Due to lack of organs in relation to the need, waiting is increased which causes anxiety and fear for patients. When a person is placed on the waiting list, an organ transplant is the last resort since the organ is highly damaged. Aim: The aim was to illustrate the patient’s experience of waiting for an organ transplant. Method: This study is a literature study and have an inductive approach and is based on eight articles. The data was analyzed and categorized in regard to the aim. Results: The result reports three categories and associated subcategories; Feelings about waiting for an organ transplant, The need for support and information and A limited life. Patients experience uncertainty and anxiety during the waiting time and a fear of death alongside hope. The uncertainty grew with lack of information from care givers. Limitations of daily life perceived tough hence the nursing and family support became paramount, all together creating strategies to inspire hope. Conclusion: The literature study shows that the waiting experience for a transplant is similar around the world. There is a need for information and support from nurses, therefore, the nursing sympathy, information and knowledge is essential in creating a safe environment.

A prerequisite for a successful transplantation program is of course that the demand for organs gets saturated by a steady supply of the same. This is unfortunately not the case as the gap between supply and demand is increasing, hence prolonging waiting times and negatively effecting the patients’ health and overall prognosis. Aim: Portrait the experiences of patients waiting for an organ transplant as well as conducting a methodological examination of the data collection method used in a selection of articles. Method: A descriptive literature that has examined 13 articles, the majority of articles were qualitative design. Results: Patients' experiences when waiting for an organ transplant is often associated with feelings such as anxiety and depression. Patients experiencing the waiting time to get an organ transplant as long and the experience is that the patient’s life is paused. Experiences which patients describes is that support from family and healthcare professionals are of great importance. Even to meet with other patients waiting for an organ transplant was considered as a support for patients. Patients described that experience of getting information from medical staff about his health situation was important for the patients. The articles selected data collection method consisted of 11 qualitative interview study. Two of the articles had quantitative design where surveys have been used. Conclusion: Patients waiting to undergo an organ transplant often have experience of waiting times are long and with time these patients are anxious and fearful. The patients experienced the support from medical staff is important to the care process and that nurses therefore have an important responsibility to adapt care to individual patient needs.

Like patients next of kins are vulnerable and it is important to explore next of kins’ experiences, this to better prepare for the burden of transplantation. The aim of the study was to explore the experience of being a next of kin in connection with heart- or lung transplantation. The study was conducted as a general literature study and the analysis was guided by a hermeneutic method. The results show that next of kin’s support for the patient is an act of love resulting in sacrificed relationships with friends and changes in their daily lives. The transplantation generated waiting and a lot of feelings for next of kins, such as joy when the call came about transplant, and later on after the transplantation worries that the patient’s condition would worsen again. Furthermore, next of kins experienced lack of availability from the health care services regarding information, attention and support in their role. In the literature the nursing process regarding next of kins’ experiences in heart- or lung transplantation is inadequately described, therefore it is of importance that future research creates new evidence based knowledge.

The disease picture is complex and the treatment of organ failure eventually becomes an organ transplant. The transplant process is long and consists of treatments and screening that affect the body both physically and mentally. However, the demand of organs is bigger than the supply, which means waiting indefinitely for the patient. Aim: To describe patients' experiences of waiting for an organ transplant. Method: Literature-based study based on eleven scientific articles with a qualitative approach and one article with a quantitative approach and one with a mixed method. Results: The analysis revealed three categories; Life feels constantly insecure, enduring struggle, support from close networks with nine sub-categories. Discussion: Life while waiting for an organ transplant involves massive suffering and limitations in everyday life. The limitations of everyday life create a need of support from the patient's environment, including the nurse. In order for the nurse to be able to support the patient during the transplant process, it’s of big importance to understand how patients experience everyday life while waiting for an organ transplant.

