This research took a qualitative approach, focusing on the perceptions and experience of consultants and consultees. The rationale is explored in the light of previous research, which concentrates on a quantitative approach and does not explore consultation from the participants' perspective. This research adds to the research base because it investigates the experience of participants in consultations from a number of perspectives. 46 questionnaires were sent to educational psychologists to gain their understanding of the pertinent elements of consultations and to aid reflection on the language of consultation. The return rate was 40%. The resultant data was used to formulate a semi-structured questionnaire, which was used to interview eight educational psychologists about their experiences of consultation in perceived successful and unsuccessful circumstances. 16 consultees were interviewed: one from each of the perceived successful and unsuccessful situations. Finally transcriptions were made from recordings of 'live' consultations of each of the original consultants. An iterative qualitative analysis was undertaken using grounded theory and focused feedback with the participants. A critique of the methodology focused on the strengths and limitations of the design adopted. The results showed significant variation in the conceptualisation and practice between consultants, with little shared understanding of the role and practice of consultation either across consultants or between consultants and consultees. The espoused practice of consultants differed from their actual practice and from the experience of consultees. Differences were found in the use of theoretical principles, power stances, the process of problem analysis and intervention strategy development. Although most participants acknowledged the importance of evaluation, in practice no time was allocated to this activity. This research considered a small sample of consultants and consultees, all of whom worked in the same county. Any generalisations drawn must be tentative. Nonetheless, the variance found within the relative homogeneity of the group of psychologists does suggest the variance would only increase within a larger sample with less homogeneity. The discussion and summary considers the implications for educational psychology practice of consultation and future research.

Democracy is being challenged by low public participation in the democratic process. This has resulted in low voter turnout and is therefore called a crisis of representative democracy. As a means of increasing citizens’ engagement with their representative bodies, Information Communication Technologies (ICTs) have been implemented to encourage public online participation. In particular, the British Parliament has adopted an e-consultation system, available on its website since 2007, as an electronic tool for online participation. As e-consultation is the only system which truly enables the public to engage the legislative process, it is worth assessing this system in order to determine its effectiveness and impact. To do this, it is necessary to conduct an analysis through case studies on past econsultations. Through this analysis, an aim of this thesis is to seek new methods by which to maximise the efficiency of the e-consultation system through the consideration and application of specific research questions. Answers to specific questions were sought by means of employing empirical studies on two e-consultation cases to determine levels of public participation: the successful topic of ‘Domestic Violence’ and the less popular topic of ‘e-Petitions.’ Analysis of the two e-consultation cases reveals that e-consultation provides the public with valuable opportunity to connect with its representatives and to engage the legislative process. However, in order to maximise the efficiency of the e-consultation system, several modifications are suggested including: adequate preparation and publicity on behalf of the organising body, a high rate of MP participation, active consideration of any contributions by the committee in charge, and the establishment of a parliamentary division unit specifically for the conduction of e-consultations.

The consultation is a distinguishing feature of general practice compared to other medical disciplines. The relationship between the doctor and patient is crucial to the successful outcome of the consultation. Despite suggestions in the literature that interruptions to the consultation are detrimental to this relationship, there is a lack of research to support this claim.The overall aim of this study was to explore the consulting style of General Practitioners (GPs) and the impact of interruptions to the consultation to further understand GP behaviour and the doctor-patient relationship during the consultation. The implication of the study was to raise awareness for GPs of their consulting style and interaction with patients, potentially leading to changes in behaviour, resulting in better outcomes from consultations.This research involved six GPs consulting six actor-patients during two video recorded simulated consultation workshops. This research consisted of three studies. The first involved observation of GP behaviour during the simulated consultations, and the impact of interruptions to the consultation; the second involved GP and patient perceptions of behaviour during the consultations, and the impact of the interruptions; and the third involved obtaining GP and patient perspectives of behaviour and interruptions to the consultations, prompted by video footage from the consultations.In Study 1, evidence was found to support a GP consultation style whereby individual GPs showed similar behaviours during each consultation despite consulting a variety of patients. Variability in GPs ability to cope with interruptions to the consultation, and the little time spent by GPs establishing a relationship with patients regardless of the consultation being interrupted was highlighted. The findings of Study 2 supported previous reports of the frequent occurrence of interruptions to the consultation. Differences between GPs beliefs and patient perceptions of the impact of interruptions to the consultation to the doctor-patient relationship were highlighted. In Study 3, the opening sequence of the consultation was found to be of importance to the doctor-patient relationship and the outcome of the consultation.Overall, the findings of this study showed GPs inability to describe their behaviour, and a lack of awareness of their behaviour during the consultation. As a result, GPs may be missing vital cues from patients during the consultation regarding their thoughts and concerns, which may have negative consequences for the doctor-patient relationship. These implications, however, require further research. This study concluded that reflection, and mindfulness could be applied to GP consultation behaviour, using video techniques, in order to raise GP self- awareness of behaviour, improve communication, and the way that GPs relate to patients, and to improve outcomes of general practice consultations.

