasonal drought, but large magnitudes of drainage were recorded during the dormant season (38–94 cm), which was consistent with seasonal precipitation. The overall effect of effluent treatment on the drain gauge measurements did not differ from the controls, as effluent only increased the drainage at some facilities. The measured drainage averaged from 35 to 62 cm among facilities. We then used the drainage measurements to calibrate hydrological models (Hydrus-1D and Water Erosion Prediction Project [WEPP]) and predict the drainage in 50 measurement plots distributed evenly among five forest water reclamation facilities. As with the observed drainage, there were no statistically significant growing season differences in the predicted monthly drainage during the growing season between the effluent and control plots, suggesting the successful use of hydrologic models to support the measured drainage findings. While both models struggled to accurately predict the quantity of drainage during the dormant season, they both successfully predicted that drainage would continue through May. WEPP also successfully predicted that the treated plots began to drain in September and October following late-season irrigation at some facilities. The models showed that the prescribed crop coefficient used by the Idaho Department of Environmental Quality was adequate in avoiding drainage during the peak summer months.

ABSTRACT Endangered Species and the use of a Biological Opinion During Spill Response In 2001, six Federal agencies signed an Interagency Memorandum of Agreement (MOA) regarding Oil Spill Planning and Response Activities under the Federal Water Pollution Control Act's National Oil and Hazardous Substances Pollution Contingency Plan and the Endangered Species Act (ESA). The agencies participating in the MOA include the U.S. Coast Guard (USCG), the U.S. Environmental Protection Agency (EPA), the Department of the Interior's Office of Environmental Policy and Compliance and the U.S. Fish and Wildlife Service (USFWS), and the National Oceanic and Atmospheric Administration's—National Marine Fisheries Service (NOAA Fisheries) and National Ocean Service (NOS). In the MOA, NOAA Fisheries and USFWS determined that oil spill response activities qualify as an emergency action, as defined by regulations implementing the ESA in 50 CFR 402.02. As such, the emergency continues to exist until the removal operations are completed and the case is closed in accordance with 40 CFR 300.320(b). To reduce the burden of processing emergency consultation paperwork during every routine oil spill clean-up action that occurs in the Northwest, the USCG and the EPA initiated formal consultation (pursuant to 50 CFR 402.14(c)) with the Northwest Regional Office of NOAA Fisheries on November 12, 2002, and submitted a programmatic biological assessment (BA). The Aassessed the effects of most response activities on ESA-listed species that may be present in the inland waters of Oregon, Washington, and Idaho (salmonids) and the offshore waters out to 200 nautical miles (salmonids, large whales, Steiler sea lion, and sea turtles). On November 6, 2003, NOAA Fisheries completed and signed the nation's first programmatic biological opinion (BO) on oil spill response activities. While NOAA Fisheries determined that the proposed action was not likely to jeopardize the continued existence of listed species or result in the destruction or adverse modification of critical habitat, the agency included reasonable and prudent measures with non-discretionary terms and conditions. The terms and conditions now serve as a “job aid” for oil spill responders in the Northwest and ensure that effects on listed species and their critical habitat are minimized during most response methods that are used. There has been some disagreement regarding the value of conducting formal consultation prior to an actual oil spill event. In addition to the upfront staff time and related costs, there is always the possibility that an incident-specific BA and BO may still have to be done. Moreover, the USCG and EPA have not yet initiated an analogous programmatic consultation with the US Fish and Wildlife Service (USFWS) so incident-specific consultations are ongoing for ESA-listed species under their jurisdiction. This paper presents the background, process, and outcomes (including pros and cons) in the development of a successful programmatic consultation on oil spill activities.

In September 1996, when I was nearing the end of my sixth year as a Justice Department environmental crimes prosecutor, one of my colleagues sent me an email that there was a “good-sounding RCRA [Resource Conservation and Recovery Act] knowing endangerment case developing in Idaho.” A twenty-year-old man named Scott Dominguez had collapsed inside a storage tank at an Idaho fertilizer manufacturing facility called Evergreen Resources. Mr. Dominguez could not be rescued for nearly an hour, because firefighters who responded to the scene did not know what was in the tank and what safety precautions they needed to take before entering the tank. The owner, Allan Elias, insisted that there was nothing in the tank that could hurt anyone, but later investigation would reveal that Elias had used the tank to conduct a cyanide-leaching operation at another facility he owned. By the time Dominguez was rushed to an area hospital, he had suffered permanent brain damage from cyanide poisoning. There was enough cyanide remaining in the tank to kill tens of thousands of people, based on total cyanide levels.

