La prise en charge d’un animal en rééducation fonctionnelle commence par une consultation ayant pour but, après un diagnostic clinique, de réaliser un bilan fonctionnel approfondi. Après une évaluation précise et objective de la douleur/cicatrisation, de l’amplitude articulaire, de la masse musculaire et de la mobilité générale de l’animal, le vétérinaire choisit les techniques de soins les plus adaptées. Elles dépendront, entre autres, des objectifs visés, des capacités du propriétaire et de la coopération de l’animal et constitueront un protocole unique et spécifique à ce patient. Pour la réalisation de ce dernier, certaines parties peuvent être déléguées à l’auxiliaire vétérinaire ou au propriétaire sous l’autorité du vétérinaire prescripteur. Il réévalue l’animal régulièrement pour adapter le protocole de soins à l’évolution de l’animal et s’assurer de l’efficacité de la prise en charge globale.

Les batteries secondaires au lithium-ion (Li-ion) sont devenues la technologie la plus répandue pour un large éventail d'appareils électroniques, des gadgets de consommation aux locomotives haut de gamme et aux stockages d'énergie dans les réseaux intelligents. La prolifération rapide des appareils mobiles et de mobilité, et les récents développements dans les véhicules électriques (VE) ont énormément augmenté la demande de batteries Li-ion (LIB) et indirectement créé une dépendance énorme de la vie de mobilité des personnes. Par conséquent, il est maintenant extrêmement important d'avoir des BUL pour alimenter continuellement les dispositifs de mobilité pendant une période de temps plus longue. Quoi qu'il en soit, en tant que source d'énergie rechargeable, les LIBs épuiseront naturellement sa capacité après un certain laps de temps permis par la demande en puissance de l'appareil et la capacité de stockage de la batterie. Par conséquent, tout mécanisme de charge qui peut recharger la batterie jusqu'à l'état complètement chargé dans le temps le plus court possible, également appelé charge rapide, est très demandé et extrêmement précieux dans ce contexte.Cependant, la charge rapide elle-même est une question très difficile en raison d'un certain nombre de raisons telles que les effets complexes (polarisation, limitation, li-dépôt, épuisement des matériaux actifs et etc.) des facteurs multidisciplinaires coexistent au sein de les réactions internes, les limites dans la mesure des facteurs électrochimiques et électrophysiques avancés, les problèmes de sécurité inhérents à l'utilisation de taux élevés et la tendance à détériorer la santé et la durée de vie d'une batterie comme conséquence la plus courante de la charge rapide. Par conséquent, un certain nombre d'approches différentes peuvent être trouvées dans la recherche de la batterie et la littérature, et principalement réalisé en trois sections différentes: l'une est l'introduction de nouvelles chimies qui peuvent stocker plus d'énergie électrique, la seconde est les changements structurels ou de conception Certains des effets négatifs de la charge rapide ou peuvent même améliorer leurs performances et le troisième et le plus intéressant est les protocoles de charge rapide basés sur l'algorithme qui peuvent également aider à tirer parti des performances dans les deux autres approches.Par conséquent, notre thèse s'est concentrée sur un nouveau protocole de charge rapide pour que les BUL puissent se recharger complètement en 20 minutes environ. Ce nouveau protocole est basé sur un concept de voltammétrie non linéaire (NLV) avec l'utilisation d'un ensemble de paramètres d'adaptation liés à l'état de charge (SOC) et à l'état de santé (SOH) de la batterie. Le fondamental de ce concept est la relation prévue que le "produit de", "le taux de la variation du courant tiré / charge (dI / dt)" et "le taux de la variation de tension correspondante (dv / dt)", est une "constante", et exprimée par (| dI / dt |) α * (dv / dt) = K. Ici, le K est une constante et le "α" pourrait être n'importe quelle valeur non nulle. L'analogie de principe ici est lorsque la tension de la batterie est augmentée, le courant résultant accepté par le circuit de charge dépendra naturellement des paramètres cinétiques intrinsèques qui ont un effet sur la charge à ce moment particulier du système de batterie. En conséquence, dans le cas d'une augmentation rapide du courant, la relation ci-dessus réglera le changement de tension pour qu'il soit une valeur plus petite, inversement proportionnelle à la rampe de courant. Inversement, une petite rampe de courant va inciter ce modèle à appliquer une forte variation de tension et laisser ainsi la cellule pénétrer rapidement dans certaines régions de tension ce qui favorise naturellement le chargement à haut débit
Lithium ion (Li ion) secondary batteries have become the most prevalent technology for a broad range of electronic devices from consumer gadgets to high-end locomotives and energy storages in smart grids. The rapid proliferation of both mobile and mobility devices, and recent developments in electric vehicles (EVs) have tremendously increased the demand for Li ion batteries (LIBs) and indirectly created a huge dependency of peoples’ mobility life. Therefore, now it is extremely critical to have LIBs to continuously power up the mobility devices for longer period of time. Anyway, as a rechargeable energy source, the LIBs will naturally drains its’ capacity after a certain period of time permitted by the power demand of the device and the storage capacity of the battery. Therefore, any charging mechanism which can charge-back the battery up to the fully charged status within the shortest possible time, also called fast charging, is highly demanded and extremely valuable in this context.However, the fast charging itself is a very challenging issue due to a number of reasons such as the complex effects (polarization, li-plating, li-deposition, depletion of active materials and etc.) of multi-disciplinary factors co-exists within the internal reactions, limitations in measuring advanced electrochemical and electro-physical factors, the inherent safety issues with the use of high rates and the tendency of deteriorating health and cycle life of a battery as a most common aftermath of fast charging. Therefore, a number of different approaches can be found in the battery research and literature, and mostly realized under three different sections: one is the introduction of new chemistries which can store more electric power, the second is the structural or design changes which can tolerate some of the adverse effects of fast charging or may be even improve their performances and the third and most interesting is the algorithmic based fast charging protocols which can also help to leverage the performance in both the other approaches.Therefore, our thesis has focused on a new fast charging protocol for LIBs to fully charge within about 20 minutes time duration. This new protocol is based on a concept of non-linear voltammetry (NLV) with the use of a set of adaptation parameters related to the state of charge (SOC) and the state of health (SOH) of the battery. The fundamental of this concept is the foreseen relationship that the “product of”, “the rate of the change of drawn/charge-current (dI/dt)” and “the rate of the corresponding voltage change (dv/dt)”, is a “constant”, and expressed as (|dI/dt|)α * (dv/dt) = K. Here, the K is a constant and the “α” could be any non-zero value. The principle analogy here is when the battery voltage is increased, the resulted current accepted by the charge circuitry will naturally depends on the intrinsic kinetic-parameters which effects on charging at that peculiar moment of the battery system. Accordingly, in case of a rapid increase of current, the above relationship will regulate the voltage-change to be a smaller value, inversely proportional to the current ramp. Conversely, a small ramp in current will encourage this model to apply a large voltage change and accordingly let the cell to quickly push in to certain voltage regions which naturally favors in charging with high rates

