Adverse effects during clozapine treatment are common, and can be divided into very common (>10%: constipation, weight gain, metabolic side effects, sedation, and sialorrhea), common (1–10%: seizures and enuresis), and cardiac (sinus tachycardia, electrocardiogram abnormalities, and orthostatic hypotension) adverse effects. Most adverse effects are benign, but often reduce the quality of life for the patient, leading to reduced adherence and thereby psychotic relapse. As a consequence, treatment of these adverse effects is important and should not be neglected. In this chapter, we present specific treatment strategies for each adverse effect. In addition, we also emphasize that, by applying simple general managing strategies, such as reducing the clozapine dose, re-arranging the dose, or augmentation with another antipsychotic drug, many of these adverse effects can be avoided or reduced, which should reduce the need for specific rescue medications.

The antidepressant and neuroprotective effects of oestradiol are described. Psychiatric consequences of oophorectomy, and treatment with tamoxifen and aromatase inhibitors, are then discussed. Androgen-deprivation therapy has temporary effects on cognitive function and mood that reflect the distribution of androgen receptors in the brain. The rapid-onset adverse psychiatric effects of high-dose glucocorticoids are presented (including ‘steroid psychosis’) and a novel, non-genomic molecular mechanism highlighted. In contrast, the depressive effect of chronic glucocorticoid use is then considered.

Streszczenie:
The case illustrates how to approach imaging in pregnancy. Some basic principles of radiation exposure are reviewed. The effects of radiation are divided into either deterministic, dose-related, or stochastic, where exposure determines the probability of an outcome. Deterministic effects have a threshold below which the effect does not occur. The threshold may be small and may vary from person to person; whereas stochastic effects occur by chance and may occur without a threshold level. Cancer caused by radiation is an example of a stochastic effect. The overall safety of magnetic resonance imaging is discussed. Determining which imaging modality to order depends on the acuity of the situation and the information needed to know in order to adequately treat the mother.
