The Royal College of Surgeons (RCS) rat is an established animal model used to study human retinal dystrophies. This study investigated whether kidney dysfunction accompanies the eye abnormalities seen in this model. Overnight urine collection procedures were used to measure protein excretion in 2, 12, and 22 month old female pigmented RCS rats and control rats (RDY). Clearance experiments were performed in anesthetized rats to measure glomerular filtration rate (GFR) and renal plasma flow rate (RPF). There was an age-related increase in protein excretion in both RCS and RDY rats, but the protein excretion was significantly higher in the RCS rats at 2 and 22 months of age. Whole kidney GFR and RPF were significantly lower in the 22 month old RCS rats, when compared to age-matched RDY rats. These findings suggest that the RCS rat exhibits both kidney and eye abnormalities.

The repatriation of the human remains of Indigenous peoples collected within a colonial context has been the subject of debate within UK museums over the last 30 years, with many museums now having returned human remains to their countries of origin. Although the repatriation of human remains is often characterised as the 'journey home', there has been a lack of consideration of the physical presence and mobility of the remains and the meanings created as they move through different spaces. This study uses the repatriations from The Royal College of Surgeons of England (RCS) to Australia, New Zealand and Hawaii as case studies to consider three key areas: (i) the impact of repatriation on museum landscapes; (ii) the journey of the repatriated remains and how this mobility intersects with wider discussions about restitution, sovereignty, identity, relatedness, memory and memorialisation; and (iii) the repatriation archives, how they are thought about by the institutions that hold them and their future potential and meaning within a post-colonial context. Taking a more-than-representational approach and engaging with the materiality, mobility and agency of the repatriated remains and the documentation that relates to them, this study bridges the gap between research considering the approach of museums to repatriation, and ethnographic studies on the meanings of the return of ancestral remains to individual communities. Combining work on museum geographies, deathscapes and absence opens up new ways of theorising and discussing repatriation through understanding the process in terms of the tension between absence and presence, and human remains as being in or out of place. Through engaging with the materiality and agency of the remains and viewing repatriation through a spatial lens, this thesis deals with aspects of the process that have received little attention in previous studies, foregrounding the challenging nature of repatriation for communities, the issues around unprovenanced remains, and discussions about the control, management and meaning of information and data, identifying that a significant legacy of repatriation for RCS is the documentation the museum continues to hold. What the journey of the ancestral remains repatriated by RCS illustrates is the emotive materiality of the remains, and agency that they and the distributed repatriation archive have as actors within social networks. It is therefore proposed that the concept of repatriation as having problematised human remains collections within UK museums is replaced with a nuanced and contextually sensitive understanding that recognises the role of the human remains in social interactions that impact on the emotional geographies of museum practice, and that rather than framing repatriation as post-colonial act that is either political or therapeutic, the return of ancestral remains be understood as part of a process of decolonisation in which there is space for discussion, disagreement and debate amongst all stakeholders.

The leading causes of loss of vision in the developed world are the degenerative diseases of photoreceptors in particular, age-related macular degeneration (AMD) and retinitis pigmentosa (RP). A common characteristic of these diseases is secondary damage affecting the vascular network, which is apparently initiated by photoreceptor loss. One problem with investigating the vascular consequences of these diseases has been the lack of a suitable animal model that can be used to investigate various potential treatments. This study has developed methods of quantifying retinal vascular damage in the pigmented Royal College of Surgeons (RCS) rat, which is characterised by the formation of vascular complexes and these methods have been used to explore strategies to retard or reverse this damage. This was done with the view to improving the retinal environment, thereby assisting other therapeutic strategies that target the primary defect causing the loss of photoreceptors. The work was divided into three areas: 1) investigation of the vascular effects of progressive photoreceptor loss and development of computerised image analysis to quantify changes, 2) pharmaceutical intervention to modify the normal sequence of events, 3) examination of the effects of RPE sub-retinal transplantation on the vascular network to determine how the retinal vasculature would react to the presence of transplanted human RPE cells at different time-points. These three areas of study validate the use of naturally occurring events in the RCS rat to provide a model of vascular pathology in human retinal degenerative diseases. This contrasts with previous models, which have relied on creating wounds to simulate conditions that occurring in the diseased human retina.

This Thesis reports on a study of the ways in which officer cadets study the Collision Regulations at Britannia Royal Naval College (BRNC), which is widely held to be difficult and, anecdotally, leads to a high failure rate on testing. The research is opportunistic since my background both in the Royal Navy and as a civilian lecturer at BRNC facilitated unprecedented access and the potential for original discoveries. I took a mixed-methods approach with both quantitative and qualitative features within a pragmatic paradigm, based upon an eclectic selection of methodological models to suit the case, in order to achieve a measure of triangulation upon the learning with the aim of understanding it: I chose not to be bound by anyone theoretical perspective. In order to facilitate this I set five research questions. My aim was to add to the existing body of research on learning in order to generate some practical recommendations for improving learning the Collision Regulations to the benefit of the College and the wider seafaring community. I reached four principal conclusions: first, that there were a number of practical ways in which the learning could be improved; second, that the biggest single factors in determining success or failure were the attitude towards learning and the confidence exhibited by the students; third, that the failure-rate is not as high as expected and the majority of students at this level do not find it as difficult as anecdotal and historical evidence suggests and fourth, that to describe and understand the unique context of learning at BRNC requires a combination of several theoretical approaches to learning.

