Objective. To examine changes in neighbourhood income-related differences in 'avoidable', and other cause, mortality in urban Canada from 1971 to 1996. Data source. Canadian Mortality Database and population censuses for 1971, 1986, 1991 and 1996. The deaths were coded to census tract and grouped into neighbourhood income quintiles. Methods. Deaths were classified as amenable to medical or public health interventions according to seven selected classification lists. Age-standardized period expected years of life lost (SEYLL) were calculated. Quintile differentials were determined using rate ratios and rate differences. Results. From 1971 to 1996, the avoidable SEYLL differences between the richest and poorest quintiles diminished 58.5--72.1% for men and 56.4--82.2% for women, depending on the classification list considered. The SEYLL differences relating to other causes increased 8.0% in men and decreased 0.5% in women. Conclusion. Deaths amenable to medical and public health interventions contributed to reducing socioeconomic mortality disparities in urban Canada.

This study assessed the predictors of the decline in physical activity levels observed over the course of a 5 year longitudinal cohort of adults aged 18--65 living in two low-income, inner-city neighbourhoods in Montreal. The current study made use of data collected as part of Coeur en Sante St. Henri, an intervention program designed to decrease cardiovascular disease (CVD) risk factors. A two-stage cluster telephone survey of a representative sample of residents was used to collect information on a variety of lifestyle behaviours. Multiple logistic regression was used to assess the independent predictors of decline in leisure time physical activity in 626 subjects. Significant predictors of the decrease in physical activity include age (OR = 1.0 (1.0, 1.1) and BMI (OR = 2.0 (1.1, 3.6), and a composite index assessing self-efficacy pertaining to physical activity (OR = 2.0 (1.2, 3.2), in males. In females, significant predictors include lack of energy (OR = 2.4 (1.2, 4.6), perceived lack of athletic ability (OR = 2.4 (1.1, 5.2), not using a neighbourhood facility for physical activity (OR = 2.8 (1.6, 4.7), BMI (OR = 2.1 (1.2, 3.7), and a composite index assessing self-efficacy pertaining to physical activity (OR = 2.1 (1.3, 3.5).

Objectives. To compare HIV risk behaviours of female street youth involved and not involved in prostitution and to identify predictors of initiation into prostitution.
Methods. Female street youth aged 14--25 years were recruited into a prospective cohort study between January 1995 and March 2000. Parametric and non-parametric methods were used to compare risk factors for HIV infection. Girls with no history of prostitution at baseline were followed prospectively to estimate the incidence of prostitution. Cox regression analysis was used to determine predictors of prostitution.
Results. Significantly higher proportions of girls with a history of prostitution reported engaging in behaviours that put them at risk of HIV infection. Such behaviours included non-injection and injection drug use, unprotected sex and risky sexual partnerships. The incidence rate of prostitution among girls not engaged in prostitution at baseline was 11.5/100 person-years. Independent predictors of initiation into prostitution were being 18 years or younger (Hazard Ratio (HR): 2.2; 95% Confidence Interval (CI): 1.0--4.8), using alcohol everyday (HR: 1.3; 95% CI: 1.1--1.5) and using at least three types of drugs (HR: 5.4; 95% CI: 1.6--18.4).
Conclusions. Girls involved in prostitution exhibited more behaviours that may place them at increased risk of HIV infection compared with female street youth. Young age and substance use characterised by the overuse of alcohol and multi-drug use were found to be independent predictors of initiation into prostitution for female street youth.

Abstract (sommario):
This thesis examines relations between charities and the state in Britain and Canada: it challenges a common view that government responsibility for welfare provision in this century has rendered charities relatively insignificant and isolated from the political process in both countries. By focusing specifically on medical research charities, evidence is presented to show that lobbying has become an increasingly important aspect of their activity, in spite of legal limitations restricting much of their involvement in the policy process. It is concluded that the law restricting charities from engaging in political activities has had limited success both because of its 'vagueness' and poor enforcement. The only countervailing force keeping medical research charities 'out of politics' to any significant degree has come from volunteers and the donating public, but, even they have had only a limited impact. The degree of political involvement by a charity is now contingent on the policy area in which it operates, the degree of 'hostility' of government policy towards the organisation and its objectives as well as the charity's financial resources. In light of cut-backs in government expenditure to medical research in the 1980s, of the need to co-ordinate scientific investigations, and of pressures from some volunteers to represent the interests of disease sufferers, as well as a number of other factors, British and Canadian medical research charities have been drawn increasingly into the political process. This evidence suggests that charity-state relations have changed dramatically since the 19th century when charities not only resisted state encroachment into many areas of social welfare, but devoted much of their resources towards encouraging state withdrawal from areas where tax revenues were already being applied. Now charities frequently criticize government policies aimed at cutting-back state funding for programmes in policy areas where charities are operating and also propose new legislation to ensure minimum levels and quality of state-funded services. Given this, the nature of charity-state relations has changed dramatically and has created difficulties for legislators who have had to reconcile the non-political qualities of philanthropy- including altruism, and community participation - with the reality that much charitable activity is devoted to participating in the policy process. In conclusion the blurring of the distinction between philanthropy and politics has meant that charities have begun to resemble more traditional forms of interest groups while at the same time maintaining their privileged 'tax exempt status'. This is a particularly interesting development given that many British and Canadian medical research charities have been co-opted by pharmaceutical companies to participate in a number of that industry's lobbying campaigns in return for corporate donations.

