The Royal College of Surgeons of England (RCS) has a proud history of delivering a large portfolio of dental and surgical examinations. We take particular care to ensure that our candidates receive a professional and efficient service. The College offers its examinations both within and outside the UK. In particular, the Membership of the Royal College of Surgeons (MRCS) examinations are available in an increasing number of centres internationally. The Joint Surgical Colleges' Fellowship Examination (JSCFE), success in which leads to eligibility to apply for Fellowship of the Royal College of Surgeons (FRCS), is also now available outside the UK.

The Royal College of Surgeons of England has always had strong links with the Caribbean and in 2001 the formal College visit to Barbados stimulated the formation of the Caribbean College of Surgeons. Since that time, there has been a continued close association between the two colleges resulting in an invitation to attend the (2006) fourth annual conference of the Caribbean College of Surgeons in Jamaica, to participate in the scientific meeting and then to stage a surgical skills course and an advanced laparoscopic skills workshop in the capital, Kingston.

Surgical postgraduate examiners and examinees in Nigeria complain of the low pass rate at all levels of the postgraduate surgical training examinations to which several factors are contributing. For several years there has been being a persistently low surgeon workforce in the country despite having two surgeon producing institutions been for at least 37 years. A review of the probable causes was carried out to shed more light on the matter. At the time of writing there are 52 National Postgraduate Medical College of Nigeria (NPMCN) and 46 West African College of Surgeons (WACS) accredited post graduate surgery training programs in Nigeria compared with 99 in the United Kingdom (UK) and 1056 in the United States (US). Based on available data Nigeria has approximately 572 surgery residency training slots yearly compared with approximately 646 in the UK and 4225 in the US. Examination pass rate was less than 40% for primary WACS compared with 98% pass rate in USMLE (United States Medical Licensing Examination) 3, pass rate at part I was 28.8% for WACS compared with 37% at MRCS (Membership Royal College of Surgeons) part A and 57% for MRCS part B. For the exit examination or part II WACS pass rate was 31.5% (general surgery) while it was 64% for Fellowship Royal College of Surgeons (FRCS) cumulative and 70% in the American board of surgery (ABS). Surgeon per 100 000 population was 0.69 for Nigeria compared with 11.7 and 25.6 for the UK and US respectively. In the last 35 years WACS has produced 1638 surgeons (2.8 times more than NPMCN) in surgery and NPMCN has produced 572. The frequency of examination were twice per year for both WACS and NPMCN examinations, 3 times per year for the USMLE step 3, MRCS (A & B) and Fellowship Royal College of Surgeons (FRCS) general surgery. The American Board of Surgery (ABS) is once per year for Qualifying Examination (QE) and 5 times per year for Certifying Examination (CE).

With health reform underway in both countries, the issues confronting the surgical workforce in the US are strikingly similar to the challenges facing the surgical workforce in England. This article describes the American College of Surgeons (ACS) Health Policy Research Institute's (HPRI) role in collecting, analysing and disseminating information about the surgical workforce in the US and suggests that HPRI might serve as a model for The Royal College of Surgeons of England to assist the UK government in workforce planning.

The 1858 Medical Act, which regulated medicine in the UK, said: 'It shall […] be lawful for her Majesty, by charter, to grant to the Royal College of Surgeons of England, power to institute and hold Examinations for the purpose of testing the fitness of persons to practise as Dentists […] and to grant certificates of such fitness.' It was in 1860 that the Royal College of Surgeons set up the LDS RCS diploma and organised the first examinations.

On the occasion of the anniversary of the foundation of the Royal Colleges of Surgery of Barcelona and Madrid, this review presents the life and activity of both colleges, as well as the illustrious people who were in charge of them. The decline of surgery in the early 18th century prompted the creation of Royal Colleges of Surgery by the Bourbon Monarchy. The Royal College of Surgery of Barcelona, promoted by Pedro Virgili, with the previous experience he acquired at the Royal College of Surgery in Cádiz, was created with the intention of supplying the Royal Army of Spain with well-trained surgeons. The Royal College of Surgery of San Carlos in Madrid was inaugurated to use the training it offered to treat not only the military and sailors, but also the rest of the population. One of the most important figures in these institutions was Antonio Gimbernat, among others, whose life and academic career were linked to the evolution of the three Royal Colleges founded in Spain.

