Abstract (sommario):

Comment des personnes avec un cancer et soignées par chimiothérapie choisissent-elles de satisfaire leur faim ? Étant donné les difficultés alimentaires rencontrées par de nombreux patients, comment leurs proches et les centres de soins en oncologie les prennent-ils en charge ? Quel est l’impact de cette gestion sur les choix alimentaires des soignés ? Pour répondre à ce questionnement, l’article proposé présente les résultats préliminaires d’une recherche anthropologique menée au Québec en 2012 et 2013 auprès de 21 femmes traitées pour un cancer gynécologique (de l’ovaire, du péritoine ou de l’endomètre) et de 10 proches sur l’expérience quotidienne d’une chimiothérapie. Il a pour objectif de prendre en compte la dimension biologique et corporelle, mais aussi le contexte social et culturel des troubles alimentaires pendant le traitement d’un cancer pour mieux en saisir l’ampleur. D’une part, sont identifiés les ressentis induits par la chimiothérapie (perte d’appétit, troubles sensoriels, fringales notamment), et leur influence sur la sensation de faim et la consommation alimentaire. D’autre part, sont analysés l’influence du personnel médical (infirmier[ère]s, nutritionnistes, pharmacien[ne]s) et le rôle de l’entourage dans les choix alimentaires des patientes et leur vécu affectif. Ils donnent à comprendre les points d’achoppement parfois problématiques dans l’observance diététique. En outre, ils permettent de saisir ce qui se joue entre les patientes et leurs proches autour de la décision alimentaire, variant selon les situations entre le choix délégué, la concertation, les cadeaux et le choix contrôlé. En définitive, entre nécessité, plaisir voire gourmandise, l’intérêt est de montrer ce que manger veut dire pendant une chimiothérapie anticancéreuse.

Abstract (sommario):

Les médicaments sont souvent écrasés pour pouvoir être pris par les personnes âgées qui ont des troubles de la déglutition ou des troubles du comportement. La Haute Autorité de Santé a publié la liste des médicaments autorisés à être écrasés. Ce travail a deux objectifs : Le 1er objectif est de déterminer l’impact des médicaments écrasés sur le goût de volontaires sains, afin de déterminer quels médicaments sont acceptables ou à déconseiller dans les aliments. Le 2ème objectif est de rechercher in vitro si certains médicaments inhibent ou au contraire stimulent la croissance microbienne et risquent d’altérer le biofilm oral. 1/ Le Test Hédonique est une étude clinique médicamenteuse de phase 1 sur volontaires sains. Seize volontaires sains ont testé les 10 médicaments les plus prescrits dans les EHPAD et identifiés comme pouvant être écrasés ou ouverts. Nous avons montré que les médicaments écrasés modifient le goût des aliments. 2/ L’analyse microbiologique : Nous avons testé in vitro si les 30 médicaments les plus prescrits en EPADH. Nous avons testé d’abord leur impact sur les souches de référence (normes AFNOR) et ensuite sur des souches orales (Escherichia coli, Staphylococcus aureus, Pseudomonas aeruginosa, Candida albicans). Seulement huit des trente médicaments testés inhibent la croissance des souches microbiennes. Ces huit médicaments ont été secondairement testés au contact de biofilms monoespèces (C. albicans et S. salivarius) en formation ou préformés. Cette étude nous a montré que les médicaments écrasés ont un impact direct sur les 5 souches bactériennes et sur C. albicans. Ces médicaments ont aussi eu un impact sur l’intégrité des biofilms
Frail elderly people are often polymedicated and they also frequently suffer from swallowing disorders (dysphagia). As a precaution, these patients are often given blended food and nursing staff is also obliged to crush their medication. The present work aimed to investigate the taste of the ten drugs and the antimicrobial properties of the 30 most prescribed drugs in nursing homes. 1/ Gustative analysis: This one-step observational study was designed like a food or wine tasting. A jury of 16 healthy volunteers was recruited among medical staff. They had to test the 10 most prescribed crushed drugs in nursing homes. We found that the taste of some drugs may be unbearable when they are crushed and mixed into food, and caregivers should avoid mixing a bad-tasting drug with the other drugs. 2/ Microbiology Study: Microbial growth inhibition of Escherichia coli, Staphylococcus aureus, Pseudomonas aeruginosa, Streptococcus salivarius, Gemella haemolysans and Candida albicans was screened by the diffusion method on agar plates. Eight drugs inhibited microbial growth. They were secondly tested on C. albicans and S. salivarius biofilms grown in liquid medium. We found that crushed medications could inhibit microbial growth, biofilm formation and diminish biofilm viability

