AbstractPatients want more information and active participation in medical decisions. Information and active participation correlate with increased adherence. A conversation guide, combining patient-relevant drug information with steps of shared decision-making, was developed to support physicians in effective and efficient prescription talks. Six GP trainees in community-based primary care practices participated in a controlled pilot study in sequential pre-post design. Initially, they conducted 41 prescription talks as usual, i.e., without knowing the guide. Then, they conducted 23 talks considering the guide (post-intervention phase). Immediately after the respective talk, patients filled in a questionnaire on satisfaction with the information on medication and physician–patient interaction, and physicians about their satisfaction with the talk and the application of the guide. Patients felt better informed after guide-based prescription talks (e.g., SIMS-D in median 10 vs. 17, p < 0.05), more actively involved (KPF-A for patient activation 2.9 ± 0.8 vs. 3.6 ± 0.8, p < 0.05), and more satisfied with the physician–patient interaction. Physicians rated the guide helpful and feasible. Their satisfaction with the conversation was significantly enhanced during the post-intervention phase. The evaluation of the duration of the talk was not influenced. Enhanced patients’ and physicians’ satisfaction with prescription talks encourages further examinations of the conversation guide. We invite physicians to try our guide in everyday medical practice.

Background: The cervical vertebrae have attracted the interest of orthodontists, as the cervical portion of the spine serves as a reference structure to guide the normal head position. Skeletal age is determined by examining changes occurring in the morphology of the cervical vertebrae. There is relatively little data available from all the examinations that compare cervical vertebral phases between the various malocclusions. The purpose of this study was to compare the cervical vertebral maturation index (CVMI) stages of skeletal Class I and Class II malocclusions in the horizontal as well as vertical growth patterns. With distinct malocclusions, however, there are differences in cervical vertebral phases. Aim: To evaluate, compare and correlate the skeletal maturation using CVMI stages in cases with Class II (horizontal and vertical) using 3D-DVT and lateral cephalogram. Methodology: In the present observational study, a total of 45 adult patients reporting at the Orthodontics department at a dental college and hospital set up in Wardha district and diagnosed with Class II Horizontal and/or Class II vertical will be recruited. The patients will be divided into three sub-groups and biosafety measures will be taken. Conclusions: This study would give us a scope to compare the efficiency of lateral cephalogram versus 3D-DVT in estimating the skeletal maturity of patients who need to undergo growth modification in the orofacial region and orthodontic treatment.

Abstract This article continues a discussion of the results of a nationwide study that reviewed 2100 impairment ratings and found a large number of errors (see the May/June issue of The Guides Newsletter). Spinal impairment ratings, for example, often are erroneous. Although the AMA Guides to the Evaluation of Permanent Impairment, (AMA Guides) clearly specifies use of the Diagnosis related estimates (DRE) method, evaluators sometimes incorrectly use the range-of-motion (ROM) method, which is fraught with potential error and typically results in higher impairment ratings. The most common problem associated with rating the lower extremities is combining multiple duplicative impairments. Multiple impairments typically are combined rather than added because the latter usually results in overrating impairments. A sidebar highlights red flags to erroneous AMA Guides ratings, and evaluators can take a number of steps to ensure accurate ratings. The first of these is to ensure an unbiased rating, preferably by a board-certified physician who, ideally, also has certification in the performance of independent medical and impairment examinations. The client requesting the evaluation should provide a cover letter describing the specifics of the evaluation, and the evaluator's report should comply with standards defined in the AMA Guides. All submitted reports should be reviewed by a physician experienced in the use of the AMA Guides; this cannot be accomplished by a nonphysician reviewer.

Abstract In the adversarial setting of an independent medical evaluation (IME), claimants may be incentivized to exaggerate the severity of their problems. The AMA Guides to the Evaluation of Permanent Impairment (AMA Guides) describes a protocol for assessing impairment associated with chronic pain, including the Impairment Impact Inventory (I3) that consists of 26 items that assess the burden of illness in three domains: pain intensity, interference with activities of daily living, and emotional distress. A study was performed to establish preliminary norms for the I3 and to compare responses of claimants undergoing IMEs with those of treatment-seeking chronic pain patients: 183 patients completed the I3, including fibromyalgia syndrome patients (FMS group), 35 multidisciplinary pain center patients (PC group), and 72 claimants undergoing IMEs. Patients in the latter two groups had a variety of chronic pain problems (and hypothetically may be more likely to exaggerate their problems). The three groups had similar mean scores on the total I3 and for each of the FMS, PC, and IME groups. Results support the hypothesis that participants, in aggregate, do not exaggerate their pain when they undergo IMEs, although some quite possibly do. These results are a step toward establishing a scientific basis for the impairment rating system described in the AMA Guides from consensus-based rules to an evidence-based system for making decisions about impairment.

Ethnobotanical exploration was undertaken to document the Wild edible plants (WEPs) utilised by the inhabitants of the Leh district. Traditionally exploited WEPs are an indispensable part of every household culinary. This place is known for its unique culture and cuisine which diversifies it from the rest of India. Pre-structured questionnaires, guided field visits, field examinations, and group discussions were conducted to gather ethnobotanical data. Detailed inquiries were made on the plant’s part used, time of collection and method of food preparation. In the present study, 40 wild consumable plants were documented, these belong to 18 families and spreads to 31 genera; out of which four are new records as wild food plants. They are Cotoneaster integerrimus, Dracocephalum heterophyllum, Astragalus frigidus, and Turritis glabra. Diverse use of wild leafy plants was observed and some of the most famous dishes are ‘Shangsho tsodma’ and ‘Kabra tsodma’ vegetables. The utilisation of wild plants helps enrich diet diversity and enhances the availability of green vegetables hence broadening food choices. Our study also reveals that the gathering of wild plants is confined to village people, shepherds, and farmers, and a majority of this knowledge exists in the memory of the elderly and these wild resources are under threat due to various anthropogenic activities. Therefore, the present finding highlights the value of these plants along with maintaining regional traditional knowledge and preserving the old ethnic traditional way of living and eating. Further, this information will provide baseline data to upcoming researchers dealing with nutrition and nutraceutical aspects. In addition, these wild plants are nutritionally rich and their consumption should be encouraged.

