In the early 1980's and 1990's, companies began to build upon the principles of Total Quality Management and developed there own unique quality systems. The most popular and well known of these systems is Six Sigma that was developed by Motorola and successfully adopted by others such as Allied Signal (now Honeywell) and most notably, General Electric. Six Sigma can be characterized as a highly formalized, process oriented improvement tool that is data focused. The Six Sigma process is normally performed by a diverse team, who attack a quality/process problem by analyzing process variation or in statistical terms, sigma. The foundations of Six Sigma are commitment from upper management, detailed training and a regimented diagnostic approach. Another quality operating system is the less known, but very successful, Achieving Competitive Excellence (ACE) operating system. This system was developed and is practiced by United Technologies Corporation (UTC). The ACE system is broader based than the Six-Sigma approach, however, ACE is not as data oriented as the Six Sigma approach. ACE revolves around the three principle categories of process improvement and waste elimination tools, decision-making tools, and problem solving tools. These tools impact issues as diverse, but not limited to, factory floor cleanliness, market feedback analysis, machine tool preventative maintenance and set up reduction. ACE is a combination of lean manufacturing and quality improvement philosophies. This paper provides an analysis of both the Six Sigma and ACE Quality Operating Systems. In the paper the systems are compared and contrasted. Further, strengths and weaknesses of each system are discussed. In particular, the analysis focuses on how ACE can leverage elements
(cont.) and aspects of Six Sigma. The analysis concludes that there are elements of Six Sigma that would benefit ACE. The paper identifies that the strength of Six Sigma's statistical approach and its positive impact on process certification could be beneficially applied to the ACE system. Further, there are recommendations for UTC to place more of an emphasis on ACE training and to accelerate its current efforts to better link quality and lean improvement to product engineering and design.

Twenty years ago, a seminal study on adverse childhood experiences (ACEs) and subsequent increased health risks catapulted ACEs into the zeitgeist of research and application. Though a validated construct, the questionnaire, particularly the child sexual abuse (CSA) item is not without limitation and yet is used by the Centers for Disease Control and state agencies to quantify need and allocate resources to services accordingly. Currently, CSA is counted only when the perpetrator is 5-years or older than the victim. This requirement makes neglect of sibling and peer assault very likely. Accordingly, this study aimed to assess whether individuals with CSA experiences within an age gap smaller than 5 years are missed by the 5-year modifier embedded in CSA assessment wording and whether this missed group would otherwise qualify for services if detected. The study also aimed to assess whether this missed group has equivalently poor health outcomes to CSA groups currently captured by the 5-year modifier and whether outcomes for all CSA groups were higher than those who did not have a CSA history. An international sample of 974 women aged 18 to 50 completed an online survey hosted by Reddit regarding their substance use, multiple domains of current health, and CSA history using the original ACEs questionnaire and an experimental version of the CSA item without the 5-year modifier. All statistical analyses were completed in R. Results indicated there was a group of survivors with CSA experiences missed by the 5-year modifier and this had implications for reducing their total ACE scores. This group was nearly equal in size to CSA groups captured by the 5-year modifier and demonstrated equivalently poor health and substance use outcomes. On nearly all variables, CSA groups demonstrated poorer health outcomes than those who had never experienced CSA. These findings suggest the language of how CSA is assessed must be thoughtfully revised to include all CSA experiences as all are equally at risk for adverse outcomes and thus all warrant consideration for services currently afforded those with CSA histories and high ACE scores.

Means of assessing CSA must be thoughtfully revised.

Aims Angiotensin-converting enzyme (ACE), which metabolizes many peptides and plays a key role in blood pressure regulation and vascular remodeling, is expressed as a type-1 membrane glycoprotein on the surface of different cells, including endothelial cells of the heart. We hypothesized that the local conformation and, therefore, the properties of heart ACE could differ from lung ACE due to different microenvironment in these organs. Methods and results We performed ACE phenotyping (ACE levels, conformation and kinetic characteristics) in the human heart and compared it with that in the lung. ACE activity in heart tissues was 10-15 lower than that in lung. Various ACE effectors, LMW endogenous ACE inhibitors and HMW ACE-binding partners, were shown to be present in both heart and lung tissues. "Conformational fingerprint" of heart ACE (i.e., the pattern of 17 mAbs binding to different epitopes on the ACE surface) significantly differed from that of lung ACE, which reflects differences in the local conformations of these ACEs, likely controlled by different ACE glycosylation in these organs. Substrate specificity and pH-optima of the heart and lung ACEs also differed. Moreover, even within heart the apparent ACE activities, the local ACE conformations, and the content of ACE inhibitors differ in atria and ventricles. Conclusions Significant differences in the local conformations and kinetic properties of heart and lung ACEs demonstrate tissue specificity of ACE and provide a structural base for the development of mAbs able to distinguish heart and lung ACEs as a potential blood test for predicting atrial fibrillation risk.