In recent years, patient-centredness has become a central focus in improving health care quality. In 2010, the Canadian Medical Association (CMA) launched a four-year action plan aimed at transforming Canada’s health care through a framework aimed at creating a culture of patient-centred care, accountability and responsibility. Several of Canada’s provincial governments proceeded to launch patient-centred action plans, including the Government of Ontario’s “Patients First” framework, which prioritizes patient engagement and increased transparency. As an example of how organizations are putting these values into practice, the College of Physicians and Surgeons of Ontario (CPSO)’s transparency initiative, which aims to make more physician-specific information available to the public in order to help patients make informed decisions about their health care, was examined. This thesis asks how physicians and members of the public feel that the transparency initiative aligns with the CPSO’s public interest mandate. Using discourse analysis, 226 responses by physicians, members of the public and organizations on a discussion forum in the Policy Consultations section of the CPSO’s website were analyzed in order to identify the main themes in arguments for or against increased transparency. The results show that physicians and members of the public tended to differ in their views on the purposes and probable outcomes of the CPSO’s transparency initiative. The majority of physicians worried about patients’ ability to accurately understand and utilize the information being provided to them, and the negative impact that this might have on individual physicians and on the physician-patient relationship more broadly. Most members of the public had a more positive outlook on the potential for transparency to build public trust, help patients become informed and engaged decision-makers and improve patient safety.

Background and aim: An audit of the Fellowship of the College of Surgeons FCS (SA) Final Examination results has not been previously performed. The purpose of this study was to review and determine any predictors of outcome. Methods: The results of the FCS (SA) Final Examinations from October 2005, to and including, October 2014, were retrieved from the College of Medicine of South Africa database. The current format of the examinations consists of: two written essay question papers, an OSCE, two clinical cases and two vivas. These were retrospectively reviewed and analyzed. Predictors of failure or success were determined. Analysis was performed using IPython for scientific computing. Assumptions for the normal distribution of numerical values were made based on the Kolmogorov-Smirnov test and quantile-quantile plots. Normally distributed variables were analyzed by parametric tests. In all other cases nonparametric tests were employed. An alpha value of 0.05 was chosen to indicate statistical significance, using a confidence level of 95%. Results: During the 10-year study period, 472 candidates attempted the examinations. A total of 388 (82,2%) candidates were successful in the written component of the examination and were subsequently invited to participate in the oral/clinical component of the examinations. 9 Overall, 296 (62,7%) of candidates passed and 176 (37,3%) failed. A total of 19 candidates achieved less than 50% for both papers, yet still managed an average of more than 45%. A total of 15 (79%) of these candidates went on to fail the examination. There were 51 candidates who were invited to the oral examinations despite an average of less than 50% in the two papers, and 34 (67%) failed the overall examination. Similarly, 126 candidates were invited having failed one of the two papers of which 81 (64.3%) ultimately failed. A total of 49 candidates failed the OSCE, 82% of these candidates failed overall. There was strong correlation between paper one and paper two (r = 0.56, p-value < 0.01), oral one and oral two (r = 0.41, p-value < 0.01) and case one and case two (r = 0.38, p-value < 0.01). Similar correlations were seen between the averages of the papers versus the orals (r = 0.52, p-value < 0.01), the papers versus the cases (r = 0.5, p-value < 0.01) and the papers versus the OSCE (r = 0.54, p-vale < 0.01). Conclusion: The written papers are the main determinant of invitation to the second part of the examination. Candidates with marginal scores in the written component had an overall failure rate of 67%. Failing one paper and passing the other, resulted in an overall failure rate 64,3%. Failing the OSCE resulted in an overall 82% failure rate. With the high failure rate of candidates with marginal scores and with the inter-examination variability of the papers, it might be prudent to revisit both the process of invitation selection and the decision to continue with the long-form for the written component.

The College of Physicians and Surgeons of Ontario (CPSO) recently published its revised policy, “Physicians and the Ontario Human Rights Code,” which establishes professional guidelines pertaining to conscientious objection. Insofar as it compels complicit action on the part of objecting physicians, the policy has engendered controversy within religious, legal and bioethical communities in Canada. To provide insight into this debate, my dissertation examines the CPSO's guidelines through the lenses of Roman Catholicism, Canadian law and the ethical framework of principlism. Whereas analysis reveals tension between the CPSO's position and the Roman Catholic doctrines on conscience and cooperation in evil, general consistency exists between the policy statement and the treatment of conscience and religion within Canadian jurisprudence. Through the lens of principlism, consistency between the policy statement and the principles of respect for autonomy, beneficence and justice is punctured by ambiguity between the CPSO's position and the principle of nonmaleficence, as well as conflict between the guidelines and respect for physician autonomy.

Dans la mesure où elle exige une action de la part des médecins, la politique a engendré une controverse à travers les communautés religieuses, légales et bioéthiques canadiennes. Afin de donner un aperçu de ce débat, ma dissertation examine les lignes directrices du CPSO selon les perspectives du Catholicisme Romain, de la loi Canadienne et du cadre éthique principisme. Bien que certaines analyses révèlent des tensions entre la position du CPSO et la doctrine Catholique Romaine sur la conscience et la coopération en mal, il existe une cohérence générale entre la politique et le traitement de la conscience ainsi que de la religion dans la jurisprudence canadienne. Dans l'optique du principisme, la constance entre l'affirmation de la politique et les principes du respect de l'autonomie, de la bienfaisance et de la justice est ponctuée par une ambiguïté entre la position du CPSO et le principe de la non malfaisance, de même qu'un conflit entre les lignes directrices et le respect de l'autonomie des médecins.