This article describes a rural education project supported by the Idaho Department of Education in which nine education teams participated in team building training. Project staff provided inservice and monitoring to the teams for purposes of establishing transdisciplinary teams, facilitating the education of children in the least restrictive environment, and attaining quality education for all students through heterogeneous grouping. A formalized approach was used to assist the teams in improving their teaming status and consequently the quality of services. Behavioral safety is the application of reinforcement theory to foster an increase in “safe behavior.” The process starts with a behavioral hazard analysis to identify unsafe workplace behaviors. A checklist is then developed to assist in the observation of work behavior. Safe and unsafe behaviors are recorded and provided as feedback (reinforcement) to the worker, which increases safe behavior leading to continuous improvement and worker involvement. Developed in the late 1970s, behavioral safety has an impressive track record. Research has shown that as safe behaviors increase, safety incidents decrease. Within the Department of Energy, behavioral safety has been instituted at industrial sites such as the Savannah River Site (SRS) and the Strategic Petroleum Reserve (SPRO), and at national laboratories such as Los Alamos National Laboratory (LANL), Idaho National Engineering and Environmental Laboratory (INEEL) and Lawrence Berkeley Laboratory (LBL). In all cases, implementing the behavioral safety process has led to an increase in safe behavior and a decrease in overall safety incidents.

This article discusses the rapid solidification process (RSP) which is being developed by Idaho National Engineering & Environmental Laboratory, a US Department of Energy research lab in Idaho Falls. This process drastically lowers the costs and lead times of production tools. In an approach that is radically different from conventional tool making, in which a mold core and cavity are machined from a block of steel, RSP equipment creates a shape by spraying molten metal onto a pattern, faithfully reproducing the pattern's shape, details, and texture. Tools can be completed in as little as three days, and are suitable for both prototyping and production runs. RSP differs significantly from other commercial spray metal techniques, broadly known as thermal spray. RSP tooling can undergo either conventional heat treatment or low-temperature heat treatment known as artificial aging. Artificial aging allows the tailoring of steel properties, such as hardness, toughness, and thermal heat resistance, without the risk of tool distortion.

But in a Washington-Idaho dryland grains region, only the use of relatively new and sometimes problematic alternative cropping systems permitted environmental and economic gains under policy reform. In both regions, a recoupling policy, which links government payments to resource-conserving farming practices, was needed to protect environmental quality when market prices for program crops were high.

The objective of this study was to analyze the trends in seven annual extreme indices of precipitation for Idaho, USA. The analyses were conducted for 35 meteorological stations, during the period from 1970 to 2006, a high quality and a fairly long-term dataset. The analyses of precipitation indices presented large spatial variability and few statistically significant trends. Thus it is not possible to conclude that significant changes in precipitation have occurred in this region during the past few decades.Keywords:Climate change, global warming, environmental impact Tendências nos Índices Extremos de Precipitação Diária sobre Idaho - USA RESUMO O objetivo deste estudo foi analisar as tendências em sete índices anuais extremos de precipitação para Idaho, EUA. AbstractIntegrated safety management requires that safety be integrated with basic management and engineering practices. The objective of this work was to add safety to the EIA‐731 Systems Engineering Capability Model Appraisal Method and thereby gain simultaneous and integrated findings. Safety appraisal information was gleaned from the Department of Energy, the Department of Interior, the Private Sector, the Occupational Safety and Health Administration, and other sources. This information was cast as a series of maturity‐oriented questions and added to EIA‐731. The integrated questionnaire was then tailored for application at Yellowstone National Park and the Idaho National Engineering and Environmental Laboratory. Results are not available at this writing due to delays in the scheduling of the appraisal, but will be included in a follow‐up paper. The integrated Safety Systems Engineering Capability Model Appraisal incorporates general safety principles that are applicable to any industry interested in project management, systems engineering and safety.

