Testo completoSelf-esteem is defined by the « feeling of self-appreciation ». It is built in childhood then evolves with the social environment and life events. College years corresponding generally to the period of young adulthood and is a particular developmental period due to changes in the social environment and frequent problems of mental health and substance use. It is also a period of vulnerability for self-esteem. College years could therefore be an interesting period to investigate the specific links between self-esteem and health. The purpose of this thesis was to conduct epidemiological studies to estimate the links between self-esteem and health behaviors, physical and mental health among university students.First, a systematic review of the literature synthesized empirical evidence on the relationships between self-esteem and health-related behaviors (i.e. substance use, sexual behavior, physical activity, and nutrition). High self-esteem was associated with more physical activity and less extreme health-related behaviors (extreme alcohol consumption, use of condoms). However, mixed results regarding alcohol consumption and number of sexual partners and a lack of research for smoking, drug use and nutrition have been highlighted. Next, two studies were conducted on a sub-sample of the i-Share cohort; a prospective French longitudinal study following nearly 15,000 volunteers studying at the university since 2013. Thus, the second study of this thesis aimed at estimating the association between self-esteem and self-rated health one year after. Of the 1,011 participants included in the study, 20% reported having average or poor health. Students with higher self-esteem were more likely to have a better perception of their health (odds ratio = 1.4, 95% confidence interval = 1.2 to 1.7). A third study aimed to investigate the mediating role of self-esteem in the link between ADHD symptoms and suicidal thoughts. Of the 2,631 participants included in the study, 17% reported having suicidal thoughts at one-year follow-up. ADHD symptoms were associated with suicidal thoughts (beta = 0.2, 95% confidence interval = 0.1 to 0.2). Self-esteem mediated 44% of the total effect and was involved in 60% of the total effect when depression was also a mediator.Self-esteem is a factor of interest in youth health and interventional research could target this psychological factor. Furthermore, because the concept of self-esteem is complex and the study population was specific, further researches could be conducted to confirm these results with different measurement tools of self-esteem (especially to reveal multidimensional aspect) and representative populations

Testo completoThis thesis aimed at emphasizing the connections between sports club participation, performance, and health as well as studying the extent of mindfulness meditation inside these interrelations. At first, the interrelations between sports club participation, performance, and health have been observed among a large sample of young people. It is demonstrated that: a) sports club participation would lead to a better level of performance and health, and b) some sports practices (more than others) are associated with greater benefits to physiological and psychological factors that influence performance and health. Then, through a systematic review, the effects of mindfulness short periods on a health psychophysiological biomarker (heart rate variability) have been evaluated. Our results underline the lack of randomised controlled trials to be able to state that the autonomic nervous system would benefit from mindfulness. Finally, we have tested the effects of mindfulness short periods meditation combined with high intensity running training on performance and health. Our randomised controlled trial highlights that: a) mindfulness meditation would produce differences (compared in a control group) in cardiorespiratory endurance, and b) mindfulness meditation would improve certain psychological skills in sport and in athletes' well-being. Our results also show that mindfulness meditation would not affect heart rate variability, ratings of perceived exertion, and other components of physical fitness

From both approaches, the nursing staff's comments were better understood and proposals were exposed to improve nursing staff's work activity as well as to arrange strategies that reconcile a bearable workload with a correct quality of healthcare

Testo completoIn this study, PE teachers’ views have been collected. These teachers’ standpoints have been analysed with regard to one of their special assignments, considered as the third objective in the official syllabus of physical education and thus defined : “access to a good knowledge of how to manage and keep us physical life”. These semi-guided interviews revealed how difficult it was sometimes for the teachers to combine EP and health education in their classes. Furthermore, most of them say that health education cannot be dissociated from their function. Nevertheless, they wonder about how to provide a sufficient range of activities and which requirements they should meet in this domain. They acknowledge that they are in need of teaching aids to take the job on. We held group interviews in the third and fourth forms of a secondary school. The transcript of the tapes shows that students recognize someone is in good health thanks mainly to what he does, what he actually is and what he communicates. Contrary to a generally accepted idea, teenagers have a nodding acquaintance with health education and they have a pretty accurate picture of what course of action they have to take in order to preserve or improve health. This first part underlines the importance of not just focusing the health education activities on the acquisition of a large body of knowledge but rather on personal commitments and specific training conditions capable of encouraging teenagers to adopt a healthy way of life. This is precisely how we envisaged mounting a major coherent operation for health at school. We devised suitable experimentation in EP classes and this is what the second part is all about

