Testo completoLike many other laws passed in criminal matters, law n° 2008-174, dated 25 february 2000, relating to security detention and criminal irresponsibility due to mental disorders, was adopted in response to the deep emotion of the public opinion generated by the media coverage of tragic events.This law has two main components : the first focuses on "security detention" for criminals with severe personality disorders, cause of a "particularly dangerous nature" ; the second part entitled "declaration of irresponsibility due to mental disorders" interested persons found not criminally responsible in application of article 122-1 § 1 of the Penal Code. While the second part of the 2008 law had a lesser media coverage than the first one, he raised and still raises heated debates. Composed of several dispositions inspired by various works related to the question of criminal responsibility due to mental disorders, that led to the formulation of sometimes surprising recommendations, the second part of the 2008 law was strongly criticized. Some of these dispositions, however, were unanimously approved. After five years of application, what conclusions should be drawn ? During the investigation phase, the results are somewhat mixed. It seems that some of the changes that were made on the occasion of the 2008 law have an uncontestable utility, but, others, however, are insufficient or unnecessary

Testo completoPsychiatric disorders (PD), such as schizophrenia, and bipolar disorder, affect approximately 12.5% of the global population in 2019. These disorders are defined by a disturbance in cognition, mood, and behaviour or an individual. Today, psychiatry is the only branch of modern medicine for which no biomarker (BM) has been validated to help make the diagnosis. However, many studies show that there is a link between the immune system (IS) and PD. It should therefore be possible to identify BMs of PDs in the blood. In this thesis, I used omics methods to investigate the IS of individuals with PD after grouping them by the similarity of their disorders or by the severity of their symptoms. I identified altered miRNA signature specific or common to broad groups of PD, including miRNAs already associated with BD as well as others that were not already associated with BD. I also studied the proteome, methylome and miRNAome of PD patients after unsupervised classification into 2 groups, according to the severity of their symptoms, using phenotypic data collected during the longitudinal PsyCourse study (IPPG, Germany). While methylome and miRNA analyses did not identify differences between the 2 severity groups, proteome analysis identified specific signatures, including an up- regulation of PLAUR, a gene known to be associated with blood-brain barrier disruption and cognitive abilities. Overall, this thesis sheds light on the molecular signatures related to BD severity

Testo completoThe present study aims to explore the exposure to stressful events during psychiatric admission and psychiatric setting, and the predictive power of peritraumatic distress and dissociation in the development of acute PTSD symptoms after exposure to such events. Psychiatric inpatients (N=239) were asked to report exposure to potentially traumatic events during their admission within 48h of their being admitted. Individuals reporting at least one potentially traumatic event during admission (n=70, 29%) were assessed for peritraumatic dissociation and distress in relation to the worst event, and five weeks later were reassessed for acute PTSD symptoms. Eight participants (12,3%) scored above the score threshold for a probable acute PTSD. Multiple regression analyses revealed that peritraumatic distress was a significant predictor of 5-week acute PTSD symptoms. Our findings suggest that patients who had hight levels of peritraumatic distress related to the worst potentially traumatic event experienced during psychiatric admission, might be at risk for acute PTSD symptoms and might benefit from increased attention. Five weeks after admission, participants (N=176) were screened about possible exposure to potentially traumatic events during hospitalization. Then, those who had experienced at least on event (N=37, 21%) responded to Peritraumatic Distress Inventory and Peritraumatic Dissociative Experiences Questionnaire (N=34). Five weeks later, 2 participants (5,9%) had probable acute PTSD. Furthermore, our results showed no association between peritraumatic reactions and acute PTSD. Finally, three month after exposure to the worst event occured during psychiatric admission or hospitalization, no participant has developped a probable chronic PTSD. This research highlights the need to be especialy careful during psychiatric admission or hospitalization procedures and to protect the most vulnerable patients

