At the present time, the tourist destination offers a stimulating laboratory for the experimentation of theoretical models and good practices on the subjects of governance, knowledge management and sustainable competition. Growing interest in the study of this territorial context gains impetus from the new approaches and tools that local administrations are starting to introduce in the phases of implementation and control of local strategies. In this respect, the Tourist Destination Observatory (OTD) represents an important innovation, offering a nerve centre for the aggregation and networking of heterogeneous data scattered over the territory as well as a model for the implementation of permanent approaches to social dialogue as prerequisites for the creation of knowledge and for an aware, shared, competitive and responsible development of the destination. The OTD can act as an efficient agent of local change, facilitating the processes of governance, and as a tool of knowledge management for the valorisation of intellectual capital. It is consequently a crucial support for the strategic repositioning of mountain resorts, which can represent valid responses to the emerging new modes of interpreting the holiday.

Extracorporeal membrane oxygenation (ECMO) can provide support as a bridge to recovery or a bridge to more definitive therapy for patients with severe respiratory or cardiorespiratory disease. In this chapter, the criteria for ECMO are discussed and a practical decision tree for mode of ECMO (venovenous or venoarterial) is presented. A stepwise approach to initiation and management of ECMO for the patient is described, including flow rate goals, ventilator management, anticoagulation, blood product replacement, identification of recovery, weaning procedures, and specific issues relating to the two different modes of ECMO.

This chapter includes sections on various modes of both invasive (i.e. via an endotracheal tube) and non-invasive respiratory support in neonates, including conventional ventilation, volume-targeted ventilation, high-frequency oscillatory ventilation (HFOV), extracorporeal membrane oxygenation (ECMO), nasal continuous positive airways pressure (nCPAP), nasal intermittent positive pressure ventilation (nIPPV), and high and low-flow nasal cannula oxygen. There is also a brief section on the care of babies with a tracheostomy as well as management of babies requiring home oxygen. Reference is made to the most recent European Consensus Guidelines. A separate chapter on neonatal respiratory problems (Chapter 7) gives further detail on common lung pathologies requiring respiratory support in neonates.

‘Mechanical Ventilation Modes’ seeks to shed light on this hotly debated topic, one that is complicated by ventilator manufacturers’ non-standardized terminology. The chapter looks at conventional modes, adaptive modes, and biphasic modes, which it classifies based on the mechanical breath types in each mode. It includes a comparison chart of the terminology used for common modes on popular IPPV ventilators. Using their signature waveforms, the author describes the assist/control, SIMV, and pressure support ventilation or PSV modes. It defines the modes by their application of spontaneous breaths and mandatory breaths. It continues with a discussion of adaptive modes and biphasic modes. It ends by discussing how to select the appropriate ventilation mode.

The chapter first covers the various ventilator modes: continuous mandatory ventilation, synchronized intermittent mandatory ventilation, spontaneous ventilation, and bi-level ventilation. Then it turns to a discussion of the various mechanical breath types: volume control, pressure control, adaptive pressure control, pressure support, and finally, adaptive pressure support. It also provides practical advice about how and when to use each mode–breath type combination. Eight Boxes in the chapter discuss each breath type, and list each type’s features, and its clinician-set parameters.

Intergroup contact, encounters, and dialogues are pervasively used in settings of protracted, ethnopolitical conflict as a device for improving relations between the sides and promoting conflict resolution and reconciliation. This chapter reviews the theoretical underpinnings of such efforts and discusses different models and modes of planned intergroup contact in conflict, as well as the potential of the intergroup encounters to bring about change, while focusing on the ethical implications and consequences of the encounter with the other and its narrative in the setting of protracted ethnopolitical conflict. Specifically, it focuses on the case study of the violent protracted asymmetric conflict between Israelis and Palestinians. In this context it discusses the extent to which different modes and models of organized intergroups can overcome moral exclusion, extend the boundaries of moral responsibility for the other, and increase support for more socially just and equitable relations.
