Objectives. Cannabis policy evaluations commonly assume equal policy exposure across a state’s population using date of implementation as the key independent variable. This study aimed to explore policy knowledge as another measure of exposure and describe the sociodemographic, cognitive, and behavioral correlates of cannabis policy knowledge in young adults in Vermont. Methods. Data are from the PACE Vermont Study (Spring 2019), an online cohort study of Vermonters (12-25). Bivariate and multivariable analyses estimated prevalence ratios (PR) for correlations between knowledge of Vermont’s cannabis policy (allowed possession for adults 21 and older) and sociodemographics, cannabis use, and harm perceptions in 1,037 young adults (18-25). Results. Overall, 60.1% of participants correctly described the state’s cannabis policy. Being younger, Hispanic, non-White race, and less educated were inversely correlated with policy knowledge. Ever (PR=1.37; 95% CI 1.16-1.63) and past-30-day cannabis use (PR=1.27; 95% CI 1.12-1.45) were positively correlated with policy knowledge. Policy knowledge was more prevalent among young adults who perceived slight risk of harm from weekly cannabis use (vs. no risk; aPR=1.28; 95% CI 1.11-1.48) or agreed that regular cannabis use early in life can negatively affect attention (vs. disagree; aPR=1.55; 95% CI 1.22-1.97). Conclusion. Findings suggest that 40% of Vermont young adults in the study were unaware of current state cannabis policy and that policy knowledge was lower in younger, less educated, Hispanic, and non-White young adults. Future research should explore using a measure of policy knowledge as an exposure or moderator variable to better quantify the effects of changes in cannabis legal status on perceptions and use in young people.

Abstract The growth, phenology, and susceptibility to winter injury of 82 black walnut provenances were compared in a northwestern Vermont provenance test plantation. After seven growing seasons, provenances from MI, central OH, northern IN, and PA exhibit the best combination of growth, budbreak, and winter hardiness characteristics and are recommended for planting in Vermont's Champlain and Connecticut river valleys. Provenances from MI appear to be particularly well-suited to the environment of Vermont's Champlain Valley, as exemplified by a provenance from Volinia, MI which is 26% taller than the plantation average and among the latest to begin growth in spring. Despite fast growth, provenances from KY, IL, and VA do not appear suitable for planting in Vermont because of their relatively early budbreak and high susceptibility to winter injury. Testing during the COVID-19 pandemic has been crucial to public health surveillance and clinical care. Supply chain constraints—spanning limitations in testing kits, reagents, pipet tips, and swabs availability—have challenged the ability to scale COVID-19 testing. During the early months, sample collection kits shortages constrained planned testing expansions. In response, the University of Vermont Medical Center, University of Vermont College of Medicine, Vermont Department of Health Laboratory, Aspenti Health, and providers across Vermont including 16 area hospitals partnered to surmount these barriers. The primary objectives were to increase supply availability and manage utilization. Within the first month of Vermont’s stay-at-home order, the University of Vermont Medical Center laboratory partnered with College of Medicine to create in-house collection kits, producing 5000 per week. University of Vermont Medical Center reassigned 4 phlebotomists, laboratory educators, and other laboratory staff, who had reduced workloads, to participate (requiring a total of 5.3-7.6 full-time equivalent (FTE) during the period of study). By August, automation at a local commercial laboratory produced 22,000 vials of media in one week (reducing the required personnel by 1.2 FTE). A multisite, cross-institutional approach was used to manage specimen collection kit utilization across Vermont. Hospital laboratory directors, managers, and providers agreed to order only as needed to avoid supply stockpiles and supported operational constraints through ongoing validations and kit assembly. Throughout this pandemic, Vermont has ranked highly in number of tests per million people, demonstrating the value of local collaboration to surmount obstacles during disease outbreaks and the importance of creative allocation of resources to address statewide needs.

