<p><strong>Background:</strong> The cause of delay in obtaining a dental care definitely depends on the individual’s anxiety status at a dental set up. Anxiety is a state of delirium experienced by an individual. Nevertheless, this feeling of discomfort may hamper the quality of life of an individual.</p><p><strong>Objectives:</strong>To assess the degree of dental anxiety and to assess the reason for dental anxiety</p><p><strong>Methods:</strong> A total of 950 participants were enrolled into the present study. Data was collected using a questionnaire. The questionnaire consisted of two parts; the first part composed of items on demographic characteristics of the patient, and the second part included a questionnaire consisting of combination of Corah’s Dental Anxiety Scale and Dental Fear Survey.</p><p><strong>Results:</strong> In a total sample of 872 patients, 538 individuals were dentally anxious (mean DAS &gt;9) of which 495 patients had a mean score of 9-12 (Mildly anxious); 30 patients had a mean score of 13-15 (Moderately anxious). Only a few subjects (13) were highly anxious (mean DAS &gt; 15). When individual situation of anxiety was assessed, the population noted highest anxiety on feeling or seeing the anaesthetic needle followed by feeling of the vibrations of the drill and being seated in a dental chair.</p><p><strong>Conclusion:</strong> As dental anxiety is a continuum, where to place the cut-off point is a crucial issue in research, which aims to compare</p><p>subjects fearful of dentistry with those who are not. The validity of many measures has not been adequately assessed, perhaps because of the difficulty of identifying an external referent against which it may be tested. Evidence in the form of behavioral, physiological or</p><p>cognitive characteristics should be considered to manage these dentally anxious individuals effectively in the dental office.</p>

Health care resources are often insufficient to meet the demands placed upon them. Managing scarcity and assessing the merits of competing priorities are therefore key responsibilities for health care decision-makers. In April 2006, the new General Dental services (nGDS) contract introduced local commissioning to NHS dentistry for the first time. Primary Care Trusts (PCTs) were charged with managing devolved and finite resources, but they have since drawn criticism as a consequence of variable quality commissioning. Economics may offer some solutions to assist NHS commissioners. Programme Budgeting and Marginal Analysis (PBMA) is a pragmatic economics-based approach which may inform and assist decision-makers to prioritise scarce resources. The research was divided into three phases: a national qualitative survey of NHS decision-makers' roles in managing scarce dental resources; a PCT -based qualitative case-study involving patients, dentists and PCT staff; and a PBMA- based dental priority setting exercise. Phases 1, 2 and 3 collectively used semi- structured interviews, focus groups, an action research approach and mixed methods. The research highlighted that dental decision-makers were constrained in their ability to commission dental services as a consequence of restrictions imposed by the national dental contract. Local priority setting and resource allocation generally lacked transparency and decision-makers typically failed to include patients and clinicians in these processes. With respect to health economics, stakeholders found the pictorial representation of the cost-value ratio (CVR) a useful starting point for further deliberation, but the full application of a traditional PBMA exercise was hindered locally by organisational change and severe PCT staffing pressures. The concerns identified by stakeholders are timely considering the Government's proposals to abolish PCTs and commission NHS dental services through the NHS Commissioning Board alongside a new national dental contract in 2014.

The purpose of this study was to determine the awareness and attitudes of dental hygienists in Ohio regarding the Advanced Dental Hygiene Practitioner (ADHP) as an occupation proposed by the American Dental Hygienists’ Association and to determine whether the ADHP could be a viable career option for Ohio dental hygienists. I developed a survey to assess dental hygienists’ awareness, attitudes, and their views regarding the new mid-level dental provider. The study was limited to licensed dental hygienists in Ohio. The study included a simple random sample of 400 of the 4100 dental hygienists in Ohio obtained by a systematic approach, a random start and selection of subjects at a constant interval. The method of data collection was electronic surveys. Fifty-four dental hygienists (13% of the sample) participated in the study. The results show that most of those responding were aware of the ADHP. Also, their attitude regarding the new mid-level dental provider was positive. Attitudes varied more when the respondents provided data on the adoption of legislation for the new provider. There was no consensus among respondents regarding the ADHP as a viable career option. More robust research is needed on the viability of the ADHP in Ohio.

