Patients with diabetes require regular examination for eye disease, usually in primary care settings. Guidelines recommend patients with at least moderate non-proliferative diabetic retinopathy (NPDR) be referred to an ophthalmologist for treatment; however, poorly targeted referrals lead to access blocks. The quality of new referrals associated with diabetes to a public ophthalmology service in Sydney, New South Wales, Australia, were assessed for referral completeness and targeting. A cross-sectional audit of medical records for new patients referred to Westmead Hospital Eye Clinic in 2016 was completed. Completeness of medical and ophthalmic information in referrals and subsequent patient diagnosis and management in 2016–17 was recorded. Sub-analyses were conducted by primary care referrer type (GP or optometrist). In total, 151 new retinopathy referrals were received; 12% were sent directly to a treatment clinic. Information was incomplete for diabetes status (>60%), medical (>50%) and ophthalmic indicators (>70%), including visual acuity (>60%). GP referrals better recorded medical, and optometrists (37%) ophthalmic information, but information was still largely incomplete. Imaging was rarely included (retinal photos <1%; optical coherence tomography <3%). Median appointment wait-time was 124 days; 21% of patients received treatment (laser or anti-vascular endothelial growth factor) at this or the following encounter. Targeting referrals for ocular complication of diabetes to public hospitals needs improvement. Education, feedback and collaborative care mechanisms should be considered to improve screening and referral in primary care.

Ozone and fine particles (PM2.5) are the two main air pollutants of concern in the New South Wales Greater Metropolitan Region (NSW GMR) due to their contribution to poor air quality days in the region. This paper focuses on source contributions to ambient ozone concentrations for different parts of the NSW GMR, based on source emissions across the greater Sydney region. The observation-based Integrated Empirical Rate model (IER) was applied to delineate the different regions within the GMR based on the photochemical smog profile of each region. Ozone source contribution was then modelled using the CCAM-CTM (Cubic Conformal Atmospheric model-Chemical Transport model) modelling system and the latest air emission inventory for the greater Sydney region. Source contributions to ozone varied between regions, and also varied depending on the air quality metric applied (e.g., average or maximum ozone). Biogenic volatile organic compound (VOC) emissions were found to contribute significantly to median and maximum ozone concentration in North West Sydney during summer. After commercial and domestic sources, power generation was found to be the next largest anthropogenic source of maximum ozone concentrations in North West Sydney. However, in South West Sydney, beside commercial and domestic sources, on-road vehicles were predicted to be the most significant contributor to maximum ozone levels, followed by biogenic sources and power stations. The results provide information that policy makers can use to devise various options to control ozone levels in different parts of the NSW Greater Metropolitan Region.

An evaluation of the effect of sex re-assignment surgery on a group of patients attending a private clinic in Sydney, Australia. Fifty-seven patients who underwent full male-to-female sex re-assignment surgery between 1987 and 2000 completed a satisfaction survey. Several factors that might influence the extent of satisfaction with surgical outcome were explored, including age, work status, social life, and the appearance and function of the new genitalia. Patients reported significantly improved social and personal satisfaction following surgery, compared with five years previously. The study challenges outcomes from previously reported studies with regard to the age of patients at the time of surgery, and the finding that from the patient's perspective, there is no fundamental association between a successful surgical outcome and a satisfactory post-operative life experience.

As in many growing urban areas, the prevention of environmental damage as Sydney spreads westward into the Hawkesbury-Nepean River basin is a major challenge for planners, managers and the local community, We surveyed macroinvertebrates at 45 river and stream sites in April-June 1996, and reviewed data from other sources, in order to assess the issues involved in conservation of the lotic macro invertebrate fauna, Regional richness is high with 443 recorded species and morphospecies, Cluster analysis showed community pattems related mainly to waterway size (separating the Hawkesbury-Nepean River from tributary streams), geology (tributaries on shale or sandstone), tidal intrusion and urbanization (impoverished faunas in urban streams). The ability of genus richness of mites and major insect orders to reflect overall genus richness at a site was limited, and Diptera and Trichoptera appear to have the greatest value as biodiversity predictors. Urban expansion is the major threat to lotic macro invertebrate communities in the region, but agriculture, flow regulation, sand and gravel ex1raction and introduced species have probably also impacted on the fauna, Streams with high conservation value for macroinvertebrates include those few on the Cumberland Plain and surrounding slopes that retain substantial indigenous vegetation in relatively undeveloped catchments. The Hawkesbury-Nepean River sfill harbours a rich faunal community and is also important for conservation, The development of effective strategies for conservation assessment and management is problematic for several reasons, Some species in the region are known to be vulnerable, but the status of most cannot be assessed because of a lack of taxonomic and zoogeographic information, Most "biodiversity indicator" concepts are of dubious value for aquatic macroinvertebrate conservation. A multi-faceted management approach emphasizing subcatchment reserves, riparian restoration and the control of threatening processes is required.

