Streszczenie:
This examination seeks to determine the influence of proximal density to nurse education resources (nursing schools) on nursing home care quality outcomes in Alabama. Motivated by the social network theory, which highlights the influence of relational closeness on shared resources and values, we hypothesize that nursing homes that have higher levels of nursing education resources within a close proximity will exhibit significantly higher nursing home quality outcomes. As proximal density to nurse education resources increases, the opportunity for nursing homes to build closer, stronger ties increase, leading to higher quality outcomes. We examine this hypothesis via ordered logistic regressions of proximal density measures developed through geographic information systems (GIS) software, nurse education resource data from Johnson & Johnson’s Campaign for Nursing’s Future (n = 37), and nursing home quality outcome data from Centers for Medicare and Medicaid Services’s (CMS) Nursing Home Compare from 2016 (n = 226). The results find that increases in proximal density to nurse education resources have a negative and significant association with nursing home quality outcomes in Alabama. Additional sensitivity analysis, which examines the degree to which the nature of this relationship is sensitive to health care facilities’ location in high-density areas, is offered and confirms principal findings. Because nursing programs generally have stronger ties with hospitals, the findings suggest that the nursing homes in areas with higher nurse education resources may actually face greater competition for nurses.

Streszczenie:
AbstractSince the 1990s, there has been substantial expansion of facility-based alternatives to nursing home care, such as assisted living facilities. This paper analyzes the relationship between expansion of the assisted living industry, nursing home market structure and nursing home private pay prices using a two-year panel of nursing homes in the State of Ohio. Fixed effect regressions suggest that the expansion of assisted living facilities are associated with increased nursing home concentration, but find no effect on private pay nursing home prices. This would be consistent with assisted livings reducing demand for nursing homes by delaying entry into a nursing home, though assisted livings are not direct competitors of nursing homes.

Streszczenie:
A nursing home provides skilled nursing care and rehabilitation services to people with illnesses, injuries or functional disabilities, but most facilities serve the elderly. Nursing homes provide various services for different residents’ needs, including daily care, assistance for the mentally disabled, and drug rehabilitation. The levels of care and quality of care provided by nursing homes have increased significantly over the past decade. The trend is toward continuous quality development and resident satisfaction; therefore, healthcare technology plays a significant role in nursing home operations. This article discusses general information about current nursing home conditions and systems in the United States and explores how technology and e-health help improve the nursing home development based on the present needs and trends. The authors also report on Thomasville Nursing Home, discussing current trends in nursing home technologies.

Streszczenie:
This study examines the association between nursing assistants’ perceptions of their psychosocial work environment and satisfaction among older people receiving care in nursing homes and home care. Cross-sectional surveys were conducted among people receiving care ( N = 1,535) and nursing assistants ( N = 1,132) in 45 nursing homes and 21 home care units within municipal old-age care. Better psychosocial work environment was related to higher satisfaction in old-age care among the recipients. Significant and stronger associations were more common in nursing homes than in home care. Perception of mastery and positive challenges at work were associated with higher recipient satisfaction both in home care and in nursing homes: social climate, perception of group work, perception of mastery, and positive challenges at work only in nursing homes. Findings suggest that recipient satisfaction may be increased by improving the psychosocial work environment for nursing assistants, both in nursing homes and in home care.

Streszczenie:
Getting old involves a lot of changes in life. Family and social relations change and mobility can decrease. These variations require new settings, and of course special care. A nursing home is a place dedicated to help with this situation. Sometimes nursing homes can be perceived as mere institutions by society, and even by future residents. Inside, senior citizens are suppose to spend the rest of their lives doing the same activities day after day. How can we improve these days? Architecture can help. This project deals with architectural design of a nursing home located in an open green area in Blacksburg, Virginia. The project's focus is in creating a home that is related to its surroundings and the proximity to nature. Also deals with all the complexity of this kind of program, but tries to create a homely feeling, and an architectural environment that can adapt to the resident's physical and mental condition over time.

Streszczenie:
Because of increasing costs and demand for nursing home care, studies are needed that can better describe the population of users and improve prediction of clinical outcomes and program requirements. The major purpose of this study was to explore the incremental and seven month outcomes of nursing home patients using the Andersen model. The design was longitudinal. Patients from one Veterans Administration (VA) hospital-based nursing home and six freestanding. VA contract community nursing homes were studied. Functional and cognitive ability were analyzed along with socioeconomic and demographic data. and utilization patterns. A second purpose was to assess associations among variables and their interaction effects in predicting outcome. A third purpose was to assess the contribution of such independent variables as case-mix and rehospitalization rates to possible cost differences evidenced by the two nursing home types. The results of this study suggest avenues for planning and allocation of resources in the two program alternatives. The Barthel Index (BI) (Mahoney & Barthel. 1965) was used to measure functional status and the Short Portable Mental Status Questionnaire (SPMSQ) (Pfeiffer. 1975) for cognitive ability. In addition to standardized measures. sociodemographic and utilization data. perceptions of health and outcomes of care were collected on all subjects. Analytical techniques included descriptive and inferential statistics. The major hypothesis was that veterans in the hospital-based versus contract statistically significant differences in characteristics and on measures of service use and clinical outcome. Findings were evaluated for policy adequacy. adherence to program intent. federal and state cost complement and other qualitative implications. Statistically significant differences were found between patients in the two settings on predisposing, enabling and need characteristics. The hospital-based NHCU patients were more likely to be married and living with someone. They also had higher incomes, more Medicare A coverage, a greater percentage of service-connected veterans and demonstrated greater limitation in functional ability than did those in contract. The predominant outcome for both groups was continued nursing home care. Statistically significant differences were also found for outcome measures. Higher income and being 76 years or older were predictive of continued nursing home residence. The type of nursing home was not significant in explaining continued care. The total number of diagnoses. age group and type of nursing home were predictive of death as an outcome. There were significantly more deaths among those 75 years or younger. among those with lower incomes and among NHCU patients. Health service utilization did not differ significantly by nursing home type. Neither group of nursing home patients demonstrated any significant improvement in functional or mental status and self-perceived health. The only differences of note were among those 75 years or less who did improve in functional ability from the third to the sixth month. The findings suggest that the two nursing home types do have different patient population profiles. However. the continued use of nursing home care by both groups indicates some lack of fit between legislative intent and actual clinical utilization.