The Keller Plan for a personalized system of instruction as applied to college-level courses is usually characterized by (a) individual pacing, (b) a mastery orientation, (c) the presence of student tutors, (d) the use of printed study guides, and (e) the inclusion of a few lectures for enrichment (Kulik, Kulik, & Carmichael, 1974). In mathematics courses the plan has led to positive reactions from students, lower dropout rates, and higher final examination scores (Anderson & Pritchett, 1977; Peluso & Baranchik, 1977; Rogers & Young, 1977; Struik & Flexer, 1977; Waits & Riner, 1975; Weir, 1977). The issue of the long-term retention of concepts in personalized mathematics courses, however, appears not to have been addressed.

Objective To review the clinical pharmacology, pharmacokinetics, and clinical efficacy and safety of intravenous amiodarone. Data Identification Articles were identified through a computer search of the English-language literature using MEDLINE (KR Information OnDisc) and the search term amiodarone. Additional articles were identified through examination of the bibliographies of the articles initially retrieved. Study Selection Relevant or representative animal studies, clinical trials, and case reports were selected for evaluation. Particular emphasis was placed on studies pertaining to the use of intravenous amiodarone in treatment-refractory ventricular fibrillation (VF) and hemodynamically unstable ventricular tachycardia (VT). Data Extraction The literature was assessed for adequate description of patients, study methodologies (e.g., study design, number of patients), and outcomes. Data Synthesis Amiodarone is an unusual class III antiarrhythmic that produces each of the four main types of antiarrhythmic action in addition to other effects, such as vasodilatory, selective antithyroid, and other activities that may be therapeutically relevant. Amiodarone pharmacokinetics demonstrate extensive interpatient variability and are characterized by wide tissue distribution (steady-state volume of distribution 40–84 L/kg), slow total body clearance (90–158 mL/h/kg), long terminal elimination half-life (20–47 d), and extensive hepatic metabolism. The onset of maximal antiarrhythmic effect is a function of both amiodarone dosage and time. The high plasma concentrations achieved with intravenous dosing do not fully replicate the electrophysiologic effects observed following long-term oral administration, particularly with respect to class III activity. Available data suggest that intravenous amiodarone is associated with an efficacy rate of 50% or more in treatment-refractory VT/VF, and has a relatively rapid (2–24 h) onset of action. The drug is relatively well tolerated, but close hemodynamic, electrocardiographic, and hepatic function monitoring are required. The value of using amiodarone serum concentrations to guide therapy remains uncertain. Conclusions Intravenous amiodarone is an effective, relatively safe antiarrhythmic for the treatment of recurrent, hemodynamically unstable VT/VF refractory to other drug therapy in the acute care setting.

Recent advances in smartphone technology now allow clinicians to use commercially produced applications when studying for nursing certification examinations. The quality of currently available CCRN review applications varies in this first generation of products. Most are limited to multiple-choice practice questions, although a few have additional elements such as study guides and reference charts. Weaknesses found in the applications evaluated include poorly written and edited content, questions limited to rote memorization rather than application and analysis of knowledge, and content too basic or outside the scope of experienced critical care nursing practice. A list of important factors for consumers to consider before purchase is provided.

BackgroundLack of tumor-infiltrating T lymphocytes and concurrent T-cell dysfunction have been identified as major contributors to glioblastoma (GBM) immunotherapy resistance. Upregulating CXCL10 in the tumor microenvironment (TME) is a promising immunotherapeutic approach that potentially increases tumor-infiltrating T cells and boosts T-cell activity but is lacking effective delivery methods.MethodsIn this study, mesenchymal stem cells (MSCs) were transduced with a recombinant lentivirus encodingCxcl10,Nrf2(an anti-apoptosis gene), and a ferritin heavy chain (Fth) reporter gene in order to increase their CXCL10 secretion, TME survival, and MRI visibility. Using FTH-MRI guidance, these cells were injected into the tumor periphery of orthotopic GL261 and CT2A GBMs in mice. Combination therapy consisting of CXCL10-Nrf2-FTH-MSC transplantation together with immune checkpoint blockade (ICB) was also performed for CT2A GBMs. Thereafter, in vivo and serial MRI, survival analysis, and histology examinations were conducted to assess the treatments’ efficacy and mechanism.ResultsCXCL10-Nrf2-FTH-MSCs exhibit enhanced T lymphocyte recruitment, oxidative stress tolerance, and iron accumulation. Under in vivo FTH-MRI guidance and monitoring, peritumoral transplantation of CXCL10-Nrf2-FTH-MSCs remarkably inhibited orthotopic GL261 and CT2A tumor growth in C57BL6 mice and prolonged animal survival. While ICB alone demonstrated no therapeutic impact, CXCL10-Nrf2-FTH-MSC transplantation combined with ICB demonstrated an enhanced anticancer effect for CT2A GBMs compared with transplanting it alone. Histology revealed that peritumorally injected CXCL10-Nrf2-FTH-MSCs survived longer in the TME, increased CXCL10 production, and ultimately remodeled the TME by increasing CD8+T cells, interferon-γ+cytotoxic T lymphocytes (CTLs), GzmB+CTLs, and Th1 cells while reducing regulatory T cells (Tregs), exhausted CD8+and exhausted CD4+T cells.ConclusionsMRI-guided peritumoral administration of CXCL10 and Nrf2-overexpressed MSCs can significantly limit GBM growth by revitalizing T lymphocytes within TME. The combination application of CXCL10-Nrf2-FTH-MSC transplantation and ICB therapy presents a potentially effective approach to treating GBM.