Distributed-memory machines offer a very high level of performance, flexibility and scalability. But the memory organization of this kind of machine determines that processes on different processors must communicate explicitly by sending and receiving messages. As a result, the programmer faces the enormously difficult task of detailed planning of algorithm-irrelevant, low-level communication issues. This level of programming resembles writing assembly programs for a sequential machine. ACE is a message-passing language with abstract communication statements. It was defined by Dr. Jingke Li at Portland State University. The communication in ACE is still explicit, but it is abstracted to a higher level. The abstraction can help balance the needs of ease of programming and high performance. This thesis discusses how those high-level communication abstractions can be transformed into low-level communication routines. It presents the design and implementation of a compiler that transforms an ACE program into a C program with low-level communication routines. The compiler is implemented for the Intel iPSC/2 hypercube multiprocessor machine. Compared to their low-level counterparts, ACE programs are easier to write and are more understandable. Compared to their high level counterparts, more efficient code can be generated since the communication information is expressed explicitly in ACE and the compiler itself is much less complex. ACE also enables the users to fine tune some critical communication segments. Some well known parallel algorithms written in ACE are compiled by the compiler as examples, and experimental results of their performance are included.

Als Schlüsselenzym im Renin-Angiotensin System übernimmt das Angiotensin Converting Enzyme (ACE) eine wichtige Rolle bei der Blutdruckregulierung. ACE spaltet das biologisch inaktive Angiotensin I zum vasokonstriktorisch wirksamen Angiotensin II, was zu einem Anstieg des Blutdruckes führt. Tier- und Humanstudien zeigten, dass die Aufnahme bekannter, aus dem Proteinabbau stammender ACE-Inhibitoren eine Absenkung des Blutdruckes bewirkte. In der Lebensmittelindustrie finden Hydrolysate von Milchproteinen, im speziellen von Molkenproteinen, Einsatz in hypoallergenen (HA) Säuglingsnahrungen. Obwohl das Phänomen einer ACE-Inhibierung durch HA-Nahrungen in vitro in der Literatur bereits Erwähnung fand, existieren bislang keine Angaben zu einer potentiellen Wirkung in vivo. In der vorliegenden Arbeit konnte für kommerzielle hypoallergene (HA) Säuglingsnahrung eine sehr starke ACE-Hemmung in vitro zeigen (IC50-Werte zwischen 20 und 104 mg Protein/liter), welche die für fermentierte Sauermilchprodukte dokumentierte Wirkung bei weitem überstieg.] Mittels RP-HPLC und ESI-TOF-MS konnte neben zahlreichen bekannten Peptiden mit ACE-hemmendem Effekt erstmals das aus der Primärsequenz von -Lactalbumin freigesetzte Dipeptid Ile-Trp in den HA-Nahrungen identifiziert und quantifiziert werden. Ile-Trp ist der bislang potenteste in Lebensmitteln nachgewiesene ACE-Inhibitor (IC50 = 0,7µM). HA-Nahrungen zeigten auch ex vivo im Zellsystem (HUVECs) einen stark ACE-hemmenden Effekt. Aus diesem Grunde wurde ein möglicher Einfluss der HA-Nahrungen auf den Blutdruck spontan hypertensiver Ratten untersucht. Hierfür wurden die Tiere im Rahmen einer Fütterungsstudie über 14 Wochen mit standardisiertem Futter, welchem HA-Nahrung (Gruppe 1), konventionelle Säuglingsnahrung (Gruppe 2) bzw. der bekannte ACE-Inhibitor Captopril (Gruppe 3) zugesetzt war, gefüttert. Eine vierte Gruppe mit Standardfutter diente als Kontrolle. Der Blutdruck wurde am wachen Tier nichtinvasiv mittels tail-cuff-Methode gemessen. Der systolische Blutdruck sank bei Verabreichung der HA-Nahrung nach 7 Wochen signifikant um 21 ± 8 mmHg ab im Vergleich zur Kontrollgruppe bzw. den mit konventioneller Säuglingsnahrung gefütterten Tieren. Captopril führte zur einer Blutdrucksenkung um 30 ± 7 mmHg.

