This paper contains an analysis of the 1971–1976 out-migration pattern of Canadian elderly females from the 23 Census Metropolitan Areas (CMAs). Migration is conceptualized as a three-level choice process. The major findings are as follows: Elderly females are substantially less migratory than young females but are slightly more mobile than elderly males. In addition, the elderly females in the more recently settled western region tend to be more mobile than those in other regions. Elderly migrants show substantially less preference for the metropolitan destinations than the young migrants; however, among the elderly, females have stronger preference for metropolitan areas than males. The destination choice pattern of metropolitan-bound out-migrants is less dispersed for the elderly than for the young. Among the older persons, in most CMAs, female migrants have a larger dispersion than male migrants. With respect to metropolitan-bound elderly migrants from the CMAs, the probability of choosing a particular destination is positively related to population size, brightness, and housing growth, and negatively related to the logarithm of distance, cultural dissimilarity, coldness, and gross rent. Environmental variables are more important than the housing variables in determining the destination choice pattern of the elderly migrants.

Abstract Clonality studies can establish the single cell origin of tumors and differentiate nonmalignant from malignant states. Detection of clonal cells may be genotype-based relying on somatic mutations to mark the clonal population (e.g. 9q+:22q– translocation in CML), or phenotype-based, where the clonal population is identified by expression of surrogate genes which facilitate tracking the clonal process. Methods for determining phenotypic clonality rely on the principle of X chromosome inactivation (XCIP), unique to women, and are based on differentiating transcriptionally active from inactive X-chromosomal genes. Detection of the polymorphic state of genes subjected to inactivation may be done by either: discrimination of DNA methylation status, detection of mRNA transcripts, or polymorphic isozyme protein products. Extreme skewing of X-chromosome allelic usage by methylation-based clonality assay has been reported in ∼30% of healthy elderly females precluding clonality studies in this population. In contrast, by X-chromosome quantitative transcriptional clonality assay (TCA), we previously reported a normal skewing range based on our determination of random X-chromosome inactivation in 8 progenitors of pluripotent hematopoietic stem cells at the time of inactivation during the blastocyst stage of development (J Exp Med, 183:748, 1996). Moreover, we have not observed clonal XCIP in TCA studies involving over 200 healthy heterozygous females, indicating the rarity of this phenomenon. However, we did not systematically study females >65 years old. Furthermore, our TCA protocol was laborious, technically demanding and required significant amounts of highly radioactive isotopes. In addition, due to susceptibility of DNA methylation to environmental factors we decided to re-investigate the issue of clonality in older females using a novel quantitative real time PCR assay based on a unique primer design, we previously reported for the JAK2V617F mutation (Exp Hematol. 2007;35(1):32–8). Females >65 years of age with no history of malignant disorders, unexplained anemia, or autoimmune disorders were recruited for our IRB approved study. Genomic DNA and total RNA was isolated from peripheral blood granulocytes, reticulocytes, and platelets where applicable. Clonality studies were performed using BTK, FHL1, IDS, G6PD, and MPP1 exonic polymorphisms (∼95% females are informative for at least one marker; Blood101:3294–301, 2003). Genomic DNA was used for genotyping exonic polymorphisms by TaqMan based allelic-discrimination assays. Thirteen elderly females (age range 65–92, mean 75.5, median 75) and 5 younger females (age range 30–40, median 36, mean 35), heterozygous for one or more markers, were identified. TCA was performed using total RNA on markers found to be informative. Neither clonal XCIP, nor extreme skewing XCIP was noted in any of the study subjects. Based on reported data, ∼30% elderly women were found to have extremely skewed XCIP; hence, we expected to find 4/13 elderly women with clonal XCIP in our study group. Statistical analysis, using an exact binomial test, indicates a low probability of false positive results by our assay (p=0.014, exact 95% CI [0,0.22]). In conclusion, hematopoiesis is not clonal in healthy elderly females. #equal contribution.