Contexte : Le goût métallique est fréquemment rapporté par les patients au cours de leur traitement d’un cancer des voies aérodigestives supérieures. Très peu d’éléments sont connus sur ce symptôme : quelle est sa fréquence ? Ses cause(s) ?Nous avons mené une étude prospective observationnelle afin d’en déterminer l’incidence et la ou les causes, potentiellement possibles d’après la littérature. Deux hypothèses étaient possibles : une lipopéroxydation au niveau de la muqueuse orale et la levée d’inhibition du nerf facial sur le nerf glossopharyngien.Méthodes : 44 patients ont été suivis avant pendant et jusqu’à un an de la fin de leur traitement du cancer des voies aérodigestives supérieures. Des évaluations par questionnaires, prélèvements salivaires et tests supraliminaires d’intensité et de reconnaissance gustatifs ont été réalisés régulièrement, jusqu’à huit fois, durant un an.Un antidote potentiel, la lactoferrine en bain de bouche, a également été proposé en cas de goût métallique. Résultats : Le « goût » métallique a touché 27.2% des patients avec une intensité modérée à sévère chez 42.9% des personnes en souffrant. Il apparaissait surtout au milieu et à la fin de la radiothérapie ou de la radiochimiothérapie, avec un lien significatif avec la présence ou non d’une mucite orale. Ce goût métallique avait un impact significatif sur la qualité de vie liée au goût mais pas sur l’échelle visuelle analogique des ingestas ni sur le poids. Les hypothèses étiologiques de la lipopéroxidation et de la levée d’inhibition du nerf facial sur le nerf glossopharyngien n’étaient pas vérifiées. Enfin, le goût métallique pouvait être atténué par le bain de bouche de lactoferrine dans 63% des cas (de manière complète dans 16% et de manière partielle dans 47%).Conclusion : le goût métallique touche fréquemment et de manière intense les patients atteints d’un cancer des voies aérodigestives supérieures, surtout en cas de mucite. Les causes précises n’ont toujours pas été identifiées. La lactoferrine en bain de bouche semble atténuer ce symptôme
Background: Metallic taste is frequently reported by patients during treatment for head and neck. Very little is known about this symptom: What is its frequency? Its cause(s)?A prospective observational study was carried out to determine the incidence and cause(s), potentially possible according to the literature. Two hypotheses were put forward: lipoperoxidation of the oral mucosa, and release of facial nerve inhibition on the glossopharyngeal nerve.Methods: 44 patients were followed before, during and up to one year after completion of treatment for head and neck cancer. Evaluations by questionnaires, salivary sampling and supraliminal taste intensity and recognition tests were carried out regularly, up to eight times, for one year.A potential antidote, lactoferrin mouthwash, was also proposed in case of metallic taste. Results: The metallic “taste” affected 27.2% of patients, with moderate to severe intensity in 42.9% of sufferers. It appeared mainly in the middle and at the end of radiotherapy or radiochemotherapy, with a significant association with the presence or absence of oral mucositis. The metallic taste had a significant impact on taste-related quality of life, but not on the visual analog scale of ingestion or weight. The etiological hypotheses of lipoperoxidation and lifting of facial nerve inhibition on the glossopharyngeal nerve were not verified. Finally, the metallic taste could be alleviated by lactoferrin mouthwash in 63% of cases (completely in 16% and partially in 47%).Conclusion: metallic taste is a frequent and intense complaint in patients with head and neck cancer, especially in cases of mucositis. The precise causes have yet to be identified. Lactoferrin mouthwash seems to alleviate this symptom