Objectives. In 2006, the Vermont Department of Health was asked to respond to a potential cluster of sarcoidosis cases related to a Vermont office building. Sarcoidosis prevalence has not been formally described for the United States. A range of <1–40/100,000 is commonly reported; however, we have not identified primary sources supporting this conclusion. Because of the wide prevalence range and lack of a local estimate, confirming existence of a cluster was difficult. Methods. We ascertained the prevalence of sarcoidosis cases in Vermont by using insurance claims data to determine whether or not a cluster of sarcoidosis cases was related to the office building. We calculated county and state annual prevalence proportions for sarcoidosis for 2004 and 2005 and annual building prevalences for 1992–2006. Results. The pooled sarcoidosis case prevalence for Vermont was 66.1/100,000. The pooled building annual prevalence (1,128/100,000) was statistically different from the county in which the building is located (odds ratio = 15.5, 95% confidence interval 3.0, 50.3). Conclusions. We reported the first statewide sarcoidosis prevalence in the United States. This prevalence exceeded previous limited and unsubstantiated U.S. reports. Even with Vermont's elevated sarcoidosis prevalence, the presence of a cluster in this building was apparent.

Outcome was assessed by clinicians blind to history.ResultsVermont subjects alive at follow-up (n = 180) were more productive (P < 0.0009), had fewer symptoms (P < 0.002), better community adjustment (P < 0.001) and global functioning (P < 0.0001) than Maine subjects (n = 119).ConclusionsOutcome differences may be due to Vermont's model programme and a policy of allowing an earlier opportunity for community life.

In May 2016, protesters affiliated with the student organization Justice for Queer and Trans Students occupied the University of Vermont’s (UVM) Dean of Libraries’ office to demand that the David W. Howe Memorial Library (then known as the Bailey/Howe Library) immediately convert select gender-segregated restrooms—meaning those labeled for use for only men or only women—to gender-free restrooms to accommodate the needs of trans and gender non-conforming students during spring finals week. The protest followed the delivery of a petition with more than 1,000 signatures seeking change, as well as ongoing conversations between the libraries and student activists. Student organizer Lindsey “Z.” McCarron told The Vermont Cynic, “This is an immediate and real need that is affecting people’s health and safety every day.”

The Vermont Self-Determination Project is a model program for enhancing the self-determination of people with developmental disabilities in a rural state. Definitions of self-determination differ in a larger political perspective from those used in systems of supports and for people with disabilities. A brief historical overview demonstrates individualized funding and self-managed supports in Canada and the United States. Rural Vermont's developmental services system enhances opportunities for self-determination through the unique program qualities of the Self-Determination Project. These qualities include circles of support, facilitating self-advocacy, system-wide change, and a triad team approach, in addition to approaches to self-determination that include self-management and self-direction. Significant project outcomes point to areas for future exploration by rural as well as urban researchers and practicioners.

The Missisquoi River originates in a densely forested, hilly, and lightly populated region in northern Vermont, USA, flowing north until it crosses the Canadian border. The upper American stretches of the river are federally designated as “Wild and Scenic” in recognition of its outstanding natural, cultural, and recreational values. This paper reports on the place-based and river-focused perspectives of rural residents who live and work along the Missisquoi River and its tributaries and who are the recipients of Vermont’s shifting river management strategies. The mixed methods research drew on participant observation, interviews, and interpretations of photovisualizations (PVZs). The PVZ method identified the different geographical imaginaries held by residents and conservation professionals, demonstrating that PVZs can be used as a method to foster dialogue about sense of place and conservation initiatives. Visual aids can help unveil the complex, temporal relationships between landowners and the adjacent waterways, which in turn influence participation in river restoration efforts.