Testo completoChildbirth can be experienced as a traumatic experience by the mother. This event may be associated with the expression of Post-Traumatic Stress Disorder (PTSD) in the postnatal period, but does it have any impact on the development of maternal bonding? What are the factors associated with postnatal PTSD and with the impairment of maternal bonding? Are there differences in risk factors for PTSD and in altered maternal bonding between two populations from two different cultures? To answer these questions, 646 French and Tunisian women were approached during the third trimester of pregnancy. Of these women, 284 were contacted two months after delivery and 190 were recalled at six months postpartum. Within this study, it was possible to adapt and validate a coping strategy scale on the Tunisian population and to build and validate a PTSD specific scale related to childbirth, according to the new diagnostic criteria of DSM 5, on French and Tunisian women. The results of this work showed that at two months after childbirth, 5.2% of French women and 16.5% of Tunisian women had PTSD. At six months postpartum, the prevalence of PTSD was 3.1% for French women and 16.3% for Tunisian women. Hierarchical regression models specific to each population showed that for French women, it was the primiparity and mode of delivery, self-blame used during the prenatal period, prenatal physical pain, as well as the negative perception of support from the health care team during delivery that constituted risk factors for the development of postnatal PTSD. For Tunisian women, it was the primiparity and mode of delivery, the negative perception of support from the health care team during delivery, the use of dramatization and self-blame after delivery that were risk factors for the development of PTSD in the postpartum period. With regard to the effect of PTSD on maternal bonding, the results showed that this disorder had a negative impact on the development of bonding in the postnatal period and more specifically on the alteration of the mother-child bond and child related anxiety for French women. The results also showed that coping strategies such as self-blame, behavioural disengagement and less planning had a mediating role between postpartum PTSD and maternal bonding. For Tunisian women, PTSD had a negative impact on bonding and especially on child related anxiety. Coping strategies that mediated the relationship between post-natal PTSD and maternal bonding in this population were dramatization, self-blame and rumination. This research also provided etiological models on the impact of post-natal PTSD on bonding and the different associated variables for the French and Tunisian populations. In conclusion, the tools developed and validated on both populations in this work as well as the results presented establish important foundations for future interventions to enable better prevention and diagnosis of post-natal PTSD as well as to prevent its deleterious impact on the mother-child bonding

Testo completoThis report aims to encourage a better understanding of the role played by spirituality in recovery from a mental disorder. A phenomenological analysis of individual interviews was conducted with three people engaged in the recovery process. An analysis of a focus group interview with four social workers was also performed to describe and understand the consideration of spirituality in practice. The general structure of the participants’ experience in the recovery process is presented via two points to reveal that spirituality is the experience of union between “oneself” and the “other” as well as being an experience of self-transformation. Social workers understand the importance of spirituality when providing support for their clients but it also has its limits. The similarities and the differences between these two groups are discussed and recommendations are made. KEYWORDS: spirituality, recovery, mental disorder, phenomenology, social work, focus groups.

Testo completoIntroduction: Attention Deficit/Hyperactivity Disorder (ADH/D) still remains controversial in France, leading to a misreading of the disorder, a lack of diagnosis, and treatment, despite the considerable impacts in patients' life, psychological as well as functional. If AD/HD is recognized in children, the lack of studies in adults needed an input of new perspectives, especially in the psychological and psychopathological fields.We focused on the impulsive dimension of AD/HD, by evaluating psychiatric and additive comorbidities, two personality dimensions (Impulsivity and Sensation Seeking), and levels of functioning impairments in several dimensions, among adults, presenting an Attentional Disorder with or without Hyperactivity. Method: 199 respondents aged between 18 and 65 (mean= 34.3 years), coming from psychiatric consultations specialized in adult ¿AD/HD, received diagnosis of AD/HD (ASRS-v1.1; CAARS; WURS-61) and were classified in two groups depending on the form of disorder presented, either ADHD or just AD. The two groups, ADHD (n=116) and AD (n=83) were evaluated for the prevalence of psychiatric and addictive comorbidities, including impulse control disorders (MINI, MIDI), personality dimensions (BIS-11; FIDI; SSS) and functioning impairment level (WFIRS-RS). Our procedure allowed both categorial and dimensional assessments of AD/HD diagnosis. Results: We found as majority psychiatric comorbidities, mood disorders (specifically dysthymia), anxiety disorders (specifically generalized anxiety), and impulse control disorders (pathological buying) among the additive comorbidities. No significant difference was found in terms of prevalence between the two groups. In addition, the personality dimensions (impulsivity and sensation seeking) and the functioning impairment levels (7 dimensions) were all significantly higher for the ADHD group (vs AD group). However, all the respondents displayed identical score profiles in the two groups, concluding that, only the intensity levels of impulsivity, sensation seeking and impairment functioning, discriminate the two groups.Conclusion: Impulsivity is the core manifestation of AD/HD, appearing through high prevalence of psychiatric comorbidities (through mood lability and pathological anxiety) addictive disorders (whether related to psychoactive drugs or behaviors), as well as impulsive personality trait, and some functional impairments (specifically those related to daily life skills). Our procedure shows that a dimensional evaluation of AD/HD diagnosis should always complete a categorial one, in order to evaluate the extent of the impulsive dimension in AD/HD. This study provides first data on psychological and psychopathological profiles of French AD/HD adults and future researches are needed to complete these data

