Knowledge of the effect of accommodation on image quality of peripheral retina is crucial for better understanding of the visual system, but only a few studies have been carried out in this area. This study was designed to evaluate the effect of accommodation on higher order aberrations from third to sixth Zernike polynomials in central and peripheral retina up to 23° off-axis. We used a Hartmann–Shack aberrometer to measure Zernike coefficients with both accommodated and non-accommodated eyes of 15 healthy subjects. Each Zernike coefficient, total higher order aberrations, spherical aberrations and astigmatism were compared between accommodated and non-accommodated status. Additionally, aberrations in the central retina were compared with the peripheral retina. Accommodation induced significant changes in the Zernike coefficients of vertical pentafoil C5−5 and secondary vertical tetrafoil C6−4 in central retina, secondary vertical astigmatism C4−2 on 23° of temporal retina, secondary vertical tetrafoil C6−4 and tertiary vertical astigmatism C6−2 on 10° of nasal retina, secondary vertical trefoil C5−3 and secondary vertical tetrafoil C6−4 on 23° of nasal retina, and horizontal tetrafoil C44, and secondary horizontal tetrafoil C64 on 23° of inferior retina (p < 0.05). Total higher order aberration was lower in each retinal area examined with accommodation, but it was statistically significant only on 23° temporal retina and 11.5° and 23° of superior retina (p < 0.05). Spherical aberration decreased with accommodation on 23° temporal retina (p = 0.036). Astigmatism was similar in non-accommodated and accommodated eyes. Overall, accommodation affected higher order aberration (HOA) asymmetrically in different peripheral retinal areas.

AbstractThe resolution of conventional electron microscopes is usually limited by spherical aberration. Microscopes equipped with aberration correctors are then primarily limited by higher order, chromatic, and misalignment aberrations. In particular the Nion third-order aberration correctors installed on machines with a low energy spread and possessing sophisticated alignment software were limited by the uncorrected fifth-order aberrations. Here we show how the Nion fifth-order aberration corrector can be used to adjust and reduce some of the fourth- and fifth-order aberrations in a probe-corrected scanning transmission electron microscope.

Purpose. To evaluate decentration following femtosecond laser small incision lenticule extraction (SMILE) and sub-Bowman keratomileusis (SBK) and its impact on higher-order aberrations (HOAs). Methods. Prospective, nonrandom, and comparison study. There were 96 eyes of 52 patients who received SMILE and 96 eyes of 49 patients who received SBK in this study. Decentration was calculated 6 months after surgery with Pentacam. HOAs and visual acuity after the surgery were examined for patients in both groups before and 6 months after surgery. Results. The mean decentration displacement in SMILE group was significantly less than SBK group (P=0.020). 89 eyes were decentered within 0.50 mm after SMILE and SBK. The association between vertical decentration and the induced spherical aberration was insignificant in SMILE group (P=0.035). There was an association between decentration and safety index, efficacy index, vertical coma, spherical aberration, and HOAs in root mean square (RMS, μm) after SBK (all P<0.05). No difference was found in uncorrected and corrected distance visual acuity, safety index, efficacy index, and wavefront aberrations between the two subgroups at any delimited value after SMILE (all P>0.05). Decentration exceeding 0.37 mm affected vertical coma and RMSh of SBK eyes (P=0.002, 0.005). Conclusion. SMILE surgery achieved more accurate centration than SBK surgery. Vertical decentration is associated with the induced spherical aberration in SMILE.

Results of calculating the radius of higher-order spherical aberrations (fifth, seventh and ninth orders) of a high-aperture focusing holographic optical element (HOE) with corrected third-order spherical aberration in the operating spectral range are discussed. As examples, high-aperture axial HOEs with relative apertures close to 1:1 in specified spectral ranges are considered. Coordinates of the point sources of a divergent reference wave and a convergent object wave of the HOE are given. It is shown that when imaging a point source emitting in the 0.250-0.281-µm and 0.500- 0.563-µm spectral ranges, the use of an HOE in the first and second diffraction orders makes it is possible to correct the third-order spherical aberration on two wavelengths and the fifth- and seventh-order spherical aberrations on one wavelength. Note that these visible-spectrum wavelengths are different from the HOE's recording wavelength of 0.532 µm.