La charge rapide des batteries est un enjeu majeur pour le développement de l’automobile électrique. Côté infrastructure, un déploiement de chargeurs de haute voire très haute puissance est en cours. Ces hautes puissances motivent une recherche sur les batteries, dans l’objectif de réduire significativement leurs durées de charge.Pour une cellule de batterie de caractéristiques données, la puissance de charge est limitée par des considérations électriques, thermiques et de durée de vie. Cette thèse souhaite alors apporter une contribution à la réduction du temps de charge, par une approche numérique de modélisation, simulation et optimisation. Elle compare également 4 références de cellules de différents matériaux et différentes densités d’énergie.Tout d’abord, des modèles du comportement électrique, thermique et de vieillissement d’une cellule sont développés séparément, puis couplés. De manière systématique, les modèles sont présentés, leurs procédures de calibration sont décrites, et ils sont comparés à des résultats expérimentaux. Une attention particulière est portée à l’effet des forts régimes de courant dans différentes conditions thermiques. Ceci a permis d’étendre leurs domaines de validité respectifs.Ensuite, une campagne de vieillissement accéléré est réalisée sur 3 références de cellules. Elle compare l’effet du courant de charge, de la tension de fin de charge et de différentes conditions thermiques sur la dégradation, dans le cadre d’un protocole de charge référence. Les résultats ont permis d’identifier plusieurs stratégies pour la réduction du temps de charge, au niveau du choix d’une référence de cellule, de la gestion thermique, et de l’optimisation du protocole de charge.Enfin, cette dernière stratégie est étudiée. Une méthode de définition par optimisation numérique d’un protocole de charge à plusieurs niveaux de courant constant est développée. Elle se base sur le modèle électro-thermique mis en place. La méthode est utilisée pour définir 5 protocoles de charge optimisés qui sont soumis à des essais de vieillissement accéléré. La dégradation est ensuite comparée à celle observée pour le protocole de charge référence. Dans des conditions comparables au protocole référence, les protocoles optimisés permettent de réduire le temps de charge et/ou la dégradation
Fast charging of batteries is a major challenge for the development of electric vehicles. A deployment of high power chargers is underway. These high power levels motivate research on batteries, with the aim of significantly reducing their charging times.For a battery cell of given characteristics, the charging power is limited by electrical, thermal and lifetime considerations. This thesis then wishes to make a contribution to the reduction of the charging time, by taking a numerical approach including modeling, simulation and optimization. It also compares 4 cell references of different materials and different energy densities.First, models of the electrical, thermal and aging behavior of a cell are developed separately, then coupled. In a systematic way, the models are presented, their calibration procedures are described, and they are compared with experimental data. Particular attention is paid to the effect of high current regimes under different thermal conditions. This made it possible to extend their respective domains of validity.Then, an accelerated aging campaign is carried out on 3 cell references. It compares the effect of the charging current, the end-of-charge voltage and different thermal conditions on the degradation, within the framework of a reference charging protocol. The results made it possible to identify several strategies for reducing the charging time, regarding the choice of a cell reference, thermal management, and the optimization of the charging protocol.This last strategy is finally studied. A method of definition of a charge protocol, containing several stages of constant current, is developed based on numerical optimization. The method makes uses the electro-thermal model implemented. It is then used to define 5 optimized charging protocols which are subjected to accelerated aging tests. The degradation is compared to that observed for the reference charging protocol. Under comparable conditions with the reference protocol, the optimized protocols make it possible to reduce the charging time and/or the degradation