Similarity Join is an important operation for data mining, with a diverse range of real world applications. Three efficient MapReduce algorithms for performing Similarity Joins between multisets are proposed in this thesis. Filtering techniques for similarity joins minimize the number of pairs of entities joined and hence, they are vital for improving the efficiency of the algorithm. Multisets represent real world data better, by considering the frequency of its elements. Prior serial algorithms incorporate filtering techniques only for sets, but not multisets, while prior MapReduce algorithms do not incorporate any filtering technique or inefficiently incorporate prefix filtering with poor scalability.

This work extends the filtering techniques, namely the prefix, size, positional and suffix filters to multisets, and also achieves the challenging task of efficiently incorporating them in the shared-nothing MapReduce model. Adeptly incorporating the filtering techniques in a strategic sequence minimizes the pairs generated and joined, resulting in I/O, network and computational efficiency. In the SSS algorithm, prefix, size and positional filtering are incorporated in the MapReduce Framework. The pairs that thrive filtering are joined suavely in the third Similarity Join Stage, utilizing a Multiset File generated in the second stage. We also developed a rational and creative technique to enhance the scalability of the algorithm as a contingency need.

In the ESSJ algorithm, all the filtering techniques, namely, prefix, size, positional as well as suffix filtering are incorporated in the MapReduce Framework. It is designed with a seamless and scalable Similarity Join Stage, where the similarity joins are performed without dependency to a file.

In the EASE algorithm, all the filtering techniques, namely, prefix, size, positional and suffix are incorporated in the MapReduce Framework. However it is tailored as a hybrid algorithm to exploit the strategies of both SSS and ESSJ for performing the joins. Some multiset pairs are joined utilizing the Multiset File similar to SSS, and some multisets are joined without utilizing it similar to ESSJ. The algorithm harvests the benefits of both the strategies.

SSS and ESSJ algorithms were developed using Hadoop and tested using real-world Twitter data. For both SSS and ESSJ, experimental results demonstrate phenomenal performance gains of over 70% in comparison to the competing state-of-the-art algorithm.

Adverse Childhood Experience Scores, more commonly known as ACE scores, have provided new insight into the effects of adverse events on longstanding health and chronic illnesses in adults. Evidence shows that individuals with history of ACEs have increased risk of developing multiple chronic illnesses with or without increased exposure to activities that are detrimental to health, i.e. tobacco, alcohol, or other drug use. Additionally, a history of ACEs significantly increases an individual’s likelihood of participating in behaviors that are detrimental to long-term health, including risky behaviors and resulting addiction issues. However, in most individuals with ACEs, their childhood experiences, or the extent of the experience, is not likely to be mentioned in casual conversation. By increasing knowledge on ACE scores and implementing scoring of all individuals possible, it would improve a physician’s ability to assess risk and health screening needs for a patient on a more individualized basis. The purpose of this study is to educate Bristol Family Medicine residents on the daily uses and clinical correlations provided by ACE scoring in an attempt to increase score documentation of future patients.We will begin by providing a short knowledge assessment about ACE scores to residents in the Bristol Family Medicine residency program. We will then give a detailed presentation on the usefulness and effectiveness of using ACE scores in clinical judgement and screening. Following the presentation, we will provide a similar knowledge assessment as well as a questionnaire to assess likelihood of residents to implement ACE scores in their current practice.