Abstract Vermont's nonindustrial, private forest lands (NIPF) have the potential of supplying 3.3 million cords of fuelwood annually to the Northeast energy market. Annual commercial production levels during the energy crisis of the early 1980s, however, only reached 660,000 cords; 342,000 cords came from NIPF. An additional 340,000 cords were cut and consumed by NIPF owners. Low stumpage prices, landowner self-consumption, adverse esthetic impacts, and low stumpage volumes restricted the actual amount of NIPF wood sold to the region's commercial energy market. Fuelwood harvest in Vermont was associated with large parcel sizes (>100 ac), the landowner's personal use of fuelwood, and the existence of a forest management plan. Even under these conditions, the percentage of NIPF owners willing to sell to the commercial market remained small (25%). If fuelwood is to establish itself as a reliable energy resource in the Northeast, large ownerships must be protected from subdivision and fuelwood stumpage prices must increase dramatically. Historians of transnational and global relations have increasingly reminded us that fighting in “someone else’s war” is not a new phenomenon. Despite the past two centuries being ones of state building and bordering, transnational volunteers have participated in many conflicts. These include soldiers who left British North America (bna) to enlist in the American Civil War. This research note describes a method for distinguishing cross-border enlistments from the many bna-born immigrants already living in the United States. By cross-referencing online data archives, it was possible to create an original database of bna-born individuals who lived in the Eastern Townships of Canada East in 1861 and crossed the border to enlist in Vermont. Next, a comparison of patterns of enlistment behaviour shows the transnational volunteers’ actions closely tracking those of native-born and naturalized Vermonters. Both groups followed the same trajectory, with high rates of volunteering in early months, a decline, and then a spike in late 1863, when federal authorities deployed more generous bounty payments as a policy instrument to alter the incentive structure for enlistment. Nonetheless, seventy percent of these bna residents had already enlisted before September 1863. This note suggests that transnational volunteering from bna to the Civil War merits further attention from both comparative and transnational historians.

Open government data has proliferated across every level of government in the 2010s, but research has focused primarily on national or municipal portals, which may obscure the challenges faced in providing open government data in less densely populated areas. This research focuses on the cases of three US states- Maine, New Hampshire, and Vermont. We examine the stated goals of each portal and any policies related to their establishment or upkeep. We then examine the portals with regard to updating, reuse, organization and other factors. Of the three cases, Vermont’s portal is moderately successful and continues to be used. New Hampshire’s strategy of linking to data on agency websites is inconsistent, but the state law requiring data published to be in open formats does mean data is more open when it is provided. Maine’s portal went dormant soon after its initial creation, and was fully taken down in the timeframe of this research. These cases illustrate that the establishment of a state portal alone does not guarantee that the portal will support the desired outcomes.

A large, stoneware jug in the collection of the Bennington Museum bears poignant witness to the temperance movement and contradictions inherent in Americans' attitudes towards the consumption of alcohol during the mid-nineteenth century. Made in 1859 by the Norton Stoneware Factory in Bennington, Vermont, the jug is an impressive 12 gallons in size and bears a prominent cobalt decoration depicting a compote of fruit resting on a base composed of two intertwined snakes. Above this decoration, on the jug's shoulder is a clay roundel inscribed: LUMAN P. NORTON/ 12 gals/1859/ IN VINO VERITAS. In addition to the date of manufacture and capacity, this mark notes who the jug was made for, a member of the family who owned the stoneware factory, and an interesting Latin phrase that can be translated, ““in wine [there is] truth.”” The combination of this phrase, the motif of the intertwined snakes, and Luman Preston Norton's role on Vermont's temperance movement are examined to provide a window into the conflicted attitudes about alcoholic consumption that existed in mid-nineteenth-century America and the way many tried to find a balance.