Ce travail de recherche a pour ambitions d’apporter un éclairage sur les spécificités psychopathologiques des réfugiés victimes de torture d’Afrique Centrale et de l’Ouest et d’évaluer l’efficacité thérapeutique d’un protocole de dix séances associant la psychoéducation, la relaxation et l’EMDR, tout en comparant ses effets en modalité de traitement hebdomadaire ou intensive. Pour cela, 52 participants ont été répartis dans trois conditions de traitement : « EMDR », « Stabilisation » et « Intention de traiter par médicaments » et ont bénéficié d’une évaluation psychologique au début, à la fin et 4,5 mois après l’arrêt de la thérapie. Nos résultats indiquent que l’État de Stress Post-Traumatique (ESPT) est le trouble le plus représenté et que l’hébergement et la situation de la demande d’asile sont les variables sociales ayant le plus d’influence sur l’intensité de la psychopathologie. Le protocole « EMDR » montre une efficacité thérapeutique forte et supérieure aux deux groupes contrôles pour l’ensemble des variables considérées comme pathologiques lors de l’évaluation initiale (ηp2 entre. 41 et. 96). De plus, la modalité de traitement intensive majore l’efficacité thérapeutique du protocole « EMDR » pour l’ESPT, la dépression, l’anxiété, et le fonctionnement global (ηp2 entre. 87 et. 93). En conclusion, cette recherche montre l’importance d’interroger les patients sur leur situation sociale et migratoire dans le pays d’accueil et fait la preuve que l’association de l’EMDR à la psychoéducation et à la relaxation est un protocole thérapeutique pertinent et efficace dans la réhabilitation psychologique de cette population, en particulier dans sa forme intensive
The aims of this research are to provide information on the psychopathological specificities of refugees torture survivors from Central and West Africa and to assess the therapeutic efficiency of a 10 session protocol associating psychoeducation, relaxation technique and EMDR while comparing its effect when applied in weekly or intensive treatment modality. To achieve these goals, 52 participants were allocated in three treatment conditions: “EMDR”, “Stabilization” and “Intent to treat with medication” and were assessed at the beginning, at the end and 4,5 months after the end of psychotherapy. Our results show that Post-Traumatic Stress Disorder (PTSD) is the most frequent pathology and that accommodation and the asylum claim situation in the host country are the social variables with the most influencing impact on psychopathology. The “EMDR” protocol shows a large and superior therapeutic efficiency in comparison with the control groups for all the variables that were considered pathological during the initial assessment (ηp2 entre. 41 et. 96). Also, the intensive treatment modality increases the therapeutic efficiency for PTSD, depression, anxiety and functioning (ηp2 entre. 87 et. 93). In conclusion, this research shows this importance to ask patients about their social and migration situation in the host country and demonstrates that associating EMDR with psychoeducation and relaxation techniques is a relevant and efficient psychotherapeutic protocol in the psychological rehabilitation of this population, especially when it is applied in a intensive way

