Hybrid dialysis involves combining peritoneal and hemodialysis (HD) in patients with end-stage renal disease. Its reported use is quite limited outside of Japan. We present a retrospective review of 18 patients at our center that received this therapy and describe their ultimate disposition. We observed that 39% of the population on hybrid dialysis ultimately transitioned to full in center HD, 28% continue until death, and 33% either transition to home HD or received a transplant. In our center, hybrid dialysis was successful as a bridging therapy or in balancing continuation of dialysis with quality of life among those with a limited prognosis.

During the COVID-19 pandemic, long-term care (LTC) centers have adopted a series of measures that have affected the physical and cognitive health of patients. The routines of the patients, as well as the interventions of professionals, have been altered. In the case presented here, our aim was to explain the effect that the strong confinement due to the spread of the first COVID-19 wave in Spain had on a 75-year-old resident in an LTC center, with cognitive and behavioral symptomatology compatible with a diagnosis of mixed dementia, as well as the measures that the center adopted to manage the lockdown situation in the best possible way, including personalized attention protocols and a video call program. Different nosological hypotheses are also raised using a semiological analysis, including the analysis of the initial and continuation diagnostic protocols, as well as the therapeutic options.

Idiopathic pulmonary fibrosis (IPF), a fibrosing interstitial lung disease, predominantly affects the elderly and is associated with a high mortality risk. Nintedanib, a tyrosine kinase inhibitor, significantly reduces IPF progression. However, data on the tolerability and efficacy of nintedanib in the elderly with IPF are limited. Therefore, this study aimed to examine the tolerability and efficacy of nintedanib in the elderly with IPF in a real-world setting. Medical records of 19 elderly IPF patients (≥ 75 years) and 46 non-elderly IPF patients (< 75 years) newly administered nintedanib were retrospectively analyzed. We compared the forced vital capacity (FVC) level, incidence and severity of adverse events, and continuation rates of nintedanib between the two groups. FVC and percent predicted diffusing capacity of the lung for carbon monoxide (DLco) were lower in the elderly IPF group at baseline. Although the elderly IPF patients had a significantly higher incidence of adverse events, such as diarrhea, nausea, and elevation of hepatic enzymes, the rate of discontinuation of nintedanib owing to adverse events was not different between the groups. The continuation rates of nintedanib treatment at 6 months and 1 year in the elderly IPF group were equivalent. Furthermore, there was a similar trend in the reduction of the annual FVC decline after nintedanib initiation between the groups. Our study demonstrated that nintedanib was tolerable in both the IPF patient groups in a real-world setting. Proper management of adverse events in the elderly with IPF would lead to a better clinical outcome.

Background: This study investigated the safety of endoscopic sphincterotomy in patients undergoing antithrombotic treatment. Methods: From January 2014 to December 2016, a single-center retrospective study was conducted. Of the 80 patients with naïve papilla receiving antithrombotic treatment who underwent endoscopic sphincterotomy, 76 patients were retrospectively analyzed. We divided the participants into two groups as follows: 45 patients who discontinued antithrombotic treatment (discontinuation group) and 31 patients who continued antithrombotic treatment (continuation group). We evaluated the safety of endoscopic sphincterotomy in patients with naïve papilla who received antithrombotic treatment. Results: The percentage of patients requiring emergency endoscopic retrograde cholangiopancreatography in the continuation group was significantly higher than that in the discontinuation group (55% vs 11%; p = 0.001). The incidence of adverse events did not differ significantly between the two groups. Neither bleeding nor perforation occurred in either group. The length of hospital stay did not differ significantly between the two groups. Conclusions: Endoscopic sphincterotomy in patients undergoing antithrombotic treatment may be safe if the guidelines for gastroenterological endoscopy in patients undergoing antithrombotic treatment are followed.

Background: This study aimed to determine the effectiveness of 36 hospital learning centers for the continued education of sick children using electronic distance learning television (eDLTV), parents’ attitudes toward their child’s illnesses and education, and service satisfaction of the centers. Methods: The sample included 4,430 children aged 4-18 years old with common illnesses, chronic illnesses and developmental disorders, as well as 4,430 parents who had taken care of the child for at least 6 months. The methods included attitude surveys, which were analyzed using chi-square tests and t-tests. Results: The factors associated with education continuation of the children were illness types (parents were less worried about children with common illness and more concerned about education of children with chronic diseases and children with disabilities), distance from home to school, transportation type, parents’ education level, marital status, and family income. About 99.8% of patients with common illnesses continued their education, followed by 99.3% of disabled children, and 95.9% of chronic patients. Satisfaction score towards the services at the learning centers were high (mean scores: 4.28 and 4.43 respectively, out of 5 = strongly satisfied). Conclusion: After completing an education program through eDLTV at a center, a total of 97.7% of children continued their education and were highly satisfied with the service at the center. Parents had positive attitudes towards their child’s illnesses and education.

