OBJECTIVES/GOALS: The Rockefeller University CCTS, Clinical Directors Network (CDN), and Carter Burden Network (CBN) received a DHHS-Administration for Community Living Nutrition Innovation grant to test whether implementation of DASH-concordant meals and a program to enhance self-efficacy, could lower blood pressure among seniors aging in place. METHODS/STUDY POPULATION: CEnR-Nav model to engage stakeholders, enroll seniors age >60 yr., eating 4 meals a week at 2 CBN congregate meal sites; Advisory Committee to facilitate dissemination; menus aligned with Dietary Approaches to Stop Hypertension (DASH) and New York City Department for the Aging (DFTA) nutritional guidelines; interactive sessions for education (nutrition, blood pressure, medication adherence); Omron 10 home BP devices for daily home monitoring. Plate Waste and Meal Satisfaction (Likert scale) to assess taste preference and cost impact. Outcomes: Primary: Change in Systolic BP at Month 1; change in percent with controlled blood pressure. Secondary: change in validated measures of cognitive (e.g. SF-12, PHQ-2), behavioral (Home BP monitoring), nutritional (food frequency) variables, satisfaction, costs. RESULTS/ANTICIPATED RESULTS: Menu alignment required multiple iterations. Plate Waste and Menu Satisfaction tools were developed. Site 1 enrollment began June 2019; educational sessions and home BP monitors and training were provided. Baseline mean blood pressure (Site 1) was 138/79 +20.5; (range: 7% hypertensive crisis, 36% stage 2 hypertension, 22% stage 1 hypertension, 22% elevated, and 13% normal). DASH-aligned meals began October 2019; Meal satisfaction declined briefly, chefs adjusted menus, and meal satisfaction rose to pre-intervention levels. Site 2 enrollment is ongoing; dietary intervention will start in 2020. Primary outcome data (change in BP) will be complete in March 2020. Secondary outcome data on social and behavioral impact of the interventions will also be presented. DISCUSSION/SIGNIFICANCE OF IMPACT: We leveraged our community-academic research partnership to conduct research addressing uncontrolled hypertension, an urgent unmet health need among seniors. The DASH Implementation Study can inform the broader aging services and healthcare community of the potential for congregate nutrition programs to improve cardiovascular health outcomes.

In 1964, the New York State Public Health Law, §2500a, set the stage for mandatory newborn screening. By 1978, testing was already under way when the last of the total of eight diseases was added. Screening for homozygous sickle cell disease was included in 1975. Specimens submitted from 1975 to 1984 totaled nearly 2.5 million, and these specimens were sent to four different laboratories in New York state. New York City, with the largest population of newborns, sent the largest proportion of specimens (1.1 million). Although testing was occurring, it was not until 1979 that citywide follow-up was established. Follow-up efforts were coordinated through the New York City Department of Health in cooperation with 52 hospitals. Of these hospitals, 25 received New York state funding for the provision of genetic services, which included testing and counseling for parents of children whose test results were positive for trait or disease. A cooperative effort was set up between the New York City Department of Health and health providers, who agreed to provide follow-up for newborns with all hemoglobinopathies at their respective hospitals. In cases of sickle cell disease, efforts were made to obtain second specimens from affected infants. In cases in which this was difficult, New York City Department of Health aids visited the home and obtained the specimen, counseled the parents, and arranged for further follow-up. Other cooperative efforts occurred when various genetic or sickle cell centers came together and organized committees. Two major committees, the Sickle Cell Advisory Committee, and its executive arm, the Implementation Committee, in which all providers of genetic services throughout the state are represented, met to discuss statewide problems, to identify needs, and to formulate policy.

One of the prestigious American universities, New York University, NYU, along with the entire city and state, found itself locked out and inaccessible for students, faculty and library staff since mid-March 2020. Restrictions imposed by the pandemic created an unanticipated stress on all systems of the large academic institution in the epicenter of the national outbreak. The article describes in brief the unique nature of the university and its library system, its global reach and associated established library practices. The focus of the article are the activities of KARMS, the cataloging department of the NYU Divisions of Libraries. It describes the way the cataloging department recalibrated its services to allay immediate crisis of the constituent communities in the middle of the final semester of an academic year. The article also details a number of long-term initiatives that received a much needed attention under the conditions of the so-called “distance working”. While the work of any cataloging department is intrinsically linked to physical objects, the article offers several examples of the projects that can be completed remotely

Abstract Paid caregivers (e.g. home health aides, personal care attendants) are formally tasked with helping older adults with functional impairment meet their basic needs at home. This study used semi-structured interviews (n=30) with dyads of patients or their proxies and their paid caregivers in New York City to 1) understand the range of health-related tasks paid caregivers perform in the home and 2) determine if these tasks are taught in the New York State government’s Department of Health curricula. We found that patients, proxies, and paid caregivers all described that paid caregivers performed a wide range of health-related tasks that were often not a part of their formal training. Creating clear competencies for paid caregivers that reflect the full breadth of health-related tasks they may perform at home will help maximize the potentially positive impact of the paid caregiver workforce on the lives of patients living at home with functional impairment.

The Athletic Department at Sizeable State University has struggled both on and off the field in recent years. With a majority of their teams finishing in the bottom half of the conference 3 years in a row, student attendance at home athletic events is at an all-time low. Despite these challenges, the women’s basketball team has found success in recent years even finishing first in the conference last season and making a trip to the NCAA tournament. With an incoming student class made up of majority females, and a growing mound of research on female sports fandom, the athletic director has challenged the marketing team to specifically target female college students with their marketing initiatives in the upcoming season. Driven by a largely female leadership team at Sizable State University, Grace Perkins, the associate athletic director for marketing, has been tasked with developing these new marketing strategies which will drive female student attendance at home athletic events, with particular focus on home women’s basketball games.