Als Schlüsselenzym im Renin-Angiotensin System übernimmt das Angiotensin Converting Enzyme (ACE) eine wichtige Rolle bei der Blutdruckregulierung. ACE spaltet das biologisch inaktive Angiotensin I zum vasokonstriktorisch wirksamen Angiotensin II, was zu einem Anstieg des Blutdruckes führt. Tier- und Humanstudien zeigten, dass die Aufnahme bekannter, aus dem Proteinabbau stammender ACE-Inhibitoren eine Absenkung des Blutdruckes bewirkte. In der Lebensmittelindustrie finden Hydrolysate von Milchproteinen, im speziellen von Molkenproteinen, Einsatz in hypoallergenen (HA) Säuglingsnahrungen. Obwohl das Phänomen einer ACE-Inhibierung durch HA-Nahrungen in vitro in der Literatur bereits Erwähnung fand, existieren bislang keine Angaben zu einer potentiellen Wirkung in vivo. In der vorliegenden Arbeit konnte für kommerzielle hypoallergene (HA) Säuglingsnahrung eine sehr starke ACE-Hemmung in vitro zeigen (IC50-Werte zwischen 20 und 104 mg Protein/liter), welche die für fermentierte Sauermilchprodukte dokumentierte Wirkung bei weitem überstieg.] Mittels RP-HPLC und ESI-TOF-MS konnte neben zahlreichen bekannten Peptiden mit ACE-hemmendem Effekt erstmals das aus der Primärsequenz von -Lactalbumin freigesetzte Dipeptid Ile-Trp in den HA-Nahrungen identifiziert und quantifiziert werden. Ile-Trp ist der bislang potenteste in Lebensmitteln nachgewiesene ACE-Inhibitor (IC50 = 0,7µM). HA-Nahrungen zeigten auch ex vivo im Zellsystem (HUVECs) einen stark ACE-hemmenden Effekt. Aus diesem Grunde wurde ein möglicher Einfluss der HA-Nahrungen auf den Blutdruck spontan hypertensiver Ratten untersucht. Hierfür wurden die Tiere im Rahmen einer Fütterungsstudie über 14 Wochen mit standardisiertem Futter, welchem HA-Nahrung (Gruppe 1), konventionelle Säuglingsnahrung (Gruppe 2) bzw. der bekannte ACE-Inhibitor Captopril (Gruppe 3) zugesetzt war, gefüttert. Eine vierte Gruppe mit Standardfutter diente als Kontrolle. Der Blutdruck wurde am wachen Tier nichtinvasiv mittels tail-cuff-Methode gemessen. Der systolische Blutdruck sank bei Verabreichung der HA-Nahrung nach 7 Wochen signifikant um 21 ± 8 mmHg ab im Vergleich zur Kontrollgruppe bzw. den mit konventioneller Säuglingsnahrung gefütterten Tieren. Captopril führte zur einer Blutdrucksenkung um 30 ± 7 mmHg.

In this thesis, a geometric framework for describing relevant information in a collection of data is applied for the general problems of selecting informative features (dimension reduction) from high dimensional data. The framework can be used in an unsupervised manner, extracting universal features that can be used later for general classification of data. This framework is derived by applying local approximations on the space of probability distributions and a small perturbation approach. With this approach, different information theoretic results can be interpreted as linear algebra optimizations based on the norms of vectors in a linear space, which are in general, easier to carry out. Fundamentally, using known procedures such as Singular Value Decomposition (SVD) and Principal Component Analysis (PCA), dimension reduction for maximizing power can be achieved in a straight forward manner. Using the geometric framework, we relate calculation of SVD of a particular matrix related to a probabilistic channel to the application of Alternating Conditional Expectation (ACE) in the problem of optimal regression. The key takeaway of this method is that such problems can be studied in the space of distributions of the data and not the space of outcomes. This geometric framework allows to give an operational meaning to information metrics in the context of data analysis and feature selection. Additionally, it provides a method to obtain universal classification functions without knowledge of the important feature of the problem. This framework is the applied to the problem of data classification and analysis with satisfactory results.

The hypoxia of high altitude (‘hypobaric hypoxia’) presents a profound physiological challenge to the human body and at extreme high altitude the human body nears the limits of its tolerance for oxygen deprivation. Physical performance in this environment is limited by two major physiological elements: ‘acclimatisation’ to hypobaric hypoxia sufficient to allow strenuous physical exertion, and the avoidance of Acute High Altitude Illnesses (AHAI). The rate and effectiveness of acclimatisation and the susceptibility to AHAI varies markedly between individuals, suggesting a possible genetic influence on high altitude performance. A polymorphism of the human Angiotensin Converting Enzyme (ACE) gene has been identified in which the deletion (D-allele), rather than the insertion (I-allele), of a 287 base pair sequence is associated with higher circulating and tissue ACE activity. This polymorphism has also been associated with physical performance phenotypes, the ACE I-allele being associated with elite endurance performance. An excess frequency of the ACE I-allele has also been identified in a small sample of elite UK high altitude mountaineers. This thesis set out to test the hypothesis that the ACE I-allele is indeed associated with successful physical performance at high altitude, and to explore the mechanism by which such an advantage may be mediated. I conducted a series of prospective geneenvironment interaction studies to assess whether the ACE I-allele is associated with successful ascent to high altitude and, if such an advantage exists, whether this is mediated by reduced susceptibility to Acute Mountain Sickness (AMS) or improved oxygen saturations. To further define the underlying mechanism, I explored the association of bradykinin 2 receptor genotype with high altitude performance to investigate whether the ACE I allele contribution is mediated by increased kinin activity. Additionally I extended these observations to those who have faced the most extreme hypoxic challenge - a successful ascent of Mount Everest.

