Aim. To study the efficacy and safety of the triple fixed-dose combination (FDC) of amlodipine/indapamide/perindopril on blood pressure (BP) profile in patients with grade I-II hypertension (HTN) in actual clinical practice.Material and methods. Data from 54 patients with paired 24-hour ambulatory BP monitoring (ABPM) data were included in the TRICOLOR subanalysis (ClinicalTrials. gov study ID — NCT03722524). The mean 24-hour, daytime, and nighttime BP were calculated at baseline and after 12-week follow-up. We determined the proportion of patients with nocturnal HTN (≥120/70 mm Hg) and nocturnal hypotension (<100/60 and <90/50 mm Hg) initially and after 12 weeks of triple FDC therapy. Patients with nocturnal BP decrease included dippers (D; 10-20%), reduced dippers (RD; 0-10%) and extreme dippers (ED; >20%), as well as those without nocturnal BP decrease (>0%, non-dipper (ND)). The smoothness index (SI) was analyzed as the ratio of the mean hourly SBP fall to its mean standard deviation in paired ABPM. To assess the BP phenotypes, two methods were used with reference values of <130/80 and <140/90 mm Hg for ABPM and office BP, respectively. Controlled hypertension (CHT), uncontrolled hypertension (UHT), white coat hypertension (WHT) and masked ineffectiveness of antihypertensive therapy were distinguished.Results. Among 1247 participants of the TRICOLOR study, 54 patients with valid paired ABPM were selected (men, 46%; mean age, 57,7 [12,1] years; mean office BP, 150,4 [16,6]/93,3 [10,7] mm Hg; HTN duration, 8,3 [7,5] years). Initially, the mean 24-hour, daytime and nighttime BP was 141,1 [15,4]/85,9 [9,9], 144,2 [15,5]/88,8 [10,5] and 132,6 [18,0]/78,1 [9,9] mm Hg, respectively. After 12-week follow-up, the mean 24-hour, daytime and nighttime BP was 123,1 [10,5]/75,6 [8,5], 125,7 [10,9]/77,9 [8,7] and 115,4 [10,2]/68,6 [8,8] mm Hg, respectively (p<0,001). After 12-week follow-up, the proportion of patients with nocturnal hypertension decreased from 64,8% to 25,0% (2,6 times) (p<0,001). The proportion of NDs and EDs decreased from 16,7% and 7,4% to 5,8% and 0%, respectively (p=0,048); the proportion of patients with RD and D patterns increased from 42,6% and 33,3 to 57,7% and 36,5%, respectively (p=0,048). With triple FDC therapy, the SI during the day was higher than 0,73 in half of the cases. According to the two methods, the proportion of patients with UHT decreased from 81,6% to 4,4%, WHT from 12,2% to 0%. The prevalence of CHT increased from 4,1% to 57,8%, while masked ineffectiveness of antihypertensive therapy — from 2,0% to 37,8%.Conclusion. Twelve-week FDC therapy of amlodipine/indapamide/perindopril led to a significant fall in the mean 24-hour, daytime and nighttime BP values. Comprehensive analysis of two techniques (24-hour and office BP measurement) identified patients requiring further triple FGC titration.