Testo completoBackground: Non-medical staff in nursing homes for the elderly are exposed to high levels of physical and psychological stress related to managing increasingly dependent residents with multiple pathologies. Our research approach is based on Green and Kreuter's public health model. This is intended to describe the physical and mental health (epidemiological diagnosis) and to explore working conditions as experienced by nursing home staff (behavioral and environmental diagnosis). Methods: A cross-sectional descriptive survey was conducted by 78 occupational physicians on 2,649 employees (706 housekeepers, 1,565 nursing assistants and 378 nurses) in 105 nursing homes for the elderly in the Rhône-Alpes Region of France. Employee data were collected on several validated questionnaires. All statistical analyses were performed on SAS software, version 9.3. Results: Respondents related elevated hardship and desired more continuous training in palliative care. They were highly exposed to psychosocial stress (strong effort, low reward, effort/reward imbalance, overcommitment). Neck and upper limb musculoskeletal complaints and signs of psychological distress were significantly associated with exposure to psychosocial stress. Conclusions: The next phase of the research plan should consist in establishing an educational diagnosis by assessing the organizational factors associated with psychosocial stress in nursing home staff

Testo completoHematopoietic stem cells allograft, or bone marrow transplantation, is one of the treatments proposed to stop the course of hematopoietic malignancies such as acute leukemia. This procedure consists of replacing the cells in the diseased blood with healthy cells from a compatible donor to maximize the chances of success. Although injecting the graft appears to be a relatively simple procedure, it is nonetheless part of a whole process that begins well before the procedure and has consequences in the months and even years following the allograft. Thus, the patient's care pathway can be divided into three stages: the pre-transplant, hospitalization in a protected sector, and the post-transplant period with the return home. At these different stages, an alteration in quality of life, the presence of symptoms of psychological distress and even a state of post-traumatic stress disorder (PTSD) can be observed. However, these psychological consequences may differ from one individual to another, suggesting that certain factors, other than medical ones, could influence both the physical and psychological consequences of the allograft. It is in this perspective and based on the Transactional Integrative and Multidimensional Model of Bruchon-Schweitzer and Boujut (2014) and the Complete State Health Approach of Keyes and Lopez (2002) that four studies were conducted in this thesis work in health psychology. The objective was to highlight the different protective and deleterious psychological factors at the different stages of allograft on the mental and physical health of patients, focusing on factors little explored to date and from the third wave of cognitive and behavioural therapies such as acceptance and commitment therapy. After validating a French version of the AFQ scale (article 1), the first study found that the level of happiness before the transplant contributes to a better quality of life before the procedure (article 2). The second highlighted the prospective effect of anxious preoccupations during hospitalization on the occurrence of post-traumatic stress disorder and the positive effect of fighting spirit on the level of resilience and happiness five months post-transplant (article 3). The third one revealed that the more people tended to be in experiential avoidance, the more likely they were to develop symptoms of PTSD, while patients with high non-judgmental scores (a facet of mindfulness) were less likely to develop such symptomatology (article 4). Finally, the fourth study shows that the experience of positive life changes in patients' lives after transplantation is far from systematic. Rather, patients show a decline in their psychological health that is related to a low level of acceptance (article 5). All of these results led us to reflect and propose an intervention adapted to the needs of patients (i.e. a digital platform) based on effective interventions in psycho-oncology and web-based therapies. In this perspective, a first device (i.e. DESP task) aimed at remedying the attentional biases involved in anxiety and depression has been developed and tested (article 6). The DESP has been the subject of a declaration of invention (declaration of invention n°0487-UCADESPTASK). Proposing a continuity of care and trying to overcome the obstacles inherent to the specificities of the allograft course is essential to improve the care of patients and accompany them during this pathway