The renin-angiotensin system (RAS) is involved in maintenance of cardiovascular function and has been implicated in coronary heart disease (CHD) and type 2 diabetes (T2D). Pharmacological inhibition of the RAS improves cardiovascular disease outcomes. Angiotensin Converting Enzyme (principal component of RAS) levels are significantly associated with the ACE(I/D) polymorphism. Large studies have demonstrated a mild effect of this polymorphism on CHD risk. However the presence of RAS in diverse tissues implicated in cardiovascular diseases justify the hypothesis that genetic polymorphisms encoding proteins in this system 'modify' the risk of such diseases in the context of harmful stimuli or affect disease progression. This thesis therefore tested the hypothesis that the ACE(I/D) polymorphism 'modifies' risk of CHD and T2D, and affects progression of Hypertrophic Cardiomyopathy (HCM). Using a study of 532 myocardial infarction (MI) survivors and 574 controls (HIFMECH), an interaction between the ACE polymorphism and lipid levels on MI risk in Northern compared to Southern Europeans is demonstrated: the MI odds with high ApoB levels in D allele carriers was much greater in the North than the South. In a prospective cohort of 3052 healthy men (NPHS2), an interaction between ACE and systolic blood pressure (SBP) on CHD risk is presented where D allele carriers were protected against CHD at lower SBP, but at higher SBP were more susceptible. In the same study, ACE variants modified the risk of T2D in obese individuals, with obese D allele carriers having a substantially higher T2D risk compared to obese II homozygotes. From a retrospective study of 541 patients with HCM a possible effect of the polymorphism on left ventricular remodelling is demonstrated. Thus the work in this thesis offers strong evidence that the ACE gene modifies risk of CHD and T2D in the presence of known risk factors.

O objectivo desta dissertação divide-se em duas áreas. A primeira consiste em explicar o modo através do qual a rede colectiva pode resolver um problema de posicionamento dos seus actores. Para este efeito, os conceitos retirados da abordagem evolucionista das competências são conjugados com o modelo de acção colectiva em redes industriais. A segunda área consiste em compreender a dinâmica das redes colectivas, procurando explicar o modo como estas redes evoluem ao longo do tempo. Em síntese, o modelo de análise apresentado neste trabalho, sendo uma extensão da literatura da abordagem das redes industriais, procura explicar os motivos, as condições e o modo de actuação das redes colectivas baseadas em competências.
Cooperation in industrial network is one of the most studied phenomenons in the network approach developed by the IMP Group (Industrial Marketing and Purchasing). In this theoretical field there are models that study collective cooperation which explain the formation of issue-based nets groups of actors that through cooperation try to cope collectively with a common issue or problem. This dissertation is divided in two areas. The first part is dedicated to explain how a collective actor is able to solve a positioning problem of actors involved. The collective action model is enriched with some of the concepts developed by the capabilities approach. The second purpose of this dissertation is to develop a more dynamic perspective of the issue-based net, seeking to explain how the collective actor develops trough time in order to fulfil its objectives. In conclusion, the analytic model presented in this dissertation, being an extension of the business network literature, explains the motifs, the conditions and the process used by which issue-based nets coordinate activities.