This study evaluated the school psychologist's perception of effective models of consultation based upon referral problem and parent, teacher, and student response to treatment. Analyses of covariance determined that (a) parents' receptivity and total number of teacher contacts significantly influenced the parents' response to treatment; (b) teacher receptivity and total number of parent contacts significantly affected teachers' response to treatment; (c) students' response to treatment was significantly affected by the model of teacher consultation and the average number of minutes spent with the school psychologists; and (d) students in a Mental Health consultation group responded significantly more favorably than s tudents in Behavioral or Collaborative consultation groups.

The main objective of this study was to investigate whether parental perceptions of the emotional, behavioral and social skills functioning of children with conduct problems, differed for parents who completed the behavioral consultation process (N = 40) compared to those who prematurely withdrew ( N = 11). A series of between group analyses were conducted to examine whether parental perceptions existed. Parents who prematurely withdrew from the behavioral consultation process reported significantly greater incidences of anxious and depressed behavior in children, than parents who remained in consultation F(1,49) = 4.24, p = .0448. Similarly, using the Wilks' criterion, overall estimates of emotional and behavioral functioning, combined with social skills functioning were also significantly affected by group membership, F(3,47) = 3.22, p = .0310. Using the same variables in a logistic regression analysis, a test of the full model with all three predictors compared to the constant only model was statistically reliable phi2 (3, N = 51) = 10.26, p = .0336, indicating that the predictors, as a set, reliably distinguished the perceptions of those who completed the consultation process, compared to those who prematurely withdrew. This model accurately predicted 77.7% of parents' group membership. The results of the study will be discussed in terms of their usefulness for future consultants to identify families at risk for prematurely withdrawing from the consultation process, so that additional resources can be offered to encourage their continued participation in the consultation process.