AIM: To compare the effect of myopia and astigmatism correction and postoperative change in higher-order aberration as results of receiving small-incision lenticule extraction (SMILE) and femtosecond laser-assisted in situ keratomileusis (FS-LASIK). METHODS: A prospective and non-randomized controlled study was conducted. The subjects are divided into two groups according to different operations received: 229 eyes of 116 patients in the SMILE group and 168 eyes of 86 patients in the FS-LASIK group. All subjects were followed up for 3mo by monitoring their uncorrected visual acuity (UCVA), best-corrected visual acuity (BCVA), spherical equivalent, higher-order aberrations, and the preoperative and postoperative complications. RESULTS: At 1wk, 1, and 3mo post-surgery, 224 eyes (97.8%), 227 eyes (99.1%) and 229 eyes (100%) had UCVA≥20/20 in the SMILE group, while 165 eyes (98.2%), 167 eyes (99.4%) and 167 eyes (99.4%) had UCVA≥20/20 in the FS-LASIK group, respectively (χ2=0.146, 2.135, and 1.124; all P&#x0026;#x003E;0.05). BCVA reduction was not observed in both groups at 1 and 3mo of post-surgery (χ2=0.734 and 1.898, P&#x0026;#x003E;0.05). There was no statistically significant difference in the spherical equivalent between the two groups at 1 and 3mo post-surgery, though the percentage of the spherical equivalent within ±0.50 D at 3mo post-surgery was 98% in the SMILE group, which was higher than that of the FS-LASIK group (92%, χ2=1.872, P&#x0026;#x003E;0.05). The root mean square (RMS) values of total high-order aberration, coma, and spherical aberration of the two groups increased significantly in the early postoperative period and decreased after 3mo, but the values were still higher than the preoperative levels (P&#x0026;#x003C;0.05); there was no significant difference between the two groups in the RMS values of total higher-order aberrations and specific higher-order aberrations (P&#x0026;#x003E;0.05). The incidence of complications in the SMILE group was lower than that in the FS-LASIK group (χ2=14.52, P&#x0026;#x003C;0.05). CONCLUSION: SMILE and FS-LASIK can effectively treat myopia, significantly improve visual acuity, and increase the total high-order aberration, spherical aberration, and coma. The incidence of complications after SMILE is relatively low.

Purpose. To report the characteristics of anterior and posterior corneal high-order aberrations in patients with different refractive errors.Setting. This study was conducted at Sohag Refractive Center, Sohag, Egypt.Design. This is a retrospective observational study.Methods. This study evaluated 750 patients (750 eyes) who were seeking refractive surgery. The eyes were stratified into five groups (150 eyes/group) based on refractive error: mild-to-moderate myopia, high myopia, hyperopia, simple myopic astigmatism, and simple hypermetropic astigmatism. All patients were subjected to comprehensive ophthalmological examination including corneal topography and corneal aberrometry using the Scheimpflug–Placido topography (Sirius, CSO, Italy).Results. Coma aberration was statistically significant when compared in all five groups (P=0.01). It was highest in the hypermetropia group (0.26 ± 0.12 μm) but lower in the moderate myopia, high myopia, myopic astigmatism, and hypermetropic astigmatism groups. Spherical aberration was lowest in the hypermetropia group and significantly different from that in the other groups. Trefoil was statistically insignificant when all groups were compared (P=0.062) but was highest in the myopic astigmatism group (0.24 ± 0.25 μm). Total RMS peaked in the hypermetropia group (0.99 ± 0.70).Conclusions. In normal corneas and regular refractive errors, the cornea-induced high-order aberration was minimal, and all types of refractive errors were associated with certain types of high-order aberrations, with a significant increase in spherical aberration in the hypermetropia group.