Cette thèse traite de la migration de processus dans les systèmes massivement parallèles. L'intérêt d'une telle fonctionnalité est de permettre à un système d'exploitation une gestion efficace des ressources. Les critères de conception sont la transparence de la migration, la réduction des couts induits, et l'adéquation entre les algorithmes de migration et l'extensibilité des architectures cibles
La migration d'un processus vers un nouveau processeur nécessite la suspension du processus, le transfert de son contexte d'exécution et la reprise de l'exécution sur le nouveau processeur. De plus, les protocoles de communication et d'accès doivent être reconsidérés afin de tenir compte de la migration de processus. Pour ces différentes actions et selon les critères de conception retenus, de nouveaux algorithmes ont été proposes. La mise en œuvre de ces algorithmes dans le noyau de système ParX nous a permis de montrer que la réalisation d'un mécanisme de migration de processus dans un système massivement parallèle peut s'effectuer sans pénaliser les performances du système de façon significative
Enfin, nous avons proposé un algorithme de répartition de charge qui utilise la migration de processus. Compare à un algorithme uniquement fonde sur le placement des processus, les expérimentations effectuées montrent que notre algorithme améliore les temps de réponse du système grâce à la migration de processus. Cette amélioration est obtenue lorsque les temps d'exécution et d'inter-création des processus sont variables et le cout de migration négligeable par rapport à la durée d'exécution des processus

Comparativement aux méthodes existant dans divers pays, le protocole MMS-GUR (Macroinvertebrates living in the Mineral Substrates of the Geomorphological Unit Rifle) présente les avantages suivants : 1) Facilité de repérage des seuils et meilleure accessibilité que les autres géoformes. 2) Aire d'échantillonnage réduite. 3) Echantillonnage de surfaces identiques grâce à l'utilisation du filet Surber. 4) Protocole applicable de la source à la disparition des seuils. 5) Gain de temps répercuté sur le coût tant sur le terrain qu'en laboratoire (détermination au genre, mais réduction possible aux EPT) . 6) Possibilité de comparer un ensemble de rivières dans des conditions analogues.

AbstractAlterations in renal metabolism are associated with both physiological and pathophysiologic events. The existing noninvasive analytic tools including medical imaging have limited capability for investigating these processes, which potentially limits current understanding of kidney disease and the precision of its clinical diagnosis. Hyperpolarized 13C MRI is a new medical imaging modality that can capture changes in the metabolic processing of certain rapidly metabolized substrates, as well as changes in kidney function. Here we describe experimental protocols for renal metabolic [1-13C]pyruvate and functional 13C-urea imaging step-by-step. These methods and protocols are useful for investigating renal blood flow and function as well as the renal metabolic status of rodents in vivo under various experimental (patho)physiological conditions.This chapter is based upon work from the COST Action PARENCHIMA, a community-driven network funded by the European Cooperation in Science and Technology (COST) program of the European Union, which aims to improve the reproducibility and standardization of renal MRI biomarkers. This experimental protocol is complemented by two separate chapters describing the basic concept and data analysis.

AbstractThe purpose of this chapter is to reflect on the future of EQ-5D-5L valuation studies, going beyond the value sets summarised in this book. This includes a number of linked themes. First, the EQ-5D-5L valuation research programme has allowed the continued evolution of methods, as methodological studies have demonstrated that aspects of the EQ-VT protocol could be strengthened or improved. This chapter describes some of the key candidates for future refinement of the methods for valuing EQ-5D-5L. Second, while the standardisation of valuation methodology is important, it is anticipated that many countries may require a less resource-intensive, but still rigorous version of the valuation protocol. This chapter outlines the progress towards developing a ‘lite’ version of the EQ-VT protocol, and considers the future possibility of valuation protocols based exclusively on discrete choice experiments, with accompanying strengths and weaknesses. Finally, the ‘shelf-life’ of value sets is considered, along with how demographic and other societal changes may manifest in how people value health, and the implications of that for the need to update EQ-5D-5L value sets.

