Was ist neu? Epidemiologie Die Häufigkeit der akuten Nierenschädigung (ANS) nimmt auf gesamteuropäischer Ebene zu. Dabei lässt sich weltweit keine signifikante Verbesserung der Prognose feststellen. ANS-Diagnosekriterien und neue Indikatoren der ANS Trotz intensiver Bemühungen konnte bislang kein Marker identifiziert werden, welcher die Serumkreatininkonzentration als ANS-Indikator ersetzen könnte. Mutmaßlich ist eher mit der Etablierung eines diagnostisch-prognostischen Marker-Panels bei ANS zu rechnen. Konservative Maßnahmen mit gesichertem Nutzen Die derzeit verfügbaren konservativen Therapieempfehlungen sind in den letzten Jahren kaum durch neue Maßnahmen ergänzt worden. Kristalloide Volumenpräparate sind kolloidalen Lösungen meist vorzuziehen. Zudem sind balancierte Elektrolytlösungen Chlorid-reichen Präparationen bei drohender/manifester ANS wahrscheinlich überlegen. Im Rahmen einer jeden Volumentherapie ist eine Hyperhydratation betroffener Patienten jedoch zu vermeiden.

Penelitian ini bertujuan untuk mengisolasi dan mengidentifikasi jamur endofit pada tumbuhan paku Asplenium nidus. Metode yang digunakan dalam penelitian ini adalah isolasi jamur endofit, pemurnian jamur endofit, dan identifikasi jamur endofit secara makroskopik dan mikroskopik. Hasil isolasi dan pemurnian jamur endofit diperoleh lima isolat jamur endofit yaitu AN1, AN2, AN3, AN4 dan AN5. Teridentifikasi sebagai genus Gliocladium, Neoscytalidium, Humicola, Phialophoroides dan Aspergillus.This study aims to isolate and identify endophytic fungi in Asplenium nidus. The method used in this research is endophytic fungal isolation, purification of endophytic fungi, and identification of macroscopic and microscopic endophytic fungi. Results of isolation and purification of endophytic fungi obtained five endofit fungal isolates are AN1, AN2, AN3, AN4 and AN5. Identified as genus Gliocladium, Neoscytalidium, Humicola, Phialophoroides and Aspergillus.

In crickets ( Teleogryllus oceanicus), the paired auditory interneuron Omega Neuron 1 (ON1) responds to sounds with frequencies in the range from 3 to 40 kHz. The neuron is tuned to frequencies similar to that of conspecific songs (4.5 kHz), but its latency is longest for these same frequencies by a margin of 5–10 ms. Each ON1 is strongly excited by input from the ipsilateral ear and inhibits contralateral auditory neurons that are excited by the contralateral ear, including the interneurons ascending neurons 1 and 2 (AN1 and AN2). We investigated the functional consequences of ON1's long latency to cricket-like sound and the resulting delay in inhibition of AN1 and AN2. Using dichotic stimuli, we controlled the timing of contralateral inhibition of the ANs relative to their excitation by ipsilateral stimuli. Advancing the stimulus to the ear driving ON1 relative to that driving the ANs “subtracted” ON1's additional latency to 4.5 kHz. This had little effect on the spike counts of AN1 and AN2. The response latencies of these neurons, however, increased markedly. This is because in the absence of a delay in ON1's response, inhibition arrived at AN1 and AN2 early enough to abolish the first spikes in their responses. This also increased the variability of AN1 latency. This suggests that one possible function of the delay in ON1's response may be to protect the precise timing of the onset of response in the contralateral AN1, thus preserving interaural difference in response latency as a reliable potential cue for sound localization. Hyperpolarizing ON1 removed all detectable contralateral inhibition of AN1 and AN2, suggesting that ON1 is the main, if not the only, source of contralateral inhibition.

Abstraction hierarchies (AHs) are essential to a work domain analysis (WDA), the “most important and unique” phase of cognitive work analysis (CWA) (Vicente, 1999). Although AHs have been the industry standard for assessing and describing work systems for several decades, they are not without limitations (Vicente, 2002). We have developed an evolution of AHs called Abstraction Networks (ANs) to address several of the limitations Vicente identified. ANs are designed to (1) improve engagement with practitioners, thereby facilitating greater shared understanding of the system, (2) better integrate with existing systems analysis tools, and (3) provide a more direct bridge between systems analysis and display design. We document a case study that uses ANs to better understand a sterile processing department (SPD) at a Southeastern tertiary care hospital, detailing the extent to which AN’s achieve these three goals.