The Psychology Department at the University at Buffalo, SUNY offers mentorship-based, research-intensive Masters’ and Doctoral degrees in Cognitive and Behavioral Neuroscience Areas of Psychology. Faculty specialize in audition, vocal production, psycholinguistics, bioacoustics, language development, attention, music cognition, and learning. Specific expertise includes auditory and vocal processing and multisensory integration across the lifespan. Study populations include mice, whales, and human children and adults, including special populations such as musicians, bilinguals, and people with conditions like aphasia, autism, dyslexia and attention deficit hyperactivity disorder. Methodologies include, but are not limited to, eye tracking, functional magnetic resonance imaging, motion capture, animal psychophysics, electroencephalograms, and surface electromyography. The program is collaborative, collegial, and supportive, and the graduate student stipends are competitive in an area of the country with a low cost of living. Students train with other students and faculty to become experts in their chosen area of study. AbstractIntroductionOn October 29, 2012, Hurricane Sandy touched down in New York City (NYC; New York USA) causing massive destruction, paralyzing the city, and destroying lives. Research has shown that considerable damage and loss of life can be averted in at-risk areas from advanced preparation in communication procedures, evacuation planning, and resource allocation. However, research is limited in describing how natural disasters of this magnitude affect emergency departments (EDs).Hypothesis/ProblemThe aim of this study was to identify and describe trends in patient volume and demographics, and types of conditions treated, as a result of Hurricane Sandy at Staten Island University Hospital North (SIUH-N; Staten Island, New York USA) site ED.MethodsA retrospective chart review of patients presenting to SIUH-N in the days surrounding the storm, October 26, 2012 through November 2, 2012, was completed. Data were compared to the same week of the year prior, October 28, 2011 through November 4, 2011. Daily census, patient age, gender, admission rates, mode of arrival, and diagnoses in the days surrounding the storm were observed.ResultsA significant decline in patient volume was found in all age ranges on the day of landfall (Day 0) with a census of 114; -55% compared to 2011. The daily volume exhibited a precipitous drop on the days preceding the storm followed by a return to usual volumes shortly after.A notably larger percentage of patients were seen for medication refills in 2012; 5.8% versus 0.4% (P<.05). Lacerations and cold exposure also were increased substantially in 2012 at 7.6% versus 2.8% (P<.05) and 3.8% versus 0.0% (P<.05) of patient visits, respectively.A large decline in admissions was observed in the days prior to the storm, with a nadir on Day +1 at five percent (-22%). Review of admitted patients revealed atypical admissions for home care service such as need for supplemental oxygen or ventilator.In addition, a drop in Emergency Medical Services (EMS) utilization was seen on Days 0 and +1. The SIUH-N typically sees 18% of patients arriving via EMS. On Day +1, only two percent of patients arrived by ambulance.ConclusionThe daily ED census saw a significant decline in the days preceding the storm. In addition, the type of conditions treated varied from baseline, and a considerable drop in hospital admissions was seen. Between 1997 and 1998, the Public Archaeology Facility, at Binghamton University, was contracted by the New York State Education Department on behalf of the Department of Transportation to conduct investigations of the Plus site (SUBi-1736; NYSM #10494), located in the Town of Dryden, Tompkins County, New York. The site occupies an inland setting in the Virgil/Fall Creek watershed, lying on a glacial gravel hill adjacent to Willow Glen Cemetery along NY Route 13. Site examination and data recovery excavations in threatened areas of the site revealed a cluster of Late Woodland cultural features including hearths, a storage/refuse pit, and postmolds. The artifactual materials from these cultural features suggest the Plus site functioned as an Iroquois remote camp occupied during the late 14th century. The Plus site represents the first Iroquois remote campsite in central New York to receive intensive subsurface investigation.

Millard, Sao Paulo, Brazil, Dr. Ely Karmon, Senior Research Scholar, International Policy Institute for Counter-Terrorism, Kerzlyia, Israel; and Dr. Harvey W. Kushner, Professor and Chair, Department of Criminal Justice and Security Administration, Long Island University, Brookville, New York, USA. Other presentations were made by Dr. David Rapoport, University of California, Los Angeles, California, USA; Niles Lathem, The New York Post, Washington, D.C., USA, Arvind Verma, Department of Criminal Justice, Indiana University, Bloomington, Indiana, USA, Dr. S. Subramanian, Raghavendra Nagar Shvrampally, Hyderabad, India, George Ballard, Grand Valley State University, Allendale, Michigan, USA and Boaz Ganor, International Policy Institute for Counter-Terrorism, Herzlyia, Israel. The papers and the sessions focused on a number of themes, including an assessment of the main contemporary trends in terrorism, the politicalization of terrorism, the effects that terrorism has on primary and secondary victims, the linkage of terrorism with organized crime, and the measures that governments, international organizations, and justice agencies can take to curtail and eradicate terrorism, including international cooperative efforts.

Florentin Smarandache is known as scientist and writer. He writes in three languages: Romanian, French, and English. He graduated the Department of Mathematics and Computer Science at the University of Craiova in 1979 first of his class, earned a Ph. D. in Mathematics from the State University Moldova at Kishinev in 1997, and continued postdoctoral studies at various American Universities such as University of Texas at Austin, University of Phoenix, etc. after emigration. He did post-doctoral researches at Okayama University of Science (Japan) between 12 December 2013 - 12 January 2014; at Guangdong University of Technology (Guangzhou, China), 19 May - 14 August 2012; at ENSIETA (National Superior School of Engineers and Study of Armament), Brest, France, 15 May - 22 July 2010; and for two months, June-July 2009, at Air Force Research Laboratory in Rome, NY, USA (under State University of New York Institute of Technology). In U.S.A. he worked as a software engineer for Honeywell (1990-1995), adjunct professor for Pima Community College (1995-1997), in 1997 Assistant Professor at the University of New Mexico, Gallup Campus, promoted to Associate Professor of Mathematics in 2003, and to Full Professor in 2008. Between 2007-2009 he was the Chair of Math & Sciences Department.

The Problem. Universities struggle to create cultures that provide a welcoming home for women and underrepresented minorities. Department chairs often emerge reluctantly from among the faculty and are ill prepared to engage faculty and staff to achieve deep culture change, yet their role is vital if such change is to occur. The Solution. Recognizing a need for a completely new type of leader, Ohio State created the President and Provost’s Leadership Institute (PPLI) to develop a pool of women and underrepresented minority faculty who might move into leadership positions. The Stakeholders. The PPLI can provide a useful template for HRD practitioners who wish to tailor leadership development and succession planning strategies to address higher education’s unique culture and leadership pool challenges. For institutions interested in broadening the diversity of individuals prepared to lead, and developing leaders prepared to lead change, the PPLI is an example of one successful approach.