Nonsurvey input—output models, especially those constructed by means of pool and quotient techniques, are suspect when applied to the case of cross-hauling. This is well known. But the US Department of Agriculture (USDA) Forest Service, through its IMPLAN model, and the US Department of Commerce (USDC), through its RIMSII model, encourage the application of pool and quotient nonsurvey models in cases where cross-hauling is likely to occur. In a study of the timber economy of the West-central Idaho Highlands we show the error caused by ignoring cross-hauling in estimates of logger—sawmill trade. We argue that pool and quotient techniques, used in nonsurvey models such as IMPLAN and RIMSII, should not be applied to a single county situation, or to any aggregation of counties that is not, in some sense, a functional economic area. This applies in many cases to full state models. We close by noting that in many applications of pool and quotient nonsurvey input—output methods, technique may be substituting for thought.

Current approaches to meeting environmental expectations lack a sound, consistent methodology by which the environmental benefit may be quantified in economic terms, thereby enabling cost benefit/cost ratios to be determined and included in the decision-making process. This is being addressed in the United Kingdom through a programme of collaborative research involving the Department of the Environment, the Water Services Association and the Environmental and Economic Regulators. An Interim Benefit Assessment Manual has been produced and is being evaluated by case studies, reporting in 1995 with a revised version of the manual planned to be available in 1996.

Environmental conditions and seedling quality interact to produce complex patterns of seedling survival and growth. Root growth potential (RGP) is one metric of seedling quality that can be rapidly measured prior to planting, but the correlation of RGP and seedling performance is not consistent across studies. Site factors including microsite objects that cast shade and competing vegetation can also influence seedling performance. We examined the effects of RGP, presence/absence of a microsite object, and competition cover on the survival and growth of three native conifers to the Inland Northwest, USA, over 5 years. We found that RGP had no effect on the survival or growth of western larch (Larix occidentalis), Douglas fir (Pseudotsuga menziesii var. glauca), and grand fir (Abies grandis) at a mesic north aspect site and a xeric south aspect site. Comparatively, the presence of a microsite increased the odds of survival by 37% for western larch and 158% for grand fir, while the absence of forb cover increased the odds of survival of western larch by 72% and of grand fir by 26%. Douglas fir was less sensitive to microsites and competition. The strong effects of neighborhood conditions around seedlings help inform silvicultural practices to enhance the establishment of western larch and grand fir, including planting seedlings near shading objects and competition control, while these practices may not be as important for Douglas fir.

Abstract Objective This study aimed to explore the associations between the organization-level quality arrangements, improvement and implementation and department-level safety culture and leadership measures across 32 large Australian hospitals. Design Quantitative observational study, using linear and multi-level modelling to identify relationships between quality management systems and clinician safety culture and leadership. Setting Thirty-two large Australian public hospitals. Participants Quality audit at organization level, senior quality manager at each participating hospital, 1382 clinicians (doctors, nurses and allied health professionals). Main outcome measures Associations between organization-level quality measures and department-level clinician measures of teamwork climate, safety climate and leadership for acute myocardial infarction (AMI), hip fracture and stroke treatment conditions. Results We received 1332 valid responses from participants. The quality management systems index (QMSI, a questionnaire-based measure of the hospitals’ quality management structures) was ‘positively’ associated with all three department-level scales in the stroke department, with safety culture and leadership in the emergency department, but with none of the three scales in the AMI and hip fracture departments. The quality management compliance index (QMCI, an external audit-based measure of the quality improvement activities) was ‘negatively’ associated with teamwork climate and safety climate in AMI departments, after controlling for QMSI, but not in other departments. There was no association between QMCI and leadership in any department, after controlling for QMSI, and there was no association between the clinical quality implementation index (CQII, an external audit-based measure of the level of implementation of quality activities) and any of the three department-level scales in any of the four departments, after controlling for both QMSI and QMCI. Conclusions The influence of organization-level quality management systems on clinician safety culture and leadership varied depending on the hospital department, suggesting that whilst there was some consistency on patient safety attitudes and behaviours throughout the organizations, there were also other factors at play.