Aim.To evaluate the acute effects of cigarette smoking on photopic and mesopic pupil sizes and wavefront aberrations.Methods.Cigarette smoker volunteers were recruited in the study. Photopic and mesopic pupil sizes and total ocular aberrations were measured before smoking and immediately after smoking. All volunteers were asked to smoke a single cigarette containing 1.0 mg nicotine. Pupil sizes and total ocular aberrations were assessed by optical path difference scanning system (OPD-Scan II ARK-10000, NIDEK). Only the right eyes were considered for statistical analysis. The changes of pupil size and total ocular aberrations after smoking were tested for significance by Wilcoxon signed ranks test.Results.Mean photopic pupil size decreased from 3.52 ± 0.73 mm to 3.29 ± 0.58 mm(P=0.001)after smoking. Mean mesopic pupil size was also decreased from 6.42 ± 0.75 mm to 6.14 ± 0.75 mm after smoking(P=0.001). There was a decrease in all the measured components of aberrations (total wavefront aberration, higher-order aberration, total coma, total trefoil, total tetrafoil, total spherical aberration and total higher-order aberration) after smoking; however the differences were insignificant for all(P>0.05).Conclusion.Our results indicate that pupil constricts after smoking. On the other hand, smoking does not alter ocular aberrations.

Human eyes is an imperfect imaging system that has various aberrations such as on axis and off axis aberrations, which often affect the imaging quality of retina. However, the available aberration methods are mainly concerned with on-axis aberration measurement, which include defocusing, astigmation, and some higher order aberration measurements. Therefore, off-axis aberration measurement under the principle of Hartmann-Shack sensor is proposed in this paper with an empirical study, the results of which showed that the imaging quality of human eyes’ peripheral field decreased with the enlargement of off axis angle. The enlargement of human eyes’ off axis aberration and the decrease of photosensory cell density of the peripheral field are two reasons for such a result.

AbstractThe successful development of third-order aberration correctors in transmission electron microscopy has seen aberration-corrected electron microscopes evolve from specialist projects, custom built at a small number of sites to common instruments in many modern laboratories. Here we describe some initial results illustrating the two- and three-dimensional (3D) performance of an aberration-corrected scanning transmission electron microscope with a prototype improved aberration corrector designed to also minimize fifth-order aberrations and a new, higher brightness gun. We show that atomic columns separated by 0.63 Å can be resolved and demonstrate detection of single dopant atoms with 3D sensitivity.

Purpose. To investigate the changes of corneal surface shape and optical quality during orthokeratology. Methods. 49 eyes of 26 patients (10.63 ± 2.02 years old) who underwent overnight orthokeratology for myopia were prospectively examined. The corneal surface shape parameters, including surface regularity index (SRI) and surface asymmetry index (SAI), were attained with an OPD-III SCAN. The higher-order aberrations and higher-order Strehl ratios were calculated under a 3 mm pupil diameter before orthokeratology, 1 month, 3 months, and 6 months after orthokeratology. A P value of less than 0.05 was statistically significant. Results. Months after orthokeratology, SRI and SAI were both showing a significant increase in comparison with those before orthokeratology (P<0.001). After orthokeratology, for a 3 mm pupil, the higher-order Strehl ratio presented a reduction of 0.217 μm (P<0.001), and the higher-order aberration root mean square (HOA RMS) showed a mean increase of 0.100 μm (P<0.001). There were significant increases in spherical aberration (P<0.001) and coma (P=0.044) after orthokeratology. Trefoil showed a slight reduction at month 6 after orthokeratology, but there was no statistical significance (P=0.722). Conclusion. Overnight orthokeratology for a correction of myopia resulted in a significant improvement in refractive error but increased corneal irregularity and ocular higher-order aberrations, especially in spherical aberration.

