This study explores the changing interpretations of Henry Tonks' pastel drawings of disfigured soldiers from the aftermath of World War I. As the context evolved from a clinical environment to art historical, concerns developed not only regarding the reconstruction of the male body, but also the restoration of manhood after the First World War. The pre-War construction of masculinity, which turned man to machine, must also be evaluated in order to understand how Tonks’ images might reinstate the wounded men’s identities. The study examines the collective identity of British men during the First World War, focusing on those who were injured in battle. It compares Tonks’ pastels with other sources, in order to understand the changing and fragile definition of masculinity from the aftermath of war, and the reconstruction of manhood and identity of disfigured soldiers.

Acid attack or vitriol attack is an instance of brutality which is also an act of slinging acid by targeting another person’s body to disfigure their face in order to harm or kill them. Moreover, acid attack is a social concern which can cause physiological and economic problems. According to previous studies, 80% of the victims all around the world are women by gender. Hence, the current study is based on designing a women's wear collection for their safety. However, acid attack victims can’t use all types of fabrics to cover their burnt body parts. Therefore, beeswax is used to design their dresses which further facilitates them with the healing of their bruises. The application of beeswax finishes on the fabric is beneficial due to its natural characteristics which include antimicrobial, antibacterial, antifungal, moisturizing / hydrating, and eco-friendly properties. The aim of this research project is to use both technology and art in designing floral and Zen motif patterns. The purpose of using them, as a source material for modifying and designing fabric, is to create an effective women’s wear collection. Furthermore, such collection is aesthetically pleasing and yields functional benefits to protect women’s skin from acid attacks as well. Aim. The aim of this article is to deal with visual dignity in the field of western art throughout the ages as it seeks to present the artistic and design devices employed by artists in order to compose and convey this aura of dignity. Moreover, it addresses the ways in which they create various kinds of atmosphere of respect and dignity felt by the observer regarding the topic and object of the painting or sculpture. Methods. In order to achieve this aim, the article examines seven masterpieces which explore the visual representations of various perceptions of dignity and their implications. Among the elements affecting the creation of dignity in the visual domain one finds: an impressive or majestic appearance, noblesse, self-esteem, pride, self-confidence, inner strength, authoritativeness and charisma. At times, the respectful treatment of the figures is clear as soon as one views the work, but at others it is implied in the interpretation given to it. It is not this article's intention to discuss the different perceptions or expressions of human dignity, historically, theologically, politically, psychologically, nationally or socially, but to present them artistically as they are the innate right of people to be appreciated, treated with courtesy and met with ethical behavior. Results and conclusion. The research done in order to reach a conclusion confirming or contradicting the premises at its base, resulted in a mosaic of situations in which human dignity is reflected. The range of possibilities at the artists’ disposal for creating this value is diverse and includes: composition, use of color, mimesis, embellishment, disfigurement or distortion of reality. The pieces examined present: a military commander at the moment of his foe's surrender; an author monumentally impressive, man of vision and power and of grand dimensions, rising up from a stone plinth; a stern-faced, disfigured dwarf considered a ‘curiosity’, seated on the ground; a family of farmers living an impoverished and meager life; and a Jesus-like mysterious intellectual and Moses the prophet. All these together at first glance appear to be a strange collection of works, in which the common denominator is unclear or seemingly absent. However, this diverse group is included in the article because of the aura of dignity the figures command or convey. At times, the respectful treatment of the figures is clear as soon as one views the work, but at others it is implied in the interpretation given to it.