Jules M Rothstein, clinician, researcher, educator, author, and speaker, entered into the field of physical therapy in 1975 following graduation from the Department of Physical Therapy at New York University. He completed his Master of Arts Degree in Kinesiology in 1979 and his Doctor of Philosophy in Physical Therapy in 1983, also at New York University. During his training, he worked as Staff Physical Therapist at Peninsula Hospital Center in Queens, as Research Fellow with the Arthritis Foundation, and in private practice in Cedarhurst, New York. From 1977 to 1980, Dr Rothstein was Adjunct Instructor in the Department of Physical Therapy at New York University. From 1980 to 1983, he was Instructor and Coordinator of Clinical Research and Training Programs at Washington University School of Medicine, and from 1984 to 1990, he was Associate Professor at the Medical College of Virginia. A tenured professor at the University of Illinois at Chicago since 1990, Dr Rothstein also served as Head of the Department of Physical Therapy at the University of Illinois at Chicago and as Chief of Physical Therapy Services at the University of Illinois Hospital in Chicago until 1999. During that period, the department obtained more than $6 million in research funding and received APTA's 1997 Minority Initiative Award for consistently recruiting and maintaining ethnic and racial diversity among its students. He continues to serve as Professor in the Department of Physical Therapy and remains active in all areas of physical therapy, practice, research, and service. Dr Rothstein's expertise in measurement and research design has been used by many professionals—across disciplines—in the allied health community. He is in great demand as an invited guest speaker, having given professional presentations and keynote speeches on the topic of rehabilitation sciences at numerous national and international forums, including Sweden, Denmark, Iceland, Japan, and Saudi Arabia. He has also served as a consultant and visiting professor in South Africa, the Netherlands, and Poland. Dr Rothstein has made extensive contributions to the physical therapy profession's body of knowledge, including the publication of more than 60 refereed articles and abstracts. In 1985, he edited the text Measurement in Physical Therapy. He chaired the APTA Task Force on Standards for Measurement in Physical Therapy that produced the first APTA Standards for Tests and Measurements in Physical Therapy Practice in 1993. As part of that task force, he co-authored the Primer on Measurement: An Introductory Guide to Measurements Issues. Since 1989, Dr Rothstein has served as Editor of Physical Therapy and has been appointed to that position for three 5-year terms by the APTA Board of Directors. Dr Rothstein is a Catherine Worthingham Fellow of the American Physical Therapy Association. He is the recipient of numerous awards, including the Golden Pen Award, the Outstanding Service Award for Research, the Outstanding Service Award for Continuing Education, and the Outstanding Therapist Award in the State of Illinois. Recovery experiences of public housing residents in the Rockaways, Queens (New York City (NYC)), after Superstorm Sandy suggest that living in NYC Housing Authority (NYCHA) properties creates circumscribed opportunities for local political participation, what I call a “differentiated state.” This differentiated state is constructed from four interconnected sociospatial features: (1) tenants’ stigmatized identities, (2) U.S. Department of Housing and Urban Development (HUD) regulations, (3) NYCHA’s “para-governmental” status, and (4) spatial concentration of the developments. I empirically demonstrate this “differentiated state” based on a grounded case study of disparities in disaster recovery participation in Rockaway. This analysis offers a new spatial lens to the scholarship on policy feedbacks and delivers a new synthesis of the limits to tenants’ political participation in conventional public housing developments in the United States, home to more than two million people.

It is no secret, unhappily, that the study of theatre in the colleges and universities of this country is a discipline under siege, but the severity of the problems received strong confirmation in New York State this fall when two of the most distinguished and long-established (over a century in both cases) programs in the country were, with little warning, faced with draconian cuts or outright extinction. The fact that one, the state University of Albany, was the flagship school of the public system, and the other, Cornell University, was one of the state's most distinguished private institutions, suggests the scope and impact of these actions. At Albany, four other programs are being terminated along with theatre—Classics, Russian, Spanish, and French—while at Cornell the extent of the severe cuts imposed on the theatre program—almost a quarter of the total budget of the department (which also shelters dance and film)—are being suffered by no other program in the university. The prominence of these two schools in a state that has long claimed a central position in American theatre makes them particularly significant symbolically of a discipline in crisis, and this has impelled me to engage in serious and sometimes painful reflections on that discipline, the basis of the present essay.

This article considers the initiation history of the compound of the Patriarch of Moscow and All Russia in Pine Bush, New York State (USA). The materials of the R-6991 foundation (the Foundation of the Council for Religious Affairs attached to the Council of Ministers of the USSR), of the State Archives of the Russian Federation, as well as the Archives of the Department for External Church Relations of the Moscow Patriarchate were used as sources of the research base. The documents identified and introduced into scientific circulation for the first time made it possible to find out the reasons for the purchase of the real estate and to disclose the plans for its development among the leadership of the American Exarchate. The main factor that caused the purchase was a difficult financial situation of the Patriarchal parishes in the USA. The initiators of the purchase hoped to develop the commercial potential of the property and create an additional source of Exarchate income from it. The archival documents indicate the sale of part of the Patriarchal Estate land for residential development and for a cemetery, and at the same time there were attempts to establish a children’s camp and a private nursing home on its territory. In addition, the complex of sources made it possible to identify the hitherto unknown stages and details of the construction of the Church in Honor of All the Saints in the Land of Russia Shining – the church located within the boundaries of the courtyard. It was determined that the construction of the church began in November 1963, and in 1969 the building had been completely built. At the same time, the article answers the question why the Pine Bush estate became the subject of discussion during the negotiations between the Moscow Patriarchate and the American Metropolitanate.

Part 4 of the Act allowed the Home Secretary to certify a person as a suspected international terrorist if he reasonably believed that the person's presence in the UK was a threat to national security and reasonably suspected that he or she was an international terrorist. If the person was subject to UK immigration control (i.e., had no right of abode in the UK, not being a British national), he or she could be removed from the UK and detained pending removal under immigration legislation. If a practical consideration (such as the absence of transport links between UK and the place to which the person could be removed) or a point of law which wholly or partly related to an international agreement (for example, where removing a person to his or her country of origin would render him or her liable to torture contrary to Article 3 of the European Convention for the Protection of Human Rights and Fundamental Freedoms (ECHR)) prevented a person's removal or departure temporarily or indefinitely, the Act permitted their detention.

On June 11, 2009, the director of the World Health Organization (WHO) raised the phase of alert in the Global Influenza Plan from level five to level six. The cause for this was the H1N1 virus which had already affected several countries. A level five alert is declared when more than one country in a single WHO geographic region is affected by the same virus. A level six declaration means that community outbreaks are occurring in at least two WHO geographic regions. Once such a declaration is made, little time remains before mitigation efforts must be planned and communicated to the public. In the wake of the WHO declaration, policy makers are clamoring for adequate disease mitigation strategies. Some health departments intend to require employees to wear personal protective equipment while on the job. Other state health departments are encouraging employees to stay home sick if they think they might have the flu. The New York State Health Department has issued an order requiring all healthcare workers to be vaccinated for H1N1 or risk being terminated. This article will explore the New York State policy and make recommendations to policy makers about how to prevent the spread of H1N1.

