Burke, Sarah Elizabeth. "The doctor-patient relationship : an exploration of trainee doctors’ views". Thesis, University of Birmingham, 2008. http://etheses.bham.ac.uk//id/eprint/125/.

Streszczenie:
Greater understanding of the ways in which medical trainees perceive the doctor-patient relationship could inform future developments in educational provision. A qualitative study was conducted, using a case study approach to explore the perceptions of postgraduate trainees in two medical specialties, general practice (GP) and otolaryngology (ear, nose and throat surgery, ENT), in the West Midlands region of the United Kingdom. Following a scoping exercise in 2002, interviews with 20 trainees (10 GP and 10 ENT) in 2004 and questionnaires from 16 ENT and 89 GP trainees in 2007 explored trainees’ views of the doctor-patient relationship, including perceptions of the nature of that relationship and how they had learnt to develop relationships with patients. Five conceptual frameworks that participants drew upon when talking about the doctor-patient relationship were identified: paternalism; guided decision-making; partnership; clinical and consumerism. Trainees described a fluid doctor-patient relationship which adapts to differing contexts, taking different forms in different situations and influenced by factors outside the doctor’s control, including time and the patient’s personality. Personal experience and observing senior colleagues were considered to have had the greatest impact on learning. Higher Specialist Training which acknowledges the complexity of the doctor-patient relationship and encourages reflective practice is recommended.

Television is a widespread, easily accessible component of popular culture that we invite into the most intimate of environments: our homes. Like other forms of popular culture, it is not only influenced by religious belief, but also has the power to transmit both traditional and subversive religious ideas to viewers. This thesis draws upon methods used in the study of religion and popular culture to argue for the potential of television to influence religious belief. Television can transform contemporary thought and renegotiate ideas of identity as well as reposition social debate and conflict in both secular and religious environments. Focusing on the British television show Doctor Who, this thesis closely analyzes recent episodes of the series to illustrate the ways in which current show-runner Steven Moffat's story arc puts forth a Freudian critique institutional religion. This thesis demonstrates that Doctor Who has developed in such a way as to reveal the deception of organized religion and its destabilization of modern notions of rationality. Rather than banishing religion to the trash heap of history, however, Moffat's Doctor Who encourages believers to reevaluate traditional religious belief and practice.

This thesis has used the methodology of an ideological criticism on the long-running science-fiction television series, Doctor Who. Argued within, is that an ideological paradigm shift occurred during the fifty years of scripted storylines. To discern evidence supporting the shift, multiple episodes were viewed from each of the first eleven Doctors. During the viewing process all aspects presented within the show, such as dialogue, movements, tactics, and traits, were explored for ideological agency. Once found, the discourse was further analyzed to understand how these ideologies were enforced. From the findings, proof was examined in order to show by the end of the eleventh Doctor, Doctor Who had moved from portraying the viewpoints of individualism to collectivism.

Can objects function as a political demand, necessitating changes in the way the world is ordered? Who for? To understand one kind of oppression in necessary sterility and another in marginalization so profound blindness can result. That is to ask, how long must one be told they do not see a thing they see before they don’t, before transgressions become norms? Evidence demonstrates the health benefits of early breastfeeding for mothers as well as infants. The World Health Organization states, "Breast milk should be given within the first hour of life." This purpose of this project was to develop a breastfeeding educational toolkit to promote breastfeeding in the hospital environment. The question that guided this project was, "Will the development of a breastfeeding education toolkit, designed to serve as a learning tool to support the breastfeeding mothers, be found to be useful and valid by professional staff and lay end-users?" The goals of the project were to provide an educational toolkit aimed at supporting new mothers, and to validate the toolkit with stakeholders from the Mother/Baby unit. The benefits of breastfeeding and breastfeeding research were established through an evidence-based search of peer-reviewed journals. The developed toolkit was reviewed by10 stakeholders from the Mother/Baby unit: 1 physician, 3- lactation consultants, 5 nursing professionals, and 1 layperson of childbearing age. To assess the quality of the guidelines, the Logic Model and the 4 domains of the AGREE II Instrument were used. The descriptive data analysis from the AGREE II instrument led to the recommendations for the toolkit. All reviewers acknowledged the need for the toolkit as a support for the mothers and their babies. The stakeholders provided their support for the developed toolkit. The breastfeeding education toolkit has the potential to support positive social change by improving the health of the mothers and babies through improved breast-feeding knowledge.