Multidisciplinary forensic examinations reflect current tendency to integrate knowledge from various fields in order to objectively and impartially solve tasks of court proceedings, as integration of knowledge of specialists with different competences opens up new prospects for development of forensic examinations. Forensic economists who currently participate in multidisciplinary forensic examinations involving various kinds of interaction, sequence and degree of participation of each specialist in solving a common task have not been left behind. Article Purpose: to study into the need to develop scientific and methodological guidelines on the issues of carrying out forensic economic examinations while considering other types of forensic examinations. Leading methods of analysis and synthesis have been applied. The research paper substantiates the relevance of issues associated with conduct of forensic economic examinations taking account other types of forensic examinations. It is stressed that forensic economic examinations, during conduct of which results of other types of forensic examinations are used, do not fall under the concept of multidisciplinary forensic examination, since conclusions of other types of forensic examinations are not the research object of forensic economic examination. The paper emphasizes the need to develop an algorithm for conducting forensic economic examinations taking into account other types of forensic examinations. This will help forensic economists to use conclusions of other types of forensic examinations in multidisciplinary research without their evaluation, which in turn will enable to reduce the time for conducting forensic examinations and contribute to obtaining objective and unbiased expert conclusions.

Purpose – The purpose of this study is to establish the following research objectives based on necessity. This paper seeks to create a strategic plan to improve tourist satisfaction and loyalty by analyzing cultural tour guides’ competencies. Design/Methodology/Approach – In order to carry out this study, subjects are tourists that experienced the interpretation service of cultural tour guides in the Gyeongju region. As the result of a survey conducted from September 1st for 2018 around one month, a total of 252 valid samples were analyzed. SPSS 22.0 was used for analysis. Factor analysis, reliability analysis, correlation analysis, and multiple regression analysis were performed. The results of the study are summarized as follows. Findings – First, communication skills and sense of calling among cultural tour guides' competencies have a significant effect on tourist satisfaction. Second, tourist satisfaction was found to affect tourist loyalty. Research Implications – It is concluded that cultural tour guides' competencies have positive influences on tourist satisfaction and loyalty. It will contribute to the management of cultural tourism guides by suggesting that efforts are needed to strengthen the competency of cultural tour guides.

Background/Aim. Color matching in prosthodontic therapy is a very important task because it influences the esthetic value of dental restorations. Visual shade matching represents the most frequently applied method in clinical practice. Instrumental measurements provide objective and quantified data in color assessment of natural teeth and restorations. In instrumental shade analysis, the goal is to achieve the smallest ?E value possible, indicating the most accurate shade match. The aim of this study was to evaluate the reliability of commercially available ceramic shade guides. Methods. VITA Easyshade spectrophotometer (VITA, Germany) was used for instrumental color determination. Utilizing this device, color samples of ten VITA Classical and ten VITA 3D - Master shade guides were analyzed. Each color sample from all shade guides was measured three times and the basic parameters of color quality were examined: ?L, ?C, ?H, ?E, ?Elc. Based on these parameters spectrophotometer marks the shade matching as good, fair or adjust. Results. After performing 1,248 measurements of ceramic color samples, frequency of evaluations adjust, fair and good were statistically significantly different between VITA Classical and VITA 3D Master shade guides (p = 0.002). There were 27.1% cases scored as adjust, 66.3% as fair and 6.7% as good. In VITA 3D - Master shade guides 30.9% cases were evaluated as adjust, 66.4% as fair and 2.7% cases as good. Conclusion. Color samples from different shade guides, produced by the same manufacturer, show variability in basic color parameters, which once again proves the lack of precision and nonuniformity of the conventional method.

Methods. Patients’ DICM data of ankle via CT examinations were introduced into the MIMICS software to design the personalized guides. Two 2mm Kirschner wires were drilled with the help of the guides; the C-arm fluoroscopy was used to confirm the position of the wires before applying the cannulated screws. The patients who underwent ankle arthrodesis were divided into two groups. The experimental group adopted the 3D-printed personalized guides, while the control group received traditional method, i.e., drilling the Kirschner wires according to the surgeon’s previous experience. The times of completing drilling the Kirschner wires to correct position were compared between the two groups. Regular follow-ups were conducted to statistically analyze the differences in the ankle fusion time and AOFAS scores between the two groups. Results. 3D-printed personalized guides were successfully prepared. A total of 29 patients were enrolled, 15 in the experimental group and 14 in the control group. It took 2.2 ± 0.8 minutes to drill the Kirschner wires to correct position in the experimental group and 4.5 ± 1.6 minutes in the control group (p=0.001). No obvious complications occurred in the two groups during and after surgery. Postoperative radiographs confirmed bony fusion in all cases. There were no significant differences in the fusion time (p=0.82) and AOFAS scores at 1 year postoperatively between the two groups (p=0.55). Conclusions. The application of 3D-printed personalized guide in assisting the accurate drilling of Kirschner wire in ankle arthrodesis can shorten the operation time and reduce the intraoperative radiation. This technique does not affect the surgical outcome. Trial Registration Number. This study is registered on www.clinicaltrials.gov with NCT03626935.