<p>This paper presents a study on mortality rates and life expectancy improvements among elderly people in Malaysia. The central age-specific mortality rates will be analyzed according to genders. The expectation of future lifetime of these old age people will be estimated using the actuarial life table approach. Two different types of life table will be developed, including life table for males and females--- to compare the results of mortality rates and life expectancies between genders. Results show that, mortality rates of Malaysian elderly, for both males and females are increasing almost in linear pattern by age, and this trend is consistent from 1950 to 2015. Comparison between genders shows that mortality of elderly females is generally lower than males at almost all ages. Nonetheless, mortality rates of Malaysian elderly males are declining faster than Malaysian elderly females. Life expentancies of females are higher than males for age groups 60 to 70, and lower than males for age 75 and above. Results also indicate that Malaysian elderly popultion is aging faster from previous generation in which elderly males age 85+ in 2010-2015 can live longer by 123% than thise in 1950-1955.</p>

Objective. To describe de prevalence of iron deficiency (ID) and anemia in a sample of Mexican elderly population from the National Health and Nutrition Survey (Ensanut) 2012. Materials and methods. 1 920 subjects ≥60 years of age were included. Hemoglobin, serum concentrations of ferritin and CRP were measured. The risk for ID and anemia adjusted for potential confounders was assessed in logistic regression models. Results. The overall prevalence of anemia was 13.9%, 15.2% in males and 12.8% females. For ID,overall it was 4.2%, males 4.0% and females 4.3%. The greatest prevalence of ID was found in males and females over 80 years old (6.9 and 7.0%, respectively). ID was present in 1.5 of 10 Mexican elders with anemia. Conclusion. The prevalence of anemia was high in the elderly, however the prevalence of ID was low; there is a need to further investigate the causes of anemia in this age group.

The U.S. divorced prevalence rates have increased steadily between the years 1963–2003 for the elderly and pre-elderly cohorts and will likely continue in coming decades. This study utilized Social Exchange principles in evaluating the comparative resource diminutions between female and male elderly divorced and between married and not married elderly. Results suggest that elderly divorced females have fewer financial resources, while having more social and emotional resources than males. Results also suggest that widowed, separated, divorced, and never married elderly have fewer resources than married ones. Separated elderly females notably had the fewest resources.

Objective. To evaluate Health-Related Quality of Life (HRQoL), medication used, and Stock of Health Capital (SHC) in physically active elderly participants in Community Exercise Programs (CEPs) compared to a sedentary group.Methods. EuroQol standardized instrument was completed by physically active elderly (n= 2,185) who participated in CEPs. Common items were compared to HRQoL data of 1,874 sedentary elderly people, taken from the Catalan Health Survey 2006 (CHS’06). Visual Analogue Scale (VAS) outcomes and medication used were assessed through parametric statistics. Dimensions of health conditions were compared, between sedentary people and physically active elderly participants in CEPs. SHC results were obtained combining the EuroQol scores and Life Expectancy (LE) values. An economic value of €34,858.70 was assigned to these years of LE.Results. Physically active subjects had better HRQoL values (75.36 in males and 70.71 in females) than CHS’06 sedentary subjects (58.35 in males and 50.59 in females). Medication used was different between physically active subjects (1.89 in males and 2.87 in females) and CHS’06 sedentary subjects (4.34 in males and 4.21 in females). SHC data for physically active elderly (€465,988.31/QALY in males and €522,550.31/QALY in females) were higher than for CHS’06 sedentary subjects (€363,689.33/QALY in males and €346,615.91/QALY in females).

Suicide rates generally increase with age. Examination of cross-national variations in elderly suicide rates may allow the generation of aetiological hypotheses. Suicide rates for males and females in the age-bands 65-74 years and 75+ years were ascertained from the World Health Organisation website for all the listed countries. Cross-national variations were examined by segregating different countries into four quartiles of elderly suicide rates. Suicide rates between males and females and between the two age-bands were compared across different countries. The main findings were: (i) there is wide cross-national variation in elderly suicide rates; (ii) elderly suicide rates were the lowest in Caribbean, central American and Arabic countries, and the highest in central and eastern European, some oriental and some west European countries; (iii) suicide rates were higher in men compared to women for both the age-bands; and, (iv) suicide rates were higher in the age-band 75+ years compared to the age-band 65-74 years for males and females. Potential explanations for regional and cross-national variations in elderly suicide rates include cross-national differences in genetic and environmental factors, prevalence of mental illness in the elderly, life expectancy, socio-economic deprivation, social fragmentation, cultural factors, the availability of appropriate healthcare services, and public health initiatives to improve the detection and treatment of mental illness, mental health and suicide prevention.