Abstract Currently, we are developing protocols for cryopreserving select strains of algae from a large culture collection. Protocol design processes for specific organisms involve modeling the coupled osmomechanical behavior of the cells during cryoprotective additive (CPA) exposure. Perfusion cryomicroscopy experiments were conducted to characterize the extent of elastic and osmotic coupling effects upon cell volume changes during exposure of Chlorococcum texanum to step changes in extracellular sucrose concentration. Volume data obtained off-line from video recordings of continuous volumetric changes was combined with inverse solution methods to obtain constitutive property data which includes the cell wall/membrane permeability coefficient for water and the cell wall elastic behavior. Network thermodynamic modeling was employed to simulate the coupling of the elastic and osmotic effects of an algal cell during CPA perfusion. Results from perfusion experiments indicate that a decoupling of elastic and osmotic stresses occurs at the point of incipient plasmolysis. The objective of this study is to construct a robust model of the coupled elastic-osmotic behavior of the algal cells to accurately simulate the volumetric response of algal cells as changes in extracellular solute concentration are encountered during cryopreservation, including severe shrinking and swelling of cells. Volumetric predictions require accurate modeling of the osmomechanical behavior across both elastic and osmotic regimes as well as the transition region of incipient plasmolysis where elastic and osmotic stresses become uncoupled.

Deos includes an industry standard lightweight TCP/IP stack (LwIP) with a DAL-A sockets library so it can provide data transport during in flight or on ground as part of its standard package. While it may have high data integrity (e.g., through CRC or other such mechanisms), TCP/IP over Ethernet is a non-deterministic protocol. As such, it is not suitable for avionics applications that require determinism or high robustness. In contrast, there are several are several redundant and deterministic data network technologies such as ARINC-664/AFDX, time triggered ethernet (TTE), and time sensitive networking (TSN). These interfaces are based on switched Ethernet technologies and can include system redundancy such that they are applicable for aircraft data network applications. Their feature set enables them to be used as a digital backbone for aircraft control and other applications where both integrity and availability are essential. Each of these solutions generally requires specific end point hardware to implement the protocols in firmware in order to meet the required communication timing and throughput. The implementation of the software device drivers for these technologies on Deos can leverage Deos' I/O Infrastructure (IOI) data distribution service for data decoupling. IOI is a DO-178 DAL-A module that can distribute data based on XML configuration files that specify the data paths, access control and optionally data formatting. It implements an inter-partition communications data interface between avionics applications including ARINC-653 partitions using the ARINC-653 APEX API sampling/queueing ports. Together, these features allow developers to readily adapt to changes in communication structures all through XML configuration files, versus recompiling which would impact the verification evidence of the module. This paper will talk about the different networking standards and how the use of Deos' IOI provides a way for the system to easily adapt to different network configurations without causing the driver library or end application(s) to be modified and thereby minimize change impact for reuse/reverification.

La luxation bilatérale non-traumatique de l’articulation temporo-mandibulaire est une pathologie aiguë qui peut évoluer vers la chronicité ou la récurrence. Chez l’enfant, peu de cas ont été rapportés dans la littérature et aucun protocole de prise en charge globale n’a été proposé à ce jour. 2 cas de luxation chronique de l’articulation temporo-mandibulaire chez le jeune enfant sont décrits et leur prise en charge discutée au vue des données issues de la littérature. Lors de la prise en charge de la phase aiguë, le protocole de sédation doit être particulièrement adapté au degré de coopération de l’enfant. La technique de réduction la plus utilisée reste celle de Nelaton. Le type de contention doit être également choisi au mieux pour permettre les fonctions de nutrition et la stabilité du dispositif. L’existence d’une chronicité doit orienter, selon les antécédents du patient, vers la recherche d’une pathologie neurologique, comportementale ou génétique. Les cas présentés sont associés pour l’un à une réponse comportementale d’opposition lors de l’alimentation, pour l’autre à des mouvements spastiques dans un contexte de dystonie. La prise en charge des récurrences sera celle de la pathologie sous-jacente, de manière médicamenteuse ou comportementale, le potentiel de croissance chez l’enfant limitant l’indication chirurgicale. Selon le contexte, l’usage d’injection de toxine botulique peut également être envisagée.

