Objectives: Clearance of the pharynx by deglutition is important in protecting the airway. The pattern of deglutition during sleep was investigated in children. Methods: Ten normal human children (8.6 ± 2.9 years) were examined via time-matched recordings of polysomnography and of surface electromyography (EMG) of the thyrohyoid and suprahyoid muscles. Results: During sleep, deglutition was episodic, and it was absent for long periods. The mean number of swallows per hour (±SD) during the total sleep time was 2.8 ± 1.7 per hour. The mean period of the longest absence of deglutition was 59.7 ± 20.3 minutes. Most deglutition occurred in association with spontaneous electroencephalographic arousal in rapid eye movement (REM) and non-REM sleep. Deglutition was related to sleep stage. The mean number of swallows per hour was 27.4 ± 27.4 during stage 1 sleep, 3.1 ± 3.5 during stage 2 sleep, 2.8 ± 3.3 during stage 3 sleep, and 0.9 ± 0.8 during stage 4 sleep. The deeper the sleep stage became, the lower the mean deglutition frequency became. The mean number of swallows per hour was 2.2 ± 2.1 during REM sleep. The EMG amplitude dropped to the lowest level of recording during REM sleep. Conclusions: Deglutition, a vital function, is infrequent during sleep in children.

The purpose of the present study was to investigate whether or not sensory input from the tongue affects deglutitive tongue movement. Subjects were seven healthy volunteers with anesthetic applied to the surface of the tongue (surface group) and seven healthy volunteers with the lingual nerve blocked by anesthetic (blocked group). We established six stages in deglutition and analyzed deglutitive tongue movement and the time between the respective stages by cineradiography before and after anesthesia. After anesthesia in both surface and blocked groups, deglutitive tongue movement slowed and bolus movement was delayed. The deglutitive tongue tip retreated in the blocked group. These results suggest that delay of tongue movement by anesthesia causes weak bolus propulsion and that deglutitive tongue tip position is affected by sensory deprivation of the tongue or the region innervated by the inferior alveolar nerve.

Background: Deglutition requires great muscular and nervous coordination. Dysphagia is defined as swallowing impairment in any of its phases: preparatory, oral, pharyngeal and esophageal. Its causes are diverse, and diagnosis is imperative to avoid detriment to the quality of life. Objective: To identify the risk factors for deglutition disorders in the institutionalized elderly population. Material and methods: Clinical and descriptive trial. We included patients above 60 years of age assessed at the Otolaryngology Department at the Hospital Español from October to December 2016. Deglutition dynamic tests were performed, and three questionnaires commonly used as geriatric scales were applied to each patient. Statistical analysis was performed using Pearson coefficient to determine the correlation and strength between variables. Results: Fifty patients were included. Data of food aspiration to the airway was found (n = 31, 62%). Systemic hypertension (61%) and neurological deficiencies (51%) showed greater prevalence when deglutition disorders were present. Folstein’s brief mini-mental is a sensitive and specific scale when deglutition disorders are suspected (S = 100%, E = 100%, LR + 100%, r = 1). Conclusions: Deglutition disorders are multifactorial. Folstein’s brief mini-mental and other geriatric questionnaires are useful tools to screen deglutition disorders.

Undernutrition and dysphagia in the institutionalized elderly are well documented in the literature. However, the clinical efficacy of diets to treat dysphagia have not been established. To offer a better understanding of the textural characteristics of the new Sainte-Anne's Hospital (SAH) modified texture reformed foods, rheological evaluations were performed. Apparent viscosity, consistency coefficient, flow behavior index and yield stress values were calculated for the thickened beverages. Texture profile analyses were performed on the reformed foods. Secondly, to evaluate the impact of SAH's reformed foods on nutritional intake and weight, a 12-week clinical trial was undertaken. Dysphagic frail elderly subjects (n = 17) of a long-term care facility of Montreal were randomly assigned to receive SAH reformed foods or traditional care. The experimental group demonstrated a significant increase in nutritional intake resulting in significant weight gain. These findings suggest that dysphagic frail elderly could reach healthy weight when adequately fed.