Informal caregivers are the main providers of care for the elderly. The aim of this study is to examine the predictive value of different variables regarding caregivers and their elderly patients with respect to the caregiver’s burden. A convenience sample of 688 informal caregivers and 688 elderly people from Ecuador was surveyed. Only households with one caregiver and one elderly person were considered for the study. For informal caregivers, the following standardized measures were obtained: burden (Zarit Burden Interview), neuroticism (Eysenck Personality Questionnaire Revised-Abbreviated, EPQR-A), caregiver’s general health (GHQ-12), and social support (modified Duke-UNC Functional Social Support Questionnaire, FSSQ11). For the elderly, we employed standardized measures of cognitive function (short portable mental status questionnaire, SPMSQ), Pfeiffer’s test, and functional dependency (Barthel scale/Index, BI). Females were over-represented in caregiving and reported significantly higher burden levels than those of males. In both male and female caregivers, the burden was best predicted by the time of caring, neuroticism, and elderly cognitive impairment. However, some predictors of burden were weighted differently in males and females. The functional independence of the elderly was a significant predictor of burden for male caregivers but not females, while caregiver competence was a significant predictor for females but not males. These variables accounted for more than 88% of the variability in informal caregivers.

The increased life expectancy of the elderly may require substantial redesigning of environments in order to accommodate age related body changes. One of the most important aspects allowing the elderly to function independently is the ability to reach for items comfortably during daily activities. Designing for an independent elder requires knowledge of reach measurements that determines the optimal design of working/living environments. This paper provides reach capability design data of elderly males and females between 65 and 89 years of age. Results show that direct and indirect reach indicators decrease substantially above the age of 80. The elderly participating in this study were compared with younger cohorts, showing significantly lower reach capabilities. This indicates the necessity of considering anthropometric data, such as reach, to design for the elderly. The need is particularly dire for those above 80 years of age.

Elders in rural areas of developing countries are particularly vulnerable to cognitive impairment and gender disparities are notable. Yet, evidence related to gender differences in cognitive impairment in these settings is scarce. This study examined gender differences in cognitive impairment among elderly people aged 65 and above in rural China. A multi-site cross-sectional survey was conducted. In total, 550 males and 700 females were recruited. The Chinese version of the Mini-Mental Status Examination was applied for cognitive impairment screening. Demographic information, lifestyle factors, psychosocial factors, and health-related information were assessed. The prevalence of cognitive impairment was 40.0%in males and 45.1% in females, respectively. Females showed significant higher prevalence after age 75 (62.7% vs. 45.4%, p < 0.005). Older age, hearing impairment, and activities of daily living dependence were common factors associated with cognitive impairment in both females and males. For males, living in a neighborhood with poor social interactions and diabetes were risk factors, while tea consumption was a protective factor. For females, vision impairment and illiteracy were additional risk factors. Individualized interventions and standardized measurements of cognitive function should be developed to suit older populations living in rural and less-developed areas, through collective efforts involving all stakeholders and multidisciplinary teamwork.