AbstractLow vitamin D level in HIV-positive persons has been associated with disease progression. We compared the levels of serum 25-hydroxyvitamin D (25(OH)D) in HIV-positive and HIV-negative persons, and investigated the role of nutritional supplementation and antiretroviral treatment (ART) on serum 25(OH)D levels. A randomised nutritional supplementation trial was conducted at Jimma University Specialized Hospital, Ethiopia. The trial compared 200 g/d of lipid-based nutrient supplement (LNS) with no supplementation during the first 3 months of ART. The supplement provided twice the recommended daily allowance of vitamin D (10 μg/200 g). The level of serum 25(OH)D before nutritional intervention and ART initiation was compared with serum 25(OH)D of HIV-negative individuals. A total of 348 HIV-positive and 100 HIV-negative persons were recruited. The median baseline serum 25(OH)D level was higher in HIV-positive than in HIV-negative persons (42·5 v. 35·3 nmol/l, P<0·001). In all, 282 HIV-positive persons with BMI>17 kg/m2 were randomised to either LNS supplementation (n 189) or no supplementation (n 93) during the first 3 months of ART. The supplemented group had a 4·1 (95 % CI 1·7, 6·4) nmol/l increase in serum 25(OH)D, whereas the non-supplemented group had a 10·8 (95 % CI 7·8, 13·9) nmol/l decrease in serum 25(OH)D level after 3 months of ART. Nutritional supplementation that contained vitamin D prevented a reduction in serum 25(OH)D levels in HIV-positive persons initiating ART. Vitamin D replenishment may be needed to prevent reduction in serum 25(OH)D levels during ART.

Introduction HIV viral suppression is important for effective treatment and for reducing new infections. In 2019, only 66% of persons on antiretroviral treatment (ART) in Jamaica were virally suppressed. We aim to compare time to viral suppression by ART initiation year and type of treatment site to understand the implications for programming. Methods We assessed time to viral suppression among 4560 persons who received viral load testing either pre or post ART initiation from 2017–2019. We used descriptive statistics and Kaplan-Meier estimates to compare survival curves by ART year (2017, 2018, 2019), sex and type of treatment site (public and non-governmental organizations). Persons were censored if suppression was not achieved. Mixed effects Cox regression was used to determine the effect of covariates on the likelihood of viral suppression. We report hazard ratios and 95% confidence intervals. Results Pre-ART viral load testing decreased from 36% in 2017 to 30% in 2019. For post-ART viral load tests, approximately 78% (n = 1589) of persons achieved suppression, 51% (n = 809) were female and 86% (n = 1341) used a public treatment site. The median time to suppression decreased by 3 months from 2017 to 2019. The likelihood of suppression was almost 2 times greater in 2018 (HR = 1.56, CI = 1.39–1.75) and 3 times greater in 2019 (HR = 3.17, CI = 2.76–3.64) compared to 2017. NGO treatment sites were also significantly associated with the likelihood of viral suppression compared to public sites. Conclusion Pre-ART viral load testing and the time to viral suppression decreased over three years. Initiating ART after 2017 and early use of NGO treatment sites were found to significantly increase the likelihood of achieving suppression. This demonstrates improvements in the national HIV response but there is need to increase the number of persons on ART and achieving viral suppression.

Over time, the societies’ relation towards people with disability has changed its course, due to the influence of scientific knowledge, as well as the influence of artistic representation of disability. Cinema is a globally present medium, so the question of its influence on the attitudes of society has emerged. In this paper, overall 20 movies in which the main character has a disability have been analyzed. The analysis shows the dominance of disabled male characters. The main characters often have motoric impairment, followed by mental disorders, and intellectual disability. Mental disorders are in analyzed movies portrayed with aggressive behavior (e. g. violence). The movies are mostly mainstream, and profit-driven. Implications are multiple: there is a need to construct inclusive cinematography – (sub)discipline within the media culture, which could adequately address the question of social stigma and social marginalization of people with disabilities, which is at this point globally present in current media (movies and series). Also, there is a need for the introduction of critical content in media culture within the education curriculums, as well as a need for promoting alternative cinema, as adequate media for promoting inclusive values.

Background Increasing HIV treatment coverage is crucial to reducing population-level HIV incidence. Methods The Botswana Combination Prevention Project (BCPP) was a community randomized trial examining the impact of multiple prevention interventions on population-level HIV incidence and was conducted from October 2013 through June 2017. Home and mobile campaigns offered HIV testing to all individuals ≥ age 16. All identified HIV-positive persons who were not on antiretroviral therapy (ART) were referred to treatment and tracked to determine linkage to care, ART status, retention in treatment, and viral suppression Results Of an estimated total of 14,270 people living with HIV (PLHIV) residing in the 15 intervention communities, BCPP identified 13,328 HIV-positive persons (93%). At study start, 10,703 (80%) of estimated PLHIV knew their status; 2,625 (20%) learned their status during BCPP, a 25% increase with the greatest increases occurring among men (37%) and youth (77%). At study start, 9,258 (65%) of estimated PLHIV were on ART. An additional 3,001 persons started ART through the study. By study end, 12,259 had initiated and were retained on ART, increasing coverage to 93%. A greater increase in ART coverage was achieved among men (40%) compared to women (29%). Of the 11,954 persons who had viral load (VL) test results, 11,687 (98%) were virally suppressed (HIV-1 RNA ≤400 copies/mL). Overall, 82% had documented VL suppression by study end. Conclusions Knowledge of HIV-positive status and ART coverage increased towards 95–95 targets with universal testing, linkage interventions, and ART. The increases in HIV testing and ART use among men and youth were essential to reaching these targets. Clinical trial number NCT01965470.

