Approved for public release; distribution is unlimitedThis thesis examined two sites from a Landsat scene of portions of Honduras and Nicaragua. One site was examined for potential water obstacles, and the other was examined for cover and concealment provided by vegetation. The results suggest that potential water obstacles can be detected. It is not clear if vegetative cover and concealment can be reliably detected. A study using better ground reference information than was available is necessary to answer that question. Several unsupervised classification algorithms were used and compared. A histogram clustering algorithm followed by a minimum distance classifier provided results comparable to the much slower K-means and isodatatype algorithms. Several methods to reduce the dimensionality of the classification problem were examined, including band subsets, between-band ratios, the principal component transformation, and the tasseled cap transformations. Band subsets provided adequate accuracy and is the easiest method to implement.

Honour killings are considered by the perpetrators the only path to maintain theirs and their family honour, preventing other's to follow behaviours that move away from traditional patriarchal values. With the aim of exploring how honour killings are characterised, a qualitative study within three European newspapers, in three different languages, was conducted. The findings showed that often the victims are characterised as young women and girls that want to live independently from their birth cultures or that acted in a way considered to be a dishonour to their family culture. Perpetrators are often described as family members, mostly males to whom honour is worth more than life. However, some of the news articles launch the debate of the thin line that divides honour crimes and domestic violence making this not a cultural problem but a widespread cross-cultural reality.

Research on heavy metal music has traditionally been framed by deviant characterizations, effects on audiences, and the validity of criticism. More recently, studies have neglected content analysis due to perceived homogeneity in themes, despite evidence that the modern genre is distinct from its past. As lyrical patterns are strong markers of genre, this study attempts to characterize heavy metal in the 21st century by analyzing lyrics for specific themes and perspectives. Citing evidence that the "Millennial" generation confers significant developments to popular culture, the contemporary genre is termed "Millennial heavy metal" throughout, and the study is framed accordingly. Utilizing prominent metal albums from the 2000-2009 decade, 250 randomly selected songs are analyzed for the inclusion of 115 different themes and their contextual usage. The frequency of each theme is then ranked by prevalence in order to illustrate common focus in the genre. Results showed that themes of death, storytelling, violence, and social commentary prevail; overall, accentuated topics and their usage point to an aptitude for confronting tenebrous realities and personal attempts to understand them. Characteristic connections between Millennial metal and the Millennial generation are also noted through emphasized individuality, social awareness, and civil liberalism.M.A.

The literature on providing an evidence base for clinicians to deliver culturally appropriate services appears to be responding to the changing demographics in the United Kingdom (UK) but appears slow to respond to the gradually changing demographic of the profession. The research on ethnic minority status is largely focused on clients with the assumption that the therapist is White (Iwamasa, 1996). Literature exploring the experiences and issues of ethnic minority therapists emerged in the United States of America (US) before being explored in the UK. Even today, a large chunk of the literature is produced in the US and more so from the counselling and psychotherapy disciplines. This qualitative study explores the practice of clinical psychologists who identify as British Asian, when working with ethnically similar and different clients. Ten clinical psychologists were interviewed about therapeutic interactions where they encountered ethnicity and culture. Thematic analysis was used to analyse the interviews of the participants who self-identified as British Asian. Three dominant themes emerged from the analysis: Addressing culture and ethnicity in therapy, challenges in the room and dilemmas in the profession. Findings revealed that culture and ethnicity were not central to all therapeutic interactions. However, when ethnic and cultural differences were present, participants responded in various ways to the issues they faced in the therapy room and dilemmas they encountered in the profession. There were varying levels of comfort in having conversations about culture and ethnic differences in therapy as well as with colleagues, indicating a need for appropriate training of all clinical psychologists in working with culture and ethnicity. The role of supervision was also highlighted in helping clinical psychologists to enable reflecting on the impact of their own culture and ethnicity on practice.

This research examined which factors of influence contributed towards efficacious systemic leadership development, within case studies in the energy and military sectors, and identified and explained common underlying themes and factors which could help formulate future interventions, as well as enhance extant theoretical knowledge. The literature showed that key challenges exist in identifying what contribution leadership development makes to enhancing organisational leadership, and as such, social capital and systemic development. To focus the research, specific definitions were constructed, as to what sort of effective leadership positively impacts upon organisational leadership; which cardinal themes of leadership development traverse interventions, namely Delivery, Contextualisation, Performance, Impact and Values integration; and what constitutes systemic leadership development. A Social Constructivist position was adopted which explains the nature of the phenomena underlying systemic leadership development.

This thesis explores the topic of Canadian-Indigenous battered women who killed their
intimate abusers and seeks to better understand these women’s experiences, their treatment within the Canadian criminal justice system, and how BWS was used in their cases. A theoretical framework comprised of Indigenous Feminisms and Intersectionality was used to guide this research study and to shed light on the lived experiences of Indigenous battered women who killed their abusers. Various important Indigenous Feminist theorists such as Dian Million (2013) and Patricia Monture-Angus (1998) were drawn upon as well as advocates for Intersectionality such as Patricia Hill-Collins (2019). A qualitative thematic analysis was performed to create four
overarching themes from eight cases where Indigenous battered women killed their intimate abusers.

This study seeks to understand and document how people recovering from opioid addiction communicate about health and identity in the context of a methadone clinic. This project emerges in response to the rising incidence of opioid addiction and overdose death in the U.S. and suggests there is an urgent theoretical and practical calling for bringing forth the stories of people experiencing addiction and recovery. Guided by the structure-centered approach (SCA) to health communication, this study employs a thematic narrative analysis through interviews with 4 methadone maintenance treatment (MMT) patients about the meanings of health, recovery, and identity. With respect to how methadone patients communicate about health and identity, study findings reveal three key themes, which I distinguish as: 1) health as normalcy; 2) health as lack of resources; and 3) recovering the self, (be)longing, and health citizenship. It is through an understanding of these themes that this project contributes, in several meaningful ways, to research and praxis on health communication in addiction treatment spaces. Limitations of research design and considerations for future research projects on health and addiction are provided.