Testo completoThis thesis explores two key determinants of health: the consequences of career events and the design of nutritional fiscal policies. The first objective is to enhance the understanding of the links between certain employment shocks and individuals' health. The first chapter examines the effects of involuntary job loss on healthcare consumption. Exogenous job losses are identified through plant closures, and causal estimates are obtained by comparing a group of displaced workers to a similar group of non-displaced workers. The findings show that job loss has long-term negative impacts on mental health, while the effects on physical health are less persistent. The second chapter investigates the effects of technological transformations in the workplace, particularly automation and robotics, on workers' health. Automation at the firm level is measured using proxies such as the stock of industrial equipment and machines, as well as imports of automation goods and industrial robots. Firm-level analyses reveal that the productivity effect offsets the displacement effect: automation is associated with an increase in the number of employees, especially among blue-collar workers. Incumbent workers increase their consumption of antidepressants and anxiolytics, though no significant changes in healthcare related to physical health are observed. The other scientific aim is to deepen the understanding of fiscal policies designed to combat obesity. The third chapter evaluates the effects of nutritional taxes on non-alcoholic beverages on overall welfare, considering the utility of consumers, firms, the government, and the externalities generated by excessive sugar consumption. The study provides empirically derived optimal designs for tiered taxes, which are then compared to optimal designs suggested by theoretical literature. This thesis, based on an empirical approach, draws on a rich and diverse set of data: administrative data (both at the firm and individual levels), surveys, and household purchase data

This is how the administration, which is nevertheless a sphere of concrete action, ismade even more abstract, a simple cog in the new instrumental reason of the world.Therefore, it is the analysis of this "black box" that this thesis proposes to carry out from aperspective of political anthropology of mental health and social epistemology. The objectiveis to shed light on the institutional evolutions of mental health in France during the decade of2010. In this conjuncture, "experience" has become a category at the foundation of a newmoral economy in public policies, resting on the experience of the disease, public conceptions of assistance, the questioned role of mental health care, the moral agency of sick people. The study is grounded in multi-location fieldwork, conceived as an assemblic ethnography, exploring local manifestations of a common problem, from the posture of a local health administration officer. Testo completoObjectives : The primary objective of this study was to describe the psychological well-being of women during pregnancy in France and the access to a health professional's consultation for psychological problems. The other objectives were to investigate the relation between antenatal maternal depression, anxiety and neonatal outcome, as well the relation between these maternal conditions and their children’s early cognitive development. Methods : The study was based on the 2010 French National Perinatal Survey, which included all births in French maternity units during a one-week period (N=14 326 women) and from the EDEN study, which is a French cohort of prenatal and early postnatal determinants of child health and development. After birth, children were followed-up until 3 years old. Results : In France in 2010, 9% of women reported poor self-rated mental health during pregnancy. Most of these women were not diagnosed and not treated. Sociodemographic characteristics indicative of social disadvantage were associated with a higher risk of poor self-rated mental health. However, more favorable social characteristics were associated with a consultation of health professional for these psychological difficulties. Significant associations were found between maternal antenatal anxiety and depression and spontaneous preterm birth. After birth, significant associations were found between mothers’ antenatal anxiety and their children’s cognitive impairment. Discussion : All pregnant women should benefit from an evaluation of their psychosocial and medical environment. Appropriate psychological screening and support of pregnant women could probably improve pregnancy experience and reduce neonatal and post-natal complications

Testo completoIn handicapped environments, those who succeed in spite of risk factors are called " school resilients ". What is the part of education and culture or the part of the individual in this success ? Do the parents transmit the desire to get through ? 220 French and 240 israelis aged 13 to 16 were observed. Their success was assessed by an average of school performance and a average of social adaptation. We have observed the interactions between risk factors of adverse environments ans protections factors such as " adequate parental education " and " percieved mental health " by the adolescents. Our results confirm that scool resiliency is the result of multiple interactions between the individual and the environment. The resilient adolescent is affected by his adverse environment (parental and cultural) but he modifies his environment and constitues an active agent of his adaptation. A state of tensions might yield progress. The resiliency is an attitude, in contre reaction to difficulties. This attitude might be stimulated in pupils having difficulties