Testo completoWe aimed at examining if the use of mental health services modified psychiatric comorbidity with substance use disorders and at studying substance use in response to psychiatric disorders (prevalences and associated factors). Our two first studies showed that the use of mental health services was associated with a decreased risk of problematic substance use among subjects with several non substance-related psychiatric disorders. The third study showed that substance use in response to psychiatric disorders was frequent. This use, with healthcare use or not, was more frequent among men, young people, low educational levels and those living alone. Subjects aged 30 or younger were more likely to use substances, without consulting a medical doctor. If mental health qtatus was severe, the combination of substance use and healthcare was more frequent. These studies might define and target interventions for drug problems prevention

Testo completoPeople with a history of childhood maltreatment are more likely to present with several psychiatric disorders. Comorbidity between disorders tends to be the rule rather than the exception in individuals who have experienced childhood maltreatment. Given the presence of multiple comorbid psychiatric disorders in this population, the transdiagnostic approach offers a conceptual frame with which to explore common dimensions and risk factors of psychiatric disorders. According to the transdiagnostic approach, dimensions and risk factors shared between psychiatric disorders could be genetic, neurobiological, psychological and neuropsychological. However, few studies have identified neuropsychological mechanisms associated with maltreatment among people with several psychiatric disorders. The general objective of the current thesis is to explore the transdiagnostic neuropsychological mechanisms associated with childhood maltreatment among people with psychiatric disorders. The first study, a meta-analysis, highlighted transdiagnostic neuropsychological mechanisms among people with several psychiatric disorders. Specifically, childhood maltreatment had a significant impact on working memory, verbal episodic memory, intelligence and processing speed. The second study, an empirical study, explored the role of neuropsychological impairments in the presence of psychiatric symptoms among teenagers with a history of childhood maltreatment and multiple psychiatric disorders. This study showed that retrieval in visual episodic memory and planning are predictors of externalizing symptoms among teenagers. Altered executive mechanisms could explain the variance of externalizing symptoms among teenagers with several psychiatric disorders. The work within this current thesis represents the first attempt to explore the common neuropsychological dimensions between psychiatric disorders in a transdiagnostic approach. Indeed, this thesis identifies neuropsychological mechanisms associated with childhood maltreatment that are common among people with several psychiatric disorders. Moreover, some of these neuropsychological mechanisms are predictors of the variance of externalizing symptoms among teenagers. In short, this thesis improves the current understanding of the complex relation between childhood maltreatment, neuropsychological mechanisms and the presence of psychiatric symptoms, according to the conceptual framework of the transdiagnostic approach.

How does psychoanalysis comprehend the perinatal moment, the becoming mother? To the never-ending interrogation surrounding the maternal and feminine, where Freud sees maternity as the accomplishment of feminity, Lacan differentiates the register of mother's satisfactions from the one of women's desire. By promoting this gap between the mother and the woman, he breaks with the Freudian idea of idealization of the mother by the woman, and renews considerably the clinic of maternity. This dissertation on the perinatal moment puts at its heart the primordial role of the symbolic fonction of the Father, and the rectifications in the relationship to the object which the phallic treatment of the child-object imposes. From fetishistic maternai jouissances to the consideration of lack, castration or the nothing in the feminine ideal, this study offers a psychopathologic and psychoanalytic reading of the unconscious dimensions of the clinic destinies of the becoming mother. Through the role of pregnancy in psychosis, through the unconscious motives of perinatal depression, through incoercible pregnancy vomiting, it thus brings to light the multiple unconscious arrangements that each woman is constrained to, in the complex work that constitutes the becoming mother

Testo completoThe aim of our projectwas to examine semantic access from an electrophysiological view. We specifically explored two Event-related Potentials (ERPs): N400, associated to semantic processing, and P600, associated to syntactic, semantic and non linguistic and general processes. In the first part of our project, we have explored these ERPs among healthy participants and psychiatric patients. People with schizophrenia exhibit thought and language disorders. In spite of common clinical manifestations with schizophrenia, patients with bipolar disorders (BDs) have rarely been assessed using neurolinguistic ERPs. We have conducted three electrophysiological studies in natural speech conditions. In the first study, we validated our original linguistic material among healthy participants. Then, in a clinical transfer perspective, we assessed manic patients suffering from BDs (second study) and patients with schizophrenia (third study, in preparation) using the same linguistic material. Our results support the hypothesis of a preserved N400 component in patients with BDs. On the contrary, preliminary results showed a disturbed N400 associated to congruent sentences among patients with schizophrenia.After discussing the limitations of the notion of clinical transfer and of our experimental studies, we also presented two alternative perspectives on speech comprehension. One the one hand, we described a linguistic model inherited from phenomenology, structuralism and morphodynamics. On the other hand, we reported first person data obtained through elicitation techniques. These experiential data enable us to better grasp semantic phenomena involved in our both experiments