Sur la base de l'analyse de plusieurs protocoles cliniques d'intervention-consultation, l'hypothese est faite d'une specificite de la demande de consultation sociale, liee a son double caractere systemique et evolutif. En effet dans le champ de la consultation sociale, toute demande renvoie a d'autres demandes. D'une part en s'adressant au consultant le sujet reprend les demandes que d'autres sujets lui adressent mais qu'il a laissees sans reponse. D'autre part sa demande est demande de demande, c'est-a-dire desir. En reconnaissant le desir qui s'y fait entendre, le consultant soutient la demande qui revele alors son caractere evolutif. L'objet demande se decouvre ainsi comme le terme d'un rapport. Il s'agit toujours d'un rapport a trois termes sujet - groupe institution mais dont chacun des termes ne mediatise pas suffisamment le rapport des deux autres. C'est precisement cela qui determine l'intervention d'un quatrieme terme, le consultant. En maintenant sa reponse dans un ecart et un rapport a la demande celui-ci engage le processus vers le franchissement de l'imaginaire de la demande. Si l'objet de la demande est rapport entre plusieurs termes, l'objet du travail de consultation est une mediation entre ces termes. Celle-ci s'elabore au travers la lecture faite par le consultant de ce qui fait rapport entre les sujets : chaque sujet, les autres sujets, et ces "tiers-sujets" auxquels ils se rapportent de par la tache et les fonctions de l'institution. Cette operation de lecture, qui met en evidence l'alterite de chaque sujet, se prolonge par une operation d'ecriture qui articule les demandes les unes aux autres. C'est pour autant que le consultant represente cette necessite de l'articulation des demandes que sa position peut etre dite comme celle du desir
A hypothesis based on several intervention-consultation clinical protocoles is made on the specificity of a social consultation request as linked to ils systemic and evolutive caracteristic. On the one hand, a subject making his request to a consultant repeats other subjects' requests which he did not satisfy. On the other, his request is a request of request : a wish. As he identifies the wish, the consultant supports the request which then reveals its evolutive caracteristic and the request object appears as a related term. It is always a three-term relation : subject-group-institution, but each term mediates insufficiently the relation to the other two. This precise fact determines the intervention of a fourth term, the consultant. As he maintains his response at a certain distance and in relation to the request, the consultant sets going a process oriented towards surpassing the imaginary of the request. If the object of request is a relation of several terms, the object of consultation is mediation between these terms. The consultant constructs it by reading what makes relation between the subjects : each subject, other subjects and those subjects to whom the previous relate by the mission and functions of the institution. This reading operation which reveals the alterity of each subject, continues through writing as it articulates the different requests. Allowing that the consultant represents the necessity to articulate requets, his position may be qualified as that of wish

The focus of this thesis is the study of the new interactive-consultation connected to the reading, this consultation is compared to other systems of writing-reading: alphabet. Ideography. We study how the type of reading, providing original cognitive tool, enhances habits giving the rules of a specific logic. Using an approach specific to communication sciences, we study the connections between a reader-actor, a reading system or mediation, a milieu characterized by peculiar cognitive model or logic. Readers have new needs, they develop a new tool: in this case: interactive interface. This interface develops new habits and soon new milieu. The study of the hci rules and the observation of users, belonging to the two different traditionnal reading system cited: japanese and french, in situation of interactive consultation, allows us to bring out the emerging rules of a new logic

