This includes drug addicts, but also food addicts, game addicts, theft addicts, etc : all those people who center all (or most of all) their psychical investments on only one axis during a period of their life. (hypercathexis of use). The first part is a quick history about drug's use. Then a shortstock of the psychanalytical works about addiction is taken (mainly french authors). The next part develops the concept of "disbinding's enjoyment" or "disconstruction's enjoyment". Then comes a clinical part centered on incarcerated drug addicts. At least a come back on theory allows to question the concept of : ego, superego, i, subject, sublimation; and to do some suggestions for a psychanalytical approach of the concerned people

Testo completoWhile Germany relies on forensic hospitals (MaBregelvollzug) since the 1930s, the French penal system develops mental health care services from the 1980s in order to deal with inmates with mental disorders. The involvement of mental health professionals in the penal system begs the question of hybridization of care and punishment. The aim of this thesis is to investigate this hybridization in a French-German comparative. Based on ethnographic field studies - carried out respectively over five months in a French and German prison - the thesis examines the various arrangements of care and punishment: while the prison of Tourion (France) can be described as a fragmented institution, in which the lines between penitentiary and mental health care services are demarcated, the prison of Grunstadt (Germany) arranges a reinforced cooperation. In both countries, however, the presence of mental health care professionals leads to ambivalent effects: it brings the mental suffering of inmates into the spotlight, but also contributes to the legitimization of the concept of therapeutic punishment -life sentence as therapy for the most "dangerous" inmates. Ultimately, this thesis illustrates the contemporary transformations of the prison to an "asylum", namely a place where punishment and treatment are intertwined, but equally, one of the last refuges for certain forms of madness. Beyond prisons, this thesis contributes to analyzing the relation between institutional transformations and the evolution of professional worlds

Testo completoUnder the Old Regime, physical suffering was an essential element of punishment. The detainees were kept in captivity for the purpose of reparation of their fault for the purpose of amendment. Since deprivation of liberty is characterized by cruelty and unhealthy conditions of detention, medicine in prisons has long been considered as "an extra soul" of the prison institution and relegated to the rank of "deputy doctor" by the entire medical field because of its submission to the administration.It is in this conception of punishment that the 1789 Declaration of the Rights of Man and of the Citizen, which states that the law must establish only those penalties that are strictly necessary, is part of this concept. At the beginning of the 20th century, the prison administration still treats its prisoners. This led to real psychological and health disasters, as the doctor depended on prison and non-health institutions. Until the Liberation, the history of health and the health professions in prison is marked by a profound continuity that is expressed, first of all through a continuous opposition between the expression of philanthropy in reforms and the confrontation with an austere reality of health in a totalitarian prison. The autonomy of those in charge of health in prison is hardly guaranteed. Prisons are still exempt from the great sanitary and hygienic revolutions.The appearance of the AIDS virus and pathologies is changing mentalities. Between the 1970s and 1980s, a movement emerged from a civil society demanding respect for human rights, while scientific and political developments, as well as political and prison developments, gradually revealed the limits of this new organization: the affirmation of a chronically poor administration, whose care is certainly not its primary mission, relegates the whole system to the bottom of a poverty funnel, while the first epidemiological approaches point to an over-morbidity in this population in terms of age, particularly in terms of infectious, addictive and dental diseases

Testo completoPrisons are responsible not only for punishing offenders sentenced to deprivation of liberty, but also for ensuring their safety and respect for fundamental rights, while promoting their social reintegration and combating recidivism. Prisons are also responsible for the medical and psychological care of prisoners who require specialized assistance. This is a mission that the institution is currently having difficulty fulfilling. A number of national and international studies have shown that the mental health of prisoners is much worse than that of the general population. However, few studies have focused on the cognitive and emotional specificities of prisoners. The scientific literature in health psychology and cognitive psychopathology shows the essential role played by these cognitive and emotional processes in the emergence and maintenance of psychopathological states. This doctoral project had two main objectives: 1) To assess how prisoners perceive their health status and how they rate certain dimensions of psychological well-being, according to their age and recidivism status. An initial study was carried out with young and elderly adult prisoners, who completed scales of perceived health (NHP, Mental Health Analog Scale), self-esteem (Rosenberg scale) and body investment (BIS). 2) To assess the role of transdiagnostic cognitive-emotional processes involved in the emergence and maintenance of psychopathological states in detention. A second study examined the links between dimensions of the impulsivity construct and the production of cognitive distortions. Participants, young adults in detention and a comparison group from the general population, completed an impulsivity scale (UPPS) and a cognitive distortions assessment scale (EDC-A).A third study examined the type and nature of cognitive emotion regulation strategies in relation to anxiety and depressive symptomatology. Participants, young adults in prison and a comparison group from the general population, completed questionnaires and scales on cognitive emotion regulation (CERQ) and anxiety and depressive symptomatology (HADS).The results show that prisoners of all ages perceive their state of health to be worse than that of individuals of the same age from the general population. In addition, there are age-related specificities. Indeed, health problems perceived by young inmates are mainly associated with body-related concerns, whereas they are more closely linked to psychological and mental concerns among older inmates. Furthermore, individuals in a situation of recidivism are characterized by a greater perception of emotional difficulties and better perceived mental health. Secondly, the results indicate that inmates display significantly more cognitive distortions, particularly negative ones. Impulsivity was also found to be positively correlated with positive and negative distortions. Nevertheless, a significant moderating effect of the length of detention on these relationships highlights their attenuation with time spent in prison. At the same time, in order to regulate their negative emotions, it appears that prisoners make greater use of dramatization and positive centration than individuals from the general population. Finally, the results highlight specific and distinct cognitive regulation profiles according to the type and intensity of anxiety and depressive symptomatology, in the prison population and in the general population

Testo completoNational and international studies and reports about preventing suicide in jail have been published, giving information about epidemiology, suicide risk profiles. They also provide prevention measures. More researches are required to improve knowledge of clinical aspects of suicidal behaviors in prison and to evaluate prevention programs. The study consisted in an assessment of 12 measures of a local preventing suicide program. We asked prisoners who had been jailed since one month for an interview. First they had to tell if they knew about the existence of a prevention program. Second, they evaluated the usefulness of each measure in (5 points - Likert scale). They freely explained their choices. We asked them about suicidal ideation’s presence since they had arrived in jail, their urgency degree at D0, D8 and D28, and the factors that contributed to these states. They filled the Reasons for Living Inventory of Linehan (RLIL) and the Mini International Neuropsychiatric Interview. Twenty of the 53 participants have had suicidal thoughts during the first month ; only 15 told they knew there was a suicide prevention program. Eight measures were noted as significantly useful. Four of them obtained a middle score : limitation of hanging points, cell doubling, special monitoring, special protection cell. The search for correlations have found different combinations of links between some measures, the suicidal states, use of the program, the RLIL and psychiatric disorders. We discuss about how psychological tools can be helpful to the evolution of each measure, to the adaptation of the global prevention program. Notably, the prisoners’ participation is in particular proposed. Moreover, preserving speaking time with freedom of expression in the application of the program was expected by the participants. At last, they proposed innovative methods that could serve as a lead for followup works
