Most immigrants have to adjust their lives to living in a new country, especially when moving from an Eastern to a Western society. Chinese immigrants may face multiple difficulties, including cultural differences and the English language barrier, which are a particularly problem for older immigrants. To overcome these difficulties, many Chinese immigrants either form new community groups or join those currently operating. Normally, Chinese community groups provide a wide range of activities for members. The literature (e.g., Diener, 1984; Cantor & Sanderson, 1999; Csikszentmihalyi, 1975) provides some understanding of the value for new immigrants in attending these group activities. Participating members report improved life satisfaction within new society and have a more positive outcome as a result of their involvement. However, the magnitude of the influence life satisfaction and well-being is not as clear. This research aimed to better understand the relationships between participation in community group activities and the life satisfaction and well-being of participating members. A quantitative research method was adopted for this study to investigate the relationships between the variables. The study utilised a questionnaire that focused on specific demographic characteristics of participants, a 5-item life satisfaction measure (Diener, Emmons, Larsen, & Griffin, 1985), a short 18-item measure (Ryff, 1989a) of the six dimensions of psychological well-being, and on other factors likely to impinge on life satisfaction and well-being. The 7-point agreement scale asked participants the extent to which they agreed or disagreed with the statements. The questionnaire was offered to participants in both English and Chinese. Questionnaires were completed by 600 Chinese-speaking immigrants from Taiwan, China, Hong Kong, Malaysia, and Singapore who were participants in educational activities and/or other activities offered through 21 different Chinese community groups in Brisbane. Four hundred took part in 20 different educational group activities, and 200 in 25 different non-educational group activities. Most participants were female, and approximately two thirds were over 50 years of age. Using AMOS, a number of structural equations models (SEMs) were tested to investigate the conceptually proposed links between the various variables. This study supports other literature, finding that both educational and general activities provided by the community groups positively influence the participating members' life satisfaction and feelings of well-being. The majority of participants in this study reported that they considered participating in community group educational activities (74.2% agreed), and general activities (66.6%), had the most impact on their life satisfaction and well-being. Most participants (70%) found that community group activities, both educational and general, were interesting. However, only around 53% of them claimed to have achieved their goals in these activities. The findings of this study indicate the complexity of the influences on life satisfaction and well-being levels facing this cohort of Chinese immigrants. Although results suggest that demographic variables such as year of migration and employment status have a greater influence on life satisfaction and well-being than educational activities and general activities, the outcomes of this study support the conclusion that the participating members have benefited from the variety of programs offered by Chinese community groups.

This research examines the relationship between food experience and Elderly Chinese Immigrants (ECIs)' cultural transition in Australia. Field studies investigated ECIs' cultural experience of food, and how they react to the cultural differences in food-related activities. Results indicate that food experience mediates ECIs' cultural transitions between ECIs' home country cultural background and their host country context. Such findings lead to the development of the Food Experience Mediated Adaptation Framework, which illustrates ECIs' cultural transition and can be used as a guide for future design practices. This research, therefore, contributes to knowledge in the experience design field.

Racism and sexism pervaded American society during the nineteenth century, creating unusual disadvantaged conditions for Chinese immigrant women. As a weak minority in an alien and often hostile environment and as a subordinate sex in a sexist society, Chinese women suffered from double oppression of racism and sexism. In addition, the Chinese cultural values of women's passivity and submission existed within Chinese communities in America, and affected the lives of these immigrant women. This work uses government document, historical statistics, accounts from newspapers and literature to examine the life experiences of Chinese immigrant women and American attitudes towards them, and to analyze the roots of the oppression of racism and sexism.

