Testo completoSustained growth in the demographic weight of seniors aged 65 and over is resulting in a growing need for home and community care services. Shared decision-making is an effective approach to informing health decisions in the care context, where seniors often face difficult decisions that gravely affect their health and well-being. Yet Canadian seniors receiving home care have little experience of share decision-making. We sought to estimate the prevalence of clinically significant decisional conflict (CSDC) among seniors aged 65 year and older who are receiving or have received home or community care over the past year and who have faced difficult decisions, and to identify factors associated with their CSDC. We used data from pan-Canadian online survey with a questionnaire based on the Ottawa Decision Support Model. CSDC was measured using the Decision Conflict Scale with a cut-off of 37.5/100. Independent variables were identified based on an adapted conceptual framework relevant to our study. The prevalence of CSDC in our target population was 16.3 %. Associated factors were related to seniors’ characteristics (age, ethnicity and quality of life), the decision-making process (role assumed, match between chosen option and preferred option, information wanted about the difficult decision, information wanted about the options, other types of information wanted, and health professional as a reliable source of information), and the kind of professional assistance the senior receive (organization and care planning, training and adaptation). Our results will inform the development of effective decision support strategies to reduce CSDC and its harmful consequences in our target population.

Testo completoThis Ph. D. Thesis in economics deals with neighborhood services, and especially with home help services to elderly people. This highly growing sector of the economy is poorly discussed, in particular on its theoretical level. E. G. , There is no theoretical study on the variety of the development models of these services. The present research aims at understanding the basic principles of the various economic logics of these services, and at applying their dynamics. Our central approach consists in thinking this diversity in terms of plurality of "worlds" (or of productive modes) and of service markets. In the first part, we attempt to sort out the complexity of this market through analyzing how these services are practically resorted to (demand genesis) and the related professional practices (supply). In the second part, we propose a theoretical framework to understand and analyze this diversity and its connections to uncertainty. This framework was worked out in reference to the theory of conventions, through its mode of identifying productive models and to an original approach to mandates. This is a dynamic model : it takes into account the developments in environment as well as in home help services to elderly people. Neighborhood services can also be classified according to five productive models divided up into two spheres : first, the formal sphere made of four models, depending on whether mandate is easy or difficult to describe and on whether it is a simple or complex activity ; and the domestic sphere including one model. In the third part, we questions the market development of these services and differentiate three forms of markets. In this part, we go through the idea that the components of this development (solvency process and reduction of uncertainties) vary according to productive models and markets

Testo completoIf the terme of home help may refer to jobs which at large aim at providing care for weakened populations, it focuses here on a particular salaried person sometimes called home help who helps ageing people carry out some chores at home. As a matter of fact it is often quite ignored a professionnal practice, little praised for limited to the bounderies of the secluded privacy, trying to answer the needs of everyone's characteristics. As a place to live in for elderly people as well as place to work in for home helps, home endows the relationship with its specificity. Standing between domesctic services and caring, work contract and free gift, professionalization and familiarization, the relationship requires building up and cannot be set the outside. Therefor the study of the help relationship within a clinic psychosocial approach based on the very words of the protagonists unveils clashes, agreements sometimes discrepancies between expectations and solutions from social subjects. Based on service it can evolve towards a relationship which has become the very object, showing at the same time the desired career of home helps from a relationship based on service to serve the relationship itself. Eventually, the purpose of the present study is to highlight the diversity of the needs of the Elderly and also focus on a professional practice which still craves for a genuine recognition from society

Testo completoThe considerable social and economic changes taking place in the 20th century have modified the ageing process. The fact that older generations no longer cohabit with their children and the rise in female employment have weakened family ties, with state welfare systems taking over. Old people are increasingly having to face old age alone, eitheir in their own home or in an institution, whereas they themselves accompanied their parents in growing old. Our study analyses the psychological and social effects on the aged, already faced with other losses of being placed in an old people's home. We studied the impact of place of residence on self-esteem, on the perception of daily life events and on the support available in time of need. Theses variables were related to the amount of stress generated ant on the coping strategies used to deal with it. Our sample consisted of 110 old people: 55 living at home and 55 in institutions. The impact of place of residence is evaluated using a series of scales (self-esteem, positive and negative events, stress abiblity to cope) and a questionnaire on support networks. Some of the results obtained confirmed our hypotheses: in old people's homes levels of self-esteem and enjoyment were lower, with wider use of passive coping strategies. However, place of residence was not a determining factor for overall stress levels, whilst tension related to state of health was greater in institutions, against a higher level of time-related tension in the home. Our results showed how a positive self-image in old age can favourably affect the psychophysiological impact of stress factors on the management of stress, the emotions and the support system. Furthermore, place of residence cannot alone explain the differences observed since two social and biological varaibles play at the beneficial role in enabling subjects to maintain a sense of identity and adapt to old age: the fact that responsabiblities have been taken throughout life and that no anti-depressant medication is taken