Policy makers and researchers are intrigued with but also frequently frustrated by each other. Although these differences are understandable and predictable, it is clear that research on a variety of educational issues has been both influential and valuable in the development of policy and practice. There is much to suggest that researchers and policy makers should be collaborating to improve student outcomes. There are important instances of sustained collaborations between educational researchers and educational policy makers. We summarize some of these efforts but describe in more detail the Education Finance Research Consortium, a long-standing collaboration between university-based researchers and the New York State Education Department. Given the current intense focus on the role of evidence in the development of education policy, some of the lessons from this collaboration may be useful to those seeking to expand the use of evidence in policies intended to improve student learning.

This article is a case study of the Morris Justice Project (MJP), a participatory action research (PAR) study in a South Bronx neighborhood of New York City (NYC) designed to understand residents’ experiences with and attitudes towards the New York Police Department (NYPD). An illustration of public science, the research was conducted in solidarity with an emerging police reform movement and in response to an ongoing and particularly aggressive set of policing policies that most heavily impacts poor communities and communities of color. The case study describes a set of ongoing participatory, research-action, “sidewalk science” strategies, developed in 42 square blocks of the South Bronx, designed to better understand and challenge the ongoing structural violence of the carceral state. Collaboratively written with members of the Morris Justice collective, we tell our story across three sections that outline the genesis of the project, describe our major commitments, and offers PAR and public science as a possible “intervention” in traditional university practice.

Many potential students, because of distance from the university campus and/or job requirements, cannot take traditional courses on-campus. This group of learners is “place-bound”—a group of learners who may be employed full-time, most likely married with job responsibilities and/or other situations demanding most of their attention. The Horticultural Science Department and Graduate School at N.C. State University are addressing place-bound limitations in several ways, including the creation and offering of a Graduate Certificate Program in Horticultural Science via distance education (DE). By using DE, high demand, low-seat-available classes can offer additional enrollment for credit. Second, courses can be offered asynchronously or with alternative delivery methods. Also, courses offered collaboratively among institutions can generate a level of interest and enthusiasm that may not exist for “home-grown” courses. Such efforts as these promise to help meet continuing education demands of “non-traditional” students. These include Cooperative Extension's more than 120 Horticultural Crops Extension Agents (“field faculty”) and over 300 other field faculty whose interests include horticultural topics.

ABSTRACT Purpose—The purpose of this paper is to analyze the classical decision making model as applied to restructuring a Department on Aging services for the State of Connecticut. Mandated by the Connecticut legislature in 2007 to reestablish the department, the Department of Social Services (DSS) commissioned Southern Connecticut State University to recommend a structure for the newly legislated department, which had been disbanded by then-Governor Lowell Weicker in 1993. Design/Methodology/Approach—Qualitative interviews of relevant stakeholders at all levels revealed that while few were satisfied by the current structure of the Department on Aging, there was no consensus concerning the design of a more effective replacement, only deeply held differences. Through the collection and analysis of both qualitative and quantitative data, the decision process on how to best structure aging services becomes hostage to a contingency where this decision model breaks down. The search stage develops a set of alternatives with no clear criteria for rational selection—all choices feature competing sets of strengths and weaknesses, successfully applied by various state agencies. Findings—Decision makers from various stakeholders did agree on two general criteria: that the new department must be feasible (defined mainly in terms of affordability), and that the new department must be strong (defined mainly in terms of control over resources). In the presence of competing values concerning the optimization of effective services, clarification of feasibility and strength through accounting analysis restored a rational basis for agreement and selection of a satisfactory choice, rationalizing an otherwise paralyzed and political decision process. Originality/Value—The process used to design the Connecticut Department on Aging offers a paradigm for other states to consider in developing a structure for elderly services. Elder affairs are clearly an area where passion and complexity are heightened. As a result, accounting analysis, tempered to ensure that profits and numbers do not overshadow the socially responsible purpose of the public organization, presented the most rational approach to develop a structure. The process shows that by managing risk and substituting satisfactory for optimal goals, accounting data emerged as a tie-breaker in the presence of strong competing values. Accounting analysis restores a rational basis for agreement by abandoning the realm of idealistic optimal states and linking alternatives to hard budgetary figures. Accounting analysis grounded estimations of risks in terms of real costs, and presented alternatives in terms of incremental budgetary shifts, which are easier to understand and debate than the philosophical nature of what constitutes the ultimate “senior friendly” community. The proposed plan was actually accepted by then-Governor of the State of Connecticut, M. Jodi Rell, who declared a restoration of a Department on Aging, based upon the final recommendations of this study, effective July 1, 2009. However, for budgetary reasons, Bill SB 841 postponed the reestablishment of a state Department on Aging, but did transfer additional programs on July 1, 2010, which included the state-funded portion of the Connecticut Home Care Program for Elders (CHCPE). Since the budgetary problems of the state intensified, the administration of complex elderly programs which involve Medicare and Medicaid continue to be administrated by DSS indefinitely.

Background: The patient-centered medical home (PCMH) model, an important component of healthcare transformation in the United States, is an approach to primary care delivery with the goal of improving population health and the patient care experience while reducing costs. PCMH research most often focuses on system level indicators including healthcare use and cost; descriptions of patient and provider experience with PCMH are relatively sparse and commonly limited in scope. This study, part of a mixed-methods evaluation of a multi-year New York State initiative to refine and expand the PCMH model, describes patient and provider experience with New York State PCMH and its key components. Methods: The qualitative component of the evaluation included focus groups with patients of PCMH practices in 5 New York State counties (n = 9 groups and 67 participants) and interviews with providers and practice administrators at New York State PCMH practices (n = 9 interviews with 10 participants). Through these focus groups and interviews, we elicited first-person descriptions of experiences with, as well as perspectives on, key components of the New York State PCMH model, including accessibility, expanded use of electronic health records, integration of behavioral health care, and care coordination. Results: There was evident progress and some satisfaction with the PCMH model, particularly regarding integrated behavioral health and, to some extent, expanded use of electronic health records. There was less evident progress with respect to improved access and reasonable wait times, which caused patients to continue to use urgent care or the emergency department as substitutes for primary care. Conclusions: It is critical to understand the strengths and limitations of the PCMH model, so as to continue to improve upon and promote it. Strengths of the model were evident to participants in this study; however, challenges were also described. It is important to note that these challenges are difficult to separate from wider healthcare system issues, including inadequate incentives for value-based care, and carry implications for PCMH and other models of healthcare delivery.