The rise of the World Wide Web attracted concerns among social science
scholars, especially those in the communication school who studied it by various
methods like content analysis. However, the dynamic environment of the World Wide
Web challenged this traditional research method, and, in turn, scholars tried to figure
out valid solutions, which were summarized in the literature review section. After 2000,
few studies focused on the content analysis of Web sites, while the World Wide Web
developed rapidly and affected people??s everyday life. This study conducted a thematic
meta-analysis to examine how researchers apply content analysis to the World Wide
Web after 2000. A total of 39 studies that used content analysis to study Web sites were
identified from three sources. Then data were collected and analyzed. This study found
that, from 2000 to 2004, content analysis of the World Wide Web proliferated. The
content analytical scholars had created new strategies to cope with challenges posed by
the WWW. The suggestions made in this study forms some guidelines in the steps of content analysis research design, potentially aiding the future research of content
analysis to Web sites in developing their own valid methods to study the rapid-paced
WWW.

PHARMAC, the Pharmaceutical Management Agency, manages the Pharmaceutical Schedule on behalf of the Government. The Agency is tasked with securing the best health outcomes that are reasonably achievable from pharmaceutical treatment and from within the amount of funding provided (§ 47 NZPHD Act, 2000). The Agency reports that it continues to improve New Zealanders’ access to funded medicines. In determining which pharmaceuticals to fund, PHARMAC’s Operating Policies and Procedures (OPPs) state that nine criteria guide its decision- making. The OPPs further state that PHARMAC can apply whatever weight it sees fit to the application of these criteria. I undertook a thematic analysis of 20 cases referred by PHARMAC’s principal medical advisory body, the Pharmacology and Therapeutic Advisory Committee (PTAC), to PHARMAC during the period February 2004 to November 2006 to determine whether these criteria were acknowledged in the official minutes of the respective bodies. PTAC is similarly required to take account of the abiding decision criteria. I also sought to determine whether other factors were apparent in guiding the decisions. There was evidence that PHARMAC consistently applied the decision criteria. PTAC was less assiduous in recording its application. In addition, I found that PHARMAC takes account of factors outside the stated criteria. I noted that PHARMAC takes particular account of the degree to which a decision might be publicly, politically or medically contentious in its decision-making. I also found evidence that consistency with prior decisions is another factor which PHARMAC takes into account, though does not apply routinely. This research indicates that PHARMAC does take account of its abiding decision criteria, applying health needs as well as fiscal criteria, though the weighting given each criterion is nowhere apparent in its official minutes. There remains an opportunity for evaluative research to determine whether fiscal considerations ‘outweigh’ needs considerations in PHARMACs decision-making.

Background: Many partners of stroke survivors assume the role of informal caregiver. Little is known about how this informal care-giving role impacts on the partnership relationship. Method: A systematic literature review evaluated the evidence of fourteen studies, ten quantitative and four qualitative, looking at the psychological impact of informal caregiving on the partnership relationship. Evidence from the wider informal caregiving population suggested that caregivers experience high levels of depression, reduced psychological functioning and deterioration in interpersonal relationships however this review found a paucity of clear and robust evidence for the spousal caregiver as distinct from other familial caregivers. To address this gap within the literature, a qualitative study was conducted using semi-structured interviews to investigate the lived experience of six female spousal caregivers whose husbands had survived a stroke. The transcribed interviews were analysed using a process of thematic analysis as described by Braun and Clarke (2006). Results: Spouses of stroke survivors experienced a dramatic change in their relationship after their partner’s stroke. Primary and secondary loss, increased responsibility, adaptation, evaluation and acceptance themes were developed into a dynamic thematic map. The loss of aspects of both their husband’s traditional role and the reflexive nature of the relationship impacted on the quality of the relationship as a whole. The processes of evaluation and adaptation both used talking as an important strategy. Conclusions: Little is known about the caregiving trajectory for spousal partners of stroke survivors. The current study makes a significant contribution to the evidence base and suggests that the changing nature of the partnership’s interpersonal dynamics may underpin some of the negative outcomes for spousal caregivers and that a greater understanding of these underlying processes may help services to provide appropriate and timely support to this population.

A thematic analysis was conducted in order to reveal and identify central themes composing the motivational aspects of the popular Let’s Play media phenomenon. Nine informants with various connections to Let’s Plays were interviewed online through the use of a 17-item interview survey developed by the researcher. A total of five major recurring themes were discovered relating to the informants’ motivational reasons for making and viewing Let’s Plays as part of their spare time activities. Based on these discoveries, it can be assumed that the Let’s Play phenomenon contributes to modern-day need fulfillment relating to online socialization, entertainment, technological competence, commercialization and interpersonal relations between the media audience and the media personalities they follow. It was concluded that the Let’s Play phenomenon, and the communities contained within it, holds potential for the future development of videogame industry as a whole as well as being an interesting new arena for media research in general. Implications for further research within the field of Let’s Plays are therefore made in order to ensure the possibility of expanding the scientific knowledge surrounding this media phenomenon.

This study used thematic analysis to investigate the blogs of parents of children diagnosed with an Autism Spectrum Condition, with a particular focus on themes that emerge when parents write about themselves, their child and their family relationships. The first stage of the study involved a broad analysis of the first six months of entries presented within eight blogs. Themes which emerged during this stage included ‘Coping’, ‘Searching for an explanation - Meaning making’ and ‘Sense of belonging vs not belonging’. The second stage of the study involved a deeper analysis of two of the eight blogs, used as case studies, which were selected due to differing on aspects of the themes found during the first stage of analysis. Themes which emerged from the second stage were a superordinate theme of ‘Searching for an explanation - Meaning making’, with subordinate themes of ‘Relationship with ASC’ and ‘Relationship with society’. There were only brief mentions of family relationships within the blogs, except the relationship with the child and it seemed that parents often made sense of their experiences through their relationship with ASC and their relationship with society. Many of the parents who blogged reported changes in their relationship with society due to feeling that others do not understand and a sense of not belonging. It seemed that blogging provided a sense of belonging and a way of being understood for these parents, as well as a source of information to help parents explore ASC and their relationship with ASC. The themes that emerged may provide important information for clinicians and the implications of the research findings are discussed. The limitations of the study are noted and there are suggestions for future research.

