Abstract Sequence analysis is an established method used to study the complexity of family life courses. Although individual and societal characteristics have been linked with the complexity of family trajectories, social scientists have neglected the potential role of genetic factors in explaining variation in family transitions and events across the life course. We estimate the genetic contribution to sequence complexity and a wide range of family demographic behaviors using genomic relatedness–based, restricted maximum likelihood models with data from the U.S. Health and Retirement Study. This innovative methodological approach allows us to provide the first estimates of the heritability of composite life course outcomes—that is, sequence complexity. We demonstrate that a number of family demographic indicators (e.g., the age at first birth and first marriage) are heritable and provide evidence that composite metrics can be influenced by genetic factors. For example, our results show that 11% of the total variation in the complexity of differentiated family sequences is attributable to genetic influences. Moreover, we test whether this genetic contribution varies by social environment as indexed by birth cohort over a period of rapid changes in family norms during the twentieth century. Interestingly, we find evidence that the complexity of fertility and differentiated family trajectories decreased across cohorts, but we find no evidence that the heritability of the complexity of partnership trajectories changed across cohorts. Therefore, our results do not substantiate claims that lower normative constraints on family demographic behavior increase the role of genes.

Background and Aims. Complications following thyroidectomy can prolong hospital stay and cause significant morbidity particularly for patients treated for benign thyroid conditions. Our aim was to administer a UK-wide survey of thyroid surgery units on frequency and timing of the onset of life-threatening airway complications & correlate to factors that might be associated with them.Methods. A questionnaire including the number of and timing of the onset of life-threatening airway complications, number of thyroidectomy procedures performed per year, surgeon years of experience, the use of difficult airway management protocol, post-operative patient destination, and patient deaths, was sent to 80 UK surgical units.Results. 23/41 hospitals responded reported no postthyroidectomy airway complications. Life-threatening airways complications all occurred within the first 12 hours postoperatively, with 9 cases occurring in the recovery room and in less than 2 hours, 3 cases occurring 2–6 hours, and 3 cases occurring 6 to 12 hours after surgery.Conclusion. The results may support recent publications that advocate thyroidectomy as a less-than-24-hour surgery procedure in selected patients. Further a larger study and standardised protocol are required to establish patients' selection criteria to determine who are likely to develop serious postoperative complication and may require HDU bed.

This paper examines the use of the 3-D virtual world Second Life to explore basic mechanics in physics. In Second Life, students can create scripts that take advantage of a virtual physics engine in order to conduct experiments that focus on specific phenomena. The paper explores two particular examples of this process: 1) the movement of an object under the influence of gravity, and 2) the movement of an object using simple forces. Findings suggest that Second Life offers a flexible and wide range of possibilities for simulations in mechanics; paradoxically, however, the environment also presents challenges for effective use by instructors and learners. Any implementation making use of the Second Life application requires technical knowledge of the system and a wide range of pedagogical and learner skills related to building, scripting, and educational design.

Transcript stability is associated with many biological processes, and the factors affecting mRNA stability have been extensively studied. However, little is known about the features related to human long noncoding RNA (lncRNA) stability. By inhibiting transcription and collecting samples in 10 time points, genome-wide RNA-seq studies was performed in human lung adenocarcinoma cells (A549) and RNA half-life datasets were constructed. The following observations were obtained. First, the half-life distributions of both lncRNAs and messanger RNAs (mRNAs) with one exon (lnc-human1 and m-human1) were significantly different from those of both lncRNAs and mRNAs with more than one exon (lnc-human2 and m-human2). Furthermore, some factors such as full-length transcript secondary structures played a contrary role in lnc-human1 and m-human2. Second, through the half-life comparisons of nucleus- and cytoplasm-specific and common lncRNAs and mRNAs, lncRNAs (mRNAs) in the nucleus were found to be less stable than those in the cytoplasm, which was derived from transcripts themselves rather than cellular location. Third, kmers-based protein−RNA or RNA−RNA interactions promoted lncRNA stability from lnc-human1 and decreased mRNA stability from m-human2 with high probability. Finally, through applying deep learning−based regression, a non-linear relationship was found to exist between the half-lives of lncRNAs (mRNAs) and related factors. The present study established lncRNA and mRNA half-life regulation networks in the A549 cell line and shed new light on the degradation behaviors of both lncRNAs and mRNAs.