The first two decades of the twentieth century saw a dramatic period of innovation and expansion in public health programmes in Britain and Canada. This thesis argues that this period of growth and change in public health was one aspect of a major reorientation of social policy. This reorientation had two major features. First, the national working classes of Britain, Canada and other countries were increasingly delimited through immigration controls and similar means of regulating international mobility. Secondly, new social programmes were developed which attempted to improve the physical, mental and moral condition of these delimited working classes, on the basis that their well-being was the foundation of national productivity. Public health played a major role in both the delimitation and improvement of national working classes. In Canada, the first major programme of immigration controls was introduced in this period, centering around the selection or rejection of immigrants on the basis of medical inspection conducted according to public health criteria. In both Britain and Canada, new public health programmes were developed which aimed to improve the condition of the working class. This was to be accomplished primarily through home visiting programmes which attempted through education and Inspection to establish standards for the domestic labour of women as mothers and home-makers. This thesis examines the contribution of public health to this reorientation of social policy primarily through the analysis of the theoretical work of key policy-makers as reported in professional journals and government documents. These officials displayed a keen sociological understanding of the broader significance of their activities in the development of a productive national working class prepared for work or war. Indeed, they understood clearly that the health of nations is an important basis of the wealth of nations.

During the 1960s and 1970s, Canadians increasingly used computers and computerized processes for government and business purposes. By the 1980s, some began to have personal computers at home. This thesis examines the experiences of emerging computerization by focusing on the Public Archives of Canada during this period. The 1960s saw the first computer projects at the Public Archives, although these efforts had mixed reviews. Many archivists feared that automated information retrieval would compromise the quality of their service, and professional position, while others argued that computers were a necessary efficiency to meet the growing demands on their institution. Overall, the decade of the 1960s was one in which many archivists encountered computers, computerized processes and computer records for the first time, and they responded with a range of feelings and reactions. By the outset of the 1970s, a select group of advocates proposed the concept of a Machine Readable Archives. When the creation of this division was approved in 1973, its staff members formed a distinct professional community within the Public Archives. They held a complex position as the computer "haves" of the federal archives and records management community, and the relative "have nots" in their communication with departmental computer personnel. The Machine Readable Archives became the hub of attempted communication and cooperation among all of these players, during the period of major technological development during the 1970s and early 1980s. By the time of the Machine Readable Archives' closure in 1986, computers were frequently used as an archival tool. A survey among archival leaders in the mid-1980s concluded that archivists, once a technologically conservative profession, had not only adopted the use of computers into their repositories, but most of them were optimistic about their profession's role in the evolving technological environment. Archivists' changing views of computers paralleled the increasing acceptance and familiarity of computer technology within Canadian society. To accommodate computerization, many Canadians adapted their work processes, and negotiated new work relationships. In Canada during these years, individuals responded to computers, personally and professionally, in complex and contradictory ways that reflected both reservations and excitement. The Public Archives of Canada, and especially the Machine Readable Archives, provide a significant focus to analyse this dynamic and changing milieu as Canadians engaged with the technological and cultural transformations of the era.

This dissertation is an economic and institutional history of the first comprehensive public health and welfare system in the Western world. Based on previously unexamined archival and archaeological evidence from several European repositories, it argues that the Republic of Venice, at the beginning of the second millennium, implemented legislation of unprecedented scale, intended to regulate and improve the health and standards of living of its population. The Venetian empire, in this period, was unrivaled in its dominance of Mediterranean trade. Economic success and the densifying networks of communications brought new challenges, and new health stresses, including communicable disease, to key commercial hubs under Venetian control, on the Dalmatian coast and islands in the eastern Mediterranean. At this time, a period commonly known as the Commercial Revolution, Venice itself became one of the most populous and wealthiest European cities. The government of the Republic allocated a substantial portion of its surplus revenues to the establishment and funding of new welfare legislation, influenced by Roman and Byzantine legal precedents. The nature of the Venetian parliamentary system gave rise to a host of detailed norms aimed at subsidizing the import of food and primary necessities. In addition, the Republic created and funded the first and largest state-sponsored staff of medical practitioners in Europe, intended to preserve the public's health in the expansive territories under its control. These practitioners were chosen, by and large, on the basis of testimonies of magistrates and patients who vouched for their expertise and reputation. Through a detailed analysis of archival, archaeological and narrative evidence, this dissertation alters our understanding of the development of pre-modern states and their contribution to the creation of what historians have broadly defined "welfare policies." Comparisons between the prices of primary necessities among multiple cities of the Mediterranean test the effects of such policies on the standards of living of European populations. A comprehensive list of all public health infrastructures in Venetian territories outlines the long-term role of the state in the creation and funding of hospitals, hospices and orphanages. By contextualizing new and old evidence, this dissertation argues that, in crafting these new policies, Venetian legislators yielded to economic and political considerations, as well as popular expectations and traditions of evergetism.