Abstract Objective Little is known about the influence that hospital quality systems have on quality at department level, in Australia and elsewhere. This study assessed the relationships between organizational-level quality management systems, and the extent to which hospital-level quality management systems and department-level quality management strategies are related. Design A multi-level, cross-sectional, mixed-method study. Setting and participants As part of the Deepening our Understanding of Quality in Australia (DUQuA) project, we invited all large hospitals in Australia (~200 or more beds) which provided acute myocardial infarction (AMI), hip fracture and stroke care. The quality managers of these hospitals were the respondents for one of seven measures of hospital quality management systems and strategies. Data across the six remaining measures were collected through site visits by external surveyors assessing the participating hospitals. Main outcome measures Relationships were assessed between three organization-level quality management system measures: a self-report measure assessing organization-level quality activities (quality management systems index, QMSI); externally assessed organization-level compliance to procedures used to plan, monitor and improve quality of care (quality management compliance index, QMCI); and externally assessed implementation of quality systems (clinical quality implementation index, CQII). Associations were also assessed between organization-level quality management systems and department-level quality management strategies: how clinical responsibilities are assigned for a particular condition; whether department organization processes are organized to facilitate evidence-based care recommendations; compliance with selected recommendations of international agencies; and whether clinical reviews are performed systematically. Results Of 78 invited hospitals, 32 participated in the study. QMSI was positively associated with QMCI and CQII, but after controlling for QMSI, no relationship was found between QMCI and CQII. There appears to be a cluster of relationships between QMSI and department-level measures, but this was not consistent across all departments. Conclusion This is the first national study undertaken in Australia to assess relationships within and between organization-level and department-level quality management systems. These quality management system tools align with many components of accreditation standards and may be useful for hospitals in continuously monitoring and driving improvement.

In this work, the climatic chamber CORE-CARE is presented. The laboratory was built at the Industrial Engineering Department of the University of Padova, in order to further investigate issues related to indoor environmental quality, the comfort perceived by people in confined spaces and thermal performance of radiant systems. CORE-CARE is equipped with radiant panels in each surface, which can be controlled separately. The room presents two real windows on the East side, equipped with shading systems. Furthermore, the mechanic controlled ventilation system installed in the room can act as integration of sensible or latent heat both in heating and in cooling conditions. The climatic chamber is provided with a data acquisition system, consisting of 38 sensors, that allow the evaluation of the surface temperatures in different positions, the air temperature at different heights, the mean radiant temperature, relative humidity and CO2 concentration. The CORE-CARE chamber is thermally characterized using a detailed model developed at the Industrial Engineering Department of the University of Padova and the simulations results are compared with test measurements. Moreover, the impact of radiant asymmetry is investigated.

Abstract Objective The aim of this study was to develop and refine indices to measure organization and care pathway-level quality management systems in Australian hospitals. Design A questionnaire survey and audit tools were derived from instruments validated as part of the Deepening Our Understanding of Quality improvement in Europe (DUQuE) study, adapted for Australian hospitals through expert opinion. Statistical processes were used to explore the factor structure, reliability and non-redundancy and descriptive statistics of the scales. Setting Thirty-two large Australian public hospitals. Participants Audit of quality management processes at organization-level and care pathway processes at department level for three patient conditions (acute myocardial infarction (AMI), hip fracture and stroke) and senior quality manager, at each of the 32 participating hospitals. Main Outcome Measure(s) The degree of quality management evident at organization and care pathway levels. Results Analysis yielded seven quality systems and strategies scales. The three hospital-level measures were: the Quality Management Systems Index (QMSI), the Quality Management Compliance Index (QMCI) and the Clinical Quality Implementation Index (CQII). The four department-level measures were: Specialised Expertise and Responsibility (SER), Evidence-Based Organisation of Pathways (EBOP), Patient Safety Strategies (PSS) and Clinical Review (CR). For QMCI, and for seven out of eight subscales in QMSI, adequate internal consistency (Cronbach’s $\alpha$ >0.8) was achieved. For CQII, lack of variation and ceiling effects in the data resulted in very low internal consistency scores, but items were retained for theoretical reasons. Internal consistency was high for CR (Cronbach’s $\alpha$ 0.74–0.88 across the three conditions), and this was supported by all item-total correlations exceeding the desired threshold. For EBOP, Cronbach’s $\alpha$ was acceptable for hip fracture (0.80) and stroke (0.76), but only moderate for AMI (0.52). PSS and SER scales were retained for theoretical reasons, although internal consistencies were only moderate (SER) to poor (PSS). Conclusions The Deepening our Understanding of Quality in Australia (DUQuA) organization and department scales can be used by Australian hospital managers to assess and measure improvement in quality management at organization and department levels within their hospitals and are readily modifiable for other health systems depending on their needs.