The study explored experiences, leadership growth, and progression approaches adopted by women working within higher professional sports leadership organisations in Ghana. An interpretive research paradigm was adopted. The data was collected using interview guides and analysed with the Interpretative Phenomenological Analysis (IPA). The study revealed interest in a particular sport propelled women in sports leadership positions to select sports-related careers and accumulate the best knowledge and experience that placed them in such advantageous positions to make meaningful contributions to sports in Ghana. The study established that women with clear-cut career advancement pathways have it easy when it comes to assuming leadership positions. It was recommended that qualitative and quantitative examinations be made to understand the differences in the experiences of males in similar sporting positions. Majoon-e-Nisyan (MJN) is a polyherbal semisolid compound formulation. Its description is present in various Unani literatures. It is used in Unani medicine for its therapeutic efficacy against amnesia. There is no report regarding its safety on long term administration. Therefore, toxicological evaluation of MJN is carried out in rats. Majoon-e-Nisyan was subjected to 90-days repeated oral dose toxicity studies as per OECD guide line 408. Wistar rats were treated at three dose levels i.e., 500, 1000 and 2000 mg/kg bw and one vehicle treated group. MJN and vehicle were orally administered daily for 90 days and animal were observed for clinical signs of toxicity, mortality, body weight and feed consumption. On completion of 90-days, blood samples were collected and analyzed for hematology and biochemistry. Necropsy was performed on all survived animals and vital organs were collected and subjected to histopathology. No post dose adverse effect was reported on survival of both male and female rats after oral administration of MJN for 90 days. No incidence of mortality was reported in MJN treated male and female rats at all tested dose levels. No abnormal clinical signs were observed in MNJ treated animals at 500, 1000 and 2000 mg/kg bw as compared to animals of control group. No significant changes were observed in biochemistry, hematology and histopathological examination. No incidence of mortality, adverse changes in clinical signs of toxicity or body weight gain of rats was noted. No changes in clinical chemistry, hematology, and histopathology were observed in MJN-treated or control group. Therefore, NOAEL for MJN may be considered more than 2000 mg/kg bw in rats. The study investigated the influence of physical facilities on academic performance of community secondary schools students’ in the form four national examinations in Longido District, Tanzania. The study was guided by one research question aimed at finding out how physical facilities influence academic performance of community secondary school students in form four national examinations. Social Learning Theory developed by Psychologist Albert Bandura in 1977 guided the study. Convergent design under mixed research method was adopted whereby probability and non-probability sampling were used in selecting respondents. The target population for this study included 9 community secondary schools,9 heads of community secondary schools, 283 teachers and 1545 form four students in Longido District, Tanzania. Stratified and simple random sampling techniques were used to obtain 4 schools, 32 teachers and 160 form four students where by 4 heads of community secondary schools were directly included from the study which make total of 196 respondents. Questionnaires, interview guides and document analysis guide were used to collect data from respondents. Cronbach Alpha was used to obtain reliability of quantitative data (r=0.78 for teachers’ questionnaire and r=0.62 for students’ questionnaire) While member checking and triangulation were used to obtain reliability of qualitative data. Quantitative data was analyzed and presented by using frequencies, percentages, means and tables while qualitative data were analyzed into words and developing themes from the research questions and presented in summaries and direct quotations. The study found out that shortage of school physical facilities like classrooms, dormitories, libraries, laboratories, dining halls and teaching and learning materials like books, chalks, manilas, laboratory apparatus and others influence poor academic performance of students. The study concluded that insufficient or sometimes absent of school physical facilities, had negative influence on students academic performance of form four students in national examinations in Longido District. The study recommends that government and all stakeholders to work together and collaborate in order to construct and provide school physical facilities in order to solve the problem of poor academic performance of students in examinations.

The theoretical-methodological foundations of the development of forensic economic examination regarding the determination of classification features of its species composition have not only theoretical-methodological, but also direct practical significance in the activities of expert economists, as well as judicial investigative bodies in the event that the latter appoint this type of examinations. This research paper’s purpose is to study the criteria for the classification of forensic economic examinations, established by soviet scientific thought, aimed at improving the systematization of their species composition. Using the techniques of analysis, synthesis, induction, deduction and generalization, it was determined that the emergence of new types and sorts of forensic economic examinations depends on factors that can be combined into two groups: 1) socio-economic relations — a change of their format; 2) judicial and investigative practice —emergence of new objects of research and tasks; 3) economic offenses — new ways, techniques of committing economic crimes. It has been established that the structural differentiation of this type of forensic examinations is due to the procedural needs of the judicial and investigative practice. It has been found that the criterion for systematization of the species composition of forensic examinations belongs to the economic plane, namely to the distribution of social production into the spheres of material and immaterial production. The next stage of the distribution of economic examinations is to the relevant industries according to certain types of activity with the definition of the main criterion of classification — according to the type of production activity. Thanks to this, compliance with the set of requirements for the scientifically based selection of classification criteria with the distinction of sorts and types of economic examinations is ensured.

Given Mrs. A's medical history of general health and her elevated stress levels prior to the depigmentation, the diagnostic procedure is carefully broken down, including physical examinations, medical history evaluations, and pertinent laboratory testing. Examining the epidemiology of vitiligo, the study clarifies its worldwide frequency, age at start, genetic predispositions, autoimmune correlations, and subtle effects on people from various ethnic origins. This case study stands out for its examination of the discrepancy between traditional textbook representations and the dynamic, Mrs. A's emotional and psychological reactions to vitiligo require individualised care. A multidisciplinary approach guides the development of treatment strategies that include photo therapy, topical corticosteroids, and specialised psychological support. This cross sectional descriptive study was conducted to observe the quality of Pharmacology professional MCQ papers of different Universities of Bangladesh. For this purpose, total 80 MCQ papers of five universities dated from January 2007 to July 2015 were reviewed against a checklist to find out the coverage of recall, understanding and problem solving type questions, content coverage and construction of each question. The mean of recall, understanding and problem based type questions of all the universities were 92.1, 7.8 and 0.1 respectively and all of them statistically significantly differed from curriculum standard. All of the MCQ papers contained more than 75% recall type questions and 27.5% MCQ papers had only recall questions, Only 1(1.2%) MCQ papers contained 100 % topics (all the 11 groups). Eighteen (22.5%) contained 10 groups and 26.3% contained 9 groups out of 11. Forty percent MCQ papers contained less than 80% topics. Most of the stems of the MCQ were in the form of incomplete sentence (53.5%), 28.4% were in the form of complete statement and 16.8% were in the form of single word. Only 0.4% stem were in the form of clinical scenario. Fifty (62.5%) MCQ papers had defective stems and 68.75% had defective options. Total 14 (17.5%) MCQ papers were without any flaw but the rest 88.5% MCQ papers were with flaws either defective stem or faulty options or both. Findings of this study may be used to redefine the distribution of different type of questions in SAQ papers and to improve the quality of question papers by ensuring their coverage. This study undertakes a thorough review of the ethnomedicinal properties of salt-tolerant plants and their potential to treat neurological disorders and enhance mental health. Aimed at bridging the gap between historical knowledge and contemporary scientific validation, our research meticulously evaluates both the traditional uses and the existing scientific evidence supporting the neuroprotective effects of these plants, leveraging in vitro and in vivo experimental findings. Through a comprehensive search of articles from 2001 to December 2023 across scientific databases, we identified sixteen species across nine plant families with demonstrated in vitro neuroprotective properties. Among these, the Chenopodiaceae and Juncaceae families emerged as the most represented, including plants such as Salicornia sp., Juncus sp., and Limonium sp., primarily recognized for their cholinesterase inhibitory activity. However, a notable disparity exists between traditional applications and scientific examination, with only six species undergoing in vivo testing. This discrepancy underscores the imperative for future research to delve deeper into validating traditional uses and elucidating the mechanisms underlying neuroprotection. Our findings highlight the need for research on salt-tolerant plants traditionally used for neurological benefits. Key steps include systematic screening, identification of active compounds through bioassay-guided fractionation, and in vivo testing. Integrating traditional knowledge with modern pharmacology, while emphasizing sustainable and ethical approaches, is essential for advancing neuroprotective drug discovery.