This dissertation analyzes struggles over the development of Barbadian health and sanitation during the period between the world wars. In doing so, it examines how the British Empire tried to use development policies to maintain its power overseas during the interwar years. During this period, British policymakers sought to improve health and sanitation to pacify restive Barbadian laborers influenced by transnational pan-African and socialist ideas following the First World War. However, white Barbadian elites, influenced by ideas of eugenics and population control, opposed metropolitan efforts to develop health and sanitation in the colony. Rather than repairing the colonial relationship, British development efforts instead resulted in a protracted legislative and public battle over health reform. White creole resistance to public health policies both destabilized British reform efforts and further undermined black Barbadian understandings of imperial identity. By the 1930s, Pan-African critiques of empire, which the British government had fought to suppress following the First World War, found renewed energy in the midst of British failures to provide basic welfare services to poor black subjects. The fractures in these bonds of empire ultimately resulted in serious labor disturbances that re-emphasized the tensions of British colonialism and redirected the course of imperial policy. By focusing on these conflicts, this project reveals how struggles over colonial reforms on the ground transformed ideas of emerging nationhood, imperial identities, and British strategies of rule in the years leading up to decolonization.

The primary question at issue in this paper is the following: given the similarities between the two countries with regard to welfare state institutions, why have the United States and the United Kingdom diverged on the issue of health care? Drawing on sociological institutionalism, a branch of the new institutionalist paradigm, this paper provides an answer to this question: during the formative years of the health care stories in the two countries, variations in institutional and cultural conditions produced contrasting policy outcomes. More specifically, this paper discusses how the combination of institutions (political, labor, and medical) and culture led to private insurance in the United States and public insurance in the United Kingdom. Of course, this paper has implications for several areas of scholarship, as well as for current policy debates on a wide range of issues.

This dissertation is about influence which is defined as the ability to move ideas forward within, and in some cases across, organizations. More specifically it is about an extraordinary organization called the Club of Rome (COR), who became advocates of the idea of greater use of systems analysis in the development of policy. The systems approach to policy required rational, holistic and long-range thinking. It was an approach that attracted the attention of Canadian Prime Minister Pierre Trudeau. Commonality of interests and concerns united the disparate members of the COR and allowed that organization to develop an influential presence within Canada during Trudeau's time in office from 1968 to 1984.

The story of the COR in Canada is extended beyond the end of the Trudeau era to explain how the key elements that had allowed the organization and its Canadian Association (CACOR) to develop an influential presence quickly dissipated in the post-1984 era. The key reasons for decline were time and circumstance as the COR/CACOR membership aged, contacts were lost, and there was a political paradigm shift that was antithetical to COR/CACOR ideas. The broader circumstances that led to the rise and fall of the COR/CACOR's influential presence in Canada from 1968 to circa 1988 also provides a fascinating opportunity to assess political and intellectual tumult and change.

Starting with the implementation of the world's first universal public pension system in 1913, the report discusses the political as well as the economic background to each major public pension reform up until today. It presents the rules and the institutional details of these reforms and discuss their implications for retirement behavior, the general state of the economy and the political environment. Parallel to the development of the public pension system, a comprehensive and quite complex occupational pension system has emerged. This report describes the historical background and the institutional details of the four largest agreement-based occupational pension schemes in Sweden.

The trickster is a powerful figure of transformation in many societies, including Native Canadian and Maori cultures. As a demi-god, the trickster has the ability to assume the shape of a variety of animals and humans, but is typically associated with one particular form. In Native Canadian tribes, the trickster is identified as an animal and can range from a Raven to a Coyote, depending on the tribal mythologies from which he/she is derived. In Maori culture, Maui is the trickster figure and is conceptualised as a human male. In this thesis, I discuss how the traditional trickster is contexualised in the contemporary texts of both Native Canadian and Maori writers. Thomas King, Lee Maracle, Witi Ihimaera, and Patricia Grace all use the trickster figure, and the tricksterish strategies of creation/destruction, pedagogy, and humour to facilitate the decolonisation of culture within the textual realms of their novels. The trickster enables the destruction of stereotyped representations of colonised peoples and the creation of revised portrayals of these communities from an indigenous perspective. These recreated realities aid in teaching indigenous communities the strengths inherent in their cultural traditions, and foreground the use of comedy as an effective pedagogical device and subversive weapon. Although the use of trickster is considerable in both Maori and Native Canadian texts, it tends to be more explicit in the latter. A number of possibilities for these differences are considered.

Abstract (sommario):
From 1868 to 1911, the Multnomah County Poor Farm off Canyon Road in the Tualatin Hills housed indigent and sick residents of Portland and surrounding areas. In 1911, county officials relocated the Poor Farm from the West Hills flanking Portland to the far eastern portion of the county. Subsequently, the site hosted a municipal golf course and is currently home to the Oregon Zoo and Hoyt Arboretum. With no physical presence left, the original Poor Farm was quickly forgotten, and the reasons for its relocation have been obscured by the passage of time. Occasional references to the farm in newspapers and blogs retell the same story, that county authorities relocated the farm after a 1910 visit by charity organizations revealed atrocious living conditions. In reality, the county had begun scouting land for the new farm two years prior to the charity visit and ensuing newspaper exposé. Conditions at the farm in 1910 may have been bad, but the relocation was not a product of altruism alone. More important was Portland's striving for greatness in the opening of the twentieth century. The early 1900s were heady times for West Coast cities, and as the century opened, Portland was still the largest city in the Northwest and the regional hub for shipping and commerce. A massive development boom, coupled with Progressive-Era reforms around parks and public health, worked to reshape the face of Portland's physical landscape. As the city grew and local boosters sought to promote its image as a prosperous and beautiful metropolis, some leading Portlanders began to see the Poor Farm as a blight on their city. With land becoming more expensive and less available, Portlanders contested who had the right to which parcels and for which purposes. Real estate, public health, and general development fervor combined to make the Poor Farm land seem undeserving of its location. The amending of the Volstead Act and repeal of national prohibition did not answer the "liquor question" but passed the issue to the states. This thesis examines New York's reaction to the change in national alcohol policy and the states decision to legalize and regulate the beverage with the establishment of the New York State Liquor Authority. It traces the activities of Jean Sheppard who led the state division of the Women's Organization for National Prohibition Reform (WONPR) and became one of the key architects of New York's modern alcohol control system. As an expert in alcohol control policies Sheppard developed a plan she believed would be respected by the public while also mitigating the problems associated with alcohol. Sheppard proposed an elaborate system of control which made temperance the objective. Through her position as Chairman of the New York State WONPR Sheppard gained the attention of Governor Herbert H. Lehman who nominated her to the New York State (Conway) Commission on Alcoholic Beverage Control Legislation. As a member of the Commission and then the New York State Alcohol Beverage Control Board, Sheppard was given the opportunity to propose her theories on control. The final legislation creating the New York State Liquor Authority embodied Sheppard's plan in regards to administrative structure but fell well short of her dream of a system that used the full power of the state to put temperance ahead of revenue.