Testo completoWe aimed to validate the modified version of the Short Emergency Geriatric Assessment (SEGAm) frailty instrument among elderly community-dwelling subjects. A longitudinal, prospective study was conducted in two French regions and subjects aged 65 years or more and living at home were included. For psychometric validation, feasibility, acceptability, internal structure validity, reliability, discriminant validity and predictive validity of the SEGAm instrument were studied. Overall, 167 patients were included. Average age was 77±7 years, the majority were women (70.7%). Feasibility and acceptability of the sheet A of the SEGAm instrument were excellent: no refusal to participate, no drop-out during administration, no missing items, no ceiling or floor effects were noted. Administration time was short (5.0±3.5 min). By factor analysis, the instrument proved to be unidimensional. It showed good internal consistency (Cronbach’s alpha coefficient: 0.68) and good test-retest (intra-class correlation: 0.88). Discriminant validity showed a significant difference for mood and depression risk, nutritional status, fall risk, dependency, and comorbidities. Frailty status at baseline was significantly associated with loss of autonomy at 12 months of follow-up (OR=4.52, 95% CI=1.40-14.68; p=0.01). The SEGAm appears to be an easy-to-use instrument that is particularly suitable for use in the community to identify frail elderly people who could benefit from early targeted interventions

Testo completoThis exploratory qualitative research documents the reality of caregivers accompanying at home an elderly person with mood disorder. It aims to 1) explore the sources of support available in order to help them to walk through their journey as a caregiver; and to 2) explore the impact of interactions between caregivers, elderly and the health system in the use or the non-selection of these sources of support. The data comes were collected from individual semi-directed interviews with nine caregivers accompanying at home an elderly person with mood disorder. Theoretical perspective uses is Blumer’s symbolic interactionism (1969). Analysis of data, inspired by grounded theory, proposes four main themes: the trajectory of the family caregivers accompanying at home an elderly person with mood disorder, interactions, the use of sources of support and the type of family caregivers accompanying at home an elderly person achievement of a mood disorder. The results of this study allow to develop a typology of the caregiver accompanying an elderly with mood disorder at home. It shows four types of caregiver: 1) Overworked 2) Forced 3) Volunteer and 4) Accomplished. Finally, the results reveal four attitudes use by health professionals to promote use of support services by caregivers: 1) harmonious interactions, 2) establishing a cooperative relationship, 3) the stability of the relationship, and 4) situational adaptability. Testo completoIn France, few studies were interested by care social workers in the accompaniment of old age and less still, in that concerning the alcohol misuse at this age. In this thesis, we study the psychological aspects of these professional all while comparing them with those to support the Alzheimer's disease. 99 social care workers answered a questionnaire of 62 professional data, a scale of self esteem (EES), a questionnaire of job satisfaction (ESVP), an inventory of burnout (MBI) and a scale of anxio-depressive symptomatology (HADS). The professionals were divided into two groups: those with the difficulties in the support of the disease of Alzheimer (n=63) and those with the difficulties with alcohol misuse (n=36). Moreover, 18 of whom were interviewed. In the support of alcohol misuse, significant results show that when they know the diagnosis of pathology, emotional exhaustion is higher and this one is correlated positively with depressive mood and negatively with the job satisfaction. The personal achievement is correlated negatively with depressive mood. The job satisfaction is correlated positively with relatives' relations. In the support of Alzheimer's disease, when they don't know the diagnosis of the pathology, emotional exhaustion is higher and this one is correlated negatively with the job satisfaction and positively with anxio-depressive symptomatology. For the alcohol misuse, the interview of difficulties with the elder is related to similarity with those showing the representations and progression of the disease, the family's relations and the feeling of ineffectiveness for supporting the pathology. The discussion of satisfactions is related to similarity with those evoking the professional experience, the representations and skills to speak with the person about the disease. For Alzheimer's disease, the discussion of difficulties with the elder is related to similarity with those evoking the representations and the progression of the disease ¿ and that ¿ with the identifications of the senior. The discussion of satisfactions is related to similarity with those talking the professional experience and the evocation of the personal reasons to work. This study allows considering the specificities of French social care workers. It opens some research perspectives and could have a clinical implication in prevention of this population