Objective: To evaluate the changes of visual acuity, contrast sensitivity, astigmatism, and higher order aberrations after blepharoplasty in patients with dermatochalasis. Methods: Two hundred six eyelids of 103 patients with dermatochalasis were prospectively studied. Visual acuity, contrast sensitivity, corneal topography, astigmatism degree, and higher order aberrations were examined and recorded before and at 1 month after upper eyelid blepharoplasty. Change in contrast sensitivity and astigmatism values were determined according to margin reflex distance of patients, which were classified into three following groups: <2 mm, 2–3 mm, and ⩾ 4 mm. A Wilcoxon signed rank test was performed to compare the difference. Results: No significant differences were observed for visual acuity (p = 0.157). The contrast sensitivity of patients significantly increased at all spatial frequencies both under glare and nonglare conditions (p < 0.05). The mean refractive astigmatism significantly decreased from ‒1.01 ± 1.3 to ‒0.79 ± 0.71, postoperatively (p = 0.029). In patients with marginal reflex distance < 2 mm, mean contrast sensitivity was increased (p < 0.001) and mean astigmatism was decreased significantly (p < 0.001) compared with those with ⩾ 2 mm. Higher order aberrations decreased significantly from 0.62 ± 0.41 µm to 0.55 ± 0.38 µm (p = 0.038) after blepharoplasty. Root mean square of the vertical trefoil (p = 0.038), vertical coma (p = 0.002), horizontal trefoil (p = 0.027), third-order aberration (p = 0.005), secondary vertical stigmatism (p = 0.001), spherical aberration (p = 0.023), secondary horizontal astigmatism (p = 0.002), fourth-order aberration (p = 0.024), vertical pentafoil (p = 0.015), secondary horizontal coma (p = 0.035), secondary horizontal trefoil (p = 0.030), and horizontal pentafoil (p = 0.048) were decreased significantly. Conclusion: Upper eyelid blepharoplasty in patients with dermatochalasis has a significant improvement in visual function in terms of contrast sensitivity, astigmatism, and higher order aberrations.

In recent years, the direct-liquid-cooled thin-disk lasers have become ahot research topic due to their advantages of extremely small volume to output power ratio and strong thermal management ability. A method of analyzing the wavefront aberration of the direct-liquid-cooled thin-disk laser is established in this paper. The influence of pumping light uniformity on the wavefront aberration is investigated by this method. The high order aberration distribution of the laser beam is calculated, when the uniformity is 92%, 80% and 70%, respectively. With the decrease of beam uniformity, the higher order aberrationsincrease gradually, while the low order aberrations remain basically unchanged. Experimentally, the pumping beam distributions respectively with uniformity of 92% and 70% are designed with and without using the waveguide structure. The wavefront jitter and wavefront aberration distribution of the whole gain module are measured in these two cases. The pump power is kept at 5 kW. The higher order of optical path difference (OPD<sub>H</sub>) values of the whole gain module are measured to be 0.66 μm and 0.79 μm, respectively. The experimental results are in agreement with the theoretical analyses.

Objective To study the one-year effect of wearing orthokeratology (OK) lenses on the visual quality of juvenile myopia. Methods The right eyes of 36 juvenile myopias were retrospectively studied in this work. Q-value, e-value, corneal curvature, strehl ratio (SR), modulation transfer function (MTF) and wavefront aberration (WA) were compared before and at 1, 3 and 12 months after wearing OK lenses. The SR, MTF and WA of cornea, internal optic and ocular were analyzed separately. The spherical and cylinder diopter, vision acuity, compensating factor (CF) and compensative rate (CF%) were compared before and at 12 months after wearing OK lenses. Results (1) The vision of LogMAR increased and the corneal curvature decreased significantly after wearing OK lenses. There was no significant difference for the e-value before and after wearing OK lenses. The Q-value increased at 1 month but decreased at 3 and 12 months remarkably. (2) The ocular and internal optic SR and MTF increased significantly at 1 month and then remained stable. The MTF in different spacial frequencies increased after wearing OK lenses. There was no significant difference for the corneal SR before and after wearing OK lenses, and the corneal MTF decreased significantly after wearing OK lenses. (3) For the ocular, the total higher order aberration (HOA), spherical, coma and trefoil aberrations increased, and the total aberration, total lower order aberration (LOA) and defocus aberration decreased obviously except astigmatism. The corneal aberrations increased significantly after wearing OK lenses except astigmatism. For the internal optic, the total aberration, total LOA and defocus aberration decreased, and the total HOA, coma and trefoil aberration increased significantly except the astigmatism and spherical aberrations. (4) The CF and CF% of total aberration, total LOA, total HOA and coma aberrations increased, and those of astigmatism and spherical decreased at 12 months. Conclusions Orthokeratology is effective in correcting the refractive error and improving the vision quality of juvenile myopia over the one-year follow-up period.

