Abstract (sommario):
BackgroundSuicide is a global concern to nurses and other health-care providers. However, deaths by suicide are only part of the spectrum of suicide, as suicidal thinking and behaviors may precede a suicide attempt. Theoretical models are used infrequently in research to explain how the individual progresses from suicidal ideation to a suicide attempt. Thus, there is a critical need to advance the study of suicide with useful theoretical models to describe and explain processes whereby suicidal thoughts transition to attempts and to suicide.PurposeThis article provides a conceptual discussion and scoping review comparing historical and contemporary ideation-to-action theories of suicide.MethodSystematic reviews and meta-analyses from three databases (CINAHL, JSTOR, and PsychINFO) were reviewed to find literature describing suicide theories.ResultsHistorically, theory applications have limited capacity to differentiate between those individuals with suicidal ideations and those who attempt suicide. Newer theories, grounded in the ideation-to-action framework, propose distinct processes explaining what moves an individual from suicidal ideations toward suicidal behaviors.Implications for PracticeThe ideation-to-action theories can guide health-care providers' assessment of at-risk individuals beyond merely asking about suicidal thinking.ConclusionThe new generation of suicide theories suggest that suicidal ideations are only one component of risk. The common factor in ideation-to-action theories that distinguishes ideators from attempters is the acquired capability for suicide.

Abstract (sommario):
Systematic review of the relationship between post-traumatic symptomatology and suicidal behavior Abstract: Evidence has consistently demonstrated the relationship between post-traumatic stress disorder (PTSD) and suicide. However, there is little research related to the differential relationships between the PTSD symptom clusters (avoidance, re-experiencing, hyperarousal and negative alterations in cognitions and mood) and risk and suicidal behavior. The present systematic review studies the relationship between these post-traumatic symptom clusters and suicidal behavior (suicidal ideation and suicide attempts). The review includes 20 articles from the databases PsycINFO, PsycARTICLES and Web of Science. Results indicate that hyperarousal significantly predicts suicidal ideation in 53.33% and suicide attempts in 42.85% of the studies reviewed. Reduction of hyperarousal symptoms should be considered a priority objective in the development of therapeutic interventions to reduce suicide risk in people with post-traumatic conditions or diagnosed with PTSD. Key words: PTSD; hyperarousal symptoms; suicidal ideation; suicide attempt; systematic review.

Abstract (sommario):
Abstract: Risk and protection variables related to suicidal attempt. Suicide is an important public health problem, being the suicidal attempt the most predictive behavior of completed suicide. The aim of this study was to detect if there are differences in psychosocial and emotional variables in people with and without suicidal ideation and attempt. The sample consisted of 166 participants (86.36% women), aged between 20 and 77 years (M= 36, SD= 14.12) with and without suicide attempts, which was in turn divided into three groups through the Scale of Suicidal Ideation. The results show that there are important differences between the three groups in the psychological variables measured. We discuss the role of psychosocial variables, which are at the base of the increased risk or protection towards the ideation or suicidal attempt, to promote public suicide prevention policies more focused on those clinical subpopulations with specific risk profiles.

Abstract (sommario):
Background: Suicide is a major threat to public health worldwide. Evidence suggests alcohol use disorders (AUD) are associated with suicide ideation. There is a paucity of studies in India regarding suicidal ideation among individuals receiving in-patient treatment for alcohol dependence. Aim: To assess the suicidal ideation and its severity among persons with alcohol use disorder. Methods: Cross-sectional research design was used. Totally 47 persons with alcohol use disorder receiving in-patient treatment were screened for suicidal ideation using a consecutive sampling technique based on inclusion and exclusion criteria at centre for addiction medicine, tertiary care teaching hospital at Bangalore. An interview schedule was used to collect the data. Tools: Mini+ suicidality was used for screening suicidal ideation and Columbia suicide severity rating scale was used to assess the frequency and severity of the suicidal ideation. Descriptive statistics such as frequency, percentage, mean were used for data analysis. Results & Discussion: Out of 47 patients, 29 reported having suicidal ideations (62%). AUD suicidal ideation was more among persons belong to below poverty line, lower education level, unemployed, married, living with family of origin, nuclear family, urban, using tobacco. Their mean age was 35 years (±6.6). Majority (62%) of them had suicidal ideation in the past one month, 43% reported having thought to kill themselves over the past one month. One-third (35%) able to control suicidal thought with lot of difficulty, 83% reported that deterrents stopped them attempting suicide, 82%wanted to end their life to end or stop the pain that they are enduring. Conclusion: The prevalence of suicidal ideations is high among persons with alcohol use disorder. Socio-demographic factors likely to have an influence on suicidal ideation among persons with alcohol use disorder.