Abstract Objectives Falls are a leading cause of injury, death, functional impairment, and a driver of health costs and utilization in older adults. However, falls among older adults are mostly preventable and therefore fall risk reduction is a high value target for intervention. Poor nutritional status is a risk factor for falls and impedes recovery from falls. Understanding the role of rurality and seasonality in the relationship between falls, nutrition, and access to and use of community nutrition programs is imperative to planning future interventions to decrease fall risk. This pilot study will evaluate these relationships over time in a cohort of older Vermonters within the context of rurality and seasonality. Methods Using an observational, analytic study design, we will collect data at two points: baseline (fall 2020) and 6 months later (spring 2021) via an online survey. Adults, 60 and older and residing in Vermont year round were eligible to participate. We collected demographic and community program utilization data as well as responses to three validated questionnaires: CDC STEADI fall risk checklist, Seniors in the Community: Risk Evaluation for Eating and Nutrition, Version III, and the 2-item Food Insecurity screen. We used descriptive statistics to analyze demographic data and chi square analyses to identify associations between food insecurity, nutrition risk, fall risk and falls. Results At baseline, 53% of the 708 respondents had an elevated fall risk, 42% had a fall in the past six months, 11% were experiencing food insecurity, and 55% were at a nutrition risk. Additionally, 72% of respondents self-identified as living in a rural setting. Overall, community resource utilization was low: 2% of respondents reported using Meals on Wheels, 7% SNAP, and 8% food banks. We found a significant association (P &lt; 0.001) between fall risk and food insecurity, fall risk and nutrition risk, and food insecurity and nutrition risk. Conclusions Our baseline data provides evidence for a significant relationship between nutrition, food insecurity, and fall risk in community-dwelling older adults. Follow-up data will provide important information for understanding these relationships over time and within the context of rurality and seasons, particularly as this data was collected during the COVID-19 pandemic. Abstract Background In 2017 The Joint Commission required all hospitals irrespective of size to implement antimicrobial stewardship programs (ASPs) using the CDC core elements (CE) for antimicrobial stewardship (AS). Critical access and rural community hospitals have struggled with developing effective ASPs. Many ASPs seek to reduce fluoroquinolone (FQ) prescribing due to its high risk for drug-drug interactions, risk of Clostridioides difficile infection, and numerous side effects, including five black box warnings from the FDA. Methods We contracted with the Vermont Department of Health to help rural VT hospitals develop ASPs that are compliant with the CDC CE for AS. Six of Vermont’s 13 hospitals were recruited between June – December 2017 (Table 1). Each hospital obtained antibiotic usage (AU) data in grams (g)/1000 (1k) patient-days (PD) from their electronic medical record (EMR), starting from January 2017. All identified FQ as frequently prescribed antimicrobials. Each hospital had unique interventions to decrease FQ prescribing (Table 1), including orderset changes and pharmacist intervention. Monthly combined FQ (ciprofloxacin + levofloxacin) administration data were collected in g/1K PD. AU data from each hospital were summed and expressed as total FQ g/1000 patient-days. The FQ prescribing trend was analyzed by linear regression. Results Prior to implementing ASP, there was a combined FQ rate of 69 g/1K PD. After 20 months of ASP interventions, combined FQ prescribing decreased to 26 g/1K PD (Figure 1, R= 0.9797, P < 0.001). This trend is also significant for each individual FQ: ciprofloxacin (R=0.8364, P < 0.05) and levofloxacin (R= 0.9801, P < 0.01). Conclusion Rural and critical access hospitals can have successful antimicrobial stewardship programs. We have shown that rural hospitals in Vermont (1) can extract AU data from their EMR, (2) develop interventions to decrease high use antimicrobial agents, and (3) be successful in decreasing FQ prescribing in less than 2 years. This survey was conducted to identify specific environmental sources of Listeria and Yersinia in Vermont dairy plants, and to further determine whether the type of plant and specific conditions existing within plants influenced the incidence of positive microbiological results. A total of 361 environmental samples, focusing on floors and other nonproduct contact surfaces, was taken from all of Vermont's 34 dairy processing plants. The incidence of Listeria monocytogenes (1.4%) was low compared to the incidence of Listeria innocua (16.1%). While only 2.5% yielded other Yersinia species, 10.5 % of the sites were positive for Yersinia enterocolitica. Sites positive for either Listeria or Yersinia were statistically more likely to produce a positive result for both (P&lt;.05). Fluid plants had the highest incidence of both Listeria and Yersinia when compared to cheese plants or other types of dairy manufacturing plants. Areas associated with case washers in fluid plants had the highest incidence of microbial contamination. An additional area of concern for all types of plants was sanitizing floor mats and foot baths from which positive microbiological results were obtained. Contamination in wet areas was significantly greater than in dry areas of the plants (P&lt;.05). Identification of the sources and conditions associated with these problematic bacterial pathogens is an important step in learning to control their incidence in dairy processing environments.