From rhetoric to jurisprudence, various disciplines have dealt with the practice of attributing human features and abilities to insensate objects. The agency of works of art acting as fictitious persons is not only recognized at the level of aesthetic experience, but also outside it, because there have been cases in which they were subject to legal liability. Personhood is not reducible to individual human beings. However, since works of art lack senses and consciousness, there is ultimately a limit to the personifying metaphor.

Objective: To examine trends in time spent viremic and initiation into antiretroviral treatment (ART) among persons newly diagnosed with HIV in San Francisco. Methods: Using HIV surveillance data, we included persons diagnosed with HIV during 2012–2020, a San Francisco resident at HIV diagnosis, alive 12 months after HIV diagnosis, and had ≥2 viral load tests within 12 months after diagnosis. Percent person-time spent (pPT) >200, pPT >1500, and pPT >10,000 copies per milliliter was calculated during the 12 months after HIV diagnosis. Multivariate regression models assessed the year of diagnosis and time spent above each viral threshold and year of diagnosis and ART initiation within 0–7 days (rapid), 8–365 days (delayed), or no ART initiation. Results: Of 2471 new HIV diagnoses in San Francisco from 2012 to 2020, 1921 (72%) were included. Newly diagnosed persons spent a mean of 40.4% pPT >200, 32.4% pPT >1,500%, and 23.4% pPT >10,000 copies per milliliter; 33.8% had rapid ART initiation, 57.3% delayed, and 9% had no ART initiation. After adjustment, persons diagnosed in years 2014–2015, 2016–2017, 2018–2019, and 2020 were associated with less time spent above all viral thresholds and lower risk of delayed or no ART initiation compared with those diagnosed in 2012–2013. Greater time above thresholds correlated with injection drug use, ages 25–29 and 30–39 years, and homelessness. Conclusions: Percent time spent above each viremic level decreased significantly, whereas rapid ART initiation increased among newly diagnosed persons from 2014 through 2020 compared with 2012–2013. Population differences in time spent unsuppressed highlight the need for targeted interventions to reduce new HIV infections and improve health.

Introduction The scale-up of Universal Test and Treat has resulted in reductions in HIV morbidity, mortality and incidence. However, healthcare system and personal challenges have impacted the levels of treatment coverage achieved. We implemented interventions to improve linkage to care, retention, viral load (VL) coverage and service delivery, and describe the HIV care cascade over the course of the Botswana Combination Prevention Project (BCPP) study. Methods BCPP was designed to evaluate the impact of prevention interventions on HIV incidence in 30 communities in Botswana. We followed a longitudinal cohort of newly identified and known HIV-positive persons not on antiretroviral therapy (ART) identified through community-based testing activities through BCPP and referred with appointments to local HIV clinics in 15 intervention communities. Those who did not keep the first or follow-up appointments were tracked and traced through phone and home contacts. Improvements to service delivery models in the intervention clinics were also implemented. Results A total of 3,657 newly identified or HIV-positive persons not on ART were identified and referred to their local HIV clinic; 90% (3,282/3,657) linked to care and of those, 93% (3,066/3,282) initiated treatment. Near the end of the study, 221 persons remained >90 days late for appointments or missing. Tracing efforts identified 54/3,066 (2%) persons who initiated treatment but died, and 106/3,066 (3%) persons were located and returned to treatment. At study end, 61/3,066 (2%) persons remained missing and were never reached. Overall, 2,951 (98%) persons living with HIV (PLHIV) who initiated treatment were still alive, retained in care and still receiving ART out of the 3,001 persons alive at the end of the study. Of those on ART, 2,854 (97%) had current VL results and 2,784 (98%) of those were virally suppressed at study end. Conclusions This study achieved high rates of linkage, treatment initiation, retention and VL coverage and suppression in a cohort of newly identified and known PLHIV not on ART. Tracking and tracing interventions effectively identified those persons who needed more resource intensive follow-up. The interventions implemented to improve service delivery and data quality may have also contributed to high linkage and retention rates. Clinical trial number: NCT01965470.