Les jus d'oranges industriels produits à partir d'oranges Navel développent une amertume progressive, quelques heures après l'expression des jus. Le phénomène s'étend également à d'autres jus d'oranges. Le composé responsable est un dérivé triterpénoïde hydrophobe, de faible masse moléculaire et de très faible solubilité dans l'eau: la limonine. De nombreuses tentatives pour désamériser les jus ont été effectuées avec plus ou moins de succès. Actuellement, les jus d'oranges amers sont désamerisés par adsorption grâce à l'utilisation de résines polystyréniques réticulées dont l'utilisation est interdite en Europe. L’étude du trouble de différents jus d'oranges (Navel, Valencia et Brésil) a été effectuée parallèlement aux tentatives de désamérisation par adsorption. Pour un même adsorbant, la désamérisation est plus ou moins efficace selon les jus utilisés. Certains jus présentant un trouble sont mieux désamérisés. Les pectines hautement méthylées (HM) semblent être étroitement associées à cette efficacité du support. L’accroissement artificiel du caractère hydrophobe de pectines HM de citrons, par acylation, leur confère des propriétés adsorbantes vis-à-vis de la limonine. Parallèlement à cette mise au point de désamérisation par adsorption, l'étude du trouble des jus a été effectuée afin de corréler la présence d'un ou plusieurs composés du trouble avec l'efficacité de la désamérisation par adsorption. Une tentative de caractérisation des associations qui prennent place dans le trouble des jus a été effectuée

Les systèmes d'élevage des petits ruminants au pâturage sont les plus répandus en zone tropicale humide mais l'exposition aux strongles gastro-intestinaux entraîne des pertes importantes de production. L'objectif de ce travail est d'évaluer un système de gestion mixte du pâturage associant diverses espèces animales permettant à priori d'accroître les performances via la réduction du parasitisme et!ou une meilleure alimentation. L'étude de la littérature a conduit à une méta-analyse confirmant de meilleures performances individuelles ou à l'hectare pour les ovins en mixte. Préalablement un dispositif visant à vérifier la non-transmission des larves d'Haemonchus contortus des caprinsaux génisses Créoles a été validé. Un second dispositif avec des chevrettes conduites en mixte, infestées ou pas par Haemonchus comortus (Ml et MnI) ont été comparées à des chevrettes contrôles infestées ou non (CI et CnI). Ce dispositif a été conduit pendant 2 ans avec des mesures du couvert pâturé, des mesures individuelles des quantités ingérées, de la digestibilité, du parasitisme et de la croissance. De plus fortes croissances ont étéobservées en pâturage mixte que les chevrettes soient infestées ou pas (43.25 et 31.68 glj pour MDI et MI vs. 32.44 et 17.91 glj) avec une moindre biomasse. Les mesures d'ingestion et d'ingéré digéré ont été corrélées aux croissances des chevrettes contrairement aux variables parasitaires et met en évidence le rôle de l'alimentation et de la résilience dans le bénéfice du pâturage mixte. Une autre gestion intégrée du pâturage a été étudiée basée sur l'utilisation du vermicompost et s' ant sur les mêmes leviers d'action: l'alimentation et le itisme
Systems of small ruminant grazing are most prevalent in the humid tropics but this environment exposes animals to gastro-intestinal with production losses. The objective ofthis thesis is to evaluate a system ofmixed management ofthe pasture combining various animal species, which allows a priori to increase animal performance by reducing the parasitism and/or better nutrition. The study of literature bas led to a meta-analysis confirming the better individual performances or calculated per ha for sheep reared mixed. The first experiment validated the non-transmission of Iarvae ofHaemonchus contortus from goats to heifers. In a second experiment, goats mixed with heifers, infested or not with Haemonchus contortus (Ml vs MnI) were compared with controls goats, reared alone, infested or not (CI and CNI). This deviee with a continuous driving was studied for two years with measurements on the sward, individual measures of intake, digestibility, parasitism and growth ofthe goats. The highest growth rates were measured with the mixed pasture as goats are infested or not (43.25 and 31.68 g / d for MN! and MI vs. 32.44 and 17,91 g / d) with lower biomass. The herbage intake and the digested herbage intake were well correlated to the growth rate, unIike parasitic variables, and highlight the role of diet in the benefit of mixed driving for goats. Therefore, the infested goats with Haemonchus, expressed greater resilience with improved growth performance compared with controls. Parallel to the study ofmixed pasture, another integrated management of grazing was studied, based on the use of vermicompost and relying on the same levers. namelv throuzh feedinz and parasitism