The Vermont (VT) Global Warming Solutions Act (GWSA, 2020) sets greenhouse gas (GHG) emissions reduction targets at 26% below 2005 by 2025, 40% below 1990 by 2030 and 80% below 1990 by 2050 for energy-related emissions only. Vermont’s omission of GHG emissions from land conversions could result in significant costs of inaction (COI), which could hinder the state’s mitigation and adaptation plans and result in climate crisis-related risks (e.g., credit downgrade). Science-based spatio-temporal data of GHG emissions from soils because of land conversions can be integrated into the conceptual framework of “action” versus “inaction” to prevent GHG emissions. The application of soil information data and remote sensing analysis can identify the GHG emissions from land conversions, which can be expressed as “realized” social costs of “inaction”. This study demonstrates the rapid assessment of the value of regulating ecosystems services (ES) from soil organic carbon (SOC), soil inorganic carbon (SIC), and total soil carbon (TSC) stocks, based on the concept of the avoided social cost of carbon dioxide (CO2) emissions for VT by soil order and county using remote sensing and information from the State Soil Geographic (STATSGO) and Soil Survey Geographic Database (SSURGO) databases. Classified land cover data for 2001 and 2016 were downloaded from the Multi-Resolution Land Characteristics Consortium (MRLC) website. These results provide accurate and quantitative spatio-temporal information about likely GHG emissions, which can be linked to VT’s climate action plan. A failure to considerably reduce emissions from land conversions would increase climate change costs and potential legal consequences for VT and beyond its borders.

This study investigates the extent to which land use and management transitions on Vermont’s farmland could sequester atmospheric carbon in the soil. We weigh the sequestration potential of several types of regenerative agricultural practices against both business as usual and afforestation scenarios using the Rothamsted Carbon Model. We split the study area into 13 Ecoregions for a finer spatial scale of analysis, with key climate, soil, and land use data specified for each. Empirical soil laboratory data are used to initialize the model to mirror current conditions under each of three agricultural land uses (crops, hay, and pasture) in each Ecoregion. We consult experts as well as the literature to parameterize the anticipated effects of alternative agricultural management practices on soil carbon inputs. In the simulation runs, we find that all non-business-as-usual scenarios sequester carbon over time, with a higher rate of sequestration in the decades immediately after a land use or management change. Among the regenerative agriculture scenarios, conversion to rotational grazing offers the highest soil carbon sequestration potential, at 1,269 kt, or 5.3% above current stocks after ten years. Of all scenarios, afforestation of farmland to non-harvested forest stores the most soil carbon, increasing stocks by 6.5% after ten years, and continuing to sequester at a high rate many decades into the future. We discuss tradeoffs and policy implications, especially in the context of the 2020 Vermont Global Warming Solutions Act, and suggest that payments for ecosystem services for farmers sequestering carbon may have strategic value.

The Ordovician Stanbridge Group of Quebec has long been considered an allochthonous nappe. It is an internally coherent unit that consists of lower slaty limestone overlain by slate, which is correlated with the Highgate and Morses Line formations, respectively, in Vermont. In Quebec, the basal limestones have been inferred to be thrust over Cambrian dolomites (Gorge Formation in Vermont) of the Rosenberg slice, part of the parauthochthonous shelf, although this contact is not exposed there. In the Missisquoi River gorge of Vermont, a conformable sequence of upper Gorge–Highgate–Morses Line formations is exposed. The map distribution of rock units indicates that this conformable relationship probably extends up to at least the middle unit of the Stanbridge Group. Therefore, the relationships in Vermont require that the Stanbridge Group must be part of the parauthochthonous Taconic foreland rather than a far-traveled nappe. The Rosenberg slice in Quebec forms a large anticline (Highgate anticline) whose western limb is truncated by the Rosenberg thrust. In Vermont, the anticline is internally cut by the Highgate Falls Thrust, which is an out-of-sequence thrust that decreases in displacement northwards to the International Border.

