Immunization is a highly successful public health intervention providing protection against serious infectious diseases. UK vaccine uptake rates are generally high, although pockets of lower uptake and social inequalities remain which compromise herd protection. The child health programme provides health professionals opportunities to introduce immunization to parents, offer ongoing information and advice, and remind them when vaccines are due. Improving and maintaining high vaccine rates depends on multicomponent strategies. In view of their relationship with families, health visitors are the key professionals to work with them to ensure that children are immunized fully and in a timely fashion, although in practice they now do not usually deliver vaccines.

Comparative research has identified two broad types of child welfare system. In child protection systems the principal remit of welfare agencies is to identify and respond to actual or potential incidences of child abuse or maltreatment. In contrast family service systems are characterised by a stronger spirit of partnership between the state and families and an emphasis on working to prevent the need for coercive state intervention. This book examines the development of children’s services reform in England over recent decades to explain a shift from family service polices towards a narrower child protection approach. Successive waves of reform in England have invariably been framed as responses to high-profile child abuse inquires and media generated scandal including the cases of Victoria Climbié and Baby P. However, this book challenges the idea that it is the apparent failings of local agencies, including child and family social workers, that drive successive waves of reform. Instead, it turns the spotlight on the process of policy-making at the national level, and highlights the role played by party political leaders and senior government ministers in driving reform. The book is informed by 45 interviews with key decision-makers including ministers, senior civil servants, children’s charity leaders, local authority directors and social work researchers.

This chapter describes issues related to cognitive-behavioral group-based interventions for children with aggressive behavior problems. It describes beneficial aspects of group formats and notes the longer-term effects of the Coping Power program. It discusses potential problems in group-based interventions with aggressive children, including the potential for deviant peer effects and deviancy training. It then describes a study of Coping Power that randomly assigned schools to either group or individual formats for aggressive children. Results indicated significant reduction in behavioral problems for children in both conditions. However, teacher ratings indicated significantly stronger effects apparent by a 1-year follow-up for children in the individual sessions compared to the group format. The chapter discusses why these differences may have emerged and how group therapist behavior may be important in protecting against longer term follow-up deviant peer effects.

In child and adolescent mental health care, the competing goals of protecting young people as a vulnerable population and their increasing legal autonomy constitute a specific ethical problem. Improving care, assessment, and treatment interventions requires research. Research that includes this vulnerable underage population has to be minimally burdensome and harmful and requires the informed consent of both children and parents. Therefore, adherence to evidence-based interventions and weighing the risks and benefits of interventions are of utmost importance in child and adolescent psychiatry. While access to mental health care can vary widely, it is crucial for at-risk populations such as children from families of low socioeconomic status, children of the mentally ill, and, in particular, children in youth welfare systems.

This chapter explores the consequences of war on the development and mental health of adolescents. The discussion begins by framing prevention and intervention programs within the context of human security. The example of war-affected youth in Sierra Leone is used to illustrate the interrelatedness of the core security needs and rights of adolescents, using the SAFE model (safety/freedom from harm; access to basic physiological needs and health care; family and connection to others; and education/economic security) of child protection as a lens for analysis. In some respects, hard-of-hearing children experience the same difficulties as deaf children, whereas other challenges might be easier or more difficult to handle for the hard-of-hearing child than it would be for the deaf child. Research has revealed great variability in the language, academic, and psychosocial outcomes of hard-of-hearing children. Universal newborn hearing screening enables early identification and intervention for this group, which traditionally has been diagnosed rather late; however, best practices regarding the scope and content of early intervention have not yet been sufficiently described for hard-of-hearing children. This chapter summarizes the current knowledge concerning psychosocial development in hard-of-hearing children. Risk and protective factors, and their implications for early intervention, are discussed with a special emphasis on preschoolers.

Legislation is one of the most important tools for empowering children. Recent years have seen several key developments in the law, policy, and practice related to child rights. The book analyses the laws in the light of court judgments and policy initiatives taken in India. It also examines the interventions and strategies employed by non-governmental organizations in recommending legislative reforms in support of children. This fully revised third edition focuses on the new legal developments in India—such as the Juvenile Justice (Care and Protection of Children) Act, 2015; the new Central Adoption Resource Agency guidelines; the Right of Children to Free and Compulsory Education Act, 2009; and the National Food Security Act, 2013—thus attempting to integrate the law in theory and field practice. Child maltreatment is a significant global public health problem that impacts children’s health and mental health while young but also can follow them into adulthood, potentially carrying forward patterns of abusive parenting. To effectively manage and eliminate child maltreatment, a uniform definition of abuse and neglect must be developed for proper monitoring of prevalence. Reporting laws and protection of children should be followed with care, and evidence-based prevention strategies and interventions should be disseminated widely. At present, research on treatment of abuse and neglect has produced several models that are scientifically supported and rated as evidence based. Sufficient research has been conducted for the field to practice within the bounds of science. However, further research is needed on implementation of evidence-based treatments.

This chapter provides an overview of prevention and treatment interventions to promote positive parenting as a way to enhance children’s psychosocial functioning. Decades of research on parenting have shed light on its role in children’s development, and in particular, the influence of parenting on risk of (or protection from) children’s behavior difficulties. We begin with an empirical and conceptual rationale for parenting as a crucial influence on children’s healthy development and functioning. We discuss the mediating role of parenting in the relationship of contextual stressors to child adjustment. We review four key parent training interventions to improve parenting among populations at risk, with a focus on three broadly disseminated programs with strong bodies of empirical evidence for their efficacy and effectiveness. We conclude by discussing the accomplishments, challenges, and opportunities for parent training programs with regard to research and clinical practice.