Although the relationship between environment and public depression has aroused heated debate, the empirical research on the relationship between environmental quality perception and public depression is still relatively insufficient. This paper aims to explore the influence of environmental quality perception on public depression and the mediating role of subjective social class between environmental quality perception and public depression. Using the China Family Panel Studies data of 2016 for empirical analysis, this study’s results show that environmental quality perception has a significant effect on public depression and subjective social class also has a significant effect on public depression. In addition, we found that subjective social class can play a partial mediating role between environmental quality perception and public depression, and the intermediary effect only comes from the contribution of the perception of living environmental quality, not the perception of overall environmental quality. That is to say, the perception of living environment quality deeply affects the subjective social class, and then induces public depression. In order to alleviate the relationship between environmental quality and public depression, it is recommended that the state environmental protection department and civil affairs department strengthen the improvement of public living environment so as to promote individual subjective social class and reduce the risk of public depression. Moreover, it is suggested that research with longitudinal design and comprehensive indicators be undertaken in the future.

ABSTRACTObjectiveWe conducted an environmental scan of quality improvement and patient safety (QIPS) infrastructure and activities in academic emergency medicine (EM) programs and departments across Canada.MethodsWe developed 2 electronic surveys through expert panel consensus to assess important themes identified by the CAEP QIPS Committee. “Survey 1” was sent by email to all 17 Canadian medical school affiliated EM department Chairs and Academic Hospitals department Chiefs; “Survey 2” to 12 identified QIPS leads in these hospitals. This was followed by 2 monthly email reminders to participate in the survey.Results22/70 (31.4%) Department Chairs/Chiefs completed Survey 1. Most (81.8%) reported formal positions dedicated to QIPS activities within their groups, with a mixed funding model. Less than half of these positions have dedicated logistical support. 11/12 (91.7%) local QIPS leads completed Survey 2. Two-thirds (63.6%) reported explicit QIPS topics within residency curricula, but only 9.1% described QIPS training for staff physicians. Many described successful academic scholarship output, with the total number of peer-reviewed QIPS-related publications per centre ranging from 1–10 over the past 5 years. Few respondents reported access to academic supports: methodologists (27.3%), administrative personnel (27.3%), and statisticians (9.1%).ConclusionThis environmental scan provides a snapshot of QIPS activities in EM across academic centres in Canada. We found significant local educational and academic efforts, although there is a discrepancy between the level of formal support/infrastructure and such activities. There remains opportunity to further advance QIPS efforts on a national level, as well as advocating and supporting local QIPS activities.

Abstract Patient safety in health care is the cornerstone of quality in nursing care. It is a duty of nurses and an objective of the health organizations. This article aims to analyze the scientific evidence on the nurses’ perception and opinion on patient safety in the emergency department. Systematic literature review with 3 steps. 1) Primary search at CINHAL and MEDLINE. 2) A broader search, using the same keywords and search terms in the remaining database of the EBSCOHost platform. 3) Search the bibliographic references of the selected articles. The selected studies were published between 2014 and 2019. Five articles were selected. The nurses’ perception reveal that the work environment, teamwork and matters related to the leadership of hierarchical superiors are fundamental factors to improve the quality of care provided and patient safety. Promoting teamwork improves patient care, reduces adverse events and improves quality. Recognizing the nurses’ perception on patient safety culture in emergency services, contributes to improving the quality of care provided.