Health care providers in the United States, United Kingdom and Australia debate the need for expanding the role of podiatrists' to include the surgical care of foot problems. Paradoxically, during a twelve month period from July 1995 to June 1996, Fellows of the Australian College of Podiatric Surgeons (FACPS) performed over 1,500 individual surgical operations on approximately 785 Australians. Few prospective investigations of podiatric surgeon outcomes have been conducted, none of which have taken place in Australia. More particularly, no studies have used valid psychometric instruments to measure the effects of care provided on patients' "health related quality of life". The research contained in this thesis deals with the conceptualisation, development and validation of a new health status instrument: the Foot Health Status Questionnaire. This instrument has been developed with the specific intent of investigating the impact of FACPS on patients' health related quality of life. One hundred and forty-two subjects treated by eleven Fellows for orthopaedic, neurological or integumentary systems diseases of the foot were recruited into a six month long quasi-experimental repeated measures (time series) study. The study identified that the vast majority of subjects (more than 92%) who underwent foot surgery experienced significant improvements' in a range of health related quality of life dimensions and indicated that they would undergo their procedure again. In particular, a MANCOVA analysis demonstrated that subjects' reported reduced levels of foot pain, increased levels of physical function, improved general foot health perception and footwear related quality of life, up to six months after their respective operations. Adverse effects of surgery identified in this study include a significant short-term functional disability for subjects' undergoing orthopaedic correction of foot problems and, in the immediate post operative phase, a significant reduction in social function for all three groups of subjects'. Generic measures of General Health and Vitality, as captured by the Short Form 36 health status instrument, were unaffected by Fellows treatment. This study did not identify any significant short to medium term morbid outcomes. Assessment of patients' satisfaction with surgery one, three and six months postoperatively reflected a general under-reporting of the beneficial effects of foot surgery. These findings support the premise that; specific health related quality of life measures provide significant explanatory power about the outcomes of care compared with the more traditional approach of evaluating patient satisfaction with surgery. In summary, it has been recommended that podiatrists, like other health care professionals, use recognised methods to determine whether their care meets professional standards and to generate evidence to prove that it does. This research contributes to meeting this important public health need.

The Royal College of Art is considered through its teaching of art and design, and its work as a centre for the training of art teachers. The ideas of some of the staff are evaluated with regard to the need for art and design education. The influence of the diplomates of the College on the areas of education, art and design is appraised with a view to assessing the value of the work of the College. The relevance of government bodies to the Royal College of Art is examined in some detail, notably the Board and Ministry of Education, the Board of Trade and the Treasury. The relationship between the Civil Servants and the College Principals, Visitors and College Council are considered. The extent to which the College was prevented from achieving its original aims and objectives is explored. This is appraised together with examples of criticism the College received from government circles and external bodies. How such criticism was adapted for future educational policy at the College is also noted. When the Royal College of Art obtained independence from the Ministry of Education the College established its status as a post-graduate institution and was able to address the requirements of modern design education. The Appendices provide details of the Royal College of Art's chronology of events, statistical information and summarised results of a questionnaire given to ex-students.

Formal agricultural education in England came into existence with the opening of the Royal Agricultural College, Cirencester, Gloucestershire in 1845. For many years, it remained the sole agricultural college in the country. This begs the questions why was it created at this time, and in this place? Why was its example not widely copied? The original intention of the founders was to provide agricultural education for the sons of working farmers in the Cotswolds, but this could not be sustained, and the target group and the catchment area were soon changed. What brought about these changes? In seeking answers to these, and other questions, the key role of a single individual, R. J. Brown, is examined. He was acknowledged as the originator of the plan to create an agricultural college. Having no English precedent, Brown looked abroad for models, on which to base his proposals and arguments. Some of the models he chose are examined for possible sources of inspiration and influence. The roles of other individuals and groups involved in the development of the Royal Agricultural College are explored. Attention is given to the fact that the College, which became a national institution, was launched by a local Farmer's Club, at a time when local and county Agricultural Societies were flourishing and the Royal Agricultural Society of England had been in existence for seven years. Brown was not the only individual to outline proposals for establishing an agricultural college. Two similarcontemporary schemes, for Kent and for Yorkshire are included for comparison, both of which failed. The foundation of the Royal Agricultural College in England is seen as part of a diffusion process starting with the pioneering work in formal agricultural education in Switzerland and Germany and its gradual spread that included the creation of establishments at Templemoyle in Northern Ireland and at Grignon in France. The creation of the Royal Agricultural College is regarded as an innovation, the result of a directed social programme, with Brown as the change agent. Use is made of Beal1s Construct of Social Action to discuss the process of the development of the Royal Agricultural College and the start of formal agricultural education in England. A retrospective view from 1907 of the achievements of the Royal Agricultural College is attempted, in the light of the evidence offered by the Principal of that time

BACKGROUND: Predicting potential complications from surgery is a crucial step to aid decision to operate. The American College of Surgeons (ACS) initiated the National Surgical Quality Improvement Program (NSQIP) which collects and analyses patients’ outcomes from surgery. ACS NSQIP developed a Surgical Risk Calculator (RC) to predict risks of postoperative complications. Aim of this study was to assess RC accuracy for predicting complications in patients undergoing colon resection. METHODS: Validation study with secondary use of administrative data conducted in a tertiary care center. Patients who received colorectal procedures in our Enhanced Recovery After Surgery (ERAS) program from November 2013 to December 2015 were enrolled. RC predictions were calculated and compared with observed NSQIP outcomes within 30 days follow-up. Observed versus predicted outcomes were compared. RC accuracy was assessed by graphical examination of the model calibration for outcomes that exceeded 50 events. Predicted versus observed length of stay (days mean±SD) was compared. RESULTS: A total of 368 patients were enrolled. RC predicted versus observed outcomes (n) were: serious complication 40.3 vs. 51; any complication 60.5 vs. 70; surgical site infection (SSI) 31.8 vs. 51; pneumonia 5.8 vs. 15; cardiac complication 16.5 vs. 9; urinary tract infection 9.8 vs. 11; venous thromboembolism 4.8 vs. 4; acute renal failure 16.6 vs. 5; return to operating room 14.6 vs. 6; death 4.2 vs. 2; Discharge to facility 20.2 vs. 12. Good calibration was observed for any complication and serious complications. SSI was underestimated but RC adjustment by surgeon improved SSI prediction. Length of stay was inaccurately predicted: 4.4±1.3 predicted versus 8.6±12.1 days observed (p <0.01, Wilcoxon Rank Sum Test). CONCLUSION: Application of RC in our population closely predicts serious and any complication but less accurately predicts SSI unless adjusted by surgeon and inaccurately predicts length of hospital stay. All outcomes including the above require analysis of greater number of events to permit final conclusions on RC use.

