Objective To investigate the manifestations and incidence of headaches caused by heroin in Chinese women. Methods This was a survey study conducted from 29 June to 3 July 2015 with women attending the Shanxi Drug Rehabilitation Centre for Women (China). All study subjects were newly admitted and had not begun their drug rehabilitation. Demographic characteristics, heroin usage and headache episodes within the previous 3 months were surveyed, especially the presence of a headache within 2 hours of heroin use. Details of the severity, location, premonitory symptoms and characteristics of headaches were recorded. Results Of the 90 heroin-dependent patients, 74 experienced headache attacks within 2 hours of heroin use, and the headaches subsided within 72 hours of discontinuation of heroin use. Most heroin-induced headaches were similar to migraines and manifested as pulsating pain in 54 patients (51/74, 68.9%); bilateral pain was reported by 46 patients (46/74, 62.2%). Approximately half of the patients with heroin-induced headaches also reported accompanying symptoms of nausea, vomiting, and light and sound sensitivity. Conclusions Heroin-induced headache may eventually be listed as a new class of headache in the International Classification of Headache Disorders.

We present selected findings from “An Ethnography of Young Heroin Users” concerning media and youth-subculture influences on the initiation and continuation of heroin use among young adults ages 18–25. One hundred and two male and female participants were administered depth interviews and structured questionnaires pertaining to heroin initiation and continuation practices. A number of participants mentioned media depictions of heroin and membership in specific youth cultures in relation to their own heroin use. This complex relationship between heroin use, media, and subcultures is discussed. A common theme emerged from the depth interviews of fatalistic life outlooks, which were often linked with negative childhood experiences as well as with heroin use. Some of these young addicts romanticized heroin use and the tragedy of overdose. These findings are discussed with reference to further research and possible interventions.

Objective: To determine the frequency and level of family support in Heroin Dependent patients presenting with 1st treatment and also the patients presenting with relapses. Study design: A descriptive study. Place & duration of study: The study was conducted in the Department of Psychiatry & Behavioral Sciences, Nishtar Hospital, Multan from September, 2013 to December, 2013. Subjects & methods: The sample consisted of 50 consecutive inpatients presenting with Heroin Dependence. They were interviewed and Family support Scale (FSS) was administered to know the level of support in heroin dependent patients. Results: Results showed that 20% patients relapsed 3 times while 38% had more than three times relapses among whome 30% patients have family support, but 70% have very low level and family support decreased with increasing number of replace. Conclusions: We concluded that higher level of family support was during in the 1st treatment of Heroin Dependence and family support decreased as the number of relapses increased.

Heroin has been prescribed in England for the treatment of heroin addiction for almost 100 years. For many years, England was almost the only country where this occurred, and the British system was consequently the subject of international curiosity. In spite of this long history, very little research has been carried out locally, and until recently, there were no guidelines as to best practice. In 1965, it was decided that only doctors with a Home Office license could prescribe heroin. Since that time, the proportion of opiate addicts treated with heroin has greatly diminished, as has the number of doctors willing to prescribe. One doctor in particular remained an enthusiastic proponent of heroin prescription. His claims impressed some Swiss clinicians, who proceeded to establish a multicentre trial in Switzerland. This was followed by a similar trial in the Netherlands. These trials apparently indicated that a proportion of treatment-resistant opiate addicts could respond well to heroin, although the researchers' conclusions have been disputed. The National Treatment Agency in England is now developing guidelines for good practice based on this new information and is planning to set up some pilot sites. It is likely that practise in England will remain somewhat different from continental practice, particularly with respect to long-term supervised injecting. It is unclear how much funding will be released to support heroin prescription. Without adequate funding, it is likely to disappear.

In answering these questions through an examination of the complex process of problem making, the article suggests it may help us to better understand how issues which policy-makers identify as ‘problems’ come to achieve such a status, and how solutions that come to be regarded as ‘realistic’ — or not — reach this point.