D’une nature juridique que le droit français peine à mettre en valeur, la procédure de consultation constitue une règle atypique qui, plutôt que de constituer une contrainte à l’exercice du pouvoir, doit être perçue comme proposant un modèle aux manifestations du pouvoir de direction de l’employeur. Là où la règle de droit tutélaire interdit à l’employeur de faire, ou, lui dicte une stricte ligne de conduite afin de protéger les destinataires de sa décision, la règle de procédure dresse un processus, un cheminement vers l’obtention d’une décision dont le contenu a fait l’objet d’une concertation avec ses destinataires. La règle de procédure borne le pouvoir de l’employeur sans le brimer. Et, elle intègre ses destinataires dans le processus d’élaboration de la décision patronale qu’ils ont vocation à subir.La diversité des manifestations du pouvoir de l’employeur rend complexe sa définition. La procédure de consultation se perçoit alors comme un outil d’analyse du pouvoir, permettant de le sonder. De la fonction empirique de la règle de procédure ressortent deux catégories juridiques de procédures de consultation : les consultations « sur un projet de décision » et les consultations « sur une politique générale ».Toutes deux bénéficient d’exigences procédurales communes : information précise et écrite, délai d’examen suffisant, réponse motivée de l’employeur aux observations, remise de l’avis, et suite à donner à l’avis. Le schéma de ces exigences procédurales est tourné vers la transformation de l’information en un savoir mobilisable lors d’un échange avec l’employeur sur la pertinence d’un projet de décision ou de stratégies, de politiques de gouvernance. Au terme de cet échange, le comité d’entreprise pourra exprimer un avis selon l’intensité requise par le code du travail : un « avis motivé ».Au-delà de l’aspect pratique que présente la catégorisation opérée, il nous semble que son principal critère de distinction, l’objet du pouvoir appréhendé par la procédure, commande le régime de la sanction civile de leur transgression. Consultations « sur une politique générale » et Consultations « sur un projet de décision » ne peuvent recevoir un traitement uniforme. La particularité de l’objet appréhendé dans certaines décisions ou savoirs par la procédure, a justifié une intervention du législateur, celui-ci décrivant avec plus de précisions les exigences procédurales (on pense notamment aux délais plus longs, à l’intervention de commissions). Autrement dit, elles ne sont pas appréhendées de manière uniforme par le code du travail, comme le laisserait présager la description générale des modalités procédurales opérées par les articles L. 2323-2 à L. 2323-4 C.T. Certains objets plus que d’autres sont aménagés de manière spécifique. Il existerait ainsi une hiérarchie des décisions et savoirs de l’employeur qui serait fonction de la spécialisation de leur régime dans le code du travail (licenciements économiques, transferts d’entreprise, modifications de la structure économique/juridique, etc.). Elle pourrait faire l’objet d’une étude spécifique.La catégorisation proposée induit un parti pris quant à leur hiérarchie. Les consultations « sur un projet de décision », qui captent l’exercice le plus autoritaire du pouvoir, doivent être assorties la sanction la plus stricte. La nullité s’impose comme la sanction adéquate. Elle ne pourra être invoquée que plus rarement dans le cadre de la transgression des règles de consultation « sur une politique générale »
Literally translated as “enterprise committee”, the French “Comité d’entreprise” (“Works Council” in the UK) owns specific abilities in regard to employer initiatives concerning especially the organization and management of the enterprise: its consultative competence. Rather than a constraint on the exercise of power, the consultation process must be seen as offering a model for demonstrations of the power of the employer. Where the “tutelary law” prohibits employers to do, or dictate a strict course of action, the procedural rule provides a process, a path to obtain a decision whose content has been discussed with its recipients, the workers. The procedural rules limit the power of the employer without bullying it.The consultation process can also be perceived as a category of analysis, a tool for describing the various ways in which managerial prerogatives are exercised by the employer. Thereby, two legal categories of procedures for consultation are emerging: consultations "on a draft decision" and consultations "on policy." Consultations "on a draft decision," which capture the most authoritarian exercise of power, must be accompanied by strict enforcement. The invalidity of the decision seems to be the appropriate sanction

This project was part of a larger research project aimed at researching and supporting the development of a new postgraduate qualification in general veterinary practice. It arose in the beginning from the establishment of an MSc group of 8 experienced veterinary general practitioners in 2001, who researched various aspects of the question ‘what are the postgraduate educational needs of the GP vet in the UK?’ After completing the original work, 5 of the group continued their research into their Doctorates, and this project was the one in the subject of consultation skills. The subject was very largely unexplored in the veterinary world, but extensive research had been done in the medical field. There were the challenges of formulating methods of collecting primary data in the veterinary field, and looking at the medical field to see if the methodologies were transferable. The choice of methodologies was strongly influenced by fear factors, in which the researcher had to make choices based on what methods of data collection the veterinary practitioners accepted. Research instruments were found and developed to explore aspects of the consultation. Key performance indicators were explored firstly separately, and then the research explored possible links between consultation skills and KPI’s. Significant links were found, which lead to a greater understanding of the importance of consultation skills and how they might be analysed in ways that had potential for veterinary surgeons to see ways of identifying their skills and improving them. The results were of high impact value.