Background: The American College of Surgeons guidelines suggest the use of intact gloves, double gloving, hands-free zone technique to pass sharp instruments, and blunt tip suture needles to protect patients, as well as the surgical team. This study estimates the extent to which these guidelines are followed in a large academic health system. Methods: Over a two-month period in the spring of 2010, 320 general surgical attendings, subspecialty surgical attendings, and surgical resident physicians practicing at a large academic health system, were approached during or after surgical conferences to participate in a cross-sectional study. Nearly 1/3rd completed an anonymous and voluntary self-administered survey. The survey included questions regarding knowledge of each technique, beliefs about effectiveness of each technique, and adherence to the guidelines. Responses were compared by surgeon rank. Results: Awareness of ACS recommendation guidelines was high among surgical attendings (68%) and residents (60%). While 60% of residents adhered to these recommendations, only 43% of attendings adhered. Both attendings (65%) and residents (64%) had similar negative perception toward double gloving in terms of tactile sensation and hand free zone hindrance during procedural operations during cases. Forty percent of residents and attendings agreed on unhindered concentration to hand free zone technique. Blunt tip suture needle use had low awareness and usage regardless of surgeon rank (~40%). Conclusion: Increased promotion of the ACS guidelines is warranted. Continuing medical education for surgical attendings may promote more widespread adoption of techniques to promote safety.

This study examines West's life, the Greenwich Chapel history, altarpiece and decorative scheme, and concludes that the designs are an example of West's stylistic flexibility and are related thematically to his Windsor Royal Chapel commission.

The establishment of the Royal College of Music (RCM) in 1883 represents the denouement of an eighteenth-century movement to found a conservatoire with a national remit in Britain. Whether motivated by the desire to rival Continental conservatoires to generate and develop an environment in which a worthy successor to Purcell could be nurtured or to create an indigenous musical workforce to obtain direct control of market forces, the RCM was seen as a panacea in the light of the demise of the experimental National Training School for Music (1876-1882) and the ineffectual Royal Academy of Music founded in 1822. The NTSM's financial concerns led Sir Henry Cole to approach the Royal Commission of 1851 for aid. In return for a meagre grant, the Commission insisted the NTSM remodel its management and constitution on pain of eviction from buildings on the Kensington Estate. Cole's approach to 1851 Commissionets precipitated the involvement of the Prince of Wales and other senior members of the Court that led directly to the establishment of the RCM in 1878.Attempts to institute the RCM as a quango to regulate the music profession alongside music education both at elementary school and university level were intended to provide ideal circumstances for inducing comprehensive treasury assistance where the NTSM failed. When this proved elusive, a contingency was provided by George Grove (first RCM Director from 1882) who, at the request of the Prince of Wales, imtiated a capital fund. The introduction of fee-paying students alongside scholars provided financial security that distanced the College & insolvency. Substantial growth in numbers during the first few years forced Grove and the Council to address the issue of a new building. Grove's appointment of an unrivalled professorial staff and the development of a rigorous curriculum, whose inspiration was to be found within the Continental traditions in France and Germany, had paid dividends. By 1894, the results of RCM's pedagogical methods were respected across Europe. The appointment of Grove's neighbour, Alexander Mackenzie, as Principal of the RAM heralded an environment for mutual co-operation between two rival institutions. The institution of local examinations under the Associated Board of the Royal Academy of Music and the Royal College of Music from 1889 marked the conclusion of further attempts to amalgamate the two institutions. The foundation of both the Associated Board was intended to provide a remedy to the shortage of suitably-qualified candidates entering for scholarships and to improve music tuition among school children as set out in the RCM's 1883 charter. The coalition created formidable opposition to Halle's proposal to establish a chartered Royal College of Music in Manchester (RMCM) in 1893 and Parliament's attempts to include music within the provision of the bill for the regulation and registration of teachers. The foundation of the Associated Board allowed Grove to begin implementing the RCM's remit to lead the music profession on both a national and imperial scale. The RCM's national and European reputation established by Grove was consolidated under the directorate of his successor, c. Hubert H. Parry, who confirmed the RCM's global reputation to which other, fledgling institutions, such as New York's Juilliard School of Music, came to aspke. Grove's initiatives, which began the process of emancipating composer and performer alike, went on to transform Britain's international musical reputation within a generation, the ramifications of which continue to affect us more than a century later.

This thesis analyses the architecture of the Royal Victoria College (Bruce Price, 1896--1899), a purpose-built women's residential college of McGill University, Montreal, and its first extension (Percy Nobbs, 1930--193 1), as material evidence of the rhetorical construction and negotiation of gender. A contextual analysis of the original RVC reveals the gender significance of the building's relationship to its affiliate institution (McGill), to an urban geography (Phillips Square), and to a commercial typology (the railway hotel), while a spatial analysis examines the significance of its women occupants as 'architects', and of changes to the building over time. The thesis concludes that the building served as an important site in turn-of-the-century gender negotiations---one that helped to contest "separate spheres" rhetoric and that stands as evidence of women's active participation in the shaping of spatial relations and social identities.