Testo completoIn developed countries, the latest statistics clearly indicate that elderly population is growing, and people are living longer. Thus, maintaining a functional independence of the elderly becomes a priority for our society. The majority of elderly people living alone are more and more exposed to risky situations; one of them is the risk of falling. Falls are the leading cause of serious and health threats in the elder population. In this thesis, we started by reviewing the problem of fall detection in the elderly. Next, we proposed non-linear methods in order to extract relevant parameters for the prevention and early detection of the risk of falling. The originality of these methods is the nonlinear analysis of the components obtained from an empirical modal decomposition. The EMD method has highlighted the existence of a chaotic component in the signal from which we can extract an effective indicator of the quality of human posture. We are particularly interested in the mathematical reformulation of the EMD method in order to generalize it to the multidimensional case. This new extension was applied later on stabilogram signals in the aim of extracting relevant parameters for the prevention and detection of falling. One of the perspectives of this work is to consider data fusion of human posture with other data from sensors embedded in a smart home

Testo completoAlzheimer’s Disease is a degenerative condition of the brain, predominantly affecting the memory, cognitive capacity and causingmotor skill disabilities affecting the patient’s daily life. The disease is associated with two types of cerebral damage : senile patches and neurofibrillaire degeneration. Age is the highest risk factor, which explains its predominance in the over 65s. In certain cases however, the disease presents in the under 65s, usually in those who have a genetic susceptability to it. In 2000, this disease affected 400,000 people in France, 8 million in Europe, and between 17 and 25 million worldwide, figures which are sure to rise. Today, over 860,000 people are affected in France and 26,6 million worldwide. The existing treatments, inhibitors of the cholinesterase (Aricept, Exelon et Réminyl) and the N-Methyl-D-Aspartate antogonists (Mémantine) are associated with reducting the cognitive decline but they are associated with reducting the cognitive decline. This is why the treatment research depends on molecules that are able to block the production of these lesions and therefore avoid the appearance and onset of the dementia. People suffering from this illness are taken care of by family members and health workers, whose aims are to help with daily life, stimulation by interaction and conversation, administer care, and most importantly, allow the sufferer to keep as much independence as possible within the constraints imposed upon them by their disease. The carer’s workload increases with the further development of the illness, leading to physical fatigue and considerable psychological stress, even more so if the carer is a family member, where the weight of responsibility is greater. They must search for information about the disease and its implications, follow healthcare professionnal’s advice, and ask for as much help as possible; from the healthcare system, but also from support groups, whose involvement proves invaluable to most. In effect, the carer of someone with Alzheimer’s Disease is synonymous with patience, devotion, emotional suffering, solitude, courage… and most of all, love

Testo completoWith the development of technology and information, there are more and more opportunities and challenges for healthcare and assistance services for disabled people as well as the elderly. In this context, this PhD work proposes and demonstrates a new solution for home monitoring. Our approach is based on the idea that existing home automation and multimedia services provide some relevant information to be used as available sensors for remote monitoring. Through the analysis of user habits, our work includes two steps. In the first step, we automate a scenario identification, based on a combination of data mining, AI, graph theory and operational research algorithms to offer scenarios self adapting to user capabilities, while facilitating user access to the services. In the second step, this sensor information is used for alert management based on the anomaly detection, meaning a deviation of usual habits. These two steps provide a low level and non-intrusive personal monitoring while giving people more autonomy and confidence in their environments. A simulation model is developed in a first stage for the generation of user database without waiting for months monitoring user activities. This simulation data allows us to develop, tune and evaluate different aspects of our approach, before being applied in a real context. Then an experimentation through the IR recording is realized to monitor the user activities. The results of these real data allow us to evaluate the performance as well as the efficiency of our solution