The diagnostics and treatment of stomach cancer is still one of most complex and actual issue, despite of morbidity decreasing tendency. Objective. Improvement of algorithm of diagnostics of patients with complicated stomach cancer. Materials and methods. It was shown the analysis of results of surgical treatment of 418 patients with complicated stomach cancer, which got the hospital treatment at GI «V.T. Zaycev Institute of General and Urgent Surgery of NAMS of Ukraine « from 2010 till 2019, aged 29 till 76. Course of the disease was complicated with bleeding in 252 (60,3 %) case, with stenosis in 89 (21,3 %), with perforation in15 (3,5 %), and with its combination — in 62 (14,8 %). Radical operations were performed in 168 (40,2 %) cases, palliative and symptomatic in 250 (59,8 %) — cases. 107 patients aging 36 till 73 were examinated with SCT to revealing and staging of tumor. Results. Patiens were examinated with using of combination of instrumental methods. SCT provides to detect the primary tumor, the process spreading, differetion of tumor from the healthy tissues of stomach, local lymphatic nodal metastasis and other organs, that definite staging of pathology, volum of operation and further prognosis. SCT has precission of 95-97 % at detecting of cancer, definition of stage — 77–80 %. Endoscopy allows to define the location, spreading of process, sizes of tumor and presence of complications. Panoramic x-ray provide to detect the free air in abdomen, that is main symptom of perforation, but doesn`t indicate the localization and root of perforation. Angiography allows to identify the sources of tumor`s blood supply, and also continuing bleeding direct and indirect markers, that leads to conversion of diagnostics into curative. Bleeding vessel`s embolization were performed in 7 cases as first treatment stage (the second one is operative), and in 11 cases as finishing curative method. There no any ideal oncomarker. Diagnostic precission of CA 72-4 is 28–80 % (40–46 % on the average). According to international guides [7], all patients were performed the diagnostic laparoscopy with researching of washout liquids of abdomen in uncertain cases. Conclusion. Only complex examination of patients with stomach cancer with using combination of SCT of abdomen, FEGDS with biopsy, laparoscopy can provide total volum of examination, staging and surgical aid.

Dampness-heat syndrome diarrhea (DHSD) is a common clinical disease with a high prevalence but still has no satisfactory therapeutic medicine, so the search for a safe and effective drug candidate is ongoing. This study aims to explore the efficacy and mechanisms of Lianweng granules (LWG) in the treatment of DHSD and to identify the blood transport components of LWG. We assessed the efficacy of LWG in DHSD by various in vivo metrics such as body weight, disease activity index (DAI), histopathologic examination, intestinal barrier function, levels of inflammatory, apoptotic biomarkers, and oxidative stress. We identified the blood components of LWG using ultra-high performance liquid chromatography-mass spectrometry/mass spectrometry (UHPLC-MS/MS), and the resolved key components were used to explore the relevant targets. We next predicted the potential mechanisms of LWG in treating DHSD using network pharmacology and molecular docking based on the relevant targets. Finally, the mechanisms were validated in vivo using RT-qPCR, Western blotting, ELISA, and immunofluorescence and evaluated in vitro using Cell Counting Kit-8 (CCK-8), small interfering RNA, cellular enthusiasm transfer assay (CETSA), and drug affinity response target stability (DARTS). Ninety-one pharmacodynamic components of LWG enter the bloodstream and exert possible therapeutic effects. In vivo, LWG treatment improved body weight, reduced colonic injury and DAI scores, lowered inflammation, oxidative stress, and apoptosis markers, and partially restored intestinal barrier function in DHSD mice. Guided by network pharmacology and molecular docking, it is suggested that LWG may exert therapeutic effects by inhibiting IL-6/STAT3/PI3K/AKT signaling. LWG significantly decreased the expression of IL-6, p-STAT3, p-PI3K, p-AKT, and other proteins. These findings were supported by in vitro experiments, where CETSA, DARTS, and siRNA evidenced LWG’s targeting of STAT3. LWG targeted STAT3 to inhibit inflammation, oxidative stress, and apoptosis in the colon, thereby restoring the intestinal barrier function to some extent and exerting a therapeutic effect on DHSD.

Breast cancer is a major killer disease among women and an overwhelming number of women have fallen victim to this deadly disease worldwide as well as in Egypt. Thus, early detection of breast cancer is crucial to decreasing the morbidity and mortality of breast cancer. This qualitative, exploratory descriptive study aimed to understand the perceptions of Egyptian middle aged women about breast self-examination (BSE). The study was conducted in two rural areas in Aswan, Egypt, using the purposive sampling technique; twelve (12) middle aged women were selected as the study participants. Data was collected through semi-structured face to face interviews using an interview guide and an audiotape recorder. The interview was in the Arabic language. The qualitative content analysis (Unrau & Coleman, 1997) using the manual method, was utilized as the analytical method to identify emerging Categories and themes. The Cognitive domain was identified as the main theme with three categories: knowledge about breast cancer, knowledge of breast self-examination and preventive measures for breast cancer. The overall findings revealed that middle-aged women living in the rural areas in Upper Egypt lack knowledge about breast cancer and breast self- examination. In addition, they have a negative attitude towards BSE, thus the practice of BSE is very low, and in some cases, non- existent among women in Upper Egypt’s rural areas This study has implications for policy makers and other stake holders. Hence, it is recommended that accurate knowledge, support and service about breast cancer and BSE be provided in the region, to improve the practice of BSE as well as to decrease the mortality rate of breast cancer in Egypt.