Streszczenie:
1 BackgroundBased on the current structural shortage of labor force and serious shortage of caregivers in China's elderly and medical services [1], the future demand for elderly care services for the elderly population is also increasingly strong [2]. At present, the proportion of Chinese elderly people living in elderly care institutions is 0.73%, which reflects that there are still some obstacles to the general acceptance of elderly care institutions by Chinese elderly people. Combined with the theory of environmental psychology, it can be seen that the environment can have an impact on behavior, psychology and other aspects, so the scientific shaping of space is inextricably linked to the enhancement of the user's spatial acceptance. Based on this, this study discusses the development history, development status and future development trend of nursing home design in China, and explores the innovative development path of nursing home design in the future. It aims to improve the acceptance and willingness to use nursing homes by the elderly and their families through optimization at the design level, so as to alleviate the current aging problem in China and the world.2 MethodDesign form language, is a kind of abstract language that can cover the design concept, design strategy, is a logical combination of structure, color, material and other figurative language in design. The change of design form language is often closely related to the change of user needs, the development of the industry now, etc. Citespace can present the structure of knowledge, the situation of information in the form of graphical distribution [3], and combined with the resulting graphs to analyze the disciplinary structure of the field of study related to the discipline structure, the hot frontier and other laws [4]. In view of the fact that the keyword maps that can be directly generated by importing literature data in Citespace cannot accurately represent the design form language explored in this study, this study organizes the design form language of each piece of literature in the valid title data for the titles, keywords, and abstracts of the 253 pieces of literature in the CNKI database, respectively.3 ResultThis study combines the data processing results of the design form language to produce three results related to the development of nursing home design in China. First, the development history of nursing home design in China is divided into three stages: the "starting period", the "high speed development period" and the "innovation enlightenment period", and there is still much room for development. Secondly, from the current development status of Chinese nursing home design, the concentration of current research is high, which also proves that the innovation of Chinese nursing home design is low at this stage. In addition, the research hotspots are mainly "people-oriented" and "integration of medical and nursing care", which reflects the current Chinese nursing home design's emphasis on the physiological and psychological needs of the elderly, and conforms to the current Chinese government's guidelines for nursing homes. Thirdly, "multi-dimensional consideration of user needs", "optimization of nursing home living mode through spatial design", and "exploration of cutting-edge fields" are proposed, The four suggestions on the direction of future development of nursing home design in China are "multi-dimensional consideration of users' needs", "optimization of living mode through spatial design", "exploration of frontier fields", and "in-depth combination of theory and practice".4 ConclusionThis study applies Citespace to analyze the literature about design form language visualization mapping in the field of Chinese nursing home design in the past 15 years, and according to the changing law of design form language, we have launched a study on the development history, development status, and future development trend of China's nursing home design, and obtained four directions of the future of China's nursing home design that are worth developing and studying in depth, which provides a directional guideline for the research of China's nursing home design.References[1] Zuo Meiyun. Streszczenie:
One of the forms of care for the elderly are the nursing homes, long-term care homes. Still, in many countries the low level of quality of such services is still the main criterion for the perception of objects as a final option, in the absence of alternative forms of care for an older person. The aim of the article is to seek answers to the questions about the expected quality of the services offered by nursing homes. The article presents the results of research on the expectations of the society in terms of quality of services, carried out on a sample of 602 Polish citizens. The study allowed to identify the key characteristics that determine the quality of services from the perspective of the future decisions related to the choice of the resort.

Streszczenie:
Marginal utility of financial resources when needing long-term care, and the related incentives for precautionary savings and insurance, may vary significantly by whether one receives care at home or in a nursing home. In this paper, we develop strategic survey questions to estimate those differences. All else equal, we find that the marginal utility is significantly higher when receiving care at home rather than in a nursing home. We then use these estimates within a quantitative life cycle model to evaluate the impact of the expected choice of care setting (home versus nursing home) on precautionary savings and insurance valuation. The estimated marginal utility differences imply a significant increase in the incentives to save when expecting to receive care at home. Larger incentives to self-insure also translate to a higher valuation of additional subsidies for home care than for nursing homes, shedding light on an efficient way to expand public long-term care subsidies. We also examine how the magnitude of our results quantitatively vary with the existing public long-term care subsidies.