L’organisation de la réhabilitation maxillo-faciale implanto-portée en oncologie tête et cou reste sujet à débat. Il n’existe pas de consensus quant au moment de la réhabilitation implantaire (pendant ou à distance de l’exérèse) ou quant aux prérequis anatomiques qui garantissent son succès. Le but de cette étude est de mettre en place un protocole de prise en charge implanto-portée de patients ayant été traités en oncologie tête et cou. Il s’agit d’une étude rétrospective entre 2013 et 2017, analysant la réhabilitation implantoportée de neuf patients traités pour un cancer tête et cou par chirurgie et radiothérapie (groupe 1 : 5 patients) ou par chirurgie seule (groupe 2 : 4 patients). Les critères étudiés étaient les données personnelles des patients, leurs antécédents oncologiques, les bilans oraux pré-opératoires effectués, les thérapeutiques utilisées ainsi que le suivi. Tous les patients ont été traités pour des carcinomes épidermödes majoritairement situés au niveau du plancher buccal. Ils présentaient tous une limitation de la mobilité linguale, une absence de vestibule et des tissus crestaux mobiles. Les projets prothétiques prévoyaient de réaliser pour sept patients une prothèse amovible complète mandibulaire stabilisée sur implants et pour deux patients des couronnes unitaires sur implant. Les patients ayant recu un lambeau libre de péroné latéral ont bénéficié d’une greffe d’apposition associée à un lambeau de FAMM. Les deux greffes ont permises la pose des implants. Sur neuf patients, cinq ont bénéficié d’une libération linguale, six ont recu une vestibuloplastie et huit ont eu une greffe épithélio-conjonctive péri-implantaire. Les implants ont été posés sous anesthésie générale avec un guide chirurgical pointeur. Il s’agissait d’implants Tissu Level RN SLA Roxolid (Straumann, Basel, Suisse). Pour les patients du groupe 2, la pose des implants a eu lieu en même temps que la vestibuloplastie. Lors de cette étude quatre patients étaient réhabilités par une prothèse sur implants. Le groupe 2 n’a pas présenté de complications. Deux patients du groupe 1 ont nécessité plusieurs libérations linguales et deux implants ne se sont pas ostéointégrés dans ce groupe. Lors de chirurgies en deux temps, des greffes épithélio-conjonctives crestales ont dû être réalisées pendant la vestibuloplastie puis pendant la pose implantaire. Deux greffes osseuses ont été réalisées chez deux patients ayant été irradiés bien que cela soit contre indiqué dans la littérature (Raoul 2009). Une greffe osseuse d’apposition peut donc être une solution viable chez ces patients afin de limiter le bras de levier prothétique. Cette étude a permis de mettre en exergue : l’importance de prévoir en amont la réhabilitation implantoportée lors de la reconstruction pour une optimisation du positionnement du greffon osseux et l’intérêt d’effectuer dans le même temps vestibuloplastie, pose des implants et aménagement péri-implantaire pour la pérennité implantaire. Cette étude rétrospective a permis de proposer un protocole de prise en charge standardisé de la réhabilitation implanto-portée de patients traités pour un cancer tête et cou.

La radiothérapie (RTE) constitue une thérapeutique majeure dans la prise en charge des cancers ORL. Elle entraîne une altération cellulaire, matricielle et vasculaire des tissus. Au niveau du tissu osseux, cela aboutit à un déséquilibre entre apposition et résorption osseuses, avec pour conséquence un risque d’ostéoradionécrose (ORN)1. Le traitement de l’ORN peut être médical ou chirurgical. Il peut alors comporter des chirurgies lourdes (interruption mandibulaire ), avec des conséquences dramatiques sur les fonctions oro-faciales2. Il apparaît donc nécessaire de développer des techniques préventives et thérapeutiques plus efficaces et moins mutilantes, afin d’améliorer la qualité de vie de ces patients. L’objectif de l’étude est de mettre au point un modèle animal (lapin), permettant de simuler une perte de substance osseuse alvéolaire sur une mandibule irradiée, afin d’évaluer la cicatrisation osseuse après extraction dentaire en territoire irradié, et d’évaluer les effets des ultras sons de faible intensité (LIPUS) dans la prévention ou le traitement de l’ORN. L’étude a été approuvée par le comité d’éthique pour l’expérimentation animale du Centre Léon Bérard (n° d’agrément 69.388.0501). Le modèle animal retenu est le lapin femelle adulte (race New-Zealand White). La première partie de l’étude consiste à confirmer le schéma de radiothérapie. 30 lapins sont inclus ; 10 sont témoins, et 20 sont irradiés selon les modalités suivantes : 8.5 Gy par séance, 5 séances, 1 séance par semaine. A la fin de la radiothérapie, un défaut osseux mandibulaire simulant une extraction dentaire est réalisé. Les animaux sont ensuite sacrifiés à J0, J7, J14, J28, J42. L’évaluation de la reconstruction osseuse au niveau du site d’extraction se fait grâce à des analyses histologiques, la réalisation d’un microscanner et des tests de microdureté. Ensuite, un protocole d’insonification est développé. Après réalisation du défaut osseux chez 12 lapins, 10 mandibules de lapins (2 lapins contrôles) sont insonifiées selon le protocole suivant : fréquence = 1,05MHz, tension = 77mV, 2ms on/8ms off, Intensité acoustique = 300mW/cm. Les sacrifices et l’évaluation de la reconstruction osseuse sont réalisés selon les mêmes modalités que précédemment. Enfin, les LIPUS sont étudiés sur des mandibules de lapins irradiés : 20 lapins sont irradiés puis bénéficient de la création du défaut osseux. 10 recoivent les LIPUS comme précédemment (10 lapins contrôles). Les sacrifices et les analyses histologiques, radiologiques et micro-mécaniques sont réalisés comme précédemment. Une étude de faisabilité est actuellement en cours, afin de déterminer les modalités de création du défaut osseux, et de préciser les paramètres d’évaluation. Différentes études ont montré que les LIPUS stimulaient le tissu osseux en stimulant la différenciation ostéoblastique3. Une utilisation dans le traitement des ORN semble donc envisageable, ce qui permettrait de diminuer les mutilations oro-faciales liées aux chirurgies réalisées actuellement.