Streszczenie:
Communication is not only a means, but especially a privileged place where relationship takes place. I here argue that trustworthy relationship can be established through communication and that communication is the favored moment when to do it. Thus, knowing and understanding language and its dynamic is essential to orient oneself during communication; this would allow the speakers to fully take the opportunity to foster mutual trust. Knowing language does not only mean managing what is said, but especially be aware of what it implies, entails, and what is unsaid. This is especially true in the case of doctor-patient communication, where one of the speakers is also the subject of the speech. In the Introduction, I suggest that communicating means also to act, thus influencing, and directing the way things are; both how and what is said are—and must be—embedded in communication to make it successful; and the implicit dimension of communication has a prominent role in defining the meaning of the whole discussion. I show the case of doctor-patient communication as representative: I consider the moment of interaction between the physician and the patient to be the chance for building and consolidating a strong therapeutic alliance. If the chance is not taken or wisely managed, it could cause the opposite, i.e., loss of trust, also possibly influencing patient’s concordance to treatment. I then analyze the non-neutrality of this special relationship, in which the speakers are not only two persons in dialogue, but especially two roles facing each other: the professional (physician) and the one who benefits from that profession (patient). This asymmetry entails implicit meanings worth to be analyzed that strongly influence what is said and understood. The structure of the thesis is composed by different articles suggesting a hypothesis, that is then applied to examples taken from real situations (mainly from my ongoing study protocol, which may be consulted in the appendix): when not already in the article, the applicative part follows the theoretical separately. The first article starts from a historical analysis to show why medicine should go beyond science, regaining its humanistic theoretical foundations; an experimental corollary follows. The second article aims to show the power of implicit and how it is intertwined with trust; strings of doctor-patient interaction are analyzed and problematized. The third article shows how manipulation could be a positive phenomenon, introducing the new definition of eumanipolation; an experimental corollary follows. Since COVID-19 outbreak abruptly changed everyone lives, the last article is a dutiful extension from doctor-patient to doctor-public communication that considers the unexpected and challenging effects of the pandemic on communication. In the Conclusion I discuss results and suggest possible future research on the topic.

Aim: To evaluate the clinical effectiveness of a counselling psychology service in primary care. Method: Comparison of SCL-90R and HAD scores of a group of clients in treatment with those of a control group receiving GP-only care, at four points in time; and with scores while waiting for treatment. The design combines elements of an open trial with elements of a randomised controlled trial, although full randomisation was not possible in practice. The results are expressed in terms of numbers of cases and effect size, as well as in terms of test scores, in an attempt to indicate levels of clinical as well as statistical significance. Comparison of number of visits made to general practitioners by participants in each group during the six months before and after treatment. Results: The results indicate that the service was clinically effective: clients improved significantly after treatment on the scales used, the numbers of 'casee decreased significantly, and the number of visits to the general practitioners also dropped. Compared to the control condition, the treated clients did better on all these indicators, but the difference between the two groups was not great enough to show statistical significance at the 0.1% level required by the use of multiple planned tests, or on MANOVA, although on the major indicators (SCL-90R GSI and HAD Depression and Anxiety) significance was reached at the 5% level. The overall effect size was coculated to be 0.32. As it was 15 greater than zero, it indicates that the service's interventions were more effective than GP-care, but the magnitude of the effect was in the small to medium range. Conclusions: The results of the research demonstrate that the counselling psychology service under study was clinically effective. On all indicators used, clients of the service improved over the period of treatment, and did so to a greater extent than patients in the control condition. However the advantage over the control group was not sufficient for statistical significance at the level required, and this is reflected in the relatively moderate effect size.

