The clinical examination is an important part of any patient consultation. After the primary survey and taking the patient history, a more in-depth examination is sometimes required to aid making a working diagnosis and help negate other differential diagnoses. The extent of this depends on the stability of the patient and may not be possible in time-critical circumstances. However, clinical examination is an increasing part of paramedic practice owing to the continued expansion of the scope of the paramedic role in both urgent and emergency care. Education on clinical examination concerning each of the main body systems is now an integral part of undergraduate paramedic curricula. This clinical examination series provides a step-by-step overview for each of the main body systems. Continuing professional development (CPD) is an essential requirement for all clinicians to maintain and to demonstrate that they are staying up to date and advancing in their roles. This series gives an overview of each type of examination to support students, newly qualified paramedics and paramedics wishing to use these articles as a CPD development activity and an aide-memoire for clinical practice. This article, which explores the the gastrointestinal system, gives an overview of initial examination considerations.

A pneumonia outbreak of unknown etiology and pathology spread to the whole world in Wuhan, China in December 2019 and this outbreak is called as COVID-19. COVID-19 is an infectious disease caused by the SARS-CoV-2 virus and it can cause various clinical pictures such as respiratory, enteric, hepatic, nephrotic, and neurological involvement in humans and animals. This outbreak has caused the death of millions of people. Vaccination studies have continued today, and vaccination of all humanity may take a long time. The best prophylactic approach to reduce the severity of such viral diseases is to enhance human host immunity. We will summarize effects of COVID-19 infection on the gastrointestinal system and we will remark the importance of probiotics in this manuscript.

The gastrointestinal tract is the primary localization site of lymphoproliferative lesions ranging from reactive lymphoid hyperplasia to lymphoma. Diagnosis of these diseases is difficult, especially in small endoscopic biopsies. And integrated approach based on clinical, morphological, immunohistochemical and molecular data is needed for accurate diagnosis. In this review, the basic histological features of non-Hodgkin lymphomas in the gastrointestinal tract and the entities included in the differential diagnosis are discussed.

Physical exercise can have both acute and enduring effects on the gastrointestinal system. Physiological changes occur during exercise and as a result of repetitive physical activity. As a lifestyle measure, routine exercise has been associated with a reduced risk of developing certain gastrointestinal diseases. Exercise can also be beneficial for patients with gastrointestinal disease. Unpleasant gastrointestinal symptoms during vigorous exercise may limit athletes in competition and conversely, competitive sport can have adverse effects on the gastrointestinal tract. This article aims to explore the relationship between exercise and the gastrointestinal system and to prepare clinicians for discussion of pertinent aspects of this relationship with patients.

Lipomas are rare benign tumors in the gastrointestinal system. Within the gastrointestinal system, 65% of the lipomas are located in the colon (sigmoid part of the colon or rectum) and rarely in the stomach and esophagus. The paper presents two gastrointestinal lipomas. First is the case of lipoma of the sigmoid colon and the other one is gastric lipoma. In both cases the material was sent for histopathological analysis due to suspicion of malignancy of the lesions. In both cases, the histopathologic analysis showed tumor made of mature adipocytes, localized in the submucosa both of the stomach and intestine. Hypercellularity and/ or atypia of the cell was found in neither case. Lipomas are shown because of its atypical localization and clinically suspicious malignancy in the stomach and sigmoid colon. These cases show that the applied methods of preoperative diagnosis of tumors in the gastrointestinal system are not sufficient to determine the origin and biological behavior of tumors. Histopathological diagnosis provides a correct insight into the nature of tumors and determine the course of treatment. This paper presents a rare localization of lipomas in the gastrointestinal system. The preoperative diagnosis of lesions in the gastrointestinal system may not be sufficient to determine the origin and biological behavior of the lesions, hence the histopathological diagnosis gives an accurate insight into the nature of the change, preventing the possibility of further aggressive therapy.
