Abstract (sommario):
OBJECTIVES/GOALS: The Rockefeller University CCTS, Clinical Directors Network (CDN), and Carter Burden Network (CBN) received a DHHS-Administration for Community Living Nutrition Innovation grant to test whether implementation of DASH-concordant meals and a program to enhance self-efficacy, could lower blood pressure among seniors aging in place. METHODS/STUDY POPULATION: CEnR-Nav model to engage stakeholders, enroll seniors age >60 yr., eating 4 meals a week at 2 CBN congregate meal sites; Advisory Committee to facilitate dissemination; menus aligned with Dietary Approaches to Stop Hypertension (DASH) and New York City Department for the Aging (DFTA) nutritional guidelines; interactive sessions for education (nutrition, blood pressure, medication adherence); Omron 10 home BP devices for daily home monitoring. Plate Waste and Meal Satisfaction (Likert scale) to assess taste preference and cost impact. Outcomes: Primary: Change in Systolic BP at Month 1; change in percent with controlled blood pressure. Secondary: change in validated measures of cognitive (e.g. SF-12, PHQ-2), behavioral (Home BP monitoring), nutritional (food frequency) variables, satisfaction, costs. RESULTS/ANTICIPATED RESULTS: Menu alignment required multiple iterations. Plate Waste and Menu Satisfaction tools were developed. Site 1 enrollment began June 2019; educational sessions and home BP monitors and training were provided. Baseline mean blood pressure (Site 1) was 138/79 +20.5; (range: 7% hypertensive crisis, 36% stage 2 hypertension, 22% stage 1 hypertension, 22% elevated, and 13% normal). DASH-aligned meals began October 2019; Meal satisfaction declined briefly, chefs adjusted menus, and meal satisfaction rose to pre-intervention levels. Site 2 enrollment is ongoing; dietary intervention will start in 2020. Primary outcome data (change in BP) will be complete in March 2020. Secondary outcome data on social and behavioral impact of the interventions will also be presented. DISCUSSION/SIGNIFICANCE OF IMPACT: We leveraged our community-academic research partnership to conduct research addressing uncontrolled hypertension, an urgent unmet health need among seniors. The DASH Implementation Study can inform the broader aging services and healthcare community of the potential for congregate nutrition programs to improve cardiovascular health outcomes.

Abstract (sommario):
In 1964, the New York State Public Health Law, §2500a, set the stage for mandatory newborn screening. By 1978, testing was already under way when the last of the total of eight diseases was added. Screening for homozygous sickle cell disease was included in 1975. Specimens submitted from 1975 to 1984 totaled nearly 2.5 million, and these specimens were sent to four different laboratories in New York state. New York City, with the largest population of newborns, sent the largest proportion of specimens (1.1 million). Although testing was occurring, it was not until 1979 that citywide follow-up was established. Follow-up efforts were coordinated through the New York City Department of Health in cooperation with 52 hospitals. Of these hospitals, 25 received New York state funding for the provision of genetic services, which included testing and counseling for parents of children whose test results were positive for trait or disease. A cooperative effort was set up between the New York City Department of Health and health providers, who agreed to provide follow-up for newborns with all hemoglobinopathies at their respective hospitals. In cases of sickle cell disease, efforts were made to obtain second specimens from affected infants. In cases in which this was difficult, New York City Department of Health aids visited the home and obtained the specimen, counseled the parents, and arranged for further follow-up. Other cooperative efforts occurred when various genetic or sickle cell centers came together and organized committees. Two major committees, the Sickle Cell Advisory Committee, and its executive arm, the Implementation Committee, in which all providers of genetic services throughout the state are represented, met to discuss statewide problems, to identify needs, and to formulate policy.

