Abstract (sommario):
Oral, small-molecule signalling pathway inhibitors, including ones that inhibit the JAK and SyK pathways, are currently in development for the treatment of rheumatoid arthritis (RA). Tofacitinib is an orally administered small-molecule inhibitor that targets the intracellular Janus kinase 3 and 1 (JAK1/3) molecules to a greater extent than JAK2 while baricitinib (formerly INCB028050) predominantly inhibits JAK1/2. Many of the proinflammatory cytokines implicated in the pathogenesis of RA utilize cell signalling that involves the JAK-STAT pathways and therefore inhibition of JAK-STAT signalling, by targeting multiple RA-associated cytokine pathways, has the potential to simultaneously reduce inflammation, cellular activation, and proliferation of key immune cells. Fostamatinib disodium is an orally available inhibitor of spleen tyrosine kinase (SyK), which is a cytoplasmic tyrosine kinase that is an important mediator of immunoreceptor signalling in mast cells, macrophages, neutrophils, and B cells. Interruption of SyK signalling may interrupt production of tumour necrosis factor (TNF) and metalloproteinase and therefore affect RA disease activity. Tofacitinib has been investigated in multiple phase 2 and phase 3 trials which have investigated its efficacy (clinical, functional, and radiographic) and safety in patients who have failed disease-modifying anti-inflammatory drugs (DMARDs) as monotherapy or in combination with DMARDs, compared to an inhibitor of tumour necrosis factor alpha (TNFα‎) and in patients who have failed TNFα‎ inhibitors. The efficacy of fostamatinib and baricitinib has been investigated in phase 2 trials; both are in large phase 3 clinical programmes. Each of these medications has demonstrated efficacy; their safety profile has been shown to be different from each other and from currently approved biological agents. This chapter discusses what is currently known and understood about their efficacy and safety.

Results of the randomized, international INSIGHT START clinical trial provide definitive proof of the benefit of antiretroviral therapy initiation in asymptomatic individuals with CD4+ counts greater than 500 cells/mm3. There are six different classes of antiretroviral agents: two types of reverse transcriptase inhibitors, two types of entry inhibitors, one class of inhibitors of HIV protease, and one class of inhibitors of HIV integrase. Combination antiretroviral therapy is recommended for all people living with HIV. The primary goal of combination antiretroviral therapy is to achieve viral suppression. Each antiretroviral class targets a unique step in the replication cycle of HIV-1.

Humans have evolved an intricate system that maintains blood in a fluid state. This relies on an intact vascular endothelium modulating vascular tone and forming a barrier between blood components and reactive subendothelial components. It also involves the production of inhibitors of both blood coagulation and platelet aggregation. In addition, haemostatic systems are primed to convert blood from its fluid state to a solid state, to allow the formation of a haemostatic plug, following vessel injury, to stem the flow of blood from or within a blood vessel. This chapter reviews the physiology of haemostasis.

The prevention of acute kidney injury (AKI) should start with an assessment of the risk to develop AKI, by identification of co-morbidities, use of potentially nephrotoxic medications, and early recognition of acute reversible risk factors associated with AKI. This chapter discusses first the most relevant general risk factors for AKI and describes the recent introduction of several surveillance systems. In addition, some specific risk factors play a role in the pathogenesis of post-cardiac surgery AKI. Finally risks associated with commonly used drugs such as non-steroidal anti-inflammatory drugs, angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, statins, and warfarin are considered.

It is well known that tax compliance is low in Italy. Many scholars have examined Italian taxpayer behavior, mainly using experiments and surveys. However, little attention has been given to the historical circumstances that have shaped divergent taxpayer behavior in Italy. This chapter uses historical data from Italian unification through the Second Republic to assess the effects of Italy’s major formal institutions (the Church, state, and political parties) and informal institutions (clientelism) on Italian tax behavior. It argues that nineteenth-century unification and Fascism had significant repercussions for the Italian state and how Italians perceive the state. Because of this, Italians lack trust in their government and their fellow citizens, which inhibits a willingness to pay taxes. The implication then is a low-trust/low-compliance equilibrium that becomes increasingly difficult to reverse.