Dialkylsilanediols, a novel class of non-hydrolyzable analogues of the tetrahedral intermediate of amide hydrolysis, have been shown to be good inhibitors of the HIV-1 protease, angiotensin converting enzyme (ACE), thermolysin, and the serine protease α-chymotrypsin. Synthesis and biological evaluation of silanediols are therefore a priority in this research. Asymmetric intramolecular hydrosilylation (AIH) of allyl silyl ethers gives silafurans which can be used directly to make chiral β-silyl acids needed for the silanediol peptide mimics. Absolute configuration determination of AIH products remains a challenge. Proton nuclear magnetic resonance (1H NMR) of the Mosher ester derivative was used to confirm the absolute configuration. This has proven to be a simple method to determine the absolute configuration of silicon-containing primary carbinols. Dialkylsilanediols (1.52) are known as good inhibitors of angiotensin converting enzyme (ACE), with inhibition constants from 3.8 to 207 nM. However, the synthesis of these silandiol peptide mimics involved a long synthetic route. A short, asymmetric synthesis of silanediol ACE inhibitors was developed using asymmetric hydrosilylation and addition of a silyllithium to a sulfinimine, 8 linear steps with an 8% over all yield. Specific inhibitors of the FXIa protease could inhibit thrombosis without completely interrupting normal hemostasis, and prevent or minimize the risk of hemostasis complications. Based on the FXIa substrate, the design and synthesis of the first five guanidine-containing silanediol FXIa inhibitors was developed: Ac-Arg-[Si]-Ala-NHMe (4.15), Ac-Ala-Arg-[Si]-Ala-NHMe (4.16), Ac-Leu-Ala-Arg-[Si]-Ala-NHMe (4.17), Ac-Pro-Ala-Arg-[Si]-Ala-NHMe (4.18), and Ac-Arg-[Si]-Ala-Ala-NHMe (4.19). Synthesis of these targets was achieved using our newly developed silyllithium preparation and silyl dianion addition to the Davis sulfinimine, 11 linear steps, gave silanediol precursor 4.60 in 1.7% yield. Inhibition constant of the FXIa inhibitors was good in range of 76 - 980μM. Human heart chymase (HHC), a chymotrypsin-like serine protease present in the left ventricular tissues of the human heart, converts angiotensin I to angiotensin II, raising blood pressure. Although the physiological role of HHC has not been fully elucidated, it may be involved in various pathological states, particularly in cardiovascular diseases. Synthesis of silanediol inhibitors of HHC, therefore, may contribute to the understanding of its physiological functions and a better treatment for cardiovascular diseases. Synthesis of a silanediol chymase inhibitor has been investigated.

Within the regulation of blood pressure, the renin-angiotensin system (RAS) plays an essential role with AngII causing vasoconstriction, which increases the blood pressure. AngII is formed after ACE ar cleaved by AngI. We hypothesize that the blunting of AngII production with concomitant exercise reduces improvements in metabolic fitness by removing an important stimulus for capillary growth in exercised muscles. The aim of this research was to expose the pathway by which AngII and ACE are implicated in exercise-induced capillary growth of human muscle. We show that highest increases in indices of metabolic strain are observed after high intense exercise bouts and that the increase in AngII is ACE genotype dependent. Furthermore, subjects with the ACE-II genotype had a significant higher capillary perfusion in the finger after exercise. Our findings of our ACE inhibition study support the view that an angiotensin-regulated mechanism affects the hypoxia-specific gene response in peripheral muscle to endurance exercise and support the notion that the muscle’s transcript regulation by ACE inhibition is related to muscle oxygenation during exercise. This indicates that there is a shift in the activation of the gene program from muscle fibres to the surrounding interstitium after ACE inhibition. In a training study we identified a correlation between increases in VO2max and the changes in capillary-to-fibre-ratio. We further observed that variability in the cardiovascular response, based on VO2max and heart rate at rest, was related to the ACE I/D genotype. Our findings that physiological improvements after exercise are dependent on ACE levels, both due to different ACE genotypes and to taking ACE inhibitors could have clinical repercussions. It may explain the individual variation in the response to exercise rehabilitation in aerobic power and oxygen uptake. These differences in exercise-induced improvements have rarely been valued in pharmacological studies of hypertension and exercise rehabilitation.