In 2013, the Chicago Public Schools shuttered 53 schools, citing budget limitations, building underutilization, and concerns about academic performance. Approximately 12,000 students were re-assigned to new schools; of those affected, 94% are low-income and 88% are African- American, leading many to level allegations of racism—a charge which district officials vehemently contest. In this study, I ask: what can disputes about the role of race in the Chicago school closings teach us about broader societal tensions regarding racism and urban school policy? I explore these questions by constructing a portrait of the South Side community of Bronzeville, an important site of African-American culture and history from the Great Migration to the present. Across four chapters, I draw from varying methods and perspectives to build an understanding of school closures and their impact on the community. I use historical sociology to explore the history of racialized sociopolitical change in Bronzeville, and the relationship of public school policy to the rise and fall of public housing in the community. Using critical discourse analysis of hearings and meetings surrounding school closure, I compare community members’ and district officials’ opinions of race and racism and their role in the policy decision. I then present the narrative case of Dyett High School, which was slated for closure and later set to re-open after a hunger strike and vehement community protest. Finally, I present a theory of institutional mourning, a framework for understanding the emotional aftermath of school closure, developed from interviews with community members, parents, teachers, and students. This study offers insight to Chicago stakeholders facing the post-closure landscape and will provoke a new set of questions for district leaders and community members across the country to consider as they evaluate the effectiveness of school closings as a policy. Further, the study models a framework for critically examining the popular conceptualization and social consequences of racism itself in order to enable more productive conversation about the role race plays in school closures and in debates about district policies more broadly.

This thesis examines the representation of the Central Intelligence Agency and its predecessor the Office of Strategic Services in Hollywood cinema from 1945-1975. It argues that the development of these cinematic representations over time has articulated a growing scepticism towards "official" narratives of the past that regard the state as the arbiter of historical authenticity. This scepticism towards state-sourced history is a consequence of increasing US government secrecy. In other words, secrecy fundamentally problematizes state-sourced approaches to historical representation, which rely on the state as an authoritative, trustworthy and relatively transparent producer of the documentary record. The epistemological problem of representing secret institutions is referred to here as the "paradox of secrecy" for historical representation. It is argued that Hollywood’s shift away from state-sourced representations of the CIA is a consequence of this paradox. This thesis is influenced by Hayden White's notion that a given form of historical representation is inherently ideological and even specifically political in its ramifications. In this sense, it is argued that the political content of the films examined here are very much a product of their approach to historical representation itself. This thesis identifies four dominant forms of the American spy film during this period. The first, which was dominant from roughly 1945 up until 1959, was the "semi-documentary". This form of spy thriller celebrated the centrality of the state as the arbiter of historical authenticity and relied upon extensive liaison between filmmakers and government. In chapter 1-3, this thesis traces the rise and fall of the semi-documentary and its ultimately frustrated attempts to represent the CIA. Chapter 4 examines the second dominant form of the spy thriller: the romantic fable. This form, epitomized by James Bond, represents an ironic "camp" reaction to state-sourced approaches to historical representation. Chapter 5 provides a detailed analysis of Alfred Hitchcock’s trilogy of Cold War spy films. It argues that Hitchcock moved away from the camp fable towards the third dominant form: realism. This form, epitomized by the novels of John Le Carré, began a move away from the playful irony of the camp spy fables and offered a far more skeptical and politicized critique of the state and Cold War espionage as Machiavellian in nature. This scepticism towards the state paved the way for the fourth dominant form of the spy film: the conspiracy thriller, which is examined in chapter 6. The 1970s conspiracy thriller represents the precise opposite of the semi-documentary in that it regards the state and state secrecy as the primary obstacle to historical veracity and authenticity. By asserting the possibility of recovering "historical truth" from the miasma of state secrecy, however, the conspiracy narrative moves away from the irony of 1960s spy cinema and articulates the possibility of the redemption of the past. This diachronic transformation of the American spy thriller from the semi-documentary to the conspiracy thriller traces the broader cultural process of growing distrust in government narratives.