Creating and visiting Web memorials represent new opportunities for post-death ritual. A content analysis was conducted on a sample of 244 of the memorials found on the largest Web Cemetery: Virtual Memorial Gardens (catless.ncl.ac.uk/Obituary/memorial.html). Analyses revealed that memorials were written, in descending order of prevalence, by children (33%), friends (15%), grandchildren (11%), parents (10%), siblings (8%), spouses (4%), and various other family members. This pattern favoring younger authors may reflect the newness of this venue and facility with computer technology. The content of such memorials often contained reference to missing the deceased, rarely spoke of the cause of death, or made mention of God or religion. Memorials were more likely to be written to the deceased (e.g., in the form of a letter) rather than about or for the deceased (e.g., eulogy/obituary or tribute). Parents, family groups, and other relatives more frequently made religious references in their memorials than did other authors. In addition to the Web as a novel, untapped data source, these memorials offer intriguing opportunities for theoretical refinement (i.e., the ongoing connection between the bereaved and the deceased).

The authors in this article each bring a particular insight to the overarching question of how pedagogical and facilitation approaches can deepen and strengthen a lifelong value attached to music learning and engagement in leisure music activities. It is argued that when learner voice is privileged and social connections are embedded in pedagogical approaches early on, for example in formal school music or extracurricular youth learning contexts, this will likely underpin lifelong habits of engagement in valued, meaningful leisure activity. A lifelong learning mindset, oriented around serious leisure, can in turn promote the quality of later life, when a potential sense of loss related to personal work and family identity can be profound.

Abstract Risk reduction and prevention of dementia in older adults is a growing research area. In the Finnish Geriatric Intervention Study to Prevent Cognitive Impairment and Disability (FINGER randomized controlled trial) a 2-year multidomain intervention -dietary counseling, exercise, cognitive training, vascular and metabolic risk monitoring- improved cognition in older adults from the general population who had increased dementia risk. The intervention was associated also with improvement of other clinical outcomes (e.g., multimorbidity, functional status). The FINGER model is being adapted and tested in different populations and settings through the World-Wide FINGERS, the first global network of multidomain prevention trials, including over thirty countries. The network goal is to identify effective and feasible solution for dementia risk reduction across the spectrum of cognitive decline - from at-risk asymptomatic states to early-symptomatic stages. Through the World-Wide FINGERS-SARS-CoV-2 initiative, the network aims to assess the effects of the COVID-19 pandemic in older adults.

Abstract Cognitive reserve (CR) refers to adaptability allowing for better cognitive outcomes given the degree of brain changes or other risk factors for cognitive decline. Despite significant research efforts, our knowledge of cognitive reserve proxies remains limited. Studies predominantly use a single sociodemographic variable (e.g., education attainment) as a proxy measure when CR can manifest in multiple domains. Studies also tend to rely on older samples, whereas adversity factors of cognitive performance may have differing onset ages, suggesting different risk or protective mechanisms at different ages. We examine two cohort datasets spanning from early adolescence (Adolescent Brain Cognitive Development study, N = 5559) up to the cusp of mid-adulthood (Colorado Adoption/Twin Study of Lifespan behavioral development and cognitive aging; CATSLife, N = 1327) to evaluate the role of CR proxies across over 100 variables. Defined as a moderator between a cognitive outcome and structural brain measure, these variables cover behavioral, environmental, physical and mental health, and psychological trait domains. Using cross-validated regularized regression, we identified dozens of factors acting as CR proxies (e.g., leisure reading as protective and screen usage as a risk factor during adolescence). We then use a within-family design to examine the moderation effect over and above genetic and environmental covariates. For example, adolescents who read more tend to show better cognitive performance given their gray matter volume (b = .093, 95%CI = [0.035, 0.152]). This study aims to identify factors that could be targeted with scalable prevention and intervention efforts earlier in life to maximize cognitive functioning.