During my first year of graduate school (1982–83), Louis Galambos congratulated me for having the courage to go into a dying field. Naïveté, not courage, had propelled me to leave my position as deputy director of income maintenance programs for the New York City Department of Social Services and study political history at The Johns Hopkins University. Although contact with the job market four years later would confirm my adviser’s warnings, at the time he issued this “heads up” I did wonder what Dr. Galambos had been smoking. After all, it seemed to me that politics, and particularly its bureaucratic incarnation, touched the lives of Americans more of ten and more forcefully than ever before. How could interest in this topic be declining? What’s more, by my third year in graduate school, I had mastered a rich body of literature that confirmed the centrality of politics.The Progressive synthesis, dating back to James Beard, Frederick Jackson Turner, and Vernon Parrington, was under fire but still compelled elegant work that had begun to focus less on Progressive presidents and more on the fate of liberalism as it engaged racism, sought to reconcile local preferences with national agendas, and grappled with Americans’ increasing distrust of the centrally directed programs that the New Deal and the Great Society spawned (Brinkley 1982, 1995; Chafe 1980; Gerstle 1989, 2001; McGirr 2001; Sugrue 1996).

The New York State Museum was created by State legislation in 1870 out of the old State Cabinet, which held the specimens collected by the State Geological and Natural History Survey, James Hall, then State Geologist and Palaeontologist within the Survey, was named Director of the Museum. Hall's need to possess and study vast quantities of paleontological specimens required space for collections storage and processing. His collections became the major supply of specimens for the Cabinet and eventually the Museum. After the original Survey was disbanded, in the early 1840's, Hall's presence gave the Cabinet a definite geological character. As the chief geological scientist, Hall considered the geological research of the Cabinet and later the Museum as a product of the "Geological Survey of New York," even though no formal designation of such a unit was ever proclaimed by state legislation. After all, other states were forming geological research units similar to Hall's and calling them geological surveys. It made sense for good communications for Hall and his predecessor State Geologists to refer to their staff as the New York State Geological Survey. Eventually, through a series of other legislative acts, most importantly in 1904 and 1945, the Museum was made the formal administrative home for the Geological Survey and, thus, its guardian. Museum Directors, therefore, have had the principle role in determining the fate of geological and paleontological research within the Geological Survey, After 1926, when the first non-geologist became director, the Museum's research scope grew faster in other natural and social history areas, such as botany, entomology, zoology, archaeology, ethnology, and history. This expansion is exemplified by the addition of a State Historian to the Education Department in 1895. During its 150-year history the Geological Survey has moved six times, and it is now housed in the Cultural Education Center in the Governor Nelson A. Rockefeller Empire State Plaza, Albany, New York.

IntroductionAccess to care is essential for patients with diabetes to maintain health and prevent complications, and is important for health equity. New York State’s Health Homes (HHs) provide care management services to Medicaid-insured patients with chronic conditions, including diabetes, and aim to improve quality of care and outcomes. There is inconsistent evidence on the impact of HHs, and care management programs more broadly, on access to care.Research design and methodsUsing a cohort of patients with diabetes derived from electronic health records from the INSIGHT Clinical Research Network, we analyzed Medicaid data for HH enrollees and a matched comparison group of HH non-enrollees. We estimated HH impacts on several access measures using natural experiment methods.ResultsWe identified and matched 11 646 HH enrollees; patients were largely non-Hispanic Black (29.9%) and Hispanic (48.7%), and had high rates of dual eligibility (33.0%), Supplemental Security Income disability enrollment (49.1%), and multiple comorbidities. In the 12 months following HH enrollment, HH enrollees had one more month of Medicaid coverage (p<0.001) and 4.6 more outpatient visits than expected (p<0.001, evenly distributed between primary and specialty care). There were also positive impacts on the proportions of patients with follow-up visits within 7 days (4 percentage points (pp), p<0.001) and 30 days (6pp, p<0.001) after inpatient care, and on the proportion of patients with follow-up visits within 30 days after emergency department (ED) care (4pp, p<0.001). We did not find meaningful differences in continuity of care. We found small positive impacts on the proportion of patients with an inpatient visit and the proportion with an ED visit.ConclusionsNew York State’s HH program improved access to care for Medicaid recipients with diabetes. These findings have implications for New York State Medicaid as well as other providers and care management programs.

After eight years on staff, Katie Harris was recently promoted from director of ticket operations to a new position as associate athletic director at Montgomery University (MU). Several months into her new position, Katie is faced with a difficult challenge. Several thousand fans from conference rival Bucks State College (BSC) attended a men’s basketball game at the 15,000-seat MU Convocation Center. The large presence of BSC fans did nothing worthy of ejection, but was able to negatively impact the experience for many MU fans. MU’s high profile men’s basketball coach contacted the director of athletics requesting if something could be done to reduce the impact of visiting fans. Though the coach understands that dealing with opposing fans is part of sport, even on a team’s home court, the environment has become a distraction for coaches, players, and many significant athletic department donors who pay premium prices for season tickets. The coach received complaints from numerous supporters indicating that unless something is done they are considering cancelling their season tickets. Though complicated by logistics, financial, and legal consequences, Katie has been asked to research the issue and share recommendations for policy development.

Abstract Older adults account for 12-24% of all Emergency Department (ED) visits and studies have shown a 25-35% increase in older adult ED visits over time. While some studies have analyzed ED encounters over a single healthcare system, a year or a specific condition, research comparing the long-term frequent use of the ED across cohorts of older adults in the United States is limited. We address this gap by identifying characteristics associated with repeat (&gt;1 visit during the time period) and serial/frequent (&gt;=4/year) ED utilization for Maryland. Using the 2017-2019 State Emergency Department Databases (SEDD), we extracted ED visits among the elderly (&gt;=60 years) population. By applying the modified New York University Algorithm by John Hopkins University (NYU-JHU-EDA), we classified ED visits into 11 categories, and we conducted bivariate and multivariate analyses to identify inter-age variations in the type of visits to the ED across single, repeat and serial users, after controlling for covariates. Of the total 5,331,843 ED visits between 2017 and 2019, 1,244,878 (~23%) were older adult visits. Relative to the 60–69-year-olds, those 80 and above had higher odds of being repeat users for emergent issues [OR=1.34, CI:1.07-1.67], primary-care treatable condition [OR=1.29, CI:1.03-1.62], and severe [1.30, CI: 1.03-1.64] and non-severe injuries [1.61, CI:1.28-2.01]. Relative to 60-69 year olds non-serial users, the oldest cohort demonstrated lower odds for psychiatric/alcohol and drug-related diagnoses [OR=0.63, CI:0.42, 0.95]. Our findings highlight the need to design age-group specific interventions to reduce the frequent ED utilization among the elderly in Maryland.