Recently published work has revealed that the dire consequences of Covid-19 have affected different life sectors worldwide, especially education. Supporting the ICT centers will enforce the infrastructure; this will lead to strong remote learning during crises. This study encouraged the ICT center to practice the professors and support both teachers and students in using digital tools such as mobile in their learning. For this study, it was interesting to identify the technical support types provided by the ICT centers in selected Palestinian Universities for lecturers. Therefore, we analyzed the possibility of improving ICT centers' quality and investigated whether we can enrich the outcomes of ICT centers from the lecturers' perspective. For data collection, online interviews were conducted with 8 participants from different Palestinian Universities. We seek an overarching framework according to the grounded theory. Slightly superior results are achieved with our framework. We found that the types of technical services from the ICT center at Palestinian universities were graded in time according to the motives of the need. The paper concludes by arguing that universities should hone lecturers' ICT app skills. These findings provide a potential mechanism for lecturers to enhance their ability to employ online tools appropriately in teaching to ensure the smooth continuation of the learning process in Palestine.

Abstract Background Isavuconazonium is an appealing anti-fungal due to its broad spectrum of activity, predictable pharmacokinetics, oral bioavailability, and lack of QTc prolongation, but real-world experience with it is limited. At our academic medical center, isavuconazonium is restricted to the infectious diseases (ID) service for treatment of invasive fungal infections, including endemic mycoses due to high prevalence, and is recommended for patients intolerant of first-line agents. The purpose of this study was to describe isavuconazonium use at our institution and assess adherence to its formulary guidelines. Methods Inpatients with an order for isavuconazonium between June 2016 and November 2018 were analyzed via retrospective chart review. Prescribing team, indication, and rationale for use were evaluated. Results There were 97 inpatient encounters with an isavuconazonium order among 57 patients. Of those, 30 were solid-organ transplants and 9 had bone marrow transplants. Indications for isavuconazonium were: histoplasmosis 25%, high-risk fungal prophylaxis 21%, invasive aspergillosis 9%, candidiasis 2%, and other 44% (Table 1). Preceding anti-fungal therapy included: voriconazole 49%, posaconazole 12%, fluconazole 9%, micafungin 7%, amphotericin B 5%, itraconazole 4%, and none 35%. The rationale for the use of isavuconazonium is described in Table 2. ID consultation occurred in 79% of patients. Those without a consult had an indication of prophylaxis or were continuation of therapy started outpatient or at an outside hospital (OSH). Conclusion Histoplasmosis was the most common infection treated with isavuconazonium, despite limited data for that indication. Further investigation of the clinical course for these patients is warranted. Reasons for use most commonly centered on concern for QTc prolongation, clinical failure, and drug interactions. Over one-third of patients received no anti-fungal therapy prior to initiation. Adherence to required ID consultation was high. Patients on isavuconazonium for prophylaxis or as continuation therapy without a consult may still benefit from ID review to assess the appropriateness of therapy. Abstract Space potential field continuation is a method frequently used for the spatial transformation of magnetic anomaly in the sea area, and is the key technology for constructing three-dimensional marine magnetic field model and the application of marine magnetic survey data. In this study, we analyze the principle of upward continuation of potential field from plane to plane, demonstrate the spatial domain algorithm and frequency domain algorithm, and propose the standards for evaluating the accuracy of the two algorithms. Using the theoretical data of the multi-dipole sphere model, we employ the two algorithms to continue the magnetic anomaly of the observation plane at the height 0 m to 100 m, 200 m, and 300 m, for noisy and noise-free conditions. The results show that the continuation error of the two methods is extremely small at the plane center, but large on the edge; the spatial domain algorithm has obviously better accuracy than the frequency domain algorithm, so it should be preferentially selected in high-precision applications, such as magnetic anomaly detection in the sea area.