Pełny tekst źródłaBackground The proliferation signal inhibitors (PSIs) sirolimus (SRL) and everolimus (ERL) are the potent immunosuppressive drugs using in organ transplantation and has been used successfully in renal transplant recipients (RTX) as well. PSIs are the key factors to overcome the allograft rejections after successful organ transplantation since the immune system starts to react against the graft. SRL and ERL prevents the action of immune system b inhibits the proliferation of T- and B-cells by inhibiting the intracellular signaling of interleukin-2. The presence of excess amount of serum proteins including albumin in the urine is considered as proteinuria, which reflects the loss of kidney function. The occurrence of proteinuria can be the result of abnormal glomerular filtration and/or impaired tubular endocytic function of renal proximal tubular epithelial cells (PTECs). Megalin and cubulin are two scavenger receptors present on epical surface of PTECs and involved in reabsorption of proteins after glomerular ultrafiltration process in the kidney. Proteinuria appears too high in renal transplanted patients during ongoing treatment with PSIs.
Aim Our study aimed to investigate and correlate the expression level of megalin and cubilin and albumin uptake in PTEC of renal transplanted patients before and after conversion to PSI.
Methods To retrieve the maximal expression of our interest molecules in renal PTECs, we optimized antigen retrieval (AR) method and primary antibody dilution for each molecule separately. An optimization experiment was performed on 3 different normal patients renal biopsies were used. Later, human renal biopsy specimens originated from 4 different renal transplanted patients were used in this study. From all the 4 patients biopsy specimens were taken before and ongoing administration of PSIs (SRL, ERL). The expression of megalin, cubilin and albumin uptake in PTEC of renal transplant patients was determined by immunohistochemical staining.
Results Based on the optimization experiments, we selected the AR method and primary antibody dilution for the expression of megalin, cubilin and albumin uptake. In 4 renal transplanted patients following administration of PSIs results in patients 1, 2, 3 expression of megalin, cubilin and albumin uptake during ongoing PSI treatment was not comparable or even more intense than before PSIs introduction. The expression of megalin, cubilin and albumin uptake was reduced in patient 4 during ongoing PSI treatment.
Conclusion Our findings suggest that the renal transplant patient 4 developed proteinuria during PSI medication. The expression of megalin, cubilin and albumin uptake was markedly decreased during ongoing PSI treatment in patient 4. We concluded that there is a direct link between PSI medication and tubular dysfunction, which might cause proteinuria

Pełny tekst źródłaThe aim of this study was to describe family members’ experience of life during the acute phase of allogenic stem cell transplantation. Seven family members of patients at, “Centrum for Allogenic Stem cell transplantation” Karolinska University Hospital in Huddinge participated. The method used was semistruktured interviews that allowed the respondents to tell about their living situation. These interviews were then analyzed through qualitative content analysis. The findings were a theme named; this present time consisting of three categories: living-pieces, family and health. There were also ten subcategories all together. The result was that when family members gives the opportunity to, with their own words, describe and tell about their living situation they tell about the present time and how they focus and mobilise all their strength and energy on their sick family member to get well. The conclusion of this study was that further research and development of interventions, for family members, are necessary and to focus on the family as a unite is an absolute condition in order to maintain health and prevent physical- and mental illness for the whole family. This ought to be beneficial not only for the family but also for the patients’ recovery.

Pełny tekst źródłaIn this study the data of 90 consecutive patients with multiple myeloma were analyzed retrospectively. There is a correlation between a good response after the induction therapy and sixty days after high-dose chemotherapy with autologous stem cell transplantation. There is no correlation between organ damages (CRAB) and the progression or the response of the high-dose chemotherapy with autologous stem cell transplantation. The chronological age is not as important as the general condition and medical history of the patient to expect the toxicity of the high-dose chemotherapy. The two main factors that influenced the process of high dose chemotherapy followed by peripheral stem cell transplantation were the duration of G-CSF therapy and the number of transplanted stem cells. The different duration of G-CSF leads only to a faster recovery of leukocytes with no relevant effect on the investigated clinical parameters temperature, duration of intravenous antibiotic days and the extent of mucositis. The increase of transplanted stem cells results in a significant faster recovery of platelets and leukocytes, a decrease in frequency of transfusion of red blood cells and a reduced use of intravenous antibiotics. In summary, the G-CSF administration from the 7th day after transplantation is sufficient for high-dose therapy, a longer administration provides no relevant clinical advantage. One should take care for having got sufficient quantity of transplanted stem cells