Cataract surgery impinges on the spatial properties and wavelength distribution of retinal images, which changes the degree of light-induced visual discomfort/photophobia. However, no study has analyzed the alteration in photophobia before and after cataract surgery or the association between retinal spatial property and photophobia. Here, we measured the higher-order aberrations (HOAs) of the entire eye and the subjective photophobia score. This study investigated 71 eyes in 71 patients who received conventional cataract surgery. Scaling of photophobia was based on the following grading system: when the patient is outdoor on a sunny day, score of 0 and 10 points were assigned to the absence of photophobia and the presence of severe photophobia prevents eye-opening, respectively. We decomposed wavefront errors using Zernike polynomials for a 3-mm pupil diameter and analyzed the association between photophobia scores and HOAs with Spearman’s rank sum correlation (rs). We classified patients into two groups: photophobia (PP) unconcerned included patients who selected 0 both preoperatively or postoperatively and PP concerned included the remaining patients. After cataract surgery, photophobia scores increased, remained unchanged (stable), and decreased in 3, 41, and 27 cases, respectively. In the stable group, 35 of 41 cases belonged to PP unconcerned. In PP concerned, there were significant correlations between photophobia score and postoperative root-mean-square values of total HOAs (rs = 0.52, p = 0.002), total coma (rs = 0.52, p = 0.002), total trefoil (rs = 0.47, p = 0.006), and third-order group (rs = 0.53, p = 0.002). In contrast, there was no significant correlation between photophobia scores and preoperative HOAs. Our results suggest that the spatial properties of retinal image modified by HOAs may affect the degree of photophobia. Scattering light due to cataracts could contribute to photophobia more than HOAs, which may mask the effect of HOAs for photophobia preoperatively.

Purpose: The aim of this study is to evaluate the root mean square error (RMS) of corneal higher- order aberrations in Syrian adult population in different refractive status (emmetropia, myopia, hyperopia and simple myopic astigmatism). Methods: This cross- sectional study was conducted at Tishreen University Hospital in Latakia, Syrian Arab Republic. A total of 420 patients (840 eyes), ages 18- 40 years, participated in the study. Refraction was measured with an autorefractor keratometer and expressed as a spherical equivalent. Total corneal higher- order aberrations (Coma, Spherical Aberration, Trefoil) were measured using Sirius Corneal Topography at a pupil diameter of 5 mm. Results: RMS of Coma and Spherical aberrations peaked in the hyperopia group (0.18 ± 0.08, 0.11 ± 0.02) respectively. Whereas, Trefoil was higher in the severe myopia and simple myopic astigmatism groups (0.25 ± 0.05, 0.25 ± 0.08) respectively. Conclusions: Among a Syrian population, some types of refractive status were associated with certain types of higher- order aberrations, with a significant increase in trefoil in severe myopia and simple myopic astigmatism.

Background: Laser-Assisted in situ Keratomileusis (LASIK) can induce corneal aberrations that can impact vision and patient satisfaction. Recent developments in laser technologies have helped minimise these aberrations. Objective: To assess the quality of vision and change in Higher-Order Aberrations (HOAs) following wavefront-optimized LASIK in low-myopic astigmatic patients. Methods: LASIK was performed on a total of 76 eyes in patients with myopia <4.0 D and cylinder <2.0 D using the WaveLight® EX500 excimer and FS200 femtosecond laser platform. Visual acuity, contrast sensitivity and HOAs were measured at 1 and 3 months postoperatively and compared to preoperative values. Subjective quality of vision was assessed pre- and postoperatively using a VF14 questionnaire. Results: Mean postoperative Spherical Equivalent (SE) was -0.09 ± 0.26 µm with 95% of patients within ± 0.5 D of attempted SE. Postoperative uncorrected distance visual acuity was 20/20 or better for 96% of patients. Contrast sensitivity increased against horizontal and vertical gratings at all spatial frequencies except for vertical gratings at 18 cycles/degree. Spherical aberration and total HOA increased by 0.085 µm and 0.13 µm respectively. The mean VF14 score increased from 89.2 ± 16.7% to 99 ± 1.4% postoperatively. Conclusion: LASIK performed using the WaveLight® EX500 excimer and WaveLight® FS200 laser platform provided improved contrast sensitivity and visual acuity with minimal introduction of HOAs, making it a suitable platform for low myopic astigmatic patients.