Passi, Vimmi. "How does positive doctor role modelling influence the development of medical professionalism in future doctors?" Thesis, University of Warwick, 2013. http://wrap.warwick.ac.uk/62713/.

Pełny tekst źródła
Streszczenie:
Background There has been an explosion of interest in medical professionalism over the past decade but at present there are no evidence based guidelines on how to effectively develop medical professionalism in future doctors (Passi et al. 2010). Role modelling has been highlighted as an important method to help develop professionalism but there is no current theory regarding the process of role modelling (Passi et al. 2013). Therefore, the aim of this PhD was to investigate how positive doctor role modelling influences the development of professionalism in future doctors. Methods A qualitative methodology using the grounded theory inquiry approach of Strauss and Corbin (2008) was used to generate a general explanation (a theory) of the process of role modelling shaped by the views of the participants. The study involved focus groups with final year medical students, semi structured interviews with consultants and semi structured interviews with consultants and final year medical students immediately after outpatient clinics. This systematic approach used involved open coding, axial coding and selective coding to reveal the processes involved in role modelling, which is illustrated in a coding paradigm diagram. Results The results revealed a new theory of doctor role modelling which is described as follows – Doctor role modelling is an important process in medical education that involves conscious and subconscious elements. It consists of an Exposure Phase followed by an Evolution Phase. The exposure phase involves demonstration of professional attributes by the doctor role models (clinical expertise; relationships with patients, students and colleagues; personality and inspirational characteristics). The evolution phase begins with observation of the role model by the modellee, following which the modellee makes a judgement whether or not to trial the observed behaviours of the role model. When the decision to trial is reached, this then leads to the Model Trialling Cycle which involves 5 stages of assembly, emulation, experimentation, adaptation and assimilation. The outcome is the evolution of a professional doctor who has developed their unique professional identity and career aspirations. Conclusion This detailed qualitative study has provided a new theory of doctor role modelling in medical education. The impact of role modelling is in the development of medical professionalism professional identity and the influence of career choice. The theory can now be incorporated in medical curriculums worldwide to enhance the development of medical professionalism. Detailed recommendations for clinical practice and future research are described.

Streszczenie:
Some health care experts suggest that people should receive health care based on "need" rather than on their ability to pay. Previous research has found that people with lower incomes use health facilities less than people with higher incomes when health status and age are taken into account and also that this tendency is lessening over time. This study supports the conclusion that income is still an important factor in determining whether people visit a doctor, which indicates that the income effect is stronger than the effect of higher opportunity cost of time. Researchers trying to determine whether medical care is distributed equitably have not looked at the type of doctor seen by the patient; the type of doctor is important because the quality of care matters and specialists can provide higher quality care than general practitioners. In order to determine whether health care is distributed by "need" or by ability to pay, it is necessary to make adjustments for the quality of care.

Drawing upon a significant weight of empirical data, collected in the field, this thesis proposes a set of four spectrums of investment engaged in by cult media fans: the spectrum of financial investment; the spectrum of what is here termed 'participatory investment'; the spectrum of investment in the idea of textual authenticity; and the spectrum of multiple investments. The spectrum model allows the individual members of the research sample to be located within specific regions of each spectrum and correlations to be drawn between the distinct spectrums, in order for any patterns which emerge to be examined. The thesis also reviews a number of relevant theoretical concerns such as fan studies, ethnography and social psychology.