Objetivo: Determinar a ocorrência de sinais clínicos de disfagia em lactentes com bronquiolite viral aguda. Métodos: Estudo transversal, com 42 lactentes, entre 0 e 12 meses, previamente hígidos, recebendo dieta via oral e internados com bronquiolite viral aguda. A avaliação clínica da deglutição foi acompanhada das medidas da frequência respiratória e oximetria de pulso. Foi elaborado escore de alterações de deglutição para estabelecer associações com demais variáveis do estudo. Realizada a concordância intra e entre examinadores. Os cuidadores responderam um questionário sobre comportamento de alimentação. O nível de significância utilizado foi p<0,05. Resultados: Foram encontradas alterações na fase oral (pausas prolongadas) e faríngea (respiração ruidosa, tosse e engasgos) da deglutição. Houve aumento significativo da frequência respiratória entre o momento pré e pós alimentação e quase metade dos lactentes apresentaram taquipnéia. Observou-se associação entre o escore de alterações de deglutição e a queda de saturação de oxigênio. Apesar da associação do número de alterações de deglutição com o aumento da frequência respiratória não ter sido significativa, as crianças com maior número de alterações de deglutição tenderam a apresentar frequência respiratória mais elevada. Não houve diferença significativa entre as duas avaliações do mesmo observador (p>0,05), entretanto na concordância entre examinadores houve diferença significativa entre os dois avaliadores quanto à cinco itens da avaliação. Os lactentes cujos cuidadores relataram dificuldades de alimentação durante a internação tiveram um número significativamente maior de alterações de deglutição na avaliação. Conclusão: Lactentes com bronquiolite viral aguda apresentaram alterações da deglutição, acrescidas de mudanças na frequência respiratória e nas medidas das taxas de saturação de oxigênio. Sugere-se, assim, risco para a disfagia.
Objective: To determine the occurrence of clinical signs of dysphagia in infants with acute viral bronchiolitis. Methods: Cross-sectional study of 42 infants between 0 and 12 months, previously healthy, receiving oral diet and hospitalized with acute viral bronchiolitis. Clinical evaluation of swallowing was accompanied by measurements of respiratory rate and pulse oximetry. Score of swallowing dysfunction was developed to establish associations with other study variables. An agreement among the examiners was reached. Caregivers answered a questionnaire on feeding behavior. The level of significance was p <0.05. Results: Alterations in the oral phase (prolonged pauses) and pharyngeal phase (wheezing, coughing and gagging) of swallowing were found in the study. There was a significant increase in respiratory rate between the time before and after feeding and nearly half of the infants had tachypnea. It was observed a relation between the score of swallowing dysfunction and fall of oxygen saturation. Although the association of the number of swallowing disorders with the increase of respiratory rate was not significant, children with larger changes in swallowing tended to have higher respiratory rate. There was no significant difference between the two evaluations of the same observer (p> 0.05), however in the agreement between examiners there was significant difference between the two raters on the five items of the assessment. Infants whose caregivers reported feeding difficulties during hospitalization had a significantly greater number of changes in the evaluation of swallowing. Conclusion: Infants with acute viral bronchiolitis showed abnormal swallowing, plus changes in respiratory frequency and measures the rate of oxygen saturation. It is suggested, therefore, risk for dysphagia.

Abstract The number of Total Glossectomy cases in the United States is seeing an increasing trend as per the Nationwide Inpatient Sample Database. Patients, who have undergone such aggressive surgical procedures, have extensive limitations performing basic oral functions such as swallowing (deglutition), eating and speaking. Current rehabilitation prostheses do little in restoring the functionality of the original tongue. This is true especially in deglutition, which is necessary to transfer a bolus to the esophagus. Such patients need advanced prosthetic devices and through this research, investigations into potential solutions for prosthetic tongues to aid in deglutition were carried out. Different designs were considered and based on a decision matrix, PneuNets (pneumatic networks) were adopted as the foundational basis for generating prosthetic tongue designs. Several prototypes were fabricated that used the Fused Filament Deposition process for producing the mold and silicone Eco-flex 00-30 for producing the mechanism. The resulting mechanism was powered using a pneumatic input and kinematic data was collected. Details from literature review, design iterations, simulations, validation processes, manufacturing challenges and conclusions will be discussed in depth in this paper.

Abstract Oral cancer can result in the loss of tongue through total glossectomy, and patients have difficulty with speech, deglutition, and mastication. Current tongue prostheses lack the ability to move for mastication and deglutition and are mainly for cosmetic purposes. The goal of this research was to investigate and develop a self-contained prosthetic tongue that could aid in deglutition by moving bolus from the front of the mouth to the back. A review of existing designs and alternatives led to the development of a linkage system mounted in a silicone cast to move the bolus. The development of this new prosthetic tongue design entailed creating silicone molds, 3D printing linkage systems and jaws, and developing new circuits. The circuit used a TinyDuino microcontroller and MOSFET. A functional mandibular Hawley retainer was added to hold the tongue to increase ease of use. The final circuit used an EMG sensor to trigger actuation and a single servo motor with a gear train to actuate the tongue. The prosthetic tongue succeeded in moving bolus from the front of the mouth to the back when lying at a 5-degree angle and actuating between 1.8–2.31cm in height. The paper will describe the development process, challenges, and future work.

Abstract Oral cancer can result in the loss of the tongue through surgical removal known as glossectomy. Patients who have undergone this procedure face challenges during speech, mastication, and deglutition. Currently, tongue prosthetics lack functionality and are mainly cosmetic. Many of these prosthetics are made of wax and connected to a retainer, which attaches to the back molars of the patient. The goal of this project was to develop a self-contained mechatronic tongue prosthesis that can fit within the oral cavity and aid in deglutition. Investigations into various techniques and sensors supporting miniaturization were carried out and magnetic actuation was found to be the most promising technique. The development process involved redesigning the silicone cast to house sensors, selecting sensors and components for magnetic actuation, magnetic field quantification and miniaturizing various other electrical components. The tongue prosthesis was tested, and the displacement was comparable to a normal human tongue. Details from literature review, design iterations, simulations, validation processes, manufacturing challenges and conclusions will be discussed in depth in this paper.