Background Aphasia is an acquired communication disorder resulting from a stroke or traumatic brain injury. Persons with Aphasia (PWA) have deficits in language comprehension and expression. Loss of communicative functions can be isolating for PWA as they may face barriers to accessing community-based leisure programs. Aphasia-friendly materials, visuals and simple written or oral instructions, can reduce such barriers and support communicative access. Objective The pARTicipate project is a student-initiated art group that was piloted with the aim of encouraging PWA to trial an art class in a communicatively accessible and low-risk environment, as part of an outpatient rehabilitation program. By using a life participation approach for aphasia, this project aimed to promote participants’ discovery, or rediscovery, of a hobby. Methods Four PWA completed six art projects over 9 weeks. Communicative access was enhanced by using aphasia-friendly instructions and demonstrations to teach the projects. Participants showcased their projects and taught their skills to family and community members on the final “Share Day”. Results Qualitative and quantitative data was gathered from satisfaction surveys. One hundred percent of the participants indicated that they enjoyed sharing their art with the community, and that they would join the group again if it was offered. Additionally, all participants indicated they would be likely to continue exploring art in the future. Conclusions It is expected that the skills learned by PWAs in pARTicipate may help increase their confidence about participating in other community-based art classes. As well, continuing to use these skills may help participants develop a sense of belonging to the artistic community. By learning to incorporate personal symbols, PWA can be deliberate in their use of art for self-expression in the future. Finally, PWA were empowered to assume a leadership role during “Share Day”. Having the opportunity to teach newly acquired artistic skills to their family and community members promoted positive feelings about the self and served to reveal their cometencies.

“Waiting to know” is a phenomenon uniquely experienced by contact persons—individuals exposed to persons with life-threatening illnesses, such as Ebola hemorrhagic fever (Ebola) but yet to exhibit signs and symptoms of the disease. Art forms representing these experiences can illuminate and enhance the appreciation of this phenomenon. Using thematic statements derived from written descriptions of experiences, selected art works such as painting and music, illustrate the experience of “waiting to know.” Through the art works’ describing thematic statements, encounters are appreciated anew, facilitating the understanding of the uncertainty of the moment. Understanding the meaningful expression of living the experience through these arts acknowledge the process of nursing as knowing persons as whole and complete in the moment, living their lives fully with hopes, dreams, and aspirations.

There is a growing interest in the role art museums might play in enriching the lives of persons with dementia. The literature has started incorporating the views of persons with dementia in the knowledge production, but in-depth explorations of their art experiences are still rare in the literature. This article adds to the research with a case study of a man with Alzheimer’s who regularly takes part in dementia-friendly guided tours at his local art museum. The article examines through a narrative analysis the role his visits to the art museum might play in the way he navigates life with Alzheimer’s. The authors argue that the art experiences are important cultural resources in the man’s effort to ‘hold his own’ faced with Alzheimer’s. The study is bound to a Norwegian context, but the art programme has similarities with related programmes at art museums in other countries.

In the Netherlands, art academies offering teacher training courses in visual arts and design pay little attention to diverse learners, such as pupils with learning disabilities, in their curricula. To form a picture of the existing perceptions of students concerning persons with intellectual and/or psychological disabilities, this study was set up to map the views of a group of first-year teacher training students of visual arts and design at the Amsterdam University of the Arts. The focus of the study was to see whether student perceptions changed after an active encounter with persons with intellectual and/or psychological disabilities ‐ in this case, a group of visiting artists with learning disabilities. The motivation for this study was influenced by the United Nations Convention on the Rights of Persons with Disabilities. As this mandate was adopted in the Netherlands in 2017, we have been intrigued by the consequences it would have for current educational settings and teacher training courses focusing on the subjects of art and design.