Testo completoIn this research, we investigated mental health, level of posttraumatic stress disorder and acculturation process of a population - victims of forced migration - because of internal conflict, after 1990, broke out in south-eastern Turkey - which has migrated in three different contexts (Paris, Izmir, Diyarbakir). Our sample consists of 94 participants in total including 32 in Paris, 32 in Izmir and 30 in Diyarbakir. We crossed, for complementary, qualitative methods (clinical interview referred to research) and quantitative (scales). The research results show that mental health of forced migrants is negatively influenced by the process of migration, and that even many years after, much of the population still manifests the symptoms of posttraumatic stress disorder. Testo completoPsychosocial work factors are a public health and occupational health issue and are the object of special prevention policies. However, the etiological role of these factors on mental health need to be developed, particularly studies using diagnostic interviews, rarely used in this context. Moreover, the role of these factors in the explanation of social inequalities in mental health is still unknown. The objectives of this PhD thesis are to study the etiological role of psychosocial work factors on mental disorders, measured using a diagnostic interview, and to evaluate the contribution of these factors to social inequalities in mental health.The data from two surveys were used: Samotrace (regional survey based on 6056 employees) and SIP (national survey based on 7709 workers). The main feature for Samotrace was the use of validated questionnaires to measure psychosocial work factors, and for SIP it was the use of a diagnostic instrument to measure depressive and anxiety disorders. Multivariate analyses were conducted, including logistic regression analysis. Interaction tests and the bootstrap method were also used. All analyses were carried out separately for men and women.Low decision latitude, overcommitment and emotional demands were found to be risk factors for depressive and anxiety disorders, for both genders. Other risk factors were observed according to gender or mental health outcome studied (job insecurity, high psychological demands, ethical conflict, low reward). Few social inequalities in mental health were observed except for self-reported health; manual workers being more likely to report poor health. Occupational factors reduced social inequalities in health by 76% and more, according to gender and occupation studied. Among occupational factors, psychosocial work factors played a substantial role, particularly low decision latitude, and to a lesser extent (according to occupation and gender): low social support, low reward, night work, work-life imbalance, physical violence and bullying.Prevention actions focussing on identified risk factors, including emergent psychosocial work factors, should be considered and a better implementation at workplace would be necessary. Improving knowledge on social inequalities of mental health may lead to adequate preventive actions targeting the most exposed social or occupational groups. As our studies were cross-sectional, our results should be confirmed by forthcoming prospective studies

Testo completoThe emotional information is processed in a privileged way in central (VC) and peripheral vision (VP), despite the low acuity of the latter, leading to a set of brain, behavioural and autonomic responses, aimed to prepare the body to act.In this context, the altered emotional processing within the visual space, observed in affective disorders such as depression and anxiety, is likely to manifest itself on all of these emotional responses. During a series of experiments, we therefore adopted an integrated approach to study the impact of the emotional scenes presented in VC and VP in depression and anxiety. First, we observed that patients with major depressive disorder showed a negativity bias at behavioural and autonomic level, but only for VC presentations, in another mode of reactivity, generalised anxiety disorder was characterised by increased sensitivity to emotional information presented in VC and VP. Despite a differentiated behavioural and autonomic reactivity, it should be noted that both disorders were associated with early brain components. Second, we showed that the autonomic manifestations of depression and anxiety were found at subclinical level. High anxiety led to greater sympathetic activation in response to VP stimuli and emotional stimuli; and high depression associated with high anxiety led to an increase in sympathetic activity at the end of the response to emotion, particularly to unpleasant presented in CV. On the other hand, anxiety, but not depression, was associated with an effective physiological recovery after a stimulation period indexed by a reduction in sympathetic activity. Thus, while depression would restrict visual capture to the CV and promote the perception of negative information, anxiety would lead to an expansion of monitored visual space and an increased sensitivity to emotional information. The research presented in this thesis opens up innovative prospects on the understanding of depressive and anxiety disorders and proposes to broaden and clarify the therapeutic strategies proposed to patients suffering from these disorders. The research presented in this thesis provides a real added value at three levels: first, it shows the relevance of the paradigm "central vision / peripheral vision" as an original heuristic model to probe affective disorders; second, it provides innovative experimental data that may otherwise include depressive and anxiety disorders; Finally, our research proposes to broaden and specify the therapeutic strategies proposed to patients suffering from these disorders