This thesis presents a bifurcation and numerical continuation study into the occurrence of shimmy instability in an aircraft main landing gear (MLG) of single side-stay, dualwheel design. The dynamics are expressed in terms of three rotational degrees of freedom (Dofs) aligned with the side-stay plane, and a fourth translational DoF representing compression of the main strut. These DoFs are modelled by oscillators that are coupled directly through the geometric configuration of the system, as well as through the tyre/ground interface. Using this representation of the MLG system we focus in this thesis on the nonlinear effects of geometric orientation and mechanical freeplay employing bifurcation analysis techniques to highlight their effects on the MLG stability. First, a changing side-stay orientation angle is investigated. Shimmy is studied by means of a two-parameter bifurcation analysis in terms of the landing gear forward velocity and loading force. For this a three-DoF model is used that does not include axial compression. This formulation along with suitable parameters allows for comparison with the existing literature, and an agreement is demonstrated with previous results for a zero side-stay angle. Subsequent variation of this angle is explored and a consistent picture presented, capturing the (transition of the two-parameter bifurcation diagram us a function of this angle. This shows a considerable increase in the complexity of the dynamics for intermediate side-stay angles. The appearance of an additional shimmy mode is observed and a region of tri-stability found where three distinct shimmy types coexist. For the study of freeplay the MLG model is extended to include axial compression; this axial DoF is required to accurately represent freeplay, introduced to the torque links of the system. Parameter values are chosen here to represent a typical mid-range civil aircraft MLG. The addition of freeplay is shown to result in shimmy oscillations that occur within the MLC operating envelope; their properties depend on both the size and 'shape' of freeplay. Freeplay and geometric coupling arc also considered together via consideration of a non-zero side-stay angle. Here, additional dynamic complexity is introduced in the presence of freeplay and, again , this coincidence with the appearance of a new shimmy mode. Further complex phenomena also appear, including multiple- frequency and chaotic-type oscillations, as well as complex transients. The non-zero geometry produce asymmetry and this results in a great sensitivity of the small-amplitude MLG behaviour to the exact shape of freeplay. Therefore, geometric orientation and freeplay a.re found to have significant effects and, when combined, they work together to produce additional complex phenomena, not otherwise observed when considered in isolation.

We hope that this study is of interest not only in the context of the nonlinear Schrödinger equations but also separately as a study of an efficient numerical algorithm for continuing (path-following) solutions to general two-dimensional periodic soliton bearing PDEs.

A Hamiltonian treatment for the motion of a charged particle in a toroidal magnetic field is given. Assuming the plasma equilibrium, Boozer coordinates are used. The Hamiltonian of the exact trajectory is given in the guiding center coordinates. The higher order corrections to the standard drift Hamiltonian are derived. It is shown that the exact Hamiltonian depends on both the field strength and the shape of the magnetic surfaces (the metric of Boozer coordinates) while the standard drift Hamiltonian depends only on the field strength. The first order correction to the standard drift Hamiltonian, in gyroradius to system size, depends in a generic way on the shape of the magnetic surface, as does the exact Hamiltonian. Numerical calculations are done to compare the trajectories of the exact Hamiltonian, the standard drift Hamiltonian and the higher order drift Hamiltonian for a quasihelical symmetric vacuum field. The numerical results show that the difference in phase space structure between the exact and standard drift Hamiltonian can be predicted by the drift Hamiltonian with the first order correction.

Information Technology (IT) equipment compaction has become a significant air cooling challenge at the data center level. Computational Fluid Dynamics and Heat Transfer (CFD/HT) models have been employed as the dominant technique for the design and optimization of both new and existing data centers. Understanding the limitations of a CFD/HT models’ ability to predict the actual data center temperature field and flow characteristics becomes a critical component in optimizing the actual data center rather than optimizing the model of the data center. This is most important near the IT equipment where temperature and flow specifications from the IT equipment manufacturers must be maintained for reliable operation. This study is a continuation of earlier comparisons of CFD/HT models to experimentally measured temperature and flow fields in a small data center test cell. This study compares the experimentally collected data for three different layouts of perforated tiles to a CFD/HT model with seven turbulence models not previously evaluated. Insight into the location of the deviation between the different turbulence models and experimental data are discussed, along with the computational effort involved in running the CFD/HT models. It was found that the zero equation (or mixing length model) and the Spalart-Allamaras turbulence models produced the smallest deviations from experimental data, but the former required only a fifth of the computational effort of the latter. The laminar flow model required the least computational effort, running more than twice as fast as the zero equation turbulence model, and produced deviations similar to those of the six different k-ε turbulence models.