The pan-specialty surgical elogbook, maintained by The Royal College of Surgeons of Edinburgh (RCSEd) on behalf of the four royal surgical colleges, is now the de facto tool for all surgical trainees. The colleges issued new terms and conditions at about the same time that general surgical trainees were obliged to transfer to the elogbook from the previously mandated intercollegiate Surgical Curriculum Programme logbook. Some of the clauses within the terms and conditions gave rise to concern, and several trainees contacted the Association of Surgeons in Training (ASiT) to seek clarification.

Joseph Lister – the largest English surgeon and scientist, the founder of antiseptics, President of the Royal society of surgeons, a member of the house of lords. Joseph Lister was born on 5 apr 1827 in England. In 1844 he graduated from high school, and in 1852, the medical faculty of the University of London and was appointed resident assistant College University hospital. The first scientific work of Lister was published in 1852 and was dedicated to the structure of the iris of the eye and its muscles. Soon Lister began working in the clinic of Professor George. Syme in Edinburgh and published lectures, devoted primarily to ophthalmology. In 1855 he became a member of the Royal College of surgeons and is a Professor in the George. Saimaa. In 1858 Lister became a surgeon of the Royal hospital in Edinburgh and at the same time began to read a course of surgery at the University. On 9 March 1860 he was appointed Professor of surgery in Glasgow. In 1867 in the journal "Lancet" published articles Lister, in which he argued the idea that wound infection is called a living beginning, introduced from the outside; was presented to combat surgical infection, comprising treating hand surgeon, surgical field and instruments, disinfection of the air by atomization of a solution of carbolic acid. In 1869 Lister was transferred to the surgical clinic in Edinburgh, and in 1877 he was given the chair of clinical surgery at king's College London. In 1884 Joseph Lister was given the title of baronet, from 1895 to 1900 he was President of the Royal society of surgeons; in 1897 appointed a member of the house of lords. In 1892 he was 65 years old and, according to the law, he had to leave the Department at the Royal College. Joseph Lister was made an honorary member of numerous universities and scientific societies, was awarded the Royal medal (1880), medal of Comenius (1877), albert (1894), Copley (1902); the order of merit (1902). Died Joseph Lister, on 10 February 1912 in Walmer. In honor of Joseph Lister has been named a genus of bacteria Listeria (Listeria), he is on the English postage stamp, issued in 1966.

For this reason in late 2008 the London Deanery commissioned the Royal College of Surgeons to provide a programme to develop the leadership skills for SAS surgeons within the deanery.

Abstract Mohs micrographic surgery (MMS) has been shown to be an effective treatment for nonmelanoma skin cancer. It provides distinct advantages over traditional excisional surgery and its success rate is dependent on the accurate microscopic evaluation of carefully mapped specimens. The Mohs surgeon must be able to translate abnormal findings on the tissue map into appropriate sequential tumour removal. A critical component of this technique is the surgeon’s skill in interpreting histological specimens. Some studies have investigated the concordance of Mohs surgeons’ interpretation of frozen sections with that of dermatopathologists. The American College of Mohs Surgeons has successfully run an annual diagnostic quality control virtual exam. The Australasian College of Dermatologists (ACD) has a well-established external and independent quality assurance programme in conjunction with the Royal College of Pathologists Australia Quality Assurance Program. The British Society for Dermatological Surgery launched its own external quality assessment programme in 2022. This study compares the quality control programmes of both Australia and the UK and highlights key differences and lessons from both.