`Hannah's Choice F1' is a new, high quality eastern type muskmelon (Cucumis melo L.) with multiple disease resistance. It was developed in the Department of Plant Breeding at the Cornell University Agricultural Experiment Station in Ithaca, N.Y. It is well adapted for northeastern U.S. conditions and shows potential for good adaptation in the northwest. With multiple disease resistance it is well suited for home gardeners, market gardeners, and commercial growers. `Hannah's Choice F1' has excellent resistance to powdery mildew races 1 and 2 (Podosphaera xanthi) and some tolerance to Fusarium root rot (Fusarium oxysporum f. sp. melonis) race 2. In addition, it has resistance to watermelon mosaic virus (WMV), papaya ringspot virus (PRSV), and zucchini yellow mosaic virus (ZYMV). This is the first commercial melon to have combined resistance to these three potyviruses. Also, it has shown some field tolerance to spider mites (Tetranychus urticae). Lastly, it has shown some field tolerance to downy mildew (Pseudoperonospora cubensis), angular leaf spot (Pseudomonas syringae pv. lachrymans), and gummy stem blight (Didymella bryoniae). In 2001, 2002, and 2003 it was grown in replicated trials in New York and in 2002 and 2003 in Oregon.

ImportancePostpartum emergency department (ED) visits may indicate poor access to care and risk for maternal morbidity.ObjectivesTo identify patient and hospital characteristics associated with postpartum ED visit rates.Design, Setting, and ParticipantsThis retrospective cohort study used data from the 2014 to 2016 New York State Inpatient Database and State Emergency Department Database. All obstetric discharges from acute care hospitals in New York State from January 1, 2014, through November 15, 2016, were included. Obstetric discharges in the inpatient database were linked to subsequent ED visits by the same patient in the ED database. Data were analyzed from February 2020 to August 2022.ExposuresPatient characteristics assessed included age, race, insurance, home zip code income quartile, Charlson Comorbidity Index score, and obstetric risk factors. Hospital characteristics assessed included safety net status, teaching status, and status as a hospital disproportionally serving racial and ethnic minority populations.Main Outcomes and MeasuresThe primary outcome was any ED visit within 42 days of obstetric discharge. Multilevel logistic regression with 2-level nested mixed effects was used to account for patient and hospital characteristics and hospital-level clustering.ResultsOf 608 559 obstetric discharges, 35 299 (5.8%) were associated with an ED visit within 42 days. The median (IQR) birth hospital postpartum ED visit rate was 6.3% (4.6%-8.7%). The mean (SD) age was 28.4 (9.1) years, 53 006 (8.7%) were Asian patients, 90 675 (14.9%) were Black patients, 101 812 (16.7%) were Hispanic patients, and 275 860 (45.3%) were White patients; 292 991 (48%) were insured by Medicaid, and 290 526 (47.7%) had private insurance. Asian patients had the lowest postpartum ED visit rates (2118 ED visits after 53 006 births by Asian patients [3.99%]), and Black patients had the highest postpartum ED visit rates (8306 ED visits after 90 675 births by Black patients [9.15%]). Odds of postpartum ED visits were greater for Black patients (odds ratio [OR], 1.31; 95% CI, 1.26-1.35; P &amp;lt; .001) and Hispanic patients (OR, 1.19; 95% CI, 1.15-1.24; P &amp;lt; .001) relative to White patients; those with Medicare (OR, 1.55; 95% CI, 1.39-1.72; P &amp;lt; .001), Medicaid (OR, 1.37; 95% CI, 1.34-1.41; P &amp;lt; .001), or self-pay insurance (OR, 1.50; 95% CI, 1.41-1.59; P &amp;lt; .001) relative to commercial insurance; births that occurred at safety net hospitals (OR, 1.43; 95% CI, 1.37-1.51; P &amp;lt; .001) and hospitals disproportionately serving racial and ethnic minority populations (OR, 1.14; 95% CI, 1.08-1.20; P &amp;lt; .001); and births that occurred at hospitals with fewer than 500 births per year (OR, 1.25; 95% CI, 1.14-1.39; P &amp;lt; .001) relative to those with more than 2000 annual births. Adjusted odds of postpartum ED visits were lower after birth at teaching hospitals (OR, 0.82; 95% CI, 0.74-0.91; P &amp;lt; .001) and metropolitan hospitals (OR, 0.74; 95% CI, 0.65-0.85; P &amp;lt; .001).Conclusions and RelevanceThis cohort study found that Black and Hispanic patients experienced higher adjusted odds of postpartum ED visits across all hospital types, particularly at safety net hospitals and those disproportionately serving racial and ethnic minority populations . These findings support the urgent need to mitigate structural racism underlying maternal health disparities.

The author is a Professor in the Department of Geography and Planning at the University at Albany, State University of New York, where he directs the Masters Program in Urban and Regional Planning. He is a member of the World Society for Ekistics (WSE), the American Institute of Certified Planners, the American Planning Association, the International Planning History Society, and many other professional and scholarly associations, and he has served as a consultant with the United Nations, UNICEF, USAID, and various projects funded by the World Bank and AID. His research and publications focus on: the history of ideas in planning and community development; metropolitan and regional development policies; the revitalization of old neighborhoods; disaster avoidance and relief; and, micro-enterprise development. The text that follows is a revised and extended version of a paper presented at the WSE Symposion "Defining Success of the City in the 21st Century," Berlin, 24-28 October, 2001.

