Pełny tekst źródłaIn this thesis, we propose different algorithms to solve a timetabling problem proposed by the SimUSanté center. The center offers a large number of training sessions, based on learning by simulation, and intended for health actors, whether they are professionals or students. The SimUSanté problem is close to the academic problem of Curriculum-Based Courses Timetabling (CB-CTT). The data and constraints of our problem are presented in detail using a 0-1 linear program.To solve this problem we propose different methods: a dedicated greedy algorithm SimUG is used as an initial and relevant solution for the SimUVNS algorithm, a variable neighborhood search (VNS) algorithm. SimUVNS combines different neighborhood stemmed from the saturator, intra, extra and extra + operators. A diversification operator is applied when research is trapped in a local optimum.Next, several Ant Colony Optimization type algorithms were proposed: SimUACO, SimUMACO-VNS, and SimUTACO. They are respectively derived from Ant System, MinMax Ant System and Ant Colony System algorithms. SimUACO is inspired by the ant behavior to find food and return back to the nest. Each ant leaves pheromones on the trail from their nest to the food source. An ant moves randomly but when it detects pheromones, it follows the trail and reinforces it by leaving additional pheromones. The more ants follow a trail, the more attractive that trail becomes. Pheromones evaporate over time and therefore the least used or slower paths become the least attractive. Ants can then find the fastest trail from the nest to a source of food. We represent the SimUSanté problem by a graph in which each possible path is associated with a quantity of pheromones and corresponds to a triplet: an activity, a time slot, and a set of resources. Any ant can decide to follow a path. This decision is based on heuristic information and on the quantity of pheromones associated with each path. If an ant follows a given path, an additional quantity of pheromones, which is proportional to the quality of the solution built, is deposed on it. SimUMACO-VNS, and SimUTACO are based on SimUACO, but different from this one by the managment of pheromones. In SimUMACO-VNS the values of pheromones is blocked within a minimum and maximum value so that no path is preponderant. Moreover, it tries to improve the solutions built by the ants using SimUVNS. SimUTACO uses a taboo path system in order to push the ants to find new solutionsSimUVNS, SimUACO, SimUMACO-VNS, and SimUTACO were tested on hundred instances derivated from those of the CB-CTT, which were adapted to the SimUSanté problem. Experiments show that the strength of SimUVNS is to plan all activities, even for the largest instances, without violating any hard constraints. SimUACO slightly improves the results of SimUVNS, while SimUMACO-VNS, and SimUTACO always outperform our other methods, and can get optimal solutions. Nevertheless, we note that only SimUTACO is able to do it regularly and for any instance. The solutions given by SimUVNS, SimUACO, SimUMACO-VNS, and SimUTACO respectively have an average difference with the optimal solutions of 5.55%, 4.80%, 2.59% and 0.15%

Constantly amended, training programmes are often complex and sometimes even reveal to be ineffective. Financing vocational training also raises questions due to the many people, both public and private, who are involved in this area. The overall system can and must be substantially improved so as to enable the expansion of vocational training wherever it deserves to be encouraged

Pełny tekst źródłaThe author introduces the behavior of firms in order to analyse the relationship between employer provided training and wages. The empirical studies are based on French data provided from the French national institute of statistics and economics which are available since the enactment of the legislation, in 1971. In a first part, the practices of training are analysed according to the characteristics of firms. We show there exist wide disparities across sectors in access to training over the last 25 years. After a first empirical estimation, three groups of sectors are distinguished according to the intensity of the pratices of training. The second part constitutes a theorical reflexion about the relationship between the practices of training accross groups of sectors and their wages polices. The results given by a second empirical estimation show that the sectors which pay the highest wages are therefore also those which offer the most opportunities for training. In the thirst part, we estimate the impact of employer provided training on wages. The models used take account the existence of selection bias. The first one is the result of the workers' repartition in the different sectors. The second comes from the selection among workers in training programs. The results show that training effect differs according to the kind of sectors. Employer provided training has no direct impact on wages in sectors with external labor market. However, the impact is positive in sectors with internal labor market

The research undertook to explore the role of professional experiences, dailylife, exchanges between peers, and all other forms of so-called « informal » learning inenhancing the professional skills of medical students and physicians. The author laboredunder the following hypothesis: all learning involves both formal and informal aspects,and the informal aspects can be found in cooperative, experiential and reflexive typepedagogical constructs. Finally, « informal » and « non-formal » learning opportunitiescertainly play a role in the acquisition of knowledge and skills among health care professionals.Three teaching mechanisms frequently used during the 3rd cycle of initial medical trainingas well as in medical continuing education programs were made available to medicalstudents and physicians working in ambulatory setting. These tools, which encourageparticipants to marshal their own experiences, to reflect thoughtfully and to cooperatewith each other, can improve the skills of medical students and physicians.Each of the three teaching mechanisms studied herein, factoring in individual backgroundsand proficiencies, contain both formal and informal knowledge and skills which,as a whole, contributed to the transformation of the participants. However, it is importantto note that the distinction between formal and informal learning is primarily theoretical,and may simply depend on context or location. Therefore, rather than opposethem, it may be more appropriate to consider that these types of learning lie on a continuum

Pour tester ces deux hypothèses, nous avons conduit deux enquêtes qualitatives auprès de MG (Grand Lyon et zones rurales de la région Rhône-Alpes).
Finalement, l'informatique est utilisée pour la gestion administrative du cabinet mais elle n'a pas fondamentalement modifié les pratiques documentaires des MG. L'activité secondaire des MG constitue souvent le moteur de la démarche de recherche d'information. Les Technologies de l'Information et de la Communication s'avèrent également peu intégrées dans les pratiques de Formation Médicale Continue.

Pełny tekst źródłaContinuing vocational training in French social law : contributing to the study of a new social insurance.For its purpose and its content, continuing vocational training belongs to the law of social protection. It allows employees to face the social risk of skills' obsolescence but also to secure their professional path. In 1973, the legislator created an artificial link between vocational training and labour law by codifying this field into the Labour Code. Labour Law exercised a constant influence on vocational training which was primarily based on the sole needs of employees. The reform of 5th March 2014 and the subsequent laws seem to have the will to break with this artificial legal framework. The legislator intends to create a system of social insurance which is universal, personal and individual. It raises the question of a potential right to vocational training and calls for a global reform of its functioning

In the field of health, the simulation stands out as one of the most successful tools contributing to the improvement of the practices.Our work concerns more particularly the very specialized area represented by cardiac surgery. We propose an educational tool based on the simulation from an in-vivo animal model.After a first stage to design the model and its relevance, two curriculum of training, one concerning basics and the other concerning continuing medical education, are proposed and assessed.To improve the quality and the risk management in France, a comparison with the initiatives already established abroad, in particular in Canada, is depicted.Finally, the School of the CEC comes to illustrate what could be, in France, an example of training curriculum for medical and paramedical professions in the field of the cardiac surgery
