Testo completoRevealing the HIV status is not always easy for people living with the HIV (PLHIV). This thesis aims to explore the factors influencing the disclosure of HIV status to sexual partners by heterosexual men Haitians living with HIV/AIDS. Three hospitals were involved in recruiting participants. Eighteen (18) semi-structured interviews were conducted. The main elements that facilitate disclosure of HIV status by participants are the consistent use of condoms, the need for love and the feeling of freeing their conscience. The main barriers cited by participants are the fear of discrimination, fear of disclosure of their status and fear of partners’ reactions. A significant association was found between the presence of symptoms and disclosure of HIV status (p˂0.05). It is important to consider these factors aimed at encouraging the unveiling. Further studies are needed not only for this population but also from other groups at risk to ultimately improve interventions programs for HIV prevention.

Testo completoWe evaluated the acceptability of pre-exposure prophylaxis (PrEP) for HIV prevention and identified the potential facilitators and obstacles that could affect the implementation of this prevention strategy among men who have sex with men (MSM) in Bénin. The results showed that PrEP is not well known to MSM. However, once informed about this prevention tool, almost all surveyed men were willing to use it if made available. Some factors were viewed as important by MSM to be considered in the implementation process. The risk of reducing or even abandoning condom use exists. However, this should not hinder the prescription of PrEP. Prescribers should accompany MSM who wish to use PrEP in making decisions about prevention options (PrEP and / or condom) in accordance with their values and desires

Testo completoFollowing phenomenological and comprehensive perspectives, this doctoral dissertation describes the contexts of men-to-men cruising for sex and sex work focussing on space, time and mobility. Ethnographic data collected in public spaces located in the south of France and Barcelona over a decade (1995-2005) challenge AIDS research categorization in term of sexual identity and marital status, and confront urban sociology to an anthropology of secrecy. Examining transitions from a place to the other and interactions where the action is, reveals “sexual territories” that extend beyond urban night scenes, articulating with daily timetable and routines. Rapidly transforming in the city, or anciently located on cadastral borders in the countryside, they raise a problem of localization and duration, partly solved here by the notion of psychological “shift” of identity, i. Testo completoThis study focuses on specific cultural phenomena (Writing the Body, popular vulgar culture, transition of modern single women) which have emerged in the Chinese society in the 90s, the influence on the traditional order of marriage of women and the emergence of the sex industry. It tries to show the contradictions and conflicts of women in the social context of women’s liberation. From 1949 the equalization of women was established by the law but since the 1980s a period of change is influencing the modern women presenting new challenges. After the implementation of a policy of openness and reform in China, the resurgence of a mass culture is one of the most visible changes of a "modernization" with Chinese characteristics. We see the emergence of a feminized mass culture dominated by sex and sensuality. The feminization begins with the women themselves, after thirty years during which the forced equality led to the loss femininity. This feminization is also a symbol of "modernization" and is part of the values of the women. A stereotype of the "professional woman" emerged, and "beauty" is becoming a very important criterion. Popular culture, production and reproduction of images of women resulted in women losing their independent personality. And thus we see internal conflicts arise between the "observer" and "observed" in the "modern woman"

Testo completoThis research focuses on the sexual violence suffered by men, as gender-based violence. It adopts a life course approach and a particular attention to age and gender relations, as well as to sexuality and class. The analyses are based on two quantitative surveys, Virage (2015) and CSF (2006), and fifty biographical interviews with Virage respondents who had reported sexual violence. In total, 3.1% of men report having been sexually assaulted in their lifetime, almost exclusively before the age of 25 and perpetrated by other men, always older than themselves. The life course approach highlights the importance of mechanisms of domination within the family institution, particularly in trivialising and silencing violence. It underlines the fact that violence is a factor of vulnerability in the life course, depending on living conditions in childhood, and later on the trajectory of health, sexuality and interpersonal relationships. This research also highlights the particular relations to gender, sexuality and conjugality norms of men who report violence. These men position themselves less unequally than men as a whole in the space of masculinities, highlighting an opposition between hegemonic egalitarian and conservative masculinities.The biographical approach is replicated to theorise the ‘pathways of violence’, with several trajectories: exposure, silencing, qualification and enunciation. These make it possible to analyse the dynamic changes in power relations, both during the violence and once it has ended. After isolating six typical exposure trajectories, two groups stand out: men who had experienced violence at a young age and/or at multiple times, particularly within the family or close social circle; and those who had suffered violence in adolescence or during their youth, in the context of school, male social life and sexual initiation. The former have a particularly difficult pathway of violence, which leaves its mark on their lives. For the latter, the violence leaves its mark but does not have a lasting effect on their lives. However, throughout the thesis, the analysis shows that non-heterosexual men are more exposed, more silent, less supported and more vulnerable throughout their lives

Testo completoThe HIV epidemic has brought the issue of risk and disease into relations between women and men. Among the strategies set up from the individual to the collective level, varied in different situations and shaped by culturally framed representations, those relating to people living with HIV are crucial. This "social treatment" is marked by differences and similarities between men and women, which induce some reconstructions of the relationships between men and women. This research, which belongs simultaneously to the fields of medical anthropology and anthropology of gender, describes and analyzes the gender dimension of the social treatment of HIV infection in the health care system in Cameroon. The analysis of disease representations through health messages from 1980 to 2000 reveals the gender stereotypes prevalent in the Cameroonian society and their use in prevention. The experience of men and women living with HIV in the health care system then shows the differences between their perceptions and behavior, whether patients or health professionals, from announcement of HIV status by health professionnals to disclosure to partner. Then men and women’s experience of antiretroviral therapy reveal gender dimensions in the health care system. Finally, the study of the management of sexuality, pregnancy and infant feeding shows how the social roles of men and women are built in a special way for people living with HIV, as a window an aspect of gender in Cameroonian society

Testo completoThe new HIV/AIDS guidelines recommend a combined package consisting of multiple preventive strategies to limit new infections among the most affected populations. In West Africa, men who have sex with men (MSM) are especially vulnerable to HIV infection. In this context, the response of HIV programs primarily relies on healthcare services provided by community-based organizations. However, there is limited data regarding HIV-negative MSM accessing a combined preventive package in West Africa as well as on their motivations for being followed-up in community-based services and on their risky sexual behaviors. This work used the data collected from an interventional cohort (CohMSM) offering a quarterly preventive follow-up to HIV-negative MSM in community-based settings in four West African countries (Mali, Côte d'Ivoire, Burkina Faso, Togo). The results demonstrate that the needs of the participants are in favor of a comprehensive sexual health offer. Access to PrEP is also considered by MSM as a priority need in view of their high-risk sexual behaviors. This also helps to better identify MSM who are at high-risk of exposure to HIV infection. Furthermore, the quarterly preventive follow-up also appears to reduce risky sexual practices in MSM most at risk. Thus, these results contribute to a better understanding of the implementation of a combined prevention package for MSM in a community setting