Persons with mental illness often have low self-esteem, a lack of self-expression, and poor social skills. This study used a quasi-experimental two-group pre-test-post-test design to investigate the effects of food art therapy on the self-esteem, self-expression, and social skills of persons with mental illness attending community rehabilitation facilities. The authors recruited persons with mental illness aged 18 years or older attending three community rehabilitation facilities. Participants in two rehabilitation facilities participated in food art therapy (experimental groups 1 and 2; n = 15 for each group), and participants in the third rehabilitation facility participated in regular programs of the facility (control group, n = 30). Participants in the experimental groups attended a total of eight sessions of food art therapy twice per week for four weeks. The Korean versions of the Rosenberg self-esteem scale, self-expression scale, and social skill rating system were administered at pre- and post-test in both the experimental and control groups. The self-esteem, self-expression, and social skills of the experimental group improved significantly compared to the control group. The findings suggested that food art therapy would be an excellent psychosocial intervention to help persons with mental illness to rehabilitate in the community.

The article examines the types of social services provided to homeless persons in accordance with the Law of Ukraine "On Social Services". On the basis of the analysis of legislative norms and scientific developments, the need to apply a comprehensive approach to the functioning of the system of providing services to homeless persons is substantiated, taking into account the state social standards, which determine the procedure for providing appropriate social services for persons who find themselves in difficult life circumstances. It was found that homeless persons are recipients of such social services as shelter, night shelter, supported accommodation of homeless persons, social integration and reintegration, and in-kind assistance. At the same time, the legislation of Ukraine provides for the possibility of applying to homeless persons such types of social services as information and social prevention, representation of interests, social support during employment and at the workplace, representation (mediation). Given the fact that the only social service within which emergency medical assistance is provided on the street is emergency (crisis) intervention, it is necessary to recognize homeless persons as its recipients. It is also proposed to supplement the list of social services for homeless people with social and psychological rehabilitation. Attention is focused on the need to harmonize the provisions of Art. 17 of the Law of Ukraine «On the Basics of Social Protection of Homeless Persons and Homeless Children» and Art. 16 of the Law of Ukraine «On Social Services» regarding the types of social services provided to homeless persons. It has been proven that temporary shelter, social, medical, legal, educational, and rehabilitation services for homeless persons in the sense of the provisions of the Law of Ukraine «On Social Services» are not provided for. Therefore, it is suggested that the provisions of Art. 17 of the Law of Ukraine «On the Basics of Social Protection of Homeless Persons and Homeless Children» in the following wording: «Homeless persons have the right to social services and rehabilitation in the order and under the conditions determined by the legislation of Ukraine.»

Guidelines, standards and protocols are now widespread in clinical oncology. Although based on randomized trial evidence where possible, most are based on professional consensus. However, the goal of incorporating patient and carer views meaningfully in such guidelines has proved elusive. We performed a qualitative, focus group study of patients with head and neck cancer, their carers and bereaved carers in the South and West of England. Patients and carers were asked to discuss their views on a series of professionally-derived standards for care. Pilot work showed that convening patient groups twice was more productive, allowing the first to discuss ‘the patient story/journey’ and the second to be more directed at specific standards, suitably translated into lay language. The results demonstrated that such methodology was applicable even to groups of patients traditionally viewed as ‘difficult’, such as laryngectomy patients, facially disfigured persons and bereaved carers. Participants were able to comment meaningfully on the standards and the process of head and neck cancer care as a whole. National (BAO-HNS) standards were modified with the results of the study. Focus groups, especially reconvened groups, are an effective way of gaining patient and carer views of professionally-derived standards in oncology. However, professional qualitative researcher help is required, and it is not necessarily ‘easy’ or cheap.