This thesis is concerned with the selection and early career education of executive branch officers in the Royal Navy c1902-1939. The thesis attempts to place naval selection and educational policy in context by demonstrating how it was affected by changing naval requirements, external political interference and contemporary educational reform. It also explores the impact of the First World War and the Invergordon mutiny upon officer education. The thesis discusses the selection of potential executive officers, exploring what methods were used, why they were used and how they were developed over time. It discusses the increasing openness of the officer corps of the Royal Navy to boys of talent, irrespective of their background; and shows that this trend was driven by political demand, fuelled by the increasing number of well educated lower middle class boys, and welcomed by many in the Royal Navy. The thesis demonstrates that the Fisher-Selborne Scheme of officer education combined existing naval practice with recent educational developments to produce a unique and innovatory educational system. It shows how many of the assumptions on which the scheme was founded were subsequently proven to be wrong, and demonstrates its gradual dismantling through the inter-war years. The thesis considers the development of the Special Entry scheme, initially in response to a shortage of junior officers but later as a means of broadening entry to the officer corps. It contrasts the fortunes of the two schemes in the inter-war period, in which the educational side of the Special Entry scheme was largely unaltered. Overall the thesis seeks to place the development of the Royal Navy’s systems for the selection and early career education of executive officers in context by exploring how and why they were developed and their response to the changing fortunes and shape of the Royal Navy.

The questions are: What are the changes in the choice of examination work for the students of the Royal University college of Fine Arts, and what can they depend on? Is the school adjusting their education to the surrounding art world, and in what way is that noticeable? By studying the catalogs from the exhibitions, newspaper reviews and other literature, a shorter conclusion is given concerning the art world, the study situation and the student’s choices of examination works the affected years. The works has gone from being a submission of the study fields during the year, to becoming free projects where stories about important subjects are told. Pressures on the school from students and teachers to be able to keep up with the surrounding view of art in the society that is constantly changing, together with reforms and investigation has led to these changes.

This thesis is an examination of policy-making and practice in design education in England from 1837-1992. It takes a longue durée approach to the history of the development of design education to provide a new narrative which shows a pattern of recurring debates concerning the purpose of design education and how it should be taught. Using the curricula of furniture design courses at three art schools to illustrate the way policy was put into practice, this thesis argues that historical context is key to understanding why debates regarding the way designers should be trained for industry have recurred since 1837. Based on a wide variety of primary source material the thesis contributes to historiography by extending the scope of previous histories of art and design education, and also, for the first time, focuses solely on the development of design education, whilst acknowledging its place in the wider development of art and design education. Following the introduction, chapter two of this thesis examines the events which led to the 1835-6 Select Committee and argues that many of the issues raised during the Committee influenced the teaching of design education through the remainder of the nineteenth century; this is further demonstrated through chapter three. Charting the development of design education into the twentieth century through chapters four, five and six, this thesis shows that changing historical contexts, such as the development of industrial methods or wider changes in higher education, have also had an impact on design education. In the light of changing historical contexts, policy makers for design education have continually questioned what design students should be taught and how they should be taught, which accounts, in part, for the recurring nature of debates in design education.

This was a study on the Utah cattle industry which compared five different feeding enterprises. These feeding enterprises included feeding cull cows, finishing beef yearling steers, finishing Holstein yearling steers, backgrounding beef steer calves, and backgrounding Holstein steer calves. The main purpose of this study was to determine which feeding enterprise was the most profitable for Utah cattle producers. Another objective of the study was to determine if LRP insurance lowered the volatility in the returns to these feeding enterprises. In order to answer these two questions of interest, a historical analysis of Utah cattle and feed prices was conducted from 1990 through 2009. Weekly sales data were used, and seasonality and price trends were determined. Next, enterprise budgets were created for each feeding enterprise to establish historical returns. Then, using the historical data as a foundation, a simulation analysis was run to forecast future returns and determine the risk associated with each feeding enterprise. LRP insurance was also added to the model to simulate the effects it had on lowering risk. After completing a simulation analysis and comparing means and standard deviations of the expected returns, portfolio theory was used to put the feeding enterprises into different portfolios to attempt to lower risk. Then stochastic dominance was used to conclude which feeding enterprise was the most preferred for Utah cattle producers. The results of the study depend upon the producer's level of risk. The majority of producers have an ARAC value between -0.0002 and 0.0012. With that knowledge, the results suggested that the majority of Utah cattle producers should finish Holstein yearling steers. If a producer was highly risk seeking, then he or she was better off to feed cull cows. If the producer was highly risk averse, then he or she preferred a portfolio of cull cows and backgrounding both Holstein and beef steers with LRP insurance. The results of the study also indicated that LRP insurance was an effective tool for lowering the variability in expected returns. However, the results suggested that the most preferred option for Utah cattle producers was to feed either cull cows or Holstein yearling steers without LRP insurance.

This thesis describes some of the features of general practice consultations in a clinic in India with reference to the Clinical Skills Assessment (CSA) of the United Kingdom Royal College of General Practitioners. There is a significant difference in the success rate in this postgraduate licensing assessment between those doctors trained in India and those trained in the UK, the reasons for which are not known. Some doctors from India feel that this is in part due to family medicine being performed differently in India. The results presented here first explore the reported experience of doctors working in family medicine in India through focus group and interviews looking at contextual aspects of practice; and then through conversation analysis explore the work done by talk-in-interaction in video recordings of actual family medicine consultations in India; a unique study. The CSA heavily emphasises the assessment of talk as used in three domains - data gathering, clinical management and interpersonal skills. I will propose a definition of ‘interactional fluidity’, based on the expectations of RCGP examiners about markers of competence, and consider its implications in this high stakes assessment process. Using a model that differentiates between ‘core business work talk’ , ‘work-related talk’ , ‘social talk’ and ‘phatic communion’, which last two are grouped as ‘small talk’, the talk in these consultations will be analysed. The impact of Indian societal norms and the risk of examiners mistaking unfamiliar patterns of talk for lack of medical competence are discussed. Reflecting on the journey from clinician-educator to practitioner-researcher the thesis describes the impact the study has had on the personal practice of the author and also the implications for maintaining fairness in training and assessing international medical graduates within the UK.