Testo completoThis dissertation looks at "specialized" mental healthcare expertise in France as a lens through which to address the institutional management and representations of cultural difference in France today. By "specialized" mental healthcare centers, I refer structures that provide culturally-sensitive mental health services to immigrants specifically. I identify and explore three contemporary expert approaches: namely, transcultural psychiatry, clinical medical anthropology, and ethnoclinical mediation. By providing a genealogy of specialized mental healthcare institutions, and by construing them as "meta-discursive nodes" - that is, as points of encounter between state, institutional, and individual ideologies - I provide an analysis of the cultural anxieties, contradictions and double-binds that arise from the opposition between a regulative, universalist republican ideology, and a field of expertise which strives to promote culturally-sensitive mental healthcare for immigrants. I argue that, as a product of the conflation of the "immigrant issue" (la question immigrée) and the "social issue" (la question sociale), "immigrant suffering" (Sayad, 2004) has become a medium that problematically couches immigrants' "difficulties" - whether they relate to mental health pathology or structural problems - in terms of cultural difference. As a result, generic cultural representations of immigrants are uncritically reproduced, making it difficult to identify and address the structural inequalities that do engender suffering

Testo completoDepression is a common mental disorder and the leading cause of disability worldwide. It has been associated with poor quality of life, physical decline, and increased risk of premature death. In addition, depression leads to a considerable economic burden, making it a major global public health. Protective and risk factors are diverse and include modifiable lifestyle indicators. Several studies have suggested that diet may play an important role in the risk of depression, but few of these studies have investigated the relationship between « a priori » dietary scores and depression. This thesis thus aims to investigate the relationship between overall diet quality evaluated using various nutritional « a priori » scores and incident depressive symptoms, using the data from SU.VI.MAX and NutriNet-Santé cohorts.Our findings showed an inverse association between adherence to the Mediterranean diet or nutritional recommendations (French and international) and the risk of incident depressive symptoms. However, the study on adherence to the Mediterranean diet showed significant results in men only. Moreover, « pro-inflammatory » diet was positively associated with the risk of incident depressive symptoms, but in the SU.VI.MAX cohort, the associations were significant only in some subgroups of the population: men, smokers and participants with low physical activity. We also showed that an increasing number of « healthy » lifestyle indicators was inversely associated with incident depressive symptoms.All of these findings suggest that promoting « healthy » lifestyles (including « healthy » diet) could be an important strategy in primary prevention of depressive symptoms

The classification of this disease is yet to be completed and stigmatizes numerous patients in a psychotic and schizophrenic structuring. Whereas in reality they are in the process of traits assembly in order to better contain their anxiety by taking into account borrowed symptoms of schizophrenia. This is what this thesis intends to demonstrate through a 90-cases sample in all three countries. Schizophrenia is said to be representing 1% of world population. We show in our research that this could be an abuse of terminology. Indeed, all subjects considered have pronounced features of schizophrenia, but from the analysis of individual tests, we hardly find a truly schizophrenic structure; it is most often disparate traits assemblies whose and expressions of schizophrenia forms are multiplied by semiological differences almost endlessly. The limits of the thesis are that our findings are not based on a longitudinal methodology of followed-up case, and also that the subjects are under the regulatory control of drugs

Testo completoThe primary results from the study of 845 couples in the general population suggest that there exists a significant spouse-similarity across the various mental health dimensions examined (psychological distress, general well-being, and role satisfaction).
The main results from the study of 17 couples in marital therapy suggest that significant sex differences exist in dyadic adjustment. Sex differences were also noted in the correlations between dyadic adjustment and depressive symptoms.
In conclusion, it appears that epidemiological research on the mental health of couples should have as its objective a simultaneous consideration of both the individual and the couple, as well as a simultaneous consideration of clinical and general populations, in order to create a double complementarity out of this apparent double dichotomy.