Part 1: Literature Review - It is well-known that burnout is high in Community Mental Health Nurses. This has been associated with the workplace environment and tasks. Despite this, Community Mental Health Nurses are often based within multidisciplinary teams. In order to provide support for fellow team members, as well as offer an ‘alternative’ perspective, Clinical Psychologists have been offering ‘team formulation’. This is of particular interest within the clinical field of ‘psychosis’ where there continues to be uncertainty about using a diagnostic or formulation-based approach; it remains a highly contested area. Part 2: Research Report - The review of the literature aimed to examine quantitative studies and determine correlates and predictors of stress and burnout within Community Mental Health Nurses. Nine papers met the inclusion criteria, all of them cross-sectional studies. A narrative synthesis of the findings is presented using a framework of ‘individual’ and ‘situational’ factors. There was strong evidence to suggest that situational factors are highly associated with stress and burnout, however factors intrinsic to the individual were not routinely reported. Whilst burnout is operationalised within research by the use of a well-known measure, the relationship to other phenomena, such as stress and distress, is ill-defined. Part 3: Critical Appraisal - The research aim was to explore the experiences of multidisciplinary team members who have attended Team Formulation sessions within Early Intervention services. A thematic analysis was undertaken on the eleven interviews and three main themes were generated: team formulation offers a different perspective; the difference is valuable; and connection within the collective. These findings are considered within the evidence base for psychological formulation, as well as reflective practice and self-care. A reflective account of the research process is contained within the critical appraisal.

Attachment theory predicts that family bereavement leads even securely attached individuals to experience temporary attachment insecurity. Attachment in/security is displayed through the way narratives are constructed, and the Adult Attachment Interview posits particular indices of ‘narrative incoherence’ for narratives related to experiences of bereavement. This thesis explores bereaved families’ therapy narratives to see if they display signs of narrative incoherence or evidence of lack of resolution as predicted by attachment theory. The thesis also examines whether there is evidence of shifts and changes changes in the stories over time that could be understood as reflecting a move towards greater coherence. Families are dynamic systems and the stories told in family therapy are co-creations between family members and the therapists: the impact of the actions of family members and therapists on narrative coherence are also analysed. Five bereaved families’ narratives were recorded during the therapy intervention ‘Telling the Story’, at the beginning and towards the end of their family bereavement therapy. An Attachment Theory informed Thematic Analysis was carried out on the transcripts and identified four themes: Evidence of Unresolved Loss, Creating Incoherence, Creating Coherence and Evidence of Coherence. Results show that there is, as predicted by the Adult Attachment Interview, evidence of narrative incoherence, and additionally there are behavioural and systemic features that create further incoherence in the narratives. The results also show how coherence can be created and what features a more coherent family story includes. The findings have implications for bereavement therapy interventions, therapist training and methodological development. Limitations and suggestions for further research are also discussed.

Drinking alcohol to excess has been identified as a problem in the Turkish-Kurdish community in Hackney, East London. Whilst NHS services work to be inclusive, with the use of interpreters for example, there are still barriers, preventing this population from seeking help, which need to be explored and further understood. The single piece of research identifying the needs of the Turkish-speaking community in Hackney made recommendations for a more culturally appropriate care package, a more balanced range of effective therapies, and a more active role for BME community members in the training of health-care professionals. This study aimed to understand more about alcohol use in the Turkish-Kurdish community by collating the opinions and experiences of members of the community and the health-care professionals that work with them. Three focus groups were conducted: one with men only, one with women only, and one with health-care professionals only (mixed gender). A total of thirteen participants contributed to this research. Analysis was conducted within a critical realist epistemology using thematic analysis. Three main themes were identified: explanations for drinking, impact of drinking and help-seeking. Participants identified personality type, stress and culture as relevant to why an individual drinks alcohol. The impact of drinking was described as predominantly harmful, but positive accounts of drinking were also given. The findings suggest a need for NHS services to offer further support in helping the community to access services for alcohol treatment, but also to work alongside the community in creating more culturally appropriate services and to help reduce stigma associated with seeking help. A need to address wider societal concerns, such as unemployment, was also deemed important by the participants when considering factors that influence drinking alcohol. Future research might investigate how the impact of gender roles on help-seeking can be addressed in service provision planning.

In order to maintain integrity and a non-judgemental approach both in general and more specifically cross-cultural therapy, counselling psychologists need to cultivate a habit of reflection, soul searching and self-exploration. It can be argued that any client at any time is always different from the therapist, even if they come from the same culture. The way in which people engage with a cultural heritage creates implications and negotiated ways of being. For the purpose of this study, cross-cultural therapy was regarded as an activity, an attitude and a way of conducting therapy. The aim was to explore how counselling psychologists understand and practise cross-cultural therapy. An in-depth thematic analysis, embedded in a relativist social constructionist paradigm, was used to examine the interviews of eight counselling psychologists from different cultural backgrounds. Two dominant themes emerged from the analysis: cross-cultural therapy as a return to individuality and cross-cultural therapy as a return to situatedness. In the theme cross-cultural therapy as a return to individuality, participants viewed clients as individuals with unique takes on their contexts while in the second dominant theme the clients were seen as embedded in their context so therefore increased knowledge of their background was valued. There 3 appeared to be a constant motion between these two main themes which is demonstrated by the word ‘return’ in their names. A mapping for the core interpretative features of both of these themes is presented alongside discussions of the power relations that played out in participants’ narrative. Issues around power within the interaction between the client and the therapist as well as in the discourse were examined. The findings support issues uncovered in the literature but also highlight the necessarily complex task of being culturally sensitive. A positive way forward would be honest self-exploration to move towards more accepting and respectful practice in cross-cultural therapy.