Abstract (sommario):
Objective: Suicide rates among psychiatric inpatients are higher than in the general population. Mental health professionals are poor at predicting suicide risk. Method: One hundred and three inpatient suicides, over a 21-year period, in a large psychiatric hospital in Melbourne were examined. The characteristics, including demographic and clinical data, for 60 of these suicides were compared with a matched comparison group stratified for the time of admission. Violent methods (including jumping in front of trains, trams and road traffic, jumping off buildings, hanging and drowning) were most frequently used. Results: Over one-third of the suicides occurred during periods of approved leave and a similar number occurred after absconding from the hospital. Suicide was associated with a diagnosis of schizophrenia, increased duration of admission, more frequent prescription of neuroleptics and antidepressants, previous deliberate self-harm, suicidal ideation at the time of admission and during the admission, suicide attempts during the admission, unstable (fluctuating) suicidal ideation during the admission and a greater number of ward transfers. Conclusions: Psychiatric units should be developed away from readily available methods of suicide. Patients with previous or intra-admission suicidal ideation or attempts, or unstable suicidal ideation, should be carefully observed to avoid absconding. Suicide risk should be carefully evaluated in such patients prior to approving periods of leave.

Abstract (sommario):
BackgroundThe burden of mental health impairment in medical students is an important research subjects. Evidence shows that medical students have a higher prevalence of symptoms of depression and anxiety than the general population. A number of studies regarding suicidal ideation among medical students have been carried out, but this topic has largely been ignored in regard to international medical students coming from Western European countries studying medicine in Eastern European countries. Suicidal ideation is the early symptom of suicidal actions. The students are the most vulnerable and easily influenced by cases of suicide, especially in a closed group with little connections with the general population like the international students.ObjectivesFollowing 2 suicides in our university, we aim to measure the prevalence of suicidal ideations in international students.MethodsOverall, 150 medical students from the French section studying at the University of Medicine and Pharmacy “Iuliu Hatieganu”, Romania, were evaluated for symptoms of depression using BDI and for suicide ideation using Beck HS.ResultsSuicidal ideation was present in 28.57% of the French international medical students. Depression and the lack of social support had a significant relationship with suicidal ideation.ConclusionsSuicide is a serious issue that has to be dealt with immediately. It is important to assist medical students who are considering the possibility of committing suicide, so that they overcome their difficulties. There is a need to identify the prevalence of suicidal ideation among medical students and for student counselling.Disclosure of interestThe authors have not supplied their declaration of competing interest.

Abstract (sommario):
Previous studies have suggested that stereotypes towards suicide, including endorsement of suicide and stigma toward suicide, may contribute to suicidal ideation and behaviors. However, this has not been examined directly. In this study, we examined whether endorsement of suicide and stigma toward suicide are involved in the pathway from suicidal ideation to suicide attempts among college students. To this end, we used the Suicidal Ideation Attributes Scale (SIDAS), the Suicidal Behaviors Questionnaire-Revised (SBQ-R), and the Stigma of Suicide Scale (SOSS) to assess suicidal ideation, suicide attempts, endorsement of suicide, and stigma toward suicide, respectively, in a sample of 944 Chinese college students (mean age, 20.97 years). Using mediation analysis, we found that suicidal ideation partially mediated the relationship between endorsement of suicide and suicide attempts and between stigma toward suicide and suicide attempts. These findings provide novel evidence that endorsement of suicide and stigma toward suicide are closely associated with suicide attempts, but partially through the influences of suicidal ideation. Future studies should elaborate on their longitudinal relationships. Implications of these findings for clinical practices are discussed with reference to the ideation-to-action framework of suicide, aiming to reduce suicidal behaviors.