Consultation is a prominent indirect service delivery method in school psychology today. Several publications endorsed by the National Association of School Psychologists (NASP) have traditionally suggested the mental health, organizational, and behavioral models of consultation as the three prominent consultation models in the field of school psychology. However, no supporting data are cited and it is unclear if these three models are still prominent in school psychology today. The present research examined the consultation literature over the last 22 years in two prominent school psychology journals, Journal of School Psychology and School Psychology Quarterly (originally called Professional School Psychology). This research involved the analysis of 145 consultation articles from the school psychology literature. The data was analyzed to obtain information regarding the frequency of consultation articles published, any trend that emerged from this data, the most frequent types of consultation mentioned, the type of research conducted, and topics addressed in the articles. Analysis of consultation trends over time indicated a decrease in the frequency of consultation articles published over time in both journals. The terms utilized to address consultation models are greatly varied, demonstrated by the 107 different types of consultation referenced in the consultation articles. Clearly, behavioral consultation is the most frequently researched and referenced consultation model in the literature. The results suggested the three traditional prominent models have been modified over time. Mental health consultation and behavioral consultation were consistently found within the top three models researched and referenced; however, organizational consultation was infrequently mentioned in the literature and researched in only 4% of the articles reviewed for this study. Therefore, it appeared that organizational consultation is no longer considered a prominent model of consultation. Analysis of the articles that were not research oriented indicated that 76% of the articles focused on application of consultation. The topics of multiculturalism, training, and ethics were the focal point to a lesser extent, with each representing fewer than 5% of articles. Research focused articles that involved aspects of both questionnaire and intervention research tools were employed more frequently than were research involving only questionnaire research or intervention research. Implications for the present findings and future directions for consultation research are discussed.

The purpose of this research is to understand the various relational dimensions that characterize the process of behavioral consultation. A relational communication perspective emphasizes that within dyadic interactions (such as those that occur between a consultant and consultee); speakers are constantly redefining their roles, positions, and relationship through conversations (Erchul, Grissom, & Getty, 2008). Although communication researchers have emphasized several relational themes that emerge in dyadic interactions (e.g., trust, similarity, depth; Burgoon & Hale, 1984; Millar & Rogers, 1976, 1987), relational communication consultation studies have almost exclusively focused on the theme of relational control (i.e., dominance-submission). This exclusionary focus on relational control has neglected other important relational themes that may characterize consultant-consultee interactions. In this study, participants listened to consultation interviews in which consultants and consultees were characterized as either high or low dominance. After listening to the interviews, participants rated consultants and consultees on several relational dimensions (e.g., involvement, trust, similarity, depth, composure, formality). Results from this study suggest that several relational dimensions are present within consultant-consultee interactions. Additionally, the presence of relational dimensions varies based on both role (i.e., school psychologist or teacher) and level of dominance. In sum, results from this current study suggest that relational dimensions other than dominance are present in consultant-consultee interactions.

The primary objective ofthe present study was to examine the effectiveness of problem-solving consultation and videotape therapy in reducing externalizing behavior problems in children with developmental delays. Multiple measures were used to determine intervention efficacy (e.g., observations ofparent-child interactions, recorded frequency of childrens' behavior, and ratings from responses on standardized parent questionnaires). A second objective was to explore the relationship between parent and child behavior, The researcher evaluated the quality of parent-child interactions, in terms of childrens' behavior problems (e.g., compliance) and parents' skills (e.g., praise), prior to and following participation in the intervention program. The relationships between parent adjustment variables (e.g., parent stress and depressive symptoms), externalizing behavior problems in children, and the quality of parent-child interactions were assessed. Parent adjustment was measured by self-report with standardized questionnaires that evaluated parent stress and depression. The twelve-week intervention program was provided to 22 children, parents, and teachers. A multiple-baseline research design was used and standardized measures were completed by parents at pre-and postintervention.[...]

Dans ce travail, on se propose d'offrir des possibilites d'interaction avec la video similaires a celles des textes structures. Pour cela, l'information video brute doit etre interpretee, indexee et structuree. Il faut egalement proposer des reperes aux utilisateurs afin de leur permettre une lecture aisee et personnalisee de l'information video. Nous distinguons deux aspects dans le travail mene. Le premier aspect concerne l'interpretation de la video brute. Il s'agit d'extraire le maximum d'informations de la video a partir a la fois de la bande image et de la bande son. Dans le deuxieme aspect, on cherche a associer une structure a la video afin d'en permettre differentes possibilites de parcours. Nous apportons des contributions a ces deux aspects. Au niveau du premier aspect, nous nous sommes limites a l'analyse de la bande image. D'autre part, nous avons cherche autant que possible a definir des procedures automatiques pour l'extraction d'informations. L'analyse de sequences d'images est l'outil utilise. Notre interet pour des procedures automatiques est motive par les grands volumes d'information generes par la video et qui rendent un traitement manuel vite fastidieux. Nous proposons donc des procedures automatiques. Nous en avons implante deux: la detection des ruptures de plans et la reconnaissance des mouvements de camera. Au niveau du deuxieme aspect, nous presentons une strategie permettant d'associer rapidement une structure a une video. La structure est conforme a un modele que nous definissons. Nous montrons enfin que la video traitee et structuree permet de multiples possibilites d'interaction avec la video

