Ein Blick in die Zukunft lässt erkennen, dass unser vermeintlich grenzenloser Konsum langsam aber sicher an seine natürlichen Grenzen stößt. Der fortschreitende Klimawandel und seine nicht ignorierbaren Auswirkungen auf die Umwelt, unser Umfeld und unsere Gesellschaft zwingen unser Wirtschaftssystem und seine Akteure dazu, wirtschaftliches Handeln nachhaltiger zu gestalten. Im Rahmen dieses Zukunftsberichtes sollen Einblicke in aktuelle Forschungsprojekte und Ergebnisse studentischer Forschungsarbeiten der Professur für Marketing der CAU Kiel gegeben werden, die sich mit dem Wandel hin zu nachhaltigerem Konsumentenverhalten beschäftigt. In qualitativen Analysen wird die Einstellung von Konsumierenden zu Themen wie dem Fleischkonsum, der Undurchsichdigkeit inflationärer CO2-Labels, dem Phänomen Flugscham und der Frage nach Nachhaltigkeits-Lehre in der Schule, beleuchtet. Darüber hinaus wird im Rahmen verschiedener Forschungsprojekte der Professur vermittelt, wie sich klimafreundliche Ernährung durch „Nudging“ fördern lässt, wie indirekte Rebound-Effekte auf Ebene privater Haushalte wirken, welche Verantwortung auf Konsumenten in Zeiten der Digitalisierung zukommt, wie sich Konsumentenverantwortung in Zeiten der Globalisierung interkulturell unterscheiden lässt und was es mit dem Phänomen der Konsumentenfeindlichkeit auf sich hat.

Postextubation stridor arises from glottic and subglottic edema caused by ischemia of the tracheal mucosa from pressure by the endotracheal tube. Multiple risk factors have been described; preventive measures include appropriate tube sizing, air leak tests, administration of steroids, and smooth airway management techniques, such as atraumatic intubation. When stridor does occur, cool humidified air as well as racemic epinephrine may be used as treatment. The patient is safe for discharge once symptoms have dramatically improved and the window for potential “rebound effect” from racemic epinephrine has passed with no further stridor.

Pulmonary vasodilators (PV) are commonly used in the intensive care unit (ICU) to treat pulmonary hypertension and/or hypoxaemia. The choice of drug is based on its pharmacokinetic and pharmacodynamic properties. The inhaled route of administration is preferred to treat hypoxaemia as it improves the ventilation/perfusion ratio. Systemic administration of PVs can lead to a decrease of mean arterial pressure and a worsening of hypoxaemia. Despite their beneficial effects, PVs have not shown improvement in mortality in acute respiratory distress syndrome patients. Rebound of hypoxaemia and/or pulmonary arterial hypertension should be prevented during PV treatment discontinuation with a slow de-escalation protocol. This chapter reviews the use of the main PV available for use in the ICU.

Sedative-hypnotic-use disorder is a serious problem in the elderly and is a growing concern in the United States. The American Geriatrics Society’s “Choosing Wisely” initiative cautions against the use of any benzodiazepines or other sedative-hypnotics as initial treatment in older adults, yet benzodiazepines are the most frequently prescribed drugs in the elderly for both insomnia and anxiety. Other classes of medication (e.g., serotoninergic antidepressants) may be substituted for benzodiazepines based on diagnosis. With advancing age, the elderly are more sensitive to the potential side effects of benzodiazepines because of altered pharmacokinetics and pharmacodynamics Increasingly, studies have indicated that older patients disproportionately experience adverse events with benzodiazepines such as falls and cognitive deficits and have difficulty reducing or stopping long-term use without experiencing rebound effects such as anxiety and insomnia. Sedative-hypnotic-use disorders among older adults are increasing in prevalence and warrant heightened clinical attention, thoughtful assessment, and active management.

This chapter analyses the evolution of fiscal and monetary variables in Ghana, from the discovery of oil in 2007 through to 2014. It documents the deterioration of fiscal and monetary discipline over this period, which resulted in a rebound of debt, a deterioration of the external balance, and a decrease in public investment. The chapter goes on to analyse the potential causes of this deterioration, including the political economy context, and the fiscal and monetary institutional framework. The suggested causes include the politics of Ghana’s dominant two-party system. Finally, the chapter discusses what Ghana could have done differently to avoid the various damaging effects associated with the oil discovery. It does not aim to provide specific fiscal policy recommendations for Ghana, but rather to give an empirical account of Ghana’s experience that may be useful for other countries that discover oil.

