Testo completoThis study examined the professoriate view of spirituality at a private university in Lima, Peru. A two-phase, sequential mixed method was used. Two hundred forty professors completed Paloutzian and Ellison’s (1982) Spiritual Well-Being Scale (SWBS) for the quantitative phase. Five research questions exploring the professors’ gender, general area of teaching (humanities vs. sciences), highest university degree earned, age, and total years of university teaching experience were analyzed to determine if any significant differences existed within those variables. Two non-parametric tests were used: the Mann-Whitney U test and the Kruskal-Wallis Test. It was discovered only gender showed a significant difference. Five surveyed professors participated in the focus group phase exploring significant gender difference on the spiritual well-being scores. Five questions explored this difference between males and females. Four themes emerged: gender role enculturation, biological motherhood, stress, and internal strength.

The Metropolitan Area of Lima has on average 3.6m² of green area per person, for a total of 10 million inhabitants. Although this is not the most accurate metric, it is the most available proxy to measure and understand the magnitude of open public space in the city. In addition, it is not equitably distributed: districts with higher socioeconomic levels and larger municipal budgets have greater area and higher quality public spaces. In a context of inequitable distribution on quantity and quality, one of the biggest threats that public spaces face is their privatization, a process in which a space is dispossessed from the public and transformed for a private or restricted use.
From sidewalks, streets, parks, and plazas, to natural spaces such as beaches and the coastal lomas natural ecosystems, in recent years, these unprotected areas have become shopping centers, supermarkets, parking lots, private clubs, formal and informal housing, amusement parks, synthetic grass courts, and other infrastructure that has altered at some degree its openness, ownership, accessibility, and function. This shift from public to private spaces ultimately reduces the opportunity of all citizens to have available open public spaces, increases social fragmentation, and ultimately deepens issues of social injustice and spatial inequalities. In such a scenario, this thesis examines the conditions under which open public spaces are privatized and identifies the mechanisms.
Through different case studies and interviews, I create three types that attempt to explain the different forms in which privatization develops to expose the motivations behind it, the processes of how it happens, the actors who are involved, and the manifestations it has in the built environment. The first type is Concession for Development, and takes place when public space is rented to private entities in the form of concessions with the excuse of bringing development and improvement. The second is Appropriation for Livelihood, and occurs when public space is informally appropriated to fulfill a basic need such as housing or a productive activity. The third is Enclosure for Control, and results when public space is enclosed and its access is restricted in order to provide safety or facilitate its management. I analyze and expose the structural governance conditions and flaws in current planning processes -- formal and informal, top-down and bottom-up --
that lead to privatization in order to help create awareness about how and why this invisible phenomenon takes place and who is most affected by it. Finally, this thesis proposes recommendations that can help Lima and other Peruvian cities promote the protection and preservation of public spaces and also encourage a more equitable distribution.

As urban transportation deteriorates, chaos increases putting aside urban patterns, systems and networks to prioritize the implementation of modern technologies. As a consequence of this situation and as a means to fulfill the requirements of the growing city, the larger built forms in the city are put in the spotlight and made busier and more complex. The connectivity that once ordered the transportation networks throughout the urban form diminishes, almost vanishes. There are different scales of movements, at different paces and levels (in all three dimensions), which need to be understood in order to get a good reading of the activity of the city. The site is located in the district of San Martin de Porres in the city of Lima, Peru and this thesis aims to fill the existing void represented by it in its context. It also aims to reconnect and re-vitalize the area of study by using transit-oriented development and spatially identifying elements. Another goals is to meet the needs of transportation networks and people through spatial relations, so as to generate a prototype that establishes processes and results to promote urban growth. This thesis presents the possibility to synthesize such movements (now scattered and unintelligible) and to provide a space for the junction and strengthening of the existing links between people and transit that occupy the urban landscape. The implementation of a Multimodal Passenger Terminal to serve both local (urban) mass transit and regional transportation is the main compositional element of the project. This will be complemented by other programmatic elements such as: a civic plaza and a communal green space, a series of smaller, more intimate open spaces, community services, retail and office space, movie theaters and a hotel. They will as a whole serve as supporting activities for the area and as a point of destination for the neighbors. The analysis of the transit network - regional and local - as well as of the area's land use and green space systems will allow for the better understanding of the context and will set the stage for the remainder of the study. Pedestrian behavioral patterns in the area also provide necessary information that is used to determine accesses and paths or concentration points in the project. The resulting elements are distilled and categorized into several design guidelines and are placed in a timeline for phased development. A series of organizational and distribution diagrams have been produced to reflect the findings. All of these could serve as a starting point for future development in other locations with similar conditions in the city or the country. They could be used individually or jointly depending on the demands and needs of the project, the people, the existing systems and the site, at a given time.

Testo completoThe following article explores the work experiences and individual perceptions of a group of professionals from Lima (Peru) regarding interlingual subtitling. Such study was conducted using qualitative methods. A total of twelve semi-structured interviews were made, including four groups of participants: companies and professionals of the translation field, as well as companies and professionals of the audiovisual communication field. Although participants believe that subtitling does not have an established market in the Peruvian capital, they do think that it is a niche with a growing demand. Likewise, there is also a distinction between the way translation and audiovisual companies and professionals work, particularly with regard to the subtitling process and the use of guidelines, as well as other criteria used to ensure the quality of the service.