Black bears (Ursus americanus) are an iconic and common species throughout much of the United States and people regularly interact with these large predators without conflict. However, negative interactions between people and bears can manifest in conflicts that can hinder conservation efforts. Black bears are highly attracted to anthropogenic sources of food, and negative interactions with people are primarily a product of trash mismanagement. In the Catskills region of New York State, home to a large population of black bears, over 400 such conflicts are reported each year. While the New York Department of Environmental Conservation (DEC) has seen progress recently in educating residents of the region on how to reduce unwanted interactions with bears, they have had less success educating the 12 million tourists that visit the Catskills each year. Understanding where conflict may occur in the future, and the environmental and anthropogenic factors that precede it, may help guide management strategies to reduce these unwanted interactions. Therefore, we designed resource selection probability functions (RSPFs) to examine the relationship between human-black bear conflicts in the Catskills with a suite of landscape and anthropogenic data, using conflicts reported to the DEC across the state of New York in 2018–2019. We found that human-black bear conflicts were more likely to occur in the residential areas of the Catskills on the urban-wildland interface; areas with relatively higher human population densities, away from dense forest, and further from heavily urbanized areas. While future work is needed to continuously validate our model predictions, our results will provide the DEC and other conservation managers in the Catskills the ability to create more targeted plans for mitigating unwanted human-black bear interactions, and provide a better understanding of the mechanisms driving human-carnivore interactions at an urban-wildland interface more generally.

The Corona Virus (Covid-19) pandemic is still ongoing from the beginning of March until now in Indonesia, seeing this the government immediately issued new policies such as carrying out activities at home for students from Early Childhood Education to Higher Education levels. The government does not recommend face-to-face learning (offline) and is replaced with online learning that can be done at home. This study aims to determine how much influence online learning has on the mental healthy of students majoring in accounting who are studying at the State University of Malang. The author conducted a study entitled "The Effect of Online Learning on Mental Healthy Students of the Accounting Department". This type of research uses a qualitative research type with a phenomenological approach. Using qualitative research in order to obtain accurate information or data in knowing the effect of online learning on students majoring in accounting at the State University of Malang and using data collection techniques through filling out questionnaires through google forms given to accounting students. The results of this study indicate that students during online learning experience difficulties in receiving the material presented and experience stress due to thinking about the workload given so that it is difficult to sleep. Therefore, it is necessary to have self-motivation to be more active in participating in learning even though online, besides being able to manage time better so that the tasks given are quickly completed and no longer a burden.

“The value of the Art Education becomes more and more apparent as a means of honorable support and of high culture and enjoyment,” stated the catalog of Ingham University in western New York State in 1863. The Art Department there would prepare “pupils for Teachers and Practical Artists.” This statement reveals some of the vocational options for women that were concomitant with the increased popularity of music and art education in the middle decades of the nineteenth century in the United States. Practical vocational concerns, along with notions of refinement and respectable entertainment, all were aspects of the impetus for music and art education. Preparing young women for occupations, whether as teachers of art and music or as commercial artists or musicians, was a particularly prominent component of education for women in the mid-nineteenth-century United States.

The challenges of society have made adjustments to the medical education system of Ukraine. In the conditions of the pandemic of the coronavirus infection COVID-19 and the introduced martial law throughout the territory of Ukraine, distance learning (DL) turned out to be the only possible form of the educational process. The purpose of the study was to study the peculiarities of teaching the discipline "Propaedeutics of Pediatrics" in the conditions of the National Academy of Sciences. The process of DL at the Department of Propaedeutics of Childhood Diseases of the Dnipro State Medical University (DSMU) took place using the MOODLE electronic portal, the Google Meet video service, the YouTube platform, the website and the electronic magazine of the Dnipro State Medical University. An anonymous survey of 210 third-year students was conducted regarding their attitude and degree of satisfaction with the educational process at the Department of Propaedeutics of Childhood Diseases using educational distance technologies. As the results showed, the majority of the interviewed students are completely satisfied with the process of DL. However, when choosing a form of education, preference is still given to the classroom format of education. Among the advantages of DL, students noted the saving of time and financial costs for moving between buildings and clinical bases, the comfort of studying in a home environment, the possibility of studying at any convenient time, the availability of information resources, and the possibility of re-processing complex material. However, the biggest drawback of the DL was the lack of opportunity to work with the patient and master practical skills. So, DL, like any other method of teaching and learning, has its advantages and disadvantages. But currently existing technologies of medical education with the help of modern digital platforms, high-quality content filling of educational web resources, introduction of an effective system of quality control of acquired knowledge make it possible to adapt to the new realities of the educational process and ensure consistently high quality of medical education

<p>This study examined the factors that affect the integration of Outcomes-Based Education (OBE) in the Home Economics (HE) education curriculum of the Technology and Livelihood Education (TLE) program of a State University in the northern part of the Philippines. Descriptive survey and qualitative design were deployed to gather, analyze, and interpret data, which were deemed significant to the study. These research methods involved questionnaires, interviews, observations, and document investigations. The unit of analysis, which was purposively selected, was the students and faculty of the HE department under the TLE program. Results disclosed that there is little understanding of the new paradigm shift—OBE; there are limited instructional resources for the faculty; laboratory equipment were inadequate; and physical facilities available do not maximize the competencies required by OBE. Therefore, the researchers proposed the formulation of a new set of educational objectives, and the design of an action plan for the succeeding five years, in order to solicit necessary budget allocation from the government funding agency. This effort will address the identified gaps in the delivery of the OBE curriculum, specifically in upgrading physical resources and instructional materials. In addition, personnel involved in curriculum implementation should undergo seminars and trainings so that they could translate theories and concepts into practical skills and lifelong learning, which are needed for global marketability in the 21<sup>st</sup> century.<strong> </strong></p>

The heavily-subsidized state theatre system of West Germany is often regarded as a model for emulation by the funding agencies of the English-speaking theatre. Yet the situation of such theatres can give rise to its own problems: and in this case-study of the rehearsals of Fassbinder's Katzelmacher at the Bayerisches Staatsschauspiel, Munich, in 1985. Lynn Sobieski (who was assistant dramaturg on the production) analyzes the resulting personal and artistic conflicts, in the context of a system which encourages a degree of complacency in the bureaucracy, and arguably permits some self-indulgence to the directors – while discouraging those of the first ‘post-war generation’ from giving real opportunities to their successors. Lynn Sobieski is presently teaching in the Department of Drama of the University of Texas at Austin, having recently been awarded her doctorate from New York University for her dissertation on ‘The Crisis in West German Dramaturgy’. Her collection, Postmodernism and Contemporary Performance, will be published later this year, and she is currently working on a study of performance art groups in Britain.