Abstract Background We describe the timing of human immunodeficiency virus (HIV) diagnosis and antiretroviral treatment (ART) initiation after implementation of universal testing and treatment policies in New York City (NYC). Methods Using NYC population-based HIV registry data for persons with HIV diagnosed from 2012 through 2015 and followed up through June 2017, we examined trends in the proportion with diagnosis soon after HIV infection (ie, with CD4 cell count ≥500/μL or with acute HIV infection) and used Kaplan-Meier plots and proportional hazards regression to examine the timing of ART initiation after diagnosis. Results Among 9987 NYC residents with HIV diagnosed from 2012 to 2015, diagnosis was early in 35%, and 87% started ART by June 2017. The annual proportion of persons with early diagnosis did not increase appreciably (35% in 2012 vs 37% in 2015; P = .08). By 6 months after diagnosis, 62%, 67%, 72% and 77% of persons with HIV diagnosed in 2012, 2013, 2014, or 2015, respectively, had started ART, with median (interquartile range) times to ART initiation of 3.34 (1.34–12.75), 2.62 (1.28–10.13), 2.16 (1.15–7.11), and 2.03 (1.11–5.61) months, respectively. Conclusions Although recommendations for ART initiation on diagnosis are increasingly being implemented, the findings of the current study suggest that immediate treatment initiation is not universal. Continued efforts are needed to expand and better target HIV testing to promote earlier diagnosis.

Latest tendencies in museum accessibility, both in the world and in Latvia, show that the social status of museums is changing from a cultural institution for a narrow circle of interested persons to a meeting place open to society, accessible both intellectually and physically to different social groups. The research aim is to study the opportunities for the use of graphic means to disclose and give access to the collection of artistic property of Latgale Culture and History Museum (LCHM) to persons with vision impairment. With the information summarised, the authors invite to use the opportunities of graphic design and technologies in the creation of art reproductions, making the graphic image of the painting perceptible by touch, i.e. tactile, and thus accessible to persons with visual impairment. The novelty of the research on the scale of Latvia will be a supplement of tactile images to the album of the collection of art reproductions of LCHM, which includes tactile graphic images and corresponding descriptions in Braille for persons with vision impairment. At this stage of the research, empirical methods were used – interview and practical activity.The aim of the research: Study and research graphic design opportunities for the integrated approach in museums in the reproduction of existing art property to ensure its accessibility to persons with vision impairment, analyse the needs of the visually impaired in cooperation with societies and organisations in Latvia and design a scheme of practical activities for persons with vision impairment.

The article is devoted to the procedural determination of explanations of persons and peculiarities of obtaining explanations of persons in the investigation of criminal offenses. Changes to Part 8. Art. 95, part 3 Art. 214 of the Criminal Procedure Code of Ukraine (hereinafter – CPC) and new Art. 298-1 CPC are analyzed. It is stated that the explanations selected in this category of the case can be considered as evidence even if they were received before the data entered in the EDDR and can be selected by all the subjects listed in item 19 of Art. 3 CPC, including defender and operational units. It is possible to select explanations from the persons listed in item 25 of Art. 3 of the CPC of Ukraine, as participants in criminal proceedings. Two discussion questions are raised. The first is how the provisions of Part 1 of Art. 63 of the Constitution of Ukraine correlates with the rights and duties of participants in criminal proceedings. Having analyzed the content of item 8 of Art. 95 of the CPC, we state that the explanation can be obtained only with the consent of the person. Thus, the Constitution of Ukraine states that a person may refuse to give explanations in relation to himself, family members, close relatives, but nothing is written about the obligation to give explanations in other cases. Part 2 of Art. 66 of the CPC does not oblige a witness to give explanations and establishes responsibility (, Art. 67 of the CPC, Art. 385 of the Criminal Code of Ukraine for refusing to give testimony, but no explanations, paragraph 4 of Part 3 of Article 72-1 of the Code of Criminal Procedure obliges the representative of the probation authority to give explanations in court, paragraph 9 of Part 1of Article 56 of the CPC provides the victim with the right to give explanations. From July 1, 2020, problems in law enforcement may arise regarding the explanation given by witnesses in the event that they did not give consent to receive them. This will be relevant in the case of obtaining explanations from witnesses in the case who do not wish to give an explanation at all, and not only in cases where it concerns the witness’s person or persons close to him. In fact, they are not responsible for these actions before the law. The number of such persons may be considerable. Therefore, it is necessary to establish at the legislative level the responsibility for refusing to give a witness an explanation. But, it is quite clear that, before questioning such persons, they need to clarify the requirements of Art. 63 of the Constitution of Ukraine. The second, in which procedural form it is necessary to issue explanations. From 1 July 2020, practitioners must have effective means of fixing explanations, since, without the proper procedural form, information about the facts contained in the explanations cannot be recognized as credible evidence. The possibility of applying Articles 103, 104 of the CPC of Ukraine by analogy is established. Namely, that the results of a procedural action – a survey – should be recorded in the protocol of the corresponding action. Abstract Background Detectable serum or plasma cryptococcal antigen (CrAg) precedes symptomatic cryptococcal meningitis. The World Health Organization recommends CrAg screening for human immunodeficiency virus–positive persons with CD4 count &lt;100 cells/μL initiating antiretroviral therapy (ART). However, an increasing proportion of patients with cryptococcosis are now ART experienced. Whether CrAg screening is cost-effective in those with virologic failure is unknown. Methods We retrospectively performed nationwide plasma CrAg testing among ART-experienced Ugandan adults with virologic failure (≥1000 copies/mL) using leftover plasma after viral load testing during September 2017–January 2018. For those who were CrAg positive, we obtained ART history, meningitis occurrence, and 6-month survival via medical records review. Results Among 1186 subjects with virologic failure, 35 (3.0%) were CrAg positive with median ART duration of 41 months (interquartile range, 10–84 months). Among 25 subjects with 6-month outcomes, 16 (64%) survived, 7 (28%) died, and 2 (8%) were lost. One survivor had suffered cryptococcal meningitis 2 years prior. Two others developed cryptococcal meningitis and survived. Five survivors were known to have received fluconazole. Thus, meningitis-free survival at 6 months was 61% (14/23). Overall, 91% (32/35) of CrAg-positive persons had viral load ≥5000 copies/mL compared with 64% (735/1151) of CrAg-negative persons (odds ratio, 6.0 [95% confidence interval, 1.8–19.8]; P = .001). CrAg prevalence was 4.2% (32/768) among those with viral loads ≥5000 copies/mL and 0.7% (3/419) among those with viral loads &lt;5000 copies/mL. Conclusions In addition to the CD4 threshold of &lt;100 cells/μL, reflexive CrAg screening should be considered in persons failing ART in Uganda with viral loads ≥5000 copies/mL.