Abstract Objective The Deepening our Understanding of Quality in Australia (DUQuA) project is a multisite, multi-level, cross-sectional study of 32 of the largest hospitals in Australia. This overview examines relationships between (i) organization-level quality management systems and department-level quality management strategies and (ii) patient-level measures (clinical treatment processes, patient-reported perceptions of care and clinical outcomes) within Australian hospitals. Design We examined hospital quality improvement structures, processes and outcomes, collecting data at organization, department and patient levels for acute myocardial infarction (AMI), hip fracture and stroke. Data sources included surveys of quality managers, clinicians and patients, hospital visits, medical record reviews and national databases. Outcomes data and patient admissions data were analysed. Relationships between measures were evaluated using multi-level models. We based the methods on the Deepening our Understanding of Quality Improvement in Europe (DUQuE) framework, extending that work in parts and customizing the design to Australian circumstances. Setting, participants and outcome measures The 32 hospitals, containing 119 participating departments, provided wide representation across metropolitan, inner and outer regional Australia. We obtained 31 quality management, 1334 clinician and 857 patient questionnaires, and conducted 2401 medical record reviews and 151 external assessments. External data via a secondary source comprised 14 460 index patient admissions across 14 031 individual patients. Associations between hospital, Emergency Department (ED) and department-level systems and strategies and five patient-level outcomes were assessed: 19 of 165 associations (11.5%) were statistically significant, 12 of 79 positive associations (15.2%) and 7 of 85 negative associations (8.2%). Results We did not find clear relationships between hospital-level quality management systems, ED or department quality strategies and patient-level outcomes. ED-level clinical reviews were related to adherence to clinical practice guidelines for AMI, hip fracture and stroke, but in different directions. The results, when considered alongside the DUQuE results, are suggestive that front line interventions may be more influential than department-level interventions when shaping quality of care and that multi-pronged strategies are needed. Benchmark reports were sent to each participating hospital, stimulating targeted quality improvement activities. Conclusions We found no compelling relationships between the way care is organized and the quality of care across three targeted patient-level outcome conditions. The study was cross-sectional, and thus we recommend that the relationships studied should be assessed for changes across time. Tracking care longitudinally so that quality improvement activities are monitored and fed back to participants is an important initiative that should be given priority as health systems strive to develop their capacity for quality improvement over time.

Knowledge of environmental factors impacting anadromous salmonids in their freshwater habitats, particularly at large spatial scales, may be important for restoring them to previously recorded levels in the northwestern United States. Consequently, we used existing data sets and an information-theoretic approach to model landscape-level attributes and snorkel count categories of spring-summer chinook salmon (Oncorhynchus tshawytscha) and steelhead (Oncorhynchus mykiss) parr within index areas in Idaho. Count categories of chinook salmon parr were negatively related to geometric mean road density and positively related to mean annual precipitation, whereas those for steelhead parr were negatively related to percent unconsolidated lithology. Our models predicted that chinook salmon parr would be in low count categories within subwatersheds with >1 km·km-2 geometric mean road densities and (or) <700 mm mean annual precipitation. Similarly, steelhead parr were predicted to be in low count categories in subwatersheds with >30% unconsolidated lithology. These results provide a starting point for fish biologists and managers attempting to map approximate status and quality of rearing habitats for chinook salmon and steelhead at large spatial scales.

IntroductionDelirium is a serious medical condition that is common in older adults in acute settings. Clinical practice guidelines recommend that all older patients in acute care settings should be screened for delirium using standardised outcome measures.ProblemIn our institution, an audit showed that only 16% of older adults presenting to the emergency department were screened for delirium. The goal of this project was to increase the number of patients being screened for delirium using Lean Six Sigma (LSS) methodology and tools and a multidisciplinary approach.MethodA multidisciplinary team in the emergency department used LSS tools and methodology over a 12-week period to first identify why patients were not being screened for delirium using root cause analysis and second to implement a multifaceted intervention including education, audits and feedback, documentation changes and team huddles. An audit was performed at the 11th week of the project to measure how many patients were being screened for delirium post project intervention.ResultsResults at 5 weeks post intervention (11th week of project) showed that the percentage of patients being screened for delirium had increased from 16% to 82%. A follow-up audit at 17 weeks post intervention showed a further improvement in delirium screening to 92%.ConclusionApplying LSS tools and methodology resulted in a healthcare quality improvement. Delirium screening in an emergency department can be improved through multifaceted interventions including education, documentation changes and team huddle changes.