This paper describes a service-based initiative to enhance capacity for evidence-based practice (EBP) in the South Western Sydney Area Health Service Division of Population Health. A working group planned an organisational response to a customised EBP needs assessment using the New South Wales Department of Health?s framework for capacity building focussing on five key action areas; organisational development, workforce development, resource allocation, leadership and partnerships. Innovative strategies to promote EBP were developed and implemented and on-site training programs that targeted specific groups of staff were conducted. Because there was commitment and leadership from senior staff for the initiative, a comprehensive approach to building capacity for EBP in population health was possible. Evidence of impact needs to be collected in the future.

Purpose: This study identifies the attitudes of participants in the root cause analysis (RCA) process and barriers to it?s implementation within one New South Wales area health service. Method: Employees and consumer representatives of the former South Western Sydney Area Health Service who participated in an RCA as either a team member or a team leader between December 2002 and October 2003 completed a self-administered survey. Results: Thirty seven of 39 eligible participants completed the survey (response rate 95%). The respondents identified formulation of causal statements, ensuring the causal statements met the ?rules of causality? outlined by New South Wales Health, and arranging times for interviews as most difficult. Team leader respondents (n = 7) ranked keeping the team focused, organising the first meeting within 7 days of the incident, and completing the RCA in three 2-hour meetings as barriers to the process. Conclusions: Training was valued by participants, however greater emphasis on the development of causal statements could be beneficial. Team leaders expressed difficulty in keeping the team focused and meeting the stipulated RCA timeframes, suggesting that additional support for RCA participants may be warranted.

A comprehensive evaluation of the performance of the coupled Conformal Cubic Atmospheric Model (CCAM) and Chemical Transport Model (CTM) (CCAM-CTM) for the New South Wales Greater Metropolitan Region (NSW GMR) was conducted based on modelling results for two periods coinciding with measurement campaigns undertaken during the Sydney Particle Study (SPS), namely the summer in 2011 (SPS1) and the autumn in 2012 (SPS2). The model performance was evaluated for fine particulate matter (PM2.5), ozone (O3) and nitrogen dioxide (NO2) against air quality data from the NSW Government’s air quality monitoring network, and PM2.5 components were compared with speciated PM measurements from the Sydney Particle Study’s Westmead sampling site. The model tends to overpredict PM2.5 with normalised mean bias (NMB) less than 20%, however, moderate underpredictions of the daily peak are found on high PM2.5 days. The PM2.5 predictions at all sites comply with performance criteria for mean fractional bias (MFB) of ±60%, but only PM2.5 predictions at Earlwood further comply with the performance goal for MFB of ±30% during both periods. The model generally captures the diurnal variations in ozone with a slight underestimation. The model also tends to underpredict daily maximum hourly ozone. Ozone predictions across regions in SPS1, as well as in Sydney East, Sydney Northwest and Illawarra regions in SPS2 comply with the benchmark of MFB of ±15%, however, none of the regions comply with the benchmark for mean fractional error (MFE) of 35%. The model reproduces the diurnal variations and magnitudes of NO2 well, with a slightly underestimating tendency across the regions. The MFE and normalised mean error (NME) for NO2 predictions fall well within the ranges inferred from other studies. Model results are within a factor of two of measured averages for sulphate, nitrate, sodium and organic matter, with elemental carbon, chloride, magnesium and ammonium being underpredicted. The overall performance of CCAM-CTM modelling system for the NSW GMR is comparable to similar model predictions by other regional airshed models documented in the literature. The performance of the modelling system is found to be variable according to benchmark criteria and depend on the location of the sites, as well as the time of the year. The benchmarking of CCAM-CTM modelling system supports the application of this model for air quality impact assessment and policy scenario modelling to inform air quality management in NSW.

We found that the prevalence of risks and special needs varied significantly by language groups in this study sample. Our results suggest that equity of access to quality care for this group of mothers and infants can be assured if models similar to the EOLO in SWSAHS are thoroughly planned and developed, and take account of cultural appropriateness for the population served.

