Testo completoAdvances in pediatric oncology have improved the prognosis of patients treated for childhood or adolescent cancer, with a 5-year survival rate of over 80%. However, these patients are at increased risk of late following cancer treatment. Cardiac diseases are a major concern in this population of survivors who have been exposed to cardiotoxic treatments such as anthracyclines and/or radiation to the heart. The ability of models to predict cardiac diseases from clinical and treatment factors is still insufficient. In similarly treated survivors, interindividual variability in cardiotoxicity susceptibility has been observed, suggesting the existence of genetic factors modulating this susceptibility. In a first part, we studied the risks associated with cardiotoxic treatments on the. occurrence of late cardiac diseases in children treated for cancer within the French Childhood Cancer Study cohort. We highlighted the role of low-volume cardiac irradiation and the interaction between cardiac irradiation and anthracycline administration. In the second part, we report on a systematic of the literature on genetic factors associated with cardiac diseases in survivors of childhood cancer. In the third part, a matched case-control study nested in the FCCSS cohort was conducted to identify transcriptome biomarkers associated with cardiac diseases and heart failure in particular. The NFE2L2 gene was identified, with high expression associated with a reduced risk of these events. In conclusion, the work of this thesis contributes to a better understanding of the factors impacting the risk of late cardiac complications in children treated for cancer. Once validated, our findings could be incorporated into current clinical practice to adapt the therapeutic management of children with cancer, as well as recommendations for their long-term follow-up

Testo completoChildhood cancer survivors (CCS) may require lifelong medical care due to the late effects of cancer treatments. However, little is known about their healthcare utilization and expenditures at long-term, especially in publicly funded health care system. This thesis aimed to estimate and analyse the long-term health care expenditures and hospitalisation rates among CCS in France and to study their associated factors.The research presented in this thesis was based on data from the French Childhood Cancer Survivors Study (FCCSS), which is a retrospective cohort of 7,670 5-year CCS diagnosed before the age of 21 years between 1946 and 2000 in France. The FCCSS is linked to the National Health Data System (SNDS), which contains comprehensive data on billing and reimbursement of the beneficiaries' health care expenditures since 2006.We found that the annual mean healthcare expenditures among CCS was € 4,255 (SD: 18,790). Expenditures on hospitalization and pharmacy represented ~60% of total expenditures. Our results also showed that female gender and being survivor of a central nervous system (CNS) tumour were associated with increased health care expenditures. We then estimated hospitalisation rates among CCS and detailed the hospitalisation-related clinical diagnoses compared with those of the French general population with the same gender, age and region of residence. We found that survivors were hospitalised more than twice as often as the matched reference population and they had more than three times as many in-patient bed-days. Our results showed that the highest risk for hospitalisation in CCS was related to neoplasm, followed by endocrine diseases, blood diseases, and diseases of the circulatory system. Finally, we compared the health care expenditures of the FCCSS with that of the general French population mentioned above and found that the annual mean excess healthcare expenditures for CCS compared with the median of the reference population was € 3,920 (95% CI: 3539 - 4301). A higher excess was significantly associated with having been treated before the 1990s and having survived a CNS tumour, whereas a lower excess was associated with not receiving treatment with radiotherapy.In conclusion, this thesis showed that CCS are at higher risk of health care use and expenditure, leading to considerable excess compared with the median health care expenditure of the general population of the same sex and age. This high expenditure was related to high hospitalisation rates among CCS. Older survivors, CNS tumours survivors, and those treated with radiotherapy had a higher risk of health service use and expenditure. These results led us to recommend that special attention be paid to this population, especially in terms of follow-up and prevention of complications, and to support the development of effective innovative treatments

Testo completoTherapeutic advances in pediatric oncology have improved survival rates reaching over 80%. In order to increase cure rates and decrease complications and treatment sequelae, international collaborative efforts led to the development of therapeutic trials stratified on major prognostic factors including biological factors. However, treatment adaptation to individual patient characteristics remains marginal.In this thesis, our objective was to better understand how somatic (tumor-related) and constitutional (patient-related) characteristics could modify efficacy and toxicity of anticancer therapies used in pediatric oncology. Several works were performed: 1- Prognostic analysis of tumor markers: assessment of the alpha-foetoprotéine (AFP) decline prognostic value in childhood malignant germ cell tumors; 2- Prognostic analysis of constitutional factors: (i) assessment of the interaction between gender and type of alkylating agents on efficacy and acute toxicity; (ii) assessment of the efficacy and toxicity impact of genetic polymorphisms in patients with osteosarcoma treated with high-dose methotrexate; 3- Risk factors analysis of long-term toxicities: analysis of severe ototoxicity in the French Childhood Cancer Survivors Study (FCCSS)

Testo completoIn the last years, the field of peadiatric oncology has experienced significant medical improvements as well as the granting of a larger autonomy to sick children. This evolution is related to the movement of children rights, but also to the influence of psychoanalysis on nursing and medical staff and to a greater concern about ethical questions raised by the inclusion of children in clinical trials. Thus, the biomedical discourse seems to mirror the evolution of the children position in the society. This doctorate dissertation questions the place of the sick child at the hospital. The dissertation aims to follow the child progression in his path as a patient, guided by the search of his own speech. This work relies on a field study in a child-oncology unit in Marseille as well as on a specific work with former patients and the analysis of sick children'mother blogs. It is first a testimony of the harsh situations faced by children, their parents and the staff. Thanks to an interdisciplinary approach, this work has reached a dimension of clinical anthropology, having effects by itself. Through transference, this work could disclose unconscious or repressed datas to those players who took part in the study. As a conclusion, the dissertation raises some methodological questions. Does the double competence, being a psychoanalyst as well as an antrhopologist, imply a specific bond to the study field? This work aims to demonstrate that the deepening of dyadic relationships with the subjects of the ethnographist is not an obstacle to the triangulation of the information and to the comprehension of interactions

Testo completoThis research stems from a ten-year experience of Adapted Physical Activities with children and teenagers who were hospitalised in an hemato-oncology ward. This research aims at describing and theorising the impact of ludic adapted physical activities on children, teenagers, the medical staff, and the members of the APA team. The unconscious aspects of the relationships between these intervenants have been studied, where individuals responded in their own way to the difficult time they had to go through. The research emphasises the importance, complexity and particularity of the pedagogical and human relationships - all the more because the APA are proposed to patients whose body also showed signs of suffering from a lethal disease. I attempted to define the nature of the necessary transference between the young patient and the person who engaged him in practising the APA. This was achieved in a constant to and fro between on-the-field practice and analysis of psychanalytical theory. The assumption was that it was necessary to analyse the APA sessions and their effects in light of the transference that had emerged between the patient and the APA teacher. The member of the APA team will then use his unconscious knowledge of this one to one relationship to adapt to the patient, and by this broaden and modify his/her theoretical approach to pedagogy and research