Bottled water is now the biggest selling soft drink in Europe (van Musschenbroek, 1995) with sales topping 700 million litres in 1994 in the UK alone (Natural Mineral Water Association, pers comm). As part of a wide ranging UK Department of the Environment Programme to assess possible risks from the consumption of drinking water from all sources, a survey of the microbiological quality of still bottled water, at the point of sale, was conducted. A total of 17 different brands of still water (both natural mineral and other bottled water) in a variety of different containers (plastic and glass, clear and coloured) were tested for total colony counts at 22°C and 37°C, total coliforms, Escherichia coli, Pseudomonas aeruginosa, aeromonads, faecal streptococci and sulphite-reducing clostridia. Bottles were purchased from various suppliers in order to obtain a range of different sell by dates.

Objective: To assess the effect of melasma on the Quality of Life (QoL) of patients using the Dermatology Life Quality Index (DLQI). Study Design: Cross-sectional study. Place and Duration of Study: Department of dermatology PAF Hospital, Jacobabad Pakistan, from Jun 2019 to 2020. Methodology: A total of 103 patients with melasma were enrolled in the study from the dermatology outpatient department after written informed consent. Patients of either gender aged 15 years and above were included in the study. Melasma Area and Severity Index assessed melasma severity. In addition, the impact on Quality of Life was assessed using the Dermatology Life Quality Index questionnaire. Results: Among the 103 patients, 67 (65%) were females, and 36 (35%) were male. The mean age of patients was 29.50±7.94 years and included 78 (75.7%) married and 25 (24.3%) unmarried individuals. Mean Melasma Area and Severity Index was 22.04±11.23, including 36 (35%) patients with mild, 43 (41.7%) moderate and 24 (23.3%) patients with severe disease, respectively. Overall mean Dermatology Life Quality Index of the sample was 11.25±7.78, while mean scores for the mild, moderate and severe diseases were 5.89±.58, 10.33±5.75 and 20.96±6.62, respectively (p=0.001), indicating that Quality of Life is significantly more impaired with increased disease severity. Conclusion: Melasma significantly affects the Quality of Life of the patients as measured by Dermatology Life Quality Index. The impact on Quality of Life is directly proportional to the severity of melasma assessed by the Melasma Area Severity Index.

On April 20, 2010, the oil rig Deepwater Horizon exploded in the Gulf of Mexico, killing eleven people and injuring seventeen more. Efforts to stop the spill failed. For the next eighty-seven days, hundreds of millions of barrels of oil poured into the Gulf. This catastrophe not only changed the lives of the families of the dead and injured and the communities who experienced the economic and social disruption of the spill – it challenged the survival of the ecosystem of the ninth largest water body in the world. The oil spill extended fifty miles offshore from Louisiana in the Gulf of Mexico and spread over an area larger than the State of Idaho. The oil spill also triggered the onset of one of the most significant civil environmental enforcement actions in the history of our nation. This Article is written by counsel for the Department of Justice’s civil enforcement case in response to the Deepwater Horizon disaster. After examining the litigation of the case from inception through settlement, this article explores the lessons learned from the litigation and the premise that this case demonstrates a proof of concept for the effectiveness of cooperative federalism, the Manual for Complex Litigation and the 2015 amendments to the Federal Rules of Civil Procedure.

BackgroundHyperemesis gravidarum (HG) is a pregnancy complication comprising severe nausea and vomiting in pregnancy. It is associated with adverse outcomes for both mother and child. Treatment consists primarily of antiemetics and intravenous fluids; however, support from healthcare professionals is also important.Local problemAt the department of obstetrics at Nordsjællands Hospital, an increasing workload caused challenges regarding patient care and organisation for patients with HG, and exploring possibilities of reorganising HG care to release midwife resources was warranted.MethodsThrough input from staff and patients, possible improvements were identified. Plan–do–study–act cycles were conducted with staff and patients, resulting in adjustments in care and organisation and thus use of resources. The specific, measurable, attainable, realistic and timely aims included patient satisfaction and number of follow-ups conducted via phone.InterventionsHG care was relocated to the department of gynaecology, where it was managed primarily by nurses. Staff and patients were actively involved in the process.ResultsHG care was successfully relocated without compromising patient satisfaction. Additionally, an option of patient-administered home treatment for selected patients was established.ConclusionThis quality improvement project describes the relocation and set-up of hospital care provided to patients with HG, resulting in high patient satisfaction. This project might serve as an inspiration to other departments of obstetrics and gynaecology.