Abstract (sommario):

Children in many parts of the world commonly play games of touch chase, in which the chaser catches the other players by tapping them, the role of chaser often then transferring to the caught player. At the time of the global spread of the Covid-19 pandemic in early 2020, reports began to crop up, especially on social media, of children playing chasing games with names like Coronavirus Tag, Corona Tip, Infection and Covid Tiggy. In these, the chaser was the virus and had to chase the others who, when caught, then ‘got the virus’. Games with ‘corona rules’ also emerged, in which children adapted the chase games to circumvent or accommodate social distancing measures imposed on them. The chapter draws on children’s testimony and adults’ observations of these coronavirus tag games, particularly Twitter posts, to trace their rapid emergence on an international scale, their varied and ingenious forms and potential functions. The focus is on the players’ perceptions and experiences in order to emphasize the fluidity and dynamism of this play and to trouble adults’ tendency to adult-centric and monolithic interpretations. The chapter argues that it is essential to consider such games both in the context of the pandemic and from a comparative and historical perspective. This highlights that what children did in making their games of coronavirus tag is in fact just one example of what they do on a day-to-day basis when there is no pandemic, drawing on mediascapes, and the material, environmental and temporal resources available to them, in often overlooked and undervalued acts of everyday creativity and resourcefulness.

Online adjunct faculty encounter a unique set of stressors compared to traditional face-to-face faculty. This chapter provides a history of concerns raised by online faculty and one of the author's experiences in mentoring them. It also centers on a two-part study intended to identify the most prominent stressors perceived by online adjunct faculty. Part 1 consisted of individual in-depth interviews with online adjunct faculty members. The results were then applied to create a 30-item rating survey of the importance of identified stressors. The items clustered into five factors perceived as most stressful: student readiness, student communications, administrator communications, teaching load, and teaching expectations. The chapter then identified implications for administrators, including recommendations for practical application to create maximally effective and minimally stressful teaching environments for online faculty.

Abstract (sommario):

This chapter discusses the fieldwork on Hinchinbrook Island. It explains that the fieldwork focused on the effects of varanid lizards as top predators on smaller lizards that are intermediate predators. Some of the struggles during the fieldwork were the insects and the operating permits since Queensland's Parks and Wildlife Service often seems to feel that scientists are annoyances that want to interfere with the proper management of parks. The chapter notes how the project remained, but some parts of it died off after Parks and Wildlife decided to build an elevated walkway past half of the designated plots. Regardless of the aftermath, many volunteers got a taste of the joys and pains of fieldwork.

People now prefer to follow trends. Since the time is moving, people can only keep themselves from being left behind if they keep up with the pace of time. There are a lot of websites for people to explore the world, but websites for those who show the public something new are uncommon. This paper proposes an web application to help YouTuber with recommending trending video content because they sometimes have trouble in thinking of the video topic. Our method to solve the problem is basically in four steps: YouTube scraping, data processing, prediction by SVM and the webpage. Users input their thoughts on our web app and computer will scrap the trending page of YouTube and process the data to do prediction. We did some experiments by using different data, and got the accuracy evaluation of our method. The results show that our method is feasible so people can use it to get their own recommendation.