Testo completoThe ageing population constitutes a major societal challenge for public policies and health systems: delaying the age of entry into dependency and improving the well-being of the elderly people. Home care for the elderly is one of the main alternatives to address this reality. In recent years, many technological and organizational innovations have been developed to improve elderly home care. These solutions are not always perceived favourably by multiple stakeholders, which are not always in favour of integrating changes into their daily lives. In order to ensure the relevance of a solution for the home care for the elderly and to increase its acceptance, it is necessary to integrate an evaluation phase in the co-design process with future users and other stakeholders. Existing acceptability assessment models do not sufficiently consider the perceptions of multiple stakeholders or a benefit-risk vision. In this context, our research work aims to propose a generic model for assessing and improving acceptability and a method for adapting it to a type of product. The work carried out is divided into five contributions: (1) State of the art on the characteristics of acceptability assessment models, (2) Proposal of a generic model for assessing the acceptability of innovative solutions, (3) Exploration of the risks for elderly people staying at home and the impact of the introduction of an innovative solution on their reduction, (4) Adaptation of the model for assessing the acceptability of food products by elderly people, and (5) Adaptation of the model and experimentation for assessing the acceptability of a food product and associated services during the pandemic crisis period

Testo completoWith the establishment of the Individual Public Allowance for Autonomy (APA) in France in 2002 and the Long Term Care (LTC) insurance in Korea in 2008, the management and the support for the dependence of the elderly population became a major issue of contemporary social policy in these two nations faced with the phenomenon of aging. Despite demographic, historical, cultural, political, economical and social contrasting contexts, they have installed devices in common with modes that appear close or substantially similar to cope with the increasing needs of the elderly. By adopting a comparative approach between France and Korea, this thesis sought to analyze the work of social gerontological professionals in charge of these devices for the elderly dependent who stay at home. For this, the application procedure of the APA in France and the LTC in Korea and the organization of the socio-medical asistance for the dependent elderly at home were studied. In both France and Korea, the professionals who are involved with the home care services are multiple. Some are working in the "front line" (home helpers, care assistance, home nurses) and others are occupied with the coordination, the supervision/organization of the foregoing (front line workers) in the "second line". This research focuses on the impact of devices which are placed on the work of professionals, especially those who are concerned with various tasks in the second line and their necessary - and difficul t- coordination. How the different policies and the strategic relationship between these various participating professionals sometimes diverge and how the conflicts intrinsic to their actions and practice within complex organizational realities are settled were investigated and explored. The observation was carried out in one area within each country : Hellemmes-Lille in France and Cheongju-Cheongwon in Korea

Testo completoThis thesis, which was part of a European research project, focuses on the problem of the reluctance of elderly people to use ICTs. More specifically, it focuses on the psychosocial factors that influence the attitudes of people aged 62 and over to ICTs. We conducted six studies in this framework, divided into two areas. The first axis, containing two studies, adresses public strategies and practices for orienting older people to ICTs. In the first one we analyzed the practices of the organizations involved in the orientation of the elderly towards gerontechnologies in the Lorraine region of France. It emerges that the access routes are very different according to the counties and communities, but also according to the first interlocutor requested by the elderly person and his / her relatives. The second study aimed to analyze public strategies and regulations encouraging and facilitating the use of ICT based services for the elderly in six regions of Europe. Different stages of development of these strategies emerged and a north / south lag was observed. In the second axis, with four studies, we examined from different angles the factors influencing the attitudes of the elderly towards ICTs. Our third study examined the perceptions of older persons regarding ICTs and their perceived needs. We identified 22 perceptual items divided into eight components forming a scale of perceptions of ICTs as well as a scale of perceived needs including two components. The fourth study assessed the impact of previous experience with ICTs on perceptions of ICTs. We carried out analyzes of the variance of ICT perceptions depending on whether a computer was owned or not and the followup of computer literacy courses. The results showed that computer ownership and course tracking were associated with significantly more positive perceptions of ICTs. The fifth study analyzed the impact of the region of residence on perceptions of ICTs. We compared the perceptions of the elderly in six regions of the EU. We found coherence between public strategies for orienting older people to ICTs and the perceptions of older people regarding ICTs. Finally, our latest study focused on the analysis of factors influencing attitudes toward ICTs for different types of activities. Depending on the dependent variable, different factors were found, including perceptions of ICTs, perceived needs, prior experience, region of residence and sociodemographic variables

Testo completoAging is associated with a high prevalence of unintentional fall in the elderly with a third of the population aged over 65 years fall at least once a year and half of those over 80 years. In Alzheimer's Disease (AD) and related patients, the risk of falling is 3 times more and four times more at Home Care. In the first week and especially the risk of fracture in AD patient is multiplied by 3 and at 6 months the mortality is 50%. Forty percent of admissions in Home care are due to these falls. An exhaustive review of the literature on falls prevention shows that several types of interventions have been implemented and evaluated using the identified risk factors. None has so far concerned the evaluation of interventions from a technological device for both the prevention and early detection of the fall and thus better target prevention strategies