Renal cell carcinoma (RCC) is one of the top 10 cancers in the United States. This study assessed RCC health disparities in American Indians/Alaska Natives (AIs/ANs) and Hispanic Americans (HAs) focusing on advanced-stage and mortality. RCC patients’ data were obtained from the National Cancer Database (NCDB) and Arizona Cancer Registry (ACR). Logistic and Cox regression analyses were performed to ascertain the effect of race/ethnicity on stage and mortality, adjusting for neighborhood socioeconomic factors, rural/urban residence pattern, and other factors. In both data sets, AIs/ANs had significantly increased odds of advanced-stage RCC in the unadjusted model, but not in adjusted models. Mexican Americans had higher odds of advanced-stage compared to non-Hispanic Whites in NCDB (OR 1.22, 95% CI: 1.11–1.35) and ACR (OR 2.02, 95% CI: 1.58–2.58), even after adjusting for neighborhood characteristics. AIs/ANs did not show increased mortality risk in NCDB after adjusting for neighborhood characteristics, while the association remained significant in ACR (HR 1.33, 95% CI: 1.03–1.72). The great risk of all-cause and RCC-specific mortality was observed in U.S.-born Mexican Americans in Arizona (HR 3.21, 95% CI: 2.61–3.98 and sub-distribution HR 2.79, 95% CI: 2.05–3.81). RCC disparities in AIs/ANs is partially explained by neighborhood factors, but not in HAs.

Zusammenfassung Hintergrund Physikalische Maßnahmen oder manualtherapeutische Techniken (MTTe) wie Mobilisationen, Manipulationen oder Weichteiltechniken führen zu einer Verbesserung des Metabolismus oder einer Senkung hypertoner Muskulatur und werden auch zur Balanceregulierung bei zentralnervösen Veränderungen des autonomen Nervensystems (ANS) eingesetzt. Bisher fehlen empirische Erkenntnisse über Wirkungsmechanismen und Reizorte von MTTe auf das ANS. Das Ziel dieses vorliegenden Scoping Reviews war es, einen Überblick zu geben über den Erkenntnisstand der Anwendung von MTTe auf diverse Niveaus der Wirbelsäule auf das ANS. Methode Als Grundlage für die Durchführung des Scoping Reviews dienten die Datenbanken CENTRAL, Osteopathic Research Web, PEDro und PubMed. Umfang und Inhalte der Literatur wurden dokumentiert. Die Ergebnisse der einbezogenen und herangezogenen Studien wurden in narrativer Weise zusammengefasst, wobei der Fokus auf den signifikantesten klinischen Aspekten lag. Ergebnisse Manipulationen, Mobilisationen, myofasziale Techniken und zervikale Traktionen wurden als MTTe definiert. In 27 von 35 Studien wurden gesunde Probanden therapeutisch behandelt. Zehn Studien analysierten unmittelbare Effekte an Patienten, während 2 Studien als Longitudinalstudie bei Bluthochdruckpatienten konzipiert waren. In einem Zeitraum von 4–8 Wochen betrug die Interventionshäufigkeit wöchentlich zwischen einer und 3 MTTe-Einheiten. Schlussfolgerung Die Studienergebnisse erweisen sich als heterogen. Aus diesem Grund lassen sich keine verbindlichen, eindeutigen und allgemeingültigen Aussagen ableiten, in welcher Form, Intensität sowie in welchem Umfang MTTe angewendet werden sollen, um gezielt positive Wirkungsmechanismen am ANS in Gang zu setzen. Für zukünftige Studien sind demzufolge Longitudinalstudien mit Follow-up empfehlenswert. Darüber hinaus sollten umfassende Effekte von MTTe bei Patientengruppen mit unterschiedlichen Ausprägungen untersucht werden.