Abstract (sommario):
One of the prestigious American universities, New York University, NYU, along with the entire city and state, found itself locked out and inaccessible for students, faculty and library staff since mid-March 2020. Restrictions imposed by the pandemic created an unanticipated stress on all systems of the large academic institution in the epicenter of the national outbreak. The article describes in brief the unique nature of the university and its library system, its global reach and associated established library practices. The focus of the article are the activities of KARMS, the cataloging department of the NYU Divisions of Libraries. It describes the way the cataloging department recalibrated its services to allay immediate crisis of the constituent communities in the middle of the final semester of an academic year. The article also details a number of long-term initiatives that received a much needed attention under the conditions of the so-called “distance working”. While the work of any cataloging department is intrinsically linked to physical objects, the article offers several examples of the projects that can be completed remotely

Abstract (sommario):
Abstract Paid caregivers (e.g. home health aides, personal care attendants) are formally tasked with helping older adults with functional impairment meet their basic needs at home. This study used semi-structured interviews (n=30) with dyads of patients or their proxies and their paid caregivers in New York City to 1) understand the range of health-related tasks paid caregivers perform in the home and 2) determine if these tasks are taught in the New York State government’s Department of Health curricula. We found that patients, proxies, and paid caregivers all described that paid caregivers performed a wide range of health-related tasks that were often not a part of their formal training. Creating clear competencies for paid caregivers that reflect the full breadth of health-related tasks they may perform at home will help maximize the potentially positive impact of the paid caregiver workforce on the lives of patients living at home with functional impairment.

Abstract (sommario):
The Athletic Department at Sizeable State University has struggled both on and off the field in recent years. With a majority of their teams finishing in the bottom half of the conference 3 years in a row, student attendance at home athletic events is at an all-time low. Despite these challenges, the women’s basketball team has found success in recent years even finishing first in the conference last season and making a trip to the NCAA tournament. With an incoming student class made up of majority females, and a growing mound of research on female sports fandom, the athletic director has challenged the marketing team to specifically target female college students with their marketing initiatives in the upcoming season. Driven by a largely female leadership team at Sizable State University, Grace Perkins, the associate athletic director for marketing, has been tasked with developing these new marketing strategies which will drive female student attendance at home athletic events, with particular focus on home women’s basketball games.

Abstract (sommario):
AbstractIntroductionOn October 29, 2012, Hurricane Sandy touched down in New York City (NYC; New York USA) causing massive destruction, paralyzing the city, and destroying lives. Research has shown that considerable damage and loss of life can be averted in at-risk areas from advanced preparation in communication procedures, evacuation planning, and resource allocation. However, research is limited in describing how natural disasters of this magnitude affect emergency departments (EDs).Hypothesis/ProblemThe aim of this study was to identify and describe trends in patient volume and demographics, and types of conditions treated, as a result of Hurricane Sandy at Staten Island University Hospital North (SIUH-N; Staten Island, New York USA) site ED.MethodsA retrospective chart review of patients presenting to SIUH-N in the days surrounding the storm, October 26, 2012 through November 2, 2012, was completed. Data were compared to the same week of the year prior, October 28, 2011 through November 4, 2011. Daily census, patient age, gender, admission rates, mode of arrival, and diagnoses in the days surrounding the storm were observed.ResultsA significant decline in patient volume was found in all age ranges on the day of landfall (Day 0) with a census of 114; -55% compared to 2011. The daily volume exhibited a precipitous drop on the days preceding the storm followed by a return to usual volumes shortly after.A notably larger percentage of patients were seen for medication refills in 2012; 5.8% versus 0.4% (P<.05). Lacerations and cold exposure also were increased substantially in 2012 at 7.6% versus 2.8% (P<.05) and 3.8% versus 0.0% (P<.05) of patient visits, respectively.A large decline in admissions was observed in the days prior to the storm, with a nadir on Day +1 at five percent (-22%). Review of admitted patients revealed atypical admissions for home care service such as need for supplemental oxygen or ventilator. In addition, a drop in Emergency Medical Services (EMS) utilization was seen on Days 0 and +1. The SIUH-N typically sees 18% of patients arriving via EMS. On Day +1, only two percent of patients arrived by ambulance.ConclusionThe daily ED census saw a significant decline in the days preceding the storm. In addition, the type of conditions treated varied from baseline, and a considerable drop in hospital admissions was seen. Data such as these presented here can help make predictions for future scenarios.GreensteinJ, ChackoJ, ArdolicB, BerwaldN. Maxey, Hannah L. "Understanding the Influence of State Policy Environment on Dental Service Availability, Access, and Oral Health in America's Underserved Communities". Thesis, 2014. http://hdl.handle.net/1805/5993.