This cross sectional descriptive study was conducted to observe the quality of Pharmacology professional written question papers of Bangladesh University of Professionals (BUP), University of Chittagong (CU), University of Dhaka (DU), Rajshahi University (RU) and Shahjalal University of Science & Technology (SUST). For this purpose total 82 SAQ papers of five universities dated from January 2007 to July 2015 were reviewed. Question papers were reviewed to find out the coverage of recall, understanding and problem solving type questions, content coverage and presence of marking scheme in SAQ papers. Mean percentage of recall, understanding and problem questions were 54.3%, 44% and 01.7% respectively in SAQ papers. Mean of the recall questions of SUST statistically significantly differed from curriculum standard 60%. Other universities had no significant differences with the standard. Mean of the understanding questions of all the universities statistically significantly differed from curriculum standard 30% except BUP. There was statistically significant difference between mean of the problem solving questions of all the universities and curriculum standard 10% . Most of the SAQ papers (62%) were without problem based questions. No question paper was found having different types question as per curriculum standard. Total 15(18.3%) SAQ papers contained 100 % topics (all the 11 groups). Thirty nine (47.6) contained 10 groups and 22% contained 9 groups out of 11. Twelve percent SAQ papers contained less 80% topics Total 29 (35.4%) SAQ papers of all the universities showed marking scheme on the questions papers, rest 64.6% were devoid of it. Maximum 87.5% SAQ papers of RU had marking scheme. It explores how a master degree holder in English education joined at university and constructed and reconstructed his identity in his teaching profession unless he retired. Data were collected from face to face in depth interviews and telephone conversation then analyzed through narrative case analysis. The findings show that a university English teacher develops and modifies his identity through engaging himself in different professional development activities, interacting with different socializing factors and experiencing various emotions in university activities. He also works as curriculum developer, examiner, and invigilator, external and internal examiner in practical examinations. He has to labour hard in his whole professional period and has to balance his emotional behaviours, that is, happy moments as well as frustrated moments. The paper concludes with some implications for language teachers in context of education campuses under faculty of education in T.U.

This research study investigates health news coverage in the two mainstream English-language newspapers of Pakistan – Dawn and The News – to understand how they covered it. The researchers have focused on the frequency of published news stories, contributors of the news items; nature of coverage;contextualization and use of jargon in the published stories. The researchers applied the Quantitative Content Analysis to answer all the outstanding inquiries of this research project. Based on the data collected in February2019, the researchers found that both the newspapers played down the health-related coverage in their pages –besides relying on the same contributors to report it. The findings revealed that The News needs to bring more diversity in its coverage of different issues related to health, while Dawn needs to work on the language – by avoiding jargon. The Gatekeeping Theory guides the findings.

ABSTRACTData quality assurance (DQA) is critical to research data sharing and reuse. There has been a growing recognition of data transparency, reproducibility, credibility, and validity in research. Although the research data curation literature is large, it lacks data quality theory‐guided examinations of DQA practices in research data repositories. This poster paper reports on the preliminary findings of a larger study that examines DQA practices in research data repositories, including their use of DQA ontologies, standards, and metadata vocabularies. In particular, the paper examines two quality standards and an ontology for their conceptualization of DQA activities and their structure. The authors used the findings of the analysis and the data quality literature to synthesize an initial model of a DQA process in research data repositories that conceptualizes three DQA activities: evaluation, intervention, and communication. This paper can inform the development of ontologies and best practice guides for designing and evaluating DQA workflows in research data repositories.

Demand for higher education in Kenya has increased despite insufficient funding from the Government. This led to the cost-sharing policy where both the Government, the students, and their households share the costs of higher education. Students are provided with loans through Higher Education Loans Board (HELB). Public universities are supported through capitation grants to meet development and recurrent expenditure. The household contributes towards tuition and upkeep of their students. At Maasai Mara University, it has been observed that at the end of every semester, some students defer their examinations due to fee balances implying that there are gaps in the existing methods of financing a university education. This study, therefore, investigated the effectiveness of the financing methods and their implications on students’ access, completion rates, and quality education based on students’ experiences at the University. The study used a descriptive survey design. The target population was 950 education students who were in their third and fourth years of study. Stratified random sampling was used to select 315 students to participate in the study. Data were collected through questionnaires and interview guides for the class representatives. Documents were also analyzed. The study found that the majority of the students depended on HELB and support from their households to finance their education. The amount of loans allocated to students was inadequate and multiple sources of funding are required to meet the full costs of education. Students from poor backgrounds resorted to menial jobs which affected class attendance and learning outcomes. Other students deferred their examinations affecting academic progression. The study concluded that the current methods of funding university education were not dependable as they negatively affected students’ access, academic progression, completion rates, and quality of learning outcomes. The study recommended the need for an alternative financial support system for students from poor families to complete their studies on schedule.