Abstract Background: The combination of 7+3 with a purine nucleoside analogue improved overall survival (OS) in patients with acute myeloid leukemia (AML). We randomized patients to receive either clofarabine (CIA) or fludarabine (FIA) combined with idarubicin and cytarabine. Methods: Patients who were diagnosed with non-CBF AML or non-APL AML were eligible and were randomized using a Bayesian design to one of the following two induction chemotherapy regimens: CIA (clofarabine 15 mg/m2 IV daily for 5 days, idarubicin 10 mg/m2 IV daily for 3 days, cytarabine 1,000 mg/m2 IV daily for 5 days) or FIA (fludarabine 30 mg/m2 IV daily for 5 days, idarubicin 10 mg/m2 IV daily for 3 days, cytarabine 1,000 mg/m2 IV daily x 5 days). Patients could proceed to up to 6 cycles of consolidation with the same drugs according to an attenuated schedule. Results: One-hundred-fifty-eight patients (97 patients, CIA; 61 patients, FIA) were treated so far. Patient characteristics and outcome are summarized in Table 1. Median age is 53 years (range, 20-66) in CIA and 49 years (range, 18-66) in FIA. All patients were evaluable for response. Responses are summarized in Table 1. MRD negativity was observed in 43 (74%) patients treated with CIA and in 19 (35%) patients treated with FIA (p=0.049). Median follow up is 21 months and 16 months for patients treated with CIA and FIA, respectively. Treatment was well tolerated with 8-week mortality rates of 1% and 2%, for patients treated with CIA and FIA respectively. The overall median EFS and OS for the whole population were 12 months and 25 months, respectively. Median EFS for patients treated with CIA and FIA was 14 months and 11 months, respectively (p=0.81). No difference in OS between CIA and FIA was observed: the 2-year OS rates were 48% and 53% (p=0.45), respectively. Furthermore, there was no difference in survival whether patients were censored or not at the time of transplantation. Compared to IA regimen in similar patients population, the triplet (FIA and CIA) showed an improvement in 2-year EFS (60% vs 34%; p=0.05) and a trend for better survival (40% vs 32%; p=0.5) in younger patients (40 years and younger). Conclusions: The combination of clofarabine or fludarabine to idarubicin and cytarabine is safe and effective with high CR and negative MRD rates in patients with newly diagnosed AML. Particularly in younger patients, CIA or FIA can lead to a superior outcome compared to 3+7. Table 1. OBJECTIVES/GOALS: Chronic kidney disease (CKD) impacts 15% of US adults and African American (AA) persons are disproportionately affected with more than 3 times higher risk of kidney failure when compared to Caucasian persons. This study evaluated the physiological and metabolomic effects of increased fruits and vegetables (F&V) on cardio-renal risk factors. METHODS/STUDY POPULATION: This pilot trial used a prospective, 2-group, randomized study design to evaluate a F&V intervention (N=46), where participants received a prescribed amount of fresh, base-producing F&V compared to a wait-list control (WL) condition (N=45). All participants were African American adults (≥18 years), had self-reported hypertension, and had CKD (Stage 1-3) on screening spot-urine microalbumin test. Participants were measured at baseline and 6 weeks post-intervention. Clinical data (i.e., systolic and diastolic blood pressure, lipid panel, hemoglobin A1C, BMI [body mass index], and albumin to creatinine ratio) were collected. Targeted metabolomic quantitative analysis was performed followed by LC-MS/MS and FIA-MS/MS. Linear mixed models evaluated analyte expression and clinical data. RESULTS/ANTICIPATED RESULTS: AA participants (N=91) were aged 58 ± 10.2 years, 66% female, and 54% had incomes ≤$50,000. T-tests compared change scores (baseline to 6-weeks) between groups. The F&V group demonstrated a significant reduction in BMI of -4.7 ± 10.5 kg/m² compared to a 1.9 ± 8.3 kg/m² increase in the WL group, p<.01. Further, the F&V group demonstrated a reduction in total cholesterol of -15.4 ± 58.8 mg/dL compared to a 17.7 ± 68.8 mg/dL increase in the WL group, p<.05. Non-significant reductions in hemoglobin A1c were found in the F&V versus the WL group. Metabolomic analysis indicated significant variation with an increase of suggestive key biomarkers for worse CKD in the WL versus F&V groups at 6-weeks. DISCUSSION/SIGNIFICANCE: Consumption of only 2 cups of F&V via a community-based intervention reduced CVD risk factors in AA adults with CKD and HTN and resulted in molecular/biochemical changes which may improve long-term kidney health. Further investigation may lead to development of cost-effective dietary intervention models to improve CKD outcomes in AA persons.