Face transplantation (FT) is a landmark in reconstructive surgery involving vascularised composite allotransplantation. A recent issue of FT for patients who are blind has arisen. Some bioethicists recommend not excluding a patient who is blind, as this may amount to discrimination. From an ethical standpoint, FT for those with blindness is appropriate in selected candidates. This article seeks to add to the clinical evidence supporting FT for those with blindness by detailing a complementary psychosocial perspective. Currently, there is little relevant research about the subjectivity of the blind. This is critical since the arguments against FT for the blind refer to their inability to see their face and to view the reaction of others to their disfigured faces. We begin with a brief look at examples of FT involving blindness and associated arguments. The next part is a multidisciplinary investigation of the experiences of the blind. These are gleaned from a close reading of the literature and drawing inferences, as direct studies are rare. The discussion analyses identity themes of the blind in relation to their faces: as they experience it; the face they wish to show to the world; and how others perceive and react to their face in a saturated environment of imagery and visual communication. Disability and the blind person’s experience of faces are well-founded considerations for medical practitioners and ethics boards in the process of FT decision-making.

Acquisition of rights under the name of another person is, as a rule, prohibited. Meanwhile, under public law rules the anonymous activity is recongnised more widely. So the rule of the Civil Code needs correction. Virtual digital person may be an image (a copy) of a certain person (traditional natural or legal person) or a fictitious one. Сopyright law knows such a notion as the character in a work of art, literature, etc. The latter and the notion of virtual digital person partly coincide. Quite often virtual digital persons are created and used in the Internet by persons not possessing the image which is reproduced. Therefore, the problem of defending the right to personal image in the WWW arises. It requires amendments in the regulation of personal non-property rights in Russia.

People with HIV (PWH) can now enjoy longer, healthier lives due to safe and highly effective antiretroviral therapy (ART), and improved care and prevention strategies. New drug formulations such as long-acting injectables (LAI) may overcome some limitations and issues with oral antiretroviral therapy and strengthen medication adherence. However, challenges and questions remain regarding their use in aging populations. Here, we review unique considerations for LAI-ART for the treatment of HIV in older PWH, including benefits, risks, pharmacological considerations, implementation challenges, knowledge gaps, and identify factors that may facilitate uptake of LA-ART in this population.