Background This retrospective, cross-sectional study investigated changes in corneal lower- and higher-order aberrations that cause visual disturbance after lateral rectus recession and medial rectus resection in children. Methods Eighty-five eyes of 85 patients (44 boys; 8.64±2.88 years) who underwent lateral rectus recession and medial rectus resection to correct intermittent exotropia were assessed. The Galilei G4 Dual Scheimpflug Analyzer was used for wavefront analysis. Risk factors (age, sex, amount of surgery, preoperative axial length, preoperative intraocular pressure) were determined. Outcome measures included simulated and ray-tracing mode keratometry with secondary defocus, oblique, and vertical astigmatism (for lower-order aberrations) and the root mean square, 3rd-order vertical and horizontal coma, oblique and horizontal trefoil, 4th-order spherical aberration, oblique and vertical secondary astigmatism, and oblique and vertical quadrafoil (2nd‒8th sums) (for higher-order aberrations). Results Myopic with-the-rule changes in low-order aberrations and increases in simulated and ray-tracing mode keratometry during the 3 months following lateral rectus recession and medial rectus resection were attributed to muscle healing and stability changes. High-order aberrations altered in the week following surgery almost returned to normal within 3 months. Axial length, the amount of surgery, age, and sex affected astigmatism due to differences in patients’ scleral states. Conclusions Clinicians should consider changes in high-order aberrations of young individuals who underwent lateral rectus recession and medial rectus resection and may not be able to verbalize changes in vision.

Adaptive optics (AO) is employed for the continuous measurement and correction of ocular aberrations. Human eye refractive errors (lower-order aberrations such as myopia and astigmatism) are corrected with contact lenses and excimer laser surgery. Under twilight vision conditions, when the pupil of the human eye dilates to 5–7 mm in diameter, higher-order aberrations affect the visual acuity. The combined use of wavefront (WF) technology and AO systems allows the pre-operative evaluation of refractive surgical procedures to compensate for the higher-order optical aberrations of the human eye, guiding the surgeon in choosing the procedure parameters. Here, we report a brief history of AO, starting from the description of the Shack–Hartmann method, which allowed the first in vivo measurement of the eye’s wave aberration, the wavefront sensing technologies (WSTs), and their principles. Then, the limitations of the ocular wavefront ascribed to the IOL polymeric materials and design, as well as future perspectives on improving patient vision quality and meeting clinical requests, are described.

Prospective, randomized, comparative, and controlled study to estimate the association between angle κ distance and higher-order aberrations (HOAs) with postoperative visual acuity after presbyopia-correcting IOL implantation. Forty-three eyes from 43 patients were included and randomly assigned in two groups for either AT LISA tri 839MP or Acrysof IQ PanOptix IOL implantation. The OPD-Scan III analyzer was utilized to assess the angle κ distance and higher-order aberration (HOAs). Twenty-three eyes were in the Acrysof IQ PanOptix group and 20 patients in the AT LISA tri 839MP group. The uncorrected distance visual acuity (UDVA) for the PanOptix group was 0.092 ± 0.10, whereas for AT LISA tri was 0.050 ± 0.06 (P=0.229). The uncorrected intermediate visual acuity (UIVA) for the PanOptix group was 0.173 ± 0.18, whereas for AT LISA tri, it was 0.182 ± 0.11 (P=0.669). Uncorrected near visual acuity (UNVA) was 0.068 ± 0.04 and 0.085 ± 0.07, respectively (P=0.221). Also, correlation coefficient between HOAs and the Strehl ratio for each group were −0.768 (P<0.0001) and −0.863 (P=0.0001). Patients implanted with both trifocal IOLs showed excellent postoperative visual performance at all distances at the six-month follow-up visit. No association was found between angle κ distance and postoperative visual acuity regardless of the angle κ magnitude or the two trifocal IOLs inner optical diameter. Also, internal aberrations demonstrated a significant inverse correlation with the Strehl ratio for both trifocal IOLs.