Abstract Background: A very important part of good medical practice is to maintain patient records. Accurate and detailed operation notes are of great importance in all surgical specialties, not only for patient care but also for providing information for research, audit, and medico-legal purposes. Operative notes are often presented in legal malpractice cases, and most of them are indefensible from a medico-legal standpoint. Incomplete and illegible notes are often a source of weakness in the defense of surgeons in courts. Clear and legible notes relating to all surgical procedures are therefore extremely important. In this study, we assess the quality of operative notes against the standards set by the Royal College of Surgeons of England with a view to improving the quality of operative notes and ensuring quality patient care. Materials and Methods: This study was conducted in a tertiary care hospital in central India. A sample size of 47 orthopedics arthroplasty operation notes was collected between March 2021 and October 2021 from the Department of Orthopaedics at SAMC and PGI, Indore, and they were retrospectively audited by senior orthopedics surgeons according to the Royal College of Surgeons of England Good Surgical Practices Guidelines. The notes were audited keeping in mind the guidelines set by the Royal College of Surgeons of England Good Surgical Practice with some additional points which was related to hip arthroplasty making up 21 points which is described in table below. Results: During the audit cycle, 47 operation notes of hip arthroplasty surgeries performed between March 2021 and October 2021, were retrospectively reviewed against the guidelines for the operation notes set by the Royal College of Surgeons of England in the recent 2014 edition; there was good compliance with respect to date documentation (85%), diagnosis (97%), name of operating surgeon (95%), assistants’ names (89%), description of the operative procedure (70%), detailed postoperative instructions (100%) and the signature (96%), name of responsible consultant (93%), details of the serial number of prosthetic used (98%). The audit highlighted two components with poor compliance: description of other procedures performed such as catheterization/calf stimulators/foot pumps (67%), details of suture used (69%). A memory aid was then designed and placed in the operation room for two months, emphasizing the poorly documented aspects. Conclusion: The format used in our hospital SAMC and PGI aligns well with the criteria described by the Royal College of Surgeons Good Surgical Practice guidelines released in 2014. Our installation of memory aid in the operation theater improved the stats of some of the criteria; however, some points in the operation notes still remain poorly filled in. We strongly recommend continuing with the implementation of audits in our clinical practice among resident doctors and consultant surgeons of the departments in order to closely analyze our practices, especially with regard to patient safety and quality of care.

The Intercollegiate Basic Surgical Skills course has a long history and runs regionally and internationally at multiple centres. The Royal College of Surgeons of England ran 81 courses for a total of 1,083 participants between 1 July 2011 and 30 June 2012. The fifth edition was officially launched on 25 September 2012 with content agreed by representatives from the four Royal Colleges of Surgeons. As a result of feedback from faculty, participants and centres running the course, it has been reduced to two days, in alignment with the ISCP for basic surgical skills and without losing vital hands-on practice.

A MOCOMP (Maintenance of Competence) Sub-Committee of the CNS Education and Manpower Committee has also been established to deal with implementation issues, and to liaise with the Royal College. Dr Adams was previously consultant anaesthetist to Addenbrooke's Hospital, Cambridge, with a special interest in ophthalmic and neuroanaesthesia, and Associate Lecturer in Cambridge University. She was Dean of the Faculty of Anaesthetists of the Royal College of Surgeons of England in 1985, now the Royal College of Anaesthetists, of which she is currently Honorary Archivist/Curator. She was Hunterian Professor in the Royal College of Surgeons in 1993, and is a past president of the History of Anaesthesia Society. Within the RSM she was president of the Section of Anaesthetics in 1985-1986 and of the Section of the History of Medicine in 1994-1995, having served as Honorary Secretary of each. She is now an Honorary Treasurer of the Society.

The modern training pathway for junior surgeons prior to specialty training encourages rotation through a variety of specialties with the aim of creating a well-rounded surgeon. While the syllabus of the intercollegiate Membership of the Royal College of Surgeons aims to prepare individuals broadly for the major surgical specialties, oral and maxillofacial surgery (OMFS) represents an additional challenge for singly qualified trainees (either doctors or dentists) working as junior trainees (eg F2 trainees, dental F2 trainees, core trainees, senior house officers/trust grade surgeons) in a specialty that ultimately requires dual qualification in medicine and dentistry.

In November 2010 I started work at the Royal College of Surgeons on a five-year project to catalogue the College's institutional archives. These are the records created by the organisation since 1745 as part of its everyday business – a unique and valuable historical research resource. The archives document many aspects of the College's history: Council decisions; examinations; College buildings; research and teaching programmes; the Hunterian Museum; and relationships with other bodies including the government and the National Health Service.

At most conferences on medical audit we are reminded that medical audit is centuries old. What is new is the push for medical audit as a formal activity in which each clinician must take part. This push came from Mrs Thatcher's NHS review in 1989, invoking the spirit of market forces in the NHS. Whether this spirit is that of a goddess or demon, it is perhaps too early to know. As many of the Royal Colleges (Hoffenberg, 1989; Royal College of Psychiatrists, 1989; Royal College of Surgeons of England, 1989) and the Standing Committee on Postgraduate Medical Education (1989) produced their guidelines, the push to make medical audit a formal activity in which each doctor should take part became reality in 1989.

Who says exams are getting easier? Born on 1 May 1785, the prodigious George Guthrie passed the entrance examination of the Royal College of Surgeons in 1800 when he was just 15 years old. While today's teenagers may struggle to find a local youth club that has survived the coalition government's cull, by the time Guthrie joined the College he was practically an old hand at surgery, having been apprenticed to a senior army surgeon since the tender age of 13.