Childhood immunizations are invaluable in preventing contagious diseases. Nonetheless, vaccines have become increasingly controversial with growing numbers of caregivers refusing to vaccinate their children. The percentage of fully vaccinated children in Vermont is one of the lowest nationally. This study set out to determine Vermont caregivers’ attitudes toward immunizations to better explain why the percentage of fully vaccinated children has fallen in Vermont. A survey regarding caregivers’ health care knowledge about children, their vaccination concerns, and their children’s vaccination status was sent to participants in the Vermont Women, Infants and Children’s Program from two districts. In total, 83% ( n = 379) of respondents reported their children received all recommended vaccinations for their age. Respondents who considered themselves highly knowledgeable regarding their children’s health care and confident about the safety of vaccinations were significantly associated with reporting their children as being current on vaccinations and with their intent to continue vaccinations. Respondents indicated highest concern regarding the safety and number of vaccinations administered during one visit. Primary care providers were indicated as important resources for addressing concerns about vaccinations and health care knowledge of children. The results help to understand low vaccination rates in Vermont and can be used for targeting health campaigns to improve vaccination rates.

Migrant dairy workers in Vermont face a wide range of occupational and health hazards at work. This research examines the environmental risks, occupational health hazards, and health outcomes experienced by migrant dairy farm workers in Vermont. This research draws on a triangulation of sources including analysis of data—surveys and interviews with migrant dairy farmworkers gathered by the organization Migrant Justice since 2015 as well as relevant key informant interviews with community organizations across the state to characterize the occupational health experiences of migrant dairy workers in Vermont. Our results show that Vermont migrant dairy farmworkers received poor health and safety training and lacked sufficient protective gear. Over three quarters of the respondents reported experiencing harm from chemical and biological risks. Close to half the survey respondents reported headaches, itchy eyes and cough; a quarter reported breathing difficulties; three fourths reported being hurt by animal-related risks. These exposures and existing health concerns are avoidable. Migrant workers require better social representation and advocates to negotiate better work-related protection and training, access to health services, and social welfare to ensure their health and safety.

Abstract Background In the United States and Vermont, men who have sex with men (MSM) make up the majority of new human immunodeficiency virus (HIV) infections every year. Despite HIV prevention campaigns and approval of antiviral therapy for pharmacologic HIV pre-exposure prophylaxis (PrEP), HIV cases in Vermont—a predominantly rural state—are on the rise. The primary objective of this study was to assess prescribing practices and barriers surrounding PrEP for adult MSM in Vermont. Methods A web-based healthcare provider survey was deployed electronically over a 10 week period in 2019 to a convenience sample of licensed primary care, sexual health, and infectious disease providers in Vermont. Questions were designed to target factors thought to influence PrEP prescribing, with a focus on prescribing behaviors and perceived barriers. Results An estimated 500 providers received the survey. There were 137 survey respondents, 106 (77%) were physicians, primarily in internal medicine. Though only 47 (34%) providers had experience prescribing PrEP to MSM patients, over 89% identified as willing to prescribe PrEP to high-risk groups. Among PrEP prescribers, screening frequency for HIV and bacterial sexually transmitted infections (STIs) while on PrEP fell below the current guideline recommendations at 72% and 53%, respectively. Less than 70% of providers routinely obtain sexual history for male patients. Among providers willing to prescribe PrEP, concern regarding medication toxicity was the only statistically significant barrier (χ 2 = 5.5, P = 0.02). Concerns regarding risk compensation behavior and lack of knowledge or experience regarding prescribing PrEP also demonstrated an association with provider willingness to prescribe PrEP, however did not reach statistical significance. Conclusion The majority of Vermont providers sampled are willing to prescribe PrEP, suggesting there is great opportunity to increase prescribing and use, potentially having an impact on reducing HIV transmission among MSM in the state. Provider education targeted toward guidelines for STI and HIV screening on therapy, obtaining sexual histories, and minimal toxicity risk may serve to increase prescribing of PrEP among Vermont providers. Does willingness to pay a premium for local specialty food products differ between consumers in Maine, New Hampshire, and Vermont? Two food categories are investigated: low-end ($5) and high-end ($20) products. Premia estimates are compared across states and across base prices within states using dichotomous choice contingent valuation methods. Results suggest that the three states of northern New England have many similarities, including comparable price premia for the lower-priced good. However, there is some evidence that the premium for the higher-priced good is greater for the pooled Vermont and Maine treatment than for the New Hampshire treatment. Vermont and New Hampshire residents are willing to pay a higher premium for a $20 than for a $5 food item, while the evidence suggests that Maine residents are not.