Testo completo
Abstract (sommario):
Indiana University-Purdue University Indianapolis (IUPUI)
Oral health is crucial to overall health and a focus of the U.S. Health Center program, which provides preventive dental services in medically underserved communities. Dental hygiene is an oral health profession whose practice is focused on dental disease prevention and oral health promotion. Variations in the practice and regulation of dental hygiene has been demonstrated to influence access to dental care at a state level; restrictive policies are associated lower rates of access to care. Understanding whether and to what extent policy variations affect availability and access to dental care and the oral health of medically underserved communities served by grantees of the U.S. Health Center program is the focus of this study. This longitudinal study examines dental service utilization at 1,135 health center grantees that received community health center funding from 2004 to 2011. The Dental Hygiene Professional Practice Index (DHPPI) was used as an indicator of the state policy environment. The influence of grantee and state level characteristics are also considered. Mixed effects models were used to account for correlations introduced by the multiple hierarchical structure of the data. Key findings of this study demonstrate that state policy environment is a predictor of the availability and access to dental care and the oral health status of medically underserved communities that received care at a grantee of the U.S. Health Center program. Grantees located in states with highly restrictive policy environments were 73% less likely to deliver dental services and, those that do, provided care to 7% fewer patients than those grantees located in states with the most supportive policy environments. Population’s served by grantees from the most restrictive states received less preventive care and had greater restorative and emergency dental care needs. State policy environment is a predictor of availability and access to dental care and the oral health status of medically underserved communities. This study has important implications for policy at the federal, state, and local levels. Findings demonstrate the need for policy and advocacy efforts at all levels, especially within states with restrictive policy environments.

Abstract (sommario):
Abstract The “official Story” is that Harvard established the Russian Research Center with the help of a preliminary grant of $100,000 from the Carnegie Corporation of New York to provide information to the State Department and other government agencies about contemporary Russia. An article in the Boston Traveler of December 5, 1947, the basis for an FBI memo of December 12, reported that the results of the Harvard research would be “made available to the State Department and other federal agencies, including the central intelligence group, through in formed channels long before publication by the university,” though the university declined to do any classified (secret) research. The State Department, though convinced of the importance of covert activities, was reluctant to engage in them itself. So were many professors, who were also eager to establish a permanent university-government relationship.

Abstract (sommario):
This essay is a personal, and slightly informal, account and study of Amiri Baraka’s work as a teacher and community member. Like many of the best-known Beat writers, Baraka had not come through the maze of university creative writing programs and, again like several others, had no advanced degree of any kind when he began teaching at the university level. Baraka’s residency as the Jenny McKean Moore Professor in the Department of English at The George Washington University is examined in detail to shed new light on Baraka as both poet and teacher. Following his time in D.C., Baraka would go on to a lengthy period on the faculty of the State University of New York at Stony Brook, in the Africana Studies Department, and another period at Rutgers, which eventuated in Baraka’s lawsuit against the university. This essay will also address briefly some of the controversy that was attendant upon Baraka’s appointment to the GWU position and will discuss how Baraka’s other academic appointments might help readers understand more deeply his work as a teacher, writer, and organizer.

In a 2010 article for The New York Times, literary theorist Stanley Fish comments on recent manifestations of the ‘crisis in the humanities’ in the form of language department closures at US universities such as at the State University of New York at Albany. Defending a ‘liberal arts’ education amid the neoliberalist reality of higher-education funding and knowledge production, Fish is nevertheless highly critical of a fellow respondent’s claims concerning the value of the humanities. This respondent asks in his letter, ‘What happened to public investment in the humanities and the belief that the humanities enhanced our culture, our society, our humanity?’ Fish advises caution concerning this line of defence, arguing:...