Background: More than 10% of UK adults suffer extreme dental anxiety. Effective verbal and non-verbal communication (VNVC) in the dental dyad following handover of a completed Modified Dental Anxiety Scale (MDAS) had been hypothesised to reduce dental state anxiety. This study aims to consider whether: (1) ‘MDAS completion and handover effects long term dental trait anxiety, and (2) ‘what VNVC elements influence patient anxiety reduction?’ Trial Design: Mixed methods: (i) a randomised cross-over study and (ii) an observational study. Methods: Six NHS Highland Salaried Dental Practices were randomised to start either with the experimental arm, where dentally anxious patients (MDAS score = 19 or 5 in any one question) complete and handover MDAS to the dentist, or the control arm where no handover occurred. Within the observational study, all participants had their dental treatment appointment videoed. The primary outcome was dental trait anxiety as measured by MDAS, pre-treatment and at 3 month follow-up. Secondary outcome was dental state anxiety, measured pre and post treatment using both the Spielberger State-Trait Anxiety Inventory (STAI-S) and Heart rate.The observational study analysed the first 2 minutes of video and related this data to the behavioural and affective outcomes. Results: 54 patients agreed to take part in the study with 47 completing 3 month follow-up. The randomised cross-over study found no difference in state or trait dental anxiety however observational outcomes showed dental state anxiety remained high when handover was blocked. A significant lack in decline in dental trait anxiety was shown when dentists blocked patient expressed cues and concerns; this occurred irrespective of patient gender, age or number of appointments received in a 3 month period (F[155.06]=7.51, P=0.009). Conclusions: VNVC flow is fundamental to dyadic balance even within the first two minutes of the patient-dentist interaction. The balanced dyad is key to dental anxiety reduction.

Many physical changes occur as one ages, including changes associated with the oral cavity. A review of the literature suggests that the provision of dental care to institutionalized elderly patients presents problems due to a variety of factors. The purpose of this study is to assess the level or dental care provided to residents of long-term care facilities located in Warren County, Kentucky. In addition, secondary objectives Include the ascertainment of who provides dental care to residents and the amount or in-service dental training made available to staff members of the facility. Each administrator of the long term care facilities located in Warren County participated in an *interview conducted by the author. During the interview, information was obtained for a 21 item questionnaire concerning the facility, the number and age range or the residents, and types of dental services provided within the facility. Results obtained from the questionnaire indicated that 77 percent or the residents in long-tern care facilities in Warren County are 70 years of age or older. No significant differences were noted in the types or dental services provided to residents. However, the dental services provided ranged from those that were obtained in a private dental office via transportation or the resident to outside dental facilities to routine oral hygiene measures carried out by staff members employed by the facility. The findings revealed significant differences in the dental status of the MRDD residents when compared to the nursing home residents. Other findings indicated that none of the long-term care facilities had dental operatories or dental radiographic equipment on the premises. Additional research would be required in order to address uncertainties discovered in the study. A followup to the questionnaire Interview with the consulting dentists may be included to determine to what capacity and to what extent they are utilized by the facilities. Other recommendations include the utilization of entrance dental examinations to determine if services offered do meet the needs of the residents and periodic dental examinations to aid in detection and thus reduce the prevalence of dental diseases in this population.

Managed care has become a prominent mechanism for insuring dental care. Empirical research suggests that managed dental plans provide lower quality care to patients. However, few studies have specifically addressed the effects of managed care on the quality of dental hygiene care. Thus, in this study the researcher examines whether dental hygienists deliver a lower level of treatment to managed care patients than to those who are not subject to managed care. Questionnaire data were gathered from 193 members of the American Dental Hygienists' Association residing in the Chicago area. The primary independent variable, managed care, was measured with an item that asked the respondents to indicate the percentage of patients they treat that are insured by a managed dental plan. The questionnaire also contained items that measured the frequency in which the respondents perform 23 tasks that are indicators of quality of dental hygiene care. Principal components factor analysis of these 23 items yielded the study's two dependent variables: periodontal procedures and appointment time. Regression analysis of the data revealed a significant negative relationship between managed care and appointment time. This relationship may be attributable to an economic incentive on the part of dentist-employers who control the amount of time scheduled for dental hygienists' patients. Dentist-employers may reduce the time available for managed care patients in order to allow longer appointments for more profitable fee-for-service patients. The study results did not support the notion that managed care affects the extent to which dental hygienists perform periodontal procedures. These mixed results suggest that future research should examine the relationships between managed care and other aspects of quality of dental hygiene care not addressed in the current study.