In 2008, Col James, a senior lecturer at the University of Sydney, initiated a collaborative design project celebrating Aboriginal culture in Victoria Park, an historic public park on the outskirts of Sydney’s CBD. This project was supported by members of the Aboriginal community, but as the project progressed concerns were raised about the consultation process. A subsequent investigation identified poor consultation as a common trigger for failure in design projects for Aboriginal peoples. A review of the existing literature showed that most consultation guidelines are written for regional or remote areas, and little information is available for urban or contested locations. This research addressed these gaps in the literature by asking whose voices, ways of knowing and conferring authority are given precedent in consultation guidelines for design projects celebrating Aboriginal culture on colonised and urban land, and how might such design projects be best negotiated. Using an ‘outsider researcher’ perspective to address the interface of Aboriginal and non-Aboriginal knowledge systems, the research adopts an outsider’s decolonising and post-colonial critical methodology informed by Linda Tuhiwai-Smith, Michel Foucault and Homi Bhabha. This approach foregrounds previously silenced voices through interviews conducted as guided conversations with Aboriginal Elders and Aboriginal and non-aboriginal design experts. Existing consultation guidelines are critically analysed to identify the different themes, world views, standpoints and agendas which inform them and the findings are then compared to identify correlations and disconnections with the interview findings. The thesis confirms that consultation for Aboriginal design projects in contested locations is a complex issue for designers requiring specialised skills, strategies and competencies to account for and accommodate for historically unequal power relationships between colonised Aboriginal peoples and non-aboriginal people. The thesis re-frames and re-presents the knowledge on consultation in a more fluid, dynamic and flexible form, revealing new understandings about design praxis that move towards the increased inclusion of Aboriginal peoples' voices, perspectives and practices in the design consultation process.

This study explores the phenomenon of the informal medical consultation, a communication event in which an individual asks for medical information, advice, or care from an off-duty health professional with whom the individual has no formal patient-provider relationship. Using surveys and interviews, the study describes these consultations from the perspective of the health care professional and the informal patient. The study explores foundational theories that offer explanations for the phenomenon. The theories considered include social support, decision-making, social exchange, perceived partner responsiveness to needs, and uncertainty management. This study suggests health care providers perceive informal medical consultations to be more problematic than do the informal patients who consult them. The problematic nature of informal consultations increases as the type of request moves from purely informational to a request for treatment. Informal patients do not perceive this distinction. The informal patient’s motivation to pursue an informal consultation instead of a formal consult is affected by the relationship with, trust in, and access to the informal consultant. The willingness of the informal consultant to engage in an informal consultation is affected by the relationship with the informal patient, the type of request made, and perception of risk/benefit for both the provider and the patient. The study supports the idea that informal medical consultations are potentially problematic within the current medico-legal-ethical environment. Alternately, these consultations may be viewed as offering positive contributions to the health and well-being of informal patients. The study suggests translational research is needed to guide health professionals in considering requests for informal medical consultations.

This study is one of the responses by the National Park Service to requirements in NAGPRA. The study was commissioned by the NPS Applied Ethnography Program in Washington, D.C., to identify individuals and tribes affiliated with the objects of cultural patrimony, sacred objects, or unassociated funerary objects at five NPS units, review those unit summaries, assist park or center staff in initiating consultation regarding those objects, and conduct a case demonstration consultation for Pipe Spring National Monument. The project was administered under Cooperative Agreement #8100 -1 -0001 between the Western Archeological and Conservation Center, National Park Service and the University of Arizona. While this study was specific to NAGPRA- related issues, the NPS does stipulate in its Management Policies (1988) that consultation with Native Americans will occur with regard to cultural resource issues. NAGPRA is not the only consultation arena the NPS is currently involved in with Native Americans.