This thesis addresses the impact of quadriceps wasting and physical inactivity across disease severity (GOLD stages I-IV) in Chronic Obstructive Pulmonary Disease (COPD) and assesses the influence of angiotensin-converting enzyme (ACE)-inhibition on quadriceps dysfunction in these patients. In a cross-sectional study of 161 COPD patients, ultrasound measurement of rectus femoris cross-sectional area was reduced in mild as well as advanced disease when compared to controls. Daily physical activity, measured using an armband accelerometer, was reduced in COPD subjects across all GOLD stages compared to controls. Physical activity was independently associated with quadriceps wasting in GOLD stage I, but not stage II-IV disease where residual volume to total lung capacity ratio was the only independent predictor of activity level. This data suggests that quadriceps wasting is not an end-stage phenomenon in COPD and highlights the need for early identification of these patients to guide lifestyle and therapeutic interventions. The effect of the ACE-inhibitor, fosinopril on quadriceps dysfunction in COPD was then investigated in a double-blind randomised controlled trial of 80 COPD patients with quadriceps weakness. Despite a significant reduction in systolic blood pressure and serum ACE activity in the treatment group compared to placebo, no significant differences were observed at 3 months in the primary outcome of non-volitional quadriceps endurance. Quadriceps strength improved in both groups, but there was a greater increase in the placebo arm. No significant changes were observed in mid-thigh cross-sectional area or incremental shuttle walk distance. The trial also assessed the effect of ACE-inhibition on vastus lateralis atrogene expression in COPD, with no significant differences observed between groups. In conclusion, although evidence from observational cohorts suggest a role for the renin-angiotensin system in the control of muscle phenotype, data from this thesis found that ACE-inhibition did not improve quadriceps function in a COPD population with quadriceps weakness.

Exposure to adverse childhood experiences (ACEs) has lasting repercussions throughout an individual’s lifetime. An adult with a history of childhood trauma is at increased risk for excessive stress reactivity, which exacerbates the development of chronic disease. It is important to investigate how this information can be used for adult trauma survivors. This study assessed the psychophysiological impacts of providing “ACE insight”. Participants completed questionnaires and were given false feedback that their childhood experiences put them at increased risk for excessive stress reactivity and the development of disease. Following ACE insight, participants underwent a speech stressor task during which cardiovascular reactivity was monitored and psychological reactions were assessed. Results indicated that participants with more adverse childhoods reported feeling more worried and less happy about feedback. Further, ACE insight caused a significant increase in cardiac output for participants with a history of childhood trauma. Implications and future directions are discussed.

The purpose of this study was to compare intramuscular temperature decreases caused by six different modalities at different levels of compression. The independent variables were modality (ice, ice and ace wrap, Game Ready? no, low, medium, high) and time (0, 1, 5, 10, 15, 20, 30 minutes). The dependent variable was intramuscular temperature. Fifteen volunteers participated in this study. The only significant difference in absolute intramuscular temperature was between ice and ace wrap and Game Ready? low. Ice and ace wrap had significantly greater change in quadriceps intramuscular temperature than Game Ready? no and low. Because it created the lowest intramuscular temperature and the greatest change in temperature, ice and ace wrap should be selected as a cooling modality over the Game Ready? device during the immediate care of injuries.

The ACE protein is a zinc-dependent dipeptidyl carboxypeptidase comprised of two homologous domains termed the C- and N-domain. The C-domain is primarily responsible for the catalytic production of Ang II, while the tetrapeptide acetyl-seryl-aspartyl-lysyl-proline (AcSDKP) is predominantly cleaved by the N-domain, and both domains play a role in the metabolism of vasodilatory peptide bradykinin. In the event of myocardial infarction (MI), cardiac output and blood pressure decreases, resulting in activation of the RAS and an increase in both Ang II production and bradykinin metabolism. While initially compensatory, prolonged RAS activation has been shown to have long-term detrimental effects, and pharmaceutical intervention in the form of ACE inhibition is the first line treatment following an MI event. The ACE inhibitors currently in clinical use target both domains equally, and it has been suggested that the major side-effects of this drug class are largely attributable to the inhibition of bradykinin breakdown. A novel C-domain selective ACE inhibitor lisinopril-Trp (lisW-S) incorporates a tryptophan moiety into the P2' position of the clinically available ACE inhibitor lisinopril.

Angiotensin converting enzyme (ACE) converts angiotensin I to angiotensin II, an important step in the control of blood pressure. The gene encoding for ACE is subject to an insertion/deletion (I/D) polymorphism which is associated with different levels of the enzyme in serum. This polymorphism accounts for 47% of the variability in serum ACE concentrations between subjects but its relevance to tissue ACE is unknown. ACE inhibition increases kinin level, for example bradykinin. Kinins have been proposed be involved in the pathogenesis of cough due to ACE inhibitors, a common adverse effect in those prescribed these drugs. A genetic link was proposed to explain the differing susceptibility of subjects to develop cough. Those susceptible may differ in the cough reflexes initially or perhaps have different degrees of tissue ACE activity. In this thesis I examined healthy subjects using substrates and inhibitors of ACE to identify any possible differences in tissue ACE between those of different ACE genotype. I also examined the natural history of ACE inhibitor cough in particular changes in cough reflex and the possible roles of kinins in its aetiology. The frequency of ACE genotype amongst those who developed cough was also examined.