Abstract (sommario):
SummaryIn a study of the general population, Weissman et al (1989), found that panic disorder is associated with an increased risk of suicide attempt. We estimated the lifetime prevalence of suicidal ideations or suicide attempts in 100 consecutive referrals for treatment of panic disorder. Sixty per cent of these patients had had suicidal ideation and 42% had attempted suicide during their lifetime. With regard to the season of onset of panic attack, we found a very skewed distribution of first panic in late spring or summer. Furthermore, patients who had their first attack in spring or summer had a higher rate of suicidal ideation and an increased risk of suicide attempt.

Abstract (sommario):
Background. Suicidal ideation is considered a major health problem associated with HIV/AIDS. Suicide rates among people living with HIV/AIDS (PLHA) are more than three times higher in the general population and that is a significant difference. This study aimed at investigating the related factors of suicidal ideations among HIV-positive patients in Southwest Iran. Study Design. A cross-sectional study. Methods. 351 adult volunteer HIV-infected patients that referred to the Voluntary Counseling and Testing (VCT) center in the south of Iran were evaluated based on convenience sampling. Data was collected utilizing a structured questionnaire from March to August 2015. Results. Over the six months prior to the study, 15.4% of the entire sample had been diagnosed with suicidal ideation. There was a significant correlation between the quality of life, spiritual beliefs, perceived stigma, and age with suicidal ideation. Suicidal ideation is significantly different in terms of gender and marital status. Perceived stigma and spiritual beliefs showed the highest effect on suicidal ideations, respectively. Conclusions. Having religious beliefs due to accelerating psychological adaptation can motivate HIV patients to survive and have also been considered effective in preventing women from suicide. Perceived stigma and quality of life are other factors that should be taken into consideration as key elements in suicide preventive programs.

Abstract (sommario):
Exposure to suicide in social relationships is known to affect suicide survivors’ suicide ideation, and high-risk drinking is known to be a risk factor in increasing suicidal ideation. By utilizing data from 1,385 Korean adults who responded to “A Longitudinal Study on the Mental Health of Suicide Survivors Who Experienced Suicide in Social Relationships,” we examined the association between experiences exposed to suicide and suicidal ideation and its interaction effect of high-risk drinking on suicide ideation. The study participants who answered that they had been exposed to suicide in social relationships was 22.4%, and the interaction effect between exposure to suicide and high-risk drinking was found to be statistically significant. Therefore, high-risk drinking was confirmed as a risk factor for negatively affecting the relationship between exposure to suicide and suicidal ideation. Based on these results, we suggest clinical and policy interventions to reduce risky behaviors among suicide survivors.