As the performance of Information Technology (IT) equipment continues to rise, so do the power dissipated and overall power density. Air cooling this increasing power has proved a significant challenge even at the data center level. The dynamics of passenger aircraft on the ground are influenced by the nonlinear characteristics of several components, including geometric nonlinearities, the aerodynamics and interactions at the tyre-ground interface. We present a fully parametrised mathematical model of a typical passenger aircraft that includes all relevant nonlinear effects. The full equations of motion are derived from first principles in terms of forces and moments acting on a rigid airframe, and they include implementations of the local models of individual components. The overall model has been developed from and validated against an existing industry-tested SimMechanics model. The key advantage of the mathematical model is that it allows for comprehensive studies of solutions and their stability with methods from dynamical systems theory, in particular, the powerful tool of numerical continuation. As a concrete example, we present a bifurcation study of how fixed-radius turning solutions depend on the aircraft’s steering angle and centre of gravity position. These results are represented in a compact form as surfaces of solutions, on which we identify regions of stable turning and regions of laterally unstable solutions. The boundaries between these regions are computed directly and they allow us to determine ranges of parameter values for safe operation. The robustness of these results under the variation of an additional parameter, specifically, the engine thrust is investigated. Qualitative changes in the structure of the solutions are identified and explained. Overall our results give new insight into the possible turning dynamics of the aircraft in dependence on three parameters of operational relevance.

Abstract Labyrinth seals are frequently used in modern turbomachinery in order to control leakage flows between static and rotating elements. Previous experiences have demonstrated that flutter is a potential risk for them and can lead to high cycle fatigue life reduction or even mechanical failure. This aeroelastic instability depends on the interaction between aerodynamics and structural dynamics and it is hard to predict accurately due to its sensitivity to the details during operation. In 2014, as part of the E-Break European Union research project, an experimental campaign aiming to study the phenomenon was conducted at Centro de Tecnologías Aeronáuticas. Three realistic seal geometries were tested at a comprehensive set of operating conditions and all of them exhibited flutter at some point. These seals will be the object of study of this work. This document is a continuation of a previous publication of the authors where the E-Break campaign was reproduced with a methodology based on Computational Fluid Dynamics simulations. The simulations showed a good agreement with experimental data and revealed the importance of accurately predicting the actual fin gaps during operation and the influence of external cavities. Here a new approach to study the stability of the seals is followed, consisting in the use of a simplified analytical model based on the works by Corral, Vega and Greco. The model is the result of solving a linearized version of the governing fluid equations under certain simplifying hypothesis. The stability results obtained are compared against both the available experimental data and the aforementioned high-fidelity simulations, showing a fairly good agreement at a vastly reduced computational cost. However, the main limitation of the model arises in those cases where the stability is dominated by the external cavities, as the simplified model is not able to account for their effect.

The most common surgical procedure used to treat right ventricular heart failure is the Fontan procedure, which connects the superior vena cava and the inferior vena cava directly to the left and right pulmonary arteries bypassing the right atrium. Many studies have been performed to improve the Fontan procedure. Research has been done on a four-way connector that can both passively and actively improve flow characteristics of the junction between the Superior Vena Cava (SVC), Inferior Vena Cava (IVC), Left Pulmonary Artery (LPA) and Right Pulmonary Artery (RPA), using an optimized connector and dual propeller system. However, the configuration of these devices do not specify propeller motor placement and has a stagnation point in the center of the connector. This study focuses on creating a housing for the motor in the center of the connector to reduce the stagnation area and further stabilize the propellers. To do this, we created a program in ANSYS that utilizes the design-of-experiment (DOE) function to minimize power-loss and stagnation points in the connector for a given geometry. First, a CFD model is created to simulate the blood flow inside the connector with different housing geometries. The shape and size of the housing are used as parameters for the DOE process. In this study, an enhanced central composite design technique is used to discretize the design space. The objective functions in the DOE are red blood cell residence time and power loss. It was confirmed that the addition of the housing did decrease the size of the stagnation point. In fact, the housing added in stabilizing the flow through the connector by creating a more defined flow path. Because the flowrates from the IVC and SVC are not the same, the best configuration for the housing was found to be asymmetric along the axis of the pulmonary artery. While this is a continuation of previous studies, the creation of an optimized housing for the motors for the propellers makes implementation of the propeller idea more viable in a real life situation. The added stability of the propellers provided by the housing can also decrease the risk of propeller failure due to rotordynamic instability.