Background: Aging is increasingly recognized as a health and social problem with challenges on family and the society. The present study aimed at assessing prevalence and types of abuse among elderly and the various associated factors. Methods: A descriptive cross-sectional study was conducted from June to November 2018 among elderly persons (, 60 years) in a community developmental block of Purba-Bardhaman district, West Bengal. Based on required assumptions (50% prevalence, 15% relative error, design effect of 2 and 10% non-response) a total sample of 378 was selected through multistage sampling. With prior informed consent, data were collected using a predesigned - pretested schedule prepared with the help of Hwalek– Sengstock Elder Abuse Screening Test (H-S EAST) tool. A multivariable logistic regression model was generated using the SPSS software. Results: Among 371 subjects studied mean age was 67.39 (±6.063) years, 51.8% were females. Overall 11.9% (44) of elderly were found to be abused and prevalence of elderly abuse among females and males were 13.5% and 10.1% respectively. Psychological abuse was most frequent (95.5%) followed by neglect (84.1%), financial exploitation (52.3%) and physical abuse was least (9.1%). Among the various factors studied, living without spouse [AOR =2.690 (1.355-5.340)] and without social-security support [AOR =4.982 (1.449-17.126)] were found to be significantly associated with elderly abuse. Conclusion: Elderly populations in this study area are vulnerable to abuse and adequate social security support might reduce elderly abuse.

Research Question: What are the health-related problems of the aged and whydo they feel lonely? Objective: To study the health-related problems and loneliness among theelderly in different micro-environment groups. Participants: Aged persons of age 65 years andabove. Design: Cross-sectional. Setting: Urban and rural area of Abbottabad. Period: 1st Jan,2012 to 31st Dec, 2012. Statistical Analysis: t-test and Z-test. Results: During the study, it wasfound that out of the total 361 aged persons of Abbott bad, 311 (86.1%) persons reported one ormore health-related complaints, with an average of two illnesses. The illness was higher amongthe females (59.5%) as compared to males (40.5%). The main health-related problems weredisorders of the circulatory system (51.2%), musculoskeletal system and connective tissue(45.7%). It was also found that loneliness was prevalent more in females (72.8%) as compared tomales (65.6%). Loneliness was more prevalent among persons who lived alone (92.2%) ascompared to those who lived with their spouse (58.9%) or when husband and wife lived with thefamily (61.4%). It was higher among the widows (85.2%) and widowers (75.8%) who lived with thefamily as compared to the aged who lived with the spouse (58.9%) and the aged husband andwife who lived with the family (61.4%). Conclusions: Special geriatric services should be startedin the hospitals as the majority of the aged have one or more health-related problems. The agedpersons should be involved in social activities to avoid loneliness among them.

The dietary data were described using the daily per capita availability of food and beverages, energy and selected nutrients (macronutrients, different lipid fractions and simple sugars). Differences between elderly household types and time trends were studied.SettingPortuguese Household Budget Survey data from 1989/1990, 1994/1995, 2000/2001 and 2005/2006.SubjectsHouseholds with members aged ≥65 years were selected and categorized as solitary elderly female, solitary elderly male or couple (composed of one elderly female and one elderly male).ResultsWhile cereals, fats/oils, potatoes and sugar/sugar products decreased, an increase occurred in milk/milk products, fruits, bottled water, fruit/vegetable juices and soft drinks (P<0·05). The highest values for foods and beverages were mostly found in couples, while the lowest ones were from solitary males. Exceptions were observed for cereals, eggs, milk/milk products, vegetables, fruits and non-alcoholic beverages, higher in solitary females; and for sugar/sugar products and alcoholic beverages, higher in solitary males. Over time, total energy and carbohydrates decreased while proteins and saturated fatty acids increased (P<0·001). Lipids increased in solitary males and couples (P<0·05). Simple sugars increased in solitary males but decreased in solitary females and couples (P<0·05).ConclusionsThe increases in fruits and vegetables in solitary females accord with a healthier food pattern, but overall imbalances in the macronutrient profile for all elderly households may imply a decreasing diet quality.