Testo completoThe persons affected by mental disorders and illnesses, as persons given the legal personality, have rights. The mental illnesses and the intellectual deficiencies having incidences on the perception of the reality, the consciousness and/or the intellectual abilities of the person, entail inevitably consequences on the capacity to will or to decide. Taking this acknowledgement into consideration, the legislator has set up rules providing the protection of the person and of his goods, while respecting the person's autonomy. There are also legal provisions facilitating the autonomy of the disabled persons. Moreover, the change in judgment of the person has consequences on the person's civil and penal liability. Furthermore, as vulnerable person, the person affected by mental disorders and illnesses needs protection. In accordance with the principle of the respect of dignity, laws and regulations guarantee a specific protection of the body and of the person. Testo completoIn the perspective based on by J. Gagnepain and known under the term of clinical anthropology, language is understood like the place of a deconstruction of analogic determinism. At the determinism of sign that specifies the language proper, appoint the << outil >>, the << personne >> and the << norme >> that make it respectively writing, speaking and speech. Particularly, this work contributes to explain, about the sign, the gnosical function, natural ability to perceptive cognition which relation to grammaticality makes the ambiguity of human language. Similarly, about the norm, we took interest in what we name aboulia that is natural ability to drive (or wish), and which relation to morality makes the ambivalence of human will. Theoretically, we present the hypothetical model of language, understood like dialectic of gnosia and grammaticality ; the model is transposed to the norm, understood like dialectic of drive (or interest) and morality. Epistemologically, we argue the necessity of dialectic conception of language, against classical conception of the reference in linguistic and against the conception of perception in psychology ; in the same order, we argue the interest of dialectic conception of will, between wish (or drive) and ethic moderation, against structuralism in psychoanalysis and economism in psychology. Clinically, we defend a method where model and nosographia are inseparably compared, and where it is matter of coherence and cohesion's syndromes. We based here on the investigation of agnosia, neurological disorder of perception, that we put up against aphasia (disorder of grammatical analysis) and schizophasia of stuttering ; for agnosia, we argue dominance of grammatical analysis, without cognitive perception. Similarly, we present an investigation of two so-called maniac-depressive patients in psychiatry, for which we suppose an aboulia, put up against caracteriels, addiction and neurosis ; about aboulia, we argue dominance of ethic moderation, without wish preference (or comportemental direction)

Testo completoRepetitive and restricted behaviours (RRB) are a core symptom of autism spectrum disorders. Their physiopathological mechanisms are still poorly understood, and an accurate and standardized description of these behaviours is needed to advance the understanding of this heterogeneous clinical dimension of autism. We have therefore developed and validated a new assessment scale for such repetitive and restricted behaviours, i.e. the RRB scale. This scale has good psychometric qualities and describes 4 clinically meaningful factors, "sensorimotor stereotypies", "reaction to change", "restricted behaviours" and "modulation insufficiency". The RRB scale constitutes a real advance towards a descriptive and neurofunctional approach to autistic disorders. It should be valuable to facilitate bioclinical comparisons and genotype-phenotype studies in research. It should also be useful in clinical practice to adapt treatment

Testo completoPrematurity, neonatal diseases and mother child separation at birth constitute for the future of parental dynamic a non negligible traumatism. Twenty vears after the opening of neonatal care units to parents, the clinical study (1990) of this early sequence in fifty families of neonates hospitalized in amiens teaching hospital, and then of the late outcome (1994) of maternal and paternal representations, shows a stil pending process of psychic elaboration. This assessment, taking the way of parental talk and behaviour as well as the one of children symptoms, shows obviously that this opening is inadequate if it is not accompanied by a real psychologic support of both parents. The specific attention paid to the father in this research underlines the importance of his talk in the maternal figuration as well as in the speeding-up of the initially manhandled indentification process. Thus, in reality as well as in maternal representation, taking into account the place of the child's father seems to us to be the keystone of any questionning on the outcome of birth inaugural perceptions and of suffering induced by separation

Testo completoSchizophrenia and bipolar disorder are major psychiatric disorders weighing colossally in economic, social and human terms, generally leading to the marginalization of the affected population, mainly in the absence of psychosocial and family therapies. This thesis is devoted to study and predict the development of these psychiatric disorders using neuro-developmental trajectories and genetic determinants for children at genetic risk of schizophrenia (SZ) and bipolar disorder (BP), i.e. those who are born to a parent affected by these illnesses. The main aim is to develop a predictive tool involving genetic risk scores and risk indicators, which may be useful in the intervention in the primary care clinic to distinguish the most offspring at risk of later transition to these disorders among the high-risk children born to a parent affected. The use of joint latent class mixed modeling (JLCM) of time-to-event and the evolution over time of several longitudinal outcomes, while taking into account the confounding variables effects was necessary for the study of a sample of 67 children from the population of Eastern Quebec to be stratified, after having presented theoretically their mathematical basis and examined empirically their identifiability.