Ankylosing spondylitis (AS) is a chronic inflammatory arthritis affecting mainly the sacroiliac joints and spine, resulting in pain, stiffness, and reduced movement. Over the past decade there have been major advances in many aspects of the disease, including a broadening of the disease description to axial spondyloarthritis (axSpA). While the many advances have transformed the lives of patients with axSpA, they have also increased complexity for non-specialists in this area. This handbook contains a timely update of the key developments and current state of play in axSpA. It is intended primarily for the many healthcare professionals who encounter patients with this condition, in both primary and secondary care settings. It will also be of interest to the wider medical and research community.The handbook is written by rheumatologists with active research programmes and clinical expertise in these conditions. Many changes have occurred over the last three decades in the field of stress ulcer gastrointestinal bleeding and its prevention. The topic is controversial, fuelled by disparate data, studies at risk of bias, and the impression that the problem is not as serious as it once was. Indeed, compared with over four decades ago when mucosal ulceration of the stomach causing serious bleeding was first described, a relatively small proportion of critically-ill patients now develop clinically important bleeding. Acid suppression is commonly prescribed for stress ulcer prophylaxis (SUP), targeting subgroups of patients at high risk in the intensive care unit (ICU), rather than universal prevention. The randomized clinical trials to date suggest a significant reduction in CIB with use of histamine-2-receptor antagonists (H2RAs) compared with no SUP, with no impact on pneumonia, ICU mortality, or length of stay. However, these trials are of moderate quality. More recent RCTs suggest proton pump inhibitors compared with H2RAs may significantly reduce the risk of CIB without influencing the risk of pneumonia, ICU mortality, or length of stay. These trials are also of moderate quality. Today, the decision whether to use SUP, and which agent to use, is complex. Clinical considerations include local epidemiological data (for centres documenting these outcomes), and patient-specific risks of gastrointestinal bleeding and infection, indexed to case mix.

This chapter provides an overview of Somali kinship studies and discusses why such studies have become marginalized within Somali studies. It is argued that cultural intimacy explains the intricacies that shape Somalis’ contestation of clan and the cultural intimacy of clan inhibits scholarship. A practice theory approach to clan is outlined and Bourdieu’s formulation of social capital serves as a foundation for understanding how clan is experienced and employed. The chapter discusses Somalis’ initial movement to Finland as accidental and later movement as a strategy to access benefits of the Finnish welfare state. The Finnish context is discussed, including xenophobia and racism, as well as strategies and challenges of conducting research including views of researchers.

Recent research shows that most fathers today seek roles that are much more integrated and engaged with the lives of their families than fathers in previous generations. However, society in general, and workplaces in particular, have been slow to adapt to these new fatherhood roles. This situation causes increased work–family conflict for fathers and leaves them in a situation in which their aspirations are often inconsistent with their realities. Although these changes are beginning to be noted, existing research in the work–family field has focused much more on the issues women face in integrating work and family than it has on the issues facing men. This predominantly gendered approach to work and family integration has a number of deleterious effects on both men and women, and also inhibits moving this critical conversation forward. This chapter reviews the current state of research on work–family issues for men and suggests areas for additional research.

Chapter 14 does not design a general theory of religious change, but develops a multiple theoretical perspective including various theoretical elements, which are instrumental for explaining religious changes and which can be combined flexibly. In brief, they are: 1. Functional differentiation as a rule stands in a strained relationship to the integrational capacity of religious communities and churches. If, however, religious identities are linked with non-religious, for example, political, national, or economic interests, religion and church are strengthened. 2. Processes of individualization mostly have an erosive impact on religious ties. 3. In contrast to the assumptions of the market model, religious pluralism does not foster religious vitality, but actually inhibits it. In the case of conflict, though, religious diversity can fuel religious passions. Other determining factors such as government regulations of the religious sector, state spending on the welfare system, social inequality, immigration, and path dependency are also taken into account.

In the face of the overall state of affairs in society, which seems to be characterised by crises, (right-wing) populism, uncertainty and radicalisation, the concept of ‘concern’ appears to be a key idea in modern society. The underlying idea behind the discussion compiled in this book is to investigate concern from the perspective of a carefree attitude. Such an approach might provide us with a clearer understanding of the multifaceted and complex phenomenon of concern. Concern can be regarded as the manifestation of a longing for a carefree life without the burden of tedious tasks or demands that are too high. However, concern can also be understood as something active—being carefree through lovingly caring for someone or behaving responsibly. This study’s approach of incorporating the idea of being carefree into the debate about concern makes the ambiguity of this concept clear: concern both inhibits and incentivises action; it is both restrictive and empowering. Fabry disease is a rare X-linked lysosomal storage disorder in which deficiency of alpha-galactosidase A leads to accumulation of substrate, mostly globotriaosylceramide (Gb3), which causes a progressive, multiorgan disease affecting predominantly the kidneys, skin, heart, and nervous system. Painful peripheral (‘acral’) neuropathy is characteristic. Proteinuria and estimated glomerular filtration rate (eGFR) are strongly associated with risk of progression, but this may be reduced by treatment with angiotensin-converting enzyme inhibitors as well as by enzyme replacement therapy (ERT). ERT was approved in 2001; it improves pain and other neuropathic symptoms, and well-being, and has been proven to clear deposits of Gb3 from tissues, at variable speeds. There is limited randomized controlled trial data but protective effects have been proven for renal outcomes, death, and better outcomes in some other organ systems. Renal function may be protected if ERT is commenced before there is heavy proteinuria or substantial loss of GFR. It is recommended to start ERT as soon as the diagnosis is made in those with very low or absent enzyme. For those with intermediate levels it is recommended to commence treatment only when signs or symptoms appear. Proteinuria and eGFR give most information from a renal point of view, but renal biopsy is also useful for confirming the renal diagnosis and staging the disease as well as monitoring progress in selected cases. Management should include regular screening for complications including myocardial and neurological assessments. It is likely that registries will show progressive rises in median survival with this condition.