Importance: Social media is widely used by various segments of society. Its role as a tool of communication by the Public Health Departments in the U.S. remains unknown. Objective: To determine the impact of the COVID-19 pandemic on social media following of the Public Health Departments of the 50 States of the U.S. Design, Setting, and Participants: Data were collected by visiting the Public Health Department web page for each social media platform. State-level demographics were collected from the U.S. Census Bureau. The Center for Disease Control and Prevention was utilized to collect information regarding the Governance of each State’s Public Health Department. Health rankings were collected from “America’s Health Rankings” 2019 Annual report from the United Health Foundation. The U.S. News and World Report Education Rankings were utilized to provide information regarding the public education of each State. Exposure: Data were pulled on 3 separate dates: first on March 5th (baseline and pre-national emergency declaration (NED) for COVID-19), March 18th (week following NED), and March 25th (2 weeks after NED). In addition, a variable identifying the total change across platforms was also created. All data were collected at the State level. Main Outcome: Overall, the social media following of the state Public Health Departments was very low. There was a significant increase in the public interest in following the Public Health Departments during the early phase of the COVID-19 pandemic. Results: With the declaration of National Emergency, there was a 150% increase in overall public following of the State Public Health Departments in the U.S. The increase was most noted in the Midwest and South regions of the U.S. The overall following in the pandemic “hotspots,” such as New York, California, and Florida, was significantly lower. Interesting correlations were noted between various demographic variables, health, and education ranking of the States and the social media following of their Health Departments. Conclusion and Relevance: Social media following of Public Health Departments across all States of the U.S. was very low. Though, the social media following significantly increased during the early course of the COVID-19 pandemic, but it still remains low. Significant opportunity exists for Public Health Departments to improve social media use to engage the public better.

This study was conducted on an assessment of the various mitigation strategies to combat desertification in Jibia and Kaita Local Government Areas of Katsina State, Nigeria. The data use includes Satellite imageries for the study such as landsat MSS of 1976, landsat TM of 1987, SPOT XS of 1995 and landsat ETM of 2006 as well as structured questionnaires. Sixty close ended copies of the Questionnaire were administered in the study. Purposive sampling method of administering questionnaires was adopted. The percentages land mass covered for each of these variables was determined and estimated in M2. literature was obtained from various agencies which were responsible for desertification control in Katsina state. It was found from the reserved forest that in 1976 the percentage of reserved forest was 2.57%. In 1987 however, it increased by 73.9% to 76.47 %. By 1995, it declined by 9.42% to 67.05% and further declined by 0.52% in 2006. Effort to combat desertification through the use of reserved forest has been quite significant over the years. Also, noticed was a declined in shelter belt from 5.91% in 1987 to 1.097% in 1995 and a shot up to 7.39% in 2006. About 37% of the respondent opined that the deforestation leads to the disappearance of trees while 33% pinioned that it leads to reduction on agricultural productivity. The major strategy adopted to combat desertification is tree planting as supported by 88% of the respondents. It found that desertification as major environmental problem of the study area has reduced drastically from 43.34% in 1976 to 1.29% in 2006. It was also revealed from this study that some organizations such as European Economic Community/Katsina State government EEC/KTSG, Katsina Afforestation Project Unit KTAPU and Local Government Councils are the major agencies that are responsible for mitigating desertification in the study area. Advances in functional neurosurgery, including neuromodulation and more recently ultrasonic ablation of basal ganglia structures, have improved the quality of life for patients with debilitating movement disorders. What is little known, however, is that both of these neurosurgical advances, which remain on the cutting edge, have their origin in the pioneering work of Russell Meyers, whose contributions are documented in this paper. Meyers' published work and professional correspondence are reviewed, in addition to documents held by the Department of Neurosurgery at the University of Iowa. Meyers was born in Brooklyn, New York, and received his neurosurgical training at hospitals in New York City under Jefferson Browder. In 1939, a chance encounter with a young woman with damaged bilateral ventral striata convinced Meyers that the caudate could be resected to treat Parkinsonism without disrupting consciousness. Shortly thereafter, he performed the first caudate resection for postencephalitic Parkinsonism. In 1946, Meyers became the first chairman of neurosurgery at the State University of Iowa (now the University of Iowa), which led to the recruitment of 8 faculty members and the training of 18 residents during his tenure (1946–1963). Through collaboration with the Fry brothers at the University of Illinois, Meyers performed the first stereotactic ultrasonic ablations of deep brain structures to treat tremor, choreoathetosis, dystonia, intractable pain, and hypothalamic hamartoma. Meyers left academic neurosurgery in 1963 for reasons that are unclear, but he continued clinical neurosurgery work for several more years. Despite his early departure from academic medicine, Meyers' contributions to functional neurosurgery provided a lasting legacy that has improved the lives of many patients with movement disorders.

Purpose: With the advances in early detection, prevention, and treatment of some cancers, mortality rates in the United States have been consistently falling. However, with these successes have come substantial increases in the cost of cancer care. Antineoplastics are the leading class in hospital drug expenditures. Cancer treatments are causing a serious financial burden for patients, families, hospitals, and society at large, especially for those who are uninsured. Prescription assistance programs (PAPs) are offered by pharmaceutical manufacturers to provide medications at no out-of-pocket cost to medically indigent patients. To assist the Cancer Care Center at Nassau University Medical Center (NUMC) with drug costs for chemotherapies, a clinical pharmacist–managed PAP was implemented to procure chemotherapy medications from pharmaceutical manufacturers. NUMC is a safety-net teaching hospital in suburban New York. It serves mostly an indigent population, and its Level I trauma center has more than 77,000 emergency department visits per year. Methods: We observed all patients requiring assistance with chemotherapy who enrolled in PAPs from January 1, 2011, through December 31, 2012. The total number of visits in the clinic over 24 months was 9,405. Individuals potentially eligible for the PAP were identified by oncologists. Patients received a referral to the PAP when they mentioned to office personnel that they had difficulty obtaining medications or expressed financial concerns related to management. Results: The clinical pharmacist spent the majority of his time assisting uninsured individuals, working with charity pools from the drug manufacturers, and obtaining medications through the PAP. The program served 341 patients in 2011 and 579 patients in 2012. Medications obtained for these patients included both oral and parenteral chemotherapy drugs and antiemetics. The total cost savings of the medications was $908,944.11 in 2011 and $1,715,538.37 in 2012. In 2012, a total of 783 prescriptions were processed, and 775 were approved by the manufacturers (98.9%). Conclusion: PAPs provide a valuable safety net to ensure that uninsured patients with cancer receive needed prescription medications. The rising cost of health care and the high proportion of indigent patients have financially burdened the hospital. For the pharmacy department, this has resulted in a lack of compensation for pharmaceuticals dispensed to indigent patients. A pharmacy-based program to procure free medications for uninsured patients with cancer has helped to defray the expenses of the Cancer Care Center in providing care at NUMC, increased patient compliance with chemotherapy protocols, and allowed many patients to receive the treatments they otherwise would not have been able to afford.