The representative of the Chagatai language, which developed as a continuation of Khwarazm Turkish, is Zahirad-Din Muhammad Babur's "Baburnama", which contributed to the Chagatai language and literature after the great literary and linguistic scholar Alisher Navoi. In Turkey, it is known as Babur's memoirs about his own life, which received worldwide attention. This work is known as Vekāyi', Vakıanama, Vakıat-ı Baburi, Vekayi'nama-i Padishahi and Baburiyye besides Baburnama. In the developed scientific centres of the world, in Uzbekistan, Turkey and some European countries, when they talk about Baburname, they compare it with the works of Western historical figures. Those whosay “Baburnama” is a unique work" are not at all few. Abstract Nuclear reactor designs are governed by postulated accident events that may occur during their operational lifetime. One type of incident is a reactivity-initiated accident (RIA), during which a sudden surge of power in the fuel components within the core may result in the latter exceeding its cooling capabilities. This could lead to a departure from nucleate boiling (DNB) event which results in a significant decrease in heat transfer capabilities. Preventing the occurrence of a DNB crisis requires a fundamental understanding of the cladding-to-coolant heat transfer under fast transient conditions, as well as the governing hydrodynamic and design parameters that influence when the critical heat flux (CHF) will be exceeded. Presently, large uncertainties in computer models used to predict CHF have led to conservative safety limits governing light-water reactor (LWR) designs. The Idaho National Laboratory (INL) is currently leading a combined effort that takes advantage of the restart of the Transient Reactor Test (TREAT) facility, to better understand the mechanism of CHF under in-pile pool boiling conditions. The goal of this laboratory directed project is to use the unique capabilities of TREAT coupled with a non-fueled nuclear heated borated stainless-steel 304 tube experiment within an experimental capsule. The borated tube will induce CHF in the surrounding coolant when subjected to a power pulse within the TREAT. The impacts of rapid surface heating effects as well as radiation-induced surface activation (RISA) will be experimentally investigated. This feature is a continuation to previous thermal hydraulics analysis that was conducted to inform on a test matrix for the design of the borated heater experiment. The borated tube was used in place of a solid rod so that the center axial region can be instrumented to allow for better experimental analysis. Therefore, it is desirable to design this rodlet so that the maximum heat flux occurs at the center of the axial length of the rod. The work presented here analyzes the potential to integrate axial boron gradients within this tube to shape its power curve. Several generic axial power shapes were initially considered. Natural boron concentrations between 0.1–2.0 wt.% were analyzed and a power coupling factor (PCF) was calculated for each. A self-shielding study was conducted to develop radial power profiles for several boron concentrations. These were then applied to three different power pulses to determine how these two parameters influence the chosen axial heat flux curve. Variations in the initial coolant temperature were investigated. Lastly, how the shape of the generic curve is affected following a DNB event was also studied. Two different CHF cases were included within the scope of this analyses; one during which CHF was exceeding along the entire axial region of the rod, and another where the former occurred at the center region only. The behavior of the curve overtime was investigated.

В ней рассматривается один из самых дискуссионных элементов программы резного декора собора - рельефы женских ликов на консолях фасадных полуколонн. Автор представляет новую интерпретацию роли этих рельефов в архитектуре и декоре храма и демонстрирует развитие семантики этого сюжета в традиции владимиро-суздальского белокаменного зодчества и фасадной резьбы. Результаты исследования дают дополнительную информацию для реконструкции первоначальных архитектурных форм Рождественского собора XIII в. и его фасадного декора. The article is a continuation of a number of studies devoted to the white stone carving of the Suzdal Nativity Cathedral. It considers one of the most debatable elements of the program of carved decor of the Cathedral - reliefs of female faces on the consoles of facade semicolumns. The author presents a new interpretation of the role of these reliefs in the architecture and decor of the temple and demonstrates the development of the semantics of this plot in the tradition of Vladimir-Suzdal white stone architecture and facade carving. The results of the study provide additional information for the reconstruction of the original architectural forms of the Nativity Cathedral of the XIII century and its facade decoration.

This study pleads for the use of the term “integral linguistics” for the theory of language created and developed by the Romanian linguist Eugenio Coseriu starting with the middle of the twentieth century. The term was proposed by Coseriu himself only in 1981, when he contributed in the second edition of the National Congress of Linguistics in San Juan with a presentation named “Fundamentas y tareas de la lingüistica integral (Basis and tasks of integral linguistics)”. The term was not so much used in the world of linguistics, except for some of Eugenio Coseriu’s disciples, who clearly understood the amplitude of his vision on language (like Johannes Kabatek in Germany or Mircea Borcilă and all his followers in the linguistic school from Cluj-Napoca, Romania). The rationale for promoting the name of “integral linguistics” for the theory created by Eugenio Coseriu is based on some arguments listed and detailed in this study: integral linguistics has a unitary object, the language as a cultural object, studied from all relevant perspectives; it is based on a solid and well integrated philosophy of language (with ideas from Aristotle, Humboldt and Hegel and many other philosophers fundamenting Coseriu’s vision on language); it includes most relevant ideas from linguists who preceded Coseriu’s theory, in a constant integrative effort; it relates to other theories of language, clarifying its position in connection with different lines of study opened especially in the second half of the twentieth century; it proposes extremely valuable instruments and concepts adequate for the study of language and it offers the perspective of continuation and development of the study of language.