Dust storms originating from Central Australia and western New South Wales frequently cause high particle concentrations at many sites across New South Wales, both inland and along the coast. This study focussed on a dust storm event in February 2019 which affected air quality across the state as detected at many ambient monitoring stations in the Department of Planning, Industry and Environment (DPIE) air quality monitoring network. The WRF-Chem (Weather Research and Forecast Model—Chemistry) model is used to study the formation, dispersion and transport of dust across the state of New South Wales (NSW, Australia). Wildfires also happened in northern NSW at the same time of the dust storm in February 2019, and their emissions are taken into account in the WRF-Chem model by using Fire Inventory from NCAR (FINN) as emission input. The model performance is evaluated and is shown to predict fairly accurate the PM2.5 and PM10 concentration as compared to observation. The predicted PM2.5 concentration over New South Wales during 5 days from 11 to 15 February 2019 is then used to estimate the impact of the February 2019 dust storm event on three health endpoints, namely mortality, respiratory and cardiac disease hospitalisation rates. The results show that even though as the daily average of PM2.5 over some parts of the state, especially in western and north western NSW near the centre of the dust storm and wild fires, are very high (over 900 µg/m3), the population exposure is low due to the sparse population. Generally, the health impact is similar in order of magnitude to that caused by biomass burning events from wildfires or from hazardous reduction burnings (HRBs) near populous centres such as in Sydney in May 2016. One notable difference is the higher respiratory disease hospitalisation for this dust event (161) compared to the fire event (24).

The use of community consultation to address socio-environmental degradation is entwined with contested democratic principles polarising views of its role. I frame this problem by examining three democratic paradigms faced with two contemporary problems. The deliberative argument that preferences require enrichment with debate mediates between the liberal-aggregative view that preferences are individual, private and amenable to aggregation and the view that participation in public life is foundational. Viewing consultation as deliberative reconciles the liberal-aggregative view of consultation as the illegitimate elevation of unrepresentative minority groups with the participationist view that consultation constitutes a step towards participatory democracy. Theorists of social reflexivity, however, point to an elided politics of knowledge challenging technoscience's exemption from politically garnered consent. Also neglected by much democratic theory is how functional differentiation renders self-referential legal, political, technoscientific and administrative domains increasingly unaccountable. I employ Habermas' procedural theory that public spheres allow social irritations into the political domain where they can be encoded into laws capable of systemic interjection in response, along with a dialogic extension accommodating the politics of knowledge. I then use this procedural-dialogic deliberative understanding of democracy to elucidate the context and outcomes of the NSW State's consultative strategy. The NSW state, institutionally compelled to underwrite economic growth, implicating itself in that growth's socio-environmental side effects provoking widespread contestation. The resulting Environmental Planning and Assessment Act (1979) and its adjunctive consultative provisions helped highlight the socio-environmental degradation of the Hawkesbury Nepean River Catchment via Western Sydney's urban sprawl, politicising the region. The convenement of a consultative forum to oversee a contaminated site audit within the region facilitated incisive lay critique of the technoscientific underpinnings of administrative underwriting of socio-environmental degradation. The discomforted NSW State tightened environmental policy, gutted the EP&A Act's consultative provisions and removed regional dialogic forums and institutions. I conclude that the socio-economic accord equating economic growth with social progress is both entrenched and besieged, destabilising the political/administrative/technoscientific regime built upon it. This withdrawal of avenues for critique risks deeper estrangement between reflexive society and the NSW State generative of electoral volatility.

Sustainable development is a multi-faceted and complex proposition, investigating such a goal required a grounded study capable of analysing real world issues. Managing such a highly diverse region as Western Sydney with its multiple demands is complicated by the plethora of government administration agencies. Contemporary land use planning policies and decisions appear frequently at odds with community values and aspirations for the region. Evidence presented in this research suggests a declining natural resource base that manifests itself in an insidious cost impost to the public sector while the benefits accrue to the private sector.Eventual developments in the resolution and maintenance of the functional integrity of the natural systems in Western Sydney may demand a major paradigm shift in economic and social policy. This research suggests that a precautionary based approach to thresholds of harm in the Western Sydney region is long overdue. Tools developed in this study appear capable of clarifying the evident land use planning paradoxes and may assist in negotiating sustainable outcomes by fostering a collaborative learning process between decision makers, experts and the community.

Abstract Historically in Australia, mixed Construction and Demolition waste, and Commercial and Industrial waste has been traditionally landfilled. As environmental impacts of landfilling is becoming more evident New South Wales policy makers and innovators have begun exploring an incineration strategy to use such waste to generate electricity. The objective of this study is to utilise life cycle assessment to evaluate GHG emissions from this waste treatment strategy and the environmental impact of a case study facility, in Sydney Australia. The system boundary includes the thermal treatment of waste through incineration, the electricity generation from the steam turbine and air pollution control processes involved within. The functional unit is based on 1 tonne of input mixed Construction and Demolition waste and Commercial and Industrial waste. GHG emissions are calculated and the result shows that the facility generates 0.994 MWh/tonne waste and 1.16 tCO2e/MWh electricity. This emission is lower than a brown coal fired powerplant emission factor 1.31 tCO2e/MWh. The results from this study assists understanding and policy making for the future of Energy-from-Waste as part of the generation mix in New South Wales, Australia.