Written examination is an important method for juddering the learning of medical students. Short Answer Questions (SAQ) and multiple choice questions (MCQ) are introduced in MBBS curriculum to assess student cognitive domain. This cross sectional descriptive study was conducted to get the teachers’ opinion about Pharmacology professional written short answer question papers of different Universities of Bangladesh. For this purpose, opinion of total 56 purposively selected teachers of the Department of Pharmacology & Therapeutics of 20 Medical Colleges were collected through self-administered semi-structured questionnaire. Most of the teachers (66% & 61%) were in favour of existing distribution of different types of question in curriculum (recall-60%, understanding -30% and problem based-10%) for short answer question paper (SAQ). Majority of the teachers (66% & 54%) were satisfied with the present coverage of recall and understanding type questions respectively but only 41% teachers were satisfied with the coverage of problem based type questions. Seventy-seven percent teachers were satisfied with content coverage in SAQ papers. Eighty-seven percent teachers were satisfied with present marking scheme on SAQ paper. Findings of this study may be used to redefine the distribution of different types question in SAQ papers and to improve the quality of question papers by ensuring their coverage. About 57% teachers were satisfied with the construction of MCQs and 78% teachers were in favour of introduction of single best answer (SBA) type MCQ. Regarding introduction of negative marking for wrong answer, 61% were agreed with the proposal. Conclusion: Findings of this study may be used to redefine the distribution of different types question in SAQ papers and to improve the quality of question papers by ensuring their coverage and construction. The aim of this study was to investigate the effects of school learning facilities on students’ academic performance of Form Four National Examinations in public secondary schools in Unguja, Zanzibar, Tanzania. The study employed a convergent research design under mixedmethod approach, which involved collecting and analyzing both quantitative and qualitative data simultaneously. The target population for the study was 3,558 individuals, and a sample size of 266 respondents was selected. The sample included six heads of schools, who were chosen using a total population sampling technique, 24 teachers were selected through stratified sampling technique, 218 students through stratified sampling technique, and 18 parents through convenience sampling technique. Data was collected using questionnaires, observation schedules, and interview guides. Quantitative data was analyzed descriptively, employing frequencies, percentages, and mean scores, which were presented in a table. On the other hand, qualitative data was analyzed through paraphrasing, quoting, and narration. The findings revealed that school learning facilities significantly affects students’ academic performance in public secondary schools. The researcher recommends that, the government should continue providing more learning facilities to public secondary schools, in order to make them readily available, parents should also be involved in providing school learning facilities to public secondary schools.

Therefore, the formation of MIA to handle investigative examinations is BPK's answer to the phenomena that occur in society related to the quality of the results of BPK investigations. However, even after the formation of MIA, there were still complaints from the public regarding the quality of the results of investigative examinations. BPK received a lot of attention because of actions that were not in accordance with the BPK auditors' code of ethics, namely integrity violations that resulted in arrest by law enforcers and undergoing trial at the Corruption Court. For this reason, auditors must always maintain an attitude of mental independence, in all matters relating to the provision of audit services, to improve audit quality. This study aims to analyze how independence and integrity can increase the quality of investigation audit. This study involved 267 investigators of Law Enforcement Agencies as respondents using a questionnaire. Furthermore, the data were analyzed using descriptive statistical analysis and SEM techniques. The results of this study provide empirical evidence that independence and integrity have a significant positive effect on the quality of investigation audit.

Through a thorough examination of literature, the study aims to uncover the underlying causes behind the prevalent animal-related offenses in the country. The findings reveal that several key factors contribute to this issue, including a lack of awareness regarding animal welfare, inadequate enforcement of animal protection laws, and cultural attitudes that diminish the value of animals. To tackle this problem effectively, the study proposes a range of potential solutions. These include raising public awareness about animal welfare, enhancing the enforcement of existing animal protection laws, and fostering a cultural shift that recognizes and respects the intrinsic worth of animals. One crucial aspect of this study is its revelation that animal crimes are not haphazard acts, but rather, they are deeply rooted in social and cultural influences. By pinpointing these contributing factors and offering practical remedies, this research holds significant importance as it can guide policy-making and direct actions to mitigate animal crimes in Malaysia effectively.

Objective: The present study was initiated to describe the gastroprotective role of quercetin (Qu) and ellagic acid (EA) on aspirin-induced gastric ulcer (GU) in rats.Methods: Forty adult female albino rats of Wistar strain were distributed into: Control group, GU group, Omeprazole group, Qu group, and EA group. Gross examination, biochemical analyses including serum adrenocorticotropic hormone (ACTH), serotonin (ST), ferritin, heme oxygenase-1 (HO-1), interleukin-2 (IL-2), advanced glycosylation end products (AGEs), and fibronectin (FN) levels were estimated. Moreover, histopathological and histochemical examinations of stomach tissue samples were carried out.Results: Gross examination of gastric mucosa of rats in GU group revealed hyperemia of the stomach mucosa. Furthermore, rats in GU group experienced a significant rise in serum ACTH, ferritin, HO-1, IL-2 and AGEs levels accompanied with significant drop in serum ST and FN levels versus control counterparts. Pre-treatment of GU group with Omeprazole, Qu or EA caused marked improvement in the measured biochemical parameters. Histopathological and histochemical examinations of stomach tissue samples documented the protective action of Omeprazole, Qu and EA with different degrees against GU caused by aspirin.Conclusion: As a conclusion to this study, we can state that both Qu and EA have gastroprotective effect against aspirin-induced GU in rat model. Of note, Qu showed superior impact than EA as an antiulcer agent in this study. The corresponding mechanisms are speculated to be associated with inhibiting stress-induced gastric lesion, attenuating the oxidative stress, iron chelation and blunting ferritin level, modulating inflammatory cascade, and promoting the healing process.

Introduction: The goal of clinical practice is to provide students with practical training experience and the ability to work as a team. Objective: This study aims to analyze the effectiveness of a phantom-based mannequin in improving midwifery students’ performance in vaginal toucher (VT) examinations Materials and Methods: This is a quasi-experimental study with a pre-test/post-test design. Participants were 70 final-year midwifery students in two groups of factory mannequin (n=35) and modified mannequin (n=35). The instrument used for assessing performance was a validated 10-item scale. Data analysis was done using paired t-test, Kolmogorov-Smirnov test, and ANCOVA. Results: Participants had a mean age of 21.6±2.2 years in the modified mannequin group and 21.3±2.4 years in the factory mannequin group. There was no significant improvement in students’ performance after using the factory mannequin, but the modified mannequin improved their skills, since there was a significant difference in VT examination performance before and after using the modified mannequin (mean difference=3.97, P=0.001). The ANCOVA results showed a significant difference in the mean post-test scores between the factory and modified mannequin groups (mean difference=1.59,η2=0.299, P<0.05). Conclusion: The modified mannequin is more effective than the common factory mannequins in improving the VT examination performance of midwifery students. For this reason, these devices are highly recommended for practical learning of VT examinations.