This study explores the origins and early evolution of the Philanthropic Society with the aim of making a contribution to our understanding of the dynamics of philanthropy. The Society was founded, in 1788, at a time of growing public concern over the failure of existing legal measures to stem a perceived rising tide of crime. Explicitly conceived as a crime prevention enterprise, the Society focused its attention on a constituency of poor children who either seemed destined for or who had already embarked on a criminal career. The Society's educational experiment in moulding them into law-abiding citizens was initially located in a group of family houses scattered around the village of Hackney. It then made a swift transition to a purpose built Institution in Southwark and remained there until a decision to establish a Reformatory Farm School, at Redhill, was taken in 1848. On one level, this study describes how the Society's development was nurtured by Philanthropists with a diversity of interests in the fields of commerce, jurisprudence, medicine, local poor-law and penal administration. It presents new information on the interplay of ideas and influences that helped shape the Society's institutional policy and practice over the period. At another level, this study takes us through a pre-modem policy landscape to the point at which a voluntary enterprise in protection, prevention and reformation attracted the support of the Government and became the subject of statutory action. By examining hitherto underused Philanthropic archival sources and previously overlooked Government documents, it traces a complex network of interaction between informal and formal agencies in the dissemination of reforming ideas and the shaping of social policy. In doing so, it describes how conventional views on the respective roles and relationships between charitable agencies and the State began to change during the early nineteenth century. Among the new technologies heralded as part of the emerging bioeconomy are plastics made from plant material, known as bioplastics. This dissertation examines the political and historical underpinnings of the bioplastics that are now being offered as an alternative to petrochemicals in the United States. As a case study of "green" technological development, bioplastics challenge dominant conceptions of innovation for sustainability. The bioplastics being developed and marketed today are the outcome of interventions in commodity crop prices, incubation of research on biomass during periods of fossil fuel dominance, and the commercialization of publicly funded research. Their origins can be traced at least as far back as the 1920s, when advocates of "chemurgy" encouraged the federal government to create research centers to discover new industrial uses of agricultural crops.

Research in science and technology studies (STS) indicates that social structures shape perceptions of problems, condition viable solutions, and limit the diversity of stakeholders and ideas present in the social construction of technology. This study examines these processes in the history and current debates about bioplastics. The dissertation asks who has influenced the social construction of bioplastics and why bioplastics have become part of a larger bioeconomic vision now. Theoretical insights are drawn from the sociological theory of the treadmill of production, which argues that environmental problems cannot be solved in a capitalist system in which the federal government, private industries, and organized labor continuously seek the expansion of production and consumption at the expense of environmental sustainability. Major players in the chemical and biotechnology industries have pursued bioplastics as a means of continuing economic growth and consumption of goods, even as petroleum becomes costly and environmentalists voice objections to petrochemicals. There are many critiques of bioplastics and their impacts at every stage of bioplastics, from sourcing feedstocks from food crops to disrupting existing recycling and composting systems. Nevertheless, the bioplastics currently on the market were not designed to resolve these environmental concerns. Increasingly, however, activists are using non-governmental institutions, particularly the development of voluntary standards, to shape the industry and technology. The study examines the extent to which such reforms might lead to the production of more sustainable alternatives to petrochemicals.

Ultimately, this dissertation presents the history and politics surrounding the field of bioplastics in order to highlight how things "might have been otherwise" and what changes in society could be useful for producing more sustainable technologies.

Starting in the early 1900s, Chileans began to address skyrocketing levels of infant mortality. Committed to establishing state welfare policies, health scientists led campaigns to improve infant health. They concentrated on reforming working class maternity. This began a historical connection among health science, public welfare and indigent mothers in Chile. Looking to expand their international role in medical philanthropy in the 1930s, the Rockefeller Foundation invested heavily in Chilean medicine. Following suggestions by leftist physicians, North American philanthropists expanded maternal and child health care. From the 1930s through the 1940s, Chilean and U.S. health professionals further collaborated to reform medical education, build schools of medicine, establish public clinics, open research centers and provide public health education. Cooperation between Chilean leftists and representatives of the Rockefeller Foundation finally succeeded in socializing medicine in 1952. The National Health Service constituted a significant part of Chile's growing welfare system. Supported by the Rockefeller Foundation and Chilean government, state medicine continued to focus on working class women and infants. Leaders from the Rockefeller Foundation's International Health Division attempted to limit their role in Chilean medicine as early as 1940. After helping Chileans to expand public health, Foundation leaders planned to withdraw from Chile. Prominent nationals, particularly leftist health scientists connected with socialized medicine, strongly protested this departure. Mutual interest between Chilean and North American health scientists in family planning persuaded the Rockefeller Foundation to remain. North Americans connected to the Rockefeller Foundation and wealthy Chileans feared social problems caused by burgeoning population. Leftists in the Chilean government worried that public funds could not match popular demand for state services. Population control advocates from the U.S., in turn, feared that growing populations in developing countries would consume world resources. Working with like-minded nationals, North American philanthropists, academics, diplomats and politicians instituted family planning in Chile. Population programs based on the mass distribution and study of previously untested intrauterine devices mushroomed. Pressure from the newly elected Communist president, Salvador Allende, as well as high-ranking U.S. politicians finally ended Chilean population control programs in the early 1970s.