BACKGROUND AND OBJECTIVES Le Centre de Sante de Référence de Douentza fait face à des nombreux défis des soins néonataux exacerbés par le contexte d’accès très limités. Plusieurs initiatives ont été envisagées notamment le support continu du Pédiatre, analyses des causes des décès et définition des plans d’action, formation au lit des malades. Pour accéder et accompagner le personnel soignant, une formation virtuelle intersectionnelle des soins néonataux a été réalisée pendant 2,5 mois pour une équipe de 7 staffs (infirmières et médecins). Le partage de cette expérience a comme objectif de montrer les bonnes pratiques possibles dans ces contextes. CASE DESCRIPTION Après cette formation virtuelle interactive, l’équipe a pris l’initiative d’identifier les grands défis à relever pour améliorer les soins néonataux à travers une évaluation approfondie (arbre à problèmes) des pratiques de soins dans le projet. La collecte d’informations comprenait des observations sur place à travers des entretiens avec le personnel de santé et avec des accompagnants, des dossiers des patients ainsi que les audits de décès. Définition et implémentation du plan d’amélioration qui a permis : • Réorganisation de la salle de néonatologie en fonction du degré d’urgence : rouge, orange, vert, zone de réanimation. • Meilleure acceptance en interne et sensibilisation sur les soins de la mère kangourou et zéro séparation. • Mise en place et maitrise du protocole de prise en charge (PEC) et de la réanimation néonatale. • Consensus et adoption des critères de PEC (admissions, référence, soins palliatifs). • Travail en amont avec l’équipe de la Promotion de santé et engagement communautaire pour le recours précoce aux soins. CONCLUSIONS La formation virtuelle a permis non seulement d’interagir, d’acquérir des nouvelles connaissances mais aussi l ́analyse et détection des causes profondes du problème, proposer des interventions ciblées à court et moyen terme ; définir le chronogramme d’implémentation ainsi que les indicateurs de suivi. Des changements perceptibles sont opérés et continueront pour des soins néonataux appropriés.