Background: One of the common complications of stroke is dysphagia. Dysphagia can be a disabling problem with negative psycho-social consequences. The effect of dysphagia on quality-of-life is not well understood among the stroke survivors. The Chinese version of Swallow Quality-of-Life Questionnaire can be a reliable instrument to provide a multidimensional evaluation on the impact of post-stroke dysphagia in the population of Hong Kong Chinese older adults. Objective: To investigate factors associated with the swallowing-related quality-of-life in Hong Kong Chinese older adults with the history of stroke during the post-acute phase, using a disease-specific quality-of-life questionnaire (CSWAL-QOL). Method: This was a cross-sectional study of 92 stroke survivors, aged over 55. They were recruited from the inpatient wards and out-patient clinics. They were assessed for any dysphagia and its severity by the bedside swallow assessment. The Mini-Mental State Examination and the Chinese version of Swallow Quality-of-Life Questionnaire were used for cognitive and quality-of-life assessment respectively. Results: The test-retest reliability of the test was moderately correlated (Spearman’s rho 0.62).Participants on modified diet/fluid, on tube-feeding, older age and post-stroke for 1-2 years were observed to have lower CSWAL-QOL score across scales. Conclusion: The effect of ageing appeared to be a factor affecting swallowing-related QOL in stroke survivors, with better QOL in younger participants(age ranged from 55 to 69) and similar QOL in old (age ranged from 70 to 85) and very old (age above 85)participants. No gradual change of QOL was found as post-stroke duration increases. The oral feeding mode and normal food and fluid consistency resulted a better QOL in stroke survivors.

Increasing number of people is diagnosed with colorectal neoplasms (CRN) in the form of polyps or cancers. Colorectal cancer (CRC) is one of the most common causes of cancer deaths among Chinese in Hong Kong. Advances in diagnosis and treatments have increased the survival rate of CRN patients, resulting in a large group of cancer survivors. Preserving and maintaining health-related quality of life (HRQOL) has become an important direction in research and clinical care of patients with CRN. This research aimed to evaluate the HRQOL and its association with socio-demographics and disease-related characteristics among patients with CRN, and illustrate how HRQOL data could be converted to preference for the estimation of quality adjustment of life years in health economic evaluations. This research comprised of three parts. First was the establishment of the validity and reliability of the traditional Chinese version of Functional Assessment of Cancer Therapy-Colorectal (FACT-C) as a HRQOL measure for Chinese patients by a cross-sectional sample of 536 adult patients with CRN. Psychometric testing and concurrent validation of the FACT-C with European Organization for Research and Treatment of Cancer Core Questionnaire plus Colorectal-specific Module Questionnaire and the Short-Form 12-item Health Survey_Version2 (SF-12V2) were carried out. Second was a longitudinal study on the HRQOL of 554 CRN patients at baseline, six (n=479) and twelve (n=414) months of recruitment. The associations of HRQOL with socio-demographics and disease-related factors, and change of HRQOL over time were explored. Comparisons of HRQOL between CRN patients and the general population and among different CRN groups were made. Cross-sectional data were used to develop mapping functions to estimate SF-6D preference scores from FACT-C subscale scores. Third was the application of the health preference scores by CRN stages collected at baseline of the longitudinal study, in combination with survival data extracted from the literature in a Markov model on the cost-effectiveness of different CRC screening strategies (colonoscopy, guaiac and immunochemical fecal occult blood tests) in comparison to no screening in terms of quality-adjusted life-years gained. Psychometric analysis confirmed that FACT-C had satisfactory reliability, construct validity and responsiveness in Chinese patients Patients with CRN reported worse physical HRQOL but better mental HRQOL and similar health preference score compared to the general population. Disease severity indicated by tumor stage at initial diagnosis was the most significant determinant of HRQOL of CRN patients. Rectal cancer also significantly associated with a decrease in physical HRQOL and health preference scores. Markov modelling showed that immunochemical fecal occult blood (I-FOBT) yearly was the most effective and two-yearly was the most cost-effective screening strategy compared to no screening.

While Quality of Life (QOL) among aging populations has been attracting more and more attention in the last few years, little QOL research has been carried out among the elderly Chinese minority. This is a cross sectional study aimed at estimating the Quality of Life (QOL) and exploring QOL indicators among the elderly Chinese people living in Montreal. Forty-one volunteer study participants, 65 years or older were recruited from 12 Chinese community centers in Montreal. Their QOL was measured by the Ferrans and Powers Quality of Life Index (1985). The study results showed that participants generally described a very high level of life satisfaction. Living arrangements, gender, education and social support were found to be important QOL indicators. This study has important implications for health service and policy and raises the need for further future research using a larger random sample.