The episcopate, cities, and provincial estates were all examples of institutions or corps that felt the effects of royal displeasure, but nothing better illustrated the phenomenon than the treatment of the parlements. This chapter explains the structure of judicial politics under the Bourbon monarchy, and looks at the many ways in which the king could use imprisonment, the internal exile or transfers of individuals or groups of magistrates from one city to another, disciplinary edicts, and many other often subtle techniques to impose his authority. It also considers the limitations of disgrace, how it could sometimes rebound to the disadvantage of the crown and the obstacles to the settlement of political crises resulting from the act of disgrace.

The benefit of prolonged application of moderate-dose corticosteroids in systemic inflammatory diseases remains controversial. In critical illness, the endogenous cortisol effect may become insufficient due to adrenal dysfunction and corticosteroid resistance to counterbalance an exaggerated and protracted inflammatory response, which has been termed ‘critical illness-related corticosteroid insufficiency’ (CIRCI). There is evidence that moderate-dose hydrocortisone (200–300 mg/day) significantly fastens shock reversal in patients with septic shock, but may improve survival probably only in patients with high risk of death. Thus, therapy should be considered only in refractory shock with poor response to fluid administration and vasopressor therapy. The indication should be based on clinical judgement and not on cortisol measurement. The application prolonged of moderate-dose methylprednisolone (1 mg/kg/day) was found to be most effective in early acute respiratory distress syndrome, and associated with improved lung function, reduction of mechanical ventilation, and faster discharge from the ICU, but a survival benefit was found only in pooled data, including cohort studies. A continuous infusion and weaning of corticosteroids may be preferable to bolus applications and abrupt withdrawal to avoid side effects such as rebound of inflammation and shock, glucose variability, or respiratory failure. There is currently no evidence that prolonged application of moderate-dose corticosteroids increase the risk of secondary infections or muscle weakness, but infection surveillance should be implemented and combination with muscle relaxants be avoided.

In the 1960s and 1970s, corporate law and finance scholars gave up on their traditional approaches. Corporate law had become “towering skyscrapers of rusted girders, internally welded together and containing nothing but wind.” In finance, the theory of the firm was recognized as an “empty box.” This essay tracks how corporate law was reborn as corporate governance through three examples of how we have usefully complicated the inquiry into corporate behavior. Part I frames the first complication, defining governance broadly as the company’s operating system, a braided framework of legal and non-legal elements. Part II adds a second complication by making the inquiry dynamic: corporate governance as a path dependent process that co-evolves with the elements of the broader capitalist regime. Part III considers unsuccessful efforts to simplify rather than complicate corporate governance analysis through static single factor models: stakeholder, team production, director primacy, and shareholder primacy. Part IV concludes by highlighting the tradeoff between a governance system’s capacity to adapt to change and its ability to support long-term investment.

Liberalism—and its promise of market-led prosperity—was in crisis well before Covid-19. Recent decades have seen a rise in concentrated unemployment, and a long-term stagnation in real wages, in many of the world’s leading economies. At the same time, the world has witnessed a dramatic rise of corporate power, and the wealth of the top 1%. Alongside this has been the failure of liberal societies to address some of the most pressing challenges of our time, including climate change. Covid-19 has only exacerbated the fragility of work, and the effects of corporate power and inequality. When Covid-19 is over, liberalism will therefore be badly in need of renovation. Indeed, to survive, liberalism will need a radical reboot—to find new ways of tackling the current challenges posed by corporate power, inequality, and climate change. This also means moving beyond recent “neoliberal” versions of liberalism toward a more truly democratic form of liberalism, or from the idea of free markets to a vision of fair markets. Fair market policies are not democratic socialist: they hold on to the idea of markets as promoting growth and freedom. But they insist that markets must be subject to wide-ranging democratic regulation. This book offers a new vision of a “fair markets” approach–and the concrete policies that could make this ideal a reality. It proposes: (1) a universal “green” jobs guarantee; (2) a significant increase in the minimum wage and government support for wages; (3) universal healthcare based on a two-track model of public and private provision, and (4) a similar public baseline for childcare and basic leave benefits for all workers; (5) a new critical infrastructure policy for nation states to sit alongside a commitment to global free trade; and (6) universal pollution taxes, with all proceeds returned directly to citizens by way of a green dividend. The common theme of all the policies is that they combine a commitment to markets with democratic commitments to equal dignity for all citizens, and the regulation of markets in line with majority interests and understandings—or the idea that markets should be both free and fair, and well-functioning, as opposed to simply “free.” Because of this, they are also policies that are “blue,” “pink,” and “green.” The book also explains how to pay for these ideas, and the kind of democratic politics needed to make them a reality.