Consultation has long been noted in the literature as a preferred role of school psychologists (Curtis & Zins, 1980; Gutkin & Curtis, 1982). Yet, few studies have examined University training practices in the area of consultation. This study, in part, replicated a study conducted by Meyers, Wurtz, and Flanagan (1981) that examined training practices 20 years ago. Currently, Program Directors from school psychology training programs in the United States were asked to describe how their training program addressed consultation. The number of courses offered in consultation, the methods used to teach consultation, and the models of consultation taught to school psychology graduate students were assessed. The results of this study indicated that there has been an increase in consultation training practices among training programs in the United States. It was also found that of the three main models of consultation present 20 years ago (i.e., Mental Health, Behavioral, and Organizational), the Behavioral model is dominant today. A number of suggestions for further evaluating the consultation training issue were made.

Telemedicine is the combination of information technology (IT) and health care and is an area that has seen increase during the last few decades due to breakthroughs in IT. Today there are still parts of the health care system that can be counterproductive with regards to the time and commitment necessary. One such part is medical consultations for private individuals where many of the simpler ones are still carried out in-person. Presented in this thesis is the implementation of a prototype telemedicine system that could be used as a substitute for in-person doctor-patient consultation. Necessary 3rd party components and their implementations into one system are described on a general use level. The result is a web application that supports login and payment via trusted sources, booking of meetings and carrying them out with video and audio. The prototype is a continuation of a telemedicine web application idea that was put on ice by a company that already provides solutions for health controls and blood analysis.
Telemedicin är kombinationen av informationsteknik (IT) och hälsovård och är ett område som har sett en ökning de senaste årtiondena på grund av genombrott inom IT. Idag finns det fortfarande delar av hälsovården som kan vara kontraproduktiva med avseende på den tid och det åtagandet som krävs. En sådan del är medicinska konsultationer för privatpersoner där många av de simplare mötena fortfarande sker i person. Presenterat i denna avhandling är implementationen av ett distribuerat telemedicinsystem som kan användas som ett substitut för de konsultationer mellan doktor och patient som sker i person. Nödvändiga tredjeparts komponenter och deras implementationer in ett system är beskrivna på en allmän användningsnivå. Resultatet är en webbapplikation som stödjer login och betalning via en pålitligt källa, bokning av möten och utförandet av dem med bild och ljud. Den presenterade prototypen är en fortsättning på en telemedicinsk webbapplikationsidé som lagts på is av ett företag som redan erbjuder lösningar för hälsokontroll och blodanalys.

The main goal of this research is focused on increasing the probability that strategic decisions will be implemented. The dissertation describes the strategic decision-making process followed by the top executive teams of two organisations, within a consultation framework employing the Process Consultation approach. A particular form of SWOT analysis (SWOT Positioning) was performed as a platform for interventions. It also comprised a basis for the employment of other analytical and creative tools. In constructing this research, it was hoped that the combination of Process Consultation with SWOT Positioning in this manner would prove helpful in achieving the research goal. This research found that the likelihood of implementation of decisions seemed greater following the current consultation process than in the two organisations' previous experience with 'expert' and 'doctor' consultants. Indeed, by-product decision implementation had already begun. The preparation for general implementation of strategic decisions and the declaration of commitment on the part of team members exceeded that of the same organisations in the past. The research cannot definitively show actual implementation, as that stage was not within its scope. However, both teams indicated a sense of ownership of and responsibility for the decisions, which had been formed gradually during the process. Creativity was nurtured in the construction of strategic action alternatives, which were then organised in a Level of Innovation Matrix. The Matrix is a tool which improves the participants' understanding of the degree of complexity and difficulty involved in the implementation of new strategies. Indeed, the quandaries which promoted this research have been somewhat diminished. This is not to claim a single conclusive solution has been discovered to all the questions asked. Rather, the statement made here is that the particular process proposed might be helpful in answering these questions in appropriate situations.