According to the Office of National Statistics (2011), approximately six million people provide unpaid care to a family member. The growth of interest in the carer role has helped establish the idea that the provision of informal care warrants attention because of the relationship between caring and burden. It has been suggested that living with someone with depression is comparable to that of other serious mental health problems, such as schizophrenia or dementia. Furthermore, there is evidence that partners are most at risk of burden within the informal caregiving context. The meta-ethnography of existing research indicates that qualitative studies which specifically explore the experiences of living with a family member with depression are somewhat heterogeneous regarding types of relationship with the depressed individual. Combining different relationships (e.g. partners, siblings and parents) within the same study makes it difficult to disentangle data and therefore gaining an in-depth understanding of specific experiences is almost impossible. This study therefore aimed to explore the experiences of living with a partner with depression. In-depth interviews were conducted with nine female and four male participants who live with a partner with depression. A critical realist perspective was held and data was analysed using Braun and Clarke’s six phases of thematic analysis (2006), with the assistance of MAXQDA. Results identified five key themes: ‘making sense of the depression’; ‘the depression cannot be compartmentalised’; ‘a light at the end of the tunnel’; ‘learning to navigate the ‘depression’ maze’; and ‘gaining a new perspective’. The findings illustrate that living with a partner with depression is not a static process and that the needs of the depressed partner are constantly changing. This study examines the coming out process for transgendered individuals using the framework of Austin's speech act: the locutionary force, the illocutionary force, and the perlocutionary force. A grounded theory analysis of 43 letters in two different Internet databases revealed similarities and differences in the coming out process for transgendered individuals, compared to what we know about the process for gay and lesbian people. Similarities were found in all three acts: labels and scripts are used in the locutionary act; confession, education, affirmation and remorse are used as framing strategies in the illocutionary act; and rejection and acknowledgement are addressed in the perlocutionary act. Unique aspects of the coming out process for transgendered individuals include the importance of and reliance on labels in the locutionary act, education in the illocutionary act, and the visualization and cooperation of others in the perlocutionary act. Word order variations in Spanish have led scholars to consider it as a language with flexible word order in comparison with languages such as French or English. Attention has been mainly centred on syntactic variations involving the placement of the subject. Today, the widespread assumption is that Spanish is a pragmatically motivated language; consequently, studies have started to focus on real occurring data in order to study the pragmatic factors that intervene in the creation of meaning. The fact that the pragmatic factors are independent of position makes it difficult to use them as indicators to explain word order variations. Further, studying language in context implies that it more complex to single out a specific feature, because every element of the system collaborates in the creation of meaning.

Aims: This study explores how training in Motivational Interviewing (MI) is experienced and given meaning by 23 multi-disciplinary health professionals. It uses a qualitative interpretative thematic analysis, aiming to identify key elements in the process of learning and applying MI consultation skills in their clinical practice. Setting/method: The health professionals were recruited from two MI training programmes in Scotland, they worked in either cardiac rehabilitation or substance abuse settings. The time elapsed since training workshops for each participant varied between 4 months and three years. Data were collected electronically via E-mail and participants completed either an open-ended questionnaire or a reflective diary. Results: The data obtained via these data collection methods was rich and informative and it revealed several key experiences and factors for successfully learning and applying MI. MI training is an emotional experience before, during and after workshops. Learning MI is challenging, and a shift in professional identity with clinicians feeling temporarily deskilled is a common experience. Practice with real clients, supervision and other reflective practices, facilitate and are crucial for learning effective MI skills, and developing competency can take years. MI is also seen as powerful and concerns about sensitive disclosure may arise, that may inhibit practicing MI skills. Clinicians also find it challenging to adjust to new ways of thinking and behaving, and often revert to the more traditional authoritarian expert approach they are used to. When clinicians become more competent and skills are consolidated, they experience an increased sense of professionalism and confidence in their ability to facilitate clients in making informed choices about their health and about illness management. They also experience less stress and dissatisfaction with resistant clients. Several additional facilitators and barriers are discussed. Conclusion: The study raises implications for MI training theory and practice and adult learning theories. The findings suggest that learning MI is emotionally demanding and tiring, and that building MI competency requires a considerable amount of time and resource. This needs to be taken into account, when planning and implementing MI training programmes if these are to succeed in embedding MI culture in health services.

This research project was conducted within the setting of English General Practice and explored the experience of nurse partnerships; from the perspective of the Nurse Partners and their colleagues. The inquiry was within a social constructionist framework; the research was inductive, emerging and shaped by the researcher's experience. A critical emancipatory perspective fully embraced and exploited the subjectivity of the researcher. Reflexivity is used throughout the report, making visible the researcher's individuality and effect on the research process. It has been possible for nurses to become partners in general practice since 1997. However, it is not possible to determine the exact numbers of nurse partners, as there is no national list or database. There is little published research about Nurse Partners. A purposive sample of five self-selecting practices was used; geographically spread across England; representing the diversity of general practice. Nineteen in-depth interactive interviews were audiotaped: five GPs; five Nurse Partners; five practice managers and four members of the nursing team. The data was thematically analysed using the Attride-Stirling method of thematic networks. The web-like networks are an organising principle and representational. Two Global Themes were developed to conceptualise the main argument and assumptions, drawing on ten Basic Themes and four Organising Themes. The Global Theme metaphors became the titles of the interconnected Networks; 'Stepping out the Box' and Toe in the Water. The findings suggest that the Nurse Partners are pioneers; combining professional nursing with an entrepreneurial perspective. However, there remain cultural issues around equality. The research practices have embraced multi-professional working, with the Nurse Partner playing a major role in the senior management team of partners. The Nurse Partners enjoy an autonomy and independence; are able to show entrepreneurship and demonstrate a commitment to the 'business' of general practice. Nevertheless these nurses have not received the recognition they deserve. It is therefore a concern that Nurse Partnerships are not being considered as part of the solution to the 'crisis' in general practice. The entrepreneurship and expertise demonstrated by these Nurse partners in multi-professional working is vital to the continuation of modern effective primary healthcare.

Psychological evaluations administered by forensic psychologist in personal injury cases are surrounded by complex issues. Although empirically-based research has legitimized that psychological damages do exist in personal injury cases there is a missing link in the way forensic psychologists are conducting these evaluations. Prior researchers suggested that some personal injury evaluations had been dismissed or overlooked due to a lack of a standard of care. Addressing the current literature, this study examined how a diverse group of 14 licensed forensic psychologists, operating in different judicial jurisdictions (Daubert, Frye, and Independent) were conducting personal injury evaluations and their perspectives on the implementation of a standard of care. A qualitative thematic analysis design was used to gain a more in-depth understanding of this phenomenon. Systems theory was the conceptual framework that informed this study and guided the methodology employed. The identified themes were organized into steps reflected in an adapted version cube model. The study promotes positive social change by fostering confidence in the field of psychology and personal injury evaluations with regard to bolstering the overall credibility, reliability, and validity of the practice and processes involved. Further, positive change can occur through the development of framework that assists in leveling the practice by keeping evaluations flexible, but consistent; basing the decision regarding implementing a standard of care on the utility of the framework, along with future findings and developments in the field.