The take-off of psychopharmacology in the mental-hospital world began in the vast asylum system of New York State in the early 1950s. Henry Brill ordered the state system to introduce chlorpromazine in 1955, which led to the first decrease in the census of the state asylum system in peacetime. Sidney Merlis and Herman Denber implemented chlorpromazine in their hospitals and, with Brill, began a series of publications on the drugs and their efficacy. Pharmacologist and psychiatrist Joel Elkes established the first department of experimental psychiatry in the world in 1951 at the University of Birmingham in England. Finally, the chapter examiunes the historical heft of the National Institute of Mental Health, which in 1953 opened the “intramural” (in-house) research program where much of the research in psychopharmacology done in the United States has occurred.

Abstract (sommario):
This chapter explores how crime and violence grew during the pre–Civil War period. Gangs such as the Bowery Boys, the Chichesters, the Kerryonians, the Forty Thieves, the Atlantic Guards, and the Plug Uglies seemed to be omnipresent in New York. Membership in such groups offered young men status, alcohol, and a chance to rebel against a system seemingly stacked against them. Violence also grew as it became politicized as ethnic, cultural, and social struggles rippled through the city, reflecting the tragic side of New York's extraordinary growth. Many gang members felt a sense of entitlement, and sought to demonstrate their rights with both their ballots and their fists, as well as ways to gain political standing and wealth through saloons, the police department and firehouses. This all led to a spike in violent encounters at polling places and in public gatherings such as in the city's theaters. This state of society is reflected in the bitter rivalry between two actors, the English William Macready and the American Edwin Forrest. At the same time, immigrants flowed into New York, looking for a new home. Many, such as the Irish, encountered widespread discrimination and competition for jobs. Even so, they made a home for themselves in the city.

Abstract (sommario):
Abstract Irving Greenberg was born in 1933 in Brooklyn, New York. His father was the rabbi of the Chevra Shas (a talmudic study group) in Boro Park. As a child, he received an Orthodox day school education that was enriched by active involvement in the Zionist youth movement Hashomer Hadati (known today as Bnai Akiva). He continued his Jewish education at the Beth Joseph Rabbinical Seminary and his secular education at Brooklyn College. In 1953, he received his B.A. in history from Brooklyn College and continued on to a doctoral program in intellectual history at Harvard. He received his Ph.D. from Harvard in 1960. Eschewing an academic career, he became a communal rabbi in Boston where he came under the influence of Rabbi Joseph B. Soloveitchik. This encounter caused a change of career plans, and Greenberg began to teach Jewish history at Yeshiva University in New York City. In 1961-1962, he was a Fulbright scholar at Tel Aviv University and while in Israel turned to deep reading about both the Holocaust and the State of lsrael. Upon his return to the United States, he became involved in Holocaust education which, in turn, led him to leave Yeshiva University in 1965 in order to accept a rabbinical position in Riverdale, New York. After seven years in this post, he once again entered academic life, accepting the chairmanship of the Department of Jewish Studies at the City University of New York. In 1979 he made another turn, leaving the university to become the director of CLAL, the National Jewish Center for Learning and Leadership, an organization that aimed to bring intense, advanced, Jewish education to the wider Jewish community. During the 1970s, Greenberg also teamed with Elie Wiesel and others to create the U.S. Holocaust Memorial Museum, and from 2000 to 2002 he served as chairman of the museum’s council.

Abstract (sommario):
From its founding in 1853, The Children’s Aid Society (CAS) identified parent leadership and involvement as a win-win strategy and incorporated it as one of its core components. So naturally, when CAS entered into a partnership with the New York City Board of Education in 1990, parents were invited to be central players from the planning phase onward. When CAS’s first community school, Salomé Ureña de Henriquez Middle Academies (Intermediate School [IS] 218), opened in 1992, a red carpet was extended for parents by other parents and the staff; 11 years later it was still extended, not only at this school but also at nine others in Manhattan and the Bronx, as well as at many adaptation sites around the country and abroad. In its work in community schools, CAS sees parents as assets and key allies, not as burdens; we aim not only to increase the number of parents involved in their children’s education but also to deepen the intensity of their involvement and to encourage greater participation in their children’s future. As we engage parents in skills workshops and advocacy events, we also create a critical link to the home, allowing us to serve and empower whole families and to foster effective leadership in their homes as well as their schools. Most of the CAS community schools are located in low-income neighborhoods that have many recent immigrants; the challenges of meeting the numerous needs inherent in immigrant communities are added to the challenges of involving parents. However, after more than a decade, a number of evaluations and reports show that each year these schools see greater numbers of parents participating in events ranging from parenting training and advocacy events to holiday dinners.1 This level of involvement represents a significant change in school culture; these parents are playing a greater role in their children’s education and in the school as a whole. By 2003 leaders from both the New York City Department of Education and the New York State Department of Education, among others, had recognized the parent involvement strategy at CAS’s community schools as a model to be emulated.