Streszczenie:
Patients' non-adherence to treatment is a widespread phenomenon in healthcare. Time preferences (how individuals value outcomes over time) are one cause for non-adherence. Using quasi-hyperbolic discounting, two options in the future are weighted consistently. However, when the early option becomes available the weighting changes. This creates the potential for non-adherence. The agency relationship that exists between patients and doctors implies hidden information. When the patient's time preferences are hidden from the doctor, the doctor must choose how to recommend treatments. Exploring how doctors make treatment decisions when time preferences are hidden from them, and how this impacts adherence, is therefore important. The first contribution of the thesis is to outline a model of the patient-doctor interaction incorporating quasi-hyperbolic discounting and hidden information. This shows that doctors should adapt to non-adherence when the probability a patient is present-biased is large enough. Secondly, a national survey of Scottish GPs explores whether doctors have different time preferences for themselves or their patients. Doctors do have the same private and professional time preferences, but value the health state differently between frames. Lastly, a laboratory experiment tests whether students in the role of a doctor adapt to non-adherence in the way predicted by the model. Students find the socially optimal level of treatment on average. Adaptation is stronger when using a performance payment, and results did not vary along demographic characteristics. The thesis highlights the importance of the patient-doctor interaction for generating nonadherence, not just patient preferences. It also shows that GPs' private time preferences may suitably substitute their preferences for patients. Finally, it points towards potential incentives for doctors to improve patient outcomes.

Streszczenie:
Over 15,000 master’s and doctoral degree students in the United States were denied admission to nursing schools in 2014 because of insufficient nursing faculty. In 2016, over 64,000 undergraduate and graduate students were unable to gain admission to nursing school due to the effects of faculty shortages. This project explored the role of the Doctor of Nursing Practice (DNP)-prepared nurse in academic settings using a systematic review of the literature to determine the role of DNP-prepared nurses in academia. Souza’s systematic review model and Melnyk’s levels of evidence were used to guide the search, review, and the selection of scholarly articles published between 2005 to 2019. A chart of preferred reporting items for systematic reviews and meta-analyses chart was used to organize and select 14 articles meeting the review criteria and included in the analysis. Four themes emerged from the analysis of literature: role in academia from the dean’s and director’s perspective, DNP role as a teacher, preparation for faculty role, and leaving the faculty role. Confusion over the role of the DNP in academia was also identified as a factor affecting DNPs in academic practice settings; however, DNPprepared nurses have the clinical experience, knowledge, and skills to provide evidence-based teaching and fill the gap in practice needed in academic settings. This project may promote positive social change by raising awareness of the role of the DNP in academia to reduce the faculty shortage.

Streszczenie:
Gamebooks represent a strange phenomenon in the literary world. Many people believe them to be a long-dead genre which could not survive the extremely rapid evolution of roleplaying games, and thus fell victim to the advent of computers and videogames. Translating a gamebook not only means to walk in its writer’s shoes, but also forces the translator to face a completely different type of roleplaying experience, given that adventure gamebooks had a very restricted fanbase during their brief history. A successful translation of this type of literature requires a remarkable deal of attention, precision and passion. The aim of this study is to present a possible translation of the science-fiction gamebook “Doctor Who: Choose your Adventure: Night of the Kraken”. In the first chapter we will be analysing the history, varieties and peculiar features of this complex interactive books, while also analysing the noteworthy educational potential still held by this type of literature to this very day. The second chapter will focus on the theme of the gamebook: this section will contain a detailed history of the Doctor Who franchise from its humble beginnings to its modern glory and widespread popularity. This chapter will also briefly discuss its educational purpose. The third and final chapter will contain the proposed translation of part of said gamebook and will focus on the analysis of said translation, describing the main difficulties encountered during the translation process and how they have been solved.

Webb, Helena. "Doctor-patient interactions during medical consultations about obesity". Thesis, University of Nottingham, 2009. http://eprints.nottingham.ac.uk/10818/.