Essay 1: Individuals with higher socioeconomic status (SES) also tend to enjoy better health. Evidence from the economics literature suggests that a potential mechanism behind this “social health gradient” is that human capabilities, that form SES, also facilitate health-promoting behaviors. This essay empirically investigates the significance of socioeconomic differences in health behaviors, using dental care consumption as an operationalization of health investments. I focus on adults at an age where lifetime trajectories for SES can be taken as given and use lifetime income to capture SES. I estimate the impact of lifetime income on dental care consumption and find robust evidence that the social gradient in dental care consumption steepens dramatically over the life-cycle. Considering that dental care consumption only reflects a small part of individuals' health investments the results suggest that lifetime effects of SES on health behaviors could be substantial in other dimensions. Essay 2: This essay studies the effect of competition on prices on a health care market where prices are market determined, namely the Swedish market for dental care. The empirical strategy exploits that the effect of competition differs across services, depending on the characteristics of the service. Price competition is theoretically more intense for services such as examinations and diagnostics (first-stage services), compared to more complicated and unusual treatments (follow-on services). By exploiting this difference, I identify a relative effect of competition on prices. The results suggest small but statistically significant negative short-term effects on prices for first-stage services relative to follow-on services. The results provide evidence that price-setting among dental care clinics responds to changes in the market environment and substantial effects of competition on prices over time cannot be ruled out. Essay 3: The Swedish dental care insurance subsidizes dental care costs above a threshold and becomes more generous as dental care consumption increases. On average, higher-income individuals consume more dental care and have better oral health than low-income individuals. Therefore, the redistributional effects of the Swedish dental care insurance are ambiguous a priori. I find that the dental care insurance adds to the progressive redistribution taking place through other parts of the Swedish social insurance (SI) for individuals aged 35-59 years whereas it reduces the progressivity in the SI for those aged 60-89 years. While the result for the oldest individuals is problematic from an equity point of view, the insurance seems to strengthen the progressitivy of the Swedish social insurance for the vast majority of patients.

Streszczenie:
The article provides information on the results of studying the materials of 150 commission of forensic medical examinations on the facts of patient dissatisfaction with the provision of medical care. The relevance of the problem of the legal relationship between a doctor and a patient is revealed and shown. The lack of information on these issues leads to a high probability of disputed situations in the providing dental care, so there is an urgent need for an integrated approach to the implementation of legally defined rights of patients. Media coverage of the above-mentioned problems plays an important role in improving the level of legal competence of patients. At the same time, the direct relationship between the doctor and the patient is the most important mechanism for implementing the patient's rights at the dental appointment and preventing conflicts. Behavior of doctors in such cases should be strictly regulated by normative legal acts. The process of information sharing with patients and transfer of information to the patient's relatives should receive in medical preventive institution specific legal basis under sections 30, 31, 48, 61 “Principles of legislation of the Russian Federation about health protection of citizens”, to be fixed in job descriptions with the designation of responsibility.

In this paper we address the question whether (i) the Dutch dental research portfolio reflects the dental care demand, and whether (ii) the results of this research does reach the dental care professionals. In order to answer these questions, we analyzed the content of the Dutch Journal of Dentistry (NTvT), a Dutch language professional journal which explicitly aims at disseminating useful knowledge to dental professionals. The research topics addressed in the journal were compared with (i) dental publications written by authors with a Dutch affiliation in international journals and with (ii) expenditures in the various types of oral healthcare. The analysis shows topical change over time, with more emphasis in NTvT on topics as social dentistry, and less attention for basic science topics. At the same time, the Dutch dental research portfolio (reflected by publications in international journals) does not reflect that trend. In addition, it appears that the largest domains of care with the highest share of oral healthcare expenditures (e.g. cariology and prevention) have the lowest attention in research. This applies to both international publications, as to the research disseminated through the professional journal NTvT.