Background: Cancer is a family disease affecting the patient and family members. To date, few studies have explored the psychological distress of family caregivers and its relation with patient quality of life (QOL), particularly among culturally diverse cancer populations. This study seeks to understand the associations between patient distress, family caregiver distress and patient QOL in a Chinese-speaking population in British Columbia, Canada. Methods: A cross-sectional survey of Chinese-speaking (study population) and Anglophone (comparison group) patients (N = 55) and their family caregivers (N = 40) was used to examine patient QOL and correlates. Multiple imputation of missing data for incomplete dyads resulted in data for 29 Chinese-speaking and 28 Anglophone dyads. Multiple linear regression and mediation analyses examined predictors of QOL, and its domains, and mediation effects of patient distress and family caregiver distress. Semi-structured interviews with a subset of the Chinese-speaking survey participants comprising ten patients and six family caregivers, including five patient-family caregiver dyads, were also conducted. Analysis of culturally-embedded experiences of patients and family caregivers regarding their distress and QOL was conducted, to provide further context and explanation for the findings from the quantitative analysis. Results: Patient distress emerged as the key factor in explaining patient QOL. The effects of patient age on patient emotional well-being were mediated by patient distress, such that lower distress in older patients explained better emotional functioning. A key theme that surfaced from the qualitative data analysis was the emotional regulation occurring in the family when coping with cancer. The negative impact of family caregiver distress on patient emotional well-being was acknowledged by both the patients and the family caregivers. Younger and older patient interviewees alike indicated anxiety and worries about the impact of their cancer diagnosis on their families. Conclusions: This study provides insights to Chinese-speaking patients’ and family caregivers’ illness experience and the interrelatedness of the dyads’ responses to cancer. In expanding our knowledge of cross-cultural cancer care, these findings highlight the important role of cultural background in shaping service needs and, in turn, service delivery to cancer patients and their families.

Positive mood is believed to enhance spiritual quality of life (QoL). A mixed-methods research approach (with 4 phases) was used to prospectively examine the relationships between positive mood and spiritual QoL in Hong Kong Chinese adults (18+ years old) with different spiritual backgrounds (Taoism, Buddhism, Christianity, agnostics and atheists) during the Chinese New Year (CNY). Phase 1: a cross-sectional survey (pilot: N=68, main study: N=445) field-tested the spiritual-religious-person belief (SRPB) domain (9 facets) in the WHOQOL-SRPB instrument. Internal consistency (α=.92) and test-retest reliability (α =.97) were good. The spiritual domain has higher correlations than the other QoL domains with the overall spiritual well-being scores (r=.76) of the Spiritual Well-Being Scale and converged with the subscale indices religious well-being (r=.62) and existential well-being (r =.71). Phase 2: focus groups (pilot: N=8, main study: N=37) examined the subjects’ CNY experiences. Verbatim thematic analysis demonstrated that positive mood and spiritual QoL changes had been elicited in the past. Some respondents had a negative perception of the festival. Respondents who have negative experiences during the CNY should be identified in the subsequent phases of this study. Phase 3: it consisted of a three-wave longitudinal survey to investigate the relationships between mood and spiritual QoL scores 2 weeks before (n=528), 2 weeks after (n=457), and 8 weeks (n=206) after the CNY. The overall negative mood decreased immediately after the CNY (from 20.56, SD 6.83 to 19.54, SD 6.93, p=.049), while positive mood was increased only in the agnostic respondents (29.57, SD=6.63 to 31.24, SD=6.14, p=.003). Atheists had lower spiritual domain scores than the agnostic and religious subsamples. Unmet expectations had led to a decrease in spiritual QoL scores and an increase in negative mood. Phase 4: follow-up focus groups (N=16) revealed that moderated mood had initiated a positive reappraisal of the CNY experience. Consequently, three spiritual facets, peace, hope and meaning in life emerged in informants regardless of their positive or negative experiences. In conclusion, the findings supported a positive correlation between positive mood and spiritual QoL. This generated propositions regarding the pathways among spiritual activities, discrete mood and spiritual QoL for further testing. Second, the inclusion of the two spiritual facets, faith and connection, in the summated score of the SRPB domain of the WHOQOL instrument demands reconsideration.