This dissertation brings together empirical analyses of the impact of social insurance programs on compensating wage differentials under different institutional frameworks. I study three periods: the late nineteenth century prior to the introduction of Unemployment Insurance, the Great Depression when Unemployment Insurance is introduced, and then the recent period, in which UI has been long established. Initially, late nineteenth century labor markets with no social programs for workers were investigated. Three different data sets were analyzed from two different states, Maine and Kansas, to examine the precautionary saving behavior of workers and the wage premium they received for the expected unemployment prevalent in their industry. Results showed that workers were receiving statistically and economically significant wage premiums in two of the three samples. Also, in two of the three samples, households were able to save against expected unemployment using family resources. In the second chapter, after reviewing the historical backgrounds of social insurance programs, namely Workers' Compensation, Compensation for Occupational Diseases, and Unemployment Insurance (UI), the empirical literature about the impacts of these programs on wages is reviewed. Later in the chapter, hours and earnings data for various manufacturing industries across forty-eight states for the years 1933-1939 are brought together with the state UI, Workers' Compensation, and Compensation for Occupational Diseases provisions to test the impact of these laws on wage rates. The economic history and origins of UI have not been elaborated before and no previous study has analyzed the simultaneous impacts of different social insurance programs. Results showed that higher accident rates, limited working hours and the higher regional cost of living had a positive impact on wages. Workers' Compensation continued to have a negative impact on wages. During its infancy, UI benefits did not have a statistically significant effect on wages. The last chapter analyzes the impact of UI and the unemployment rate for the labor market of the worker on wage rates using micro level modern data. Results from the analysis of the National Longitudinal Survey of Youth suggest that expected UI benefits have a negative and statistically significant impact on wages, holding worker and labor market characteristics constant. However, the unemployment rate of the labor market did not have a statistically significant impact on wages.

This dissertation provides an analysis and evaluation of factors influencing states to enact inpatient health care transparency laws between 1971 and 2006 inclusive, using event history analysis. The primary research question investigates "What factors influence a state legislature to enact a health care transparency law?" To narrow the scope of study, I focus on factors influencing states to enact health care transparency laws to collect and publicly report inpatient data.

The Unified Model of State Policy Innovation, developed by F.S. Berry and W.D. Berry (1990, 1999), provides the framework for the study hypotheses and the analysis of inpatient health care transparency law enactments by states. The Unified Model of State Policy Innovation posits a unified explanation for state policy adoptions. The model unifies the internal determinants and regional diffusion approaches of analysis for state policy adoption.

This study tests eight hypotheses using event history analysis (EHA). EHA is an analytical technique that allows for the testing of a state government innovation theory that incorporates internal determinants and regional influences on state policy adoption. Although there are numerous methods to conduct event history analysis, this study uses the Cox proportional hazards model (also known as Cox regression). Cox regression is a popular method for studying time-to-event data for policy adoption and diffusion studies. This study's quantitative analysis provides support for legislative ideology and unified party control of state government acting as factors influencing inpatient health care transparency law enactments by states. Additionally, the health care crisis and neighbors variables were statistically significant, but in an opposite direction than predicted.

The findings of this research suggest that state adopters of an inpatient health care transparency law are more likely to enact an inpatient health care transparency law when the state government is increasing in liberalism and when unified political party control of the governor and the governorship of both houses of the state legislature is increasing.

To generate new insights into the enactment of inpatient health care transparency laws, I conduct a case study of a national health care data professional association using several techniques, including telephone interviews. The qualitative analysis provides support for professional associations and policy champions as diffusion agents for inpatient health care transparency law enactments by states.

This dissertation supports variables traditionally used in policy adoption research including legislative ideology and unified political party control in state government. However, it will be interesting to see whether internal determinants such as professional associations gain traction over the traditional regional diffusion influences such as states sharing borders as factors influencing state policy adoption. Meanwhile, as evidenced in this study, there continues to be support for a model incorporating both internal and regional influences to explain policy adoption by states. The theory of policy innovation and diffusion to predict the factors influencing the spread of policies and the use of Berry & Berry's (1990, 1999) Unified Model of State Policy Innovation prosper as their applicability to numerous public policy areas, including health care, are continually demonstrated. Similarly, event history analysis and specifically the Cox regression method continue to gain support as their value as analytical methods and appropriateness for use in public policy studies is repeatedly demonstrated.

The outlook for the future of the health care transparency movement looks promising. The health care transparency movement promotes improved access to information, patient empowerment, improved patient safety and quality of care, improved provider accountability, and lower health care costs. This movement is not a fad, but rather a permanent change being implemented in all health care settings across the United States. Improved health through reliable, accessible data and data-supported decisions is increasingly becoming the norm and less an idealistic scenario to be realized in the distant future.

Two sets of theories are used sequentially in the analysis of the policymaking behind Learnfare. The first series consIsts of theories of the overall pollcymaking process, and includes the rational comprehensive, incrementalist, and two models of "organized anarchy" including the "garbage can" and "policy windows" perspectives. The second series focuses more specifically on the role of policy analysis itself within the larger process, and includes models I abstracted from several recent writings on the subject. The task here is to characterize the nature of analysis and the work of the analyst. These models consist of the "anti-analytic" and "analyst subordination" theses, and the perspective of "policy analysis as art and craft". For the most part, both sets of models afforded helpful and distinct insights into the Learnfare policymaking process.