Friedrich Engels (1820-1895) in his study, The condition of the working class in England, argued that the cause of illness and death amongst the working class was due to their living conditions such as poor housing, over-crowding, poor sanitation, food shortage, low paying jobs and a lack of material resources. The objective of the study was to understand the experiences of six South African individuals who have shared their experiences and challenges of living with TB on the TB&ME blog, and to show how TB is linked to the living conditions of these individuals. The study found that the challenges experienced by TB patient bloggers are of a social nature and confirms Engel’s study findings on the conditions of the working class in England. For example, a disease such as TB has a direct association with the living conditions of people, especially the poor. Thus, socio economic status of TB patient bloggers plays a role in the escalation of their ill health. Further, the study found that gender is central in understanding non-compliance to treatment. This is significant as it highlights the need to not only focus on issues of socioeconomics, but gender issues in fighting TB. Despite the negative consequences associated with living with TB, the bloggers have noted that the support from loved ones and other stakeholders in the fight against TB alleviates the challenges inherent in living with TB.

Objectives. Recent literature indicates variance in psychosocial treatment preferences for negative symptoms of schizophrenia. Attempts at defining therapeutic aims and outcomes for negative symptoms to date have not included major stakeholder groups. The aim of the present study was to address this gap through qualitative methods. Design. Thematic Analysis was applied to qualitative semi-structured interview data to gather the opinions of people who experience negative symptoms, carers, and healthcare professionals. Participants were recruited from two mental health sites (inpatient/community) to increase generalisability of results. Ten people participated in the research. Methods. Semi-structured interview scripts were designed utilising evidence from the review in Chapter 1 of effective psychosocial intervention components for specific negative symptoms. Interviews were audio recorded and transcribed verbatim. Thematic analysis was employed to analyse data. Results. A common theme across groups was the need for a personalised approach to intervention for negative symptoms. Other themes indicated different opinions in relation to treatment targets and the need for a sensitive and graded approach to all aspects of therapy. This approach needs to be supported across systemic levels of organisation with specific training needs for staff addressed. Conclusions. There is disparity in treatment preferences for negative symptoms across major stakeholders. The findings suggest an individualised approach to intervention of negative symptoms that is consistent with recovery. Implementation barriers and facilitators were identified and discussed. There remains a need to develop a better understanding of treatment preferences for patients.

The broad aim of this study is to investigate the efficacy of Theme/Rheme analysis in identifying underlying ideologies in English and Arabic news reports. The study takes as its starting point the assumption that the different choices of Theme/Rheme, their organization at the local and global structures and the presentation of Given/New information can elucidate ideological factors that underlie news presentation in English and Arabic. This thesis designs a conceptual framework which combines structural, ideological and linguistic approaches to Discourse Analysis. It also develops an innovative methodological model to analyse Theme/Rheme in Arabic Verbal and Nominal clauses and enhances the linguistic tools to analyse the thematic structures in English and Arabic texts. The thesis applies the developed model, methods and tools to a selected corpus of English and Arabic news reports in order to identify ideological representations of Syria in mainstream online news media. The study seeks to analyse and compare the different choices of Theme/Rheme, nominalization and lexical variations in the thematic structures, on the one hand, and their implications and attribution of causality and responsibility, on the other hand. These issues are investigated in a corpus of English and Arabic news reports during times of political assassinations in Lebanon and the implication of Syrian involvement. Based on the recurrent preferences in the thematic structures of the selected corpus, the study concludes that developed model, methods and tools help identify underlying ideologies in the presentations of Syria in both English and Arabic news reports.

Mischlingskinder, also referred to as brown babies were the illegitimate children of African American occupation soldiers in post-World War II Germany. The complexities of their existence are often discussed in the context of national identity, racial identity and diplomacy. Their existence in Germany presented social struggles for the children as well as their mothers as a post Nazi German began towards society of racial acceptance. A few cases, through the cooperation of both the US and German government, some of the children were eligible for adoptions by African American families in America through the Brown Baby Adoption Plan. A thematic analysis was performed on 20 archived newspaper articles to uncover the different themes in which the children are discussed. My question is whether these themes connect to a lager theoretical concept of the “priceless child”. The results uncovered themes in which the brown babies were discussed which included their treatment in Germany, neglect, adoption and arrival in adoptive homes. The narratives of the children change over time in relation to the specific themes.

Introduction: This study investigates the experiences of individuals with autism spectrum conditions (ASC) as they experienced transition into employment in the UK. The high prevalence of ASC has led to increased social and economic pressures (Gerhardt & Lainer, 2010), with the estimated yearly cost to the UK for each individual at £90,000 (Knapp, Romeo & Beecham, 2007). Employment may help to reduce economic pressures, benefit individuals and society (Howlin, Alcock & Burkin, 2005). Individual gains in cognitive functioning (García-Villamisar & Hughes, 2007), quality of life and self-esteem (Eggleton et al., 1999) may be experienced. However, individuals with ASC tend to find transitioning to employment difficult, due to implicit characteristics associated with the condition (Berney, 2004). Transitions represent challenges to individuals’ social identity and self-concept, require behaviour change and may contribute to distress (Liddle, Carlson & McKenna, 2004). These factors may be exacerbated in individuals with ASC due to their difficulties with social and behaviour change processes (American Psychiatric Association [APA], 2010). Transition and employment support strategies have been shown to facilitate successful employment for individuals with ASC (Howlin et al., 2005). The National Institute for Health and Clinical Excellence ([NICE], 2012) propose how best to support people with ASC, based on the synthesis of available evidence. However, the evidence lacks in consideration of individuals’ experiences and psychological theories of support and transition. Considering these perspectives may facilitate service users to influence effective service provision, through disseminating their views and experiences of transition to work by means of a rigorous qualitative study. Method This study investigates the experiences of 10 participants with ASC during their transition to employment settings in the UK and aims to provide them with a platform to express their experiences and views of this process. An inductive thematic analysis methodology (Braun & Clarke, 2006) is used to explore semantic information obtained from retrospective interviews relating to the experiences of transition to work and identify frequent and salient themes within the data (Buetow, 2010). Results Analysis identified three main themes that were particularly salient and frequent for the interviewed participants: Diagnosis; comprised of subthemes ‘Access to support’, ‘Identifying with ASC’, ‘Other-understanding’ and ‘Delay in diagnosis’. Barriers to transition; comprised of ‘Socio-economics’, ‘Physical accessibility’ and ‘ASC characteristics’. Transition support; comprised of ‘Emotional needs’, ‘Clear guidance and honest feedback’, ‘Practical support’ and ‘Social integration’. Discussion Thematic analysis facilitated investigation into the experiences of 10 participants with ASC as they transitioned to work. Participants’ difficulties and needs were reflected in the existing evidence base and aims of current legislation and guidance, but short-comings in the delivery of these were also highlighted. In addition, a unique insight into both the positive and negative emotional impact of transition to work for individuals with ASC was obtained. Psychological models of support and transition are considered in relation to the experiences of transition and associated challenges to self-concept. Potential support measures for more effective and less distressing transition to work are discussed.