Testo completoThe objective of this research is to contribute to the analysis of health care networks, and particularly of gerontology networks as promising organizational arrangements. This research tries to answer the following question: How can the logics of action and gerontology strategies of deciders meet the needs of (global) public policies and local needs (local) in order to foster social innovation and serve users ? Gerontology network is considered not as a toolbox, but as a living body, and just like every human body, these networks are characterized by their morphological (anatomical) and physiological features (Part I), and by their behavior (Part II). We have chosen to accompany the text of caricatured illustrations associated with some verbatim from interviewed stakeholders. Thus, the network is as a person, a puppet of the government, a robot - reflecting creativity and originality - or even a clone - symbolizing an ideal organizational type. In the same perspective, the innovative logics of action would be those that meet the "biorhythm" of network in order not to degrade its effectiveness and those adopting appropriate strategies to its spatiotemporal environment. Willing to analyze some innovative cases of gerontology networks in France, Switzerland and Greece (Part III) we have created a model based on the articulation of regulation and proximity. Health networks are positioned on this model, and criteria such as their organizational capacity of interference and arrangement in time, their position in the life cycle, their choice of proximity and control allow them to be more innovative

Its purpose is to answer the need of elderly people who wish to successfully age in place by providing evolutionary ambient assisted living services at home. In order to support MEDeTIC developments, the ambition of our work was to engage a psychosocial diagnosis of elderly people needs concerning home, technologies and services. Through the focus of psychosocial and ergonomics tools, these three sources of innovations are conjointly analyzed in the present dissertation in order to identify what is hindering and driving congregate housing implementation (studies A1, A2, A3). Providing acceptability issues, this thesis firstly analyzes on the one hand service functionalities compatibility with activity limitations encountered by elderly people (study B1) and on the other hand familiar experience with technologies (study B2). Secondly, experiments C1 and C2 focus on model prediction and on individual (i.e. age, experience), socials (i.e. care givers opinions) and organizational (i.e. voluntary service usage settings) determinants in order to apprehend cognitive and social processes that drive usage of technical services by the elderly. Testo completoThis thesis studies the practices, conditions and contemporary division of care for the elderly, by examining skilled and ‘unskilled’ labour as well as formal and informal labour, at home and in care institutions. It is based on a qualitative survey carried out in Ile-De-France. We conducted 128 semi-Structured interviews with employees who have worked or work with the elderly directly and through organisations (two nursing homes, one home-Support association). Comparing the dynamics of professionalization, as well as the forms of organisation and the development of hierarchies of labour, has brought to light the different ways in which boundaries are built between social work, healthcare and ‘unskilled’ labour. The fragmented concept of care contributes to its devaluation. In this context, we have developed a methodological and theoretical approach that shows the importance of the active relationship between caregiver and care-Receiver that develops over time and encompasses all aspects of care. Based on the experiences of undocumented immigrant women and the issue of poor quality of work life, we have examined the role of gender, class and race in care relationships. This thesis touches on several fields of sociology: labour, gender, migration and emotions. We also include the multidisciplinary contributions of care theories

Testo completoIs it possible to avoid, through a solidarity mechanism, that medical dependence (needing help to perform daily activities) be converted into social dependence? Is it possible to do so in such a way that we preserve the ideal of autonomy for the dependent elderly? In the present context, the autonomy of the dependent elderly appears as a real challenge because it conveys the notion of an opportunity for these people to remain in control of their lives, able to make decisions. This thesis examines the question of autonomy both in theoretical and practical terms, and shows the conditions under which its effectiveness is possible. Specifically, this thesis highlights (i) the conditions of implementation for decision-making autonomy: the presence of caregivers and developments in architectural space, (ii) the notional issues surrounding the concept of dependency: differentiation of the concept of autonomy , connections with disability and, especially, situations of dependence (iii) the characteristics of the caregiver-relationship that oscillates between asymmetrical and reciprocal relationship, (iv) the important role played by individual and institutional actors in the achievement of independence, (v) the analysis of situations that undermine freedom as non-domination such as consent-giving (in connection with nursing homes), denial of care, coercive restraint, paternalism, (vi) the ethics of solidarity.