Abstract (sommario):
Suicide is one of the most tragic issues in mental health. Suicide has traditionally been studied as an outcome of specific psychiatric disorders. The goal of this study was to consider whether there might be underlying dimensions across psychiatric disorders that are related to suicidality. This study proposed a transdiagnostic model of suicidality. Psychiatric symptoms were hypothesized to load onto three factors: Negative Mood, Impulsivity, and Arousal. Factors were then examined as predictors of suicidal ideation and suicide attempts. Data were drawn from the National Comorbidity Survey-Replication Study (NCS-R). Structured interviews were conducted in two parts using the Composite International Diagnostic Interview (CIDI). All survey respondents (N = 9,282) were administered the core diagnostic assessment. Those who met criteria for a psychiatric disorder, met subthreshold lifetime criteria and sought treatment at some time in their life, or ever in their life made a plan to commit or attempt suicide were administered Part II of the interview and are the sample of interest in this study (N = 5,692). The sample was representative of non-institutionalized civilian adults ages 18 or older whose primary language was English. Factor analyses revealed three modified factors: a Negative Mood/Emotional Arousal factor, a Negative Thinking factor, and a Recurrent Substance Use factor. Negative Mood/Emotional Arousal was a strong predictor of suicidal ideation. Suicidal ideation mediated the relationship between Negative Mood/Emotional Arousal and suicide attempts, controlling for Negative Thinking. Negative Thinking, Recurrent Substance Use, and suicidal ideation predicted suicide attempts. When number of suicide attempts was examined as the dependent variable, the model did not fit the data, suggesting that these factors were not as helpful in predicting highly recurrent suicide attempts. Recurrent Substance Use moderated the relationship between Negative Mood/Emotional Arousal and suicide attempts, demonstrating that, as negative mood increases, people with high levels of recurrent substance use are more likely to make a suicide attempt compared to people with mean or low levels of recurrent substance use. In sum, results of this study support two distinct pathways to making a suicide attempt: a direct relationship between negative thinking and suicide attempts, as well as an interaction between negative mood/emotional arousal and recurrent substance use. Several limitations including issues of sampling, the cross-sectional nature of the data, self-report bias, and the structure of the CIDI instrument, were taken into account in interpreting the results. Clinical implications and future directions for research are discussed.

Abstract (sommario):

Suicide is a significant public health problem. It is among the top three chief causes of death for young people, ages 10-24 years (CDC, 2016). The majority of studies on suicide focus on what causes suicidal thoughts, suicidal attempts, and completed suicides (e.g., Dougherty, 2010; Shneidman, 1993; Van Orden et al., 2010). While these studies are crucial, it is also vital to understand why and how people survive and thrive. Focusing on adolescent suicide prevention through the lens of resiliency, the research questions are as follows: Why and how did suicidal adolescents decide against suicide?

Thirty-two participants (16 males and 16 females) from diverse ethnic and socioeconomic backgrounds, ages 21 to 50 years, completed an anonymous online self-report questionnaire. The data were analyzed, using Braun and Clarke's (2006) thematic analysis. The results of the analysis identified four areas in which suicidal adolescents find resiliency from suicidal thinking: social support and professional help, positive life event or circumstance, responsibility and or purpose, and religious and/or spiritual experience. Drawing from the linkages of the themes, the adolescent suicidal ideation theory was developed and explained.

The theoretical explanation, the first of its kind in regard to understanding why and how suicidal youths decide against suicide, is an important contribution in the fields of psychology and adolescent suicide prevention. It also serves to inform mental health professionals, educators, parents, adolescents, and children. Implications in the research highlight the buffering effects of personal connection (i.e., social and professional help), positive emotions, responsibility and purpose, and a religious and/or spiritual foundation in fostering suicidal ideation resilience. Suggested recommendations for policymakers include developing comprehensive training classes to promote healthy families and effective parenting; promoting age appropriate coping and problem-solving skills and resilience strategies to children and adolescents, starting a kindergarten through the twelfth grade (K-12), and six other recommendations are outlined in the discussion section of the paper. Further research could be conducted to strengthen the evidence-base of humanistic, existential, transpersonal, integrative, and trauma-informed therapies to help depressed and suicidal young people recover and acquire positive mental emotional health and wellness.

Abstract (sommario):
Background. There are few cross-national comparisons of the rates of suicide ideation and attempts across diverse countries. Nine independently conducted epidemiological surveys using similar diagnostic assessment and criteria provided an opportunity to obtain that data. Methods. Suicide ideation and attempts were assessed on the Diagnostic Interview Schedule in over 40000 subjects drawn from the United States, Canada, Puerto Rico, France, West Germany, Lebanon, Taiwan, Korea and New Zealand. Results. The lifetime prevalence rates/100 for suicide ideation ranged from 2·09 (Beirut) to 18·51 (Christchurch, New Zealand). Lifetime prevalence rates/100 for suicide attempts ranged from 0·72 (Beirut) to 5·93 (Puerto Rico). Females as compared to males had only marginally higher rates of suicidal ideation in most countries, reaching a two-fold increase in Taiwan. Females as compared to males had more consistently higher rates for suicide attempts, reaching a two- to three-fold increase in most countries. Suicide ideation and attempts in most countries were associated with being currently divorced/separated as compared to currently married. Conclusions. While the rates of suicide ideation varied widely by country, the rates of suicide attempts were more consistent across most countries. The variations were only partly explained by variation in rates of psychiatric disorders, divorce or separation among countries and are probably due to cultural features that we do not, as yet, understand.