Background: The term “stress” was first coined by the endocrinologist Hans Selye in the year 1930’s. Stress causes imbalance in the sympathetic and parasympathetic functions which can lead to disturbance in the body homeostasis. Medical education in India is vast, extensive and is more stressful. During undergraduate course medical students have to undergo many examinations such as internal assessment and university examinations, examination is one of the most common stressor for students. As performance in examinations determines the future of students, it is important for medical educators to know the prevalence, and causes, of stress among students at different time intervals. Aims and Objectives: The aim of the study was to study the influence of examination stress on 1st MBBS students of BRIMS, Bidar from there admission (no examination) to final university examination on blood pressure and pulse rate. Materials and Methods: The present study was conducted after obtaining the Ethical clearance from the institutional research and ethical committee of Bidar Institute of Medical Sciences, Bidar. The study is carried out in the department of Physiology, BRIMS, Bidar. Out of 150 1st year MBBS students, healthy 102 students aged 17 to 20 years were selected for the study after following the strict inclusion and exclusion criteria. After recording basic physical characteristics, Hemodynamic parameters such as Pulse Rate (Beats per minute), systolic blood pressure (SBP) (mmHg), and diastolic blood pressure (mmHg) were recorded in three sessions, 1st session 1 month after the admission, 2nd session 6–8 days before 1st internal assessment, and 3rd session 6–8 days before final university examinations. The values obtained were analyzed by using t-test. P < 0.05 was considered as statistically significant. Results: Results revealed that there was statistically significant increase in the values of pulse rate (bpm), SBP (mmHg), and diastolic blood pressure (mmHg) in the study subjects when non examination period (1st session) was compared with the values of before 1st internal assessment (2nd session) and before final university examination (3rd session). Conclusion: Thus our study concluded that the values of pulse rate, SBP, and diastolic blood pressure were observed to be higher before 1st internal assessment and before university examinations when compared with after 1 month of their admission to college (i.e., no examination time).

Polycystic ovary syndrome (PCOS), is a heterogeneous disorder found in 3% - 10% of women worldwide, and it constitutes about 80 % of anovulation infertility. In cases of resistance to CC surgical option in form of ovarian drilling, for using maneuver with less invasive than laparoscopy we study the effect of transvaginal ovarian drilling under ultrasound guide in patients with resistant polycystic ovary syndrome to clomiphene citrate This study was conducted o evaluate the effectiveness of transvaginal ovarian drilling under ultrasound guide in patients with resistant polycystic ovary syndrome to CC. In this, A prospective, clinical trial study 42 infertile women with PCOS resistant to 3 months course of treatment with CC enrolled in this study. The preoperative assessment includes history, physical examination, the serum levels of FSH, LH, AMH, testosterone, and TVS was done for ovarian morphology. Transvaginal ovarian drilling under ultrasound guide using a needle connected to manual vacuum pressure that punctured each ovary between 3 – 6 punctures. Postoperatively Signs of spontaneous ovulation, levels of FSH, LH, AMH, testosterone, and pregnancy rate were recorded. The results revealed that There is a significant reduction in the level of LH, FSH, Testosterone, and Anti-Mullerian hormones. Signs of spontaneous ovulation was developed, and the pregnancy rate after the operation was recorded. This low cost, no recorded adverse effects, and quickly done manoeuvre has an excellent result in improving fertility rate in resistant PCOS women to CC.

Abstract (sommario):
The purpose of the study was to establish the role of Instructional Materials in the Learning of Geography Subject in Secondary Schools of Busega District. The study adopted the mixed research approach under which convergent parallel design was used. The study involved 23 secondary schools in Busega District whereby four secondary schools were purposively sampled to participate in the study because the schools had exhibited a severe declining performance trend in Geography at the National examinations for last five years. The study used purposive, stratified and simple random sampling techniques to select 1 DEO, 4 Heads of Schools, 4 Heads of Geography Department, 8 Geography Teachers and 80 students to participate in the study. Questionnaires and interview guides were used for data collection. Quantitative data was analyzed using descriptive statistics with the help of the SPSS software while the qualitative data was analyzed thematically. The study concluded that the use of learning materials in geography contributed to enhanced creativity and innovativeness in learners. Learning materials further assisted learners to comprehend difficult topics in Geography. It also made learning very interesting and this motivated learners to fully participate in the learning process. The study recommended that the use of instructional material should be emphasized in public secondary schools as they played a key role in enhancing productive learning of geography.

This study conducted to find out the information seeking pattern and determine the major resources of information major source of information satisfying the needs of Indian and foreign tourists with special preference to tourist visitors of Kovalam tourist region, Trivandrum district of Kerala state. Questionnaire methodology adopted for data collection. The sample population is the tourist visitors of Kovalam tourist region in the period of Jan 2012 to Dec 2013.Results shows that Online searching are the major method used for seeking and collecting information for Indian and foreign tourists but tourist guides the major source of collecting reliable information for both Indian and foreign tourists.

Background: Examinations are the tools for assessing medical students. Traditional summative assessment helps in assessing knowledge and success of the student at the end of the year whereas formative assessments help in giving timely feedback. Aims and Objectives: This study was done to compare the effect of repeated examinations otherwise formative assessment with the assessment done after 3 months on the performance of medical students. Materials and Methods: This study was conducted in the Department of Physiology, ESIC Medical College, Hyderabad. A total of 98 students were taken, they were assessed in the physiology first internals with the essay type questions without any weekly tests then weekly tests were given and assessed in the physiology second internals with the essay type questions after 3 months. Results: Results of first internal and second internals were compared using Student’s test. It was found that there was a significant improvement in the performance of the medical students (P < 0.001) after conducting weekly tests. Conclusions: This study revealed that there is a link between the regular weekly test and increase in the performance of the medical students. It also builds confidence among the medical students.