Abstract Coronary reactive hyperemia (CRH) is impaired in cardiovascular diseases, and angiotensin-II (Ang-II) exacerbates it. However, it is unknown how Ang-II affects CRH in Tie2-sEH Tr (human-sEH-overexpressed) versus wild-type (WT) mice. sEH-overexpression resulted in CRH-reduction in Tie2-sEH Tr versus WT. We hypothesized that Ang-II exacerbates CRH-reduction in Tie2-sEH Tr versus WT. Coronary flow in Tie2-sEH Tr and WT was measured by Langendorff system. The hearts were exposed to 15-second ischemia, and CRH was assessed in 10 mice each. Repayment volume was reduced by 40.50% in WT-treated with Ang-II versus WT (7.42 ± 0.8 to 4.49 ± 0.8 ml/g) and 48% in Tie2-sEH Tr-treated with Ang-II versus Tie2-sEH Tr (5.18 ± 0.4 to 2.68 ± 0.3 ml/g). Ang-II decreased repayment duration by 50% in WT-treated with Ang-II versus WT (2.46 ± 0.5 to 1.24 ± 0.4 min) and 54% in Tie2-sEH Tr-treated with Ang-II versus Tie2-sEH Tr (1.66 ± 0.4 to 0.76 ± 0.2 min). Peak repayment flow was reduced by 11.2% in WT-treated with Ang-II versus WT (35.98 ± 0.7 to 32.11 ± 1.4 ml/g) and 4% in Tie2-sEH Tr-treated with Ang-II versus Tie2-sEH Tr (32.18 ± 0.6 to 30.89 ± 1.5 ml/g). Furthermore, CF was reduced by 43% in WT-treated with Ang-II versus WT (14.2 ± 0.5 to 8.15 ± 0.8 ml/min/g) and 32% in Tie2-sEH Tr-treated with Ang-II versus Tie2-sEH Tr (12.1 ± 0.8 to 8.3 ± 1.2 ml/min/g). Moreover, the Ang-II-AT1-receptor and CYP4A were increased in Tie2-sEHTr. Our results demonstrate that Ang-II exacerbates CRH-reduction in Tie2-sEH Tr mice.

The objective of this research was to evaluate the effects of various non-genetic factors on first lactation and lifetime productivity in Nili-Ravi buffaloes, including season of birth, period of birth, age at first calving, season of first calving, period of first calving, and number of lactations completed. The livestock data on first lactation and lifetime performance traits relevant to 501 Nili-Ravi buffaloes were collected from the history sheet of the animal for a period from 1983 to 2017 (i.e., 35 years) and maintained at the Indian Council of Agricultural Research—Central Institute for Research on Buffaloes (ICAR-CIRB) sub-campus, in Nabha, Punjab, India. To evaluate the least-squares means (LSMs) and the effect of non-genetic factors on performance traits, a least-squares analysis model was applied. The overall LSM for age at first calving (AFC) was 45.03 ± 0.40 months and ranged from 34 to 54 months. The results indicated that the effect of AFC was highly significant (p ≤ 0.01) on first lactation total milk yield (FLTMY), first lactation standard milk yield (305 days or less) (FLSMY), first peak milk yield (FPY), and first lactation length (FLL) in the Nili-Ravi breed of buffaloes. FLTMY, FLSMY, and FPY were highest (2,250.08 ± 48.16 kg, 1,944.68 ± 31.20 kg, and 9.32 ± 0.16 kg/day, respectively) for animals with an AFC of 42–48 months. Furthermore, FLTMY, FLSMY, and FLL were highest (2,411.02 ± 68.68 kg, 2,008.81 ± 44.49 kg, and 357.43 ± 8.13 days, respectively) in animals that first calved in the autumn season. However, the lowest first dry period (FDP), first service period (FSP), and first calving interval (FCI) (110.63 ± 7.42 days, 125.48 ± 9.04 days, and 443.63 ± 9.00 days, respectively) were found for animals that first calved in the rainy season. The overall LSM for herd life (HL), productive life (PL), productive days (PDs), unproductive days (UDs), total lifetime milk yield (total LTMY), standard lifetime milk yield (standard LTMY), milk yield per day of productive life (MY/PL), milk yield per day of productive days (MY/PDs), and milk yield per day of herd life (MY/HL) were estimated as 3,779.84 ± 31.86 days, 2,078.55 ± 24.32 days, 1,552.74 ± 20.06 days, 525.81 ± 12.44 days, 10,229.71 ± 195.31 kg, 9,203.64 ± 173.52 kg, 4.86 ± 0.08 kg/day, 6.46 ± 0.08 kg/day, and 2.66 ± 0.04 kg/day, respectively. The effect of AFC on HL, PDs, UDs, total LTMY, and standard LTMY was highly significant (p ≤ 0.01). Furthermore, the effect of season of first calving on HL, PL, and PDs was significant (p ≤ 0.05). In addition, the effect of the period of first calving on HL, PDs, standard LTMY, and total LTMY was significant (p ≤ 0.05). In these contexts, it can be concluded that the buffaloes of the Nili-Ravi breed with an AFC of 42–48 months performed better than animals with a later AFC in terms of production, reproduction, and lifetime traits.