Following a review of Vital Signs – indicators of ecosystem health – in the coastal parks of the Northeast Coastal and Barrier Network (NCBN), knowledge of shoreline change was ranked as the top variable for monitoring. Shoreline change is a basic element in the management of any coastal system because it contributes to the understanding of the functioning of the natural resources and to the administration of the cultural resources within the parks. Collection of information on the vectors of change relies on the establishment of a rigorous system of protocols to monitor elements of the coastal geomorphology that are guided by three basic principles: 1) all of the elements in the protocols are to be based on scientific principles; 2) the products of the monitoring must relate to issues of importance to park management; and 3) the application of the protocols must be capable of implementation at the local level within the NCBN. Changes in ocean shoreline position are recognized as interacting with many other elements of the Ocean Beach-Dune Ecosystem and are thus both driving and responding to the variety of natural and cultural factors active at the coast at a variety of temporal and spatial scales. The direction and magnitude of shoreline change can be monitored through the application of a protocol that tracks the spatial position of the neap-tide, high tide swash line under well-defined conditions of temporal sampling. Spring and fall surveys conducted in accordance with standard operating procedures will generate consistent and comparable shoreline position data sets that can be incorporated within a data matrix and subsequently analyzed for temporal and spatial variations. The Ocean Shoreline Position Monitoring Protocol will be applied to six parks in the NCBN: Assateague Island National Seashore, Cape Cod National Seashore, Fire Island National Seashore, Gateway National Recreation Area, George Washington Birthplace National Monument, and Sagamore Hill National Historic Site. Monitoring will be accomplished with a Global Positioning System (GPS )/ Global Navigation Satellite System (GNSS) unit capable of sub-meter horizontal accuracy that is usually mounted on an off-road vehicle and driven along the swash line. Under the guidance of a set of Standard Operating Procedures (SOPs) (Psuty et al., 2022), the monitoring will generate comparable data sets. The protocol will produce shoreline change metrics following the methodology of the Digital Shoreline Analysis System developed by the United States Geological Survey. Annual Data Summaries and Trend Reports will present and analyze the collected data sets. All collected data will undergo rigorous quality-assurance and quality-control procedures and will be archived at the offices of the NCBN. All monitoring products will be made available via the National Park Service’s Integrated Resource Management Applications Portal.

Forest ecosystems are experiencing the impacts of climate change in many forms, however, comprehensive monitoring efforts are not always available to identify changing baselines. In order to improve our understanding of the impacts of climate change on ecosystem processes, the FEMC developed the Forest Impacts of Climate Change: Monitoring Indicators tool (Version 1.0). The Forest Impacts of Climate Change: Monitoring Indicators tool was developed for use by researchers and professionals to be able to easily access protocols used to monitor high priority indicators of the impacts of climate change in New England and New York. The monitoring protocols provide information for landowners and managers to implement their own monitoring programs that will be comparable to other studies being conducted across the region. By centralizing information about this network of monitoring sites, more data will become available to the community to help discern how forest ecosystems are changing. This report describes the methods and implementation used to build this tool. To develop the Forest Impacts of Climate Change: Monitoring Indicators tool, FEMC formed a committee of partners to select indicators and provide guidance about the literature review and eventual tool. The committee identified four ecological categories as important for monitoring climate change in the Northeast: Wildlife, Forest Systems, Trees, and Aquatic Systems. FEMC identified who is currently conducting monitoring efforts, what monitoring protocols are available for replication, gaps in monitoring data, and how we can make data and monitoring information easily available so that land managers can have the most up-to -date information possible. The developed tool compiles over 350 studies across 24 different indicators of the impacts of climate change. Through a filterable webtool users can find these studies, as well as 168 replicable protocols to direct implementation. The tool helps to identify gaps in monitoring efforts and provides a platform for users to contribute to regionally cohesive datasets. Monitoring of indicators across systems is critical for tracking and understanding climate change impacts. The Forest Impacts of Climate Change: Monitoring Indicators tool, developed for use by researchers, professionals, and land managers across the region, lets users find methods and protocols for monitoring climate change impacts and see where these monitoring efforts are already being conducted in our region. In addition, you can quickly visualize where there are gaps in our monitoring. As contributors in the Cooperative region share more information about their own monitoring efforts, this will become available to the community through this tool, increasing our ability to track and identify change in our forested ecosystems.

Before COVID-19, Mozambique’s Community-Led Total Sanitation (CLTS) protocol, introduced in 2008 and referenced in the Strategy of Rural Sanitation 2021-2030, broadly aligned with the original approach proposed by Kar and Chambers in the CLTS Handbook (2008). It included participatory pre-triggering, triggering and post-triggering activities bringing whole communities together to promote collective behaviour change on sanitation and health in the community. During the pandemic, UNICEF Mozambique worked with the Government of Mozambique (GoM) and other partners to adapt the CLTS protocol in line with government restrictions to support continued implementation of the approach. With restrictions now reducing, clarity on a post-pandemic CLTS protocol is needed. This rapid study explored how CLTS programming was adapted and implemented during the pandemic and the successes, failures, and lessons to inform recommendations moving forward. With only eight years to go to achieve the SDGs, it is timely for Mozambique to review and adapt the existing tools to ensure the required acceleration towards the elimination of open defecation is possible. The study focused on how adaptations to the CLTS protocol were devised and implemented, what the changes involved, different stakeholders’ roles and responsibilities, and what the lessons and recommendations are.