Abstract Rowan-Virtua School of Osteopathic Medicine, The New Jersey Institute for Successful Aging (NJISA), in collaboration with the New Jersey Division of Aging Services (DoAS) and Respecting Choices, implemented a state-wide project training workforce professionals in Advanced Care Planning (ACP) conversations with older adults. The concept: proactively leverage the long-term care management relationships in community-based, high-touch programs. NJISA identified Respecting Choices, an evidenced-based program approved by the Administration for Community Living. Respecting Choices offers standardized certification for ACP facilitators and instructors, paving a pathway for workforce sustainability. To achieve state-wide penetration, NJISA partnered with NJ DoAS to target staff and professionals serving the state aging network. NJ DoAS provided access to county-level case managers serving the Jersey Assistance for Community Caregiving (JACC) program, Program of All-Inclusive Care for the Elderly (PACE) nurses and social workers, and NJ DoAS staff. Also included in training efforts were staff from select senior affordable housing sites. With funding support through the New Jersey Health Foundation, NJISA and Respecting Choices offered virtual, live training course for certification of First Steps ACP facilitators and instructors and Design and Implementation for state aging networks, and ACP Instructors. NJ DoAS modified the electronic documentation system utilized by JACC staff to capture on-going ACP conversations and outcomes. Despite barriers and mid-course corrections, training totals include (30) certified ACP Facilitators, (3) leaders for Design and Implementation, and (5) certified ACP Instructors. Response to curriculum was positive; participants indicating greater confidence navigating ACP conversations. Future plans to continue dissemination have been discussed.

Cardiac involvement is a well-known feature of neuromuscular diseases. Most commonly cardiac manifestations occur later in the course of the disease. Occasionally severe cardiac disease, including conduction disturbances, life-threatening arrhythmias, and cardiomyopathy, with its impact on prognosis, may be dissociated from peripheral myopathy. We report a case of bundle branch reentrant ventricular tachycardia as primary manifestation of myotonic dystrophy and discuss associated diagnostic and treatment challenges.

Aim/Purpose: Systematic literature reviews seek to locate all studies that contain material of relevance to a research question and to synthesize the relevant outcomes of those studies. The primary aim of this paper was to synthesize both research and practice reports on micro-credentials (MCRs). Background: There has been an increase in reports and research on the plausibility of MCRs to support dynamic human skills development for an increasingly impatient and rapidly changing digital world. The integration of fast-paced emerging technologies and digitalization necessitate alternative learning paradigms. MCRs offer time, financial, and space flexibility and can be stacked into a larger qualification, thereby allowing for a broader range of transdisciplinary competencies within a qualification. However, MCRs often lack the academic rigor required for accreditation within existing disciplines. Methodology: The study followed the PRISMA framework (Preferred Reporting Items for Systematic Reviews and Meta Analyses), which offers a rigorous method to enhance reporting quality. The study used both academic research and practice reports. Contribution: The paper makes a theoretical contribution to the discourse about the need for innovation within existing educational paradigms for continued relevance in a changing world. It also contributes to the debate on the role of MCRs in bridging the gap between practice and academia despite the growing difference between their interests, and the role that MCRs play in the social-economic plans of countries. Findings: The key findings are that investments in MCRs are mainly in the Science, Technology, Engineering and Mathematics (STEM) and Education sectors, and have taken place mainly in high-income countries and regions – contexts that particularly value practice-accredited MCRs. Low-income countries, by contrast, remain traditional and insist on MCRs that are formally accredited by a recognized academic institution. This contributes to a widening skills gap between low- and high-income countries or regions, which results in greater global disparities. There is also a growing divide between academia and practice concerning their interest in MCRs (a reflection of the rigor versus relevance debate), which partially explains why many global and larger organizations have gone on to create their own learning institutions. Recommendations for Practitioners: We recommend that educational mechanisms consider the critical importance of MCRs as part of innovative efforts for life-wide (different sectors) and life-long (same sector) learning, especially in low-income countries. MCRs provide dynamic mechanisms to fill skills gaps in an increasing ruthless international battle for talent. Recommendation for Researchers: We recommend focused research into skills and career pathways using MCRs while at the same time remaining responsive to transdisciplinary efforts and sensitive to global and local changes within any sector. Impact on Society: Work and society have transformed over time, and more so in the new digital age, yet academia has been slow in adapting to the changes, forcing organizations to create their own learning institutions or to use MCRs to fill the skills gap. The purpose of education goes beyond preparing individuals for work, extending further to creating an environment where individuals and governments seek their own social and economic outcomes. MCRs provide a flexible means for co-creation between individuals, education, organizations, and government that could stem global rising unemployment, social exclusion, and redundancy. Future Research: Future research should focus on the co-creation of MCRs between practitioners and academia.