This textual and visual analysis of the account Coping_with_PTSD found Instagram to be an information-rich platform for supportive communication. Individuals affected by Posttraumatic Stress Disorder (PTSD) were able to provide and seek supportive communication through text, photos and emojis. Key themes identified for the account and its followers providing supportive communication were posting health information, normalizing PTSD, sharing personal narratives and providing encouragement. Additionally, account followers used tagging and direct messaging as supportive communication. Further, account users sought supportive communication through asking and disclosing personal narratives. Three themes were found for emojis: reinforcement, emotional insight and “emoji speak” as supportive communication. The examination of emojis extended qualitative textual analysis methodologies to an analysis of emojis. The study resulted in the development of a framework for studying the relationship between text, visuals and emojis. The methodology and framework presented in this paper should be developed and utilized as emojis continue to grow and replace text communication in social media.

Discussion surrounding interprofessional education (IPE) among graduate and professional health sciences students is beginning to appear more frequently in academic journals, government reports, and health care reform recommendations. Recommendations from an international conference cited in the Lancet provide an interesting overview from world leaders about interprofessional care and its ability to improve health system performance. Since 1995, the Academic Health Sciences Center (AHSC) at East Tennessee State University (ETSU) has shown the value of IPE and continues to do so through the Interprofessional Education Program (IPEP). In this study, a focus group was conducted in fall 2014 to collect qualitative data on AHSC students’ perceptions of IPE and interprofessional training at ETSU. Two out of five AHSC colleges were represented by a total of four focus group participants. These participants included students from the Colleges of Public Health and Medicine, as well as a student from the Department of Psychology and one medical resident. Through use of a semi-structured interview procedure, a skilled facilitator asked participants to describe their academic experiences related to IPE at ETSU. A series of open-ended questions were presented in order to determine the general perceptions, attitudes, barriers, and beliefs of health science students regarding interprofessional experiences. The focus group was audio- and video- recorded, and the recordings were transcribed by the author. Four study staff members met and conducted separate thematic analyses to determine consensus on overarching themes from transcriptions. Themes identified through these analyses included: 1) specific emphasis on the importance of communication; 2) respect for the academic rigor of other health profession programs; 3) the desire for more IPE opportunities; and 4) having a positive and open mind was a characteristic considered impactful interprofessional education. The implications of these findings and considerations for methodological improvements will be discussed.

The bill will allow student-athletes to receive compensation for their name, image and likeness in the state of California, and has influenced other states to bring forth legislation of their own. The NCAA announced on October 29, 2019, that it would change its policies and bylaws to allow student-athletes to profit from their name, image and likeness. This qualitative thematic analysis seeks to discover how the issue of student-athletes in the case of SB 206 was framed by the media before and after the bill was passed, and after the NCAA announced its policy change. From the analysis, three themes emerged to support student-athlete compensation, eight themes emerged in opposition, and 24 sub themes emerged for both sides. Anti-compensation framing strategies prevailed throughout news coverage before and after SB 206 passed, and after the NCAA changed its policy. The present study explored how seven women experienced having a mother with dementia. The focus was not specifically on the experience of care-giving and so daughters who lived separately from their mothers were selected. Thematic analysis was used to examine how they made sense of their experience by analysing verbatim transcripts of their interviews. The study provided personal and intimate accounts from the daughters and two main themes of bewilderment and transformed relationships were identified. These highlighted the daughters’ struggle to recognise and contend with the changes in their mothers. The context in which the themes arose appeared to be dominated by a prevailing fear associated with dementia. This seemed to be widely experienced by the mothers, the daughters and perhaps mirrored the prevalence in wider society. The experiences described were, at times, distressing for the daughters and there was a perceived lack of professional help to support both mothers and daughters through the changes. However, it was noted that some daughters also experienced positive, transformative ways to make thoughtful new connections with their mothers and aspects of the relationship were described as having improved. Therefore, for these daughters the experience appeared to be painful and distressing but also memorable and special. The new connections, that some daughters achieved, could be further explored, as this may help daughters and other extended family members to achieve more positive and rewarding relationships with their mothers and other relatives with dementia.

In face of the current economic-political changes facing the UK and its State institutions and of the new evidence about the impact of social inequality on human distress, this study attempts to understand the increasing practice of delivering psychological therapy by the British clinical psychology profession. A review of the critical histories of the profession in the UK identified the need for a more detailed study of the “history of the present” to reveal the discursive operations that construct professional practice. A discursive thematic analysis (DTA) based on the theoretical concepts of the late post-modern scholar Michel Foucault was used to explore public available documents produced by British clinical psychologists between 2010 and 2014. Two dominant professional discursive themes were identified: alternative and leadership. These themes were found to be supported by the discursive sub-themes of applied science, well-being, Cognitivism and therapy which align the aspiration of the profession with those of the State. The tension between the applied scientist and the therapist role - specifically the need to establish simultaneously the profession’s scientific credibility and its therapeutic abilities in order to respond to market pressures – showed recurrences of the conflicts of the early history of professionalization of clinical psychology. The positioning of clinical psychology against the use of functional psychiatric diagnosis and the challenges and opportunities identified by the opening of the NHS market to ‘any willing provider’ revealed how professional discourses operate to maintain the status quo. This study recommends that the socio-historical construction of the profession should be investigated further, in particular through the subjugated discourse identified here.