Testo completoAcute respiratory infections (ARI) are the main cause of morbidity and mortality in children under 5 years worldwide. Bordetella pertussis is a highly contagious bacterium that can cause serious illness, and approximately half of infected infants less than 1 year old are hospitalized. Also, pertussis immunization series is not completed until six months of age, leaving young infants vulnerable to pertussis. In Peru, pertussis is an increasing health problem despite immunization efforts, and the role of B. pertussis in ARI is unknown. We determined the prevalence of B. pertussis among children under 5 years old admitted to Hospital Nacional Cayetano Heredia in Lima with diagnosis of ARI between Jan-2009 and Dec 2010. Epidemiological and clinical features were collected, and presence of B. pertussis was determined by PCR (pertussis toxin and IS481 gene). A total of 596 nasopharyngeal samples among children under 5 years were analyzed. In 114 (19.1%) samples were positive for B. pertussis. 32.5% of sample positive to B. pertussis were diagnosed as viral pneumonia at diagnosis. Importantly, 71.9% of cases were under 12 months of age and 58.8% have been contact with other ARI infected people. Significant differences in clinical symptoms between the total ARI cases and B. pertussis cases were not found. The most frequent symptoms in B. pertussis cases were fever (100%), rhinorrhea 78%, cough 71.9% and respiratory distress 60.5%. One child died due to the infection. B. pertussis cases showed a seasonal distribution with peaks during the months March June and November. This study shows the high prevalence of B. pertussis in infants who were hospitalized due to severe acute respiratory infections in Lima, Peru. Epidemiologic surveillance programs for B. pertussis are essential in the future in Peru

Testo completoThe present research seeks to show the importance of applying process management techniques and food safety norms in the operational processes of the supply chain to know the meaning and the need for an integrated hybrid model. The article reviews the sugarcane distillery sector of Peru and its main operational problems. Based on the literature reviewed and discussed with academics who have knowledge of the food supply chain, an integrated hybrid model was developed to help any distillery with lower levels of competitiveness than its competitors in other sectors, such as pisco, applying techniques of process management and food safety to increase the efficiency of liquor distilleries. The findings confirm that distilleries can increase their efficiency, thanks to the higher performance of their operations after their alignment with the integrated model.

Introduction: There is limited and controversial information regarding the potential impact of depression on glycemic control. This study aims to evaluate the association between depression and poor glycemic control. In addition, the prevalence of depression and rates of poor glycemic control were determined. Methods: Cross-sectional study performed in the endocrinology unit of two hospitals of ESSALUD in Peru. The outcome of interest was poor glycemic control, evaluated by glycated hemoglobin (HbA1c: < 7% versus ≥ 7%), whereas the exposure of interest was depression defined as 15 or more points in the Patient Health Questionnaire-9 tool. The association of interest was evaluated using Poisson regression models with robust standard errors reporting prevalence ratios (PR) and 95% confidence intervals (95% CI) adjusting for potential confounders. Results: A total of 277 participants, 184 (66.4%) males, mean age 59.0 (SD: 4.8), and 7.1 (SD: 6.8) years of disease were analyzed. Only 31 participants (11.2%; 95% CI: 7.5%–14.9%) had moderately severe or severe depression, whereas 70 (25.3%; 95% CI 20.3%–30.8%) had good glycemic control. Depression increased the probability of having poor glycemic control (PR = 1.32; 95% CI 1.15–1.51) after adjusting for several potential confounders. Conclusions: There is an association between depression and poor glycemic control among type 2 diabetes patients. Our results suggest that early detection of depression might be important to facilitate appropriate glycemic control and avoid further metabolic complications.

Testo completoObjectives: Diagnosis of tuberculous meningitis (TM) is a challenge in countries with a high burden of the disease and constrained resources and clinical prediction rules (CPRs) could be of assistance. We aimed at developing a CPR for diagnosis of TM in a Latin American setting with high tuberculosis incidence and a concentrated HIV epidemic. Methods: We enrolled adult patients with clinical suspicion of TM attending two hospitals in Lima, Peru. We obtained information on potential anamnestic, clinical and laboratory predictive findings that are easy to collect and promptly available. We independently diagnosed TM according to a composite reference standard that included a series of microbiological tests. We performed bivariate analysis and constructed a logistic regression model to select the predictive findings associated with TM. With the selected predictors included in the model, we developed a score-based CPR. We assessed its internal validity and diagnostic performance. Results: Of 155 analysed patients, 59 (38%) had TM. The CPR we derived includes three predictors: cough for 14 days or more, 10–500 cells in CSF and adenosine deaminase ≥ 6 U/l in CSF. It classifies patients into high-, moderate- or low-score groups and has an overall area under the ROC curve of 0.87. 59% of patients were assigned to either the high- or the low-score group, permitting prompt decision-making. In patients in the high-score group, it attains a positive likelihood ratio for TM of 10.6 and in patients with low scores, a negative likelihood ratio of 0.10. Bootstrap analysis indicated high internal validity. Conclusion: This CPR could support decision-making in patients with clinical suspicion of TM. External validation and further assessment of its clinical impact are necessary before application in other settings.

Testo completoIntroduction: There are few studies that examine the factors associated with the different levels of health anxiety in medical students. The objective was to determine the factors associated with the levels of health anxiety in medical students in 2018. Methods: An analytical cross-sectional study was carried out with 657 medical students from a private Peruvian university. Participants answered a questionnaire from which information was collected regarding levels of health anxiety (SHAI). For the analysis, linear regression was used to calculate crude and adjusted betas, and their 95% confidence intervals. Results: The mean health anxiety score was 14 ± 6.7. An association between health anxiety and the year of study is reported, with the second year showing the highest scores. In addition, an association between health anxiety and smoking is highlighted, as there are higher levels in occasional smokers, as well as a weak inverse correlation with age. No association was found with sex, place of birth, or having a first-degree relative that is a doctor or health worker. Conclusions: The present study showed that age, year of studies and smoking are associated with health anxiety levels. More studies are required, especially of a longitudinal nature.