L'étude comprend la description, la localisation ainsi que le profil linguistique de chacun des manuscrits. Le commentaire repose sur l'analyse des différents types de corrélations existant entre H, T et R, les sources utilisées ainsi que le choix du texte de base. Une politique éditoriale détaillée contenant une étude des abréviations recensées dans les trois manuscrits clôt la première partie. La seconde partie est composée de quarante textes édités à partir de H et quatorze dont deux exempla à partir de R, accompagnés d'un apparat. In this thesis I present new insights into aspects of Peter Guthrie Tait's life and work, derived principally from largely-unexplored primary source material: Tait's scrapbook, the Tait–Maxwell school-book and Tait's pocket notebook. By way of associated historical insights, I also come to discuss the innovative and far-reaching mathematics of the elusive Frenchman, C.-V. Mourey. P. G. Tait (1831–1901) F.R.S.E., Professor of Mathematics at the Queen's College, Belfast (1854–1860) and of Natural Philosophy at the University of Edinburgh (1860–1901), was one of the leading physicists and mathematicians in Europe in the nineteenth century. His expertise encompassed the breadth of physical science and mathematics. However, since the nineteenth century he has been unfortunately overlooked—overshadowed, perhaps, by the brilliance of his personal friends, James Clerk Maxwell (1831–1879), Sir William Rowan Hamilton (1805–1865) and William Thomson (1824–1907), later Lord Kelvin. Here I present the results of extensive research into the Tait family history. I explore the spiritual aspect of Tait's life in connection with The Unseen Universe (1875) which Tait co-authored with Balfour Stewart (1828–1887). I also reveal Tait's surprising involvement in statistics and give an account of his introduction to complex numbers, as a schoolboy at the Edinburgh Academy. A highlight of the thesis is a re-evaluation of C.-V. Mourey's 1828 work, La Vraie Théorie des quantités négatives et des quantités prétendues imaginaires, which I consider from the perspective of algebraic reform. The thesis also contains: (i) a transcription of an unpublished paper by Hamilton on the fundamental theorem of algebra which was inspired by Mourey and (ii) new biographical information on Mourey.

Selection of trainees for surgical training is widely acknowledged to be both complex and important. Doctors embarking on surgical careers expend considerable time and resources in training, as do those responsible for facilitating their learning. The aim of surgical training is to prepare surgeons to be competent, effective, ethical practitioners in unsupervised clinical practice as consultant surgeons (Carroll, Kennedy, Traynor & Gallagher, 2009; Elfenbein, Sippel, McDonald, Watson, Scarborough & Migaly, 2015). The aim of selection must be to admit those who are most likely to succeed in surgical training and beyond. Admitting candidates who are inadequately suited to surgical training can result in trainees who unduly struggle or are unable to satisfy training requirements, and may ultimately jeopardise patient safety. In contrast, not accepting candidates who are well-suited to surgical training may be unfair to both candidates and communities that miss out on skillful surgeons. These aspects are among those that make selection to surgical training an extremely ‘highstakes’ activity. Those responsible for selection grapple with assessing candidates’ current skills and attributes, and with ascertaining their likely future performance. Limitations of, and tensions between interacting elements—human agency, requirement specifications, instruments, processes, influences—in selection to surgical training, mean that the long-term outcomes of selection can never be assured. This study identifies principal elements in selection to surgical training, discusses influences on, and interrelationships between the elements, and reviews connections between selection and surgical training assessments. This study has appraised the current instruments used for selection into the Royal Australasian College of Surgeons’ (RACS) General Surgery (GS) training program in Australia and New Zealand, to establish their effectiveness in predicting trainees’ performance in assessments during the first two years of training. Data were considered for selection and assessment items for trainees for three yearly cohorts, selected in 2008, 2009, and 2010. The study compared the performance of trainees in three selection instruments—a structured CV, a structured referee report, and a multi-station interview—to their performance in three examinations and three work-based assessments. Firstly, Pearson product-moment correlations were calculated to examine the extent of relationships within each set of variables to determine intra-relationships of the selection items, of the examination items, and of the work-based assessment items. Secondly, Pearson productmoment correlations were conducted to determine degrees of association between selection items and performance in each of the subsequent assessment items. Thirdly, multiple regression analysis was conducted to determine the extent to which trainees’ scores in the selection items (independent variables) predicted scores in each of the assessments during training (dependent variables). The model fit and strength was assessed using the analysis of variance (ANOVA) step within the regression analysis. The relative strength of the associations between dependent variables and the independent variables were assessed using regression coefficients. The findings of this study have shown that performance in RACS GS selection partially predicts performance in assessments during training. In general, candidates’ performance varied across each of the selection items, performance in all examinations was highly consistent and performance in major end of term work-based assessments was also consistent. Most correlations between performance in selection and performance in assessments during training were positive, with the exception that performance in the CV was usually inversely correlated with subsequent assessments. The referee reports and total selection scores were predictive of performance in the major end of term work-based assessments. This study reviewed many factors that affect selection processes and outcomes. Several of these—such as the role of procedural justice, identification of desired attributes of trainees, and the validity, reliability, fairness, and acceptability of selection instruments and protocols— implicitly framed RACS GS selection. However, to maximise the effectiveness of selection instruments and protocols, these and other relevant influences could be explicitly defined for the local context. The findings regarding the predictive capacity of the selection instruments— particularly for the referee reports and the interviews—differ from some other studies, but highlight that the implementation and content of selection instruments are key aspects affecting their performance. Combining scores from multiple instruments reduces the influence of any individual selection instrument. Overall, with the possible exception of the CV, the study has shown that the RACS GS selection tools are performing moderately well. However, the RACS GS selection instruments and processes could be modified to maximise their effectiveness and new, emerging selection activities could be considered.