This thesis addresses two key questions: First, how did those involved in the provision of public housing in twentieth-century England conceptualise the people who they were providing houses for? Second, how did their ideas change over time? These questions are important and need answering because, although there has been a great deal written about the history of public housing in England, there has up until now been very little thought given to the manner in which the council estate tenants themselves were actually identified and conceptualised as subjects in need of state-funded housing. My thesis begins to redress this imbalance by providing an overview of the changing forms and practices through which prospective tenants were conceptualised and acted upon by those in positions of power in England between 1920 and 1970. Using records from local authority archives, sociological surveys, architectural and town planning journals, central government publications, Mass Observation reports and tenant handbooks, and focusing primarily on council estates in London, Manchester and Sheffield, it shows how ideas about what prospective tenants needed from their homes changed dramatically over the course of this period, with the narrowly sanitary and biopolitical approaches of the 1920s and 1930s increasingly being challenged and complemented by a host of new ideas and discourses which placed far more emphasis upon the prospective tenant’s emotional, social and personal needs. As such, this thesis not only adds substantially to our understanding of the changes that took place in the English public housing sector between 1920 and 1970, but also adds to the burgeoning literature on questions of governmentality; contributing in the process to our understandings of modern modes of power.

The Emergency Immigrant Education Act of 1984 (EIEA) was passed by the 98th U.S. Congress to provide funds to states to "meet the costs of providing immigrant children supplementary educational services" (Emergency Immigrant Education Act of 1984, Title VI, Sec. 607). This study analyzes the culture, values, and political context in which the Emergency Immigrant Education Act of 1984 was developed, passed, and amended through its most recent reauthorization. EIEA is the only federal legislation that specifically targets new immigrant students. However, EIEA has been largely overlooked by education policy analysts, because new immigrant students are rarely considered as different from limited English proficient (LEP) students. The study employs historical document and content analysis, applying Kingdon's (2011) theoretical framework of agenda-setting and Manna's (2006) concept of borrowing strength to explain EIEA's path to the agenda. In addition, it applies McDonnell and Elmore's (1987) policy framework to EIEA to understand how policymakers sought to realize EIEA's goals, as well as that of Wirt, Mitchell, and Marshall (1988) to identify the cultural and political values revealed in the rhetoric of the legislation. In tracing EIEA's 30-year route, I describe how the nature of the legislation changed from a primarily capacity-building policy to more of an inducement. In addition, the study revealed a change in an egalitarian culture to one that emphasizes quality.

In the last decades, OECD countries witnessed more than one way of designing students’ aid policies, under a predominant trend of decentralizing their governance. However, this decentralizing process carried the seeds of its own contradictions. The central paradox is that these policies remained multi-issue and multi-actor, making them likely to fall under the double-hand of different governmental levels. Federal countries constitute a laboratory to study the “decentralization experiment” in its absurdity. For this reason, the PhD project proposes a comparison between two federal cases (i.e. the United States and Canada), and in time (between 1930 and 2018), using a mixed-method. The analysis is also extended in a discussion of four embedded deviant case studies (Alaska, New York, Prince Edward Island and Quebec). Albeit usually simplified as paradigmatic cases for liberal, elitist, loan-oriented and tightfisted aid systems, the United States and Canada have proved to be more generous over student aid than what is expected from their respective welfare regime. Nevertheless, behind this engagement, I uncover an intergovernmental contention of powers and responsibilities embedded within deeply rooted federal institutions. With a focus on federal structures and dynamics, I also reveal potential political and economic incentives behind student aid. Why was national alcohol Prohibition repealed in the United States? Prohibition's repeal is unique in several respects. Alcohol Prohibition is the only American drug prohibition to ever be repealed, and the only constitutional amendment to ever be repealed. Furthermore, the volatility of Prohibition policy serves as a useful case for political sociology, which tends to focus on stable policies and government agencies. Prohibition's repeal is important substantively because it is the only American drug prohibition to be repealed. The question of repeal requires examination of several theoretical issues. First, is the process of creating a new policy fundamentally different from the process of dismantling an existing policy? Second, what effect does an exogenous crisis (like World War I or the Great Depression) have on state actor's response to the demands of a social movement? Third, what is the role of elites in a social movement? Fourth, what effect does the implementation of a policy have on those constituencies supporting it? I examine the substantive and theoretical issues of Prohibition's repeal using a variety of primary and secondary sources. National Prohibition resulted from the combined effects of crisis and elite social movement activity. Both were necessary for passage of the 18th Amendment. Implementation of the amendment proved difficult and had a destabilizing effect on Prohibition's supporters. Repeal of Prohibition resulted from the combined effects of implementation and crisis. The passage and repeal of Prohibition were the result of very different processes, suggesting that dismantling a policy is a different kind of political project than creating a policy.

Guinea, a former French colony, experienced an abrupt severing of relationships with the colonial power when President Sékou Touré rejected De Gaulle’s offer of becoming part of a French commonwealth of nations and opted for total independence instead. France withdrew all support, trade and personnel within a matter of days, and Sékou Touré attempted to develop this new nation along strongly independent and ideological lines. He made a verbal commitment to social development and evolved an ambitious programme to develop the health and education services. This thesis uses dependency theory as a tool of analysis to ascertain whether independent, autonomous development indeed took place, particularly in the fields of health and education, once the break with the metropolis was made. The period under review, 1958 - 1984, was the period of Sékou Touré’s presidency. The four criteria used to assess the measure of development achieved are: a) The successful rejection of Western models of development. b) Equal development and an equal distribution of resources between the regions. c) No urban/rural imbalance. d) Services available for all rather than limited to an elite. In the light of these criteria, Guinea was indeed able to experience some measure of independent, autonomous development, more particularly in education. The health sector had been less developed as only 2.1% of the national budget was devoted to it in 1981, compared to 17.6% spent on education in 1980. The inequalities in the health service were particularly noticeable in the urban/rural imbalance, Conakry in particular enjoying a larger share of available resources, but this was not the case in education where no such imbalance appeared to exist. The area of Labé emerged as the least developed area of the country but the discrepancies in provision were not too marked. Guinea also achieved much in the promotion of women and the eradication of elites, especially among the different ethnic groups. With the death of President Sékou Touré in 1984, Guinea's experimenting with a revolutionary form of development came to an end. The nation's future, now in the hands of the military, is uncertain.