Abstract First E&P commenced her development drilling campaign in the ALA field with a set of four batch wells (ALA west Phase 1A). Casing design and well architecture for the four wells were determined using standard design software, as the field had significant appraisal activities. However, actual drilling of the top-hole section on the four wells were very challenging such that, only in one of the four wells was the surface casing successfully run to depth and cemented as planned. Issues encountered included but not limited to: Trouble drilling through very reactive gumbo clay. Trouble pulling out of hole drilling BHA. Trouble running surface casing through long section of reactive clay causing stuck pipe (Casing got stuck while running in hole). Although, a leading contribution to getting stuck was differential sticking which can be attributed to not being able to run the casing with centralizers installed (bow spring centralizers) because they got hung up at the conductor shoe which was a buckled at the shoe due to piling effects. As a result, this study was carried out to investigate the possibility of mitigating the identified challenges, while optimizing the drilling of the top holes to improve casing and cementing operations. A starting point was to estimate the theoretical minimum casing setting depth. Using a Pressure Balance method, calculations were made to derive a mathematical model for the kick tolerance. The kick tolerance requirements were then derived in line with company policy and pore/fracture pressure information from offset wells and studies data (MDT & LOT) to arrive at the minimum casing setting depth. A second mathematical model based on limited gas kick model load case was also derived from pressure balance calculations, to estimate casing internal pressure profile when a gas bubble reaches surface during well control circulation using drillers method. A realistic criterion for estimating burst load, consistent with the definition of kick tolerance, was then proposed, to optimize the casing design. The study estimated cost savings of up to $1 million/well could be realized. The approach confirms huge cost savings can be realized by optimizing casing setting depths, and that illustrates the impact of safety factors. The depth proposals from this study were like the depths planned for the new drilling campaign of phase 1A+ wells, the result of actual drilling of the phase 1A+ wells (ALA-5 & ALA-6) confirmed the following, Surface holes in ALA field can be drilled to shallower depths. This may have shortened the time to run the casing to depth given historical challenges. The actual depths of the surface casings are like the depth proposed by this study. Problems of drilling with water-based mud can be mitigated if drilling intervals are short. A price comparison of top-hole actual drilling and running casing of ALA-3 a phase 1 well and ALA-5 & 6 phase 1A+ wells showed significant cost saving estimated above $1 million. This is like the cost saving estimated from the initial study. The surface hole drilled total depth and casing shoe depth on the phase 1A+ wells confirmed the pre-drill kick tolerance estimated in the study for these depths in the phase 1A+ wells.

En 1728, le Chevalier de Mailly publie Principales Merveilles de la Nature, un texte qui se trouve à mi-chemin entre l’œuvre encyclopédique et le récit de voyage. L’auteur offre un très riche recueil d’extraits littéraires, concernant des éléments ou des phénomènes naturels, tirés de la production d’une grande variété d’écrivains anciens et, surtout, modernes. Mailly consacre huit chapitres de son ouvrage à l’eau dans son état liquide et solide, ainsi qu’à des variations de sa composition dans diverses régions de la Terre.Qu’elle soit de source, de rivière, de lac ou de mer, l’eau est nécessaire à la vie et elle se révèle utile au quotidien des hommes dans plusieurs facettes. Or il arrive que ce liquide acquière des propriétés rares, voire inouïes. L’écrivain présente un classement selon la nature des eaux qu’il cite : pierreuses, huileuses, enivrantes, brûlantes, etc. Quant à notre étude, elle dégage une perspective utilitaire des particularités de ces eaux qui se changent en pierre, en verre ou en bois –et qui servent à la construction de bâtiments–, des sources huileuses –tantôt utiles à goudronner les bateaux, tantôt utiles à fabriquer des médicaments ou des baumes–, des fontaines qui ont le goût du vin, des fleuves où la baignade rétablit la santé et, bien sûr, des lacs où la pêche non seulement garantit la subsistance, mais encourage le commerce. Toutefois, la nature des eaux que décrit Mailly n’est pas toujours bénéfique à l’homme, puisqu’il faut aussi retenir des cas où les sources empoisonnent, causant un malaise temporel, l’aliénation mentale ou la mort. L’eau devient aussi un danger dans la mesure où certains lacs engloutissent les êtres qui troublent leur calme.