The retinal remodeling is known in a human inherited disease, retinitis pigmentosa (RP), which leads to the loss of vision caused by photoreceptor degeneration. It has been reported in the Royal College of Surgeons (RCS) rat which is one of animal models for studying human retinitis pigmentosa that the degenerated photoreceptors cause a dramatic reorganization of retina, including the inner nuclear layer and the ganglion cell layer, at later stages in the RCS rat. At present treatments for restoring vision in RP founded on the intact properties of remaining neurons, and the remodeling of retinal degeneration at later stages were neglected. However, the morphological features of retinal ganglion cells (RGCs) in the RCS rat at later stages of degeneration have not been carefully studied. Therefore, the goals of this study were to characterize the properties of RGCs at later stages in morphology and electrophysiology. In the present study, we characterized morphological properties of RGCs in two aged groups of RCS rats (P331-418 and P526-575) and two strains of normal rats (SD and LE). In addition, examination of the physiological properties of RGCs in SD and RCS rats was also attempted. The responses of RGCs upon light and electrical stimulations in SD rats, and those upon electrical stimulation in RCS rats were recorded. Microinjection with Neurobiotin was used to assess the dendritic morphologies and coupling patterns of RGCs. In collaboration with Prof. CY Wu (National Chiao Tung University) in retinal prosthesis program, the electrical stimulation was provided by activating a silicone-based microphotodiode array with a 532 nm laser from the subretinal side. Although we did not find RGC responses upon electrical stimulation in the RCS rat, the morphological features of RGCs, including total dendritic length, number of branching points, appearance of dendritic beads, and tracer coupling patterns in the RCS rats were significantly different from those in the normal rats. The finding indicates that RGCs in the RCS rat may undergo significant changes during the retinal remodeling at later stages of degeneration. This also suggests that the strategy of treating RP at later stages cannot assume intact RGCs.

Lebone II College of the Royal Bafokeng, the first private school initiated with the wealth generated from the platinum mining of the Royal Bafokeng Nation comes into the limelight after many years of work. Like a butterfly, the school has undergone a metamorphosis of ideology, philosophy and geography to make it what it is today. The principal tenet of this institution was to produce agents of social change within the Royal Bafokeng Nation schools but at the same time geared at being the key instrument for the broader education strategy of the Royal Nation project. A large majority of students in this school were selected from the public schools of the Royal Bafokeng Nation. As such, this research investigates the experiences of the students as they transition from the public to the private schools arena so as to determine if the envisaged goal is being achieved.

RESUMO: O Royal College of Psychiatrists tem um programa de voluntariado internacional desde 2005. O interesse no Reino Unido tem crescido nos últimos 15 anos com muitos projetos novos. Os benefícios para vários países têm sido reconhecidos. O que está menos claro são os benefícios da experiência para o Reino Unido e do programa de voluntariado do Royal College of Psychiatrists. O objectivo desta dissertação é explorar os benefícios do voluntariado para o Sistema Nacional de Saúde focando principalmente, mas não exclusivamente, o programa de voluntariado do Royal College of Psychiatrists. Nesta dissertação abordamos primeiro o contexto antes de discutirmos dois grandes estudos de psiquiatras e hospitais do Sistema Nacional de Saúde no Reino Unido. Incluimos no estudo todos os psiquiatras registados como Voluntários no Royal College of Psychiatrists e o Grupo de Interesse Especial associado. Foi ainda possível incluir os Diretores Médicos de todos os hospitais do Reino Unido. Os estudos foram desenhados para analisar as opiniões dos Voluntários do Reino Unido sobre os benefícios para os países recetores de baixo e médio rendimento e para o Reino Unido. Todos os 60 hospitais do Sistema Nacional de Saúde foram incluí dos para analisar a opinião sobre Voluntariado. As limitações dos estudos foram a baixa taxa de resposta, mas esta foi comparável a outros estudos do College. É provável haver um enviezamento de resposta e favorecimento dos que estão envolvidos no Voluntariado. Os resultados mostraram um interesse forte no voluntariado e benefícios no Sistema Nacional de Saúde, nas. áreas de liderança, transculturalismo, gestão e recursos e capacidades pessoais. Os hospitais do Sistema Nacional de Saúde valorizaram o voluntariado e, em contrapartida, o valor acrescido para competências profissionais. Os obstáculos contra o Voluntariado a nível individual e dos hospitais foram maioritariamente de natureza prática, tais como obter dispensa do trabalho, substituição no trabalho e custos. As implicações destes estudos são que o programa de voluntariado do College precisa de ser fortalecido. Para assegurar que este trabalho continua é necessário existir uma sensibilização nacional e maior responsabilização sobre os benefícios para o Reino Unido e a nível global. -------------------------- ABSTRACT: The Royal College of Psychiatrists has had an international volunteering programme since 2005. The interest in UK has grown over the past 15 years with many new projects. The benefits in various countries has been acknowledged. What has been less clear are the benefits of the experience back in the UK and of the Royal College of Psychiatrists Volunteer Scheme. The aim of this dissertation is to explore the benefit of volunteering to the NHS focusing mainly, but not exclusively on the Royal College of Psychiatrists Volunteering scheme. In this dissertation we first look at background information before discussing two large surveys of Psychiatrists and NHS Trusts in UK. We surveyed all those registered as Volunteers at the Royal College of Psychiatrists and the associated Special Interest Group. We also were able to survey the Medical Directors of all UK Trusts. The Surveys were designed to assess views of UK Volunteers of benefits to hosts in LMIC and back in UK. All 60 NHS Trusts were surveyed to assess the view of Volunteering. Limitation of the surveys were the low response rate but this was comparable to other College surveys. There is likely to be a bias in response and favour those who are engaged in the Volunteering agenda. Results showed a strong interest in volunteering and perceived benefits in NHS. These areas included leadership, transcultural, resource management and personal skills. NHS trusts valued volunteering and added value to professional competencies on return. Obstacles to Volunteering at individual and Trust level were mainly practical issues such as getting time off, cover and costs. Implications of these surveys are that the College volunteering scheme needs strengthening. There needs to be a National advocacy to ensure that this work continues and greater accountability as to benefits in UK and globally.