Testo completoTo study mental disorder the first instinct is to turn to medicine, especially psychiatry. However, the law does have some sort of connection with this theme and the following study aims at comprehending the legal treatment of mental disorder. Based on the polysemy of the word “treatment” we will analyze the different ways the law deals with mental disorder and the level of freedom to which the mentally ill are entitled. As the most commonly accepted meaning of the word “treatment” refers to the way a person is addressed, the purpose of the following study will be to understand how the law deals with the mentally ill as well as the difference of attention given to the patients who are committed to a mental institution and those who are detained in prison. As the second meaning of the word refers to the way of solving a problem, this study will focus on how lawmakers reacted to the convictions passed by the European Court of Human Rights and to the unconstitutionality rulings rendered by the French constitutional judge. The second meaning of the word will also enable to study the root causes of the phenomenon of communicating vessels between mental institutions and prisons. We will see that the borders between law and psychiatry are porous; that is why the study will aim at analyzing the connections between law and psychiatry as well as their consequences on the mentally ill. The attitude towards sick people will depend entirely on the relationships between the two subjects. By giving a legal framework to psychiatry, the law attempts to promote access to healthcare and to reassert the value of individual freedom. On the other hand, psychiatry will influence the law, which will have to put in place systems to monitor and keep the mentally sick under control. The study of the legal treatment of mental disorder. Some mentally ill persons with HIV/AIDS will be involuntarily hospitalized. The second chapter compares the mental health law of Quebec and Ontario governing involuntary hospitalization.
The third chapter examines consent to or refusal of treatment by mentally ill persons with HIV/AIDS in particular with respect to their competence. It explores the law governing substitute decision-making for incompetent persons.
The last chapter considers what protections of the rights of mentally ill people with HIV/AIDS are provided under Quebec and Ontario human rights codes and the Canadian Charter of Rights and Freedoms.

Testo completoIn some of Quebec’s unionized sectors, important initiatives have been deployed in order to prevent mental health problems in the workplace. For over three decades, visionary union representatives have built working peer assistance structures, won important legal struggles and have developed innovative approaches aiming to correct or improve work organization. As the neoliberal ideology and its related work organization principles contribute to intensify work, leading to a weakening of the workers psyche, and as the balance of power favors employers more and more, it seems like an opportune time to survey the experience of those visionary union representatives in order to get a better understanding of union actions regarding mental health in the workplace and its structuring process. This thesis studies Quebec’s union initiatives concerning mental health in the workplace and aiming to prevent and correct problems like psychological distress, burnout, harassment, depression, violence, work related suicide, etc. In order to achieve this objective, a mixed theoretical approach is used. On the one hand, a large philosophical perspective is adopted. Authors Thomas Hobbes, Adam Smith, Karl Marx and Hannah Arendt are referred to in order to forge a better understanding of human interactions and action. By extracting their respective views of the world, of action and of social link, an analytic model is built, which could tie the life experience of union representatives to these different views, therefore achieving a better understanding of the ideological basis on which they built up their action and how it had influenced their individual and collective history. On the other hand, the social experience theory (Dubet, 2009; 1994) is selected as a way to analyze more precisely the work of union representatives. This theory identifies three logics of action, all complementary and in tension with one another, with which all social actors must deal with: integration, strategy and subjectivation. The coexistence of these three logics means that the actor’s experience of the living world is not a simple reproduction mechanism, applying social determination learned from their predecessors. Actors are also subjects of their own experiences and capable of distancing themselves from their own socialization in order to understand their act; they take place in the world in a critic dialectic. This theory sheds a light on what eases or restrains individual and collective action in regard to mental health in the workplace and describes how union representatives mobilize themselves in order to respond to the many needs of their members. This qualitative research uses a life trajectory methodology (Rhéaume 2008; Bertaux 2006). Twenty union representatives describe the suffering work experiences (Dejours, 2008) lived by workers and present initiatives they deployed in order to help them. The situations described by the participants show how some work organization elements are linked to domination experiences (Martuccelli, 2004) and have impacts on mental health in the workplace: the collateral damage of productivism and hyperflexibility, occupational injuries, occupational diseases and horror situations in the workplace, pernicious workplace social dynamics and abusive uses of the judicial system. The study also demonstrates the extent to which union initiatives contribute to solve these problems and the key factors in that regard: interdependence, empowerment, social justice and struggle for dignity. Four categories have been forged to describe those initiatives: social link care with peer assistance, legal defense, collective agreement clauses and actions targeting the work organization. Finally, the thesis presents three union representative profiles: militance which tries to build an us in the workplace, professionalization which tries to have its utility and competence recognized, and the peer assistants which aims to develop an action centered on the self. Their meeting shows the development of a union praxis which promotes and protects dignity of work and workers.