Pełny tekst źródłaIschemia and reperfusion leads to damage at transplantation of organs due to formation of oxygen radical species. This ischemia reperfusion damage seems to be involved in the generation of chronic rejection. To evaluate the correlation between different ischemic and reperfusion periods and the behaviour of the antioxidative enzyms and the glutathion content and the lipidperoxidation status this study was done with heterotopic transplantation of rat hearts. The results show long cold ischemia leading to a significant raise of activity of the antioxidative enzymes superoxid dismutase, catalase, glutathion peroxidase and glutathion reductase together with reduction of the glutathion redox ratio (meaning a shift from reduced to oxidized glutathion) all together supporting the thesis of extensive oxidative stress in the ischemic myocard. With beginning of reperfusion the enzyme activities alternate with variing times of ischemia and reperfusion. A dramatic loss of reduced and oxidized glutathion together with a loss of the acitvity of the glutathion reductase was seen. This alterations point out a massive myocardial stress revealed in the formation of lipid peroxidation products and so direct cell damage after long ischemic periods. In conclusion long ischemic time with following reperfusion induces oxidative stress, which is compensated in short term examinations by protection mechanisms of the myocard as the regulation of antioxidative enzymes and the exclucion of oxidized glutathion. There was no graft failure but increased levels of lipid hydroperoxides suggest that perhaps initial damage of the heart leads earlier to apperent transplant vasculopathy

Pełny tekst źródłaFree microsurgical transfer of a radial forearm flap is a valuable surgical option for repairing orofacial soft-tissue defects. Harvesting this flap in the ventral forearm region leaves a donor-site defect, which is often being surgically treated via full-thickness skin grafting. Both the ventral forearm and the orofacial region are counted among the most highly exposed and heavily stressed human body parts. Functional and aesthetic outcomes after surgical intervention in these two areas have therefore a not insignificant influence on patients’ postoperative quality of life. Hence, objective follow-up examination of both the donor and recipient site of the radial forearm flap, as well as questioning patients on their subjective complaints, is important for evaluating this reconstructive surgery technique. In order to appraise this treatment procedure, a retrospective cross-sectional clinical study took place between 2007 and 2009 in the Oral and Maxillofacial Surgery Clinic at the University of Würzburg, Germany. Surgical outcomes at both the donor and recipient site of the radial forearm flap were evaluated by reference to the medical records of 50 patients. Donor and recipient site of the radial forearm flap, plus groin area as the full-thickness skin graft donor site, were examined in 30 of the 50 cases from both a functional and aesthetic viewpoint, using a standardised procedure, and additionally questioning the patients on complaints and on assessment of the treatment results. The appraisal of surgical outcomes by means of the patients’ medical records revealed a success rate of 96 % for transferring the radial forearm flap, as well as a success rate of 100 % for closing the radial forearm flap donor-site defect with a full-thickness skin graft. The follow-up examination at each the donor and recipient site of the radial forearm flap showed a rapidly progressing recovery of function and aesthetics during postoperative healing process. In the patients examined a year after surgery or later, all full-thickness skin grafts, as well as 91 % of the radial forearm flaps, had already regained sensation. Concerning the radial forearm flap donor-site morbidity in the patients examined 12 months after surgery or later, there were minimal joint mobility deficits in only 8 % of the cases, while 15 % showed a low-grade muscle strength decrease. Scar assessment score in each the forearm and groin region also tended to be higher, if surgery was performed longer ago. Patients’ contentment with treatment results showed a distinct improvement over the course of time, too. Regarding aesthetics, as early as one year after surgery there were no more complaints, and with respect to function, only a sixth of the patients questioned 12 months after surgery or later, voiced complaints as to each the donor and recipient site of the radial forearm flap. A comparison of these trial outcomes to results already published, confirmed the high reliability of this surgical procedure as to functional and aesthetic recovery at both the donor and recipient site of the radial forearm flap. In conclusion, free microsurgical transfer of a radial forearm flap, as a well-proven and highly suitable procedure of repairing orofacial soft-tissue defects, is further on just as recommended as full-thickness skin grafting, which is a safe and standard surgical option for closing the radial forearm flap donor-site defect in the ventral forearm region