Abstract (sommario):
Born into a free black family in the early nineteenth century, Josephine Silone Yates was a pioneering woman faculty member at the historically black Lincoln Institute (now University) in Jefferson City, Missouri, where she headed the Department of Natural Sciences. Yates later rose to prominence in the black women’s club movement of the late nineteenth and early twentieth centuries, serving as president of the famed National Association of Colored Women (NACW) from 1901 to 1905. Josephine was born in 1852 in Mattituck, New York, to Alexander and Parthenia Reeve Silone. She was their second daughter. Her maternal grandfather, Lymas Reeves, had been a slave in Suffolk County, Long Island, New York, but was freed in 1813. Lymas owned a house in Mattituck, and Josephine’s parents lived with him. 1 Josephine’s mother was well educated for the time, and she taught her daughter to read and write at home. Josephine’s earliest and fondest memories were of being taught to read from the Bible while snuggled on her mother’s lap. Her mother made her call out the words as she pointed to them. Josephine began school at age six, where her teachers immediately recognized her preparedness and advanced her rapidly through the elementary grades. At the age of nine, she reportedly studied physiology and physics and possessed advanced mathematical ability. Silone also advanced her writing career at the age of nine, by submitting “a story for publication to a New York weekly magazine. Though the article was rejected for publication, she received a letter of encouragement, which increased her ambition to succeed.” Josephine’s uncle, Reverend John Bunyan Reeve, was the pastor of the Lombard Street Central Church in Philadelphia. Because of his interest in the education of his niece, he convinced his sister, Parthenia, to send Josephine at the age of eleven to live with him in Philadelphia so that she could attend the Institute for Colored Youth directed by Fanny Jackson-Coppin. It was probably felt that Josephine’s education would progress better under the mentorship of Jackson-Coppin.

Abstract (sommario):
Abstract Medical, or iatrogenic, injury is by no means an insignificant phenomenon. A 1964 study reported that 20% of patients admitted to the medical wards of a teaching hospital over an 8-month period experienced one or more untoward episodes.1 A 1981 report in the New England Journal of Medicine reported a 36% rate of iatrogenic injury (defined as “any illness that resulted from a diagnostic procedure or from any form of therapy” as well as “harmful occurrences ... that were not natural consequences of the patient’s diseases”) among 815 consecutive patients on a general medical service of a university hospital; of these, 9% were reported to have suffered “major complications.”.A Department of Health, Education and Welfare (DHEW) study based on records from two hospitals placed the incidence of iatrogenic injury at 7.5%.3 Two large-scale independent studies of hospital records-one undertaken in California in the mid-1970s4 and the recent Harvard Medical Practice Study of New York hospitals5-yielded similar findings, indicating that approximately 4% of all hospitalizations result in adverse events from medical treatment and one-quarter of these incidents involve substandard care.6 In other words, negligent adverse events were found to occur in 1% of all discharges.7 As a result, the Harvard study concluded that in New York State in 1984 nearly 99,000 patients suffered disabling injuries, of which more than 13,000 resulted in death.8 Extrapolated to the U.S. population as a whole, this suggests that of 40 million patients hospitalized every year, 1.5 million suffer some kind of disabling injury, that 180,000 of these patients die as a result of medical treatment, and that over half of these deaths are due to negligence.9 The number of deaths from medical injury thus appears to exceed mortalities associated with both motor vehicle accidents (about 40,000 per year) and workplace accidents (6,000 per year)..