Testo completoObjectives. To examine the associations between mental health and physical health (headaches and overweight) in young children aged 6 to 11 years old. Methods. Survey conducted in Provence-Alpes Côte d'Azur region in 2004-2005 among 2341 children aged 6 to 11 years old attending primary schools. Child mental health was assessed using standardized and validated instruments: the Dominique Interactive for children and the Strengths and Difficulties Questionnaire SDQ for parents. Statistical associations were examined in univariate then multivariate analysis using logistic regression adjusted on potential confounding factors. Results. Response rate was 57. 6% for parents and 76. 5% for children. Prevalence rates were 10. 9% (confidence interval 95 % CI = 9,1-12,7) for headaches and 17. 3% for overweight (IC95% = 14,9-19,8). After adjustment, factors associated with headache were: physical comorbidities (odds ratio OR = 1,75; IC95% = 1,13-2,73), SDQ total difficulties (OR = 1,76; IC95% = 1,03-3,01), particularly emotional problems OR = 1,99; IC95% = 1,13-3,52), and low punitive attitudes (OR = 0,41; IC95% = 0,18-0,94). Factors associated with headaches werre: attending school in Disadvantaged School (OR = 1,88; IC95% = 1,03-3,44) and SDQ peer relationship difficulties (OR = 2,06; IC95% = 1,27-3,35). Conclusion. This research work illustrates by two examples the link between child mental health and physical health

Testo completoThe roots of international criminal law may be found in the individual criminal liability of the major criminals of the Second World War. The Nuremberg and Tokyo International Military Tribunals were followed in the 1990's by the two ad hoc Tribunals for Former Yugoslavia and Rwanda, created by the United Nations Security Council, and then in 1998 by an independant and permanent tribunal with a universal vocation : the International Criminal Court. Among the underlying acts of war crimes, crimes against humanity and genocide, a leitmotif stands out : physical and mental integrity. Without a uniform definition in national legal systems, this notion is defined on the basis of statutory provisions and becomes clear in the international criminal tribunals' case law. Establishing an autonomous right to physical and mental integrity in terms of genocide and war crimes, the case law of ad hoc tribunals alsouses the notion to complete the liste of underlying acts of crimes against humanity and to define some other crimes. Chambers are also protecting this integrity without any incrimination. Because beyond physical and mental integrity of victimes, arises the question of the physical and mental integrity of other actors of conflicts and proceedings : soldiers, humanitarian workers, witnesses and accused.Studying the right to physical and mental integrity in international criminal case law uncovers the emergence of a right unknown to national legal systems as such. This right raises questions of legal certainty as much as demonstrates the autonomy of international criminal law