The aim of this dissertation was to investigate peak-to-average power ratio (PAPR) reduction techniques for filter bank multicarrier (FBMC) modulated systems. Research into current PAPR reduction techniques in FBMC and orthogonal frequency division multiplexing (OFDM) found that the active constellation extension (ACE) method has not been exploited for use in FBMC systems. This is, in part, due to the complexities involved in signal processing inherent of FBMC systems, as well as FBMC being a relatively new subject matter in the telecommunications field. The outcomes of the dissertation were four novel PAPR reduction techniques for FBMC systems. These techniques can be split up into two namely linear programming optimization ACE-based techniques and overlapping ACE techniques. The techniques compensate for the overlapping symbols in FBMC by utilizing a frame based approach. This characteristic makes them ideal candidates to be applied in burst transmission. Furthermore, because an ACE based technique is used, they do not require the transmission of side information. The PAPR performance of the techniques are shown to match, or in some cases improve, current PAPR techniques for FBMC. The analysis of the proposed approaches also address some complexity issues with current FBMC implementations. The out-of-band (OOB) interference of the techniques under clipping scenarios is investigated. It is shown that the OOB interference can be minimized whilst still maintaining good PAPR performance. Additional results are also provided by means of a study of the PAPR reduction techniques proposed as applied to a high power amplifier based model. The corresponding power amplifier efficiency as well as bit-error rate (BER) degradation are investigated and compared to other standard techniques for PAPR reduction in FBMC systems. It was found that the proposed techniques can achieve a higher amplifier efficiency than existing techniques at a marginal trade-off in BER degradation. This BER degradation is lower than existing techniques due to the marginal increase in average transmit power. As illustrated by exhaustive simulations, the single scaling and overlapping smart gradient-project ACE-based techniques proposed, are ideal candidates for practical implementation in systems employing the low complexity polyphase implementation of FBMC modulators. The methods are shown to offer significant PAPR reduction and increase the feasibility of FBMC as a replacement modulation system for OFDM.
Die doel van hierdie verhandeling is om top-tot-gemiddelde drywingsverhouding (PAPR) verminderingstegnieke vir filterbank veelvuldige draer (FBMC) -gemoduleerde stelsels te ondersoek. Huidige PAPR verminderingstegnieke in FBMC en ortogonale frekwensie verdeling multipleksering (OFDM) is ondersoek en daar is bevind dat die aktiewe konstellasie uitbreidingsmetode (ACE) nog nie vantevore ontgin is in FBMC stelsels nie. Dit is gedeeltelik as gevolg van die inherente kompleksiteit van seinverwerking in FBMC stelsels en ook omdat FBMC n relatief nuwe onderwerp in die veld van telekommunikasie is. Die resultate van hierdie verhandeling is vier unieke PAPR verminderingstegnieke vir FBMC stelsels. Hierdie tegnieke kan opgedeel word in lineêre programeringsgeoptimiseerde tegnieke en oorvleuelende ACE tegnieke. Die buite band (OOB) steuring van die tegnieke onder geknipte scenarios word ondersoek. Daar word getoon dat die OOB steuring verminder kan word terwyl goeie PAPR verrigting gehandhaaf word. Addisionele studieresultate word ook voorsien deur n studie van die voorgestelde PAPR reduksietegnieke toegepas op n hoë drywing versterker (HPA)model. Die ooreenstemmende drywingsversterker doeltreffendheid asook die degradering van die bit fout waarde (BER) is ondersoek en vergelyk met ander tegnieke vir PAPR vermindering in FBMC stelsels. Daar is bevind dat die voorgestelde tegnieke n hoër drywing versterker doeltreffendheid kan bereik as bestaande tegnieke afgespeel ten koste van n marginale BER degradasie. Hierdie degradasie is laer as bestaande tegnieke as gevolg van die marginale toename in gemiddelde uitsendingskrag. Uitvoerige simulasies toon dat die voorgestelde enkel skalering en oorvleuelende slim gradiënt projek ACE gebaseerde tegnieke die ideale kandidate is vir praktiese toepassing in stelsels wat gebruik maak van die lae kompleksiteit veelfase toepassing van FBMC modulators. Dié metodes bring n beduidende PAPR vermindering te weeg en maak FBMC meer haalbaar as n vervanging modulasie stelsel vir OFDM.