Abstract (sommario):
The present study attempted to identify demographic and psychological variables predictive of adolescent suicidal ideations and attempts. Data from 90 adolescents, who completed an intake form at a university community mental health clinic or were student volunteers, were utilized. Two judges tabulated information regarding age, gender, number of siblings, ethnicity, parental marital status, drug use, depression, suicidal ideation, and previous suicidal attempts. A multiple regression analysis was performed to identify which variables had predictive significance. Depression was the best predictor of both suicidal ideations and attempts. Ethnicity was also predictive; white adolescents were found to be at higher risk for suicide than individuals from other ethnic groups. It is suggested that additional studies be done exploring other predictors of suicide among adolescents.

A history of past suicide attempts is a risk factor for future suicide, and risk is increased by more serious, more frequent, or more recent attempts. Over 90% of individuals who die by suicide have at least one psychiatric disorder. Patients with schizophrenia, alcohol and other substance use disorders, and borderline and antisocial personality disorders are at increased risk for suicide. Familial transmission of suicide risk appears to occur independent of the familial risk for psychiatric disorders; impulsivity seems to be an inherited trait that makes individuals more vulnerable to suicide. Hospitalization should be considered if suicidal ideation is present in a patient who is psychotic or who has a history of past attempts, particularly if near lethal, and may be the safest option in patients with other contributing medical conditions, limited family or social support, or lack of access to timely outpatient follow-up.

Abstract (sommario):
Suicide is a significant concern for clinicians working with clients experiencing major depressive disorder (MDD). Previous research has indicated that MDD is the diagnosis more frequently associated with suicide, with approximately two-thirds of those who die by suicide suffering from depression at the time of death by suicide. This chapter reviews data regarding the prevalence of suicidal behavior among those with depressive disorders. It then reviews risk factors for suicide ideation, self-injury, and death by suicide. Finally, the chapter provides an empirical overview of treatment studies aimed at decreasing risk for suicide, as well as an overview of several recent treatment approaches showing promise in the reduction of suicidal behavior.

Abstract (sommario):
According to the World Health Organization, every 40s a person dies of suicide in the world. Among young people aged 15 to 29, suicide is the second largest cause of death. Yet such deaths can be prevented. In this scenario, social networks like Twitter can become sources of information in real time and help in suicide prevention. The present work makes an initial exploration of the problem of identifying individuals at risk of suicide in social networks in Portuguese Language. As a main result, a manually labeled dataset of tweets has been constructed that can be used in future research on the subject.