The present study examined parents' and teachers' acceptability of conjoint behavioral consultation for children with problem behaviors at school and at home. Also examined was the relationship between perceived treatment acceptability and treatment outcome, and time to treatment effectiveness. An A/B design was used and participants included 14 children, their parents, and teachers. Results indicated that conjoint behavioral consultation was an equally acceptable intervention prior to and following implementation for both parents and teachers. Results also indicated that parents' perceptions of treatment acceptability at posttest were not related to the effectiveness of the intervention as measured by direct observations. On the other hand, teachers' perceptions of treatment acceptability following implementation were found to be positively related to the effectiveness of the intervention. Furthermore, both parents and teachers perceptions of treatment acceptability were found to be significantly related to perceptions of treatment effectiveness and time to effectiveness. Results are discussed in light of their practical and theoretical implications.

Type 2 diabetes is a common condition that is associated with high rates of morbidity, mortality and healthcare costs. Multifactorial management has been shown to be effective in preventing complications and inducing remission or cure of the disease. Despite this, the rate of new cases and the overall burden of the disease remains high. There is a need, therefore, to refocus efforts on strategies to prevent complications in type 2 diabetes. Most of the disease in the UK is now managed in primary care consultations and there is increasing emphasis in health policy on 'making every consultation count'. Accordingly, the overall aim of my thesis is to contribute new knowledge and understanding on the role of the primary care consultation as a strategy in managing and preventing complications of diabetes, with a particular focus on the contribution of patient experiences. Data from the ADDITION-Cambridge and ADDITION-Plus population-based cohorts were used to quantify associations between experiences of primary care consultations measured with the Consultation and Relational Empathy Measure (CARE), and i) cardiovascular (CVD) risk factors, ii) CVD events, iii) all-cause mortality and iv) remission of type 2 diabetes. This was followed by further observational analyses to examine the role of patient health behaviours as a mediator of these associations. Qualitative analysis of semi-structured interviews with practitioners and patients were carried out to further elucidate theory that might explain how primary care consultations relate to the management of diabetes, and to understand the practicalities and barriers in delivering optimal experiences. Social practice theory was then used as a frame of reference to collate findings and contextualise the primary care consultation in managing and preventing complications of diabetes from a wider social perspective. The results of the observational analyses suggest that the primary care consultation may have a small impact on CVD risk factors early in the course of illness (higher levels of high-density lipoprotein cholesterol, lower systolic blood pressure and lower diastolic blood pressure). However, these associations are unlikely to be mediated through patient health behaviours. Over the longer term, consultation experience was significantly associated with the incidence of all-cause mortality; a 40% lower risk of mortality was found in patients with type 2 diabetes who reported better primary care consultation experiences compared to those with poor experiences. Similar trends were observed with CVD events but these were not statistically significant. Consultation experiences were additionally associated with remission of diabetes in adjusted models; 2-3% lower odds of remission with better patient-reported experiences of the consultation according to the CARE measure. The qualitative chapters identified issues around consultation length, relational continuity, face-to-face contact and type of practitioner as important contributors to primary care consultation experience. Moreover, my findings highlighted the difficult balance that needs to be achieved within the consultation between patient experience and disease management, and also between patient-centred and doctor-centred consultations. Further interviews with practitioners emphasised how the wider problems of pressures on the service and increased workloads in primary care are impacting consultation experience. This highlighted competing priorities which could distract from effectively utilising the consultation in diabetes. The final analysis chapter used social practice theory to collate the findings across chapters and suggested that a whole system and societal approach is needed to develop and inform the delivery of effective diabetes strategies. Overall, my thesis suggests that the primary care consultation experience has the potential to be an important strategy in managing and preventing diabetes complications. There are opportunities to more effectively use the consultation in primary care in a way that considers the individual, healthcare system and wider society together. More research is required to establish the causal pathways to explain how these experiences relate to outcomes and to understand what form a future whole system societal intervention might take.