Testo completoContext: There is a lack of scientific evidence concerning the cumulative effects of adverse psychosocial work factors from the Efforts-Reward Imbalance (ERI) model on mental health. Also, previous studies mainly used self-reported mental health problems (MHP). There is a lack of studies investigating the effect of cumulative exposure to ERI. Finally, no previous prospective study has evaluated the effect of combined exposure to adverse psychosocial work factors from the Demand-Control (DC) and ERI models. The current dissertation aimed to address this gap in knowledge. Objectives: 1) Evaluate the effect of cumulative exposure to ERI on the prevalence of psychological distress and the persistence of this effect over a 5-year period. 2) Evaluate the effect of ERI on the 5-year incidence of medically certified sickness absence for MHP. 3) Examine the independent and combined effect of adverse work factors from the DC and ERI models on the 5-year incidence of medically certified sickness absence for MHP. Methods: This study was a prospective cohort study. More than 2,000 white-collar workers (63.3% women) from three public organizations were followed over a mean duration of five years. Psychosocial work factors and psychological distress were measured using validated self-administrated questionnaires at baseline, and at 3- and 5-year follow-ups. Medical absences from work were collected from employers’ files. Multivariate analyses were performed using the log-binomial and the Cox regression models. Analyses were performed separately for men and women, while controlling for confounders. Results: Compared to unexposed workers, men and women with repeated exposure to ERI at 3-year follow-up had a higher prevalence of high psychological distress. The deleterious effects observed at 3-year persisted at 5-year follow-up among women (PR=2.48 95% confidence interval (CI) 1.97–3.11) and men (PR=1.91 95% CI 1.20–3.04). These effects were stronger than those found when considering a single baseline measurement (+0.30 to +0.94). Also, men and women exposed to ERI had a higher risk of medically certified sickness absence for MHP, compared to unexposed (Hazards ratio (HR) =1.38, 95% CI 1.08–1.76) workers. Low reward was significantly associated with a high risk of medically certified sickness absence among men (HR=2.80, 95% CI 1.34–5.89) but not in women. (HR=1.24, 95% CI 0.90–1.73). Among women, high strain (1.50 95% CI 1.12–2.07) and ERI (1.34 95% CI 0.98–1.84) were each independently associated with medically certified sickness absence for MHP. Women having a combined exposure to high strain and ERI also had a higher risk of medically certified sickness absence for MHP than unexposed women (1.97 95% CI 1.40–2.78). The results provide no support for either the independent or the combined effect of high strain and ERI among men. Conclusion: The results from this dissertation provide support for the deleterious effect of ERI on the prevalence of psychological distress and the risk of medically certified sickness absence among both men and women. Among women, independent and combined effects of high strain and ERI on the risk of medically certified sickness absence for MHP were also observed. These results suggest that reducing adverse psychosocial work factors can help to reduce MHP, including medically certified sickness absence for mental health problems.

Disability outcome measures should ideally include objective measures such as physical performance tests. This work demonstrates the test-retest reliability of a new walking test (the 400-m usual pace walk test), the association between calf circumference and muscle-related disability and the predictive value of the short physical performance battery on mortality. Physical activity appears to be an effective strategy to delay disability. This work adds further evidence, especially in the frail elderly population (patients with Alzheimer's disease, obesity or malnutrition) of its health benefits

Testo completoThe aim of this study is to evaluate the consequences of intimate partner violence (IPV) on mental and adaptative women's health, as there is a lack of data on this subject in France. We interviewed 200 women: 73 victims of psychological and/or physical violence, 41 victims of sexual violence and 86 women without IPV. We measured the mental and adaptative health asking about depression, post-traumatic stress disorder, post-traumatic cognitions, self-esteem, coping, social support, life maladjustment, quality of life. The statistical analysis shows that the more the intimate partner violence is severe (sexual > psychological and/or physical > without), the more the participant's mental and adaptative health is damaged, and this, for all the variables. We also observe that women who have been victims from previous intimate partner violence present a lower level of self-esteem and availability of social support, and a higher level of depression, self-critic, blame, life maladjustment compared to women without historical of IPV. Finally, we notice, among the victims of IPV, that those who consult a legal medical service obtain a higher level of depression, denial and life maladjustment, and a lower level of humour, feeling expression, positive reinterpretation, and quality of life. Our research is essential to redesign psychological treatment for the victims of IPV and campaign of IPV prevention