Changes in the profile of adolescent boys' and young mens' mental health and behaviour has ocurred over the last twenty years, with increases in rates of suicide, parasuicide and conduct disorders. Factors contributing to these changes are unclear but have been theorised by academics within the fields of social psychology, clinical psychology and sociology to be linked to a contemporary 'crisis' in masculinity. This study explored the multiple ways in which five adolescent boys talked about their experiences which had led to their admission to an adolescent unit. The study set up a theoretical framework for researching the ways in which the boys constructed their accounts. It used a combination of narrative, thematic and discourse analytic methods, focusing on tensions within the boys' narratives and how they drew on wider cultural discourses. The main findings suggest that the boys talked about their distress and emotions in multiple and diverse ways which may not be immediately apparent. The positionings they took up within their accounts appeared constrained by influences from cultural discourses, power relations of their immediate and wider social contexts and their life-histories. The research highlights the need for clinicians to be sensitive to issues of gender subjectivities and culture in their work and in the future development of services for young men. Limitations of the study are discussed.

Background: A growing concern within the looked after community is that there is an over-representation of Black, Asian and minority ethnic young people in the care system; the largest group being children from Black and mixed heritage backgrounds. Aim: The purpose of this research is to contribute to our understanding of the processes involved in peer support development for Looked After Children (LAC) from minority communities Method: The study gathered information through face-to-face multi-method, interviews informed by a topic guide. The sample comprised of eight young people, both male and female, with their ages ranged from 11- 16 (mean age = 12.88). Results: The three main themes emerged from the data were: learning about each other, levels of closeness, and a sense of belonging. It also found young people from back and dual heritage background hold fairly realist views of LAC by believing they feel sad, experience stigma, struggle with building trusting relationships but should be treated no differently to others. Discussion: This research has also highlighted the potential process used to inform their inclusive nature towards LAC and how this may be related to prosocial qualities and past experiences that are unique to black and dual heritage young people. Conclusion: This research has extended our knowledge on what the views and perceptions black and dual heritage young people have of LAC and can serve as a preliminary base for future studies.

Inflammatory Bowel Disease (IBD) is a condition causing inflammation in the gastrointestinal tract which can be debilitating for patients, causing bowel/digestive issues and also more systemic symptoms such as fatigue and pain. IBD affects significant numbers of people and is increasing in prevalence worldwide. This thesis aimed to develop a greater understanding of patients’ experiences of living with IBD, with a particular interest in self-conscious emotions. Literature review: Pain manifestation is a common experience in IBD, yet no systematic reviews have been conducted into psychological and social factors associated with pain in this population. The current review examined 16 quantitative studies identified from searching five databases, and identified that pain correlated with quality of life (QOL), psychological distress, specific thinking and coping styles, and family responses to patient communications. However, methodological weaknesses of the included studies precluded firm conclusions from being drawn. Further investigation in this area with more robust study designs (including improved pain measurement) is recommended. Empirical study: The research study investigated experiences of self-conscious emotions in people living with IBD. Fifteen people took part in semi-structured interviews which were analysed using thematic analysis (Braun & Clarke, 2006). Two themes, each with three sub-themes, were developed to interpret participants’ experiences, which included a spectrum of self-conscious emotions including embarrassment, shame, humiliation and guilt. These are discussed from an evolutionary, relational position, with implications for clinicians working therapeutically with this population also outlined. Suggestions for additional study in this area are considered, including further investigation of the consequences of self-conscious emotions for relationships. Critical Appraisal: The critical appraisal details reflections on the research process, including learning points that the researcher intends to take forward in her career.

Aim: To describe medical nurses’ experiences caring for patients and families within an environment with open visiting hours (OVH).
Background: OVH is an approach where families and friends can visit patients without restrictions. OVH is a strategy used by hospitals to promote patient-and-family-centered care (PFCC). In an OVH environment, the increased presence of family can alter nurses’ working environments. However, research examining nurses’ perspectives on OVH is limited. As OVH becomes more widely implemented, it is essential to understand the influence of OVH on the nurse.
Design: A qualitative descriptive approach was used.
Methods: Semi-structured interviews were conducted with 10 registered nurses on two medical units in a large urban Canadian hospital. Thematic analysis was used to analyze the data inductively. Strategies were used to enhance rigour.
Results: Four main themes resulted which described participants’ experiences with OVH as they related to the processes and philosophy of OVH, the care of patients and family, and the influence families had on patient care and nurses working environments. An overarching theme of Reliance and Resistance reflected participants’ mixed feelings toward OVH. While OVH facilitated PFCC and alleviated nurses’ work, at times it also hindered opportunities for nurses to get to know patients and added to their work. Further, increased family presence created challenges related to space, overcrowding, increased noise levels, and created concerns about safety.
Conclusion: The findings add to an understanding of the clinical realities of OVH from the perspectives of medical nurses, and the potential implications for PFCC and nurses’ working environment.
Relevance to clinical practice: OVH creates an environment to foster PFCC. However, nurses may benefit from training to facilitate communication and engagement with families. Organizations who wish to implement OVH should consider how contextual factors may influence nurses’ environments and their practice.

Post-traumatic stress disorder (PTSD) is a common psychological effect of military combat. However, little empirical research has been done to highlight specific complications and needs in treating this population, and even less has been done using the psychedelic drug MDMA. This study used the qualitative research method of thematic analysis to explore the lived experiences of combat veterans engaged in MDMA-assisted psychotherapy for PTSD. This study reviewed video, as well as narrative data from MDMA psychotherapy sessions. Thematic analysis was used to analyze the transcribed audio data. The specific primary aim of the present study was to investigate the following research question: What is the lived experience of combat veterans in MDMA-assisted psychotherapy sessions, and how can this experience be understood through a psychotherapeutic perspective on well-being? Exploration of psychoanalytic and transpersonal aspects of the psychotherapy process are highlighted in their facilitation of healing in combat veterans.