A eficiência energética do parque imobiliário é um tema que tem ganho expressão nos últimos anos. O Sistema de Certificação Energética, que surge na sequência da implementação da diretiva europeia EPBD, permitiu estabelecer medidas para atingir os requisitos mínimos relativos à eficiência energética nos edifícios. Porém, existe um grupo de imóveis que estão excluídos deste sistema de certificação como é o caso dos Edifícios de Elevado Valor Patrimonial.Este trabalho teve como objetivo analisar as implicações da adoção de medidas de eficiência energética em intervenções que devem salvaguardar os valores existentes. Para esta análise foi necessário avaliar as Orientações, Cartas e publicações da UNESCO a fim de investigar quais as orientações relacionadas com a salvaguarda dos valores culturais, arquitetónicos e construtivos, assim como a existência ou não de linhas orientadoras quanto ao desempenho e eficiência dos edifícios classificados; a legislação europeia a fim de perceber quais as metas e considerações estabelecidas pela Europa para a diminuição do CO2; e a legislação portuguesa esclarecendo o que é que está abrangido pelo Sistema de Certificação Energética. Foram também estudadas algumas boas práticas de intervenções em edifícios europeus com valor cultural reconhecido.O caso de estudo utilizado para este trabalho foi o Real Colégio das Artes, pertencente ao conjunto Universidade de Coimbra – Alta e Sofia, inserido na Lista de Património Mundial da UNESCO em 2013. Com base na legislação europeia, nacional e orientações da UNESCO, foram traçadas algumas medidas de intervenção que pretendem melhorar a gestão energética do edifício fazendo um equilíbrio entre a preservação dos valores culturais e a eficiência energética.Foi possível constatar que, no caso do Real Colégio das Artes, existem várias opções projetuais passíveis de serem aplicadas para cada um dos elementos construtivos equacionados, com maior ou menor impacto nos valores culturais e arquitetónicos e com maior ou menor resultados ao nível do desempenho térmico. Para este caso específico, é possível afirmar que pode existir uma correlação entre o grau de desempenho energético que se pretende atingir e o seu impacto nos valores culturais e arquitetónicos do edifício.
The energy efficienncy of the building stock is a theme that has been developed in recente years. The Energy Certification System, wich follow on from the implementation of the European Directive EPBD, has enabled the establishment of measures to meet the minimum requirements for energy efficiency in Buildings. However, there is a group of buildings that are excluded from this certification system: High-Value Buildings.This paper aimed to analyze the impact of energy efficiency measures in interventions that should safeguard existing values. For this analysis it was necessary to study the UNESCO Guidelines, Letters and Publications in order to know wich guidelines are related to the safeguarding of cultural, architectural and constructive values, as well as the existence or not of guidelines regarding the performance and efficiency of classified buildings. It was necessary to study also European legislation to understand the goals and considerations set by Europe for the reduction of CO2 and Portuguese legislation clarifying what is covered by the Energy Certification System. Some good practices of interventions in European buildings with recognized cultural value were also studied.The case study used in this project was the Royal College of Arts, belonging to the plot Universidade de Coimbra – Alta e Sofia, included in the UNESCO World Heritage List in 2013. Based on European National and UNESCO guidelines, some intervention measures were proposed to improve the energy management of the building by balancing the preservation of cultural values and Energy efficiency.It was possible to realized that in Royal College of Arts, there are several design options that can be applied, with more or less impact on cultural and architectural values and with more or less results in terms of termal performance. For this particular case, it’s possible to say that can exist a correlation between the degree of Energy performance to be achieved and its impact oh the building’s cultural and architectural values.

In this thesis, there is an investigation into the factors that contributed to the ascendancy of printmaking in South Australia in the 1960s and the development of political printmaking in the 1970s. An analysis of key individuals is contextualised within the institutional and political frameworks operating in Adelaide at this time. An important aspect of this thesis is the examination of the transition from teaching craft and trade-based print subjects to fine art printmaking courses at the South Australian School of Art (SASA), one of the oldest art schools in Australia. Some of the research was based on the SASA archival material at the University of South Australia, which included the prospectus booklets, presentation of diplomas and prizes leaflets, SASA principal’s reports, and The Advertiser newspaper listings of students’ results. Paul Beadle and Charles Bannon were responsible for key developments in printmaking in South Australia. Beadle was a dynamic and far-sighted principal of the SASA from 1958-60. Bannon taught at St Peter’s College, where he instituted a ‘Bauhaus-style’ education methodology in the preparatory school. When Bannon was placed in charge of high school classes, he chose German printmaker Udo Sellbach to carry on his educational methods in the preparatory school. Beadle invited Sellbach to set up a graphics studio at the SASA and Sellbach and his then wife, Karin Schepers, became leading figures in the revitalisation of fine art printmaking in South Australia. Case studies of Charles Bannon, Barbara Hanrahan, Ann Newmarch and Olga Sankey are employed to extend the thesis narrative of printmaking education and professionalism in South Australia. In each case study, the formative years, studies, overseas travel and printmaking careers are considered in relation to their contribution to printmaking in South Australia. Despite the outstanding achievements of printmaking in Adelaide in the 1960s and 1970s, this has been a neglected area of research. In this thesis, important new research is presented and a number of reasons are canvassed as to why there was a subsequent contraction in printmaking in South Australia, especially in relation to the national context.