Racial/ethnic minority groups have a disproportionate burden of kidney cancer. The objective of this study was to assess if race/ethnicity was associated with a longer surgical wait time (SWT) and upstaging in the pre-COVID-19 pandemic time with a special focus on Hispanic Americans (HAs) and American Indian/Alaska Natives (AIs/ANs). Medical records of renal cell carcinoma (RCC) patients who underwent nephrectomy between 2010 and 2020 were retrospectively reviewed (n = 489). Patients with a prior cancer diagnosis were excluded. SWT was defined as the date of diagnostic imaging examination to date of nephrectomy. Out of a total of 363 patients included, 34.2% were HAs and 8.3% were AIs/ANs. While 49.2% of HA patients experienced a longer SWT (≥90 days), 36.1% of Non-Hispanic White (NHW) patients experienced a longer SWT. Longer SWT had no statistically significant impact on tumor characteristics. Patients with public insurance coverage had increased odds of longer SWT (OR 2.89, 95% CI: 1.53–5.45). Public insurance coverage represented 66.1% HA and 70.0% AIs/ANs compared to 56.7% in NHWs. Compared to NHWs, HAs had higher odds for longer SWT in patients with early-stage RCC (OR, 2.38; 95% CI: 1.25–4.53). HAs (OR 2.24, 95% CI: 1.07–4.66) and AIs/ANs (OR 3.79, 95% CI: 1.32–10.88) had greater odds of upstaging compared to NHWs. While a delay in surgical care for early-stage RCC is safe in a general population, it may negatively impact high-risk populations, such as HAs who have a prolonged SWT or choose active surveillance.

Acute mountain sickness (AMS) is a common syndrome characterized by headache, dizziness, loss of appetite, weakness, and nausea. As a major public health issue, obesity has increased in high altitude urban residents and intermittent commuters to high altitudes. The present study investigated acute hypoxic exposure and hypoxic exercise on hypoxemia severity and AMS symptoms in a physically active obese man. In this case analysis, peripheral oxygen saturation (SpO2) was used to evaluate hypoxemia, heart rate (HR) and blood pressure (BP) were used to reflect the function of autonomic nervous system (ANS), and Lake Louise scoring (LLS) was used to assess AMS. The results showed that acute hypoxic exposure led to severe hypoxemia (SpO2 = 72%) and tachycardia (HRrest = 97 bpm), and acute hypoxic exercise exacerbated severe hypoxemia (SpO2 = 59%) and ANS dysfunction (HRpeak = 167 bpm, SBP/DBP = 210/97 mmHg). At the end of the 6-h acute hypoxic exposure, the case developed severe AMS (LLS = 10) symptoms of headache, gastrointestinal distress, cyanosis, vomiting, poor appetite, and fatigue. The findings of the case study suggest that high physical activity level appears did not show a reliable protective effect against severe hypoxemia, ANS dysfunction, and severe AMS symptoms in acute hypoxia exposure and hypoxia exercise.

Zusammenfassung. Die Messung der Herzfrequenzvariabilität (HRV) erlaubt Rückschlüsse auf die Aktivität des autonomen Nervensystems (ANS). Eine autonome Dysbalance (AD) findet sich als gemeinsame Komponente zahlreicher Erkrankungen. Vielfach geht diese einer Erkrankung voraus und korreliert mit dem Therapieansprechen. Sie hat damit neben dem pathologischen auch einen prädiktiven Wert. Daneben stellt die Herzfrequenzvariabilität (HRV) in Ruhe ein psychophysiologisches Phänomen mit breiter Aussagekraft dar. Psychisches Erleben wirkt sich über das ANS auf die physiologische Homöostase, einschliesslich Immunprozesse, aus, was psychosomatische Effekte biologisch untermauert und durch HRV messbar macht. Das autonome Nervensystem (ANS) als Schnittstelle dieser psychophysiologischen Regulation gewinnt durch die HRV-Forschung zunehmend Bedeutung und ermöglicht ein besseres Verständnis der Zusammenhänge zwischen Psyche, Lebensstil, autonomer Regulation und chronischen körperlichen Erkrankungen. Sie bedingt eine systemische, Organ-übergreifende Sichtweise sowie eine Orientierung an langfristigen Prozessen. Die Beurteilung und Normalisierung der AD stellt dadurch eine neuartige therapeutische Strategie dar, von der sich zahlreiche Interventionen und Lebensstilmodifikationen ableiten lassen. So erhalten Interventionen, die den Vagotonus stärken (aerobes bzw. moderates körperliches Training, Relaxationstechniken, Vagusstimulation etc.), eine zunehmende Bedeutung. Gegenüber der einfachen nicht-invasiven Messung und Beliebtheit als Forschungsinstrument steht die Komplexität der Interpretation und die zurückhaltende Umsetzung in der klinischen Praxis. Nichtsdestotrotz existieren Guidelines und Normwert-Sammlungen, derer sich der Anwender bedienen kann.