Parmi ces méthodes, la classification supervisée a fait l'objet d'une attention particulière, ce qui a conduit à la mise au point de méthodes de classification efficaces. Ces méthodes visent à déduire la classe de chaque observation en se basant sur une nomenclature de classes prédéfinie et en exploitant un ensemble d'observations étiquetées par des experts. Grâce aux importants efforts de recherche de la communauté, les méthodes de classification sont devenues très précises. Néanmoins, les résultats d'une classification restent une interprétation haut-niveau de la scène observée puisque toutes les informations contenues dans une observation sont résumées en une unique classe. Contrairement aux méthodes de classification, les méthodes d'apprentissage de représentation sont fondées sur une modélisation des données et conçues spécialement pour traiter des données de grande dimension afin d'en extraire des variables latentes pertinentes. En utilisant une modélisation basée sur la physique des observations, ces méthodes permettent à l'utilisateur d'extraire des variables très riches de sens et d'obtenir une interprétation très fine de l'image considérée. L'objectif principal de cette thèse est de développer un cadre unifié pour l'apprentissage de représentation et la classification. Au vu de la complémentarité des deux méthodes, le problème est envisagé à travers une modélisation hiérarchique. L'approche par apprentissage de représentation est utilisée pour construire un modèle bas-niveau des données alors que la classification, qui peut être considérée comme une interprétation haut-niveau des données, est utilisée pour incorporer les informations supervisées. Les modèles proposés sont ensuite testés dans le contexte particulier de l'imagerie hyperspectrale où la tâche d'apprentissage de représentation est spécifiée sous la forme d'un problème de démélange spectralNumerous frameworks have been developed in order to analyze the increasing amount of available image data. Among those methods, supervised classification has received considerable attention leading to the development of state-of-the-art classification methods. These methods aim at inferring the class of each observation given a specific class nomenclature by exploiting a set of labeled observations. Thanks to extensive research efforts of the community, classification methods have become very efficient. Nevertheless, the results of a classification remains a highlevel interpretation of the scene since it only gives a single class to summarize all information in a given pixel. Contrary to classification methods, representation learning methods are model-based approaches designed especially to handle high-dimensional data and extract meaningful latent variables. By using physic-based models, these methods allow the user to extract very meaningful variables and get a very detailed interpretation of the considered image. The main objective of this thesis is to develop a unified framework for classification and representation learning. These two methods provide complementary approaches allowing to address the problem using a hierarchical modeling approach. The representation learning approach is used to build a low-level model of the data whereas classification is used to incorporate supervised information and may be seen as a high-level interpretation of the data. Two different paradigms, namely Bayesian models and optimization approaches, are explored to set up this hierarchical model. The proposed models are then tested in the specific context of hyperspectral imaging where the representation learning task is specified as a spectral unmixing problem

This study expands current crisis communication research by exploring the communication of instructing information via Twitter. Drawing from the internalization, explanation, and action components of Sellnow & Sellnow's (2013) IDEA Model, this study analyzes live tweets posted by the Boston Police Department during the 2013 Boston Marathon bombing crisis. Examining Tweets posted during a crisis allows us to better understand what constitutes useful, valuable instructing information that can be communicated via social media in real time. Further, scholars have just begun exploring social media's implications for crisis communication. This study extends the IDEA Model to reach crisis communication and social media. Findings also indicate the three components of the IDEA Model are valuable topics to consider when communicating instructing information via Twitter. Finally, we learn that Twitter's 140 character limit does not impede the social media platform as a vehicle to communicate instructing information during a crisis.

People around the globe rely on their mobile devices for instant access to entertainment and social media. Children comprise a large majority of individuals who use smartphone applications, particularly for gaming and learning opportunities. Understandably, these apps become part of the identity development process, including the formation of one's gender identity. App developers include gendered content to capture and maintain children's attention, but much of the existing research examines children in late childhood and early adolescence, leaving the ages of six to eight relatively undiscussed. The researcher utilized a thematic analysis to review 20 children's mobile applications for instances of gender stereotypes. Social cognitive theory offers a guiding principle for understanding the process of developing one's gender identity, as well as the role that external stimuli, such as digital media examples and parent models, play. This study aimed to determine whether mobile applications targeting the identified age group use gender stereotypes, as well as how they employ these stereotypes within the application. The researcher randomly selected 20 top children's applications on the Apple App Store and examined them for gendered instances, such as occupations and interests as well as character depictions. The results reflect that instances of gender stereotypes do occur in the children's mobile applications. Many of the applications portrayed feminine stereotypes surrounding nurturing and caregiving tasks ("Mommy in Training"), making it one of the most frequently exploited feminine stereotypes in the sample. The "Boys will be Boys" stereotype comprised the most frequently displayed masculine stereotypes across the studied applications. These findings represent the idea that society places higher value on these stereotypes than others, such as social relationships ("The Power of Motivational Friendship") or recklessness ("The Risk Taker"). Implications include modeling of traditionally masculine and feminine stereotypes for young users by utilizing popular characters recognizable by most children in the target age range.Master of Arts
Handheld electronic devices, such as smartphones and tablets, encompass some of the most widely used electronic devices in today's society. Most families in America have at least one mobile device with internet capability. Apple, the manufacturer of perhaps the most popular brand of electronic devices, pre-install their App Store on all devices they sell, giving users instant access to hundreds of thousands of different mobile applications that offer functions to make every aspect of life simpler. Young children spend a significant amount of their time playing games on these devices, although the American Academy of Pediatrics (2017) recommends that parents limit their children's daily screen time to no more than 2 hours, depending on the age of the child. The games that children download and play impose a number of different messages and stereotypes on their users, including gender stereotypes. Due to the substantial time children dedicate to these apps, the messages communicated regarding gender play crucial roles in the development of their gender identity. Social cognitive theory offers valuable insight and guidance into the gender identity development process. Therefore, the present study examines the gender stereotypes conveyed within 20 of the top children's mobile applications available on the Apple App Store in April 2018.