Background:Antiretroviral therapy (ART) is the cornerstone of HIV clinical care and is increasingly recognized as a key component of HIV prevention. However, the benefits of ART can be realized only if HIV-infected persons maintain high levels of adherence.Methods:We present interview data (collected from June 2007 through September 2008) from a national HIV surveillance system in the United States—the Medical Monitoring Project (MMP)—to describe persons taking ART. We used multivariate logistic regression to assess behavioral, sociodemographic, and medication regimen factors associated with three measures that capture different dimensions of nonadherence to ART: dose, schedule, and instruction.Results:The use of ART among HIV-infected adults in care was high (85%), but adherence to ART was suboptimal and varied across the three measures of nonadherence. Of MMP participants currently taking ART, the following reported nonadherence during the past 48 hours: 13% to dose, 27% to schedule, and 30% to instruction. The determinants of the three measures also varied, although younger age and binge drinking were associated with all aspects of nonadherence.Conclusion:Our results support the measurement of multiple dimensions of medication-taking behavior in order to avoid overestimating adherence to ART.

The study investigated the relationship between the Jungian psychological functions Sensing (S) and Intuition (N) and preference for art. It was hypothesized that S-persons would prefer realistic and representational art, where N-persons would like abstract art. The Myers-Briggs Type Indicator was administered to 179 subjects to assess the preference for either the S- or N-function. Subjects indicated how much they liked 20 painted portraits. The hypothesis was partly confirmed. Not only S-subjects ( n = 100) but also N-subjects ( n = 79) prefer realistic paintings over abstract ones. On the whole, abstract paintings are valued less than realistic paintings, but N-subjects like abstract paintings more than S-subjects.

Abstract The reconstitution of Mycobacterium tuberculosis antigen-specific CD4 T cells in a cohort of HIV-infected persons starting antiretroviral treatment (ART) in a high tuberculosis endemic area is described. Restoration of the antigen-specific CD4 T-cell subsets mirrored the overall CD4 T-cell compartment. Activation (assessed by HLA-DR expression) decreased during ART but remained elevated compared to HIV-uninfected persons. Despite known M. tuberculosis sensitization determined by interferon-γ release assay, 12/23 participants had no M. tuberculosis-specific CD4 T cells detectable by flow cytometry, combined with overall elevated T-cell activation and memory differentiation, suggesting heightened turnover. Our data suggest early ART initiation to maintain polyfunctional immune memory responses.

ObjectivesTo compare risky sexual behaviours between HIV-positive persons initiated on antiretroviral therapy (ART) (ART-experienced) and persons waiting to start on ART (ART-naive) and assess predictors of risky sexual behaviours among HIV-infected patients in rural Rakai district, Uganda.Study designThis is a cross-sectional study that used data from the Rakai Community Cohort Study (RCCS) database between 2013 and 2014. A structured questionnaire was used for data collection. We used stepwise logistic regression as an index to estimate the adjusted ORs for the association between risky sexual behaviours and ART treatment status.Study settingThis study was conducted in Rakai district, located in south-western Uganda. The data for this study were extracted from the RCCS. RCCS is an open prospective cohort of approximately 15 000 consenting participants aged 15–49 years.ParticipantsHIV-positive participants aged 18–49 years who had sex at least once a month with any partner prior to the start of the study.Main outcome measuresInconsistent/no condom use in the last 12 months, alcohol use at last sexual encounter, and two or more sexual partners.ResultsART-naive participants were more likely to report inconsistent condom use (OR=1.74, 95% CI 1.11 to 2.73) and more likely to drink alcohol at last sexual encounter (OR=1.65, 95% CI 1.11 to 2.46), compared with ART-experienced patients. ART treatment status (p<0.001) was a significant predictor of risky sexual behaviours. Both marital status (p=0.016) and occupation level (p=0.009) were positively associated with inconsistent condom use, while sex (p<0.001) correlated with alcohol use at last sexual encounter.ConclusionART-naive participants were more likely to exhibit risky sexual behaviours than the ART-experienced participants. The intensity of risk reduction counselling should be increased for HIV-positive persons waiting to start ART but already in HIV care.