Streszczenie:
The current “obesity epidemic” is a global concern for governments and healthcare organisations. Obesity is seen as a medical problem of excess body weight which can be resolved through interventions to encourage weight loss, most particularly diet and exercise regimes. Much existing sociological work focuses on moral understandings of obesity as a perceived symbol of individual greed and laziness in a culture that prioritises self-control and effort. This neglects the ways in which the condition is actively discussed and managed in relevant settings such as medical encounters. This thesis addresses this research gap by analysing talk during obesity-related medical consultations. Talk is central to all medical encounters and has particular resonance in treatments for obesity where most interventions are carried out by the patient away from the medical gaze. Patients must report on their treatment behaviours in ways that enable practitioners to evaluate them and offer further relevant advice. Talk is not only a means through which treatment is delivered but a form of treatment itself. Fieldwork took place in two UK NHS outpatient clinics specialising in weight loss treatment for obese patients. A sample of 18 patients and 1 doctor consented to have their consultations video-recorded over a period of 9 months. This resulted in 39 recorded interactions which were analysed according to the principles of Conversation Analysis (CA) to identify recurring patterns of interaction. The thesis describes how talk between doctor and patient functions to achieve certain tasks. In particular, it analyses how the specific institutional setting shapes and is shaped by talk. A dominant theme is that clinic interactions frequently invoke normative issues concerning knowledge, responsibility and effort. These issues are consistent with moral dynamics perceived to surround the condition of obesity and patient responsibilities. Doctor and patient collaboratively construct obesity as a moral issue. This has consequences for the conduct of the consultation. The findings extend existing CA knowledge on medical interactions and demonstrate the utility of an interactional approach to the sociological study of obesity. They also have relevance to healthcare policy and practice.

Jones, Louise. "Stage action as metaphor in Marlowe's Doctor Faustus". Virtual Press, 1991. http://liblink.bsu.edu/uhtbin/catkey/774755.

Streszczenie:
The purpose of the study is to establish the critical need for stage action in order to understand fully the theme of Marlowe's Doctor Faustus. Marlowe's primary intent is to invert the morality play, illustrating the distortions and ambiguities of a systematized religion and to establish the human dilemma when man is faced with moral choices. To illustrate this inversion, Marlowe uses emblematic action for an effect opposite to that of the traditional moralities: Often this action goes beyond the emblem, becoming a metaphor for Marlowe's theme, man as a victim, conflicting within himself and within the system which governs his morality.Chapter one introduces this theme and the crucial need for staging Marlowe's ideas. The first chapter also establishes a compromise of the textual problems inherent within any study of Doctor Faustus. Since the study argues that audience reaction is important to Marlowe's intent, attention is paid to how audience response governs the play's interpretation.Chapter two is a critical review of the historical staging practices which must be considered when studying the dramatic text. Included are stage size, costuming, and special effects.Chapter three is the advancement of the thesis in a scene by scene analysis of the text with special attention to the action as metaphor. Considered is how audience reaction represents part of Marlowe's purpose; the increasing tension of the audience furthers Marlowe's concept of the ambiguities present when humans are faced with moral choices. This purpose is traced scene by scene with specific attention to how it is metaphorically portrayed on stage.Chapter four is separate as a director's book, with the text reproduced, together with the researcher's marginal notes on specific blocking and with footnotes emplacing and expanding on the metaphorical action as it appears in the text.

Streszczenie:
This thesis attempts to explain the older-patients and doctors’ relationship through negotiation. The relationship between older-patients and their doctors is important because, first, the patient-doctor relationship is basic to the health care system and is a foundation for all patient care; second, populations are ageing and this group often has a higher incidence of chronic illnesses. Therefore, the older patient-doctor consultation becomes of paramount importance in enabling patients and the health care system to manage their illnesses effectively. This thesis describes a negotiation process (from reception, consultation, to outcome) and outcome patterns between older patients and doctors in medical consultation. It could be explained by the symbolic interaction perspective. Simply, both patients and doctors had gone through many internal thoughts and came to each other with a purpose or an expected outcome before the negotiation (i.e. the styles). Then there were gives and takes during the negotiation process (i.e. the consultation process), establishing a final equilibrium of a relationship falling into the 16 negotiation patterns developed. The study has been undertaken three principal methods in addition to reviewing the published literature.