Chronic pain and fatigue have negative effects on the health, ADL, work, and hobbies of the elderly. As the proportion of people 65 years of age and older in the population increases, chronic pain and disability research regarding this group is receiving more consideration. However, little empirical evidence of the association between chronic pain, fatigue, and physical disability between the sexes is available. This study investigated the association between chronic pain, fatigue, and instrumental activities of daily living among community-dwelling elderly people by sex in Japan. Concerning the presence of chronic pain, 61% of males and 78% of females reported chronic pain, indicating that many elderly people living in the community suffer from chronic pain and fatigue on a daily basis. The number of sites of chronic pain was higher in females than in males (p = 0.016), with more chronic pain in the knees (p < 0.001) and upper arms (p = 0.014). Regarding chronic pain, males showed a higher correlation with QuickDASH-DS (rs = 0.433, p = 0.017) and QuickDASH-SM (rs = 0.643, p = 0.018) than females. Furthermore, fatigue also showed a higher correlation with QuickDASH-W (rs = 0.531, p = 0.003) in males than in females. These results indicate that the association between chronic pain, fatigue, and QuickDASH differed between the sexes among community-dwelling elderly people in Japan. A better understanding of the risk factors for elderly chronic pain and fatigue among sexes will facilitate the development of elderly healthcare welfare and policies.

The aim of the study is to determine the size and to identify the features of histoarchitectonics of the uterine tube ampulla of an old-aged female. Materials and methods. The ampulla of both uterine tubes was examined in 26 nulliparous and 78 giving birth elderly women using morphometric, histological, immunohistochemical, and micrometric research methods. Results. The length and outer diameter of uterine tubes at the points of transition of isthmus into the ampulla and ampulla into the infundibulum as well as in the central part of ampulla are characterized by the absence of a statistically significant difference in parameters in nulliparous and giving birth women. The histoarchitectonic features of the ampullae of the uterine tubes in nulliparous and giving birth women in old age consist in the flattening of the epithelium of the mucous membrane, which forms many closely spaced thickened folds that create an uneven narrowing of the ampulla lumen, and the proliferation of connective tissue between the bundles of muscle fibers of the muscular membrane. The presence of Ki-67 expression in single cells of less than 1%, as well as a thin layer of epithelial membrane antigen in the epithelium of the ampulla along with the complete absence of its expression in the stroma reflect normal tissue regeneration and act as an equivalent to the anatomical norm in elderly females. When comparing lumen areas of ampulla and its wall at mid-section as well as the circumference of epithelial lining, a predominance of parameters in the right uterine tube was revealed (p<0.01). Conclusion. The study showed the similar qualitative and quantitative morphological characteristics of the uterine tube ampullae both in nulliparous and giving birth females in old age.

Gait change in the elderly may be a strategy to maintain postural stability while walking. However, gait laterality is accompanied by back pain or an increased risk of a fall. This study aimed to examine group-related differences and gait laterality in elderly females with mild or severe unilateral knee pain. Seventy-five elderly females (66–87 years old) were included, which comprised the following groups: 47 with mild unilateral knee pain and 28 with severe unilateral knee pain. They completed a 12 m walk test with maximum effort. Stance time, swing time, and step length were selected as evaluation parameters. A two-way ANOVA (group × leg) was used for analysis. No significant differences were found in interaction or in either main factor of the group and leg. In conclusion, elderly females do not show group-related differences or gait laterality regardless of the degree (mild or severe) of unilateral knee pain.

The aim of this study was to report on the average daily energy expenditure by elderly males and females in their normal day-to-day activities. The study was based on an analysis of data collected over six months from daily diary recording of activities. The subjects, 82 males and 93 females, were non-institutionalized volunteers with a mean age of 68 years. A multivariate analysis of variance indicated that there were no differences between males and females in total mean daily energy expended and energy expended on Sporting activities. Females expended over twice as much energy as males on Household Chores (F = 36.3, p < 0.001). Males expended over three times as much energy as females on Gardening (F = 56.7, p < 0.001) and over seven times as much on Maintenance (F = 53.6, p < 0.001). There were also differences between males and females on energy expended while Walking (F = 6.1, p < 0.02) and on Miscellaneous activities (F = 6.3, p < 0.05). It is concluded that sex stereotypical roles for activities required at younger ages are continued into old age. The implications of these findings for prescribing regular exercise in the elderly are discussed, indicating specific activities that might prove to be beneficial.