Abstract (sommario):
Approximately 40 million adults use marijuana annually, making marijuana one of the most commonly used substances in the United States (SAMHSA, 2019). Men and emerging adults (ages 18-25) report higher prevalence of cannabis use disorders (CUDs) relative to women and older adults (CBHSQ, 2015; Khan et al., 2013). More frequent marijuana use is associated with greater likelihood of suicidal ideation (Ilgen et al., 2009), and past year use in emerging adults is associated with future suicide attempts (Pedersen, 2008). Similar to correlates of marijuana use, emerging adults and men have higher rates of suicidality (SAMHSA, 2019; Krug et al., 2002). Limited research has tested gender and age differences in comorbid CUDs and suicidality. The current study evaluated gender and age differences in CUDs only, suicidality only, or comorbid CUDs and suicidality in a national sample of adults. We hypothesized that men and emerging adults would be over-represented in comorbid CUDs and suicidality and CUDs only groups. Data were from four consecutive years (2015-2018) of the National Survey of Drug Use and Heath. Multinomial logistic regressions tested gender and age differences in adults with DSM-IV cannabis abuse or dependence (CUDs) only, suicidality only, and comorbid CUDs and suicidality, all compared to adults with neither CUDs or suicidality. Four separate regressions were conducted for passive suicidal ideation, active suicidal ideation, suicide planning, and suicide attempts. Gender was coded as male or female. Age groups were 18–25, 26–34, 35–49, and 50 years or older. Analyses controlled for survey year, race/ethnicity, sexual orientation, education, household income, past year major depressive episode, past year DSM-IV alcohol abuse or dependence, and past year illicit drug abuse or dependence other than CUDs. Men disproportionately reported CUDs only (ORs=1.73-2.19, p<.001) and comorbid CUDs and passive suicidal ideation, active suicidal ideation, and suicide planning (ORs=1.72-2.12, p<.01), but not attempts (OR=1.16, p=.45) relative to women. Men reported 22% higher odds of active suicidal ideation than women. Women reported 15% higher odds of suicide attempts than men. Gender differences in passive suicidal ideation and planning were not statistically significant. Compared to older age groups, emerging adults were significantly more likely to report CUDs only (ORs=1.74-10.49, p’s<.01) and showed 2.36 to 14.24 times greater odds of comorbid CUDs and all four forms of suicidality (p<.001). Emerging adults were at 18% to 66% higher odds of either passive or active suicidal ideation alone compared to all older age groups (p’s<.001). This study investigated the relations between CUDs, suicidality, gender, and age in a nationally representative sample of adults. Results indicated that men and emerging adults consistently reported the highest likelihood of negative outcomes. Next steps include determining the direction of the relationship between CUDs and increasing severity of suicidality. Further, development and investment in programs for emerging adults with CUDs and suicidality are vitally important given the striking risk profile compared to other age groups. Future research should include program development and evaluation as well as gathering more information on risk and protective factors for these populations.

Abstract (sommario):
Cannabis is the most commonly used substance that remains federally illegal in the United States. With its rising legality in many states across the nation, it is important to understand the influence that cannabis can have on a user's physical and mental well-being. According to the Centers for Disease Control and Prevention (2022), suicide is among the top nine leading causes of death in the United States for individuals aged 10 to 64. Research suggests that individuals diagnosed with depression who experience suicidal ideation, plans, and attempts is positively correlated with both daily cannabis use and non-daily cannabis use and that this correlation is more significant in women than in men (Han et al., 2021). This study seeks to generalize this finding by investigating both the prevalence and intensity of 30-day cannabis use amongst individuals who endorse suicidal thoughts in the last 12 months and the moderating effect of gender identity on the relationship between 30-day cannabis use and suicidal thoughts in the last 12 months. This study will utilize a sample of college students who completed the American College Health Association-National College Health Assessment Survey sometime between the Fall academic semester of 2019 to the Spring academic semester of 2021. The survey was completed on campuses across the United States with data collected on students’ health habits, behaviors, and perceptions. This data set consists of 198,848 participants aged 18 to 98 years of age (MAge = 23.16), with 66.27% of participants identifying as cisgender women and 30.83% identified as cisgender men. Within the dataset, 40.30% of participants endorsed thinking about or planning suicide in the last 12 months and 19.34% of participants reported using cannabis in the last 30 days. We anticipate that individuals who reported cannabis use in the last 30 days will show an increase in suicidal ideation in the last 12 months compared to individuals who reported never using cannabis. Similarly, we anticipate that gender identity will moderate and strengthen the relation between cannabis use and suicidal ideation in such that individual who identify as women will have a stronger and more significant correlation between cannabis use and suicidal ideations compared to individuals who identify as men. If these results are supported, future research will be warranted to further investigate how the nuanced interaction between gender identity and cannabis use increases our understanding of the upward trend in suicidality among individuals with and without mental health diagnoses.