This thesis aims to explore the relationship between the geographies of law, society, economy and the physical environment in late eighteenth and early nineteenth century rural England. It uses as its exploring ground the operation of the Old Poor Law in rural Oxfordshire. This county was chosen because it was both a microcosm of the farming landscape of Southern England and was one of the counties where the problem of poor relief was most acutely felt. Chapter 1 establishes that the mapping out of spatial diversity, and the consideration of the forces moulding it, is fundamental to an understanding of the functioning of the Old Poor Law. Chapter 2 uses data contained in the parliamentary returns to demonstrate some clear regional differences in the level of poor relief and the chronology of change. Chapters 3 and 4 show that these regional averages and trends do not make intelligible the kaleidoscopic welter of local variations indicated by a closer examination of parish records. Chapters 5 and 6 consider poor relief expenditure in four parishes: Cropredy, Pyrton, Spelsbury and Stoke Lyne. These show that differences in the level of poor relief expenditure cannot automatically be taken to indicate variations in the level of what we might think of as unemployment or poverty. The generosity of disbursements, and therefore the real incomes of the poor, could also vary markedly between parishes. Thus, the Old Poor Law cannot be detached from the particular places in which it acquired its meaning and saliency. Its impact upon the daily lives of ratepayers, administrators and recipient can be established and the poor treated as individuals rather than as abstract units of labour.

This historical-descriptive study examined Southern Appalachian settlement schools as early initiators of integrated health and social services with education from the 1900s through the 1970s. Three schools were studied: Hindman Settlement School (KY), Pine Mountain Settlement School (KY) and Crossnore School, Inc. (NC). The purpose of the study was to determine the type and extent of services provided, the relationship of the settlement schools with their respective county public school system, and the transfer, if any, of integrated services from the settlement schools to public schools as the public schools took over educational responsibilities once offered by the settlement schools. The conclusions of this study were that extensive integrated services were offered, changing in type over time, the relationship of the settlement schools with their respective county public school systems was, for the most part, cooperative and sometimes collaborative, and there was no transfer of integrated services from the settlement schools to the public schools. Instead, the settlement schools became an integrated service to the public schools. An additional finding was that Pine Mountain Settlement School engaged in a primitive form of privatization with the Harlan County Board of Education. Further, all three settlement schools, through the wide range of services offered, were builders of communities.

This thesis explores contemporary (post-1980) British constructions of Canada or "Canadianness" as these have been conceived through the reading and reception of English-Canadian literary texts in Britain. I am arguing that in recent years Canada has been construed in Britain as an ideal, and furthermore, that this idealization has taken place in response to a perceived cultural and socio-economic malaise within contemporary British society. I use a combined postcolonial and object-relations approach to discuss the psychic investment involved in this construction of Canada as a post-imperial role model. These readers engage with the Canadian object as a sort of phantasy space, projecting onto Canada a self-image which expresses the British desire for postcolonial diversity. Canada thereby enables the dodging of the quagmires of imperiled national identity (and personal subjectivity), for its diffuse and decentralized makeup is balanced by an essentialized notion of cultural and national uniqueness. Throughout I take issue with the ways Canada tends to get celebrated in these writings as a postmodern ideal of unproblematized pluralism and endless diffusion, knowable by the sheer extent to which it seems to defy collective identity. These celebrations of Canada as a new (postmodern) Eden succeed only in emptying the Canadian domain of anything remotely contestatory or political. Indeed, this vision of Canada utilizes a limited version of postmodernism as an idealistic play of pluralities without any sense of accompanying political strife or contradiction.

This thesis consists of three papers that explore variation in individual fertility and fertility preference. The setting for all three papers is the contemporary UK, though the conclusions have utility for a general understanding of human fertility. All three papers are motivated by theories arising from evolutionary biology, principally inclusive fitness theory and life history theory. The first two papers investigate actualised fertility and whether patterns of fertility in contemporary Britain are consistent with inclusive fitness theory. Both papers conduct secondary data analysis of the British Household Panel Study. Inclusive fitness theory predicts that because relatives share genes an individual may obtain fitness benefits by increasing the reproduction of a relative. Results support this hypothesis showing that for contemporary British women kin having more opportunities to influence reproductive decision-making is associated with pro-fitness fertility outcomes. In the first paper I find kin accelerate the transition to first birth, and the second paper shows kin also accelerate the transition to second birth. The final paper tests a different hypothesis derived from evolutionary theory. Life history theory predicts that reproductive strategy should have ‘plasticity’ and be liable to alter as perceived environmental risk changes. This paper uses primary data collected from University students using an internet experiment and finds that priming respondents using preceding questions on mortality does alter reported fertility preferences, though the effects depend upon the priming, fertility preference measure and the sex of the respondent. The paper also has methodological relevance as it demonstrates the potential for ‘context effects’ from preceding questions to influence the reporting of fertility preferences. All three papers present evidence that the incorporation of theories from evolutionary biology have utility in the understanding of contemporary fertility patterns and processes.
